Can Asymmetry in Total Knee Arthroplasty Design Lead to More Normal-Like Postoperative Kinematics? A Multi-Implant Evaluation

BackgroundFew studies have evaluated the effects of symmetrical versus asymmetrical implant designs, more specifically the femoral condyles, trochlear groove, joint line, and bearing surfaces. The objective of this study was to investigate multiple posterior cruciate-retaining (PCR) total knee arthroplasty (TKA) designs influencing factors related to TKA asymmetry, and to investigate whether asymmetry can improve postoperative knee kinematics. MethodsIn vivo tibio-femoral kinematics for 99 subjects was evaluated in this retrospective study. Overall, 10 subjects had a nonimplanted, normal knee, and 89 subjects had 1 of 3 PCR TKAs with varying degrees of asymmetry within their femoral and tibial components (PCR #1 = 30, PCR #2 = 29, PCR #3 = 30). All TKAs were implanted by the same surgeon and were analyzed using fluoroscopy during a deep knee bend. ResultsAt full extension, all 3 PCR TKAs experienced a more posteriorized position of the femoral condyles compared to the normal knee, with the 2 asymmetrical PCR TKAs experiencing more anteriorization compared to the third, symmetrical PCR TKA. Both the normal knee and the PCR TKA with greatest amount of asymmetry experienced statistically more posterior femoral rollback of the lateral condyle than the other 2 PCR TKAs. The PCR TKA with greater asymmetry also experienced statistically greater range of motion than the other 2 PCR TKAs. ConclusionsWith increasing flexion, the design with the most asymmetry also experienced the most posterior femoral rollback, axial rotation, and greatest range of motion. The results in this study seem to suggest that the inclusion of asymmetry in a TKA could be beneficial for achieving more normal-like kinematics and greater weight-bearing knee flexion.

Increased risk of aseptic loosening for posterior stabilized compared with posterior cruciate-retaining uncemented total knee replacements: a cohort study of 13,667 knees from the Dutch Arthroplasty Registry

Background and purpose: While registry studies have suggested a higher risk of revision for posterior-stabilized (PS) compared with posterior cruciate-retaining (CR) total knee replacements (TKR) using cement, it is unknown whether this is also the case for uncemented TKR. We aimed to compare the revision rates of PS and CR designs in patients receiving primary uncemented TKR.Patients and methods: Data from the Dutch arthroplasty register (LROI) was analyzed, comprising 12,226 uncemented primary CR TKRs and 750 uncemented PS TKRs registered between 2007 and 2022. Competing risk and multivariable Cox regression analyses were used to compare revision rates, risks of revision, and reasons for revision between groups. Sensitivity analyses were performed to analyze the risk, concerning the 5 most commonly used implants and performing hospitals for each group.Results: Uncemented PS TKRs had higher 10-year revision rates for any reason and aseptic loosening (6.5%, 95% confidence interval [CI] 4.6–9.2 and 3.9%, CI 2.6–6.7) compared with uncemented CR TKRs (4.2%, CI 3.8–4.7 and 1.4%, CI 1.2–1.7). PS TKRs were 1.4 and 2.5 times more likely to be revised for any reason and aseptic loosening, respectively. These results remained consistent after adjustment for age, sex, BMI, previous surgeries, bearing mobility, and surface modification, with sensitivity analyses.Conclusion: We found that uncemented PS implants have a higher rate of revision than uncemented CR implants, mainly due to a higher risk of aseptic loosening.

Cruciate-Retaining Total Knee Arthroplasty: Current Concepts Review.

Posterior cruciate-retaining (CR) total knee arthroplasty for osteoarthritis of the knee is a popular implant choice. At present, there is no consensus on whether sacrifice or retention of the posterior cruciate ligament (PCL) offers superior outcomes. This review explores the current literature available on CR total knee arthroplasty (TKA). PubMed was searched by keyword to find relevant articles for inclusion. Additional sources came from article references and joint registry reports. CR design knees have distinct kinematic gait patterns from posterior-stabilizing (PS) knees and exhibit paradoxical anterior femoral movement with less femoral rollback. While CR implants offer less flexion than PS designs, the difference is not clinically detectable as clinical scores are similar in the short and long term. CR implants have better long-term survival compared to PS knees, likely due to lower risk of aseptic loosening. CR total knee arthroplasties also have shorter operating times and lower risk of peri-prosthetic fractures. Because the CR implant is unconstrained, there may be an increased risk of instability compared to PS designs, but the literature is mixed. Overall, the current literature supports the continued use of CR TKAs due to their lower risk of complications, durability, and demonstrated equivalence in function to posterior-substituting models.

The effect of posterior tibial slope on anteroposterior stability in posterior cruciate retaining total knee arthroplasty

BackgroundIt has been suggested that the posterior tibial slope (PTS) plays an important role in increasing the anteroposterior stability following total knee arthroplasty. Although the relationship between the PTS and the flexion range has been investigated many times, studies on the relationship between PTS and anterior-posterior stability are limited. The primary aim of this study was to investigate the relationship and effects of PTS on anteroposterior stability in posterior cruciate retainer total knee arthroplasty.Methods154 primary TKAs were identified retrospectively to analyze the any association between PTS and anteroposterior laxity following posterior cruciate-retaining total knee arthroplasty in the overall study populations. Anteroposterior displacement was measured at the final follow-up based on the following two procedures: KT-1000 arthrometer and sagittal drawer radiographic images. In addition, the relationship between PTS and functional scores-ROM was examined.ResultsThere was no correlation between patients’ posterior tibial slope and postoperative VAS (r: -0.060, p:0.544), WOMAC (r:0.037, p:0.709), KSS (r: -0.073, p:0.455). In addition, there was no significant correlation between postoperative knee ROM and postoperative PTS (r:0.159, p:0.106). Moreover, no correlation was found between KT-1000 arthrometer and 20 degrees AP translation with PTS. There was a negative correlation between PTS and 70 degrees AP translation (r: -0.281, p:0.008).ConclusionsThis study aimed to clarify the association between instability and AP laxity in flexion of implanted knees, and to determine what degree of AP laxity results of instability. A fundamental finding of this study was that; the optimum TS angle to increase anterior-posterior stability after total knee arthroplasty is between ≥ 4 to < 6 degrees, we also proved that there is no relationship between stability and patient satisfaction.

Posterior cruciate-retaining total knee arthroplasty exhibits small kinematic changes in the first postoperative year.

Fluoroscopic knee kinematics have historically been quantified at least 1year after total knee arthroplasty (TKA). The purpose of this study was to longitudinally assess knee kinematics at 6-12weeks, 6months, and 1year after TKA to determine if earlier evaluation may be justified. Twenty-one patients participated after undergoing TKA with a posterior cruciate ligament-retaining fixed-bearing prosthesis. Fluoroscopic examinations of lunge, kneel, and step-up activities were performed at 12 ± 4weeks (V1), 7 ± 2months (V2), and 13 ± 2months (V3) postoperatively. Images were analyzed using a three-dimensional to two-dimensional image registration technique. Maximum flexion poses for lunging and kneeling were compared between visits with repeated-measures statistical tests. For the step-up activity, mixed-effects linear models were constructed for condylar anteroposterior (AP) contact points and tibial internal rotation throughout flexion. Estimated marginal means of fitted values were plotted with 95% confidence intervals and used to compare mean kinematics between visits. There were no significant changes in maximum lunging flexion over time (p = 0.405), though significant increases in maximum kneeling flexion were observed between V1 (106 ± 8°) and V2 (110 ± 9°) (p = 0.006), and V1 and V3 (113 ± 9°) (p = 0.0003). While statistical differences were calculated for lunging medial condyle AP translation and kneeling tibial internal rotation, absolute differences in condylar AP contact locations were less than ~ 2mm between all visits during both movements. For the step-up activity, tibial internal rotation increased with flexion, and there were pair-wise significant differences at all flexion angles between V1-V2 (p < 0.001) and V1-V3 (p < 0.001). Anterior medial condylar translation was observed with flexion, with pair-wise significant differences present for V1-V3 (p = 0.005) and V2-V3 (p < 0.001). The lateral condyle exhibited initial posterior translation followed by anterior translation with increasing flexion, with pair-wise differences between all visits (p < 0.005 for all comparisons). Though statistical differences were observed between visits for all activities, variations in estimated mean condylar positions were within ~ 2mm from ~ 12weeks to 1year. Considering measurement error averages approximately 1mm for sagittal plane translations, these results indicate that knee kinematics during kneel, lunge, and step-up activities may be sustained from as early as 12weeks after TKA.

The compartmental approach to revision of partial knee arthroplasty results in nearer-normal gait and improved patient reported outcomes compared to total knee arthroplasty

PurposeThis study investigated the gait and patient reported outcome measures of subjects converted from a partial knee arthroplasty to combined partial knee arthroplasty, using a compartmental approach. Healthy subjects and primary total knee arthroplasty patients were used as control groups.MethodsTwenty-three patients converted from partial to combined partial knee arthroplasty were measured on the instrumented treadmill at top walking speeds, using standard gait metrics. Data were compared to healthy controls (n = 22) and primary posterior cruciate-retaining total knee arthroplasty subjects (n = 23) where surgery were performed for one or two-compartment osteoarthritis. Groups were matched for age, sex and body mass index. At the time of gait analysis, combined partial knee arthroplasty subjects were median 17 months post-revision surgery (range 4–81 months) while the total knee arthroplasty group was median 16 months post-surgery (range 6–150 months). Oxford Knee Scores and EuroQol-5D 5L scores were recorded at the time of treadmill assessment, and results analysed by question and domain.ResultsSubjects revised from partial to combined partial knee arthroplasty walked 16% faster than total knee arthroplasty (mean top walking speed 6.4 ± 0.8 km/h, vs. 5.5 ± 0.7 km/h p = 0.003), demonstrating nearer-normal weight-acceptance rate (p < 0.001), maximum weight-acceptance force (p < 0.006), mid-stance force (p < 0.03), contact time (p < 0.02), double support time (p < 0.009), step length (p = 0.003) and stride length (p = 0.051) compared to primary total knee arthroplasty. Combined partial knee arthroplasty subjects had a median Oxford Knee Score of 43 (interquartile range 39–47) vs. 38 (interquartile range 32–41, p < 0.02) and reported a median EQ-5D 0.94 (interquartile range 0.87–1.0) vs. 0.84 (interquartile range 0.80–0.89, p = 0.006).ConclusionThis study finds that a compartmental approach to native compartment degeneration following partial knee arthroplasty results in nearer-normal gait and improved patient satisfaction compared to total knee arthroplasty.Level of evidenceIII.

Leveraging subject-specific musculoskeletal modeling to assess effect of anterior cruciate ligament retaining total knee arthroplasty during walking gait.

Bi-cruciate retaining total knee arthroplasty has several potential advantages including improved anteroposterior knee stability compared to contemporary posterior cruciate-retaining total knee arthroplasty. However, few studies have explored whether there is significant differences of knee biomechanics following bi-cruciate retaining total knee arthroplasty compared to posterior cruciate-retaining total knee arthroplasty. In the present study, subject-specific lower extremity musculoskeletal multi-body dynamics models for bi-cruciate retaining, bi-cruciate retaining without anterior cruciate ligament, and posterior cruciate-retaining total knee arthroplasty were developed based on the musculoskeletal modeling framework using force-dependent kinematics method and validated against in vivo telemetric data. The experiment data of two subjects who underwent total knee arthroplasty were obtained for the SimTK "Grand Challenge Competition" repository, and integrated into the musculoskeletal model. Five walking gait trials for each subject were used as partial inputs for the model to predict the knee biomechanics for bi-cruciate retaining, bi-cruciate retaining without anterior cruciate ligament, and posterior cruciate-retaining total knee arthroplasty. The results revealed significantly greater range of anterior/posterior tibiofemoral translation, and significantly more posterior tibial location during the early phase of gait and more anterior tibial location during the late phase of gait were found in bi-cruciate retaining total knee arthroplasty without anterior cruciate ligament when compared to the bi-cruciate retaining total knee arthroplasty. No significant differences in tibiofemoral contact forces, rotations, translations, and ligament forces between bi-cruciate retaining and posterior cruciate-retaining total knee arthroplasty during normal walking gait, albeit slight differences in range of tibiofemoral internal/external rotation and anterior/posterior translation were observed. The present study revealed that anterior cruciate ligament retention has a positive effect on restoring normal knee kinematics in bi-cruciate retaining total knee arthroplasty. Preservation of anterior cruciate ligament in total knee arthroplasty and knee implant designs interplay each other and both contribute to restoring normal knee kinematics in different types of total knee arthroplasty. Further evaluation of more demanding activities and subject data from patients with bi-cruciate retaining and posterior cruciate-retaining total knee arthroplasty via musculoskeletal modeling may better highlight the role of the anterior cruciate ligament and its stabilizing influence.

Comparative study of cruciate retaining vs posterior stabilized prosthesis in total knee replacement

Introduction: Although being debated for many years, the superiority of Posterior Cruciate-Retaining (CR) Total knee Arthroplasty (TKA) and Posterior Cruciate - Stabilizing (PS) TKA remains controversial. The primary objective of this study was to compare functional outcome in patients who underwent primary total knee arthroplasty surgery using posterior stabilized and cruciate retaining implants. Material and method: Sixty diseased osteoarthritis knees in 42 patients were included in the study, with 30 cases (Knees) operated with CR prosthesis and 30 cases (Knees) with PS Prosthesis. The two groups were then compared using disease specific functional scores; 2011 knee society score. Pain, range of motion, any flexion, varus, valgus deformity, instability at knee joint was assessed preroperatively and postoperatively. Functional outcome (e.g. walking, climbing stairs) was assessed. Result: Postperative objective knee score and range of motion was significantly better in PS group than CR group, with significant p value. No significant difference in functional outcome between both the groups was seen. Conclusion: No significant difference in the functional outcome is seen between both the groups. Genesis 1 posterior cruciate-retaining total knee arthroplasty with asymmetric tibial tray: An 18-to-26-year long-term clinical outcome study

BackgroundTo present the long-term clinical and radiological outcomes of the Genesis I posterior cruciate-retaining total knee arthroplasty (TKA) (Smith & Nephew Orthopaedics, Memphis, TN, USA) (one of the first designed with an asymmetric tibial tray). MethodsProspectively collected data from 117 cemented TKAs performed on 95 patients were evaluated using this design, with a mean follow-up of 21.5 years (range, 18–26). Failures, complication rates, and clinical (both subjective and objective) and radiological outcomes were assessed for all patients. ResultsSeven failures occurred (wear and structural failure of the polyethylene insert) and six revision surgeries were performed at a mean of 10 years (range, three to 14) from the index operation. No revision of either the femoral or tibial components for aseptic loosening was performed. Survivorship analysis showed a cumulative success rate of 93.53% (95% CI, 92.75–94.32%) at 20 and 25 years, with revision for any reason as an end point. All patients showed a statistically significant improvement (P = 0.001) in the Knee Society and Oxford knee rating scores. ConclusionsThis study demonstrated satisfactory long-term clinical outcomes for this TKA design, with revisions being related to polyethylene wear structural failure.

A comparative study of flat surface design and medial pivot design in posterior cruciate-retaining total knee arthroplasty: a matched pair cohort study of two years

BackgroundComponent design is one of the contributory factors affecting the postoperative flexion angle. The purpose of this study was to compare short-term outcomes of flat surface and medial pivot designs in posterior cruciate-retaining (CR) total knee arthroplasty (TKA).MethodsA retrospective, case-control, and observational cohort study consisted of matched-pairs of the flat surface design (Hi-Tech Knee II) and the medial pivot design (FINE Knee) in CR-TKA with a two-year follow-up period.ResultsHi-Tech Knee II and FINE knee groups each included 7 males and 38 females. Surgical time was significantly shorter in the FINE Knee group than in the Hi-Tech Knee II group (104.8 min versus 154.9 min, p = 0.001). Estimated total blood loss was significantly lower in the FINE Knee group than in the Hi-Tech Knee II group (654 ml versus 1158 ml, p = 0.001). The postoperative flexion angle was significantly better in the FINE Knee group than in the Hi-Tech Knee II group (119.3 degrees versus 112.5 degrees), and was positively correlated with the preoperative flexion angle. Postoperative Knee Society scores were significantly better in the FINE Knee group than in the Hi-Tech Knee II group (93.0 points versus 85.0 points, p = 0.001), especially for postoperative pain relief (46.0 points versus 39.0 points out of 50, p = 0.001). Complications were not observed in either group over a two-year follow-up period.ConclusionThe short-term outcome of the medial pivot design used in CR-TKA was more favorable than the flat surface design, especially for surgical time, estimated total blood loss, postoperative flexion angle, and knee pain.

Choice of total knee arthroplasty: posterior cruciate ligament preserved or not

Total knee arthroplasty is an effective method for the treatment of end-stage knee osteoarthrosis, which can effectively relieve joint pain and reconstruct the integrity of the joint. Whether the posterior cruciate ligament should be preserved during surgery or not, which is still in dispute. In recent years, posterior cruciate-retaining and substituting total knee prostheses are both applied in clinical practice. Both domestic and international studies have shown that there are no significant difference in patient satisfaction, knee flexion, survival rate of the prosthesis and the main clinical manifestations between two prostheses. However, posterior cruciate-retaining total knee prosthesis is more consistent with the normal physiology and biomechanics of the human body. The gait is more balanced and proprioceptive when walking up and down the stairs, but when the joints are buckling, the femur is abnormal to move back to the tibia, resulting in abnormal motion. While posterior cruciate-substituting total knee prosthesis can correct severe deformity of the knee, and keep better balance between flexion and extension of the knee joint, but there is a potential complication of patellar clunk syndrome. Therefore, under the same conditions, the younger patients may prefer to chose posterior cruciate-retaining total knee prosthesis, while elder patients may prefer to chose posterior cruciate-substituting total knee prosthesis. This paper reviews the function of posterior cruciate ligament, as well as the advantages and disadvantages of two prostheses, so as to provide some references for clinic.

Effect of tibial slope changes on femorotibial contact kinematics after cruciate-retaining total knee arthroplasty.

The present study was undertaken to evaluate the effect of tibial slope (TS) changes on the femorotibial articular contact kinematics in subjects undergoing posterior cruciate-retaining total knee arthroplasty (CRTKA). Eighteen knees in nine patients with medial osteoarthritis who underwent CRTKA using the same size prosthesis were analysed preoperatively and 2years after TKA. TS changes were calculated on lateral radiographs taken before and after TKA. Knees were classified into two groups according to the change in TS obtained by subtracting the post-operative value from the preoperative value: group 1 (>3°) and group 2 (<3°). The femorotibial articular contact kinematics of knees during weight-bearing flexion were compared between the two groups by two-dimensional/three-dimensional registration. Group 1 showed a continuous posterior translation of the medial femoral condyle during the process of knee flexion, whereas in group 2 the medial femoral condyle experienced paradoxical anterior motion from 20° to 90° of knee flexion. The lateral femoral condyle continuously moved posteriorly in both groups. A greater reduction in TS after TKA compared with preoperative TS reduces paradoxical medial femoral condylar movement. This may contribute to improved patient satisfaction after CR TKA. III.

Posterior Cruciate Ligament Retention versus Posterior Stabilization for Total Knee Arthroplasty: A Meta-Analysis.

IntroductionAlthough being debated for many years, the superiority of posterior cruciate-retaining (CR) total knee arthroplasty (TKA) and posterior-stabilized (PS) TKA remains controversial. We compare the knee scores, post-operative knee range of motion (ROM), radiological outcomes about knee kinematic and complications between CR TKA and PS TKA.MethodsLiterature published up to August 2015 was searched in PubMed, Embase and Cochrane databases, and meta-analysis was performed using the software, Review Manager version 5.3.ResultsTotally 14 random control trials (RCTs) on this topic were included for the analysis, which showed that PS and CR TKA had no significant difference in Knee Society knee Score (KSS), pain score (KSPS), Hospital for Special Surgery score (HSS), kinematic characteristics including postoperative component alignment, tibial posterior slope and joint line, and complication rate. However, PS TKA is superior to CR TKA regarding post-operative knee range of motion (ROM) [Random Effect model (RE), Mean Difference (MD) = -7.07, 95% Confidential Interval (CI) -10.50 to -3.65, p<0.0001], improvement of ROM (Fixed Effect model (FE), MD = -5.66, 95% CI -10.79 to -0.53, p = 0.03) and femoral-tibial angle [FE, MD = 0.85, 95% CI 0.46 to 1.25, p<0.0001].ConclusionsThere are no clinically relevant differences between CR and PS TKA in terms of clinical, functional, radiological outcome, and complications, while PS TKA is superior to CR TKA in respects of ROM, while whether this superiority matters or not in clinical practice still needs further investigation and longer follow-up.

Effect of Posterior Tibial Slope on Flexion and Anterior-Posterior Tibial Translation in Posterior Cruciate-Retaining Total Knee Arthroplasty

Reduced posterior tibial slope (PTS) and posterior tibiofemoral translation (PTFT) in posterior cruciate-retaining (PCR) total knee arthroplasty (TKA) may result in suboptimal flexion. We evaluated the relationship between PTS, PTFT, and total knee flexion after PCR TKA in a cadaveric model. We performed a balanced PCR TKA using 9 transfemoral cadaver specimens and changed postoperative PTS in 1° increments. We measured maximal flexion and relative PTFT at maximal flexion. We determined significant changes in flexion and PTFT as a function of PTS. Findings showed an average increase in flexion of 2.3° and average PTFT increase of 1mm per degree of PTS increase when increasing PTS from 1° to 4° (P<.05). Small initial increases in PTS appear to significantly increase knee flexion and PTFT.

Control of paradoxical kinematics in posterior cruciate-retaining total knee arthroplasty by increasing posterior femoral offset.

Balancing the posterior cruciate ligament (PCL) with posterior cruciate-retaining total knee replacement (PCR-TKR) aims to restore femoral rollback. In practice, paradoxical roll forward persists. The purpose of this study is to propose a technique for optimizing PCL tension. Because PCL function starts above 60° of flexion, we hypothesize that PCL balancing requires flexion gap tightening by oversizing the femoral component and increasing posterior condylar offset (PCO). PCR-TKR was performed in 21 osteoarthritis patients with a gap-balancing technique. The femoral component was oversized if more than a 5-mm posterior drawer existed after tibial component implantation. Kinematics was recorded intra-operatively in two steps with dedicated navigation software (Praxim, La Tronche, Isère, France): antero-posterior (AP) displacements of condylo-tibial contact points were observed in native and implanted knees, with each knee serving as its own control. The absence of paradoxical displacements was verified once the final implants were inserted. Paradoxical medial condyle displacement (11 mm) persisted in a single case. On average, posterior displacement of the medial condyle decreased from 9 ± 9 to 1 ± 6 mm (p = 0.001) and that of the lateral condyle from 16 ± 14 to 6 ± 6 mm (p = 0.006). In the 0°-30° flexion interval, posterior displacement was 2 times less than before implantation for the medial condyle (p = 0.001), and 4 times less for the lateral condyle (p = 0.004). The course of the lateral condyle decreased from 2 ± 3 to 0 ± 4 mm in the 90°-120° flexion interval (p = 0.046). Six-month flexion was 124° ± 17°. Femoral component oversizing allows us to control paradoxical forward displacements in 95 % of cases. When balancing PCR prostheses, AP laxity should be taken into account. Increasing PCO appears to be a reliable technique for adjusting PCL balance. Thus, it may optimize extensor mechanism action and, subsequently, the functional results of PCR-TKR. 14-Year Median Follow-Up Using the Press-Fit Condylar Sigma Design for Total Knee Arthroplasty

Median 14-year follow-up (mean 11.8years) of a cemented primary posterior cruciate-retaining total knee arthroplasty (TKA) utilizing the Press-Fit Condylar (PFC) Sigma design was evaluated in 77 patients (79 TKA). Follow-up assessment included implant survivorship, x-rays, Knee Society rating system, and clinical evaluation. Radiographic analysis demonstrated minor non-progressive osteolysis in 40% (10/25) knees. Two revisions, one for instability at 4years and one for polyethylene wear at 10years were performed. Survivorship of the PFC Sigma knee implant was 97% using revision for any reason and 100% using aseptic loosening as endpoints. The PFC Sigma had excellent survivorship at 14years, the longest clinical follow-up reported.

Significant effect of the posterior tibial slope and medial/lateral ligament balance on knee flexion in total knee arthroplasty

The intra-operative femorotibial joint gap and ligament balance, the predictors affecting these gaps and their balances, as well as the postoperative knee flexion, were examined. These factors were assessed radiographically after a posterior cruciate-retaining total knee arthroplasty (TKA). The posterior condylar offset and posterior tibial slope have been reported as the most important intra-operative factors affecting cruciate-retaining-type TKAs. The joint gap and balance have not been investigated in assessments of the posterior condylar offset and the posterior tibial slope. The femorotibial gap and medial/lateral ligament balance were measured with an offset-type tensor. The femorotibial gaps were measured at 0°, 45°, 90° and 135° of knee flexion, and various gap changes were calculated at 0°-90° and 0°-135°. Cruciate-retaining-type arthroplasties were performed in 98 knees with varus osteoarthritis. The 0°-90° femorotibial gap change was strongly affected by the posterior condylar offset value (postoperative posterior condylar offset subtracted by the preoperative posterior condylar offset). The 0°-135° femorotibial gap change was significantly correlated with the posterior tibial slope and the 135° medial/lateral ligament balance. The postoperative flexion angle was positively correlated with the preoperative flexion angle, γ angle and the posterior tibial slope. Multiple-regression analysis demonstrated that the preoperative flexion angle, γ angle, posterior tibial slope and 90° medial/lateral ligament balance were significant independent factors for the postoperative knee flexion angle. The flexion angle change (postoperative flexion angle subtracted by the preoperative flexion angle) was also strongly correlated with the preoperative flexion angle, posterior tibial slope and 90° medial/lateral ligament balance. The postoperative flexion angle is affected by multiple factors, especially in cruciate-retaining-type TKAs. However, it is important to pay attention not only to the posterior tibial slope, but also to the flexion medial/lateral ligament balance during surgery. A cruciate-retaining-type TKA has the potential to achieve both stability and a wide range of motion and to improve the patients' activities of daily living.

Increased Long-Term Survival of Posterior Cruciate-Retaining Versus Posterior Cruciate-Stabilizing Total Knee Replacements

Considerable debate remains regarding the use of posterior cruciate-retaining or posterior cruciate-stabilizing designs for total knee arthroplasty. Multiple studies have investigated kinematic, radiographic, and clinical outcomes of both. Nevertheless, long-term survivorship analyses directly comparing the two designs have not been performed, to our knowledge. Our goal was to analyze the fifteen-year survival of posterior cruciate-retaining and posterior cruciate-stabilizing total knee replacements at our institution. A retrospective review identified 8117 total knee arthroplasties (5389 posterior cruciate-retaining and 2728 posterior cruciate-stabilizing) that had been performed from 1988 to 1998. This range was chosen because both designs were used in high volumes at our institution during this period. Patients were followed via our total joint registry at one, two, and five years after the arthroplasty and every five years thereafter. Aseptic revision surgery was the primary end point of our analysis. Implant survival was estimated with Kaplan-Meier curves. Survival at fifteen years was 90% for posterior cruciate-retaining total knee replacements, compared with 77% for posterior cruciate-stabilizing total knee replacements (p < 0.001). In knees with preoperative deformity, the fifteen-year survival was 90% for posterior cruciate-retaining total knee replacements, compared with 75% for posterior cruciate-stabilizing total knee replacements (p < 0.04). Likewise, in knees without preoperative deformity, the fifteen-year survival was 88% for posterior cruciate-retaining total knee replacements, compared with 78% for posterior cruciate-stabilizing total knee replacements (p < 0.001). After adjustment for age, sex, preoperative diagnosis, and preoperative deformity, the risk of revision was significantly lower in knees with a posterior cruciate-retaining total knee replacement (p < 0.001; hazard ratio = 0.5; 95% confidence interval, 0.4 to 0.6). In evaluating the implants used at our institution for total knee arthroplasty during the study period, posterior cruciate-retaining prostheses had significantly improved survival in comparison with posterior cruciate-stabilizing prostheses at fifteen years. Furthermore, this significant difference remained when accounting for age, sex, diagnosis, and deformity.

Influence of intra-operative joint gaps on post-operative flexion angle in posterior cruciate-retaining total knee arthroplasty

The purpose of this study is to examine the joint component gap and its relationship with post-operative flexion angles in posterior cruciate-retaining (CR) total knee arthroplasty (TKA). In posterior-stabilized (PS) TKA, an inverse correlation between the joint component gap and the post-operative flexion angle was reported. However, the kinematics of the joint component gap has a different pattern in PS and CR TKA. It was hypothesised that CR TKA has a different correlation between the joint component gap and the post-operative flexion angle compared to PS TKA. The joint component gap was measured with an offset-type tensor. The joint component gaps were measured at 0°, 10°, 30°, 60°, 90° and 120° knee flexion angle and various values of the change in the joint component gap were calculated; 10°-0°, 30°-0°, 60°-0°, 90°-0° and 120°-0°. Multivariate regression analysis was used to determine the influencing of these parameters to post-operative knee flexion angle. The post-operative flexion angle was positively correlated with a joint component gap of 90° and 120° and the values of the change in the joint component gap of 90°-0° and 120°-0°. Multivariate regression analysis demonstrated pre-operative knee flexion angle, and the values of the change in the joint component gap of 90°-0° had a significant independent factor of post-operative knee flexion angle. Post-operative flexion angle is multi-factorial. However, it is important to avoid flexion component gap tightness as well as excessive flexion component gap looseness for acquisition of better flexion angle in CR TKA. IV.

Can High-Flexion Tibial Inserts Improve Range of Motion After Posterior Cruciate-Retaining Total Knee Arthroplasty?

Interest in high-flexion total knee arthroplasty (TKA) prostheses designed to provide better postoperative range of motion (ROM) is widespread. We sought to determine whether changes in surface geometry of the tibial polyethylene insert could improve postoperative ROM in a consecutive series of patients undergoing TKA with retention of the posterior cruciate ligament (PCL). Two cohorts with Smith & Nephew (Memphis, Tennessee) Posterior Cruciate-Retaining Genesis II total knee prostheses were compared, 79 knees (65 patients) using standard tibial inserts and 85 knees (72 patients) using high-flexion inserts. The standard insert has a slightly raised posterior lip, whereas the high-flexion insert is recessed downward at the posterior margin to facilitate femoral rollback in flexion and eliminate impingement of the femoral component on the back of the polyethylene during rollback. Mean ROM 1 year postoperatively was 112.0° in patients receiving the standard insert and 119.3° in patients receiving the high-flexion insert. Preoperative ROM was similar in both groups. Flexion improvement in the high-flexion group over the standard insert group was statistically significant (P<.001). Final Knee Society Scores did not differ amongst patients receiving the standard and high-flexion inserts. Our study demonstrates that improved postoperative flexion can be achieved without changing surgical technique, bony cuts, or metallic prosthetic parts. This is the first report that we are aware of that documents improvement in ROM after PCL-retaining TKA through the use of high-flexion inserts.
