Abstract LB077: CSF-1R inhibition with Pimicotinib (ABSK021) enhanced anti-tumor efficacy of KRASG12C inhibitors in preclinical non-small cell lung cancer mouse models

Abstract Background: KRAS mutations are highly prevalent oncogenic drivers in human cancers and KRASG12C is the most frequent mutations found in KRAS-mutant non-small lung cancer (NSCLC). It is reported that tumor associated macrophages (TAMs) are enriched in KRAS-driven transgenic lung cancer models. Depletion of macrophage results in a significant decrease in tumor burden and increased survival. Sotorasib (AMG510), the first KRASG12C inhibitor approved by FDA, has been found to modulate tumor immunity, impacting the recruitment of macrophages. CSF-1R signaling is critical for macrophage proliferation and survival. CSF-1R inhibition depletes TAMs and reprograms tumor microenvironment (TME), resulting in enhancement of anti-tumor immunity. Therefore, the combination of KRASG12C and CSF-1R inhibitors may synergize to enhance anti-tumor efficacy. Pimicotinib (ABSK021) is a potential best-in-class small molecule inhibitor of CSF-1R in clinical development of multiple indications. Here, we conducted in vivo experiments to explore the combined effects of KRASG12C and CSF-1R inhibitors. Methods: In vivo efficacy studies were performed to evaluate the combined treatment of KRASG12C inhibitors (AMG510 and ABSK071, a next-generation inhibitor targeting KRASG12C with better inhibitory activity) and CSF-1R inhibitor (ABSK021) in NCI-H2122 PBMC humanized mice and LLC syngeneic mice. RNA sequencing was employed to investigate the mechanism of action underlying the synergistic anti-tumor effect. Immunohistochemistry (IHC) was utilized to examine the changes of immune cell populations in TME. Results: The combinatory regimen of ABSK021 and KRASG12C inhibitors showed improved anti-tumor effects than each single agent alone in both mouse NSCLC models. Bioinformatics and follow-up analysis revealed a reduction in immune suppressive TAMs in tumors treated with ABSK021 alone or in combination with KRASG12C inhibitors. Furthermore, modulation of TME related to anti-tumor immunity, such as activated CD4 T, CD8 T and NK cells, was increased, and pro-tumor immunity, such as MDSC, regulatory T cells, was decreased in combination group. IHC results confirmed that ABSK021 addition led to macrophage depletion and more CD8 T cell infiltration in TME. Conclusions: For the first time, we demonstrate that the combined inhibition of KRASG12C and CSF-1R leads to superior therapeutic efficacy in preclinical NSCLC mouse models. This results suggests a potential novel therapeutic regimen that could yield improved clinical benefit to patients. Natural Product-Based Glycolysis Inhibitors as a Therapeutic Strategy for Epidermal Growth Factor Receptor-Tyrosine Kinase Inhibitor-Resistant Non-Small Cell Lung Cancer.

Non-small cell lung cancer (NSCLC) is a leading cause of cancer-related deaths worldwide. Targeted therapy against the epidermal growth factor receptor (EGFR) is a promising treatment approach for NSCLC. However, resistance to EGFR tyrosine kinase inhibitors (TKIs) remains a major challenge in its clinical management. EGFR mutation elevates the expression of hypoxia-inducible factor-1 alpha to upregulate the production of glycolytic enzymes, increasing glycolysis and tumor resistance. The inhibition of glycolysis can be a potential strategy for overcoming EGFR-TKI resistance and enhancing the effectiveness of EGFR-TKIs. In this review, we specifically explored the effectiveness of pyruvate dehydrogenase kinase inhibitors and lactate dehydrogenase A inhibitors in combating EGFR-TKI resistance. The aim was to summarize the effects of these natural products in preclinical NSCLC models to provide a comprehensive understanding of the potential therapeutic effects. The study findings suggest that natural products can be promising inhibitors of glycolytic enzymes for the treatment of EGFR-TKI-resistant NSCLC. Further investigations through preclinical and clinical studies are required to validate the efficacy of natural product-based glycolytic inhibitors as innovative therapeutic modalities for NSCLC.

177 (PB057) - BLU-945 or BLU-701 as single agents versus their combination with osimertinib in EGFR L858R driven tumor models

Background: The most common activating EGFR mutations (EGFRm) in non-small cell lung cancer (NSCLC) are exon 19 deletions (ex19del) and L858R substitutions. EGFR tyrosine kinase inhibitors (TKIs) as first-line (1L) therapy have improved outcomes in EGFR-driven advanced NSCLC; however, treatment resistance eventually occurs. Patients with EFGR L858R-driven advanced NSCLC (39% of all EGFR mutations in NSCLC) have shorter progression-free survival than those with EGFR ex19del, suggesting a significant unmet need. BLU-945 and BLU-701 are investigational, reversible, selective, orally available TKIs optimized for use as singleagent or combination therapy to suppress activating and on-target resistance EGFR mutants while sparing wild-type EGFR. in vivo studies have shown antitumor activity of BLU-945 against EGFR L858R, L858R/C797S, L858R/ T790M and EGFRm/T790M/C797S mutants, and BLU-701 against EGFRm and EGFRm/C797S mutants. Combination of BLU-945 and BLU-701 has also shown in vivo activity in EGFRm/C797S-driven mutant NSCLC. Combining BLU-945 or BLU-701 with osimertinib could potentially enable superior coverage of frequent on-target resistance mechanisms as well additional potency on the primary EGFR activating mutations. Central nervous system (CNS) metastases are an ongoing challenge in NSCLC. BLU-701 has already shown preclinical CNS activity; and both BLU-945 and BLU-701 are currently being investigated for potential treatment or prevention of CNS metastases. Due to poorer outcomes in patients with EGFR L858R-driven advanced NSCLC treated with osimertinib, and the inevitability of treatment resistance, studies were conducted to evaluate the activity of BLU-945 or BLU-701 in combination with osimertinib in prolonging duration of response (DOR) in preclinical NSCLC tumor models driven by the L858R mutation. Material and Methods: The antitumor activity of BLU-945 or BLU-701, as single agents or in combination with osimertinib was evaluated in EGFR L858R-driven tumor models. Results: Administration of single-agent BLU-945 or BLU-701 resulted in tumor growth inhibition in EGFR L858R-driven PDX models. BLU-945 in combination with osimertinib resulted in prolonged DOR and survival when compared with BLU-945 or osimertinib as single agents. BLU-701 in combination with osimertinib resulted in superior tumor growth inhibition compared with BLU-701 or osimertinib as single agents. Conclusions: The in vivo antitumor activities of BLU-945 and BLU-701 as single agents suggest both BLU-945 and BLU-701 have the potential to be used as 1L therapy in patients with EGFR L858R-driven NSCLC. The superior in vivo antitumor activity of BLU-945 or BLU-701 in combination with osimertinib in prolonging the DOR or increasing tumor growth inhibition in these models may have clinical application in improving outcomes of patients with EGFR L858R-driven NSCLC in 1L settings. EP16.01-024 Differential Efficacy of Combined GITR Co-stimulation with PD-1 Blockade in Preclinical Non-small Cell Lung Cancer Models

Lung cancer remains as the most lethal cancer disease in Hong Kong. Although blockade of inhibitory immune checkpoints achieved remarkable clinical outcomes, continuous efforts are being made to explore novel therapeutic strategies. We have previously found that anti-programmed cell death protein 1 (anti-PD-1) induced systemic and intra-tumoral upregulation of an immunoregulatory molecule, glucocorticoid-induced tumor necrosis factor receptor-related protein (GITR) in a subcutaneous murine lung cancer model.

CLRM-10 METIS (EF-25): A PIVOTAL, RANDOMIZED CONTROLLED STUDY OF TUMOR TREATING FIELDS IN PATIENTS WITH 1–10 BRAIN METASTASES FROM NON-SMALL CELL LUNG CANCER

BACKGROUNDTumor Treating Fields (TTFields) are electric fields that disrupt cancer cell division. TTFields treatment showed efficacy in preclinical non-small cell lung cancer (NSCLC) models. Furthermore, TTFields therapy improved survival with a tolerable safety profile in patients with glioblastoma. The objective of the pivotal METIS trial [NCT02831959] is to evaluate the efficacy and safety of TTFields therapy in NSCLC patients with brain metastases. METHODSNSCLC patients (N=270) with 1–10 brain metastases will be randomized 1:1 to stereotactic radiosurgery (SRS) followed by continuous TTFields therapy using NovoTTF-200M (150 kHz, recommended >18 h/day) with best standard of care (BSC) or SRS followed by BSC alone. Follow-ups will be conducted every 2 months until second intracranial progression. Key inclusion criteria are: Karnofsky Performance Status ≥70, new diagnosis of 1 inoperable or 2–10 supra- and/or infratentorial brain metastases from NSCLC amenable to SRS, and optimal therapy for extracranial disease. Key exclusion criteria are: prior whole brain radiotherapy, single operable, or recurrent brain metastases. Primary endpoint is time to first intracranial progression. Secondary endpoints include time to neurocognitive failure, overall survival, radiological response rate (RANO-BM and RECIST V1.1), quality of life, adverse events, time to first/second intracranial progression for patients with 1–4 and 5–10 brain metastases, bi-monthly intracranial progression rate from 2–12 months, and time to second intracranial and distant progression. The study is powered at 80% (2-sided alpha of 0.05) to detect a hazard ratio of 0.57. In July 2021, an independent Data Monitoring Committee (DMC) reviewed the study data and recommended continuation as planned. The trial is currently recruiting at 92 sites in North America, Europe, Israel, mainland China and Hong Kong.

Discovery of a novel ALK/ROS1/FAK inhibitor, APG-2449, in preclinical non-small cell lung cancer and ovarian cancer models

BackgroundTyrosine kinase inhibitors (TKIs) are mainstays of cancer treatment. However, their clinical benefits are often constrained by acquired resistance. To overcome such outcomes, we have rationally engineered APG-2449 as a novel multikinase inhibitor that is highly potent against oncogenic alterations of anaplastic lymphoma kinase (ALK), ROS proto-oncogene 1 receptor tyrosine kinase (ROS1), and focal adhesion kinase (FAK). Here we present the preclinical evaluation of APG-2449, which exhibits antiproliferative activity in cells carrying ALK fusion or secondary mutations.MethodsKINOMEscan® and LANCE TR-FRET were used to characterize targets and selectivity of APG-2449. Water-soluble tetrazolium salt (WST-8) viability assay and xenograft tumorigenicity were employed to evaluate therapeutic efficacy of monotherapy or drug combination in preclinical models of solid tumors. Western blot, pharmacokinetic, and flow cytometry analyses, as well as RNA sequencing were used to explore pharmacokinetic–pharmacodynamic correlations and the mechanism of actions driving drug combination synergy.ResultsIn mice bearing wild-type or ALK/ROS1-mutant non-small-cell lung cancer (NSCLC), APG-2449 demonstrates potent antitumor activity, with correlations between pharmacokinetics and pharmacodynamics in vivo. Through FAK inhibition, APG-2449 sensitizes ovarian xenograft tumors to paclitaxel by reducing CD44+ and aldehyde dehydrogenase 1-positive (ALDH1+) cancer stem cell populations, including ovarian tumors insensitive to carboplatin. In epidermal growth factor receptor (EGFR)-mutated NSCLC xenograft models, APG-2449 enhances EGFR TKI-induced tumor growth inhibition, while the ternary combination of APG-2449 with EGFR (osimertinib) and mitogen-activated extracellular signal-regulated kinase (MEK; trametinib) inhibitors overcomes osimertinib resistance. Mechanistically, phosphorylation of ALK, ROS1, and FAK, as well as their downstream components, is effectively inhibited by APG-2449.ConclusionsTaken together, our studies demonstrate that APG-2449 exerts potent and durable antitumor activity in human NSCLC and ovarian tumor models when administered alone or in combination with other therapies. Abstract 207: MET inhibition enhances the effect of radiation in MET mutated non-small cell lung cancer brain metastasis patient derived xenografts

Abstract Objective: The MET receptor is mutated in 3-4% and amplified in 1-6% of patients with non-small cell lung cancer. The most common MET mutation is in exon 14, which results in deletion of the intracellular juxtamembrane domain of the receptor, leading to enhanced signaling. Mutation at Asp-1000 in the juxtamembrane region has also been reported and can lead to inhibition of apoptosis and cell proliferation. MET is involved in multiple pathways associated with radiation response. Therefore, we investigated the effects of inhibiting MET in combination with radiation in two preclinical non-small cell lung cancer (NSCLC) brain metastasis patient derived xenograft (PDX) models. Methods: Surgically obtained tissue was implanted subcutaneously into immunodeficient mice. Histology and DNA loci were compared between original tumor and PDX. DNA sequencing was performed on tumors for mutation analysis. In vivo growth responses to the MET inhibitor capmatinib or savolitinib with and without radiation were assessed. Radiation was delivered in 10 daily fractions of 2 Gy. Drug was administered by oral gavage daily 1 hour prior to radiation at a dose of 2.5 mg/kg for savaolitnib or 20 mg/kg for capmatinib for 10 days. Immunohistochemistry (IHC) was performed to evaluate MET signaling and proliferation. Results: PDXs were successfully established from two patients with MET mutated NSCLC brain metastases: a lung adenocarcinoma with a MET exon 14 skipping mutation and a lung sarcomatoid carcinoma with adenocarcinoma component with an Asp-1000 frameshift mutation. Morphologically, strong retention of cytoarchitectural features was observed between original patient tumors and PDXs. Short tandem repeat analysis confirmed 100% matching of alleles between patient tumors and PDXs. DNA sequencing confirmed METex14 and MET Asp-1000 frameshift mutation. In the METex14 PDX, savolitinib alone significantly inhibited tumor growth with a growth inhibition value of 46% (p<0.01). Combination of savolitinib and radiation significantly delayed growth compared to vehicle control, savolitinib alone or radiation alone (p<0.001). The absolute growth delay was 42.4 days for savolitinib plus radiation treatment compared to 9.3 days for savolitinib alone and 28.2 days for irradiation. In the MET Asp-1000 PDX, capmatinib and radiation significantly delayed growth compared the other treatment arms (p<0.01). IHC demonstrated inhibition of phospho-MET and pS6 and a decrease in Ki67 with MET inhibition. Conclusion: Inhibition of MET enhanced the effect of radiation in our two preclinical in vivo MET mutated NSCLC brain metastasis models. Additional studies are currently underway evaluating the mechanisms of radiation sensitization and the efficacy of this combination in MET amplified PDX models. Citation Format: Kwangok P. Abstract 3328: Antitumor activity of BLU-945 and BLU-701 as single agents and in combination in EGFR L858R-driven models of NSCLC

Abstract BACKGROUND: Lung cancer is the leading cause of cancer death globally. EGFR mutations are the most common genomic drivers of non-small cell lung cancer (NSCLC), occurring in ~17% and up to 50% of Caucasian and Asian patients, respectively, with exon 19 deletions (ex19del) and L858R substitutions being the most prevalent activating EGFR mutations (EGFRm). Outcomes have improved in patients with EGFR-driven NSCLC since the introduction of EGFR tyrosine kinase inhibitors (TKIs); however, treatment resistance almost inevitably occurs. It has been reported that patients harboring the EGFR L858R mutation, who represent 3.4% of lung adenocarcinoma cases in North America and 23% in Asia, have poorer outcomes than those with EGFR ex19del, suggesting a significant unmet need in this population. BLU-945 and BLU-701 are investigational, reversible, selective, and orally available TKIs optimized for use as single-agent or combination therapy to effectively suppress activating and on-target resistance EGFR mutants while sparing WT EGFR, and have the potential to treat or prevent central nervous system metastases. In vivo studies previously demonstrated the antitumor activity of BLU-945 against EGFR L858R/T790M and EGFRm/T790M/C797S mutants, and BLU-701 against EGFR ex19del and EGFRm/C797S mutants. Because EGFR L858R-driven NSCLC constitutes a large population of patients who may not be receiving the same benefit from third-generation (3G) TKIs as those with EGFR ex19del, studies were conducted to evaluate the antitumor activity of BLU-945 and BLU-701, as single agents and in combination, in preclinical NSCLC tumor models driven by L858R in the absence of the T790M mutation. METHODS: The in vivo antitumor activities of BLU-945 and BLU-701, as single-agents or in combination, were evaluated in 2 EGFR L858R-driven patient-derived xenograft (PDX) subcutaneous tumor models and in an engineered EGFR L858R/C797S-driven Ba/F3 cell line-derived xenograft (CDX) subcutaneous tumor model. RESULTS: Oral daily administration of single-agent BLU-945 and single-agent BLU-701 resulted in sustained tumor regression in 2 PDX models of EGFR L858R NSCLC. Compared with single agents in an EGFR L858R/C797S Ba/F3 CDX tumor model, combination of BLU-945 with BLU-701 resulted in marked antitumor activity and prolonged tumor regression. CONCLUSION: The in vivo antitumor activities of BLU-945 and BLU-701 in preclinical tumor models suggest that both BLU-945 and BLU-701 have the potential to be used in patients with EGFR L858R-driven NSCLC, including those who are treatment naïve or previously treated with 3G TKI. Combining BLU-945 and BLU-701 may enable coverage of frequent on-target resistance mechanisms, including the EGFR C797S mutation, in addition to the common L858R activating mutation. First-in-human phase I results of APG-2449, a novel FAK and third-generation ALK/ ROS1 tyrosine kinase inhibitor (TKI), in patients (pts) with second-generation TKI-resistant ALK/ROS1+ non–small cell lung cancer (NSCLC) or mesothelioma.

9071 Background: Investigational agent APG-2449 is a novel, orally active FAK inhibitor and a third-generation ALK/ROS1 TKI that has shown potent activity against a range of ALK-resistant mutations, including G1202R, L1196M, V1180L, E1210K, S1206F, G1269A, F1174L, I1171S, and C1156Y in preclinical NSCLC, mesothelioma, and other solid tumor models. Methods: This dose escalation and dose expansion study evaluated APG-2449 in patients with second-generation TKI-resistant ALK/ROS1+ NSCLC or mesothelioma. APG-2449 was administered orally once daily at the assigned doses on a 28-day cycle using a “3+3” dose escalation design under fasted/fed conditions. Study aims were to assess safety/tolerability, recommended phase 2 dose (RP2D), pharmacokinetics (PK), pharmacodynamics (PD), and efficacy. Results: As of the data cutoff date of December 30, 2021, 84 pts (median age 52 [range 21-78] years; 42% female) with NSCLC or mesothelioma enrolled were treated with APG-2449 at doses ranging from 150 to 1,500 mg. PK analyses indicated an approximately dose-proportional increase in plasma exposure under fed conditions across dose levels tested. Cerebrospinal fluid PK analyses showed that APG-2449 was brain-penetrant. Low-fat meals increased APG-2449 Cmax and AUC by approximately 40% to 80% compared to fasting conditions. Based on PK, biomarker, efficacy, and safety results, the RP2D was determined to be 1,200 mg. Four partial responses (PRs) were observed in 14 ALK+ pts resistant to second-generation TKIs treated at the RP2D. Another pt with the G1202R mutation following alectinib treatment had tumor shrinkage of 27.9%. Among 8 pts with brain metastases, 1 complete response and 3 PRs were observed intracranially. In 10 TKI-naïve pts, the overall response rate was 80% (ALK+, 6/8; ROS1+, 2/2) and the disease control rate (DCR) was 100%. Preliminary biomarker data showed decreased FAK phosphorylation in peripheral blood mononuclear cells and increased IFN-γ levels in serum after multiple doses of APG-2449. No dose-limiting toxicity was observed. A total of 66 (78.6%) pts experienced treatment-related adverse events (TRAEs). The most frequent TRAEs included elevated blood creatinine (33.3%), ALT (25.0%), and AST (19.0%) levels and gastrointestinal disorders: nausea (22.6%), vomiting (17.9%), and diarrhea (13.1%). Only 6 (7.1%) TRAEs were grade ≥ 3. Conclusions: APG-2449 has a favorable safety and PK profile and was well tolerated in 84 subjects. Preliminary efficacy was observed in pts whose disease was resistant to second-generation TKIs, especially among those with brain metastases, and in TKI-naïve pts. Abstract LBA035: Canakinumab or Pembrolizumab as Monotherapy or in Combination as Neoadjuvant Therapy in Patients With Resectable Non-Small Cell Lung Cancer: CANOPY-N Trial

Abstract Background: Complete surgical resection is the standard treatment for patients with stage I-IIIA non-small cell lung cancer (NSCLC). Five-year survival rates in this population range from 19% to 50%, with most patients dying from distant recurrence. As neoadjuvant/adjuvant chemotherapy yields a modest 5% absolute overall survival benefit for these patients with NSCLC, new treatment options are needed. Preliminary data with PD-1 or PD-L1 inhibitors as neoadjuvant therapy have shown major pathologic responses (MPRs) or pathologic complete responses in patients with resectable NSCLC. In the CANTOS study, reduced incidence of NSCLC and decreased lung cancer–related mortality were observed in a dose-dependent manner in patients with atherosclerosis who received canakinumab (interleukin 1β inhibitor) versus those who received placebo. In preclinical NSCLC humanized models, treatment with canakinumab ± anti–PD-1 inhibitor has shown promising activity, and a synergistic effect was observed with this combination, which may enhance the efficacy of PD-1 inhibition by modulating pro-tumor inflammation in the tumor microenvironment. In this context, the CANOPY-N study was designed to evaluate the effect of canakinumab or pembrolizumab as monotherapy or in combination as neoadjuvant treatment for patients with resectable NSCLC. Methods: CANOPY-N (NCT03968419) is a Phase II, randomized, open-label study evaluating the effect of canakinumab or pembrolizumab as monotherapy or in combination as neoadjuvant treatment in patients with resectable NSCLC. Patients with histologically confirmed stage IB-IIIA NSCLC (excluding N2 and T4 tumors), no prior systemic therapy, Eastern Cooperative Oncology Group performance status 0 or 1, and eligibility for surgery with a planned resection in approximately 4-6 weeks after first dose of study treatment are eligible to participate. An archival (if obtained up to 6 months before first day of treatment) or new biopsy is required. ATR Inhibitor M6620 (VX-970) Enhances the Effect of Radiation in Non-Small Cell Lung Cancer Brain Metastasis Patient-Derived Xenografts.

M6620, a selective ATP-competitive inhibitor of the ATM and RAD3-related (ATR) kinase, is currently under investigation with radiation in patients with non-small cell lung cancer (NSCLC) brain metastases. We evaluated the DNA damage response (DDR) pathway profile of NSCLC and assessed the radiosensitizing effects of M6620 in a preclinical NSCLC brain metastasis model. Mutation analysis and transcriptome profiling of DDR genes and pathways was performed on NSCLC patient samples. NSCLC cell lines were assessed with proliferation, clonogenic survival, apoptosis, cell cycle, and DNA damage signaling and repair assays. NSCLC brain metastasis patient-derived xenograft models were used to assess intracranial response and overall survival. In vivo IHC was performed to confirm in vitro results. A significant portion of NSCLC patient tumors demonstrated enrichment of DDR pathways. DDR pathways correlated with lung squamous cell histology; and mutations in ATR, ATM, BRCA1, BRCA2, CHEK1, and CHEK2 correlated with enrichment of DDR pathways in lung adenocarcinomas. M6620 reduced colony formation after radiotherapy and resulted in inhibition of DNA DSB repair, abrogation of the radiation-induced G2 cell checkpoint, and formation of dysfunctional micronuclei, leading to enhanced radiation-induced mitotic death. The combination of M6620 and radiation resulted in improved overall survival in mice compared with radiation alone. In vivo IHC revealed inhibition of pChk1 in the radiation plus M6620 group. M6620 enhances the effect of radiation in our preclinical NSCLC brain metastasis models, supporting the ongoing clinical trial (NCT02589522) evaluating M6620 in combination with whole brain irradiation in patients with NSCLC brain metastases.

Abstract CT237: Canakinumab or pembrolizumab as monotherapy or in combination as neoadjuvant therapy in patients with surgically resected non-small cell lung cancer: CANOPY-N trial

Abstract Background: Complete surgical resection is the standard treatment for patients with stage I-IIIA non-small cell lung cancer (NSCLC). Five-year survival rates range from 19% to 50%, with most patients dying from distant recurrence. Neoadjuvant or adjuvant chemotherapy improves overall survival (OS) by only 5% in patients with NSCLC, and new treatment options are needed. Preliminary data for PD1 or PD-L1 inhibitors as neoadjuvant therapy have shown major pathologic responses (MPRs) or pathologic complete responses (pCRs) in patients with early-stage NSCLC. The CANTOS study demonstrated reduced incidence of NSCLC and decreased lung cancer-related mortality with canakinumab (interleukin 1β inhibitor) versus placebo, in a dose-dependent manner for patients with atherosclerosis. In preclinical NSCLC humanized models, treatment with canakinumab ± anti-PD-1 inhibitor could lead to antitumor activity. RTID-01. RADIOSURGERY FOLLOWED BY TUMOR TREATING FIELDS FOR BRAIN METASTASES (1-10) FROM NSCLC IN THE PHASE 3 METIS TRIAL

Abstract BACKGROUND Tumor Treating Fields (TTFields) are non-invasive, loco-regional, anti-mitotic treatment modality comprising alternating electric fields. TTFields have demonstrated efficacy in preclinical non-small cell lung cancer (NSCLC) models. TTFields treatment to the brain was safe and extended overall survival in newly-diagnosed glioblastoma. The objective of the METIS study [NCT02831959] is evaluation of the efficacy and safety of TTFields in NSCLC patients with brain metastases. METHODS NSCLC patients (N=270) with 1-10 brain metastases were randomized 1:1 to stereotactic radio surgery (SRS) followed by continuous TTFields ((150 kHz, > 18 hours/day) within 7 days of SRS or supportive care. The portable device delivered TTFields to the brain using 4 transducer arrays, while patients received the best standard-of-care for systemic disease. Patients were followed every two months until second intracranial progression. Key inclusion criteria: KPS ≥70, new diagnosis of 1 inoperable or 2–10 supra- and/or infratentorial brain metastases from NSCLC amenable to SRS; and optimal therapy for extracranial disease. Prior WBRT, surgical resection of metastases, or recurrent brain metastases were exclusionary. Primary endpoint was time to 1st intracranial progression. Secondary endpoints included time to neurocognitive failure (HVLT, COWAT and TMT), overall survival, radiological response rate (RANO-BM and RECIST V1.1); quality-of-life; adverse events; time to first/second intracranial progression for patients with 1–4 and 5–10 brain metastases; bi-monthly intracranial progression rate from 2–12 months; and time to second intracranial and distant progression. The sample size (N=270) was calculated using a log-rank test (Lakatos 1988 and 2002) with 80% power at two sided alpha of 0.05 to detect a hazard ratio of 0.57. On September, 2019, an independent Data Monitoring Committee (DMC) reviewed METIS trial data collected to that point. The DMC concluded that no unexpected safety issues had emerged and recommended continuation of the METIS study as planned.

Abstract 4455: Combination of bempegaldesleukin and anti-CTLA-4 prevents metastatic dissemination after primary surgery or radiation therapy in a preclinical model of non-small cell lung cancer

Abstract Purpose: Surgical resection or stereotactic ablative radiation therapy (SBRT) in early stage non-small cell lung cancer (NSCLC) have local control rates of 90% or higher. Current treatment paradigms effectively control primary tumor sites, but distant relapse remains a challenge with metastatic failure in more than 30% of patients with primary tumors larger than 5 cm. We hypothesized that adjuvant immune checkpoint blockade with anti-CTLA4 (C4) and /or bempegaldesleukin (NKTR-214), a CD122-preferential pegylated-interleukin-2 (IL2) pathway agonist,in combination with primary tumor ablative radiation or surgical resection would reduce metastases in a spontaneously metastatic syngeneic Lewis Lung Carcinoma (LLC) model. Methods: LLC flank tumors were established in C57BL/6 mice (n=6 per replicate, 12 total). When tumors were ~100 mm3 they were randomized to receive: vehicle only (VO); NKTR-214 (16 μg on Days 6, 15, 24) + C4 (200 μg IP on Days 4, 7, 10); RT (8Gy x 3 on Days 1, 2, 3); RT + C4, RT + NKTR-214; RT + C4 + NKTR-214; Primary surgery (Day 16), or Surgery + NKTR-214 + C4. Primary tumor size, survival, and a binary assessment of metastatic disease was made for each mouse. Flow cytometry studies examined tumor immune cell infiltrates on Day 21 in mice treated with VO, RT, RT + C4, C4 + NKTR-214, or RT + C4 + NKTR-214. Results: NKTR + C4 prevented development of spontaneous metastasis (p < 0.01) but did not control primary disease compared to VO treatment. RT alone significantly reduced primary tumor size compared to VO control (p < 0.01), but these mice all developed metastases. Adjuvant NKTR-214 + C4 after RT reduced development of spontaneous metastasis in mice treated by this regimen by 91% and 73% compared to RT alone and RT + C4 respectively. RT + C4 + NKTR-214 also significantly improved survival compared to both RT + C4 and RT + NKTR-214 (p = 0.02, p = 0.003). 58% of mice treated with RT + C4 + NKTR-214 cleared their primary tumors compared to 16% with RT + C4 and 0% in RT + NKTR-214 at day 30 (p = 0.03, p < 0.01). All mice clearing primary tumors rejected LCC rechallenge. Flow cytometric immuneprofiling of the primary tumors demonstrated increased CD8+T cells, NK cells (NK1.1+CD3−), and NKT cells (NK1.1+CD3+) with NKTR-214 + C4 w/wo RT treatment compared to VO controls. NKTR-214 + C4 prior to surgical resection also reduced the development of spontaneous lung metastases compared to surgery alone (p < 0.01). Conclusions: Addition of systemic NKTR-214 + C4 treatment to RT or primary surgical resection reduces development of distant metastatic disease in a preclinical NSCLC model. Moreover, treatment with NKTR-214 + C4 increases infiltration of innate and adaptive immune effector cells in the primary tumor. This treatment strategy has important translational potential to prevent distant relapse in patients undergoing definitive local therapies for early stage NSCLC. Citation Format: Ryan J. Abstract 2196: SRF231, a fully human CD47 antibody, potentiates the effects of opsonizing antibodies and cytotoxic chemotherapies in preclinical cancer models

Abstract CD47 is widely expressed on tumor cells and is an important checkpoint of the innate immune response. SRF231, an investigational fully human IgG4 anti-CD47 antibody, blocks CD47-SIRPα interaction and exerts anti-tumor activity via phagocytosis and cell death in a manner dependent on Fc-engagement with CD32a, leading to enhanced myeloid activity. Human IgG1 Fc-containing antibodies that target cancer antigens (opsonizing antibodies) can stimulate macrophage-mediated antibody-dependent cellular phagocytosis (ADCP) in addition to NK cell-mediated antibody-dependent cellular cytotoxicity (ADCC). Also, several chemotherapies trigger cell death response pathways that generate additional “eat-me” signals on tumor cells, rendering them more susceptible to SRF231. Given the dual mechanism of action of SRF231, combination therapies that induce ADCP or cell death were evaluated for their ability to potentiate SRF231-mediated anti-tumor responses in hematologic and solid tumor models. The effect of SRF231 alone and in combination with opsonizing antibodies was evaluated in multiple myeloma (MM) models. In vitro, SRF231 induced phagocytosis of MM cells by human monocyte-derived macrophages. Enhanced ADCP was observed with SRF231 in combination with opsonizing antibodies elotuzumab (anti-SLAMF7) and daratumumab (anti-CD38). In vivo, SRF231 monotherapy following single- and repeat-dosing regimens resulted in 50-80% tumor growth inhibition and significantly increased median survival time in the preclinical MM xenograft models OPM2 and MM1.S. Furthermore, SRF231 strongly synergized with a sub-optimal dose of elotuzumab leading to increased complete response (CR) rates in both models. The effect of SRF231 alone and in combination with chemotherapies was also assessed in non-small cell lung cancer (NSCLC) models. In vivo, SRF231 monotherapy induced anti-tumor activity and significantly increased median survival time in preclinical EGFR mutant (H1975) and EGFR wildtype (A549) NSCLC xenograft models. Cisplatin monotherapy induced modest H1975 anti-tumor activity; however, combination with SRF231 potentiated the effect of cisplatin in this model. Additionally, combination of SRF231 with docetaxel resulted in a 45% CR rate and a significant delay in H1975 tumor regrowth, as compared to a 100% regrowth rate with docetaxel monotherapy. In summary, SRF231 demonstrated significant anti-tumor activity in combination with the anti-SLAMF7 opsonizing antibody, elotuzumab, in preclinical MM xenograft models. Moreover, SRF231 potentiated the effects of taxane and platinum-based standard of care chemotherapies in preclinical NSCLC xenograft models. Prior WBRT or surgical resection of metastases, a single resectable lesion or recurrent brain metastases were exclusionary. Primary endpoint was time to 1st intracranial progression. Secondary endpoints included time to neurocognitive failure (HVLT, COWAT and TMT), overall survival, radiological response rate (RANO-BM and RECIST V1.1); quality-of-life; adverse events; time to first/second intracranial progression for patients with 1–4 and 5–10 brain metastases; bi-monthly intracranial progression rate from 2–12 months; and time to second intracranial and distant progression. The sample size (N=270) was calculated using a log-rank test (Lakatos 1988 and 2002) with 80% power at two sided alpha of 0.05 to detect a hazard ratio of 0.57. In August 2018, an independent Data Monitoring Committee (DMC) performed a review of the METIS trial data collected to that point. The DMC concluded that no unexpected safety issues had emerged and recommended continuation of the METIS study as planned.

1594TiP - Radiosurgery followed by tumour treating fields (TTFields) for brain metastases (1-10) from NSCLC in the phase III METIS trial

BackgroundTumor Treating Fields (TTFields) are non-invasive, loco-regional, anti-mitotic treatment modality comprising alternating electric fields. TTFields have demonstrated efficacy in preclinical non-small cell lung cancer (NSCLC) models. TTFields treatment to the brain was safe and extended overall survival in newly-diagnosed glioblastoma. The METIS study [NCT02831959] investigates the efficacy and safety of TTFields in NSCLC patients with brain metastases. Trial designNSCLC patients (N=270) with 1-10 brain metastases are randomized 1:1 to stereotactic radio surgery (SRS) followed by continuous TTFields ((150kHz, > 18hours/day) within 7 days of SRS or supportive care. The TTFields portable device delivers TTFields to the brain using 4 transducer arrays, while patients receive the best standard-of-care for their systemic disease. Patients are followed every two months until second intracranial progression. Key inclusion criteria: KPS ≥70, new diagnosis of 1 inoperable or 2–10 supra- and/or infratentorial brain metastases from NSCLC amenable to SRS; KPS ≥70; and optimal therapy for extracranial disease.
