Vesical Calculus Around Neglected Foley’s Catheter: Case Report

Background: Long term bladder catheterization is complicated by bacteriuria, encrustation by mineral salts, stone formation etc. In the presence of an indwelling catheter, the development of bacteriuria is inevitable and the colonization of the catheter withurease producing organisms alkalinizes the urine, promoting the precipitation of calcium phosphate salts and a biofilm of bacteria on the surface of the catheter. Aseptic techniques at catheter insertion and prompt treatment of urinary tract infections will thereforeminimize the occurrence of intravesical calculi formation. A review of literature suggests that the longest reported case of retained bladder catheter with intravesical calculus formation is two years. We report two cases of long term indwelling catheter, five yearsand two years respectively, with intravesical calculus formation around the catheter.Case Report: Case 1. A 44 year old man with urethral strictures retained a latex suprapubic Foley’s catheter for five years and reported with pericatheter leakage of urine A stone found to have formed around the catheter was removed at open suprapubic cystolithotomy. Case 2. A 61year old man retained his urethral cathetern assed for urine retention for two years. He presented in acute retention and a vesical calculus found to be around the catheter was removed at suprapubic cystolithothomy.Conclusion: Long term indwelling catheter usually gets complicated by urethral encrustation and stone formation. Aseptic techniques at urethral catheterization and proper patient education on regular catheter change are required to minimize stone formation around urethral catheter.

Calcium phosphate precipitation in nitrified wastewater from the potato-processing industry

ABSTRACTIncreasing environmental concerns and the awareness of the finite nature of natural resources make the valorization of waste materials to become a real challenge. The objective of the current research is to investigate the possibility of phosphate recovery as calcium phosphate salts from the wastewater from the potato-processing industry. Batch tests demonstrated that at high pH, struvite and calcium carbonate precipitations are competitive processes and that bicarbonate inhibits the precipitation of calcium phosphate salts. A biological nitrification of the wastewater removed the buffering capacity, the competitive formation of struvite and paved the way for phosphate precipitation as calcium phosphate salts as it also led to the simultaneous removal of (bi)carbonates. It is demonstrated that 75% of the phosphate precipitated as calcium phosphate at a [Ca2+]/[P] ratio of 2.5 at pH 8.5 and as such it provides a convenient alternative for the currently applied struvite processes in the agro-industrial industry.

Crystallisation of minerals from concentrated saline dairy effluent

An understanding of crystallisation within saline effluents is important for the design of both brine crystallisers and brine disposal ponds. In this work, crystallisation of a saline effluent concentrate from the Australian dairy industry has been examined at 22 wt% and 30 wt% total solids and at temperatures between 10 and 70 °C. The precipitation occurs more rapidly at higher temperatures. This trend is dictated by precipitation of calcium phosphate salts, albeit the major constituents of the mixture are NaCl and lactose. The crystallisation induction time can be shortened by introducing cavitation induced by ultrasound. In particular, the use of two short acoustic pulses between 3.7 J/g and 16 J/g at 20 kHz spaced ten minutes apart has maximum impact upon both induction time and crystal size. It is believed that the first ultrasound pulse either generates new nuclei or enhances the mass transfer of solute toward the surface of sub-micron growing crystals. Conversely, the second pulse disrupts the growing crystals and forms secondary nuclei. The ultrasound cannot shift the solution equilibrium and so is not able to improve the low crystal yield. To increase this total yield, further evaporation is necessary. The work provides direction to personnel in the dairy industry of the feasibility of brine crystallisation with respect to energy demand and solid recovery.

Sand consolidation with calcium phosphate–polyelectrolyte composites

A new method for the consolidation of loose sand formations has been developed. The method involves in situ precipitation of a composite calcium phosphate–polyelectrolyte salt that binds together with loose sand grains, thus resulting to their consolidation. Three different polyelectrolytes (PE) were tested, i.e., polyacrylic acid (PAA), polyallylamine hydrochloride (PAH), and polyethylenimine (PEI). The effect of PE tested on the thermodynamics and the kinetics of precipitation of calcium phosphate salts was investigated. Three types of experiments were done. Investigation of the adsorption of PE on either hydroxyapatite (Ca 5(PO 4) 3OH, HAP) crystals or on sand grains. Measurement of the kinetics of heterogeneous nucleation of HAP on the solid substrates and the mechanical properties of the obtained crystals in batch experiments of low and high supersaturation solutions, respectively. Evaluation of the consolidation in sand packs in order to investigate the effectiveness of the method. The crystallization rates, R p, on HAP crystals in the presence of the PE tested were found in the order R p PAA > R p PEI > R p PAH , while nucleation and crystal growth on silicate sand took place only in the absence of adsorbed PE. PAH favored strongly the consolidation process, whereas PEI and PAA resulted in the formation of poorly consolidated grain agglomerates.

Chemical synthesis of phosphoseryl-phosphoserine, a partial analogue of human salivary statherin, a protein inhibitor of calcium phosphate precipitation in human saliva.

Human salivary secretions are supersaturated with respect to basic calcium phosphates but spontaneous precipitation of these salts from saliva, or surface-induced precipitation of calcium phosphates onto dental enamel, does not normally occur. This unexpected stability has been attributed to the inhibitory activities of two kinds of salivary phosphoproteins: statherin and the acidic, proline-rich phosphoproteins (PRP). Investigation of the structure-function relationships of statherin, the most potent inhibitor of primary (spontaneous) and secondary (seeded) precipitation of calcium phosphate salts in human saliva has been limited to studies of peptide segments obtained from the native peptide by specific proteolysis. Solid phase peptide synthesis (SPPS) is a useful and potentially more flexible alternative. Phosphoserine residues (positions 2 & 3) play critically important roles in the precipitation-inhibition activities of statherin, but SPP synthesis of these phosphorylated peptides is precluded because of the instability of phosphoserine residues in the presence of HF. Thus, this peptide was synthesized by solution-phase methods. The dipeptide possessed substantial inhibitory activity in assays for inhibition of both primary and secondary precipitation of calcium phosphate salts, but was not as active as either N-terminal tryptic hexapeptide of statherin or intact statherin. Syntheses of other model phosphorylated peptides are underway to expand the structure-function relationships.

Inhibition of calcium phosphate precipitation by human salivary acidic proline-rich proteins: structure-activity relationships.

Absence of precipitation of calcium phosphate salts onto tooth surfaces from human saliva, which is supersaturated with respect to calcium phosphate salts, has been attributed in part to the presence in the salivary secretions of a group of acidic proline-rich phosphoproteins (PRP). These macromolecules are considered to act by adsorbing onto dental enamel where they inhibit surface-induced precipitation of calcium phosphate salts. The inhibitory activity is known to be associated primarily with the amino-terminal region of the PRP. The aim of this study was to determine the features of the primary structure of this molecular segment responsible for inhibitory activity. The 30-residue, amino-terminal segment of PRP-3, which contains the two phosphoserines and 11 of the 13 carboxyl groups present in PRP-3, was obtained by tryptic digestion. This peptide, designated PRP-3(TI), was treated with thermolysin to give the monophosphopeptides, Val-PSer-Gln-Glu-Asp-Val-Pro and Leu-Val-Ile-Ser-Asp-Gly-Gly-Asp-PSer-Glu-Gln, and with alkaline phosphatase to give the dephosphorylated analog, PRP-3(TI)DP. The inhibitory activities of PRP-3(TI) and the derived peptides, a synthetic peptide, phosphoseryl-phosphoserine (PSer-PSer), and O-phosphoserine (PSer), were determined using an assay based on inhibition of seeded precipitation of calcium phosphate. Inhibitory activities, expressed as concentrations of inhibitors required to give standard inhibitory activities, were PRP-3(TI), 0.59 microM; PSer-PSer, 3.5 microM; the two monophosphopeptides, 29 and 32.5 microM; PRP-3(TI)DP, 56 microM; PSer, 329 microM.(ABSTRACT TRUNCATED AT 250 WORDS)

Saturation of human salivary secretions with respect to calcite and inhibition of calcium carbonate precipitation by salivary constituents.

The state of saturation of human salivary secretions with respect to calcite has been investigated. This property cannot be calculated exactly because of uncertainties in the values of the solubility product constant of calcite, the dissociation constants of carbonic acid, and PCO2 values of saliva. Minimum and maximum limits for this saturation, however, can be established using appropriate values for the constants and salivary PCO2. Values that give the minimum degree of saturation show that 8 of the 70 samples of human saliva investigated would be supersaturated with respect to calcite, while 64 of the 70 samples appeared to be supersaturated when values giving the maximum degree of saturation were used. In the latter case, the ratio of ionic activity products to solubility product was above 10 for several samples and over 18 for the most supersaturated sample. Since these results show that supersaturation of saliva with respect to calcite may be a common condition, human salivary secretions were investigated for the presence of inhibitors of calcite precipitation. Inorganic phosphate and the acidic proline-rich proteins, known to be inhibitors of calcite precipitation, and human salivary statherin, now shown to have a similar activity, are present in saliva at concentrations considerably higher than those required to inhibit calcite precipitation under salivary conditions. Quantitatively, phosphate is by far the most important inhibitor of calcite precipitation present in saliva, suggesting that inhibition of calcite precipitation by the macromolecules may be of secondary significance. It seems more likely that the function of these molecules is to inhibit precipitation of calcium phosphate salts, as previously proposed. These different inhibitory activities, however, are likely to be factors in the differences in composition of oral and dental calculi in different species, and may need to be considered in the formation of calcite stones in the pancreas.

Relationship between concentration of human salivary statherin and inhibition of calcium phosphate precipitation in stimulated human parotid saliva.

Human salivary secretions are supersaturated with respect to the calcium phosphate salts which form dental enamel, a property which provides important protection for the teeth. We previously proposed that statherin, a 43-residue phosphopeptide, plays a key role in this protective system by inhibiting or delaying potentially harmful precipitation of calcium phosphate salts in the salivary glands and mouth. The purpose of the present study was to determine if the concentrations of statherin in saliva, despite their wide normal range, are high enough to fulfill this function. Concentrations of statherin in stimulated human parotid saliva samples from 36 female and 32 male subjects, aged from 17 to 30 years, were determined by a single radial immunodiffusion method. Values found ranged from 3.0 to greater than 27.3 microM, with a mean value of 12.8 (S.D. +/- 5.46) microM. At concentrations below these values, statherin inhibited spontaneous precipitation of calcium phosphate salts from an assay system which was more supersaturated with respect to dicalcium phosphate dihydrate, and comparably supersaturated with respect to hydroxyapatite, than were human saliva samples. The inhibitory activities of five of the 65 stimulated parotid saliva samples assayed were greater than would be anticipated from their statherin concentrations. This unexplained discrepancy is not associated with the presence of the acidic proline-rich proteins in saliva, although these proteins also affect calcium phosphate precipitation. The results of this study show that statherin is present in stimulated human parotid saliva at concentrations and levels of activity which are consistent with its proposed biological function, and support the proposal that statherin plays a significant role in a system which provides a protective and reparative but stable environment for the teeth.

Primary structure of the active tryptic fragments of human and monkey salivary anionic proline-rich proteins.

The primary structures of the four human salivary anionic proline-rich proteins and an analogous protein from the saliva of a monkey (Macaca arctoides) have been further investigated. These proteins possess the unusual property of inhibiting crystal growth of calcium phosphate salts, and it has been proposed that they play an important role in the mouth, by preventing precipitation of calcium phosphate salts from the supersaturated saliva. The tryptic fragments responsible for this activity have been isolated from all five proteins and their complete amino acid sequences determined and compared. These active fragments have been unequivocally identified as the amino-terminal segment in all five proteins. The structures of the four human fragments are identical except for the presence of Asn at residue 4 in PRP-1 and -3 instead of Asp found in PRP-2 and -4. Comparison of the 30 residue human fragments with the monkey fragment shows 18 residues to be identical in these peptides, providing that residue 1 of the monkey fragment is aligned with residue 3 of the human proteins. Theses studies constitute the next step in determining the mechanism of action of these unusual proteins, and in determining their minimum chain length required for inhibitory activity.
