Inadequate weight gain and factors influencing it among preterm neonates in neonatal intensive care units in the Amhara region, Ethiopia, in 2022.

Adequate weight gain is crucial for the health and development of preterm neonates admitted to neonatal intensive care units (NICUs). Understanding the factors influencing weight gain in this vulnerable population is essential for improving outcomes. This study aimed to assess the weight gain status and associated factors among preterm neonates admitted to NICUs in specialized hospitals in the Amhara region of Ethiopia. A cross-sectional study design involving 363 preterm neonates admitted to NICUs in specialized hospitals within the Amhara region was used. Data were collected using structured questionnaires and the Kobo Tool Box. Daily weight measurements were recorded for three consecutive days. Descriptive statistics, logistic regression analysis, and graphical presentations were utilized for data analysis and presentation. The study revealed that a significant proportion (80.8%) of preterm neonates experienced poor weight gain during their NICU stay. The factors significantly associated with poor weight gain were older maternal age, delayed initiation of enteral feeding, lack of kangaroo mother care (KMC), and inadequate antenatal care visits. Addressing the identified factors, such as providing adequate support during the antenatal period, promoting a timely initiation of enteral feeding, and encouraging KMC practices, is crucial for improving weight gain outcomes in preterm neonates. The findings highlight the importance of a comprehensive approach to neonatal care targeting both maternal and neonatal factors. Policymakers and healthcare providers should prioritize interventions aimed at optimizing weight gain in preterm neonates to improve these neonates' overall well-being and long-term outcomes.

Feasibility and acceptability of a novel biomedical device to prevent neonatal hypothermia and augment Kangaroo Mother Care in Kenya: Qualitative analysis of focus group discussions and key Informant Interviews.

Hypothermia is a leading newborn complication, especially among premature and/or low birth weight infants. Within low/middle-income countries where incubators and radiant warmers are often in short supply, leading to gaps in the thermal care chain, neonatal hypothermia underlies high rates of newborn morbidity and mortality. Kangaroo Mother Care/Skin-to-skin care is an effective method for prevention of hypothermia in premature and low birthweight babies but can be very burdensome for families and healthcare providers. Our international multidisciplinary team has developed a prototype for a wearable biomedical device ("NeoWarm") to provide continuous thermal care and augment kangaroo mother care practices in low-resource settings. The objective of this study was to assess the feasibility and acceptability of NeoWarm and to obtain user design feedback for an early prototype from among adult end-users in Western Kenya. We performed key informant interviews (n = 17) among healthcare providers and 5 focus group discussions (FGDs) among 3 groups of adult stakeholders of premature babies, including: (1) parents/family members of premature babies aged 6 weeks or less (3 FGDs); (2) healthcare providers of newborns (e.g., nurses; physicians; 1 FGD); (3) community opinion leaders and stakeholders (e.g., traditional birth attendants; pastors; village elders; 1 FGD). Content and thematic analyses of transcripts indicate that NeoWarm is acceptable and feasible in promoting facility-based kangaroo mother care in the Kenyan setting. Novel findings derived from respondents include (1) the ability of the device to potentially overcome several barriers to traditional kangaroo mother care methods and (2) user-driven encouragement to expand the use case of the device to potentially include community-based kangaroo mother care and neonatal transport. User design feedback obtained during the interviews informed several key design iterations for subsequent prototypes of the device.

Kangaroo Mother Care Practices in Tertiary Care NICUs Across India: A Cross-sectional Questionnaire-based Survey

Background: KMC is a global standard for LBW and preterm neonates, supported by evidence for reducing morbidities and mortality. Integrated into India’s Newborn Action Plan since 2014, aiming for single-digit neonatal mortality by 2030, KMC practices in India have not reached the optimal levels. Hence, we attempted to study the KMC practices in tertiary care NICUs across India. Methods: A cross-sectional online national survey among neonatologists in level III NICUs in India was conducted. Utilizing Google Forms and a structured questionnaire of 30 questions on KMC, one respondent per unit participated. Results: Of the 110 NICUs invited to participate, a total of 52 units responded (47.3%). All units have a written KMC policy and conventionally provide KMC to neonates with BW < 2000 g, while 36.5% units provide KMC for neonates with BW between 2000 and 2500 g. Though 86.5% of NICUs provide KMC to infants on respiratory support, only 13.5% practice KMC for mechanically ventilated neonates. The time to initiate KMC within 48–72 hours is prevalent in 44.2% of NICUs, with 54% of NICUs providing an average KMC duration of 4–8 hours/day. A significantly lower percentage of NICUs (28.9%) have a KMC follow-up program. Conclusion: The study highlights the need to boost the ongoing KMC training programs for HCWs to increase the coverage of KMC for LBWs and sick preterm neonates. This includes requiring more uniform and structured KMC practices across the country, especially upscaling of infrastructure, earlier initiation and longer duration of KMC, better monitoring of neonates in KMC, and strengthening the follow-up program.

Scenario of Newborn Care Practices in Private Healthcare Setting in India: An Insights From NFHS-5 Survey

Background: The private sector contributes significantly to health care service delivery in India. Study shows the prevalence of newborn care practices like early initiation of breastfeeding, kangaroo mother care, and hepatitis B birth dose practices in private sector hospitals across India. Methods: We conducted a basic analysis of unit-level data from the NFHS-5 survey; to understand the newborn care practices in private hospitals in India. Result: The place of the delivery has a significant impact on newborn care practices like kangaroo mother care, early breastfeeding within an hour of birth, and hepatitis-B vaccination at birth. The private sector accounts for 20.73 percent of deliveries wherein, 39.56 percent of private sector hospitals follow EIBF practice. Pre-lacteals were provided to 14.95 percent of the children within the first three days after birth. The better practice of kangaroo mother care was seen in the public sector. The prevalence of the hepatitis B vaccine at birth is very low with 3.25 percent in India.

Zero separation policy in small but stable neonates weighing 1500-2000 grams at birth: a single center study in New Delhi, India

Background Small but stable low birth weight (LBW) neonates have needs similar to babies of normal weight with the need for extra support with feeding and temperature maintenance. Most health facilities admit such infants in the neonatal unit leading to the separation of the mother and baby. This separation exposes the infant to a potentially contaminated environment of the Neonatal intensive care unit (NICU) which is hazardous for stable infants (Invasive interventions, Intravenous alimentation, Infections) and also hampers the establishment of breastfeeding. This study evaluated short-term outcomes of stable neonates weighing 1500-2000 grams at birth cared for in the mother-baby unit in the same room. as their mothers as per the ‘Zero-separation Policy’. Methods Neonates born vaginally with a birth weight of 1500-2000 grams with stable vitals were moved with their mothers to a 12-bed ’’mother-baby unit (MBU)“. Mothers were counseled regarding breastfeeding (BF), Kangaroo mother care (KMC), maintenance of general hygiene, and identification of danger signs. Infants developing moderate to severe hypothermia, hypoglycemia, feed intolerance (FI), jaundice nearing exchange transfusion range, respiratory difficulty, sepsis, seizures, or apnea, were moved to a neonatal unit for further management. Results Over 3 years, 489 neonates with a mean (± SD) birth weight of 1738 ± 102 grams and median gestation of 34 weeks (range: 32-41 weeks) were cared for with their mothers at the MBU. Seventy percent of infants exclusively received their mother’s own milk on day 1, which increased to more than 95% from day 4 onwards. Similarly, two-thirds of the mothers provided KMC for at least 5-8 hours on day 1, increasing to 85% by day 5. Neonatal hyperbilirubinemia requiring treatment was the most common morbidity (28.8%), most of which was managed at the bedside, followed by hypoglycemia (4.7%). Only 8% of neonates (n= 39/489) required transfer to the neonatal unit, mostly for hypoglycemia and hyperbilirubinemia. No baby developed hypothermia, apnea, FI, seizures, or hemodynamic instability. Successful discharge to home was accomplished in all neonates with no mortality. Conclusions Zero-separation policy is feasible in clinically stable low-weight or small neonates (of 1500-2000 grams) who can be nurtured with their mothers right from birth, ensuring timely feeding, KMC, and good hygienic practices.

Separation of Maternal and Newborn Care in US Hospitals: A Systemic Threat to Survival, Health and Well-Being

ABSTRACT Through greater understanding of past social, cultural, economic, political, scientific and technological forces which shaped our current health systems to separate mothers and newborn infants, we can begin to devise effective approaches to reshape these systems to meet the needs of mothers and newborn infants today. Medical science and technology have evolved vastly in the last century; however, effects of historical factors persist in our current health care systems, reflected in separate maternal and neonatal care in different departments with distinct guidelines, providers, and treatment locations. This separation prevents maternal-infant skin-to-skin contact and bonding, which significantly affects infant development, well-being, and that of their caregivers. We explore historical precedents for the separation of maternal-newborn care, including the transition from midwifery home care to hospital obstetric care, reasons for the increase in hospital births and hospital nursery development, and the effects of world wars, federal acts, health insurance, rooming-in practices, and the development of medical advances such as antibiotics, on hospital infrastructure. This information is evaluated in the context of modern scientific advancements to show that the conditions which shaped health systems to separate mothers and newborns in the past no longer hold. The insights gained will help to identify strategic actions to reshape health care systems to enable more integrated maternal-newborn care and the practice of Kangaroo Mother Care, and to improve survival outcomes and well-being for mothers, families, and their newborn infants.

Exploring factors influencing the uptake of kangaroo mother care: key informant interviews with parents

BackgroundThe updated World Health Organization (WHO) guideline recommends immediate kangaroo mother care (KMC) for all infants, particularly those born preterm. However, its uptake and sustainability have been unsatisfactory. Therefore, we aimed to gain deeper insight into factors influencing the uptake of KMC practice in our setting, and thereby identify possible solutions for the development of relevant interventions to improve its adoption and make KMC a routine practice for all infants.MethodsUsing the Triandis model of social behaviour as our framework, we conducted key informant interviews with parents and healthcare providers. Trained interviewers conducted interviews with nine parents, recruited via purposive sampling. These parents were parents of preterm infants who had been introduced to KMC. Data was transcribed and analysed based on Triandis’ Theory of Interpersonal Behaviour. This paper only reports the results of the parent interviews.ResultsMajor findings were how positive feelings like warmth and contentment, the sense of parenthood with KMC, the benefits of KMC for their infant and parents being enablers for KMC uptake. Conversely, the lack of KMC awareness, the initial negative feelings such as fear, uncertainty and embarrassment, the prioritization of time for milk expression, overcrowding in the ward, lack of space and privacy, limited visiting hours, lack of support and poor communication resulting in misapprehension about KMC were major barriers.ConclusionA deeper understanding of the factors influencing the uptake of KMC using the Triandis behavioural model provided a way forward to help improve its uptake and sustainability in our settings.Trial registrationThis study was registered with the National Medical Research Registry (NMRR-17-2984-39191).

Determinants of Kangaroo Mother Care among low-birth-weight infants in low resource settings.

Kangaroo Mother Care involves direct contact between a baby's bare skin and a caregiver, typically the mother. It has many benefits for both baby and caregiver and is often used to regulate body temperature, promote breastfeeding, enhance growth, and bonding. This study aims to explore factors associated with Kangaroo Mother Care uptake in low-resource countries for babies born with low-birth-weight. Demographic and Health Survey data from 34 low- and middle- income countries were analyzed. Cross-sectional data of 57,223 children were pooled and analyzed. Hierarchical multivariable analysis was performed to determine the factors associated with skin-to-skin contact. Statistical significance was set to 5%. The prevalence of Kangaroo Mother Care ranged from 11.04% to 84.36%; highest in Benin (84.36%), Tajikistan (80.88%), and Uganda (80.86%) and lowest in Burundi (11.04%), Bangladesh (16.58%), and Pakistan (19.24%). Higher odds of Kangaroo Mother Care were estimated among low-birth-weight infants who were put to breast immediately, had low-birth-weight (≥1.5kg), born through normal delivery, born at health facility, those whose mothers were exposed to media, had high antenatal care visits, had formal education, and in the younger age bracket. Also, women living in communities with high illiteracy, countries in the lower-middle income region had higher odds of Kangaroo Mother Care. Women domiciled in Europe and Central Asia, Sub-Saharan Africa, Latin America and The Caribbeans, and East Asia and Pacific had lower odds of Kangaroo Mother Care. This study found a low uptake of Kangaroo Mother Care in countries with limited resources, which is a concerning issue that requires urgent attention. Increasing awareness, education, and support for mothers and families to practice Kangaroo Mother Care, as well as training healthcare practitioners, can lead to better outcomes for newborns and reduce neonatal death.

Postnatal Kangaroo mother care practice at home and comparison of improvement in vital parameters in low-birth-weight babies in-home setup and non-teaching hospital setup in rural coal mines area Jharkhand, India: A community-based observational study.

Low birth weight (LBW) newborns especially those <2000 g are more prone to hypothermia due to which other physiological parameters gets deteriorated in the 1st week of life. The objective of this observational study was to continue Kangaroo mother care practice at home and to ascertain whether KMC was effective in improving the vital parameters of LBW babies when it is given at home in a rural coal mines area, in Jharkhand, India. This study was a community-based prospective observational study, done over three years from November 2019 to November 2022. In this study, we included 156 pairs of both mothers and LBW babies (weight 1500 g to <2000 g). After discharge from the hospital on day 3, KMC was continued at home on day 4, day 5, and day 6. Data of four physiological parameters, namely, temperature, oxygen saturation, respiratory rate, and heart rate were collected before and after KMC and analyzed. Among 400 newborns, 156 LBW babies (39.0%) who were given KMC at home showed similar but statistically significant improvement of vital parameters, especially in temperature and oxygen saturation (P < 0.0001) compared to the same babies 156 (39.0%) given KMC in the hospital (P < 0.001). Kangaroo mother care, which was continued at home, has a significant role in the Improvement of vital parameters, especially concerning temperature and oxygen saturation. If the babies in the weight range of 1500 g to <2000 g are healthy, well-breast-fed, and have no other risk factors, they can be discharged early and managed at home by delivering supportive care and nursing care along with Kangaroo mother care with continuous follow-up.

Experiences of Xhosa women providing Kangaroo mother care in a tertiary hospital in the Western Cape, South Africa

Kangaroo mother care has been recognized as a credible intervention to improve preterm birth outcomes by the World Health Organization. Kangaroo mother care requires high user engagement and consists of continuous skin-to-skin contact between the mother and infant and exclusive breastfeeding. We conducted a qualitative study of Xhosa women ( n = 10) practicing Kangaroo mother care in a tertiary hospital in the Western Cape, South Africa. All interviews were conducted in isiXhosa, audio-recorded, and transcribed. The transcribed data were analysed using thematic analysis. Four themes emerged: (1) Kangaroo mother care, a beneficial but foreign concept; (2) distress in the Kangaroo mother care ward; (3) the missing umbilical cord: cultural experiences of mothers in the Kangaroo mother care ward; and (4) the Kangaroo mother care village: interpersonal relations in the ward. Our study showed that cultural practices still pose a challenge to fully accepting Kangaroo mother care. We suggest more studies on cultural sensitivity to encourage acceptance of interventions that affect culturally diverse groups.

Health professional assisted Kangaroo mother care practice in Ethiopian health care facilities: evidence from the 2016 Ethiopian demographic and health survey

BackgroundWorldwide, 15 million children born prematurely every year and over one million of them died because of prematurity caused complications. However, three-fourths of deaths from preterm related complications are preventable by using Kangaroo Mother Care (KMC). The Ethiopian government has been implementing a guideline that declares putting all low birth weight neonates at KMC. The aim of this study was to assess health professionals’ assisted KMC practice and its associated factors among Ethiopian mothers who gave birth at health facilities.MethodologyThis study used the 2016Ethiopian Demographic and Health Survey data (EDHS). The 2016EDHS used a stratified two stage sampling method to select a representative sample using validated questioner. The sample we used in this study after cleaning the children’s data set from the 2016EDHS was 2,960. Logistic regression model was used to assess the association of health professional assisted KMC practice and predictor variables.ResultsMothers who gave birth in health facilities and practiced kangaroo mother care were 1808(62.1%). In the multivariable logistic regression analysis, women from poorest (AOR, (95%CI)), (0.60, (0.43, 0.81)) and poorer (0.62, (0.46, 0.86)) socio-economic status were not practicing KMC.ConclusionsThe coverage of health professional assisted KMC practice was far lower than the expectation for mothers who gave birth in health facilities (100%). Low socio-economic status was associated with not practicing KMC. A further study on why mothers from low wealth index did not practicing KMC while they were in health facilities may be needed.

A pilot study of Kangaroo mother care in early essential newborn care in resource-limited areas of China: the facilitators and barriers to implementation

BackgroundImplementation of Kangaroo Mother Care (KMC) in resource-limited areas of China may face unique barriers, such as a lack of resources, geographic location and more traditional culture among others. This qualitative study analyses the facilitators and barriers to implementing KMC in county-level health facilities in resource-limited areas of China for the promotion of KMC on a larger scale.MethodsParticipants from 4 of the 18 pilot counties where early essential newborn care was implemented through the Safe Neonatal Project and 4 control counties not enrolled in Safe Neonatal Project were selected using purposive sampling. A total of 155 participants were interviewed, including stakeholders of the Safe Neonatal Project such as national maternal health experts, relevant government officials and medical staff. Thematic analysis was used to process and analyse the interview content in order to summarise the facilitators and barriers to implementing KMC.ResultsKMC was accepted in the pilot areas but still faced certain challenges due to institutional regulation, resource provision and the perceptions of health staff, postpartum mothers and their families as well as COVID-19 prevention and control regulations. The facilitators identified were government officials and medical staff acceptance and the incorporation of KMC into routine clinical care. The barriers identified were a lack of dedicated funding and other resources, the present scope of health insurance and KMC cost-sharing mechanism, providers’ knowledge and practical abilities, parental awareness, postpartum discomfort, fathers’ inadequate involvement, and the impact from COVID-19.ConclusionThe Safe Neonatal Project pilot experience indicated the feasibility of implementing KMC in more areas of China. Optimising institutional regulations, providing necessary supporting resources and enhancing education and training may help to refine the implementation and scale-up of KMC practice in China.

Knowledge, attitude and issues in practicing kangaroo mother care at home after discharge from a kangaroo mother care ward of a tertiary care hospital in central Lahore

Background: Kangaroo mother care (KMC) is well known intervention in preterm care. Mothers gets training from health professionals to provide skin to skin care, breast feeding which helps in infections prevention and early discharge. It is required to inquire the mothers experience during home environment to promote this technique after discharge from hospital. This study focused on gaining the knowledge, attitude of unsupervised mothers practicing KMC at home and determine the most common issues faced by them.&#x0D; Subjects and methods: A cross-sectional study, conducted at KMC ward of the Department of Pediatrics, Unit-1, Services Institute of Medical Sciences (SIMS), Lahore from March 2020 to April 2021 after ethical approval. Hospitalized mothers of stable pre-term infants were taught KMC techniques by trained health care provider. After two weeks of discharge from hospital mothers were interviewed according to structured questionnaire by the trained doctor about their KMC knowledge, experience and issues faced at home. &#x0D; Results: Out of 156 mothers 145 (92.9%) conducted KMC at home and 93 (59.6%) performed intermittent KMC during nighttime (p=0.001). 152 (97.4%) mothers felt good, 133 ( 85.3%) were comfortable and 122 (78.2% ) deemed the process safe for their babies. 82 (52.6%) felt ease with provided kits (p=0.032).120 (76,9%) husbands felt KMC was essential for their babies. 102 (65.4%) mothers were allowed by their husbands to do KMC at home (p=0.000) and 68 (43.6%) fathers sometimes performed KMC themselves (p=0.001). Family and friends were also supportive in 104 (66.7%) cases while 112 (71.8%) mothers felt the positive attitude of the community (p=0.000). 127 (81.4%) mothers were willing to recommend KMC to new mothers (p=0.019).&#x0D; Conclusion: Most of the mothers performed intermittent KMC at home. They were assisted by husband and family members to carryout KMC at home. They are willing to recommend it to new mothers.

Knowledge, attitude, practice and determinants of exclusive breastfeeding among women in Ethiopia: Systematic review and meta-analysis

ObjectiveThis study aimed to assess the pooled estimate of Ethiopia's women's knowledge, attitudes, practices, and determinants of exclusive breastfeeding. MethodsPubMed, Google Scholar, Scopus, Science Direct, and Addis Ababa University online library were searched. Data were extracted using Microsoft Excel and analyzed using STATA statistical software (v. 14). Publication bias was checked by forest plot, Begg's rank test, and Egger's regression test. To look for heterogeneity, I2 was computed, and an overall estimated analysis was carried out. Subgroup analysis was done by region, study setting, and publication. The pooled odds ratio for associated factors was also computed. ResultsOut of 995 studies reviewed, 33 met our eligibility criteria and were included in this meta-analysis study. The total number of study participants was 13,397. The pooled prevalence of good knowledge, positive attitude, and poor practice of kangaroo mother care were 74.2% (95% CI: 62.9–85.4), 77.2% (95%CI: 68.3–86.0), and 58.3% (95% CI: 49.9–66.6), respectively. In sub-group analysis, the highest prevalence of knowledge was in institution-based studies (78.3%) and unpublished studies (76.3%). A positive attitude was also highest in institutional-based studies (81%). The highest practice prevalence was in Afar (68%) and the lowest was in Addis Ababa (34.6%).Women who had a secondary level of education (AOR = 3.3; 95%CI: 1.8–6.0) were a housewife (AOR = 3.1; 95%CI: 2.1–4.7), delivered vaginally (AOR = 2.0; 95%CI: 1.4–2.9), health facility delivery (AOR = 3.3; 95%CI: 2.1–5.1) and attending antenatal care were predictors of exclusive breastfeeding. ConclusionAlthough women have good knowledge and positive attitude toward exclusive breastfeeding, there is a significant gap in exclusive breastfeeding practice. Maternal education, occupation, vaginal delivery, institutional delivery, and antenatal care visit were predictors of exclusive breastfeeding. It is recommended to strengthen maternal and child health services.

Maternal Knowledge of Kangaroo Care: Level of Practice in Health Facilities in Calabar Metropolis

This study is aimed to assess the level of knowledge and identify the level of practice of Kangaroo care among mothers with preterm and low birth weight babies in health facilities in Calabar Metropolis. The study was a quantitative, descriptive survey design study. Four objectives and four corresponding research questions guided the study. A self-designed questionnaire was used to collect information and the Statistical Package for Social Sciences (SPSS), Version 24 was used to analyse the data. Demographic data revealed that 30.5% of the respondents were between 18 – 30years, 53.4% were between 31 – 40years, 10% were between 41 – 50years while 6.1% were between 50years and above, 16% had no formal education, 10.8% had primary school education, 20% had secondary school education while 53.2% had tertiary education. The result of the study revealed that 72.5% of the total respondents has little or no knowledge of Kangaroo mother care while only 27.5% has knowledge of Kangaroo mother care, 24.1% of the respondents had practiced Kangaroo mother care while 75.9% had not practiced Kangaroo mother care, lack of assistance from husband and family member in home chores is part of the factors that limited the practice of Kangaroo mother care among women with preterm and low birth weight babies in health facilities and that assistance from husband and family member in home chores enabled them practice Kangaroo mother care. The study concludes that the level of knowledge and practice is low among women in health facilities in Calabar Metropolis. Therefore, the study recommends that training and education about Kangaroo Mother Care practice should be carried out on a regular basis.

Practice of Kangaroo Mother Care Among Low-Birth-Weight Infants Discharged from Health Facilities and Its Outcome in Mekelle City, Tigray, Northern Ethiopia.

Kangaroo mother care is a proven intervention shown to be effective in reducing neonatal mortality among low-birth-weight infants. The paucity of evidence regarding the practice at home can be highlighted. This study aimed to assess the practice and outcome of kangaroo mother care at home among mothers having low-birth-weight infants discharged from two hospitals in Mekelle, Tigray, Ethiopia. A prospective cohort study was conducted among 101 paired mothers and low-birth-weight neonates discharged from Ayder and Mekelle Hospitals. Non-probability purposive sampling was used to select 101 infants. Data were collected from patient charts in both hospitals using interviewer-administered structured questionnaire, anthropometric measurements and were then analyzed using SPSS version 20. Characteristics were analyzed using descriptive statistics. Bivariate analysis was used, and variables with p-value <0.25 were exported to multivariable logistic regression and statistical significance was set at a p-value <0.05. Kangaroo mother care was continued at home in 99% of the infants. Three of the 101 infants died before the age of 4 months with a possible cause of death from respiratory failure. Exclusive breastfeeding was provided for 67% of the infants, and it was higher in infants who started kangaroo mother care within 24 hours of life (AOR 3.8, 95% CI 1.07-13.25). Malnutrition was higher in those with birth weight <1500 grams (AOR 7.3,95% CI 1.63-32.59); small for gestational age (AOR 4.8,95% CI 1.41-16.31) and those provided kangaroo mother care for <8 hours per day (AOR 4.5,95% CI 1.40-16.31). Early initiation and prolonged duration of kangaroo mother care were associated with increased exclusive breastfeeding practice and decreased incidence of malnutrition. Kangaroo mother care should be promoted at the community level.

Magnitude of kangaroo mother care practice and its associated factors in Tigray region, Northern Ethiopia, 2019: cross-sectional study design

kangaroo mother care is an evidence based approach care of preterm and low birth weight infants carried skin-to-skin with the parents that can decrease morbidity and mortality of infant. Country level adoption and practice of kangaroo mother care has been limited and global coverage remains low and few studies have examined the reasons for low practice. The aim of this study was to assess the magnitude of kangaroo mother care practice and its associated factors in Tigray, northern Ethiopia, 2019. an institutional-based cross-sectional study design was conducted in public general hospitals of Tigray, northern Ethiopia, 2019. Two-stage sampling technique was used and an interviewer-administered questionnaire were used to collect the necessary information. The data were cleaned and entered using epi-Data version 3.1 then exported to stoical package for social science (SPSS) version 22.0 for analysis. Bivariate logistic regression and multivariable analysis were carried out at adjusted odds ratios (AOR) with 95% CI and significance level p-value (<0.05). out of the total 844 selected mothers with their infants, 840 were participated in the study yielding to a response rate of 99.5%, of these respondent's kangaroo mother care practice was found to be 70.2%. Being mothers housewife [(AOR=4.12, 95% CI: (1.5, 0.11)], maternal age [(AOR=9.3, 95% CI: (2.5, 33.9 )], currently mode delivery [(AOR=5.39, 95% CI: (2.3, 12.25)], number of children [(AOR=8.38, 95%: (4.6, 15.3)], mother having ≥5 children [(AOR=18.2, 95%CI: (9.4, 35.4)], antenatal care [(AOR=3.299 95%CI: (1.54, 7.07)] were factors at p-value (<0.05) significantly associated with kangaroo mother care practice. in this study, maternal age, parity, antenatal care, occupation and mode of delivery were factors that influence kangaroo mother care practice in the study area, so healthcare providers, policymakers and other stakeholders should give special focuses on those influencing factors.

Compliance toward key elements of kangaroo mother care and its predictors among postnatal mothers with preterm and/or low birth weight newborns in southern Ethiopia, 2021: A count analysis.

Although kangaroo mother care has been field-tested and found to be effective for preterm and/or low birth weight newborns at the health facility and community level, implementation and scale-up in low-income countries, including Ethiopia, have proven difficult. There was a dearth of evidence that shows compliance of mothers toward components of the kangaroo mother care. Hence, this study aimed at assessing the compliance of postnatal mothers toward World Health Organization-recommended elements of kangaroo mother care and the factors that influence it in southern Ethiopia, 2021. A hospital-based cross-sectional study was conducted among 257 mothers with preterm and low birth weight newborns from 1 July to 30 August 2021. A pretested, structured, interviewer-administered questionnaire and document review were used to collect data. The practice of kangaroo mother care was considered as a count variable. Analysis of variance and independent t-tests were used to examine the variation in the mean score of kangaroo mother care across covariates, and variables with a p value of 0.05 were eligible for the multivariable generalized linear regression model. The effect of each independent variable on the dependent variable was examined using multivariable generalized linear regression with a negative binomial log link. The mean (±standard deviation) practice score of kangaroo mother care items was 5.12 (±2.39), with 2 and 10 as the minimum and maximum item scores, respectively. Place of residence (adjusted odds ratio = 1.55; 95% confidence interval:1.33-2.29), mode of delivery (adjusted odds ratio = 1.37; 95% confidence interval: 1.11-2.21), birth preparedness and complication readiness plan (adjusted odds ratio = 1.63; 95% confidence interval:1.32-2.26), maternal knowledge of kangaroo mother care (adjusted odds ratio = 1.40; 95% confidence interval: 1.05-1.87), and place of delivery (adjusted odds ratio = 0.67; 95% confidence interval: 0.48-0.94) were identified as significant predictors of compliance toward key elements of kangaroo mother care. The overall practice of mothers toward key elements of kangaroo mother care was low in the study area. Health care providers who work in the maternal and child health service delivery points should pay special attention to women who come from rural areas and have had cesarean sections, by encouraging and guiding them to practice kangaroo mother care. Women should be counseled during antenatal care and after delivery to improve their knowledge of kangaroo mother care. Health workers in antenatal care clinics should place a strong emphasis on enhancing birth preparedness and complication readiness plans.

Challenges Faced by Mothers Practicing Kangaroo Mother Care (KMC) in a Resource-Limited Setting During the COVID-19 Pandemic: Insights From the Voices of Mothers of Preterm Babies and Their Suggested Solutions.

Objectives. We investigated the challenges faced by mothers of preterm babies practicing KMC during the Covid-19 pandemic and documented the mothers' suggestions of possible interventions. Methods. We conducted a cross-sectional descriptive study at the New Born Unit of Kenyatta National Hospital(KNH). We interviewed 82 mothers using a researcher-administered questionnaire with closed and open-ended questions. Qualitative data generated from open-ended questions was analyzed thematically. Results. KMC was majorly affected by; fear of the baby contracting COVID-19, fear of contracting COVID-19, and social distancing restrictions. The mothers' suggested interventions included; the need for more KMC rooms, provision of appropriate clothing, strict application of COVID-19 prevention guidelines, and greater support of practice by the family. Conclusion. The mother's pivotal role as a partner in decision-making in regard to the improvement of KMC during pandemics and possibly other settings was demonstrated. Targeted interventions should incorporate the suggestions from the mothers.

A Quality Improvement Initiative on sustaining Kangaroo Mother Care Practices During COVID Pandemic

Introduction: COVID-19 pandemic has affected the KMC practices in our SNCU. Lack of clear guidelines and training about continuing KMC in COVID-19 times has further lead to decrease in KMC practices. Methods: This is a prospective single centered quality improvement study conducted in the inborn unit of a tertiary care hospital. Intervention phase was done in August and September 2020. Preterm mother-infant dyads who were admitted in the inborn Level 2 Neonatal care unit with birth weight less than 2000 grams were enrolled in this study. A QI team comprising of resident doctors, nurses and supporting staff and a lactation counsellor was formed. The potential barriers for prolonged KMC were evaluated using fish bone analysis. A variety of measures were introduced and subsequently tested by seven plan-dostudy-act (PDSA) cycles. Data on percentage of initiation of KMC was measured by bedside nurses on daily basis and the data was plotted on run chart every week during implementation phase. Results: 116 eligible mother-infant dyads were studied during implementation period (50 days). We achieved our goal by step-wise implementation of changes through multiple PDSA cycles. The percentage of initiation of KMC among eligible preterm infants has increased from baseline of 43.4% to 83.3% and duration of KMC from 1.5 mean hours to 4.5 hours over a period of eight weeks. Conclusions: Ongoing quality improvement measures increased the percentage of initiation and duration of KMC among eligible preterm infants without addition of extra man power.
