Analysis of the Causes and Conditions for the Formation of a High Incidence of Acute Respiratory Infections (ARI) in the Population of the Altai Krai (Part 1. Features of Long-Term and Intra-Annual Dynamics of ARI Incidence in 2011–2021)

Relevance. The Altai Krai (AK) is among the least prosperous subjects of the Russian Federation in terms of the incidence of acute respiratory infections (ARI).Aim: to analyze the causes and conditions for the formation of a high incidence of ARI among the population of the Altai Krai based on an assessment of the long-term and intra-annual dynamics of the epidemic process in 2011–2021.Materials and methods. In the course of a retrospective epidemiological study according to the annual forms No. 2 of the state statistical observation (SOS) for 2011-2021 a comparative analysis of the incidence of ARI and community-acquired pneumonia (CAP) in various groups of the population of the AK, the Siberian federal district (SFD) and the population of the Russian Federation as a whole was carried out. Data for 2020–2021 («covid» period) was analyzed separately in comparison with the precovid period of 2011–2019. The intra-annual dynamics of the incidence of ARI and CAP in the AK were studied using the method of trend-seasonal decomposition according to the monthly forms No. 2 of the SOS.Results and discussion. It is established that in 2011–2021. the epidemic process of ARI in the AK was characterized by a higher level and rate of increase in the incidence of all population groups (adults and children, urban and rural) compared to the all-Russian and average for the Siberian Federal District; a higher proportion of children in the overall structure of ARI cases, despite the fact that the proportion of the child population did not exceed (or was even less) than in other regions; no decrease in the incidence of ARI in children in the period 2020–2021; the presence of a trend towards an increase in the incidence of ARI in both urban and rural residents in the pre-covid period; higher incidence in the rural population in 2011–2019 compared to the whole of the Russian Federation or the Siberian Federal District. A less pronounced role of the seasonal component in the registration of CAP was revealed, in comparison with ARI. In some years, an increase in the registration of CAP was observed in the spring-summer period. For 2020-2021 there was a close coincidence of the curves of the intra-annual dynamics of the registered incidence of ARI and COVID-19 among the general population, CAP and COVID-19.Conclusion. The obtained results indicate the need to study the possible impact of environmental factors, as well as the shortcomings of the etiological interpretation and differential diagnosis of some natural focal and zoonotic infections that occur with fever and respiratory symptoms on the formation of a high incidence of ARI in the Altai Territory.

Функціонально-екологічна експертиза Шаргородського району Вінницької області

Концепція функціонально-екологічної експертизи (ФЕЕ) регіонів екологічного (радіаційного) контролю принципово доповнює прийняті форми дозиметричного і тиреодозиметричного контролю за рахунок конкретизації стану здоров'я людей на контрольованих територіях. Метою нашої роботи стало за допомогою ФЕЕ провести верифікацію радіаційної залежності функціонального здоров'я (адаптаційного потенціалу) і дозиметричної паспортизації населених пунктів сіл Голинчинцi, Джурин та Рахни Лiсовi Шаргородського району Вінницької області, які належать до зони посиленого радіоекологічного контролю (IV зони). За допомогою функціонально-вегетативної діагностики (ФВД) за методом В. Г. Макаца обстежено 700 дітей різних статевовікових груп, з яких хлопці – 342, дівчата – 358, які постійно проживають в цих населених пунктах. У центрі уваги ФЕЕ є функціональне здоров'я (адаптаційний потенціал) дитячого населення, яке стає "біоіндикатором" індивідуального здоров'я і характеризує екологічну динаміку в регіоні компактного проживання. Екологічним критерієм ФЕЕ конкретного регіону проживання населення є системно залежне зіставлення пригнічення функціональної активності організму (парасимпатичної активності (ПА), вегетативної рівноваги (ВР) та симпатичної активності (СА). На цій основі формується інтегральна характеристика екологічного стану і визначаються рівні (зони) його екологічного тиску. Внаслідок проведення дослідження вперше з'ясовано, що основною характеристикою, яка відображає негативний вплив екологічних факторів, є зменшення кількості обстежених людей у зоні вегетативно-функціональної рівноваги і збільшення їх у зоні парасимпатичної активності. Наші спостереження довели, що функціонально-вегетативне здоров'я дітей екологічно залежне, а його показники є найчутливішими біоіндикаторами екологічного та радіаційного забруднення. Результати проведення ФЕЕ обстежених населених пунктів Шаргородського району збігаються і принципово доповнюють офіційну дозиметричну паспортизацію населених пунктів, що свідчить про її достовірність і ефективність. Моніторинг вегетативного здоров'я (адаптаційного потенціалу) дітей має стати основою сучасної функціональної диспансеризації дитячого населення і доповнювати державну дозиметричну і тиреодозиметричну паспортизацію населених пунктів.

Multiple system atrophy: genetic or epigenetic?

Multiple system atrophy (MSA) is a rare, late-onset and fatal neurodegenerative disease including multisystem neurodegeneration and the formation of α-synuclein containing oligodendroglial cytoplasmic inclusions (GCIs), which present the hallmark of the disease. MSA is considered to be a sporadic disease; however certain genetic aspects have been studied during the last years in order to shed light on the largely unknown etiology and pathogenesis of the disease. Epidemiological studies focused on the possible impact of environmental factors on MSA disease development. This article gives an overview on the findings from genetic and epigenetic studies on MSA and discusses the role of genetic or epigenetic factors in disease pathogenesis.

A model for raw material management as a response to local and global environmental constraints

Identifying the causes of visible changes in the archaeological record is one of the major goals of archaeological and anthropological research. Environmental shifts have been often suggested as one of the primary drivers of anthropological changes, due to their impact on biotic resources. Considerably less attention has been given to changes in the availability of lithic raw materials. If raw materials for tool production were available in constant, predictable amounts then stability or slow shifts of technological strategies would be expected in the archaeological record. However, this is not the case in the Iberian Paleolithic, even considering that some of the observed changes may be driven by culture, setting or function. In this paper, we investigate the possibility of changes in raw material availability through the action of geodynamic processes, which could have exerted selective pressures on the technological strategies employed by human populations. This approach highlights the possible impact of environmental factors on human populations not just through food and habitat, but also through geology.The maritime climates of southwestern Iberia are good candidates for such a connection because the region faced dramatic fluctuations between cold-dry and warm-wet periods during the Late Pleistocene, with well-documented impacts on biotic and geomorphic systems. A link between environmental conditions and human tool-making strategies is suggested by the correlation between cold climate during the last glacial maximum and an abrupt shift in lithic assemblages toward reliable technological strategies. Using the climatic conditions that define this “Reliable Phase” of Upper Paleolithic technology between 17 and 23 ka BP, two climate-driven scenarios are suggested for technological strategies in the Iberian Paleolithic: a cold-dry phase with restricted availability of raw materials favoring a technological shift towards reliability; and a warm-wet phase with greater raw material availability and a more flexible, maintenance-based lithic technology. Applying this reasoning to paleoenvironmental proxy records for southwestern Iberia, it is likely that numerous episodes of climate-induced raw material stress occurred towards the end of the Middle Paleolithic and around the time of the Middle–Upper Paleolithic transition. At present, insufficient data exist to properly test this hypothesis but ongoing investigations at sites throughout Portugal hold promise toward achieving this goal.
