Analysis of Ultrasonic Emulsification Surgery and Small Incision Cataract Extracapsular Extraction Surgery for Cataract Clinical Treatment Level Improvement

Objective: To analyze the efficacy of ultrasonic emulsification and small incision cataract extracapsular extraction in cataract patients. Methods: 96 cataract patients admitted from May 2021 to May 2023 were selected and randomly grouped into group A (ultrasonic emulsification) and group B (small-incision extracapsular cataract extraction), with 48 cases each. Results: At 1 week, 1-month, and 3 months post-operation, the visual acuity of group A was higher and the astigmatism value was lower than that of group B (P < 0.05); at 12h, 24h, and 48h post-operation, the intraocular pressure of group A was higher than that of group B (P < 0.05); the thickness of macular area of group A was lower than that of group B at 1 week and 1-month post-operation (P < 0.05). Conclusion: Ultrasonic emulsification in cataract patients was slightly better than small incision cataract extracapsular extraction in correcting astigmatism, improving visual acuity, and regulating macular thickness. However, due to the high energy of ultrasonic emulsification, the risk of complications such as high postoperative intraocular pressure was higher. Small-incision extracapsular cataract extraction has better application value in economically disadvantaged areas.

The effect of irrigation and aspiration on the corneal endothelial cell density in patients undergoing Implantable Collamer Lens with a central hole implantation for myopia correction.

In the conventional technique, viscoelastic agents are typically rinsed away with balanced salt solution (BSS), but it may lead to a series of complications such as viscoelastic residue, anterior chamber instability and intraoperative TICL rotation. The utilization of irrigation and aspiration (I/A) has been shown to be effective in maintaining anterior chamber stability, reducing the incidence of postoperative high intraocular pressure, and minimizing postoperative fundus complications. However, there is a lack of previous studies investigating the impact of I/A on corneal endothelial cells during ICL implantation. The objective of this study was to examine the effect of I/A on corneal endothelial cells in patients undergoing myopia correction through implantation of Implantable Collamer Lens with a central hole (V4c ICL). A retrospective selection was made of 344 eyes from 172 patients who underwent V4c ICL implantation and I/A to remove viscoelastic agent from the anterior chamber between 2021 and 2022. The intraocular pressure (IOP) was measured at 1h, 2h and 3h after surgery. Corneal endothelial cell density (ECD), coefficient of variation in cell size (CV), standard deviation of cell area (SD), and percentage of hexagonal cells (HEX) were evaluated at 1week postoperatively to assess corneal endothelial cells. The first two represent polymegethism or morphological variation, while the third parameter represents the degree of polymorphism of the corneal endothelial cells. Electronic medical records were utilized for data collection purpose. All surgeries proceeded without complications. The IOP was 16.50 ± 3.42mmHg (range: 11.5-22.3mmHg) prior to surgery and increased to 21.25 ± 5.61mmHg (range: 9.5-34.8mmHg), 19.85 ± 5.18mmHg (range: 11.4-36.2mmHg) and finally settled at an average of 18.81 ± 4.57mmHg (range: 10.1-38.8mmHg) at the respective time points of 1h, 2h and 3h after surgery. The preoperative ECD was recorded as being approximately 3004 ± 295 cell/mm2, which exhibited a marginal decreased of 1.17% postoperatively, resulting in an average ECD value of 2969 ± 303 cell/mm2 one week after surgery (P = 0.12). Similarly, the preoperative CV was determined as 31.10 ± 3.78%, and it experienced a slight reduction with an average CV value of 30.74 ± 3.77% at week after surgery (P = 0.21). And, the preoperative SD was reported as 104.76 ± 17.26, and it remained virtually unchanged with an average SD value of 104.85 ± 18.75 at one week after surgery (P = 0.95). The preoperative HEX was calculated as 55.38 ± 8.94%, and it remained its stability with an average HEX value of 55.45 ± 8.73% one week after surgery (P = 0.92). The utilization of I/A led to a slight decrease in postoperative ECD when compared to conventional surgical techniques. Nevertheless, the reduction in ECD remained within acceptable limits, taking into accout the avervantaged it offered, such as stabilization of the anterior chamber and decreased occurrence of viscoelastic residue after surgery. It is challenging to anticipate the long-term safety of corneal endothelial cells based on current short-term studies. However, this study provides a valuable reference indicating that neither anterior chamber irrigation nor I/A aspiration have an adverse impact on the safety of corneal endothelial cells in the short term. Further research is imperative to enhance our understanding of their effects over an extended period.

Outcome of combined trabeculectomy with cataract surgery in patients on prostaglandin analogs and aqueous suppressants.

To compare the effect of prostaglandin analogs (PGA) against other glaucoma medications (non-PGA) on the intraocular pressure (IOP) outcomes of combined trabeculectomy with phacoemulsification, and the conjunctival cell profile in persons with primary open-angle (POAG) and pseudoexfoliation glaucoma (PXFG). A prospective cohort study was conducted among 116 patients with POAG or PXFG on glaucoma medications for a minimum of 3 months undergoing glaucoma triple procedure. Patients were divided into two groups (PGA and non-PGA) based on preoperative exposure to PGA. IOP outcomes were assessed for up to 2 years. Conjunctival biopsy specimens were obtained at the time of surgery, and histopathological analysis was performed. Forty-two patients were in the PGA group, 67 were in the non-PGA group, and seven were lost to follow-up. The non-PGA group had lesser mean postoperative IOP and needed fewer postoperative medications compared to the PGA group in all visits up to 2 years. The non-PGA group had better complete success rate (50.7% vs. 14.3%, P < 0.001). Kaplan-Meier survival estimates showed a significant difference in cumulative complete success rate between non-PGA (67%) and PGA (26%) by 24 months ( P < 0.001). The Cox proportional model showed the type of drug to be significantly associated with surgical failure. Histopathological analysis revealed that the PGA group had higher numbers for each type of inflammatory cell (except mast cells) compared to the non-PGA group. Patients on PGA are likely to have a higher postoperative IOP and may need more medications for IOP control after a glaucoma triple procedure.

Primary Posterior Continuous Circular Capsulorhexis without the Use of Ophthalmic Viscosurgical Device.

Performing primary posterior capsulorhexis (PPCCC) during cataract surgery is an effective prevention of secondary cataracts. It is important not to damage the anterior vitreous membrane to minimize the possible complications and adverse side effects of this step of the surgery. Most authors use a viscoelastic material to protect the anterior vitreous membrane, injected into the space between the posterior lens capsule and the vitreous membrane through a small hole in the posterior capsule. The creation of posterior capsulorhexis using 25G forceps under an implanted intraocular lens while continuously filling the anterior chamber with irrigation fluid from an irrigation cannula, without the use of a viscoelastic material is, in our opinion, a safe, fast and cheap technique of PPCCC. This technique could theoretically lead to a lower incidence of postoperative complications such as higher postoperative intraocular pressure or anterior segment inflammatory response.

Risk factors for intraocular pressure elevation in a six-month period after ab interno trabeculotomy using a Kahook Dual Blade

BackgroundTo examine the risk factors for an early postoperative intraocular pressure (IOP) increase after ab interno trabeculotomy using a Kahook Dual Blade (KDB trabeculotomy).MethodsA retrospective study was performed in 76 exfoliation glaucoma (EXG) eyes and 56 primary open angle glaucoma (POAG) eyes that underwent KDB trabeculotomy, with or without cataract surgery at Kumamoto University Hospital. Postoperative high IOP was classified as IOP≥20 mmHg (within three months after surgery, whether persistent or temporary), transient IOP≥20 mmHg (IOP≥20 mmHg after surgery, then dropped below 20 mmHg), and the presence of IOP spikes (≥ 10 mmHg from baseline). Risk factors were examined using logistic regression analysis.ResultsThe preoperative mean IOP (SD) was 24.98 (7.23) mmHg in patients with EXG and 21.28 (6.58) mmHg in patients with POAG. IOP was reduced by 32.1% in patients with EXG and by 17.7% in patients with POAG at 6 months after surgery. Postoperative IOP≥20 mmHg was observed in 56.6% of EXG patients and in 51.8% of POAG patients. IOP spikes occurred in 15.8% of EXG patients and in 14.3% of POAG patients. Logistic regression analysis showed that factors with significant odds ratios (ORs) were age (OR = 0.866, 95% CI = 0.793–0.945), preoperative medication use (OR = 2.02, 95% CI = 1.17–3.49), trabeculotomy in combination with cataract surgery (OR = 0.0674, 95% CI = 0.015–0.303), and IOP at day 1 (OR = 1.41, 95% CI = 1.18–1.68) for postoperative IOP≥20 mmHg, the IOP at day 1 (OR = 1.1, 95% CI = 1.03–1.17) for transient IOP≥20 mmHg, and age (OR = 0.948, 95% CI = 0.901–0.997) and preoperative IOP (OR = 0.83, 95% CI = 0.736–0.936) for IOP spikes.ConclusionAlthough KDB trabeculotomy is an effective treatment for patients with EXG and POAG, patients who take multiple preoperative medications and have a high IOP on day 1 require careful follow-up to prevent postoperative IOP elevation.

Pseudo-reversal of optic disc cupping in a case of pseudo-exfoliation glaucoma following trabeculectomy

Reversal of glaucomatous cupping is rare in adults. A case of pseudo-reversal of total cupping in an elderly gentleman with pseudo-exfoliation glaucoma who underwent combined trabeculectomy with cataract surgery is reported. Early laser suture lysis to control high post-operative intra-ocular pressure (IOP) led to sudden lowering of IOP with subsequent optic disc edema, which manifested as reversal of cupping, albeit temporarily. Eight months later, the visual field was stable, but the macula had developed an epiretinal membrane, probably secondary to hypotony-associated inflammation, thus highlighting the importance of timing the suture lysis and aggressive control of intra-ocular inflammation.

Factors Associated with Deterioration of Primary Angle Closure after Lens Extraction.

The purpose of the study was to explore factors associated with glaucomatous deterioration in eyes with primary angle closure (PAC) after lens extraction, including PAC suspect (PACS), PAC, and PAC glaucoma (PACG). We retrospectively analyzed data of 77 eyes with PACS, PAC, and PACG that underwent lens extraction with more than 2 years postoperative follow-up. Postoperative glaucoma progression was analyzed by either structural (optic disc/retinal nerve fiber layer (RNFL) photographs or optical coherent tomography (OCT)) or functional (visual field (VF)) criterion. Cox proportional hazard analysis (hazard ratio (HR)) was used to determine risk factors for progression using uni-and multivariate analysis. The analysis was conducted in groups with or without glaucomatous optic neuropathy (PACS/PAC vs. PACG). Forty-one eyes with PACS/PAC and 36 eyes with PACG were included. The mean postoperative follow-up period was 3.5 ± 1.4 years. Intraocular pressure (IOP) was reduced postoperatively from 23.1 ± 14.4 to 13.4 ± 2.1 mmHg. In the PACS/PAC group, seven eyes (17.0%) showed structural progression, but none showed progression in VF. Preoperative RNFL thickness was the only risk factor for structural progression (HR = 0.928, p = 0.002) in the PACS/PAC group. In the PACG group, 24 eyes (66.7%) showed structural progression and 12 eyes (33.3%) showed VF progression. Thinner preoperative RNFL thickness (HR = 0.964, p = 0.043) and high postoperative IOP fluctuation (HR = 1.296, p = 0.011) were significantly associated with VF progression; none of the factors were associated with structural progression. Angle closure eyes with thinner baseline RNFL thickness and higher postoperative IOP fluctuation may require careful follow-up for glaucoma progression after lens extraction.

Effects of Preoperative Intraocular Pressure Level on Surgical Results of Microhook Ab Interno Trabeculotomy.

To assess the roles of preoperative intraocular pressure (IOP) level on the IOP reducing efficacy of microhook ab interno trabeculotomy (µLOT), 126 consecutive open-angle glaucomatous eyes (90 primary open angle glaucoma, 36 exfoliation glaucoma) of 126 Japanese subjects (60 men, 66 women; mean age ± standard deviation, 70.5 ± 11.4 years) who underwent µLOT alone (25 eyes, 20%) or combined µLOT and cataract surgery (101 eyes, 80%) were retrospectively included, and subdivided into four groups based on the quartile of preoperative IOP: Q1, ≤15 mmHg; Q2, 15–18 mmHg, Q3, 18–21 mmHg, and Q4, >21 mmHg. Preoperative and 12 months postoperative IOPs and numbers of antiglaucoma medications were compared among IOP groups. Factors associated with postoperative IOP were assessed using multivariate analysis, and the success of IOP control was assessed with various definitions. Postoperatively, IOP was significantly higher in Q3 (p < 0.0146) and Q4 (p = 0.0320) groups than Q1 group, while the number of medications was not significantly different among four IOP groups (p = 0.1966). Older age was associated with lower postoperative IOP, and higher preoperative IOP was associated with higher postoperative IOP, while sex, glaucoma type, surgical procedure, lens status, extent of trabeculotomy incision, and preoperative number of medications were not associated with postoperative IOP. The success rates for IOP ≤ 18 and ≤15 mmHg were higher in lower preoperative IOP groups (i.e., Q1 and Q2) than higher preoperative IOP groups (Q3 and Q4), while the success rate for ≥20% IOP reduction was higher in higher preoperative IOP groups than in lower preoperative IOP groups; the success rate for ≥0% IOP reduction was equivalent among groups. By reviewing the previous studies in ab interno trabeculotomy/goniotomy procedures, positive correlation between preoperative and postoperative IOPs was preserved throughout the studies and surgical procedures. After the µLOT, larger %IOP reduction was achieved in higher preoperative IOP groups than in lower preoperative IOP groups, while postoperative IOP was still lower than in lower preoperative IOP groups.

Common and rare complications following filtering surgery for children with congenital glaucoma; a5 years study.

To determine the incidence of postoperative complications following combined trabeculotomy trabeculectomy with mitomycin C (CTTM) for congenital glaucoma (CG), in addition to documentation of some unusual complications and possible ways to manage these complications safely with minimal morbidity. A retrospective observational study was performed on 190 eyes with CG had a CTTM procedure.Included eyes were operated upon between February 2015 and February 2020 in Sohag University Hospitals. Medical records were reviewed and postoperative complications were reported. Incidence of postoperative complications and their management were the main outcome measures. Early complications developed the form of shallow anterior chamber (AC) (16.3%), hyphema (10.5%), serous choroidal detachment (2%), hemorrhagic choroidal detachment (2%). Unusual and rare complications occurred in the form of iatrogenic Cushing syndrome, unilateral toxic keratopathy developed, vitreous hemorrhage (VH), intercalary staphyloma and decompression retinopathyLate complications included: high postoperative intraocular pressure (IOP) (16.5%), thin cystic blebs (13.1%), hypotony disc edema (2%), cataract (3.2%), one eye developed blebitis and one eye developed endophthalmitis. Surgical management of CG is full of complications in early and late postoperative periods such as shallow AC, hyphema and thin cystic blebs. Some unusual and rare complications developed such as iatrogenic Cushing syndrome, unilateral toxic keratopathy, vitreous hemorrhage, intercalary staphyloma and decompression retinopathy. If identified early; all these complications could be managed properly leading to a successful and favorable outcome.

Descemet's stripping endothelial keratoplasty (DSAEK) in failed penetrating keratoplasty patients: graft survival rate and its associated factors

Objective: To explore graft survival rate and its associated factors of DSAEK in patients with endothelial decompensation after penetrating keratoplasty (PKP). Methods: A retrospective case study was conducted, including 51 patients (51 eyes) that underwent DSAEK for endothelial decompensation after PKP in Department of Ophthalmology in Peking University Third Hospital from June 2009 to September 2017. The mean recipient age was (43.3±21.0) years. There were 36 males and 15 females. Data comprising demographic details,surgical methods, complications,follow-up durations,final states of grafts,preoperative and postoperative risk factors were collected. Kaplan-Meier analysis was used to determine the cumulative probability of graft survival. Cox regression model was developed to examine for factors associated with graft survival. Results: Among 51 eyes,the overall graft survival rate was 78%(40/51). The cumulative probability was 85% (95%CI: 0.73-0.96) and 69% (95%CI: 0.52-0.86) at 1 year and 2 years, respectively. The median survival time was 42 months. The diameter of DSAEK graft and postoperative intraocular pressure correlated with the long-term survival of graft. Larger diameter of DSAEK graft was a protective factor for graft survival (HR=0.29, 95%CI: 0.12-0.73), while postoperative high intraocular pressure was a risk factor (HR=1.08, 95%CI: 1.00-1.16, HR=1.10, 95%CI:1.01-1.12). Recipient age, sex, lens status, the number of previous PKPs and previous glaucoma surgery had no significant effect on long-term survival (P>0.05). Conclusions: DSAEK is an effective treatment for endothelial decompensation after PKP,with favorable graft survival rate. Larger diameter of graft is beneficial to the long-term survival,while high postoperative intraocular pressure is a risk factor for graft failure. (Chin J Ophthalmol, 2019, 55: 428-434).

Factors related to filtration-bleb morphology after Ex-PRESS® surgery.

Purpose:We investigated the factors influencing the morphology of filtration blebs after Ex-PRESS® surgery. We analyzed the thickness of the bleb wall and the height and the volume of blebs.Methods:This was a retrospective non-randomized study. After excluding patients who had undergone an additional glaucoma surgery, we analyzed the cases of 145 consecutive patients (180 eyes) who underwent trabeculectomy with Ex-PRESS® for the first time at Toyama University Hospital and were followed for >1 year. We used anterior segment optical coherence tomography to analyze the morphology of the blebs after Ex-PRESS surgery. We also examined potential influencing factors including age, central corneal thickness, type of glaucoma (primary open-angle glaucoma [POAG] or pseudo-exfoliation glaucoma [PEXG]), preoperative intraocular pressure (IOP), postoperative IOP, history of trabeculotomy, and operation method: Ex-PRESS surgery only, or simultaneous cataract surgery.Results:Ex-PRESS surgeries significantly decreased the IOP from 24.5 ± 8.8 mmHg to 11.1 ± 3.4 mmHg after 1 year (P < 0.001). The cases with higher blebs, larger volume of blebs, and thinner bleb wall had better surgical outcomes.Conclusion:Advanced age, higher postoperative IOP, PEXG, and simultaneous cataract surgeries were found to decrease the volume and height of blebs. Younger age, higher postoperative IOP, POAG, and simultaneous cataract surgeries were found to thicken the wall of blebs. Among the younger patients, there were many cases in which surgery was unsuccessful in spite the large volume of blebs. The reason for this may be that the wall of the bleb is thick.

Factors influencing the filtration-bleb volume after Ex-PRESS® surgery.

PurposeThe aim of this study was to investigate the factors influencing the volume of the filtration bleb after Ex-PRESS® surgery.MethodsThis was a retrospective non-randomized study. After excluding patients who had undergone an additional glaucoma surgery, 99 glaucoma patients who underwent Ex-PRESS® surgeries and were followed up for >2 years were analyzed. We used anterior segment optical coherence tomography to measure the bleb volume, and compared the volume at 1 year with that at 2 years after surgery. We also examined potential influencing factors, including age, number of medications, central corneal thickness, type of glaucoma, preoperative intraocular pressure (IOP), postoperative IOP, reduction ratio of IOP, history of trabeculotomy, and operation method (Ex-PRESS® surgery only or simultaneous cataract surgery).ResultsEx-PRESS® surgeries significantly decreased the IOP from 26.3±9.7 mmHg to 11.6±3.7 mmHg after 24 months (P<0.001). The success rate at 2 years was 81.2% (IOP ≥21 mmHg or ≥20% reduction from the baseline IOP). The results showed that age, postoperative IOP, glaucoma type, and simultaneous cataract surgeries influenced the filtration-bleb volume.ConclusionAdvanced age, higher postoperative IOP, pseudo-exfoliation glaucoma, and simultaneous cataract surgeries were all found to decrease the volume of the filtration bleb.

Analysis of cause of recurrent herpes simplex keratitis after vitrectomy

Objective To analyze the related factors about the recurrence of herpes simplex keratitis (HSK) after vitrectomy. Methods The data of 108 eyes of 108 patients with HSK from Mar. 2013 to Jul. 2016 were collected and analyzed retrospectively. Herpes simplex keratitis was recurred in some patients after operation. The preoperative, intraoperative and postoperative factors related with recurrence of HSK were compared and analyed between the recurrent cases and no recurrent cases. Results In 108 eyes, 36 (33.3%) eyes were found recurrent HSK after vitrectomy. The size of vitrectomy cutter head, intraoperative observation system, intraocular tamponade, postoperative capsular membrane integrity were not relative with recurrence of HSK. And the age, diabetes, preoperative HSK recurrence times, operative time, surgical methods, corneal epithelial curettage, postoperative high intraocular pressure, corneal lesions and influenza were relative with recurrence of HSK. Conclusion The factors affected the function of corneal epithelial cells and the local or systemic immune are related with the postoperative recurrence of HSK. A novel suturing technique for filtering glaucoma surgery: the accordion suture.

To present a novel scleral flap suturing technique for filtering glaucoma surgery in order to control high postoperative intraocular pressure (IOP). Description of 'the accordion suture' technique for mitomycin C augmented trabeculectomy. In cases of postoperative high IOP, pulling the loop of the suture helps to lift up the scleral flap by an even pressure on both edges. By means of this technique, the scleral flap opens up in an "accordion" manner, thus preventing flap obstruction and providing adequate aqueous flow. Our study group consisted of 8 eyes of 8 patients with neovascular glaucoma. Mean age of the subjects was 67.42±8.21y and female/male ratio was 4/4. Mean preoperative IOP was 37±7.48 mm Hg. Mitomycin C augmented trabeculectomy was carried out on the subjects without any complications. The scleral flap closure is performed with three separate sutures; initially, our accordion suture through the center of the flap, and two releasable sutures on both corners. All the patients received removal of two side releasable sutures concomitant with pulling the accordion suture, without any complications. The average traction time was 3.5±0wk postoperatively. The mean postoperative IOP was 11.37±2.72 mm Hg. No suture related complications were observed. This technique can be the suture of choice for filtering glaucoma surgery in experienced hands by its easy learning curve for precisely indicated patients.

Analysis of causative factors of high intraocular pressure after vitrectomy combined with silicon oil tamponade

Objective To analyse the causative factors of high intraocular pressure (IOP) after vitrectomy combined with silicon oil tamponade. Methods The data of 78 eyes of 78 patients who underwent pars plana vitrectomy combined with silicon oil tamponade between July 2013 and January 2015 in our hospital were retrospectively analyzed. The follow-up time was 12 months, and the high IOP was defined as IOP≥24 mmHg(1 mmHg=0.133 kPa). Statistical methods were used to analyze the possible causative factors of postoperative high IOP. Results Postoperative high IOP occurred in 27 cases (34.62%). There was no significant difference in age, gender, diabetes mellitus, previous intraocular surgery or ocular trauma between high IOP group and normal IOP group(P> 0.05). Logistic multivariate regression analysis showed that the OR values of high myopia, combined with lentectomy, emulsification of silicone oil, uveitis and time of silicon oil tamponade over 6 months were 10.569(1.519-73.555), 15.325(1.470-159.731), 17.828(2.268-140.171), 25.783(2.579-257.759), 4.327(1.436-13.035), respectively. They were independent risk factors of high IOP after surgery. Conclusion The risk factors of high IOP after vitrectomy combined with silicon oil tamponade from high to low are uveitis, emulsification of silicone oil, combining with lentectomy, high myopia and time of silicon oil tamponade for too long. Outcomes of implantable collamer lens V4 and V4c for correction of high myopia - a case series.

To evaluate the visual outcomes and intraocular pressure changes after Visian Implantable Collamer Lens (ICL) implantation V4b and V4c (with centraflow technology) for correction of myopia. A prospective, consecutive, comparative interventional case series of V4b and V4c ICL implantation done for correction of high myopia (>-6 diopter D) in patients unsuitable for laser vision correction. The outcome measures that were evaluated included preoperative and postoperative uncorrected distant visual acuity (UDVA), best spectacle corrected distant visual acuity (CDVA), endothelial cell count (ECC), presence of lens opacification, intraocular pressure (IOP) and ICL vaulting. A follow-up of upto 1 year was done. A questionnaire was given at the end of follow-up period. A total of 30 eyes (24.56±4.8 years) underwent V4b ICL implantation (10 non-toric, 20 toric ICL-TICL) with intraoperative peripheral iridectomy (PI) and 34 eyes (26.13±3.8 years) had implantation of V4c ICL with centraflow (12 non-toric, 22 TICL). The mean preoperative manifest spherical equivalent (MSE) was 8.98±2.8 D and 9.24±2.4 D in the V4b and V4c groups respectively which reduced to postoperative values of -0.28±1.3 D and -0.19±1.18 D respectively. The mean preoperative astigmatism was -1.8±1.2 diopter cylinder (Dcyl) and -1.9±1.6 Dcyl which respectively reduced to -0.8±0.8 Dcyl and -0.9±0.3 Dcyl. At the end of 1 year follow up, mean ECC loss was 7.6% and 7.1%, mean vault was 583.12±231.12 μ and 602±241.24μ respectively in the V4b and V4c groups. Anterior subcapsular opacities were present in 6.66% and 2.94% of eyes with V4b and V4c groups respectively. Two eyes from both V4b (10%) and V4c (8.33%) had rotation of more than 30 degrees and required re-rotation surgery done successfully. Two eyes (6.66%) with V4b ICL implantation had high postoperative IOP (>35 mm Hg) and required Nd:Yag laser iridotomy later done with successful control of IOP. The safety indices were 1.12 and 1.15 and efficacy indices were 1.5 and 1.6 in the V4b and V4c groups respectively at the end of 1 year. The most common visual complaint was glare and haloes in 24% and 27% in the two groups respectively. However, they were not annoying enough to cause visual disability. Both V4b and V4c Visian ICL implantations are comparable in terms of visual outcome and safety profile for correction of high myopia. However, V4c ICL offers these advantages without the requirement of an additional PI.

Risk Factors of Disease Progression After Cataract Surgery in Chronic Angle-closure Glaucoma Patients.

To evaluate clinical risk factors of disease progression after cataract surgery using phacoemulsification with posterior chamber intraocular lens implantation, in eyes with chronic angle-closure glaucoma (CACG) and coexisting cataract. Retrospective study. The data of 56 eyes of 45 CACG patients who had undergone uncomplicated phacoemulsification with posterior chamber intraocular lens implantation were retrospectively analyzed. Disease progression was defined as glaucomatous optic nerve change or visual field (VF) deterioration according to the European Glaucoma Society guideline. Correlations between VF progression and various preoperative and postoperative factors were determined by χ and independent t tests. Linear regression analysis [(odds ratio (OR)] was used to determine predictive risk factors for disease progression using univariate and multivariate analyses. The mean postoperative follow-up period was 45.13 ± 17.54 (24 to 84) months. Fourteen eyes (25%) with cataracts diagnosed with CACG progressed after phacoemulsification, but the remaining 42 eyes (75%) did not. According to univariate analysis, disease progression was significantly associated with older age, more number of preoperative/postoperative antiglaucoma drugs, higher scores of preoperative pattern standard deviation, and lower scores of preoperative and postoperative visual field index (VFI) (P<0.05). Using multivariate analysis, a lower score of preoperative VFI (OR: 0.86, P=0.044) and lower postoperative intraocular pressure (IOP) reduction, which was not sustained below 20% less than the preoperative mean IOP, were significantly correlated with disease progression after cataract surgery (OR: 8.44, P=0.048). CACG patients with low preoperative VFI and high postoperative IOP are at risk for disease progression even after uncomplicated cataract surgery.

Implantable collamer lens V4b and V4c for correction of high myopia

PurposeTo evaluate the visual outcome and intraocular pressure changes after Visian Implantable Collamer Lens (ICL) implantation V4b and V4c (with central hole) for correction of high myopia. MethodsA prospective, consecutive, interventional comparative case series of V4b and V4c ICL implantation was done in high myopic patients who were unsuitable for laser vision. The main outcome measures studied were uncorrected and corrected distant visual acuity (UDVA, CDVA), ICL vault, intraocular pressure (IOP), endothelial cell count (ECC), and development of subcapsular lens opacities. The patients were evaluated at postoperative 1,3,6, and 9 months. ResultsA total of 62 eyes of 32 patients (24.56 ± 4.8 years) underwent V4b ICL implantation (21 non-toric, 41 toric ICL-TICL) with intraoperative peripheral iridectomy (PI), and 10 eyes of 5 patients (26.13 ± 3.8 years) had implantation of V4c ICL (4 non-toric, 6 TICL). At the end of 9 months follow-up, mean ECC loss was 6.4% and 6.1%, mean vault was 573.13 ± 241.13 μ, and 612 ± 251.14 μ, respectively, in the V4b and V4c groups. Anterior subcapsular opacities were present in 6.9% and 3.14% of eyes with V4b and V4c groups, respectively. Four eyes from V4b (9.75%) and 1 eye from V4c (16.66%) had rotation of more than 30° and required realignment surgery, which was done successfully. Two eyes (3.22%) with V4b ICL implantation had high postoperative IOP (>35 mm Hg) due to blocked PI and required Nd:Yag laser iridotomy, which was done with successful control of IOP. The safety indices were 1.11 and 1.14, and efficacy indices were 1.4 and 1.5 in the V4b and V4c groups, respectively, at the end of 9 months. ConclusionICL implantation is a safe and effective surgery for correction of high myopia. Implantation of ICL with a central hole showed negligible postoperative IOP fluctuations without a peripheral iridectomy.

Application of Triamcinolone acetonide in vitreoretinal surgery

Objective To observe the clinical application of triamcinolone acetonide in vitreoretinal surgery. Methods One hundred eyes of 100 patients in whom the posterior vitreous detachment was not disagnosed before and during the vitreoretinal surgery were chosed, and triamcinolone acetonide suspension 2 mg was injected to stain the vitreous after resection of most vitreous and then the posterior cortex of vitreous was completely removed. The vitreous residue and postoperative intraocular pressure was observed. Results Triamcinolone acetonide was injected in all patients and the residual vitreous can be fully recognized. The posterior vitreous detachment was completed, and the vitreous was thoroughly cleared. The postoperative high intraocular pressure happened in 3 cases, and the incidence rate was 6%. After symptomatic treatment, the high intraocular pressure was controlled and recovered. After following up for 3~4 months, no cataract or recurrent retinal detachment occurred. Conclusion Triamcinolone acetonide has high application value on vitreoretinal surgery, which is helpful to fully understand, identify and remove the vitreous, improve the vitreous clearance rate, reduce the incidence of postoperative complications and the recurrence rate of retinal detachment. Clinical observation of the treatment of cataract surgery after glaucoma filtering surgery

Objective To observe the clinical efficacy of cataract surgery after glaucoma filtration.Methods Phacoemulsification and intraocular lens implantation through clear corneal incision were performed on 43 eyes of 32 cases after glaucoma filtering surgery.Results The postoperative visual acuity was improved in all cases compared with preoperative varying degrees.The visual acuity of 0.3 ～ 0.5 was 10 eyes (23.26％),0.6 to 0.8 was 15 eyes (34.88％),≥ 1.0 was 8 eyes (18.60％).The postoperative complications were intraoperative posterior capsular rupture in 1 eye (2.32％),the cornea with above moderate edema 8 eyes (18.60％),postoperative hyphema 2eyes (4.65％),postoperative high intraocular pressure 6 eyes (13.95％),postoperative iritis 3 eyes (6.98％) and optic atrophy 6 eyes(13.95％).No serious complications occurred.Conclusion The advantages of the surgery were simple,small damage,faster visual recovery and the good effect reducing the intraocular pressure.
