Prospective Outcomes of Microlaryngoscopy Versus Office Laser Photoangiolysis for Vocal Fold Polyps.

The objective of this study is to compare treatment outcomes for vocal fold polyps (VFPs) between operating room microlaryngoscopy and office-based photoangiolysis with the potassium titanyl phosphate (KTP) laser. Prospective nonrandomized cohort study of patients with VFPs undergoing microlaryngoscopy ("OR group") or KTP laser photoangiolysis ("KTP group"). Voice outcomes (patient-reported outcome measures [Voice Handicap Index-10 (VHI-10) and Singing VHI-10 (SVHI-10)], auditory-perceptual measures [Consensus Auditory-Perceptual Evaluation of Voice (CAPE-V)], videostroboscopic characteristics [Voice-Vibratory Assessment of Laryngeal Imaging (VALI)], and acoustic and aerodynamic measures) were performed at baseline and regular intervals after intervention. Forty-four subjects (17 OR group, 27 KTP group) with VFPs were enrolled. Mean VHI-10 significantly improved from baseline to each follow-up interval in both groups, except for the 1-2-week interval in the OR group. Mean SVHI-10 improved for both groups at some intervals. Growth curve models and time-to-event analyses for patient-reported outcomes did not differ between groups. There were significant improvements in all categories of auditory-perceptual voice quality and some categories of videostroboscopic characteristics in both groups. No significant trends were identified in acoustic and aerodynamic measures. Improvements in most outcomes did not significantly differ between groups or based on polyp size. There were no major complications. Significant improvements in patient-reported voice outcomes measures, auditory-perceptual voice evaluation, and videostroboscopic characteristics occur following surgical treatment of vocal fold polyps with either microlaryngoscopy or office-based KTP laser. High fluence in laser stapedotomy aggravates transient subjective dizziness

Background Laser fenestration in stapedotomy has thermal effect to the vestibule. Aims/Objectives To evaluate the role of energy density (fluence) in the severity of postoperative vestibular symptoms. Materials and methods The retrospective chart-review study included 84 patients with otosclerosis that underwent primary laser stapedotomy. Surgical outcomes, including nystagmus, and subjective vestibular symptoms during one-month follow-up, were compared between potassium titanyl phosphate (KTP) and CO2 laser. According to this study and literature, we assessed the relationship between laser parameters and the incidence of persistent vestibular symptoms lasting more than one week after surgery. Results The KTP and CO2 laser group included 48 and 36 patients, respectively. Fluence was different between the KTP (637 J/cm2) and CO2 (141 J/cm2) laser (p < .001). The KTP group showed gradual decrease in dizziness during one-month observation period, while the CO2 group exhibited a steep recovery curve in the first postoperative week (9 and 4 d of duration, respectively). The incidence of persistent vestibular symptoms was correlated with both fluence (r = 0.80, p = .01) and spot size (r = −0.74, p = .01). Conclusions and significance Appropriate setting of parameters with lower fluence is desirable for the efficiency and safety of laser stapedotomy. To Assess the Efficacy of Various Disinfection and Hemostasis Procedures in Providing Postoperative Pain Relief Following Pulpotomy in Cases of Symptomatic Irreversible Pulpitis.

The purpose of this study is to assess the efficacy of various disinfection and hemostasis procedures in providing postoperative pain relief following pulpotomy in cases of symptomatic irreversible pulpitis. The data from a cohort of 50 patients who received treatment with sodium hypochlorite (NaOCl) and another cohort of 50 patients who received treatment with potassium titanyl phosphate (KTP) laser were subjected to analysis. The patients were provided a questionnaire to evaluate pain levels before and after surgery. The patients documented their levels of postoperative pain at specific time intervals, including the sixth hour, first day, second day, third day, and eighth day. This was done using a 100 mm visual analog pain scale, where a marking of 0 mm indicated no pain and a marking of 100 mm indicated the highest level of pain, reflecting the severity of the pain experienced. The pain score of group B was significantly lower than that of group A on the first day. While no notable disparity was detected among the groups during the remaining postoperative periods, it is worth noting that the KTP laser exhibited comparatively lower pain scores. In both groups, the initial pain score before surgery was found to be significantly higher than the pain scores recorded at all subsequent time intervals after surgery. Within group A, it was observed that the pain score during the sixth hour after the surgical procedure was notably greater compared to the pain scores recorded during all other time intervals following the operation. In group B, the pain score at the sixth hour exhibited a statistically significant increase compared to the pain scores observed on the third day and eighth day. The KTP laser group exhibited a lower postoperative pain score in comparison to the NaOCl group. The utilization of KTP laser-assisted pulpotomy demonstrated enhanced efficacy in alleviating pain among individuals diagnosed with symptomatic irreversible pulpitis.

Effect of Virtual Reality Distraction on Satisfaction and Tolerability in Patients Undergoing In-Office KTP Laser Procedure

To study the effect of virtual reality (VR) on satisfaction, discomfort, stress, and cooperation in patients undergoing in-office potassium titanyl phosphate (KTP) laser procedure. Prospective study. Thirty-seven patients were enrolled in this prospective study. The State Anxiety Scale of Spielberg's State-Trait Anxiety Inventory was used to measure the level of state anxiety. Satisfaction, discomfort, pain, stress, acceptance of VR, relaxation with VR, and willingness to wear VR were evaluated using a 100-mm visual analog scale (VAS). A 5-point Likert-like scale was used to rate the patient cooperation. All procedures were completed successfully with cooperation of patients. Satisfaction score in VR group was 88.3±9.0, and in control group was 81.6±9.7 (P=0.040). There were significant differences in both nasal cavity and laryngopharynx discomfort between two groups (P=0.030 and P=0.016, respectively). The pain score of control group was higher than that of VR group but it was not statistically significant (P=0.140). The stress of control group during procedure was more obvious than that of VR group (30.5±24.0 versus 17.0±9.2, P=0.021). The mean VAS scores of acceptance of VR were all more than 75. The results of regression analysis showed that VR had significant effects on satisfaction with the procedure (P=0.004), discomfort of nasal cavity (P=0.030) and laryngopharynx (P=0.016), and feeling of stress (P=0.021) during the procedure. Distraction of VR can enhance satisfaction in both procedure and stress management for patients undergoing in-office KTP laser procedure. Acceptance of VR in VR group was relatively good.

KTP laser alone or combined with bleomycin for pharyngolaryngeal hemangioma: a lesion grade-based determination of prognosis

Background No standard potassium titanyl phosphate (KTP) laser treatment strategy is available yet for pharyngolaryngeal hemangioma. Objectives To explore the therapeutic effect of KTP laser, alone or combined with bleomycin injection, for pharyngolaryngeal hemangioma. Material and methods This observational study included patients with pharyngolaryngeal hemangioma treated between May 2016 and November 2021: KTP laser under local anesthesia, KTP laser under general anesthesia, or KTP laser combined with bleomycin injection under general anesthesia. Results A total of 60 patients were recruited, including 17, 19, and 24 patients with grade 1, 2, and 3 hemangiomas, respectively. A total of 21 patients underwent KTP laser under local anesthesia, 31 underwent KTP laser under general anesthesia, and 8 underwent KTP laser under general anesthesia combined with bleomycin. The cure rates were 100%, 89.5%, and 20.8% for grade 1, 2, and 3 lesions, respectively. The prognosis was significantly different among the grades of hemangioma (p < .001). Conclusion KTP laser treatment might be an effective treatment for adult patients with pharyngolaryngeal hemangioma. The size of the hemangioma might be the major factor influencing the prognosis. The method of anesthesia and whether it was combined with bleomycin injection might not affect the prognosis.

Use of final irrigant potassium titanyl phosphate laser; Sapindus Mukorossi and Fotoenticine on the bond values of zirconia post-to-canal dentin

AimsAssessment of the efficacy of final root canal irrigants Sapindus mukorossi (SM), Potassium titanyl phosphate laser (KTPL), and Fotoenticine (FTC) on the push-out bond strength (PBS) of zirconia post. Materials and methodsThe root canal procedure was initiated using the 10 K file and the working length was determined on single-rooted human premolar teeth after decoration. The canals were then enlarged using ProTaper universal system and filled using single cone gutta-percha (GP) and AH Plus resin sealer. Post space was prepared by removing 10 mm of GP from the canal. All the teeth were then allocated into four groups based on the final irrigating regime used (n = 10) Group 1: 5.25% NaOCl+17% EDTA, Group 2: 5.25% NaOCl + KTPL, Group 3: 5.25% NaOCl + FTC and Group 4: 5.25% NaOCl+ SM. Zirconia posts were cemented in the canal space. The specimens were sectioned and implanted in auto-polymerizing acrylic resin. A universal testing machine and stereomicroscope at 40x magnification were used for PBS and failure mode analysis. ANOVA and Tukey post hoc test were used to make group comparisons(p = 0.05). ResultsGroup 4 (5.25% NaOCl+ SM) coronal section displayed the highest PBS (9.29±0.24 MPa). However, group 3 (5.25% NaOCl+ FTC) apical third (4.08±0.14 MPa) showed the lowest bond values. Group 2 (5.25% NaOCl+ KTP laser) and Group 3 at all three-thirds unveiled no significant difference in PBS(p>0.05). However, Group 1 (5.25% NaOCl+17%EDTA) and Group 4 displayed comparable outcomes of bond strength(p>0.05) ConclusionSapindus mukorossi has the ability to be used as a final root canal irrigant alternative to EDTA. However, future studies are still required to conclude the outcomes of existing research.

Efficacy of Potassium Titanyl Phosphate Laser and Sodium Hypochlorite on Postoperative Pain Intensity Following Pulpotomy in Teeth with Symptomatic Irreversible Pulpitis: A Randomized Clinical Trial.

Introduction: This study evaluated the postoperative pain intensity in permanent teeth with symptomatic irreversible pulpitis treated by pulpotomy using physiological saline, sodium hypochlorite (NaOCl), or the potassium titanyl phosphate (KTP) laser. Materials and methods: Ninety patients diagnosed with symptomatic irreversible pulpitis in permanent molars were randomly divided into three groups according to the pulpotomy procedures. Preoperative pain levels were recorded. Complete hemostasis was achieved with physiological saline (control group), NaOCl, or the KTP laser after initial bleeding control. Patients were asked to mark their pain level on the visual pain scale at 6, 24, 48, and 72 h and 7 and 30 days depending on the severity of pain. The permanent restoration was completed after 7 days. Data were statistically analyzed using Kruskal-Wallis, Mann-Whitney U, Friedman, and Bonferroni tests at a significance level of p < 0.05. Results: According to the present study, the statistical difference between the groups in terms of preoperative pain and percussion pain levels was insignificant (p > 0.05). There was no significant difference between groups in terms of demographic data (p > 0.05). The postoperative pain level of the KTP laser group was significantly lower at 6 h compared with the saline group (p < 0.05). There was no significant difference between groups in terms of the postoperative pain level at other time intervals (p > 0.05). The highest pain scores were observed in all groups at 6 h (p < 0.05). Conclusions: The KTP laser can be preferred primarily for reducing postoperative pain in pulpotomy treatments. The KTP laser or NaOCl-assisted pulpotomy can be an effective treatment for pain reduction in permanent teeth with symptomatic irreversible pulpitis. Surgiflo® hemostatic matrix versus NasoPore® nasal packing following postassium titanyl phosphate laser surgery for hereditary hemorrhagic telangiectasia: A randomized controlled trial.

To help ensure adequate hemostasis immediately following potassium titanyl phosphate (KTP) laser treatment, many centres treating hereditary hemorrhagic telangiectasia (HHT) routinely use nasal packing post-operatively. The purpose of this study was to compare hemostatic thrombin matrix with standard packing for postoperative bleeding, patient pain, and comfort. A prospective, randomized, double-blinded, non-inferiority study was conducted with participants at an HHT centre of excellence (COE) and randomized to the treatment group with reconstituted thrombin gelatin matrix (Surgiflo®) or control group with a biodegradable synthetic polyurethane foam (NasoPore®). Adult subjects with confirmed HHT and moderate to severe epistaxis (a minimum calculated epistaxis severity score [ESS] of 4.0) warranting KTP laser treatment were recruited. Data was collected 2 weeks post operatively by a blinded reviewer completing a visual outcomes evaluation and each patient completing a subjective symptoms questionnaire. Non-parametric statistical analysis was employed. Twenty-eight adult patients were randomized to the treatment and control arms with comparable preoperative epistaxis severity scores. Postoperative nasal bleeding was equivalent. Significantly less pain was found in the treatment arm (p=.005). While there were trends towards less obstruction and increased satisfaction in the treatment group as well as less crusting in the control group, these findings were not statistically significant. Allocation to the treatment group was associated with an approximately $75 higher cost. When compared to NasoPore® for hemostasis, Surgiflo® hemostatic matrix performed equivalently while causing less discomfort in HHT patients following nasal KTP treatment. 1b.

Adjunctive use of Er, Cr: YSGG laser, Potassium titanyl Phosphate laser, and photosensitizer on extrusion bond strength of radicular dentin to bioceramic sealers.

To assess the impact of adjunctive final root canal disinfection regime curcumin photosensitizer (CP) activated by Photodynamic therapy (PDT), Er, Cr: YSGG laser (ECL), and Potassium titanyl Phosphate (KTP) laser on the bond integrity of two endodontic Bioceramic (BC) sealers. Decoration of eighty human single-rooted teeth was performed up to a cementoenamel junction. After instrumentation, all samples were irrigated with 2.25% NaOCl solution followed by 17% EDTA. The specimens were divided arbitrarily into 8 groups (n=10) based on the adjunctive final irrigation method used. Group 1 and 5 (Control): No adjunctive regime, cleaned using 2.25% NaOCl+17% EDTA, group 2 and group 6: CP activated by PDT, Group 3 and Group 7 using ECL, Group 4 and Group 8: KTP laser. Following disinfection samples in groups 1, 2, 3, and 4 were obturated using Sankin apatite root canal sealer (SARS). Specimens in groups 5, 6, 7, and 8 were obturated using Pro-root endo sealer (PRES). Sectioning of samples was performed. The sectioned specimens were subjected to a compressive loading exerted via a steel plunger moving at a 0.5mm/min crosshead speed till debonding. Means and standard deviations (SD) of EBS of BC sealer with the root canal dentin were assessed using one-way analysis of variance (ANOVA) and Post Hoc Tukey multiple comparison tests. The level of significance was measured at p=0.05. The coronal section of samples in group 5 (Na-E-PRES) exhibited the highest bond value. The lowest PBS was displayed by the middle third of specimens in group 6. Group 1 (Na-E-SARS), group 3 (Na-E-ECL-SARS), group 5 (Na-E-PRES), and group 7 (Na-E-ECL-PRES) exhibited comparable bond values (p>0.05). Whereas, group 2 (Na-E-PDT-SARS) and group 4 (Na-E-PTPL-SARS), group 6 (Na-E-PDT-PRES), and group 8 (Na-E-PTPL-PRES) were found to be comparable (p>0.05). Adjunctive use of Er, Cr: YSGG laser displayed better extrusion bond strength with BC root canal sealer compared to CP activated by PDT and KTP laser. Bond strength is influenced by the type of irrigation.

The aim of this study was to evaluate and compare the effect of different laser prototypes [Er, Cr: YSGG laser (ECYL), potassium titanyl phosphate laser (KTP), and Femtosecond laser (FSL)]and curcumin photosensitizer (CP) activated by Photodynamic therapy (PDT) on the bond strength of Pre-fabricated fiber reinforced composite (PFRC) post-bonded to radicular dentin. A total of fifty mandibular single-rooted closed apex teeth were extracted carefully, assembled, and decoronated up to the cementoenamel junction. The working length of all specimens was determined by using a 10 K patency file and later, were cleaned and shaped with Protaper NiTi system using the crown down approach, dried, and obturated with gutta-percha using an AH Plus sealer. Post space was prepared by guiding peeso-reamer. Based on the method of disinfection, the samples were allocated to five groups (n=10) at random: samples in group 1: curcumin photosensitizer (CP) activated by PDT, samples in group 2 disinfected using 5.25% NaOCl+17% EDTA, samples in group 3 disinfected using 5.25% NaOCl+17% EDTA+FSL, specimens in group 4 sterilized using 5.25% NaOCl+17% EDTA+KTP and samples in group 5 cleaned with 5.25% NaOCl+17% EDTA+ECYL. The fiber post was cemented via self-etch resin cement into the post space. All specimens with posts were dissected perpendicularly into apical, middle, and coronal dentin and subjected to the universal testing machine for push-out bond strength (PBS) testing. Statistical analysis was performed using a One-Way analysis of variance and Post Hoc Tukey multiple comparison tests. The highest PBS was corroborated when the radicular canal was disinfected with 5.25% NaOCl +17% EDTA+ ECYL at all three root levels (coronal, middle, and apical) and the lowest was adjudicated by decontamination with CP activated by PDT at all inspected root levels. Intergroup comparison presented that specimens in group 2: 5.25% NaOCl+17% EDTA (control) and group 4: 5.25% NaOCl+17% EDTA+KTP revealed comparable PBS outcome to group 5 (p>0.05) while samples in group 3 revealed the equivalent PBS values to group 1 (p<0.05) at all three root levels. Er, Cr: YSGG laser and potassium titanyl phosphate laser when used in combination with the conventional canal disinfection 5.25% NaOCl and 17% EDTA demonstrated the highest push-out bond strength values at coronal, middle, and apical levels of the root.

Caries-affected dentin disinfection using Triphala, Indocyanine green, and Potassium Titanyl Phosphate laser and their effect on adhesive bond strength.

To assess the impact of new cavity sterilization types using ozone water (OW), Potassium Titanyl Phosphate laser (KTPL), Triphala, and indocyanine green photosensitizer (ICGP) on adhesive bond strength. Fifty human permanent molars were included based on the criteria of International Caries Detection and Assessment System (ICDAS) scores 4 and 5 visual examinations, surface hardness valuation, staining by a caries detector solution, and radiographs. Caries-affected dentin (CAD) samples were randomly distributed into groups based on methods of disinfection (n=10). Group 1: chlorhexidine, group 2: ow, group 3: KTPL, group 4: Triphala, and group 5: ICGP. Following disinfection, specimens were applied with Adper prime and bond NT and then restored with resin composite. All specimens were positioned in a jig of the universal testing machine for shear bond strength (SBS). The debonded samples were analyzed under a stereomicroscope at 40x magnification to determine the type of failure. Analysis of variance (ANOVA) and Tukey HSD tests were used keeping the level of significance p<0.05. The highest bond of adhesive resin was displayed by group 2 specimens cleaned using OW (16.71±0.19 MPa). Whereas, the lowest bond integrity was observed by group 5 specimens disinfected using ICGP (11.21±0.16 MPa). It was observed samples disinfected with OW (16.71±0.19 MPa) in group 2 and samples in group 4 (16.09±0.26 MPa) treated with Triphala displayed comparable outcomes of bond strength (p>0.05). It was observed that group 2 (70%) and 4 (80%) treated specimens accounted mostly for cohesive failure. However, groups 1 (80%), 3 (60%), and 5 (70%) displayed an adhesive failure pattern. OW and Triphala displayed better bond integrity of adhesive resin to CAD and therefore have the potential to be used as a cavity disinfectant in clinical settings. However, ICGP and KTPL displayed weak bonds of adhesive to CAD. More work is needed to extrapolate the findings of the present study.

The Impact of Laser Thermal Effect on Histological Evaluation of Oral Soft Tissue Biopsy: Systematic Review

The aim of the study is to review the literature to observe studies that evaluate the extent of the thermal effect of different laser wavelengths on the histological evaluation of oral soft tissue biopsies. An electronic search for published studies was performed on the PubMed and Scopus databases between July 2020 and November 2022. After the selection process, all the included studies were subjected to quality assessment and data extraction processes. A total of 28 studies met the eligibility criteria. The most studied laser was the carbon dioxide (CO2) laser, followed by the diode laser 940 nm-980 nm. Six studies were focused on each of the Erbium-doped Yttrium Aluminium Garnet (Er:YAG), Neodymium-doped Yttrium Aluminum Garnet (Nd:YAG) lasers, and diode lasers of 808 nm and 445 nm. Three studies were for the Potassium Titanyl Phosphate (KTP) laser, and four studies were for the Erbium, Chromium-doped Yttrium, Scandium, Gallium, and Garnet (Er,Cr:YSGG) laser. The quality and bias assessment revealed that almost all the animal studies were at a low risk of bias (RoB) in the considered domains of the used assessment tool except the allocation concealment domain in the selection bias and the blinding domain in the performance bias, where these domains were awarded an unclear or high score in almost all the included animal studies. For clinical studies, the range of the total RoB score in the comparative studies was 14 to 23, while in the non-comparative studies, it was 11 to 15. Almost all the studies concluded that the thermal effect of different laser wavelengths did not hinder the histological diagnosis. This literature review showed some observations. The thermal effect occurred with different wavelengths and parameters and what should be done is to minimize it by better adjusting the laser parameters. The extension of margins during the collection of laser oral biopsies and the use of laser only in non-suspicious lesions are recommended because of the difficulty of the histopathologist to assess the extension and grade of dysplasia at the surgical margins. The comparison of the thermal effect between different studies was impossible due to the presence of methodological heterogeneity.

Disinfection of caries-affected dentin using potassium titanyl phosphate laser, Rose bengal and Ozonated water on shear bond strength of deciduous teeth

AimTo assess the efficacy of disinfection of caries-effected dentin (CAD) using KTP laser and different decontamination methods using ozonated water (OW), Rose Bengal photosensitizer (RBP), chlorhexidine (CHX), and Er, YAG laser on the shear bond strength (SBS) of adhesive resin bonded to deciduous teeth. Material and methodsA total of 50 extracted and radiographically verified carious primary molars were collected and scrutinized according to ICDAS criteria. Specimens were allocated randomly into five groups (n = 10) as per the type of CAD disinfectants. KTP laser, OW, RBP, CHX (control), and Er, YAG laser. After cavity sanitization, a resin adhesive (prime and bond NT) was smeared on the dentinal exterior followed by incremental composite filling. SBS evaluation was performed by employing specimens in the universal testing machine. The debonded surface was assessed under 40x magnification in a stereomicroscope to ascertain fracture mode. Statistical analysis was done by using the ANOVA and the Post Hoc Tukey multiple comparison tests (p < 0.05). ResultsThe highest SBS was exhibited by group 2 i.e., when OW was employed for CAD disinfection (10.25 ± 0.24 MPa). Whereas, the lowest SBS bond value was unveiled by samples in group 3 when RBP was applied for dentin surface sanitization (7.85 ± 0.59 MPa).CAD disinfection with KTP laser (8.25 ± 0.41 MPa), CHX (8.19 ± 0.73 MPa), and RBP displayed comparable bond values (p > 0.05). ConclusionOzonated water and Er, YAG laser could be employed as cavity disinfectants in primary teeth as they demonstrated better shear bond strength without jeopardizing the adhesive binding capacity of restorative resins bonded to caries-affected dentin.

Comparing KTP and CO2 laser excision for recurrent respiratory papillomatosis: A systematic review.

ObjectiveThe CO2 laser and 532 nm potassium titanyl phosphate (KTP) laser have been applied to treat recurrent respiratory papillomatosis (RRP). This systematic review sought to compare outcome differences between these two methods.Data SourcesEmbase, Web of Science, PubMed, and the Cochrane Library.Review MethodsCO2 laser and KTP laser studies were obtained by keyword searches of four authoritative medical databases. Articles were screened and retained when conforming to inclusion criteria. The primary outcome was cure rate; the secondary outcomes were recurrence, death, remission, clearance, and human papillomavirus (HPV)‐detected rates, as well as laser effectiveness rates. Postoperative complications rate was the safety outcome measure. All outcomes were summarized within the CO2 and KTP groups, with results statistically compared (p < .05).ResultsOverall, the cure rates were 87.25% (KTP group) and 75.98% (CO2 group; p < .05). Complication rates significantly differed between the KTP (2.32%) and CO2 (17.71%) groups (p < .0001). There was a relatively higher but not significant difference in the recurrence rates between the CO2 (18.6%) and KTP (10.87%) groups (p = .1595). The CO2 group remission rate was considerably lower (38.9%) than the KTP group (88.46%, p < .0001). HPV‐detected and clearance rates were only reported for the CO2 group. The bias risks were 13.1 ± 1.45 (CO2) and 13.6 ± 1.52 (KTP) for the two groups, indicating evidence was of fair quality.ConclusionOverall, KTP laser excision showed significantly better postoperative clinical outcomes than the CO2 laser, with a lower failure rate. Available fair‐quality evidence suggests KTP laser excision might be better for treating RRP. Nevertheless, more high‐quality randomized controlled studies are needed to compare these two surgical techniques, particularly in terms of reporting functional data such as vocal outcomes.

A Comparison of Potassium Titanyl Phosphate Laser and Pingyangmycin as Treatment for Adult Laryngeal Hemangioma.

Objective: To evaluate the curative effect of Potassium titanyl phosphate (KTP) laser and pingyangmycin injection for adult laryngeal hemangiomas (ALH). Methods: This was a retrospective study conducted on patients treated with either KTP laser or pingyangmycin injection to assess the efficacy of both treatment and compare the effects on different types of adult laryngeal hemangioma. Results: The ordinal logistic regression test showed the surgery methods had no effect on the therapeutic results. On the contrary, the shape of ALH and the interaction between surgical procedures and ALH shape affected the prognosis of the ALH. This meant that the shape of ALH might be a confounding factor affecting the therapeutic effect of ALH. Thus, the Cochran-Mantel-Haenszel test which is a stratification analysis was used to assess the interaction between surgical procedures and ALH shape. Then better results were achieved using the KTP laser for the plane and raised types of ALH. Conclusions: The selection of surgical procedures (the KTP laser or pingyangmycin injection approaches) affects the treatment of ALH. For plane and raised ALH, the KTP laser may be recommended.

Comparative Analysis of the Effectiveness of Modern Endoscopic Methods of Treatment of BPH

Since the 2000s, new energy systems for surgery for benign prostatic hyperplasia have rapidly become popular, including systems using bipolar energy and various laser systems such as holmium laser, potassium titanyl phosphate (KTP) laser, thulium laser and diode laser Over the past 10 years, the trend in the surgical treatment of benign prostatic hyperplasia has shifted from monopolar TURP to laser therapy and bipolar TURP. Based on the data on the effectiveness of the HoLEP technique, it becomes clear that HoLEP is ready to replace all these methods as a new standard, based on almost two decades of data that consistently demonstrate its better results and lower complication rate. This review summarizes the available literature by comparing HoLEP and traditional BPH treatments that are widely used and have long-term efficacy data. Despite the fact that there is such a wide arsenal of surgical treatment of BPH, each of these methods has its own advantages and disadvantages. This review article contains a significant portion of the best randomized data directly comparing HoLEP with alternative surgical treatments.

Comparison on the Efficacy and Safety of Different Surgical Treatments for Benign Prostatic Hyperplasia With Volume >60 mL: A Systematic Review and Bayesian Network Meta-Analysis of Randomized Controlled Trials.

The objective of this study was to compare the efficacy and safety of 10 different surgical treatments for benign prostatic hyperplasia (BPH) with volume >60 mL. A systematic literature review and network meta-analysis of randomized controlled trials (RCTs) within a Bayesian framework was performed. A total of 52 parallel-group RCTs included, reporting on 6,947 participants, comparing open prostatectomy (OP), monopolar/bipolar transurethral resection of prostate (monopolar/ bipolar TURP), thulium, holmium and diode laser enucleation of prostate (LEP), bipolar enucleation of prostate, potassium titanyl phosphate laser vaporization of prostate (KTP LVP), bipolar vaporization of prostate (bipolar VP), and laparoscopic simple prostatectomy (laparoscope SP). Compared with OP, laparoscope SP identified better maximal flow rate (Qmax; mean differences [MDs] = 2.89 mL/s) at the 24th month, but bipolar VP demonstrated worse Qmax (MD = −3.20 mL/s) and International Prostate Symptom Score (IPSS; MD = 2.60) at the 12th month. Holmium LEP (MD = 1.37) demonstrated better International Index of Erectile Function–5 at the 12th month compared with OP. However, compared with OP, KTP LVP demonstrated worse postvoid residual volume (PVR) at the sixth (MD = 10.42 mL) and 12th month (MD = 5.89 mL) and monopolar TURP (MD = 6.9 mL) demonstrated worse PVR at the 12th month. Eight new surgical methods for BPH with volume >60 mL appeared to be superior in safety compared with OP and monopolar TURP due to fewer complications. Bipolar VP and KTP LVP maybe not suitable for prostates more than 60 mL due to short- and middle-term worse Qmax, IPSS, and PVR than OP.

Potassium titanyl phosphate laser surface tympanoplasty as a novel treatment for chronic granular myringitis in a series of 14 patients.

Granular myringitis is a chronic and difficult-to-treat condition of the tympanic membrane. This paper presents a minimally invasive treatment technique using the potassium titanyl phosphate laser. A retrospective case review of patients who underwent potassium titanyl phosphate laser treatment between 2015 and 2020 was performed. All patients underwent final telephone follow up in 2020 to ascertain whether they had any ongoing myringitis symptoms, and all were offered further face-to-face follow up. Fourteen patients with myringitis were identified, with one patient having both ears affected. Of the 15 affected ears, 2 required a second treatment, resulting in a total of 17 laser treatment cases. At the first post-operative review, the appearance of the drum had improved in 10 out of 17 treatments (59 per cent). Potassium titanyl phosphate laser surface tympanoplasty may provide safe, quick and effective resolution of myringitis.

KTP laser endoscopic cricopharyngeal myotomy and pharyngeal pouch diverticulotomy: how I do it.

Various endoscopic methods of dividing the cricopharyngeus muscle and the common wall of pharyngeal pouches have been described, most commonly the carbon dioxide laser and stapler. The potassium titanyl phosphate (KTP) laser is an alternate endoscopic method of treatment. The KTP laser is used in a similar fashion to the carbon dioxide laser to carefully divide the cricopharyngeus muscle fibres ± the common wall of the pharyngeal pouch, under microscopic visualization. The KTP laser is an alternative and safe technique for these surgeries.

Use of 532 nm Potassium Titanyl Phosphate Laser on Vocal Fold Scars Under Topical Anesthesia: A Pilot Study

This pilot study aims to evaluate the efficacy of 532 nm potassium titanyl phosphate (KTP) laser under topical anesthesia in patients with vocal fold scars. A series of 18 patients with vocal fold scars of varying degrees were treated. The KTP laser was used under local anesthesia in the outpatient clinic. It was set to deliver 6 W of power using a continuous output mode. Close-to-contact mode was used for laser irradiation, and contact mode was used for ablation and excision of the lesions. Some of the patients received laser scar ablation on both vocal folds; the scarred vocal fold on one side and the hypertrophic vocal fold on the other. Parameters include glottic closure, amplitude, and mucosal wave pattern were measured using laryngeal stroboscopic examination. Aerodynamic and voice evaluations were carried out using maximum phonation time (MPT), jitter, shimmer, Voice Handicap Index questionnaire (VHI-30), and GRBAS scale. In total, 21 surgeries were performed on 18 patients. Glottic closure, amplitude, and mucosal wave pattern showed improvement 2 months postoperatively (P < .05). There was significant improvement in the postoperative scores for VHI-30, VHI-emotional sub-scale, VHI-physical sub-scale, and GRBAS (P < .05). There was no significant difference in the MPT and VHI-functional sub-scale before and after the operation (P > .05). Re-adhesion of the anterior commissure was observed in 2 patients with Type III scars. The 532 nm KTP laser is an effective tool for the treatment of vocal fold scars. Further research is required to determine if serial laser applications could improve outcomes for this challenging condition. Level IV.
