Predictors and outcomes of quality of life in elderly patients with heart failure

Study objectiveThis study aims to identify predictors of health related quality of life (HRQoL) among patients with heart failure (HF) and assess whether HRQoL was a predictor of rehospitalisation and mortality, and if age influenced the findings. DesignObservational cohort study. SettingSeven hospitals in the Northern Sydney Local Health District, Sydney, Australia. ParticipantsCommunity dwelling patients who completed a Minnesota Living with HF questionnaire (MLHFQ) within 30 days of discharge after a HF hospitalisation. Main outcome measureMultivariable linear regression models were used to identify predictors of MLHFQ scores (higher score = worse HRQoL) and adjusted Cox regression models to assess the impact of MLHFQ scores on one-year rehospitalisation and mortality. Separate analyses were conducted for those aged ≤80 or >80 years. Results1911 patients of mean age 79 years (57 % aged >80 years) were included in this analysis. Among those aged ≤80 years; younger age, lower haemoglobin and presenting symptoms at hospitalisation of exertional dyspnoea, peripheral oedema and fatigue were predictors of worse post-discharge MLHFQ scores. In patients aged >80 years, living alone, chronic kidney disease, exertional dyspnoea and peripheral oedema were predictors of worse MLHFQ scores. Worse MLHFQ scores predicted one-year HF readmissions in those aged >80 years (HR 1.22, 95 % CI 1.07–1.37) but not those aged ≤80 years (HR 0.90 95 % CI 0.71–1.10). ConclusionsIn-hospital predictors can be identified for worse HRQoL post-discharge for HF. These vary according to age, and should be addressed prior to discharge.

Preoperative predictors of health-related quality of life changes (EQ-5D and EQ VAS) after total hip and knee replacement: a systematic review

BackgroundPatient-reported outcomes are of ever-increasing importance in medical decision-making. The EQ-5D is one of the generic instruments measuring health-related quality of life (HRQoL) in arthroplasty. This review aimed to identify possible predictors of HRQoL changes for patients undergoing total knee replacements (TKR) or total hip replacements (THR).MethodsA systematic literature review according to the PRISMA guidelines was conducted, searching several databases. Preoperative to postoperative HRQoL changes were evaluated in patients undergoing THR or TKR, using the EQ-5D visual analog scale (VAS) or the preference-based EQ-5D Index were evaluated. Articles were considered with prospectively or retrospectively collected data, as well as registry data, each with statistical analyses of patient-related factors.ResultsEight hundred eighty-two articles were found, of which 21 studies met the inclusion criteria. Predictors were distinguished in alterable and non-alterable ones. The EQ-5D Index indicated a tendency towards beneficial improvements for patients with a high body mass index (BMI) (> 40) and no significant results for the VAS. Additionally, one study found that patient education and preoperative physiotherapy appeared to enhance HRQoL. Some evidence indicated that male gender was negatively associated with changes in the VAS and the EQ-5D Index, but one study reported the opposite. Changes in VAS and EQ-5D Index were lower for older patients, whereas a higher educational level seemed to be advantageous. A high Charnley class led to deteriorating changes in VAS, although a high Kellgren Lawrence classification was positively associated with the EQ-5D Index, in a limited number of studies. For all results, clinical relevance was calculated differently and mainly reported as uncertain or small.ConclusionsThe literature on this topic was weak and offers only limited guidance. Results for alterable predictors, such as the BMI, indicated valuable improvements for highly obese patients. Predictors of Health-related Quality of Life among Warfarin Patients

Objective: Warfarin is amongst the most frequently used oral anticoagulant, that is often prescribed to control and prevent various thromboembolic diseases like venous thromboembolism, stroke, atrial fibrillation, and valvular heart disease. This study aimed to determine various predictors and their impact on overall HRQoL among warfarin patients.
 Methods: A cross-sectional study using WHOQOL-BREF research tool was conducted among warfarin patients. Data was collected by convenience sampling method. Descriptive, comparative, and inferential statistics were used by Statistical Package for the Social Sciences (SPSS) ver. 24 to determine the predictors of HRQoL among warfarin patients.
 Results: The majority of the studied warfarin patients were females than males (n=221, 69.3%, and n=98, 30.7% respectively). In univariate analysis, statistically non-significant differences (p >0.05) were observed in gender, age, marital status and work. In multivariate analysis, significant differences (p <0.05) were observed in education, warfarin usage, and warfarin therapy duration.
 Conclusion: These results indicated that education, warfarin usage, and warfarin therapy duration were the pure predictors of HRQoL among the studied cohort of the warfarin patients.

Evaluating the key predictors of health-related quality of life in patients with heart failure and reduced ejection fraction: results from the DAPA-HF trial

Abstract Background The DAPA-HF trial demonstrated that dapagliflozin was superior to placebo at preventing cardiovascular death and hospitalisation for heart failure (hHF) events in patients with chronic heart failure with reduced ejection fraction (HFrEF). The trial also demonstrated a clinically important benefit of dapagliflozin on health-related quality of life (HRQoL). However, key predictors of HRQoL in HFrEF patients remain uncertain. The objective of this study was to determine, using DAPA-HF trial data, the patient characteristics and disease-related events associated with patient HRQoL, measured by health state utility values. Methods Mixed effects regression models were developed based on pooled individual patient data from DAPA-HF to determine patient utility estimated from responses to the EQ-5D-5L questionnaire, incorporating a subject specific random intercept. In line with NICE guidance, utility estimates were derived using UK-specific utility tariffs after mapping EQ-5D-5L data to EQ-5D-3L values. Univariable analysis was first undertaken to assess candidate predictors of utility; followed by a multivariable model including statistically significant predictors, e.g. Kansas City Cardiomyopathy Questionnaire Total Symptom Score (KCCQ-TSS) and the incidence hHF events, and controlling for differences in baseline characteristics. All variables were included in a single model to provide independent (adjusted) estimates for each covariable. Results 19,983 EQ-5D-5L questionnaires from 4,744 patients were included. Mean patient utility at baseline was 0.716 (95% CI: 0.711, 0.722). Univariable analysis demonstrated NYHA, KCCQ-TSS, T2DM, BMI, age, geographic location, non-ischaemic/unknown aetiology and atrial fibrillation were statistically significant in their association with patient utility while prior hHF, race, eGFR and left ventricular ejection fraction were not. Multivariable analysis results are summarised in Fig. 1. The baseline characteristic with the greatest impact on EQ-5D was KCCQ-TSS quartile, with EQ-5D increasing with KCCQ-TSS and the difference in utility between patients in quartile 1 (lowest score) and quartile 4 (highest score) estimated at 0.233 (0.226, 0.240). When controlled for baseline characteristics, being post-event was significantly associated with HRQoL; patients who experienced hospitalisation for HF had 0.036 (0.014, 0.058) lower utility on average within one month of the event and 0.025 (0.011, 0.039) lower utility up to one-year after the event. For patients who had stroke or myocardial infarction events there were reductions in utility of 0.206 (0.141, 0.272) and 0.108 (0.039, 0.177) respectively at 1 month. Conclusion HF symptoms, measured by the KCCQ, were strongly associated with patient health utility. Therapeutic interventions that can improve HF symptoms have the potential to improve HRQoL and reduce the burden of HF. Funding Acknowledgement Type of funding sources: Private company. Identifying high-need patients with multimorbidity from their illness perceptions and personal resources to manage their health and care: a longitudinal study

BackgroundA proactive person-centred care process is advocated for people with multimorbidity. To that aim, general practitioners may benefit from support in the identification of high-need patients, i.e. patients who are high or suboptimal users of health services and/or have a poor quality of life. To develop such support, we examined whether knowledge about patients’ illness perceptions and personal resources to manage their health and care is useful to identify high-need patients among multimorbid general practice populations.MethodsSurvey data, collected in 2016 and 2017, of 601 patients with two or more chronic diseases (e.g. COPD, diabetes, Parkinson’s disease) registered with 40 general practices in the Netherlands were analysed by logistic regression analysis to predict frequent contact with the general practice, contact with general practice out-of-office services, unplanned hospitalisations and poor health related quality of life. Patients’ illness perceptions and personal resources (education, health literacy, mastery, mental health status, financial resources, social support) were included as predictors.ResultsThe four outcomes were only weakly associated among themselves (Phi .07–.19). Patients’ illness perceptions and personal resources were of limited value to predict potentially suboptimal health service use, but they were important predictors of health related quality of life. Patients with a poor health related quality of life could be identified by their previously reported illness perceptions (attributing many symptoms to their chronic conditions (B = 1.479, P < .001), a high level of concern (B = 0.844, P = .002) and little perceived control over their illness (B = -0.728, P = .006)) combined with an experienced lack of social support (B = -0.527, P = .042) and a poor mental health status (B = -0.966, P = .001) (sensitivity 80.7%; specificity 68.1%).ConclusionsMultimorbid patients who frequently contact the general practice, use general practice out-of-office services, have unplanned hospitalisations or a poor health related quality of life are largely distinct high-need subgroups. Multimorbid patients at risk of developing a poor quality of life can be identified from specific illness beliefs, a poor mental health status and unmet social needs.

Patient\u2019s early satisfaction with care: a predictor of health-related quality of life change among outpatients with substance dependence

BackgroundAlthough research on health-related quality of life (HRQoL) has increased in the addiction field, few studies have focused on the determinants of HRQoL changes. This study aimed to describe dependent patients’ HRQoL changes at a 3-month follow-up and to assess whether satisfaction with care can predict those changes among outpatients starting care for alcohol or opioid dependence.MethodsHRQoL was measured with the SF-12 at baseline and 3 months later in a prospective cohort of dependent outpatients. Satisfaction was assessed with the EQS-C early after inclusion. Data on sociodemographics, clinical characteristics and patients’ levels of anxiety and depression were also collected. A multivariable analysis was performed to identify factors associated with HRQoL changes in both the physical and mental component summary scores (PCS and MCS, respectively).ResultsOf the 172 patients included at baseline, a total of 136 patients assessed their satisfaction with care. The mean PCS and MCS scores were initially low, and HRQoL improvement was significant after 3 months for both the PCS and MCS. Never having been married (β = 5.5; p = 0.001) and a lower baseline PCS score (β = − 0.6; p < 0.0001) were associated with significant PCS improvement, whereas being legally compelled to undergo drug treatment (β = − 5.9; p = 0.02) was associated with less PCS change. Higher early satisfaction with care (β = 0.1; p = 0.02) and a lower baseline MCS score (β = − 0.7; p < 0.0001) were associated with significant MCS improvement.ConclusionThe study supported the hypothesis that greater satisfaction with care may predict HRQoL improvement among dependent outpatients. Further studies are needed to understand the factors that affect patients’ early satisfaction to identify areas of improvement and thus improve HRQoL.

Predictors of health-related quality of life among older adults living with HIV in Thailand: results from the baseline and follow-up surveys

ABSTRACT The current longitudinal study consisted of baseline and follow-up surveys among older adults living with HIV (OALHIV) in Thailand. The health-related quality of life (HRQoL) was assessed using the Medical Outcomes Study HIV (MOS-HIV) questionnaire. We performed multiple linear regression analysis to document correlates of HRQoL at baseline and the predictors of the changes in HRQoL at follow-up. Of the 364 participants recruited at baseline; 327 (89.9%) completed the follow-up survey. The mean (SD) Physical Health Summary (PHS) and Mental Health Summary (MHS) scores were respectively 49.8 (7.3) and 53.2 (6.4). There was a significant increase in the mean score of most of the MOS-HIV domains, ranging between 1.3 for the PHS and 26.9 for the energy/fatigue dimension. In contrast, the mean score significantly decreased by 4.1 and 10.3 points, respectively for the cognitive and social functioning. Female gender was a predictor of the decline in social (β = −11.37; P = 0.031) and cognitive (β = −8.05; P = 0.002) functioning at follow-up, while being married was related to an increase of in the score of energy/fatigue (vitality) (β = 5.98; P = 0.011) at follow-up. Physical exercise was associated with an increase in social functioning (β = 9.38; p = 0.042). Overall the HRQoL of OALHIV improved or was maintained over time.

Predictors of Health-Related Quality of Life in Indians with Metabolic Syndrome Undergoing Randomized Controlled Trial of Yoga-Based Lifestyle Intervention vs Dietary Intervention

The present study explores the efficacy of 12-week yoga + diet-based lifestyle intervention (YBLI) vs dietary intervention (DI) on health-related quality of life (HRQoL) and identifies the predictors of change in HRQoL in Indians with metabolic syndrome (Met S). Data from the historical randomized controlled trial was used including adults (n = 260, 20–45 years) with Met S. Four domains of HRQoL were measured at baseline, 2 and 12 weeks using WHOQOL-BREF questionnaire. Generalized estimating equation and chi-square test was used to compare 12-week changes in HRQoL domains and proportion of subjects, respectively. Changes in HRQoL were predicted using regression models concerning changes in body mass index (BMI), physical activity, total calorie intake, adiponectin, and superoxide dismutase (SOD) levels. Exploratory mediation analysis was carried out using Baron & Kenny approach. YBLI resulted in a significantly greater increase in the physical domain score of HRQoL than DI. A significantly greater proportion of subjects in YBLI group (71%) showed an increase in physical domain scores compared to DI (51%). A unit change in BMI negatively predicted a unit change in physical, psychological and environmental health. Whereas, a unit change in adiponectin and SOD levels positively predicted a unit change in physical and environmental health. Partial mediation between YBLI intervention and physical HRQoL domain was observed via adiponectin. In conclusion, a 12-week YBLI has a positive and greater effect on HRQoL physical domain score than following DI alone. Changes in BMI, adiponectin, and SOD levels may predict changes in HRQoL domains after lifestyle intervention.

Health related quality of life and its correlates among people with depression attending outpatient department in Ethiopia: a cross sectional study

BackgroundDepression is a common mental disorder negatively affects the cognitive, emotion, behavior, functionality and quality of life of people. Poor quality of life results in high rates of relapse, inability to perform occupational and social activities, impaired future outlook, and increases overall health care related costs. However, there is no available evidence regarding the health related quality of people with depression in Ethiopia. Therefore, evaluating the quality of life of people with depression is crucial.ObjectiveThe aim of this study was to assess the health related quality of life and its correlates among people with depression at Amanuel Mental Specialized Hospital, Addis Ababa, Ethiopia.MethodsAn institutional based cross-sectional study was conducted from May 1st to 30th, 2018. A randomly selected 394 clients with depression were participated in this study. Health related quality of life was measured using world health organization quality of life brief. The collected data were coded and entered to SPSS version 20 for analysis. Step wise multiple linear regression analysis was used to identify the correlates of quality of life and the strength of the correlation was measured by β coefficient with 95% confidence interval.ResultsThe mean (±SD) scores of quality of life of people with depression were 41.3 ± 7.5, 42.8 ± 8.2, 38.9 ± 8.9 and 41.8 ± 6.5 for physical, psychological, social and environmental domains, respectively. The Multiple regression analysis showed that age of respondents, age of onset of depression, perceived stigma, living arrangement, social support level and duration of illness were statistically significant predictors of health related quality of life of people with depression in all or at least one domain of quality of life.ConclusionsThis study revealed that nearly half of study participants scored below the mean score in each domain of health related quality of life. This demonstrates a need for improving the quality of life of people with depression through the integration of a positive mental health approach and bio-psychosocial view together with the pharmacological treatments of depression. Moreover, strengthening social support, early identification and treatment of depression and prevention of stigma are also highly recommended to improve the quality of life of people with depression.

Affirmative answers on loco-check as a predictor of health-related quality of life and locomotive syndrome progression in the elderly: A cross-sectional study

Objectives: To clarify the correlation between locomotive syndrome (LS) stages and the loco-check or health-related quality of life (HRQoL) and whether the loco-check can predict HRQoL and LS stages.Methods: Overall, 298 subjects (106 men, 192 women) were evaluated for LS and HRQoL using EuroQoL-5 dimensions (EQ-5D) and the EuroQoL-visual analog scale (EQ-VAS). The correlation between LS stages and HRQoL using EQ-5D and EQ-5D-VAS scores and the number of affirmative answers on the loco-check were evaluated.Results: The numbers of non-LS, Stage 1 LS, and Stage 2 LS subjects were 143, 130, and 25, respectively. EQ-5D and EQ-5D-VAS scores decreased from non-LS to Stage 2 LS. EQ-5D and EQ-5D-VAS scores in LS Stage 2 subjects were significantly lower. The number of affirmative answers on the loco-check increased from non-LS to Stage 2. Two affirmative answers on the loco-check was the cut-off to detect Stage 2 LS. The number of affirmative answers on the loco-check was significantly negatively correlated with HRQoL in elderly subjects.Conclusion: The number of affirmative answers on the loco-check positively correlated with LS stages and negatively correlated with HRQoL. The loco-check might help to predict LS stage and HRQoL, especially in elderly people aged over 65 years.

Background: Diabetes mellitus disease is the universal principal cause for decreased quality of life in adulthood. This study seeks to indicate the psychological and socio-demographical determinants of the health related quality of life in patients with diabetes mellitus in Tetovo.Material and method: The research instrument used consists of socio-demographic questions and four tests: Cohen’s stress measurement questionnaire, Beck’s depression measurement questionnaire, Beck’s anxiety measurement questionnaire and the quality of life measurement questionnaire (WHOQOL-Breef ). The sample of the study consists of N=100, including patients with diabetes mellitus treated in Clinical Hospital in Tetovo.Results: Findings revealed that psychological variables (stress, depression and anxiety) indicates significant negative correlations with quality of life and its dimension. In terms of demographic variables, duration of disease is in significant correlation with QOL and psychological variables, type of duration is in significant correlation with QOL, age is in negative significant correlation with stress and QOL. There was no significant role at prediction of health related quality of life in terms of patient's age, gender, residence, employed, education and marital status. Conclusion: Demographic variables, such as: gender, age, employment, education and economic class are in significant relation with stress, depression, anxiety and the quality of life of patients with diabetes, while the type of disease and its duration are in that kind of relations only with the quality of life. Thus, it is important to emphasize the worth of tailored intervention for addressing these issues in therapeutic and rehabilitation agendas. Likewise, the identification and treatment of psycho-social correlates in these patients could be very important. Predictors of health related quality of life three years after myocardial infarction with ST segment elevation

Background/Aim. Health-related quality of life (HRQoL) is an important indicator of patient condition following myocardial infarction. It may serve as a predictor of mortality and new hospitalization. The aim of this study was to evaluate the association of selected sociodemographic and clinical characteristics with HRQoL in the Serbian cohort of patients with myocardial infarction with ST segment elevation (STEMI) that were treated with primary percutaneous coronary intervention (pPCI). Methods. Patients were recruited from the population of patients with STEMI who were hospitalized in the Clinical Center of Serbia in Belgrade, between 1st December, 2009 and 30th June, 2010. The study was conducted among 507 STEMI patients treated with pPCI. The HRQoL was assessed using the Questionnaire Short Form Health Survey (SF-36). Multivariate logistic regression models were used for each components score in order to determine independent predictors of HRQoL. Results. The patients with the lowest tertiles of Physical component score (PCS) and the Mental component summary score (MCS) were older, likely to be females, unpartnered, with a poor economic status, with diabetes, with prior myocardial infarction and with more extensive coronary artery disease. There were more employed and the individuals with smoking history in the group of patients with the higher scores. The characteristics of patients with lower PCS score were: the higher presence of hypertension, prior cerebrovascular insult and left anterior descending artery as infarct artery. This study demonstrated that HRQoL was significantly associated with patient?s age, gender, diabetes mellitus, a poor way of living and loneliness. Furthermore, the presence of previous cerebrovascular insult seems to affect the physical component score. Conclusion. Knowledge of predictors of HRQoL in the STEMI patients may provide indications for optimal treatment and anticipate their impact on the treatment outcome.

Abstract 270: Prediction of Health-Related Quality of Life at 12 Months From Baseline Modifiable Factors in Patients With Heart Failure

Introduction: Patients with heart failure (HF) report that good health-related quality of life (HRQOL) is more valuable than longer survival, and improvement in HRQOL after discharge has been associated with lower rates of hospitalization. However, HRQOL in patients with HF is remarkably poorer than that in patients with other chronic diseases. To improve HRQOL effectively, it is critical to examine comprehensive modifiable predictors. However, to our knowledge, this has not been examined in patients with HF. Purpose: To examine baseline psychosocial, behavioral, and physical predictors of HRQOL at 12 months in patients with HF. Methods: We collected data on HRQOL at 12 months using the Minnesota Living with Heart Failure Questionnaire and other data at baseline from 94 patients with HF (mean age 58 ± 14 years). We collected data on all psychosocial variables, one behavioral variable, and one physical variable using questionnaires: depressive symptoms (Patient Health Questionnaire), perceived control (Control Attitudes Scale-Revised), social support (Multidimensional Scale of Perceived Social Support), self-care management (Self-care management subscale of the Self-Care of Heart Failure Index), and HF symptoms (Symptom Status Questionnaire-HF). We assessed one physical factor (New York Heart Association [NYHA] functional class) using in-depth interview. We assessed two behavioral factors using objective measures: medication adherence (Micro-Electro-Mechanical Systems) and sodium intake (24-hour urine). Covariates were age and comorbidity. Multiple regression analysis was used to address the purpose. Results: After controlling for covariates, among multidimensional psychosocial, behavioral, and physical variables, 3 modifiable factors (medication adherence [p = .001], HF symptoms [p = .011], and NYHA functional class [p &lt; .001]) significantly predicted 12-month HRQOL (F = 11.853, R 2 = .59, p &lt; .001). Conclusion: The findings of this study demonstrate multidimensional, modifiable predictors that interventions can target to improve HRQOL. Clinicians and researchers can target improvement in self-care, HF symptoms, and NYHA functional class to improve HRQOL, and, in turn, to reduce hospitalization rates.

Objectives:To determine (i) levodopa (L-Dopa) responsiveness on cortical functions, health related quality of life and fatigue severity (ii) relationship between cortical functions, health related quality of life and fatigue severity post L-Dopa treatment of patients with idiopathic Parkinson’s Disease (I-PD).Methods:Participants included 50 patients diagnosed with I-PD who were attending Civil and Bahawal Victoria Hospital, Bahawalpur, Pakistan during May 2016 to July 2017 and 50 healthy individuals (HI) took part in the study. Participants completed Cortical Function Assessment, Parkinson’s Disease Questionnaire and Fatigue Severity Scale. Patients were tested twice on these measures: pre and post- L-Dopa treatment.Results:Patients with I-PD showed cortical functioning deficits, deteriorated health related quality of life and experience of severe fatigue, in contrast with HI. There was significant improvement in cortical functioning and quality of life while reduction in fatigue severity was observed after three months of L-Dopa medication in I- PD patients. Higher cortical functioning deficits correlated with deteriorated health related quality of life and severe fatigue. Cortical functioning was a significant predictor of health related quality of life and fatigue severity.Conclusion:L-Dopa is an effective treatment for cortical dysfunctions, health related quality of life and fatigue in I-PD. Profile and predictors of health related quality of life among type II diabetes mellitus patients in Quetta city, Pakistan

BackgroundThis study aims to assess the profile and predictors of Health-Related Quality of Life (HRQoL) in Type II Diabetes Mellitus (T2DM) patients in Quetta, Pakistan.MethodsThe study was designed as a questionnaire based, cross sectional analysis. 300 Type II diabetic patients attending public and private hospitals were targeted for data collection. In addition to demographic and disease related information, Euroqol Quality of Life was used to measure HRQoL. Moreover, Drug Attitude Inventory and Michigan Diabetes Knowledge Test were used to assess medication adherence and diabetes related knowledge respectively. Treatment satisfaction was assessed by patient’s experience towards health care professionals and available facilities. Descriptive statistics were used to elaborate patients’ demographic and disease related characteristics. Binary logistic regression was used to predict factors independently associated with HRQoL. SPSS v. 20 was used for data analysis and p < 0.05 was taken as significant.ResultsPatients in the current study reported poor HRQoL with a mean score of 0.48 ± 0.36. Age, duration of disease, number of prescribed drugs, medication adherence and treatment satisfaction were significantly associated (p < 0.05) with HRQoL in the cross tabulation analysis. The significant variables were entered into the model that showed significant goodness of fit with highly significant Omnibus Test of Model Coefficient (Chi-square = 12.983, p = 0.030, df = 4). Medication adherence was reported as a significant predictor of HRQoL with an increase of one adherence score was associated with improvement of HRQoL by a factor of 1.75 provided other variables remain constant.ConclusionThe study presents a model that is associated with HRQoL with patient with T2DM, where medication adherence shaped as a predictor of HRQoL. Healthcare professionals should pay special attention on patients’ medication taking behavior and should put their efforts in explaining the benefits of the medication adherence to the patients.
