Short-term outcome of diverting loop ileostomy reversals performed by residents: a retrospective cohort prognostic factor study

AimThe reversal of diverting loop ileostomy (DLI) is one of surgical trainees’ first procedures. Complications of DLI reversal can cause life-threatening complications and increase patient morbidity. This study compared DLI reversals performed by surgical trainees with those by attending surgeons.MethodThis retrospective cohort study was performed at a single primary care center on 300 patients undergoing DLI reversal. The primary outcome was morbidity, according to the Clavien-Dindo classification (CDC), with special attention paid to the surgeon’s level of training. The secondary endpoint was postoperative intestinal motility dysfunction.ResultsSurgical trainees had significantly longer operation times (p < 0.001) than attending surgeons. Univariate analyses revealed no influence on the level of training for postoperative morbidity. First bowel movement later than 3 days after surgery was a significant risk factor for CDC ≥\\documentclass[12pt]{minimal} \\usepackage{amsmath} \\usepackage{wasysym} \\usepackage{amsfonts} \\usepackage{amssymb} \\usepackage{amsbsy} \\usepackage{mathrsfs} \\usepackage{upgreek} \\setlength{\\oddsidemargin}{-69pt} \\begin{document}$$\\ge$$\\end{document} 3 (OR, 4.348; 96% CI, 1670–11.321; p = 0.003). Independent risk factors for surgical site infections (SSIs) were an elevated BMI (OR, 1.162; 95% CI, 1.043–1.1294; p = 0.007) and a delayed bowel movement (OR, 3.973; 95% CI, 1.300–12.138; p = 0.015). For postoperative intestinal motility dysfunction, an independent risk factor was a primary malignant disease (OR, 1.980; 95% CI, 1.120–3.500; p = 0.019), and side-to-side stapled anastomosis was a protective factor (OR, 0.337; 95% CI 0.155–0.733; p = 0.006).ConclusionEven though surgical trainees needed significantly more time to perform the surgery, the level of surgical training was not a risk factor for increased postoperative morbidity. Instead, delayed first bowel movement was predictive of SSI.

Effect of L-arginine on the resolution rate of postoperative ileus in patients with urgent abdominal pathology

Background. Postoperative ileus is widespread in the postoperative period and occurs not only after extended operations on the abdominal cavity, retroperitoneal cavity, but also after minor surgical interventions, including laparoscopic operations [1, 2]. The aim of this study is to increase the effectiveness of intensive care after urgent abdominal surgeries by developing a drug correction for bowel motility disorders based on studying of the motor and evacuator function of the gastrointestinal tract, inflammatory markers, hemodynamic parameters and electrolyte blood composition.&#x0D; Materials and methods. 51 patients were examined, who underwent urgent surgical interventions with laparotomy access for infringed hernias, acute intestinal obstruction, perforation of duodenal ulcers. Depending on the variant of drug correction of postoperative intestinal motility disorder, patients were divided into 2 groups. The first (control) group (n = 27). In the postoperative period, the pharmacological correction of the motor-evacuator function of the gastrointestinal tract was performed by combining a solution of 0,5% metoclopramide and neostigmine bromide. The second (main) group (n = 24). In the postoperative period, the pharmacological correction of the motor-evacuator function of the gastrointestinal tract was performed by combining 0.5% metoclopramide solution and L-arginine (Tivortin, Yuriya-Farm, Ukraine). Motorevacuation disorders of the gastrointestinal tract (the frequency of development of bloating, nausea, vomiting, delay in the escape of gases, an independent moving of gases, measurement of intra-abdominal pressure), parameters of hemodynamics, the level of systolic, diastolic, mean arterial pressure, cardiac index, stroke volume, total peripheral vascular resistance, heart rate, minute volume of blood circulation and left ventricular power, markers of inflammation (leukocyte count, fibrinogen, IL-lα, TNFα, IL-10, calculation of leukocyte intoxication index, the study of electrolytes of blood serum (Na+, K+, Ca2+, Cl- ) were studied before and after 1st, 3rd and 7th days after the operation.&#x0D; Results. The use of L-arginine leads to a decrease in the phenomena of peripheral vasospasm, which, improving the microcirculation of the intestinal wall leads to a faster recovery of the motor-evacuation function of the gastrointestinal tract. With standard prokinetic therapy (combination of metoclopramide with neostigmine bromide), the number of patients with elevated intra-abdominal pressure increases in 92,1%, оn the second day after surgery, the number of patients with elevated intra-abdominal pressure exceeds the norm in 100%. The resumption of intestinal motility 3 days after the operation is accompanied by the normalization of intra-abdominal pressure in 55% of patients. Against the backdrop of the use of L-arginine with metoclopramide on day 1 after the operation, the number of patients with an increase in intra-abdominal pressure does not exceed 50%. 37,5% of them have elevated intra-abdominal pressure, 8,3% have abdominal hypertension of the 1st degree. On the second day of the observation, intraabdominal pressure in 41,7% patients normalized, abdominal hypertension of 1 degree was noted in 16,7%. of them. On the 3rd day after surgery, intra-abdominal pressure corresponded to the norm in 79,2% of patients, abdominal hypertension of 1st degree was registered in 8,3% of patients.&#x0D; Сonclusions. The use of a combination of metoclopramide with L-arginine reduces the stress on the myocardium, activates - anti-inflammatory link of the cytokine system, accelerates the normalization of intra-abdominal pressure, increases the rate of self-evacuation, independent defecation, and reduces the patients’ stay in the intensive care, which makes it possible to recommend it as the most optimal remedy for resuming motor activity intestines after urgent operations in abdominal surgery.

Use of Gelatin Sponge Affects Postoperative Morbidity In Cesarean Section Patients.

BackgroundThis study aimed to determine the effects of use of a local hemostatic gelatin sponge (GS) on postoperative morbidity in patients undergoing cesarean section (CS).Material/MethodsThe records of 318 patients who underwent CS surgery were retrospectively evaluated. Group 1 consisted of 59 patients with gelatin sponge (GS) applied, and Group 2 consisted of 259 patients with no GS applied. The groups were compared for time to the first flatus, nausea and vomiting, requirement for anti-emetic drugs, development of postoperative ileus, and the length of hospitalization.ResultsThe patients in Group 1 and Group 2 were statistically similar in mean age, gravida, parity, and body mass index (BMI) (p=0.352, p=0.275, p=0.458, and p=0.814, respectively). No significant difference was determined in the number of patients with nausea, vomiting, anti-emetic drug use, febrile morbidity, and postoperative ileus (p=0.063, p=0.436, p=328, p=0.632, and p=0.179, respectively). Time to the first flatus and length of hospitalization were significantly longer in Group 2 (p<0.001 and p<0.001, respectively).ConclusionsDelay in recovery of bowel motility may be due to the local hypersensitivity reaction caused by GS and/or dislocation of this local hemostat. Women who receive gelatin sponge treatment during CS should be monitored closely for the recovery of postoperative intestinal motility.

The effect of Daikenchuto on postoperative intestinal motility in patients with right-side colon cancer.

Daikenchuto (DKT) has a stimulant effect on intestinal motility and reportedly has a positive effect on postoperative intestinal motility in patients with sigmoid colon cancer. In this study, we investigated the effects of DKT in patients with right-side colon cancer. This retrospective study included 88 patients with right-side colon cancer. We orally administered 7.5g of DKT in the DKT group and did not administer any DKT to patients in the no-DKT group. All patients ingested radiopaque markers 2h before surgery, which were used to assess intestinal motility. The postoperative intestinal motility was radiologically assessed by counting the numbers of residual markers in the large and small intestines. The DKT and no-DKT groups showed no marked differences in the total number of residual markers or number of residual markers in the small intestine. However, in the elderly subgroup, the total number of residual markers in the DKT group was significantly less than in the no-DKT group. Although DKT had some small effect on the postoperative intestinal motility for most patients, it may have positive effects in elderly patients.

Dysmotility by mechanical bowel preparation using polyethylene glycol

BackgroundThe effects of mechanical bowel preparation (MBP) on morbidity (e.g., anastomotic leakage and surgical site infection) have been evaluated. Its effect on early recovery after surgery has drawn renewed attention, and its use is discouraged in the postsurgical management of enhanced recovery. However, most surgeons in Japan prefer polyethylene glycol (PEG) for MBP. We investigated the effect of MBP with PEG on postoperative intestinal motility. Materials and methodsWe prospectively evaluated a consecutive series of 258 colon cancer patients who underwent colonic resection and primary anastomosis. We orally administered 2000 mL of PEG in the PEG group and did not administer PEG to patients in the no-PEG group. Postoperative gastrointestinal motility was assessed with radiopaque markers. All patients ingested radiopaque markers 2 h before surgery. Postoperative intestinal motility was radiologically assessed by counting the number of residual markers. Abdominal radiography was conducted on postoperative days 1, 3, and 5 to count residual markers in the large and small intestines. ResultsThe total number of residual markers in the no-PEG group was less than that in the PEG group on day 5 (P < 0.01) but not on days 1 and 3. On all 3 d, the numbers of residual markers in the small intestine were significantly less in the no-PEG group than in the PEG group (P < 0.001). There were no differences in postoperative complications between the no-PEG and PEG groups. ConclusionsPEG can negatively affect postoperative intestinal motility, and MBP using PEG is unnecessary in elective colon cancer surgery.

The clinical significance of pathological studies of congenital intestinal atresia

ObjectiveThe purpose of this study was to explore the mechanisms of postoperative intestinal motility disorders in intestinal atresia patients by investigating the expression profiles of proteins, including calretinin (CR), glial-derived neurotrophic factor (GDNF), bone morphogenetic protein 2 (BMP-2), c-kit, α-smooth muscle actin (α-SMA), and S-100 protein; to decipher the correlation between the area of the pathological segment and the alteration of the above 6 proteins; and thereby to provide a clinical specific reference values to determine the removal length for intestinal tract resection. MethodsImmunohistochemistry technique was applied to detect the CR, c-kit, GDNF, BMP-2, α-SMA, and S-100 protein in specimens of atretic, proximal, and distal intestine from 25 cases of intestinal atresia and samples of intestinal walls from 10 non-atresia control specimens. The alteration of the enteric nervous system, nerve growth and its regulatory factors, the interstitial cells of Cajal (ICCs), and the enteric muscle system were examined, with particular attention being paid to pathological changes and the lesion area. ResultsThe expression of all of the abovementioned 6 proteins in the proximal side of the atresia was significantly lower than in control group. The expression of the abovementioned proteins tended to be higher farther away from the atresia site. The expressions of both GDNF and BMP-2 had returned to normal level at 10 cm proximal to the atresia site, whereas the expressions of CR, c-kit, α-SMA, and S-100 protein only returned to normal at 15 cm proximal to the atresia site. On the distal side, the expression of all 6 markers at 3 cm distal to the atresia site was normal. ConclusionPathological deterioration of the myenteric ganglia, nerve growth factor, and ICCs are the causes of intestinal motility disorders after the surgical repair of intestinal atresia. Our data support resecting an intestinal segment extending from 15 cm proximal to 3 cm distal to the atretic segment. In proximal jejunal atresia, when it is not possible to resect 15 cm, we suggest resecting as much of the hypertrophic proximal intestine as possible. Based on our data, we believe this surgical practice could improve postoperative dysmotility in these patients.

Changes in intestinal motility and in the myenteric plexus in a rat model of intestinal ischemia–reperfusion

Background/Purpose Early histologic changes induced by intestinal ischemia–reperfusion injury (IIRI) have been extensively studied using animal models. However, information regarding late effects on intestinal motility is lacking. The aim of this study was to investigate the late effects of IIRI on myenteric plexus histology and intestinal motility. Materials and Methods Thirty-two postweaning male mice weighing between 58 and 103 g were divided randomly into 4 groups: Control (unoperated), Sham (celiotomy), 30-minute ischemia (celiotomy and superior mesenteric artery ischemia for 30 minutes), and 45-minute ischemia (celiotomy and superior mesenteric artery ischemia for 45 minutes). Postoperative intestinal motility was assessed by weighing total fecal output for 24 hours on the 3rd, 7th, 14th, and 21st day after surgery. Segments of duodenum, jejunum, and ileum were examined at light microscopy for changes in the myenteric plexus. Results Three weeks after IIRI, the ganglion cells from the myenteric plexus appeared in light microscopy, spongy or foamy, containing many vacuoles in their cytoplasm. The neuronal nucleus became irregular, with degenerative signs. These alterations did not occur among animals from the control or sham groups. Although the animals of the 45-minute ischemia group showed a significant drop in fecal output in the 21st postoperative day, this appeared to have no effect on weight gain. Conclusions The results suggest that intestinal ischemia–reperfusion causes late neuronal damage. These changes resulted in alterations of intestinal motility, which, within the conditions of the present study, had no repercussion on general weight gain.

Postoperative Intestinal Motility Following Conventional and Laparoscopic Intestinal Surgery

To evaluate the influence of conventional vs laparoscopic right-sided colectomy on postoperative motility of the stomach, small bowel, and large bowel, and to study the effects on postoperative intestinal motility of anesthesia only and of laparotomy vs laparoscopy only. Prospective randomized study in a canine model. Animal research laboratory. Twelve mongrel dogs weighting 23.4 to 29.6 kg. (1) Laparotomy or laparoscopy and (2) 1 week after complete recovery from the first procedure, right-sided colectomy with ileocolic anastomosis using conventional or laparoscopic techniques. End points of the study were (1) the postoperative time until the electrical response activity of the stomach, small intestine, and large intestine returned to a normal interdigestive pattern and (2) the time until first postoperative bowel movement occurred. Median time until return to normal interdigestive pattern of myoelectrical activity after laparoscopic colectomy was about 40% less than after conventional colectomy (P < .05). Time until first bowel movement was 27% reduced (P < .05). Recovery from postoperative ileus following laparoscopic surgery is more rapid than after conventional surgery in the canine model. Confirmatory human studies should be performed.

Epidural anaesthesia and postoperative colorectal motility--a possible hazard to a colorectal anastomosis.

The effects of epidural anaesthesia (EDA, mepivacaine) and EDA in combination with atropine and neostigmine on postoperative intestinal motility were studied in 17 patients undergoing operation for cancer of the rectum or sigmoid colon. Motility was recorded by a volumetric technique. Epidural anaesthesia (EDA) increased motor activity in the small bowel as well as in the left colon and rectum. Phasic motility dominated in the small intestine whereas tonic and segmental contractions were recorded from the large bowel. EDA induced a powerful tonic contraction with a concomitant shortening of the rectum. This effect was inhibited by atropine. The influence of atropine/neostigmine on left colonic motor activity was studied in six patients before and during EDA in a cross-over fashion. When administered alone, atropine/neostigmine did not cause any motility increase. Atropine/neostigmine administered during EDA, however, elicited a significant increase of motility. The increase of intestinal motor activity induced by EDA may expose a newly constructed colorectal anastomosis to undue strain in the immediate postoperative period. When EDA is used in combination with general anaesthesia, particular attention should be directed towards the use of neostigmine for reversing the effect of nondepolarizing muscle relaxants. Atropine appears under such circumstances not to protect from the excitatory effects of this drug on colorectal motility.

Pharmacological manipulation of postoperative intestinal adhesions.

Paralytic ileus and intestinal adhesions are common events following intra-abdominal surgery. The theoretical hypothesis 'that stimulation of the postoperative bowel will reduce intestinal adhesions' was studied in a rat model for intestinal adhesions in which postoperative bowel motility was pharmacologically manipulated. Immediate postoperative stimulation of gastrointestinal motility by the prokinetic agent, Cisapride, resulted in a significant reduction in both the number and extent of adhesions. Inhibition of postoperative intestinal motility with the anticholinergic agent, atropine, resulted in a greater number of more dense adhesions involving an increased length of bowel.

Postoperative intestinal motility in dogs and sheep.

Electrical activity of the small intestine was recorded from chronically implanted electrodes in dogs and sheep. The effects of laparotomy and surgical procedures were identified under general anesthesia. Laparotomy induced in both species a complete inhibition of electrical spiking activity during the stage of muscular and peritoneal incision. The inhibitory period due to laparotomy was shorter in dogs than in sheep and in both species less marked at the duodenal level. A secondary period of inhibition (ileus) lasting from 6 hr for a resection of the colon to 24 hr for a transection of the small bowel was observed in dogs after a transient phase of normal activity during 3-6 hr. The period of inhibition due to abdominal surgery lasted from 48 to 72 hr and occurred at the time of surgery in sheep. In both species, inhibition due to laparotomy and surgical procedures was prevented by splanchnicectomy. The results showed that myoelectric activity of the different parts of the small intestine is specifically inhibited and/or disorganized through peripheral reflex pathways. They are activated at different degrees by laparotomy and abdominal surgery.
