Reducing CTV to PTV Margins with Daily Adaptive Radiotherapy in the Postoperative Treatment of Endometrial and Cervical Cancer

In the postoperative treatment of endometrial and cervical cancer, a PTV expansion of 1.5cm or greater would be generally recommended to ensure adequate coverage of CTV with no or minimal image guidance. Online adaptive radiotherapy (oART) has demonstrated to be feasible to reduce inter-fractional radiotherapy errors as it re-optimizes treatment plan every fraction. In this study, we hypothesized using daily cone-beam computed tomography (CBCT)-guided oART would reduce CTV to PTV margins in the postoperative treatment of endometrial and cervical cancer. Seven patients from a single-center with postoperative endometrial and cervical cancer treated with daily CBCT-guided oART were retrospectively reviewed. A total dose of 45Gy is prescribed in CTV-vaginal cuff (CTV-V) and CTV- regional lymph nodes (CTV-N) with daily fractions of 1.8Gy for five fractions per week. A total of 175 radiotherapy fractions of daily pre-treatment CBCTs and post-treatment CBCTs scans were uploaded to oART emulator for CTV-V and CTV-N contouring by a single observer. CTVpre to PTVpre margins of 5, 7, 10, 12, 15mm was used in all directions. Each post-treatment CBCT was rigidly registered to the pre-treatment CBCT with respect to bony anatomy. The CTVpost was projected onto the PTVpre and assess required planning margins to encompass the CTVpost. The average total treatment time (post-treatment CBCTs - pre-treatment CBCTs) was 23 min 14s (range: 18min 53s ∼ 28min 53s). For all fractions, the adapted plan was selected, and the volume of the PTV-V and PTV-N receiving 100% of the prescribed dose or more (V100%) was more than required 95%. A uniform three-dimensional CTV-Npre to PTV-Npre margin of 5mm could encompass CTV-Npost in all fractions (100%; 175/175f). A uniform three-dimensional CTV-Vpre to PTV-Vpre margin of 10mm could encompass CTV-Vpost in all fractions (100%; 175/175f) and 5mm could encompass 98% fractions (172/175f). In the fractions of 5mm margins that failed to encompass CTV-Vpost, the volume of CTV missed three times in anterior-posterior directions and once in lateral direction. Compared with scheduled plan, adapted plan with 5mm margins significantly reduced the mean dose to bladder (Dmean: 103.97 cGy vs 107.20 cGy, P<0.05) and rectum (Dmean: 117.82 cGy vs 123.67 cGy, P<0.05), and achieved better PTV-N (V100%: 97.8% vs 90.0%, P<0.05) and PTV-V (V100%:96.8% vs 85.4%, P<0.05) dose coverage in 175 fractions. In this study, 5mm CTV to PTV margins was adequate to encompass 100% fractions of CTV-N and 98% fractions of the CTV-V. Adapted plan with 5mm margins significantly reduced the dose to critical organ at risks while improving target coverage.

Evaluation of changes in the maxillary sinus of orthodontic patients with maxillary molar distalization using cone-beam computed tomography

This study aimed to evaluate the possible effects of maxillary molar distalization on maxillary sinus mucosa thickness and sinus volume in 3-dimensions. The study was conducted with 46 patients: 73.9% (n= 34) females and 26.1% (n= 12) males. The patients had a mean age of 27.89 ± 6.62 years, ranging from 18 to 45 years. The study included patients who were admitted to the Faculty of Dentistry at Van Yüzüncü Yıl University between 2010 and 2021 and underwent cone-beam computed tomography (CBCT) before and after orthodontic treatment for various reasons. The CBCT scans of patients were divided into 2 groups: the first group underwent orthodontic treatment and maxillary molar distalization with Class II elastic, whereas the second group consisted of patients who did not get orthodontic treatment (control). In CBCTs, sinus mucosa thickness was measured on the anterior, posterior, medial, lateral, superior, and inferior walls of the maxillary right and left sinuses. Three-dimensional volume calculations were made using CBCT images taken before and after the treatment of both orthodontic treatment patients and control patients. Statistical significance was set at P<0.05. The orthodontic treatment group had a mean distalization of 2.86 ± 0.27 mm. The mean distalization time was 8.01 ± 0.64 months. When the differences between the pretreatment and posttreatment CBCT scans were examined in terms of sinus mucosa thickness in the anterior, posterior, medial, lateral, and superior walls of the right and left sinuses, there was no statistically significant difference between treatment and control groups (P>0.05). The change in sinus mucosa thickness in the right and left sinus inferior wall from T1 to T2 was significantly higher in the control group than treatment group (P<0.05). In addition, the increase in maxillary sinus volume was statistically significantly higher in the treatment group than in the control group (P<0.05). We observed that maxillary molar distalization caused an increase in maxillary sinus volume. The change in sinus mucosa thickness in the right and left sinus inferior wall was lower in the maxillary molar distalization group.

Effect of Maxillary Expansion Treatment Protocols On Maxillary Sinus Volume, Pharyngeal Airway Volume, and Hyoid Bone Position: A Prospective, Cone Beam Computed Tomography (CBCT) Study

Aim: This study aimed to investigate the changes in maxillary sinus volume, pharyngeal airway volume, and hyoid bone position in patients treated with Rapid Maxillary Expansion (RME) and Alternate Rapid Maxillary Expansion and Constriction (AltRaMEC) – Facemask therapy and compare between the groups. Materials and Methods: The records of 15 patients between the age group of 8-14 years with Class I and Class III malocclusion were collected, and the patients were assigned into two groups. The first group comprised of 8patients (4 girls and 4 boys) who were treated with Rapid Maxillary Expansion. The second group comprised 7 patients (5 boys and 2 girls) treated with expansion devices using Alt-RaMEC protocol followed by maxillary protraction using a facemask. Pre and post-treatment CBCT scans were obtained and analyzed for changes in the pharyngeal airway, maxillary sinus volume, and hyoid bone position. The maxillary sinus volume was calculated in 3 dimensions by measuring the craniocaudal height, anteroposterior depth, and mediolateral width. Measurements were taken before and after the procedure to compare the groups and between the groups. The data were analyzed using Paired t-test and an Independent t-test. Results: In the RME group, a significant increase in the total maxillary sinus volume (mean difference = 520±576.57mm3), especially in the craniocaudal height, was seen, but no changes were noted in the pharyngeal airway and hyoid position. The AltRAMEC group showed a statistically significant increase in the maxillary sinus volume (anteroposteriorly) and nasopharyngeal volume with a p-value &lt;0.005. Comparing the maxillary sinus volume changes in RME and AltRAMEC groups showed a statistically significant increase in the volume of the maxillary sinuses in both groups, significantly higher in the RME group. Conclusion: RME and AltRAMEC protocol followed by facemask therapy caused a significant increase in the maxillary sinus volume, while no changes were noted in the nasopharynx, oropharynx, hypopharynx, and hyoid bone position. While comparing the treatment effects between the groups, RME showed a greater increase in maxillary sinus volume (p &lt; 0.05), specifically in the anteroposterior dimension. Since there are no changes in airway volume and hyoid position with the skeletal expansion protocols, there will be no difference in nasal resistance to airflow.

Optimized Adaptive Radiotherapy with Individualized Plan Library for Muscle-Invasive Bladder Cancer Using Internal Target Volume Generation.

Simple SummaryThe bladder is a mobile target and is subject to filling variation. This poses a considerable challenge for effective radiotherapy (RT) delivery. We applied an internal target volume to the plan library to resolve intra-fractional errors caused by bladder filling during treatment. Adaptive radiotherapy using ITV is easy to perform and a feasible treatment approach. In this study, image-guided RT-based adaptive RT showed good survival outcomes with a high local control rate.The bladder is subject to filling variation, which poses a challenge to radiotherapy (RT) delivery. We aimed to assess feasibility and clinical outcomes in patients with bladder cancer treated with adaptive RT (ART) using individualized plan libraries. We retrospectively analyzed 19 patients who underwent RT for muscle-invasive bladder cancer (MIBC) in 2015–2021. Four planning computed tomography (CT) scans were acquired at 15-min intervals, and a library of three intensity-modulated RT plans were generated using internal target volumes (ITVs). A post-treatment cone-beam CT (CBCT) scan was acquired daily to assess intra-fraction filling and coverage. All patients completed the treatment, with 408 post-treatment CBCT scans. The bladder was out of the planning target volume (PTV) range in 12 scans. The volumes of the evaluated PTV plans were significantly smaller than those of conventional PTV. The 1-year and 2-year overall survival rates were 88.2% and 63.7%, respectively. Of eight cases that experienced recurrence, only two developed MIBC. There were no grade 3 or higher RT-related adverse events. ART using plan libraries and ITVs demonstrated good survival outcomes with a high local control rate. Irradiated normal tissue volume and treatment margins may be reduced through this approach, potentially resulting in lower toxicity rates.

Cone-beam computed tomography assessment of maxillary anterior alveolar bone remodelling in extraction and non-extraction orthodontic cases using stable extra-alveolar reference.

To explore alveolar cortical positional change in response to tooth movement in extraction and non-extraction orthodontic cases, using cone-beam computed tomography (CBCT) and stable extra-alveolar references. The pre-treatment (T1) and post-treatment (T2) CBCT scans of 25 extraction (EXT) and matched 25 non-extraction (Non-EXT) orthodontic cases were imported into Dolphin Imaging 3D, and oriented uniformly. Sagittal and axial CBCT cross-sections were traced using customized software-generated guides. The displacement of teeth and alveolar bone cortices were automatically measured using the palatal plane (PP) and the line perpendicular to PP and passing Sella as reference. Intra- and inter-group differences between T1 and T2 were analysed. Subjects were also superimposed three-dimensionally using Geomagic Control X for qualitative analysis of cortical remodelling. The EXT group showed incisor retraction, while the Non-EXT group exhibited statistically significant incisor anterior tipping (P < .05). In EXT, both the labial and palatal cortices are resorbed. Non-EXT showed labial cortex anterior modelling, and statistically significant palatal cortex resorption (P < .05). In both groups, statistically significant decrease in total and palatal alveolar widths, increase in labial widths, and palatal dehiscence were observed. Comparatively, EXT showed significantly more incisal total and palatal width decrease and palatal vertical bone loss. Labial cortical remodelling was shown to follow anterior tooth movement, but the palatal cortical response to incisor retraction and labial cortical remodelling in general remained inconclusive. Narrowing of the alveolar housing and palatal dehiscence were observed regardless of extraction following orthodontic treatment.

Association between nonextraction clear aligner therapy and alveolar bone dehiscences and fenestrations in adults with mild-to-moderate crowding

This study aimed to assess the association between nonextraction clear aligner therapy (CAT) and the presence of alveolar bone dehiscences (ABDs) and fenestrations (ABFs) in adults with mild-to-moderate crowding. Cone-beam computed tomography images from 29 adults were obtained before and immediately after nonextraction CAT. Total root lengths were evaluated in axial and cross-sectional slices. Linear measurement for dehiscence (LM-D) was defined as the distance between the alveolar crest to the cementoenamel junction of each root (critical point set at 2 mm). Linear measurement for fenestration (LM-F) was recorded when the defect involved only the apical one-third of a root (critical point set at 2.2 mm). Counts of ABDs/ABFs and magnitudes of LM-Ds/LM-Fs were recorded before and immediately after nonextraction CAT at buccal and lingual root surfaces. Binary logistic regression analyses and repeated measures analyses of variance were performed. Counts of ABDs/ABFs and magnitudes of LM-Ds/LM-Fs increased at most jaw locations and root surfaces. Nonextraction CAT was associated with an increased presence of ABDs and ABFs. Nonextraction CAT was associated with a higher magnitude of LM-Ds but not LM-Fs. Immediate posttreatment cone-beam computed tomography scans showed that nonextraction CAT is associated with increased ABDs and ABFs in adults with mild-to-moderate crowding.

Changes in the nasal and pharyngeal airway volume with miniscrew-assisted rapid palatal expansion appliances

Objectives: The purpose of this study was to assess the volumetric changes in the nasal and pharyngeal airway volume in patients with maxillary transverse deficiency treated with miniscrew-assisted rapid palatal expansion (MARPE) appliances. Material and Methods: The pre- and post-treatment cone-beam computed tomography scans of 20 subjects who underwent treatment with the MARPE appliance as part of their orthodontic treatment were included in the study. Pre- and post-expansion airway volume measurements of the nasal cavity, nasopharynx, and oropharynx were measured. Linear measurements of the skeletal structures surrounding the nasal cavity were taken to assess the pattern of airway expansion. Paired t-tests were used to evaluate nasal and pharyngeal airway volume changes with expansion. Pearson correlation test was utilized to assess the relationship between nasal and pharyngeal volume changes. Results: A total of 17 out of 20 samples had a successful split of the mid-palatal sutures. A significant increase in nasal airway volume (12.0%) was found post-expansion. In addition, a significant increase in linear measurements of the nasal cavity base width in the frontal plane (7.1%) and both the anterior and posterior widths in the transverse dimension (9.4% and 3.4%, respectively) were found. Significant positive correlations were found between airway volumes and increase in width of the maxillary expansion appliance. Conclusion: The effects of maxillary expansion with a MARPE appliance include an increase in nasal cavity volume, but not the nasopharyngeal and the oropharyngeal airway volume. The nasal cavity expands in a triangular pattern in both the frontal and transverse dimensions following orthopedic expansion with the MARPE appliance. Increase in airway volume is a result of the increase in width of the maxillary expansion appliance.

Three-dimensional craniofacial changes with maxillary expansion in young adult patients with different craniofacial morphology

Objectives: Skeletally mature patients with transverse deficiency are best treated with surgically assisted rapid palatal expansion (RPE) procedure. Recent studies have shown that microimplant-assisted RPE (MARPE) appliances can be effective in achieving skeletal expansion in young adults. This retrospective study aimed to evaluate the skeletal and dental alveolar changes in response to treatment with MARPE appliances in three types of anteroposterior skeletal malocclusions using cone-beam computed tomography (CBCT) scans. Material and Methods: Seventy-eight subjects diagnosed with maxillary transverse deficiency and treated with the MARPE appliance (mean age of 22.9 ± 4.2 years) were divided into skeletal Class I, II, and III malocclusions with 26 subjects in each group. Pre- and post-treatment CBCT scans were used for superimposition to examine the skeletal and dentoalveolar changes following maxillary expansion treatment. Results: Significant lateral separation of the maxilla was found at the levels of the nasal floor, interzygomatic bones, and the inferior palatine margin of the alveolar process (P &lt; 0.05) in the whole sample. Most of the sagittal and vertical variables change significantly in the whole sample and each studied group separately. Intergroup comparisons revealed no significant differences among the three skeletal classes except for the left frontozygomatic angle, left maxillary inclination angle, and torque in the first and second premolars. In Class III patients, the maxilla moved forward significantly in most of the cases (eight of 26 cases) (0.88°, P &lt; 0.05) and the mandible moved downward and backward improving the anteroposterior skeletal relationship. Significant differences were also found in the vertical measurements (N-Me, MMP, and MP/SN, P &lt; 0.05) in all three types of anteroposterior malocclusions. Conclusion: Maxillary expansion with the MARPE appliance in young adult patients induced different skeletal and dentoalveolar changes in the anteroposterior and vertical dimensions in each skeletal malocclusion with no significant difference among the three skeletal classes.

Airway changes after fixed functional appliance treatment in children with and without morphologic deviations of the upper spine: A 3-dimensional CBCT study

Functional appliances (FA) have a positive effect on the upper airway volume and minimal cross-sectional area (MCA) in children. An association between morphologic deviations of the upper spine (MDUS) and reduced treatment response was found in appliances used to treat adults with obstructive sleep apnea. This study aimed to: (1) compare airway changes after FA treatment in children with and without MDUS and controls; (2) identify if MDUS causes a smaller upper airway. Pretreatment and posttreatment cone-beam computed tomography scans were included from 21 children with MDUS and 42 without MDUS treated with a fixed FA, along with a pair-matched control group (matched for chronological age, skeletal age, gender, and mandibular inclination) who received orthodontic treatment for minor malocclusions without an FA. The influence of MDUS on changes in upper airway volume and MCA were evaluated with 3-dimensional cone-beam computed tomography scans using standardized, previously validated methods and mixed-effects linear regression. There was a significantly increased volume and MCA in the FA groups with and without MDUS compared with control (P= 0.003 and P= 0.049) and in the FA group without MDUS compared with the MDUS group (P= 0.008 and P= 0.011) after treatment. There was no significant pretreatment difference in airway dimensions between the MDUS and non-MDUS FA groups. The airway response with fixed FA is significantly reduced in MDUS children. MDUS caused no significant pretreatment airway differences in children. However, MDUS may be important in predicting airway changes in FA treatment.

Effect of orthodontic maxillary posterior en masse intrusion anchored with miniplates on maxillary sinuses volume. Retrospective CBCT study

ObjectiveTo evaluate, by means of cone beam computed tomography (CBCT), the volume change of maxillary sinuses in patients that underwent orthodontic maxillary posterior en masse intrusion anchored with miniplates. Materials and methodsThe sample consisted of pretreatment (T1) and posttreatment (T2) CBCT scans obtained from 14 patients (4 male and 10 female), with a mean age of 32.4 years, that underwent orthodontic maxillary posterior en masse intrusion anchored with miniplates in the zygomatic crest. The mean treatment duration was 20 months and the mean intrusion movement was 2.4 mm. Maxillary sinus volume was measured by means of the software ITK SNAP (version 3.8.0) in T1 and T2 CBCT scans. The changes in sinuses volume were calculated by T1-T2 values. Data were analyzed statistically with Wilcoxon test at 5% of level of significance and the method error was analyzed with Wilcoxon test, intraclass correlation and Dahlberg's formula. ResultsThe mean difference (T1-T2) was -242.85 mm³ (p = 0.396) for the right sinus and -32.5 mm³ (p = 0.875) for the left sinus. A slight increase in the volume of the sinuses was shown although these results were not statistically significant. ConclusionThe orthodontic maxillary posterior en masse intrusion anchored with miniplates did not influence significantly the maxillary sinus volume.

Analysis of nasal airway symmetry and upper airway changes after rapid maxillary expansion

The objectives of this study were to assess the changes in right vs left nasal cavity volumes and minimum cross-sectional width, nasopharyngeal, and oropharyngeal volumes of the upper airway in response to rapid maxillary expansion (RME). Pretreatment and posttreatment cone-beam computed tomography scans of 28 patients with a mean age of 9.86±2.43years and 20 age- and sex-matched controls were digitized and linear, angular, and volumetric measurements were obtained. Nasopharyngeal volume, right, and left nasal cavity volumes, and minimum cross-sectional widths increased significantly 2years post RME (P<0.05). These measurements did not show any significant increase in the control group (P>0.05), whereas the oropharyngeal volume increase for both groups was comparable (P=0.92). In the experimental group, the right and left nasal cavity volumes were not significantly different at baseline or posttreatment. However, the change that occurred was significantly larger for the left nasal cavity. This change for the control group was more significant for the right nasal cavity. Maxillary right and left molar inclinations were positively correlated to the nasal cavity volume, showing that the more buccally inclined the maxillary molars were, the smaller the nasal cavity volume. Nasopharyngeal and right and left nasal cavity volumes and minimum cross-sectional widths increase significantly after RME in young children. Expansion decreases the degree of difference in volume between the right and left nasal cavities. The buccal inclination of maxillary molars is correlated with nasal cavity volume.

Evidence-Based Planning Target Volume Margin Reduction for Modern Lung Stereotactic Ablative Radiation Therapy Using Deformable Registration

PurposeStandard planning target volume (PTV) margins for lung stereotactic ablative radiation therapy (SABR) are 5 mm. High-dose-rate volumetric modulated arc therapy delivered using flattening filter-free (FFF) beams with modern immobilization systems may allow for PTV margin reduction. This study assesses whether PTV margins can be reduced from 5 to 3 mm. MethodsTarget intrafractional motions derived from pretreatment and posttreatment cone beam computed tomography (CBCT) scans for 33 patients receiving lung SABR treated with 10XFFF energy and 5-mm PTV margins from 2016 to 2019 were used to calculate the required PTV margin. Deformable registration of the planning CT scan and internal gross tumor volume (IGTV) contour to posttreatment CBCT scans for 36 consecutive patients with 4 fraction schedules was completed to capture volume changes and intrafractional movement. Plans were replanned with 3-mm margins and recalculated on each deformed CT scan to assess deformed IGTV (d-IGTV) coverage and organ-at-risk doses. ResultsMargin analysis showed PTV margins may be reduced to 3 mm. The mean d-IGTV coverage (percentage of the d-IGTV receiving ≥100% of the prescription dose [V100%] and the minimum dose covering 99.9% of the d-IGTV volume [D99.9%]) over 4 fractions for each patient was >95% with both margins. With 5-mm PTV margins, all 144 fractions had a d-IGTV V100% of >95% and a D99.9% >95%. With 3-mm PTV margins, the d-IGTV V100% was >95% in 99.3% of fractions (143 of 144) and the D99.9% was >95% in 98.6% of fractions (142 of 144). With 3-mm PTV margins, significant reductions in body V50%, body V80%, the volume of the lung receiving ≥20 Gy, and the mean lung dose and chest wall dose to 0.035 cm3 and 30 cm3 were observed (all P < .001). Using theoretical models, the normal tissue complication probability for radiation pneumonitis decreased by a mean of 0.8% (range, 0.1%-2.7%), and the mean 2-year tumor control probability was 96.1% and 95.2% with 5-mm and 3-mm PTV margins, respectively. ConclusionWith modern treatment and immobilization techniques in lung SABR, 3-mm PTV margins maintain acceptable IGTV coverage, modestly reduce toxicity to organs at risk, and maintain a calculated 2-year local control rate of >95%.

Three-dimensional comparative evaluation of customized bone-anchored vs tooth-borne maxillary protraction in patients with skeletal Class III malocclusion

The aim of this prospective study was to assess the 3-dimensional skeletal and dental effects induced by a new maxillary protraction approach using customized miniplates for anchorage compared with a control group of traditional tooth-borne maxillary protraction. Forty-one growing patients at prepubescent stage with skeletal Class III malocclusion participated in this study. These patients were randomly split into 2 groups. In group 1 (n=20), patients underwent maxillary protraction anchored with customized miniplates. The miniplates were individually designed and inserted using the surgical templates. In group 2 (n=21), patients underwent tooth-borne maxillary protraction. Pretreatment and posttreatment cone-beam computed tomography scans were obtained, and 30 measurements of cone-beam computed tomography images were acquired and calculated. The changes after treatment and the comparison of the 2 groups were assessed. After maxillary protraction, group 1 showed a greater forward movement of the maxilla than group 2. The maxilla length increased more in group 1 than in group 2. The rotations of the palatal and mandibular planes in group 1 were less than those in group 2. In group 1, the maxillary incisors proclined less, the mandibular incisors retroclined less, and the maxillary first molars extruded less. Compared with tooth-borne maxillary protraction, customized miniplates anchored maxillary protraction produced more maxillary growth, fewer dental changes, and less maxillary and mandibular plane rotation.

Volumetric Evaluation of Pharyngeal Airway after Functional Therapy.

The aim of this study was to evaluate three-dimensional (3D) effects of Twin-block functional appliance (TB) on the pharyngeal airway by using cone beam computed tomography (CBCT). A total of 30 patients (14 females, 16 males; mean age 12.50 ± 1.23 and 12.83 ± 1.17 years, respectively) with skeletal Class II malocclusion were included in this study and were treated with TB. On the pretreatment (T1) and posttreatment (T2) CBCT scans, volumetric changes in the pharyngeal airway; SNA, SNB, and ANB angles; and bilateral effective mandibular (Co-Gn) and midfacial length (Co-A) were also evaluated. The statistical differences were accessed by Wilcoxon signed-rank tests, and Mann-Whitney U tests were used to analyze the scores of male and female subjects. In this study, an increase was observed in SNB and Co-Gn (p < 0.01) while a decrease in ANB and SNA (p < 0.01 and p < 0.05, respectively) was found. However, increase in midfacial length was not statistically significant (p > 0.05). In the evaluation of volumetric pharyngeal airway changes, statistically significant increases (p < 0.01) in the upper and lower division and total airway volume were determined. Gender differences were insignificant for all measurements (p > 0.05). Volumetric changes in the pharyngeal airway after functional therapy can be successfully evaluated by CBCT images. The anterior repositioning of the mandible by TB increases the mandibular length and pharyngeal airway volume in patients with retrognathic mandible.

Pretreatment and posttreatment cone-beam computed tomography (CBCT) scans were collected from 69 patients who underwent nonextraction treatment with clear aligners (Invisalign; Align Technology, San Jose, Calif). Integrated 3-dimensional models of the virtual incisor position (ClinCheck; Align Technology) and the posttreatment incisor position (from posttreatment CBCT scans) were superimposed over the pretreatment position (from pretreatment CBCT scans) using Mimics software (Materialise, Leuven, Belgium). On the basis of the location of the rotation center, incisors showing pure tipping (>10°), controlled tipping (>10°), translation (>1mm), or torque (>10°) movements were selected. Efficacy was determined by comparing the predicted and achieved incisor movement, and differences with efficacy were analyzed using Kruskal-Wallis and Shapiro-Wilk tests (α=0.05). In measurements for 231 incisors, the mean efficacy of incisor movements in the sagittal plane was 55.58%. The most and least predictable movements were pure tipping (72.48%) and torque (35.21%), respectively. Labial root movement was significantly more predictable than lingual root movement, and labial movement of the mandibular incisors was significantly easier than that of the maxillary incisors. The type of tooth movement achieved was different from the type designed. The efficacy of incisor movement in the sagittal plane using clear aligners varies with designed movement type, and labial root movement appears to be more accurate than the lingual root movement. The biomechanics of clear aligners remains to be further elucidated to achieve more predictable treatment results.

Predicted and actual outcome of anterior intrusion with Invisalign assessed with cone-beam computed tomography

The purpose of this study was to compare predicted anterior teeth intrusion measurements with the actual clinical intrusion measurements using cone-beam computed tomography. Understanding the precision of the software in anticipating changes may help practitioners predict the need for overcorrection. Twenty-two patients, with a mean age of 23.74years, who underwent Invisalign (AlignTechnology, Santa Clara, Calif) clear aligners treatment for both arches only after having completed treatment with an initial series of aligners were included in this study. The pretreatment and posttreatment cone-beam computed tomography scans after the initial series were acquired by a single orthodontist practitioner. ClinCheck measurements were recorded with Align Technology. The long axis of the anterior tooth intrusion movement was measured in 142 teeth. A comparison between the predicted and actual measurements of anterior intrusion of the teeth was performed, and the intraclass correlation coefficients showed an almost perfect agreement in the linear measurements. A statistically notable difference between the predicted and actual measurements of anterior intrusion. The predicted intrusion movement of the maxillary canines (P=0.001), maxillary lateral incisors (P<0.0001), and maxillary central incisors (P<0.0001) significantly differed from the actual values. Similarly, the intrusion movement in the mandibular teeth seemed to be inaccurate, with significant differences in the mandibular canines (P<0.0001) and mandibular lateral and central incisors (P<0.0001). The mean precision of true anterior intrusion with Invisalign clear aligners was 51.19%, and the mean amount of correction was 48.81%. The use of other supplementary methods of anterior teeth intrusion may be helpful to reduce the rate of midcourse corrections and refinements.

Midpalatal suture density ratio: Assessing the predictive power of a novel predictor of skeletal response to maxillary expansion

Rapid maxillary expansion (RME) is a common orthodontic treatment to correct maxillary transverse deficiency; however, the inability to determine the precise timing of fusion of the midpalatal suture creates difficulty for clinicians to prescribe the appropriate treatment, surgical or nonsurgical expansion. The purpose of this study was to assess the predictive power of the midpalatal suture density ratio (MPSD) for a skeletal response to RME. Pre- and posttreatment cone-beam computed tomography scans were obtained from 78 orthodontic patients aged from 8 to 18years treated with RME. MPSDs were calculated from pretreatment scans, and a prediction was made for the amount of skeletal expansion obtained at the level of the palate after comprehensive orthodontic treatment. Predicted values were compared with actual outcomes as assessed from posttreatment scans, followed by regression analyses to investigate correlations between MPSD and skeletal expansion and equivalence testing to analyze the performance of the predicted measurements. The MPSDs were not statistically significantly (P>0.05) correlated with the amount of skeletal expansion achieved. In addition, the predicted skeletal expansion using MPSD was not statistically equivalent to the skeletal expansion achieved using an equivalence margin of ±0.05. The results suggest that the MPSD obtained from pretreatment cone-beam computed tomography scans were not correlated well enough with the amount of skeletal expansion achieved to be an effective predictor of the amount of long-term skeletal expansion after RME.

Effects of a fixed functional appliance on upper airway volume: A 3-dimensional cone-beam computed tomography study

Although functional appliances (FAs) are primarily used to treat Class II malocclusions, their effects on the upper airway have generally been reported in studies involving 2-dimensional analysis of cephalometric radiographs. This study aimed to use 3-dimensional cone-beam computed tomographyto (1) determine the effects on upper airway volume and minimum cross-sectional area (MCA)when an FA is used for orthodontic treatment and (2) identify pretreatment markers for airway changes. Pre- and posttreatment cone-beam computed tomography scans were selected from 73 children whose orthodontic treatment involved using a fixed FA (37 girls and 36 boys; mean age 12.0years); a control group was constituted and included the scans of 73 children who were matched for chronological age, skeletal age, sex, and mandibular inclination and who received orthodontic treatment for minor malocclusions without an FA. Changes in upper airway volume and MCA were evaluated by a standardized, previously validated method analyzed with mixed-effects linear regression. Upper airway volume and MCA increased over time for both the FA and the matched control groups (P<0.05). The FA caused an additional significant increase in upper airway volume and MCA (P<0.0001) when compared with the control group. An increased ANB angle, a reduced mandibular inclination, and a reduced skeletal age pretreatment had a positive effect on upper airway volume and MCA (P<0.05). A fixed FA was associated with a significant increase in upper airway volume and MCA in children when compared with controls. Pretreatment ANB angle, skeletal age, and mandibular inclination may be predictors for positive changes in the upper airway. The results may prove valuable in treating children with Class II malocclusions with compromised upper airways.

3D effects of a bone-anchored intra-oral protraction in treating class III growing patient: a pilot study

ObjectivesThe aim of this prospective case series study is to assess the three-dimensional (3D) skeletal and soft tissue effects of the alternate rapid maxillary expansion and constriction (Alt-RAMEC) protocol in conjunction with a miniscrew-supported class III elastic wear in class III growing patients.Materials and methodsFourteen consecutive participants (mean age 12.05 ± 1.09 years), who displayed class III malocclusions with retrognathic maxillae, were recruited. A cone beam computed tomography (CBCT) scan was acquired before commencing treatment (T1). All participants were treated with a tooth-bone-borne rapid maxillary expansion (hybrid MARME) appliance that was activated by the Alt-RAMEC protocol for 9 weeks. This was followed by full-time class III elastics, delivering 400 g/side, to protract the maxilla. When a positive overjet was achieved, protraction was ceased and a post-treatment CBCT scan (T2) was taken. The 3D analysis of pre- and post-treatment CBCT scans was blinded. The scans were registered on the anterior cranial base. The Euclidean distance between the two extracted surface models of the pre- and post-treatment scans was displayed as a color surface map.ResultsAll participants completed the intervention successfully. The majority of the participants showed protraction of the anterior maxillary region (71.4%) and in the zygomatic processes (64.2%). The maxilla significantly protracted (SNA 1.87° ± 1.06°; Vert.T-A 3.29 ± 1.54 mm), while the mandibular base significantly redirected posteriorly (SNB − 2.03° ± 0.85°, Vert.T-B − 3.43 ± 4.47 mm) and that was reflected on the ANB and Wits measurements. No adverse effects were observed.ConclusionClass III elastics combined with the Alt-RAMEC activation protocol of the hybrid MARME appliance is an effective treatment method for mild/moderate class III malocclusions. A long-term follow-up and comparisons with other treatment modalities are required.

Pharyngeal airway dimension in patients before and after treatment of myofascial pain syndrome

ABSTRACT Objective: This study aimed: (1) to assess the localization of the anatomic landmarks of the pharyngeal airway on cone beam computed tomography (CBCT) images; and (2) to evaluate if resolution of myofascial pain syndrome (MPS) changed the airway dimensions. Methods: Twenty-nine patients with pre- and post-treatment CBCT scans were randomly selected to locate five landmarks twice, with a two-week interval. The same landmarks were used to measure the airway volume and minimal cross-sectional area (CSAmin). Results: The intra-observer reliability (ICC) was 0.99–1.00 for volumetric and CSAmin measurements, based on the five landmarks used. The paired t test showed no significant difference in the airway volume (p = 0.68) and CSAmin (p = 0.96). Discussion: The outcomes showed that the landmarks used had excellent ICCs for the volumetric and CSAmin measurements. There was no change in volume and CSAmin of the pharyngeal airway after resolution of MPS.
