Hallux valgus treatment by percutaneous scarf-like screwless osteotomy: surgical technique and medium-term outcomes

BackgroundHallux valgus is a common condition. There is a consensus that distal metatarsal osteotomies have positive outcomes for the correction of mild-to-moderate deformities, and that proximal osteotomies are preferable for more severe ones. The well-known scarf osteotomy technique is considered powerful enough for both types of deformation and is described without internal fixation. We aimed to describe a new surgical technique for hallux valgus percutaneous scarf like osteotomy (PSLO) without internal fixation, and to report the medium-term radiological and clinical outcomes. HypothesisA combination of a PSLO without internal fixation will lead to optimal results. Patients and methodsThis retrospective case series reports on 126 cases involving 106 patients who underwent hallux valgus surgery with the PSLO technique +/− Akin, +/− lateral release. The osteotomy was stabilized by a bandage, and immediate weight bearing was allowed. The surgeries took place in 3 clinics in France from March 2016 to July 2017. ResultsAll radiological parameters: hallux valgus angle (HVA), the 1–2 intermetatarsal angle (IMA) and the distal metatarsal articular angle (DMAA) showed statistically significant improvement. The mean preoperative HVA, IMA and DMAA were 27.7 °, 14.2 ° and 12.7 ° respectively. The mean postoperative HVA, IMA and DMAA were 8.23 °, 8.1 ° and 3.8 °, respectively. Clinically, 97% were satisfied or very satisfied with the results, 92% could wear “normal” shoes (45 days - 6 months), 99% returned to the same athletic activities after surgery (3–5 months), and the average time to return to work was 4 weeks (1 day - 51 days). DiscussionThe combination of scarf -like osteotomy and the percutaneous technique provide sufficient initial stabilization through the large horizontal area of contact between the fragments and the preservation of the attachment of the soft tissue. The lack of internal fixation significantly shortens the surgery time, cost, and reduces x-ray exposure without compromising the results. Level of evidenceIII, retrospective comparative study

Developing Machine Learning-Based Predictive Models for Hallux Valgus Recurrence Based on Measurements From Radiographs.

Machine learning (ML) is increasingly used to predict the prognosis of numerous diseases. This retrospective analysis aimed to develop a prediction model using ML algorithms and to identify predictors associated with the recurrence of hallux valgus (HV) following surgery. A total of 198 symptomatic feet that underwent chevron osteotomy combined with a distal soft tissue procedure were enrolled and analyzed from 2 independent medical centers. The feet were grouped according to nonrecurrence or recurrence based on 1-year follow-up outcomes. Preoperative weightbearing radiographs and immediate postoperative nonweightbearing radiographs were obtained for each HV foot. Radiographic measurements (eg, HV angle and intermetatarsal angle) were acquired and used for ML model training. A total of 9 commonly used ML models were trained on the data obtained from one institute (108 feet), and tested on the other data set from another independent institute (90 feet) for external validation. Optimal feature sets for each model were identified based on a 2000-resample bootstrap-based internal validation via an exhaustive search. The performance of each model was then tested on the external validation set. The area under the curve (AUC), classification accuracy, sensitivity, and specificity of each model were calculated to evaluate the performance of each model. The support vector machine (SVM) model showed the highest predictive accuracy compared to other methods, with an AUC of 0.88 and an accuracy of 75.6%. Preoperative hallux valgus angle, tibial sesamoid position, postoperative intermetatarsal angle, and postoperative tibial sesamoid position were identified as the most selected features by several ML models. ML classifiers such as SVM could predict the recurrence of HV (an HVA >20 degrees) at a 1-year follow-up while identifying associated predictors in a multivariate manner. This study holds the potential for foot and ankle surgeons to effectively identify individuals at higher risk of HV recurrence postsurgery.

Distal Rotational Metatarsal Osteotomy (DROMO) for the treatment of hallux valgus associated with first metatarsal pronation: Surgical technique and initial results

BackgroundHallux valgus (HV) is a very common deformity worldwide. Most of the surgical techniques used in the treatment of HV only correct these deformities in two planes, that is sagittal and transverse planes. The importance of the first metatarsal pronation as an etiological factor of hallux valgus is validated by numerous authors and it is usually unaddressed. Few surgical techniques have focused on the correction of rotational deformity of the hallux valgus. We aim to first report a detailed technique and a case series using the Distal Rotational Metatarsal Osteotomy (DROMO) surgical technique, which is less invasive and can address the pronation deformation. MethodsThe inclusion period was 6 months finishing in April 2021. The study analyzed the following x-ray parameters: preoperative and postoperative intermetatarsal angle (IMA), preoperative and postoperative hallux valgus angle (HVA), preoperative and postoperative coronal rotation of the first metatarsal according Hardy and Clapham’s classification as described by Kim et al., preoperative and postoperative shape of the first metatarsal head as described by Ono et al. Results33 patients matched our inclusion and exclusion criteria. Most patients underwent the surgery for the left foot (n = 18), compared to the right foot (n = 15). We found statistically significant differences between preoperative and postoperative IMA (p < 0.001), preoperative and postoperative HVA (p < 0.001). Preoperative and postoperative coronal rotation of the first metatarsal as classified by Hardy and Clapham was significantly different (p < 0.001), as well as preoperative and postoperative shape of the first metatarsal head (p < 0.01). ConclusionsDROMO can correct the metatarsal rotation through minimal invasive surgery. From our perspective, DROMO technique should be another alternative for HV correction technique which in time can be associated as a local anesthetic technique, DROMO results are attractive for an ambulatory procedure.

Significant Early Loss of Correction in Modified Lapidus Compared to Original Lapidus for Hallux Valgus

Category: Bunion; Other Introduction/Purpose: The Lapidus procedure (first tarsometatarsal joint (TMT1) fusion) is an established treatment for correcting hallux valgus with 1st ray hypermobility. The original Lapidus (OL) technique involves fusing the TMT1 joint and first metatarsal base to the second, while the modified Lapidus (ML) technique involves fusing only the TMT1 joint. The purpose of this study was to investigate whether the ML procedure results in an early loss of correction. Methods: This retrospective study analyzed the outcomes of 45 feet in 40 patients with hallux valgus who underwent either the ML (21 feet) or OL (24 feet) procedure between 2014 and 2022 at a single center. All fixations were performed with 3.5 cortical screws, except for 9 cases that were fixed with a plantar locking plate. Differences in the immediate postoperative (6 weeks) and postoperative (6 months) intermetatarsal angle (IMA) and hallux valgus angle (HVA) were analyzed using Mann-Whitney tests, and complications were reported. Results: The mean preoperative IMA and HVA for the OL were 15.8° ± 3.5° and 36.4° ± 9.5°, respectively, and 14.5° ± 2.3° and 33.0° ± 7.0° for the ML. The immediate postoperative IMA and HVA were similar for both procedures (7.2° ± 2.0° and 7.7° ± 4.3° for OL, 7.3° ± 2.1° and 8.5° ± 5.5° for ML). Although from 6 weeks to 6 months postoperatively, the loss of correction of the HVA did not differ between both procedures (4.2° for OL, 5,6° for ML), the IMA loss of correction was significantly higher in the ML (1.1° ± 1.4° vs 0.5° ± 1.1°) (p &lt; 0.05). One case of delayed union was reported in the OL group, but it did not require revision. Conclusion: The study results suggest that the ML procedure does not provide the same stability as the OL procedure in the early postoperative period, as there was a significantly greater early loss of correction of the IMA at 6 months postoperatively in the ML group compared to the OL group. Although there were no differences in the loss of correction of the HVA angle (HVA), an observed loss of 5 degrees remains a concern. Further studies are necessary to better understand the indications of the modified and original procedures.

Early Radiological Outcome of Minimally Invasive Bunion Correction Using a Guided Trajectory System

Category: Bunion; Other Introduction/Purpose: Hallux Valgus correction with Minimally Invasive Surgery (MIS) is a popular procedure due to its potential advantages such as shorter operative time and quicker recovery than open surgery. Third-generation Minimally Invasive Chevron Akin (MICA) osteotomy has shown excellent clinical and radiological outcomes. The optimal fixation criteria are 3-point fixation (medial cortex, lateral cortex, and the lateral half of the 1st metatarsal head) for the proximal screws and 2-point fixation (medial cortex and central position in the 1st metatarsal head) for the distal one. To address the steep learning curve of the free hand technique, we describe a new guided trajectory system for third-generation MICA to increase precision and reduce the risk of suboptimal K-wire or screw placement. Early radiological outcomes and complications were reported. Methods: From April 2022 to January 2023, 13 consecutive MICAs were performed on 11 female patients (bilateral in 2 patients) with an average age of 61 by a single fellowship-trained foot and ankle orthopaedic surgeon who developed the device. These were the 13 first cases performed with the new device. Preoperative and postoperative intermetatarsal angle (IMA), hallux valgus angle (HVA), distal metatarsal articular angle (DMAA) and tibial sesamoid position (TSP) were measured in all patients using weight-bearing radiographs. Also evaluated in the postoperative radiographs were the number of cortical purchase of the screws and their position in the metatarsal head. Additionally, any difficulties or intraoperative complications and operative time were reported. All parameters were analysed with the one-tailed non-parametric Wilcoxon test. Results: All the radiographic parameters improved significantly. The median IMA improved from 14.5 degrees (interquartile range [IQR]: 11.5-16) to 5 degrees (IQR: 4.0-6.0) (p &lt; 0.005). The median HVA also decreased from 27.5 degrees (IQR: 25.3-34.5) to 7.0 degrees (IQR: 4.0-8.5) (p &lt; 0.005). The median DMAA fell from 15.5 degrees (IQR: 13.3-19.3) to 6.0 degrees (IQR: 6.0-8.0) (p &lt; 0.005). The median TSP was 2 (IQR: 2-3) pre-operatively and 0 (IQR : 0-1) (p &lt; 0.005) post-operatively. All the proximal screws for Chevron osteotomy had 3-point fixation and the distal anti-rotation screws had 2-point fixation No intraoperative complications were reported. The mean operative time was 52 (SD:10,4) minutes. Conclusion: Our study demonstrates the successful use of a guided trajectory system for minimally invasive bunion correction with optimal screw placement, good early radiological outcomes, and without extensive operating time. The absence of intraoperative complications or difficulties further confirms the efficacy of this system. Our findings suggest that the use of a guided trajectory system can potentially improve the consistency and success of third-generation MICA procedures. These results emphasize the benefits of incorporating guided trajectory systems in bunion correction surgeries and their potential to improve patient outcomes.

Weight-bearing CT Scan Analysis in Hallux Valgus Treated with Minimally Invasive Surgery

Category: Bunion; Midfoot/Forefoot Introduction/Purpose: Hallux valgus (HV) is one of the most prevalent foot deformities. Coronal metatarsal rotation (pronation) has been recognized as a key element in HV deformity and an important prognostic factor for recurrence. With the advent of weight-bearing CT (WBCT), accurate pronation measurement has improved. The Minimally Invasive Bosh Akin (MIBA) surgery has proven to be effective in treating HV deformity. The literature analyzing correction of pronation in HV surgery is lacking. Purpose: To compare the transverse and coronal plane deformity correction by WBCT in HV patients treated with MIBA surgery. Methods: We retrospectively analyzed HV deformity in patients who underwent MIBA surgery with preoperative and at least 3 months postoperative WBCT for comparison. Intermetatarsal angle (IMA), hallux valgus angle (HVA), and alpha angle (AA) for first metatarsal pronation were measured by 2 different observers. The intraclass and interclass correlation were determined. Pre and postoperative measurement changes were established by T-test for related samples. the p-value for statistical significance was set at &lt; 0.05. Results: 37 feet were included. Intraclass correlation was 0.97 /0.92 for IMA; 0.95/0.91 for AA and 0.99/0.97 for HVA preoperative. Postop it was 0.97/ 0.96 for IMA, 0.97/0.92 for AA, and 0.98 and 0.96 for HVA. Interclass correlation was 0.95 for IMA; 0.7 for AA and 0.82 for HVA preoperative. Postop, was 0.82 for IMA, 0.8 for AA, and 0.79 for HVA. Mean preoperative / postoperative AA was 8.9 degrees and 6.9 degrees respectively, resulting in a first ray pronation change of –2.0 degrees, p&lt; 0.054. The media preoperative and postoperative IMA was 13.6 degrees and 6.8 degrees, demonstrating a significant change (P &lt; .005). The media preoperative and postoperative HVA was 28.7 (8.8) and 13.1 (9.0), demonstrating a significant change (P &lt; .005). Conclusion: To the best of our knowledge, this is the first study that evaluates the radiological changes in HV surgery with WBCT. The MIBA osteotomy for HV deformity has significant power to correct the transverse plane deformity. Even though the coronal plane deformity correction wasn’t statistically significant, it showed a strong tendency, which might be demonstrated by utilizing a bigger sample. The only measurement that didn’t have an excellent agreement was the preop AA.

Comparison of Intraoperative Radiation Exposure Between Minimally Invasive Chevron Akin (MICA) and Open Procedures for Hallux Valgus Correction

Category: Bunion; Midfoot/Forefoot Introduction/Purpose: The minimally-invasive Chevron-Akin (MICA) procedure for hallux valgus correction is an increasingly popular technique that has yielded comparable radiographic and clinical outcomes to open procedures3–5. Intraoperative radiation dosages for certain foot and ankle surgeries have been reported relative to the international occupational radioprotection thresholds6. In particular, the mean intraoperative radiation exposure during the MIS Scarf osteotomy has been reported to be 14-times greater than open osteotomy7. However, no study to date has reported on the intraoperative radiation exposure during MICA procedures in relation to that of open procedures. Methods: This is a retrospective comparative study from a single institution. Patients underwent a MICA (n=25) or open (n=40) hallux valgus correction by a single foot and ankle fellowship trained surgeon. To account for the learning curve8, the number of intraoperative fluoroscopy shots for the first 50 consecutive MICA procedures were analyzed, and the first 25 procedures were excluded. Four data points were retrieved from the mini C-arm, including 1) number of fluoroscopy shots taken; 2) total fluoroscopy time (s); 3) total radiation dose (mGy); and 4) total dose area product (DAP; mGy*cm2). Preoperative and postoperative radiographs were referenced to measure the hallux valgus (HVA) and intermetatarsal angles (IMA) as previously described.9 Categorical variables were compared using the chi-square test, and continuous variables were compared using Student’s t-test or the Mann-Whitney U test. Analysis of variance was used to determine the statistical significance of differences in the fluoroscopy data between groups. Results: The average total dose absorbed (4.00 vs. 1.34 mGy, p&lt; 0.001), total fluoroscopy time (226 vs. 48.5 s, p&lt; 0.001), and number of fluoroscopy shots (125 vs. 22.3, p&lt; 0.001) were 2.9, 4.6, and 5.6 times greater in the MICA versus the open group, respectively. The total DAP (4.22 vs. 2.56 mGy*cm2, p=0.175) was 1.6 times greater in the MICA group but the difference was not statistically significant. The mean preoperative and postoperative HVA and IMA were greater in the MICA cohort compared to the open cohort. However, there was no statistical difference in change in HVA (MICA -17.43 vs. Open -20.05, p=0.125) or IMA (MICA -8.08 vs. open -9.31, p=0.11) between the two cohorts, with each resulting in normal postoperative angles. Conclusion: In this single surgeon series, the MICA procedure demonstrated significantly greater dose absorbed, total fluoroscopy time, and number of fluoroscopy shots compared to the open procedure. The extent of radiation exposure to the surgeon and the patient in relation to the radioprotection thresholds must be closely monitored to specify the health risks involved with this procedure.

Comparison of Radiographic and Clinical Outcomes between Distal and Proximal Minimally Invasive Chevron and Akin Osteotomy for Severe Hallux Valgus Deformity

Category: Midfoot/Forefoot; Midfoot/Forefoot Introduction/Purpose: The surgical treatment of severe hallux valgus (HV) deformity is challenging due to the difficulty in achieving sufficient reduction and the increased risk of complications such as recurrence, delayed or non-union. Minimally invasive surgery (MIS) has been increasingly used to treat HV deformity, with the third-generation minimally invasive Chevron and Akin osteotomies (MICA) technique being widely accepted. This retrospective cohort study aimed to compare the radiographic and clinical outcomes between the distal and proximal MICA techniques for severe hallux valgus deformity. It was hypothesized that the proximal MICA technique would provide better radiographic results due to the stronger deformity correction achieved by the proximal metatarsal osteotomy. Methods: Seventy-four feet with severe HV (defined as HVA [≥40 degrees] and/or IMA [≥20 degrees]) treated with distal MICA (40 feet) or proximal MICA (34 feet) were included in this study after at least one year of follow-up. Clinical assessment included the American Orthopaedic Foot &amp; Ankle Society (AOFAS) score, pain visual analog scale (VAS), patient satisfaction rating, and complications. Radiographic assessments included HV angle (HVA), intermetatarsal angle (IMA), and distal metatarsal articular angle (DMAA). Results: There were no differences in the mean AOFAS score, pain VAS score, and patient satisfaction rate at the final follow-up between the two groups (all p &gt; .05). The mean postoperative HVA, IMA, and DMAA significantly improved at the final follow-up in both groups (all p &lt; .001). There was no significant difference in the improvement of radiographic values after surgery between the two groups (all p &gt; 0.05). Although not statistically significant, there was a trend toward a higher recurrence and delayed or nonunion rate in the distal MICA group (7.5% vs. 2.9%, P =.218 and 10% vs. 2.9%, P =.072). Conclusion: Both types of minimally invasive techniques resulted in satisfactory outcomes in terms of significant improvement in clinical values, large deformity correction, and reasonable complication rates. However, the proximal MICA technique appears to provide more stable maintenance of deformity correction than the distal technique.

Republication of "Treatment of Hallux Valgus Deformity Using a Suture Button Device: A Preliminary Report".

Hallux valgus (HV) is commonly treated with proximal or distal first metatarsal osteotomy. Despite good correction, these procedures have inherent risks such as malunion, nonunion, metatarsal shortening, loss of fixation, and avascular necrosis. Suture button fixation has been used for HV treatment. It avoids the risks of corrective osteotomies while maintaining reduction of the intermetatarsal angle (IMA). The goal of this study was to assess the radiographic and functional outcomes of patients undergoing HV correction with a distal soft tissue procedure and proximal suture button fixation. The authors retrospectively reviewed the charts and radiographs of 22 patients who had undergone HV correction using a distal soft tissue correction and proximal fixation with a miniature suture button device (Mini TightRope; Arthrex, Inc, Naples, FL). Mean follow-up was 27.7 months. The IMA, hallux valgus angle (HVA), and sesamoid station were measured on radiographs obtained preoperatively as well as in the immediate postoperative period and at final follow-up. Preoperative and postoperative Short Form-36 (SF-36) and Foot and Ankle Ability Measure (FAAM) scores were collected. Postoperative complications, and any additional operative procedures performed were also recorded. The mean preoperative IMA and HVA were 16.9 and 32.6 degrees, respectively. The mean immediate postoperative IMA was 5.2 degrees (P < .0001) and the mean HVA was 9.8 degrees (P < .0001). At final follow-up, the mean IMA was 8.2 degrees (P < .0001) and the mean HVA was 16.7 degrees (P < .0001). The average change in HVA from preoperative to final follow-up was 16.0 degrees and the average change in IMA from preoperative to final follow-up was 8.6 degrees (P < .0001). Sesamoid station assessment at the 2-week follow-up showed that 22 patients (100%) were in the normal position group; at final follow-up, 17 patients (77%) had normal position and 5 patients (23%) had displaced position. Although there were no clinically symptomatic recurrences, asymptomatic radiographic recurrence was noted in 5 patients (23%) who had a final HVA >20 degrees. All components of the FAAM and the SF-36 showed improvement from preoperative to final follow-up, although these changes were not statistically significant. Three patients experienced complications, including an intraoperative second metatarsal fracture, a postoperative second metatarsal stress fracture, and a postoperative deep vein thrombosis. The use of a distal soft tissue procedure in conjunction with proximal suture button fixation is a safe and effective procedure for treating symptomatic HV deformity. Our results show that this technique can correct the IMA, HVA, and sesamoid station without the need for osteotomy. Level IV.

Comparison of Radiographic and Clinical Outcomes of First Tarsometatarsal Arthrodesis with the Lapiplasty System vs Cross-Screw Fixation

Category: Midfoot/Forefoot; Bunion Introduction/Purpose: Hallux valgus (HV) is recognized as a triplanar deformity comprising axial abduction, sagittal hypermobility, and coronal pronation of the first metatarsal. A first tarsometatarsal (TMT) arthrodesis may be used to address all three planes of deformity. The Lapiplasty system (Treace Medical Concepts, Inc., Ponte Vedra, FL) was developed to reproducibly address triplanar deformity. However, the Lapiplasty system is costlier than other fixation constructs. Because healthcare is increasingly value-focused, we sought to determine if the more expensive Lapiplasty system improved radiographic outcomes or decreased complication rates over standard cross-screw fixation. We hypothesized that HV patients undergoing first TMT arthrodesis with the Lapiplasty system would have equivalent radiographic correction and similar complication rates when compared to patients treated with cross-screw fixation. Methods: In this multi-center, IRB-approved retrospective study, consecutive series of patients who underwent 1st TMT arthrodesis for HV, either with cross-screw fixation at Institution A, or Lapiplasty at Institution B, were identified by registry search. Inclusion criteria included age over 18 and preoperative and minimum 6-month postoperative weight-bearing AP and lateral radiographs. Exclusion criteria included primary conditions other than hallux valgus and previous first ray surgeries or ipsilateral arthrodeses. Hallux valgus angle (HVA), intermetatarsal angle (IMA), and tibial sesamoid position (TSP) were measured preoperatively and at final follow-up, and clinic notes were reviewed to identify complications and reoperations. To compare cohorts, Mann-Whitney U tests and Fisher's Exact tests were applied for continuous and discrete variables, respectively. To examine preoperative-postoperative changes, Wilcoxon signed rank tests were used. Multivariable regressions tested the effect of fixation constructs on postoperative radiographic parameters, adjusted for preoperative radiographic measurements, age, sex, and BMI. Results: 65 patients were included in each group, with 121 (93.8%) female patients, mean (SD) age 54.7 (14.8), BMI 25.8 (5.2), and radiographic follow-up time 8.9 (6.7) months. The Lapiplasty cohort had greater BMI (26.8 vs. 24.7, P = .048), and there were no significant differences between groups in age, sex distribution, and radiographic follow-up time. Radiographic measurement data is displayed in Table 1. After adjusting for age, sex, BMI, preoperative HVA, and preoperative IMA, fixation with Lapiplasty was significantly associated with lower postoperative IMA (b=-2.03, 95% CI = [-2.78, -1.37], P &lt; .001) and lower postoperative HVA (b= -7.48; 95% CI = [-9.69, -5.26]; P &lt; .001), but not postoperative sesamoid position. There were no significant differences between cohorts in the incidence of complications or reoperations. Conclusion: We found that use of the Lapiplasty system was associated with better radiographic hallux valgus correction, as evidenced by lower postoperative HVAs and IMAs, compared to standard technique with crossing screws. The clinical significance of these findings is unclear, as complication and reoperation rates were similar between groups. Future research assessing longer- term outcomes, patient-reported outcome data, and cost-benefit analysis will be necessary to further compare techniques and fixation systems.

Comparative Analysis of Hallux Valgus Surgical Techniques: A Systematic Review

Category: Bunion; Midfoot/Forefoot Introduction/Purpose: Hallux valgus is a foot deformity where the distal portion of the hallux deviates laterally, forming a bunion. Here we review three surgical techniques for correction of the deformity: lapidus arthrodesis (LA), minimally invasive surgery (MIS), or proximal chevron osteotomy (PCO). The purpose of this study was to compare the hallux valgus angle (HVA) and intermetatarsal angle (IMA) for radiologic outcomes, and the American Orthopaedic Foot &amp; Ankle Society Score (AOFAS) score for quality of life outcomes, between treatment groups. Methods: We performed a literature search, with strict inclusion and exclusion criteria, for identifying pertinent references. After review, and resolution of any disagreements by consensus among 5 raters, a total of 42 studies were included in this analysis. Results: The mean HVA difference (postoperative to preoperative) was 23.68 for PCO and 21.40 for LA. The mean IMA difference was 9.53 for PCO and 8.12 for LA. The mean of postoperative HVA was calculated to be11.81 for MIS and 9.13 for LA. The mean of postoperative IMA was calculated to be 8.39 for MIS and 6.60 for LA. The postoperative AOFAS mean was 86.00 for MIS and 86.46 for LA. No studies directly compared surgical methods; the results that follow describe each technique summarized separately. Conclusion: We discovered that HVA and IMA difference means were higher in PCO vs LA, with a larger difference signifying better radiologic correction. HVA and IMA postoperative means were lower in LA vs MIS. A smaller postoperative angle signifies more physiologic correction. Postoperative AOFAS means were similar in MIS vs LA. Further research should be done to evaluate an ideal procedure and explore better techniques for hallux valgus correction.

Cohort Comparison of Radiographic Correction and Complications Between Minimal Invasive and Open Lapidus Procedures for Hallux Valgus.

Background:The Lapidus procedure corrects hallux valgus first ray deformity. First tarsometatarsal (TMT) fusion in patients with hallux valgus deformity using minimally invasive surgery (MIS) is a new technique, but comparative outcomes between MIS and open techniques have not been reported. This study compares the early radiographic results and complications of the MIS with the open procedure in a single-surgeon practice.Methods:47 MIS patients were compared with 44 open patients. Radiographic measures compared preoperatively and postoperatively were the intermetatarsal angle (IMA), hallux valgus angle (HVA), foot width (FW), distal metatarsal articular angle (DMAA), sesamoid station (SS), metatarsus adductus angle (MAA), first metatarsal to second metatarsal length, and elevation of the first metatarsal. Early complications were recorded, as well as repeat surgeries.Results:The mean follow-up was 82 (range, 31-182) months for the open group and 29 (range, 14-47) months for the MIS group. In both techniques, postoperative measures (IMA, HVA, DMAA, FW, and sesamoid station) were significantly improved from preoperative measures. When comparing postoperative measures between both groups, the IMA was significantly lower in the open group (4.8 ± 3.6 degrees vs 6.4 ± 3.2 degrees, P < .05). Differential between pre- and postoperative measures for both techniques were compared, and the open group was associated with more correction than the MIS group for IMA (12.4 ± 5.3 degrees vs 9.4 ± 4.4 degrees, P = .004) and HVA (25.5 ± 8.3 degrees vs 20 ± 9.9 degrees, P = .005). Wound complication and nonunion rates trended higher in the open group (4 vs 0) (P = .051).Conclusion:Both techniques resulted in good to excellent correction. However, the open technique was associated with lower postoperative IMA values and more correction power for IMA and HVA, than the MIS.

Patient-Reported and Radiologic Outcomes With the Lapidus Procedure

Background: This is a retrospective review of patients who have had a Lapidus procedure for hallux valgus deformity correction, performed by a single surgeon. Methods: The clinical records, operation notes and available weightbearing preoperative and postoperative radiographs of all patients who had a Lapidus procedure between February 2008 and July 2018 for hallux valgus deformity were reviewed. The preoperative and postoperative hallux valgus angle (HVA), intermetatarsal angle (IMA), and talometatarsal angle (TMT) on anteroposterior (AP) and lateral radiographs were recorded. The postoperative Foot and Ankle Ability Measure (FAAM) scores were obtained from phone interviews. Results: Two hundred thirteen Lapidus procedures were identified between February 2008 and July 2018 and included in the final analysis. The average preoperative and postoperative HVA was 36.3 and 18 degrees, respectively, with an average improvement of 18.3 degrees. The average TMT angles showed a preoperative and postoperative difference of 1.1 degrees on the AP radiograph and 2.4 degrees on the lateral. The average preoperative and postoperative IMA angles were 16.5 and 7.4 degrees, respectively, with an improvement of 9.1 degrees. There were 9 cases (4.2%) of nonunion confirmed on radiography, of which 3 cases (1.4%) had revision fixation with bone graft and went on to radiologic union. Final FAAM scores were available for 151 patients (70.9%). From this group, 74.8% (113/151) scored higher than 90%, 16.6% (25/151) scored between 70% and 90%, and 8.6% scored between 50% and 70%. The patient satisfaction rate was 92.9% overall. Conclusion: This single-surgeon retrospective review of a relatively large number of Lapidus procedures reports acceptable radiologic correction and patient satisfaction. Bilateral vs Unilateral Minimally Invasive Hallux Valgus Surgery: A Propensity Matched Case-Control Study with 2 Year Clinical Patient Reported Outcomes

Category:Bunion; Midfoot/ForefootIntroduction/Purpose:Hallux valgus deformity correction using a third-generation Minimally Invasive Chevron Akin osteotomy technique (MICA) is becoming increasingly popular. There is limited patient reported outcome measure (PROM) outcomes for patients undergoing bilateral hallux valgus corrective MICA surgery during a single operative case.Methods:Between July 2014 and May 2018, 59 consecutive patients comprising 118 feet underwent bilateral MICA in a single operative case. 1:1 propensity score matching based on preoperative PROM assessment, age, and radiographic deformity data was carried out to match each foot in the bilateral cohort with 118 feet who had undergone unilateral MICA. The primary outcome measure was the Manchester-Oxford Foot Questionnaire (MOXFQ) which was prospectively measured pre-operatively, six months and two years post-operatively. Secondary outcomes included complication rates and radiographic deformity correction with regards to the hallux valgus angle (HVA) and inter-metatarsal angle (IMA).Results:At minimum 2 year follow up, the MOXFQ post-operative scores had significantly improved for every domain (p <0.001) in both unilateral and bilateral cohorts compared to pre-operative assessment. For all domains of the MOXFQ, there was no statistically significant difference in 6 month or two year outcomes between patients undergoing bilateral and unilateral MICA surgery (p >0.05 for all outcome measures). There was no statistically significant difference between the two cohorts in complication rate (p=>0.05) or post-operative IMA or HVA (p >0.05).Conclusion:This study demonstrates that there is no significant difference in patient reported outcome measured at two year follow-up between patients who underwent bilateral versus unilateral MICA. This study should reassure surgeons who perform bilateral hallux valgus surgery in a single case that, in the medium-term, there is no difference in clinical or radiographic PROMs compared to unilateral surgery. There are potentially positive health economic benefits of performing bilateral surgery in a single sitting if clinically indicated.

Hallux Valgus Pronation Correction by Scarf Osteotomy: Prospective Case Series with WB-CT Scan

Coronal metatarsal rotation (pronation) has been recognized as a key element in hallux valgus deformity and an important prognostic factor for recurrence. New surgical techniques have been developed in order to specifically address pronation deformity but tend to be technically demanding. With the advent of weight bearing CT (WBCT), accurate pronation measurement has improved. Deformity correction by means of metatarsal osteotomies has been an effective way to correct transverse plane deformity but its capacity for pronation correction is not well established. Purpose: Assess transverse and coronal plane rotation deformity correction by WBCT in hallux valgus patients after scarf osteotomy.Methods:We prospectively analyzed hallux valgus deformity in patients who underwent Scarf osteotomy for symptomatic hallux valgus with preoperative and at least 3 month postoperative WBCT for comparison. Intermetatarsal angle (IMA), hallux valgus angle (HVA) and alpha angle (AA) for first metatarsal rotation (pronation) was measured by 3 different observers. The intraclass correlation was determined. Pre and postoperative measurement change was established by T-test and Wilcoxon for related samples.Results:40 feet in 25 patients were included. The interclass correlation between observers was 0.9 for AA (pronation), 0.92 for IMA and 0.95 for HVA in preoperative evaluation and 0.94 for AA, 0.65 for IMA and 0.82 for HVA in postoperative evaluation. The mean preoperative and postoperative AA of the first metatarsal was 8.9 degrees (range -8.83-24,40, SD 6.8) and 6.3 degrees (range -10.7-28.4, SD 8.3) respectively, resulting in a significant first ray pronation change of -2.5 degrees (P =.006). The media preoperative and postoperative IMA was 13.1 degrees (SD 2.9) and 4.9 degrees (SD 2.0) respectively, demonstrating a significant change (P <.001). The media preoperative and postoperative HVA was 25 degrees (SD 6.7) and 5.9 degrees (SD 4.2) respectively, demonstrating a significant change (P <.001)Conclusion:The Scarf osteotomy for hallux valgus deformity has a significant power to correct both transverse and coronal plane (pronation) deformity.

Clinical and Radiographic Outcomes of Percutaneous Chevron-Akin Osteotomies for the Correction of Hallux Valgus Deformity.

Hallux valgus is a common cause of pain and dysfunction of the foot, sometimes requiring surgical correction when conservative measures fail. Although there are many methods of correction, one of the newer techniques is minimally invasive chevron-Akin (MICA). The aim of the current study is to evaluate clinical and radiographic effectiveness of MICA and narcotic use in a large patient cohort. All patients in this retrospective study were treated by a single fellowship-trained foot and ankle orthopaedic surgeon. Patient demographics were collected for all cases. Preoperative and postoperative intermetatarsal angle (IMA) and hallux valgus angle (HVA) were measured in all patients on weightbearing 3-views radiographs. The Foot Function Index (FFI) was obtained pre- and postoperatively at each visit. All patients were prescribed regular use of ibuprofen for 3 days with acetaminophen and oxycodone reserved for breakthrough pain. Use of narcotic pain medication was recorded. A total of 274 feet in 248 patients were included in the study. Overall, 87.9% were female and 12.1% were male. The mean preoperative IMA and HVA were 13.4 and 29.1 degrees, respectively. The postoperative IMA and HVA were 4.9 and 8.9 degrees, respectively. The mean FFI score part A was 92 preoperatively and 43 postoperatively. Patient satisfaction was 91.6%. The mean postoperative 5 mg oxycodone pill consumption was 2.2. MICA is good method to correct hallux valgus deformity with low postoperative narcotic use. A Novel Manipulation Technique for Lapidus Fusion in Correction of Hallux Valgus Deformity With Underlying Metatarsus Adductus: A Case Series

Underlying metatarsus adductus (MA) is commonly seen in patients with hallux valgus (HV) deformity, with implications regarding procedure selection and hallux valgus recurrence. Lapidus, or first tarsometatarsal fusion, is commonly performed allowing reduction in intermetatarsal angle (IMA) but this procedure has not been established as an approach to provide partial correction of MA deformity. Retrospective assessment of preoperative and postoperative metatarsus adductus angle (MAA), IMA and hallux abductus angle (HAA) in patients treated with Lapidus fusion for HV. Significance was determined via paired t test with a p value of <.05. All cases involved manual transverse plane manipulation to reduce both IMA and MAA during screw insertion. Intermetatarsal angle and Engel's angle were measured on preoperative AP radiographs to determine the presence of underlying MA in patients undergoing Lapidus fusion for HV. Ten weeks and 1 year postoperative radiographs were measured to determine degree of correction of IMA, HAA, and MAA. Thirty-four patients met inclusion criteria, which is approximately 46% of our sample population. The average preoperative IMA was 19.4˚ (range 12-32) and the average postoperative IMA was 9.7˚ (range 6-14). The average preoperative Engel's angle was 27.4˚ (range 24-34) and the average postoperative Engel's angle was 22.6˚ (range 15-28) with mean improvement in MA of 6.6˚. Of the 34, 27 (79.4%) patients had a normal Engel's angle at 10 weeks postoperatively. All measures of change met level of significance (p < .05). Of the 34 patients, 21 had radiographs taken beyond the 1 year mark (average 53 weeks). These patients were found to have an average Engel's angle of 23.0˚, which is not statistically significantly different from their 10 week measurements. Of the 21 patients, 17 (81%) maintained normal Engel's angle past 1 year. Metatarsus adductus varies regarding degree of reducibility and complicates preoperative angular measurement and correction of HV. Based on these findings, we recommend Lapidus fusion using this specified manipulation technique to obtain comprehensive transverse plane correction.

Modified Lapidus vs Scarf Osteotomy Outcomes for Treatment of Hallux Valgus Deformity.

The Lapidus procedure and scarf osteotomy are indicated for the operative treatment of hallux valgus; however, no prior studies have compared outcomes between the procedures. The aim of this study was to compare clinical and radiographic outcomes between patients with symptomatic hallux valgus treated with the modified Lapidus procedure versus scarf osteotomy. This retrospective cohort study included patients treated by 1 of 7 fellowship-trained foot and ankle surgeons. Inclusion criteria were age older than 18 years, primary modified Lapidus procedure or scarf osteotomy for hallux valgus, minimum 1-year postoperative Patient-Reported Outcomes Measurement Information System (PROMIS) scores, and minimum 3-month postoperative radiographs. Revision cases were excluded. Clinical outcomes were assessed using 6 PROMIS domains. Pre- and postoperative radiographic parameters were measured on anteroposterior (AP) and lateral weightbearing radiographs. Statistical analysis utilized targeted minimum-loss estimation (TMLE) to control for confounders. A total of 136 patients (73 Lapidus, 63 scarf) with an average of 17.8 months of follow-up were included in this study. There was significant improvement in PROMIS physical function scores in the modified Lapidus (mean change, 5.25; P < .01) and scarf osteotomy (mean change, 5.50; P < .01) cohorts, with no significant differences between the 2 groups (P = .85). After controlling for bunion severity, the probability of having a normal postoperative intermetatarsal angle (IMA; <9 degrees) was 25% lower (P = .04) with the scarf osteotomy compared with the Lapidus procedure. Although the modified Lapidus procedure led to a higher probability of achieving a normal IMA, both procedures yielded similar improvements in 1-year patient-reported outcome measures. Short-Term, Retrospective Radiographic Evaluation Comparing Pre- and Postoperative Measurements in the Chevron and Minimally Invasive Distal Metatarsal Osteotomy for Hallux Valgus Correction

Various techniques exist for correction of mild to moderate hallux valgus (HAV) deformity. Recently, minimally invasive distal metatarsal osteotomy (MIDMO) has gained popularity for HAV correction. This retrospective radiographic review aims to report the surgical correction obtained by the chevron and MIDMO osteotomies at a single institution between January 2012 and December 2017. Radiographic parameters, such as intermetatarsal angle (IMA), hallux abductus angle (HAA), and tibial sesamoid position (TSP), were compared on weight-bearing anterior-posterior and lateral radiographs. Sixty-one patients who underwent distal first metatarsal osteotomies were separated into 2 groups. Group A included 30 patients with a chevron bunionectomy performed by Surgeon A; Group B consisted of 31 patients who had MIDMO performed by Surgeon B. Mean follow-up was 26.6 months for Group A and 18.7 months for Group B. Both groups had statistically significant radiographic correction for pre- and postoperative IMA, HAA, and TSP. Group A: IMA measured preoperatively 11.6° ± 4.0° to 6.8° ± 4.1° postoperatively, HAA preoperative 22.2° ± 9.1° to 12.3° ± 6.9° postoperative, and TSP preoperative 1.3 ± 0.9 to 0.7 ± 0.6 postoperative. Group B: IMA measured preoperatively 12.0° ± 2.9° to 5.9° ± 3.3° postoperatively, HAA preoperative 27.9° ± 8.6° to 12.0° ± 6.6° postoperative, and TSP preoperative 2.0 ± 0.8 to 0.7 ± 0.6 postoperative. Postsurgical retrospective radiographic review demonstrated chevron and MIDMO procedures provide comparable radiographic correction of IMA, HAA, and TSP.

Effect of Metatarsus Adductus on Hallux Valgus Treated With Proximal Reverse Chevron Metatarsal Osteotomy.

This study aimed to compare clinical and radiographic outcomes and recurrence rates after reverse proximal chevron metatarsal osteotomy (PCMO) for patients with hallux valgus (HV) with or without metatarsus adductus (MA). We hypothesized that patients with MA would have poorer outcomes and a higher radiographic recurrence rate than those without MA. This retrospective single-surgeon series comprised 144 patients (173 feet) with moderate to severe HV, treated with PCMO and Akin osteotomy without lesser metatarsal procedures, who were grouped according to the presence (MA group) or absence of MA (non-MA group). Clinical assessment included the American Orthopaedic Foot & Ankle Society (AOFAS) score, pain visual analog scale (VAS), and patient satisfaction rating. Radiographic assessments included metatarsus adductus angle (MAA), HV angle (HVA), and intermetatarsal angle (IMA). The prevalence of the MA was 24.2%. The mean MAA was 23.1 ± 3.3 degrees in the MA group. There were no differences in the mean AOFAS score and pain VAS score at the final follow-up between the 2 groups (all P > .05). The patient satisfaction rate was 73.8% in the MA group vs 90.1% in the non-MA group (P = .017). The mean postoperative HVA and IMA significantly improved at the final follow-up in both groups, respectively (all P < .001). Preoperative and postoperative HVA were larger in the MA group vs non-MA group. However, no significant difference was found in the improvement of HVA and IMA after surgery between the 2 groups (all P > .05). The recurrence rate was 28.6% in the MA group and 6.1% in the non-MA group (P < .001). HV patients associated with the MA had a higher degree of preoperative HV, lower correction of the HVA, higher radiographic recurrence rate, and poorer patient satisfaction than those without MA post-PCMO without lesser metatarsal procedures. Therefore, a more extensive HV correction procedure or the addition of a lesser metatarsal realignment procedure may need to be considered. Level III, retrospective comparative series.
