Anatomical variants of acne differ in their impact on social perception.

Acne negatively affects quality of life, however quality-of-life scores poorly correlate with disease severity scores. Previous research demonstrated existence of facial areas in which skin lesions have greater impact on gaze patterns. Therefore, we hypothesized that anatomical variants of acne may be perceived differently. The aim was to investigate effect of anatomical variants of acne on natural gaze patterns and resulting impact on social perception of acne patients. We tracked eye movements of participants viewing neutral and emotional faces with acne. Images were rated for acne-related visual disturbance, and emotional faces were rated for valence intensity. Respondents of an online survey were asked to rate their perception of pictured individuals' personality traits. All faces with acne were perceived as less attractive and received poorer personality judgements with mid-facial acne presenting smallest deviation from healthy faces. T-zone and mixed acne exhibited the least significant difference in respondents gaze behaviour pattern from each other. In addition, there was no significant difference in respondents' grading of acne visual disturbance or ratings for attractiveness, success and trustworthiness. U-zone adult female acne was rated as the most visually disturbing and received the lowest scores for attractiveness. Happy faces with adult female acne were rated as less happy compared to other acne variants and clear-skin faces. Anatomic variants of acne have a distinct impact on gaze patterns and social perception. Adult female acne has the strongest negative effect on recognition of positive emotions in affected individuals, attractiveness ratings and forming social impressions. If perioral acne lesions are absent, frontal lesions determine impact of acne on social perception irrespective of the presence of mid-facial lesions. This perceptive hierarchy should be taken into consideration while deciding treatment goals in acne patients, prioritizing achieving remission in perioral and frontal area.

Recognition of Facial Emotion Expressions in Patients with Depressive Disorders: A Functional MRI Study

The present study evaluated the cortical activation during emotional information recognition. The study group included 16 patients with depression, and 16 healthy subjects were enrolled as a control group. Patients received eight weeks of antidepressant therapy. Functional MRI evaluated the cortical activation twice in the patient group and once in the control group. The fMRI task processed the emotional information with face demonstration from the PennCNP test battery. During the processing of emotional information, patients showed activation in the middle and the inferior frontal gyri, the fusiform gyrus, and the occipital cortex. After treatment, patients showed a significant decrease in the frontal cortex activation for negative face demonstration and no frontal activation for positive emotion recognition. The left superior temporal gyrus activation zone appeared in patients after treatment and in the control group. Healthy subjects showed more intense frontal cortex activation when processing neutral emotions and less when showing happy and sad faces. Activation zones in the amygdala and the insula and deactivation zones in the posterior cingulate cortex were revealed in the controls. This study confirms the hypothesis that anomalies in the processing of emotional stimuli can be a sign of a depressive disorder.

Emotion recognition and baseline cortisol levels relationship in early Alzheimer disease

BackgroundEmotion recognition is often impaired in early Alzheimer’s disease (AD) and can be evaluated using the Reading the Mind in the Eyes Test (RMET). Similarly, cortisol levels can affect cognition and could be considered a biomarker of AD. ObjectivesThe aim of this study was to analyse the relationship between the emotion recognition task and cortisol levels in participants with early Alzheimer Disease (AD). MethodsComplex emotion recognition was assessed with RMET, and plasma cortisol levels were determined by mass spectrometry in participants classified into mild cognitive impairment (MCI) due to AD (n = 25), mild dementia (MD) due to AD (n = 20), MCI non-AD (n = 34), MD non-AD (n = 13) and healthy controls (HC) (n = 16) groups. ResultsSignificantly lower positive emotion recognition was found in the MCI non-AD group (p = 0.02) and lower emotion recognition in MD (AD and non-AD) groups (p < 0.01) compared to the healthy group. In addition, significant differences were observed between cortisol and all RMET scores among the MCI and MD groups (p < 0.01). A significant correlation was also obtained between total and neutral RMET scores and cortisol levels in MD groups (p = 0.01). ConclusionsThese outcomes suggest that detection of positive emotion dysfunction could help to identify MCI non-AD patients. Furthermore, general impaired emotion recognition and high cortisol levels may be associated with cognitive impairment at mild dementia level.

ABSTRACT Positive affect can increase sensitivity to expressions of joy on people’s faces. One of the sources of positive emotions is the awareness of having important and valued attributes, such as agency and communion. The study investigated whether recalling experiencing agency and communion enhanced positive emotion recognition on emotionally ambiguous faces of other people. 180 students (50% women) were randomly divided into three groups: agency activation, communion activation and control group. In the first two conditions, participants were asked to recall a situation when they experienced agency or communion, in the last one they completed a control task. Then, they had to indicate what emotions were presented on faces showing ambiguous emotional expressions. Recalling agency led to the evaluation of ambiguous faces as more joyful compared to the control group, but the results depended on sex of the participants and of the presented face.

Tryptophan-enriched diet or 5-hydroxytryptophan supplementation given in a randomized controlled trial impacts social cognition on a neural and behavioral level

Understanding of emotions and intentions are key processes in social cognition at which serotonin is an important neuromodulator. Its precursor is the essential amino acid tryptophan (TRP). Reduced TRP availability leads to weaker impulse control ability and higher aggression, while TRP supplementation promotes confidence. In a double-blind placebo-controlled fMRI study with 77 healthy adults, we investigated the influence of a 4 week TRP enriched diet and an acute 5-hydroxytryptophan (5-HTP) intake on two social-cognitive tasks, a moral evaluation and an emotion recognition task. With 5-HTP, immoral behavior without negative consequences was rated as more reprehensible. Additionally, during story reading, activation in insula and supramarginal gyrus was increased after TRP intake. No significant effects of TRP on emotion recognition were identified for the whole sample. Importantly, emotion recognition ability decreased with age which was for positive emotions compensated by TRP. Since the supramarginal gyrus is associated with empathy, pain and related information integration results could be interpreted as reflecting stricter evaluation of negative behavior due to better integration of information. Improved recognition of positive emotions with TRP in older participants supports the use of a TRP-rich diet to compensate for age related decline in social-cognitive processes.

High Schizotypy Predicts Emotion Recognition Independently of Negative Affect.

Introduction: Deficits in Emotion Recognition (ER) contribute significantly to poorer functional outcomes in people with schizophrenia. However, rather than reflecting a core symptom of schizophrenia, reduced ER has been suggested to reflect increased mood disorder co-morbidity and confounds of patient status such as medication. We investigated whether ER deficits are replicable in psychometrically defined schizotypy, and whether this putative association is mediated by increased negative affect.Methods: Two hundred and nine participants between the ages of 18 and 69 (66% female) were recruited from online platforms: 80% held an undergraduate qualification or higher, 44% were current students, and 46% were in current employment. Participants were assessed on psychometric schizotypy using the O-LIFE which maps onto the same symptoms structure (positive, negative, and disorganised) as schizophrenia. Negative affect was assessed using the Depression Anxiety and Stress Scale (DASS-21). Emotion Recognition of both positive and negative emotions was assessed using the short version of the Geneva Emotion Recognition Task (GERT-S).Results: Negative schizotypy traits predicted poorer ER accuracy to negative emotions (β = −0.192, p = 0.002) as predicted. Unexpectedly, disorganised schizotypy traits predicted improved performance to negative emotions (β = 0.256, p = 0.007) (primarily disgust). Negative affect was found to be unrelated to ER performance of either valence (both p > 0.591). No measure predicted ER accuracy of positive emotions. Positive schizotypy traits were not found to predict either positive or negative ER accuracy. However, positive schizotypy predicted increased confidence in decisions and disorganised schizotypy predicted reduced confidence in decisions.Discussion: The replication of ER deficits in non-clinical negative schizotypy suggests that the association between negative symptoms and ER deficits in clinical samples may be independent of confounds of patient status (i.e., anti-psychotic medication). The finding that this association was independent of negative affect further suggests ER deficits in patients may also be independent of mood disorder co-morbidity. This association was not demonstrated for the positive symptom dimension of the O-LIFE, which may be due to low levels of this trait in the current sample.

Emotion recognition in individuals with cocaine use disorder: the role of abstinence length and the social brain network.

Emotion recognition is impaired in drug addiction. However, research examining the effects of cocaine use on emotion recognition yield mixed evidence with contradictory results potentially reflecting varying abstinence durations. Therefore, we investigated emotion recognition and its neural correlates in individuals with cocaine use disorder (CUD) parsed according to abstinence duration. Emotion recognition performance was compared between current cocaine users (CUD + , n = 28; cocaine-positive urine), short-term abstainers (CUD-ST, n = 23; abstinence < 6months), long-term abstainers (CUD-LT, n = 20; abstinence ≥ 6months), and controls (n = 45). A sample subset (n = 73) underwent structural magnetic resonance imaging to quantify regional gray matter volume (GMV) using voxel-based morphometry. CUD + demonstrated greater difficulty recognizing happiness than CUD-ST and controls, and sadness and fear compared to controls (p < 0.01). For fear, CUD-ST also performed worse than controls (p < 0.01), while no differences emerged between CUD-LT and controls. Whole-brain analysis revealed lower GMV in the bilateral cerebellum in CUD + compared to CUD-LT and controls; a similar pattern was observed in the amygdala (CUD + < CUD-LT) (pFWE < 0.01). Collapsed across all participants, poorer recognition for happiness was associated with lower right cerebellar GMV (pFWE < 0.05). Emotion recognition is impaired with current cocaine use, and selective deficits (in fear) may persist with up to 6months of abstinence. Lower cerebellar GMV may underlie deficits in positive emotion recognition. Interventions targeting emotional-social-cognitive deficits, especially among active users, may enhance treatment success for individuals with CUD.

Multi-Target Positive Emotion Recognition From EEG Signals

Compared with the widely studied negative emotions in which different classes are easy to distinguish, nowadays less attention is paid to the recognition of positive emotions that are not fully independent. In this paper, we propose to recognize multiple positive emotions by analyzing brain activities and explore the neural representation of different positive emotions. Thirty-seven participants volunteered to participate in our study, in which their brain activities were recorded when watching five selected film clips. First, 150 well-known power features extracted from Electroencephalography (EEG) signals and 105 multimedia content analysis features were collected as the pool of candidate features. Second, based on the collected features, we propose to use a linear model and a nonlinear model to predict the percentage of five positive emotions. Then, percentage values were converted to ranking numbers and Kendall rank correlation coefficients were calculated. Our results showed that (1) ensemble of regressor chains using LSTM as unit regressor obtained both the best regression results and the best Kendall rank correlation coefficient on EEG features merely, and (2) top features from alpha frequency bands of EEG signals could represent different positive emotions. These results demonstrate the effectiveness of selective EEG features on recognizing different positive emotions.

Social Cognition Deficits Are Pervasive across Both Classical and Overlap Frontotemporal Dementia Syndromes

Objectives: Frontotemporal dementia (FTD) syndromes are a complex group of disorders characterised by profound changes in behaviour and cognition. Many of the observed behavioural abnormalities are now recognised to be due to impaired social cognition. While deficits in emotion recognition and empathy are well-recognised in behavioural-variant (Bv)FTD, limited information exists about the nature of social cognitive impairment in the language variant primary progressive aphasia (PPA) that includes progressive non-fluent aphasia (PNFA) and semantic dementia (SD), and in the motor variants FTD amyotrophic lateral sclerosis (FTD-ALS) and FTD progressive supranuclear palsy (FTD-PSP). This prospective study sought to explore the nature and profile of social cognition deficits across the spectrum of FTD. Methods: Sixty patients on the FTD spectrum, i.e., classical (16 with BvFTD and 20 with PPA) and overlap FTD syndromes (13 with FTD-ALS and 11 with FTD-PSP) were evaluated by means of the social cognition tasks, the Interpersonal Reactivity Index (IRI) for empathy, and pictures of facial affect (POFA) for emotion recognition. General cognition and behaviour were also assessed. Results: A significant impairment in emotion recognition and empathy was detected in both the classical and overlap FTD syndromes. The recognition of positive emotions was relatively preserved compared to that of negative emotions. Among the FTD subtypes, maximal impairment of empathy was demonstrated in FTD-PSP. Conclusion: Social cognition impairment is pervasive across the spectrum of FTD disorders, and tests of emotion recognition and empathy are clinically useful to identify the nature of behavioural problems in both classical and overlap FTD. Our findings also have implications for understanding the neural basis of social cognition in FTD.

Relationship between aberrant salience and positive emotion misrecognition in acute relapse of schizophrenia.

Aberrant salience is the incorrect assignment of salience or significance to innocuous stimuli, and been hypothesized to be a central mechanism in the development of psychosis. In addition to aberrant salience, social-cognitive models of psychosis suggest that the way people process information about the self is important in all stages of psychosis. The aim of the present study is to explore the relationship between aberrant salience and emotion processing in schizophrenia patients with psychotic relapse. A sample of 42 patients with relapse was recruited. Aberrant salience was measured with the Aberrant Salience Inventory (ASI). Assessment of social cognition was carried out using the Facial Emotion Identification Test (FEIT). Partial correlations were controlled for possible confounding variables. The ASI factors "increase in meaning" and "heightened cognition" positively correlated with impaired recognition of positive emotions, and ASI total score inversely correlated to time to response to task. Most of incorrect answers corresponded to misclassification of positive emotions. Our findings show that there is evidence for a relationship between aberrant salience and emotion processing during a psychotic episode; we propose that aberrant salience and alterations in emotion processing trigger the loss of modulating feedback from the external world to produce a self-referential mental state.

Development, factorial structure and reliability of the Multidimensional Scale of Emotional Regulation for Adolescents (MSERA): Preliminary analysis

The evaluation of emotion regulation has been focused on the cognitive component and the emotion dysregulation. However, it is also important to include emotion identification and modulation of the response for the evaluation of the regulatory process. The aim of this study was to develop a multidimensional measure of emotion regulation process in adolescents and to examine its factorial structure and reliability. An instrumental study was performed with 887 Mexican adolescents aged 13 to 18. A preliminary version of the Multidimensional Scale of Emotion Regulation for Adolescents (MSERA) was developed based on focus groups and considering the assumptions of the theoretical models. The exploratory factor analysis yielded 56 items distributed in eight factors: Recognition of positive emotions, Expression of positive emotions, Emotional control, Suppression, Cognitive change, Physical reactions, Recognition of negative emotions and Difficulty to regulate. A confirmatory factor analysis using the Robust Unweighted Least Squares, corroborated the initial solution of eight factors and showed acceptable fit indexes, considering 47 items. Ordinal alpha coefficients obtained for the factors of the final model, indicated a good internal consistency (.85-.95). Results suggest that the MSERA can be used as a valid and reliable measure of emotion regulation in adolescents.

Maltreatment and Emotion Recognition Among Brazilian Adolescents.

The experience of maltreatment can impair child development, including changes in the process of emotions recognition, which may result in impairment of social interactions and behavioral disabilities. In order to measure the association between maltreatment and changes on emotion recognition among Brazilian adolescents, the Emotional Recognition Test on Human Faces (ERTHF) was applied to a sample of 50 adolescents who had suffered different intensities and types of abuse. The social and clinical characteristics of the participants were analyzed and, from ERTHF data, the accuracy and response time for the emotion recognition. Males were 60%, with mean age of 13 years and 3 months; 60% were living in shelters. Emotion recognition changes were associated with intensity and types of maltreatment. Physical neglect (48%) was associated with changes in neutral and negative emotions recognition. Emotional neglect (48%) and emotional abuse (46%) were associated with changes in both positive and negative emotions recognition. Physical abuse (38%) was associated with changes in positive emotion recognition only. False recognition of anger was the most common outcome of maltreatment, being associated with physical neglect (p = 0.015) and emotional neglect (p = 0.047). Our results point out to the need to add emotional and facial recognition's rehabilitation interventions to better attend the specific demands of maltreated children and to increase the chances of social and family reintegration.

Jumping for Joy: The Importance of the Body and of Dynamics in the Expression and Recognition of Positive Emotions.

The majority of research on emotion expression has focused on static facial prototypes of a few selected, mostly negative emotions. Implicitly, most researchers seem to have considered all positive emotions as sharing one common signal (namely, the smile), and consequently as being largely indistinguishable from each other in terms of expression. Recently, a new wave of studies has started to challenge the traditional assumption by considering the role of multiple modalities and the dynamics in the expression and recognition of positive emotions. Based on these recent studies, we suggest that positive emotions are better expressed and correctly perceived when (a) they are communicated simultaneously through the face and body and (b) perceivers have access to dynamic stimuli. Notably, we argue that this improvement is comparatively more important for positive emotions than for negative emotions. Our view is that the misperception of positive emotions has fewer immediate and potentially life-threatening consequences than the misperception of negative emotions; therefore, from an evolutionary perspective, there was only limited benefit in the development of clear, quick signals that allow observers to draw fine distinctions between them. Consequently, we suggest that the successful communication of positive emotions requires a stronger signal than that of negative emotions, and that this signal is provided by the use of the body and the way those movements unfold. We hope our contribution to this growing field provides a new direction and a theoretical grounding for the many lines of empirical research on the expression and recognition of positive emotions.

Behavioral and Neuroimaging Evidence for Facial Emotion Recognition in Elderly Korean Adults with Mild Cognitive Impairment, Alzheimer's Disease, and Frontotemporal Dementia.

Background: Facial emotion recognition (FER) is impaired in individuals with frontotemporal dementia (FTD) and Alzheimer’s disease (AD) when compared to healthy older adults. Since deficits in emotion recognition are closely related to caregiver burden or social interactions, researchers have fundamental interest in FER performance in patients with dementia.Purpose: The purpose of this study was to identify the performance profiles of six facial emotions (i.e., fear, anger, disgust, sadness, surprise, and happiness) and neutral faces measured among Korean healthy control (HCs), and those with mild cognitive impairment (MCI), AD, and FTD. Additionally, the neuroanatomical correlates of facial emotions were investigated.Methods: A total of 110 (33 HC, 32 MCI, 32 AD, 13 FTD) older adult participants were recruited from two different medical centers in metropolitan areas of South Korea. These individuals underwent an FER test that was used to assess the recognition of emotions or absence of emotion (neutral) in 35 facial stimuli. Repeated measures two-way analyses of variance were used to examine the distinct profiles of emotional recognition among the four groups. We also performed brain imaging and voxel-based morphometry (VBM) on the participants to examine the associations between FER scores and gray matter volume.Results: The mean score of negative emotion recognition (i.e., fear, anger, disgust, and sadness) clearly discriminated FTD participants from individuals with MCI and AD and HC [F(3,106) = 10.829, p < 0.001, η2 = 0.235], whereas the mean score of positive emotion recognition (i.e., surprise and happiness) did not. A VBM analysis showed negative emotions were correlated with gray matter volume of anterior temporal regions, whereas positive emotions were related to gray matter volume of fronto-parietal regions.Conclusion: Impairment of negative FER in patients with FTD is cross-cultural. The discrete neural correlates of FER indicate that emotional recognition processing is a multi-modal system in the brain. Focusing on the negative emotion recognition is a more effective way to discriminate healthy aging, MCI, and AD from FTD in older Korean adults.

Unique and Interactive Associations Between Maltreatment and Complex Emotion Recognition Deficits and Psychopathic Traits in an Undergraduate Sample.

Psychopathy is defined by affective and interpersonal deficits, deviant lifestyle, and antisocial behaviors. Poor recognition of emotions and childhood maltreatment are two risk factors implicated in psychopathy. The current study examined whether childhood maltreatment and complex emotion recognition deficits showed unique and interactive associations with psychopathic traits among 261 undergraduate students. Results indicate that maltreatment was related to higher general psychopathy scores within a bifactor model comprising a general psychopathy factor and four specific factors tapping underlying dimensions of psychopathy (i.e., affective, interpersonal, lifestyle, and antisocial). A significant interaction emerged whereby maltreatment was related to higher antisocial factor scores among individuals showing poor recognition of positive emotions. In an intriguing interaction, more maltreatment was related to lower interpersonal factor scores among individuals with low/mean levels of neutral emotion recognition. The interaction of positive emotion recognition deficits and maltreatment highlights a potential intervention target among antisocial individuals who have experienced maltreatment.

Problems of Face Recognition in Patients with Behavioral Variant Frontotemporal Dementia.

Introduction:Faces are very special as they are most essential for social cognition in humans. It is partly understood that face processing in its abstractness involves several extra striate areas. One of the most important causes for caregiver suffering in patients with anterior dementia is lack of empathy. This apart from being a behavioral disorder could be also due to failure to categorize the emotions of the people around them.Patients and Methods:Inlusion criteria: DSM IV for Bv FTD Tested for prosopagnosia - familiar faces, famous face, smiling face, crying face and reflected face using a simple picture card (figure 1).Exclusion Criteria:Advanced illness and mixed causes.Observations:46 patients (15 females, 31 males) 24 had defective face recognition. (mean age 51.5),10/15 females (70%) and 14/31males(47. Familiar face recognition defect was found in 6/10 females and 6/14 males. Total- 40%(6/15) females and 19.35%(6/31)males with FTD had familiar face recognition. Famous Face: 9/10 females and 7/14 males. Total- 60% (9/15) females with FTD had famous face recognition defect as against 22.6%(7/31) males with FTD Smiling face defects in 8/10 female and no males. Total- 53.33% (8/15) females. Crying face recognition defect in 3/10 female and 2 /14 males. Total- 20%(3/15) females and 6.5%(2/31) males. Reflected face recognition defect in 4 females.Results:Famous face recognition and positive emotion recognition defect in 80%, only 20% comprehend positive emotions, Face recognition defects are found in only 45% of males and more common in females.Conclusion:Face recognition is more affected in females with FTD There is differential involvement of different aspects of the face recognition could be one of the important factor underlying decline in the emotional and social behavior of these patients. Understanding these pathological processes will give more insight regarding patient behavior.

Emotion recognition specialization and context-dependent risk of anxiety and depression in adolescents.

BackgroundSome adolescents function poorly in apparently benign environments, while others thrive despite hassles and difficulties. The aim of this study was to examine if adolescents with specialized skills in the recognition of either positive or negative emotions have a context-dependent risk of developing an anxiety or depressive disorder during adolescence, depending on exposure to positive or harsh parenting.MethodsData came from a large prospective Dutch population study (N = 1539). At age 11, perceived parental rejection and emotional warmth were measured by questionnaire, and emotion recognition skills by means of a reaction-time task. Lifetime diagnoses of anxiety and depressive disorders were assessed at about age 19, using a standardized diagnostic interview.ResultsAdolescents who were specialized in the recognition of positive emotions had a relatively high probability to develop an anxiety disorder when exposed to parental rejection (Bspecialization*rejection = 0.23, P < 0.01) and a relatively low probability in response to parental emotional warmth (Bspecialization*warmth = −0.24, P = 0.01), while the opposite pattern was found for specialists in negative emotions. The effect of parental emotional warmth on depression onset was likewise modified by emotion recognition specialization (B = −0.13, P = 0.03), but the effect of parental rejection was not (B = 0.02, P = 0.72). In general, the relative advantage of specialists in negative emotions was restricted to fairly uncommon negative conditions.ConclusionsOur results suggest that there is no unequivocal relation between parenting behaviors and the probability to develop an anxiety or depressive disorder in adolescence, and that emotion recognition specialization may be a promising way to distinguish between various types of context-dependent reaction patterns.

'Can you look me in the face?' Short-term SSRI administration reverts avoidant ocular face exploration in subjects at risk for psychopathology.

Anxiety and depression are associated with altered ocular exploration of facial stimuli, which could have a role in the misinterpretation of ambiguous emotional stimuli. However, it is unknown whether a similar pattern is seen in individuals at risk for psychopathology and whether this can be modified by pharmacological interventions used in these disorders. In Study 1, eye gaze movement during face discrimination was compared in volunteers with high vs low neuroticism scores on the Eysenck Personality Questionnaire. Facial stimuli either displayed a neutral, happy, or fearful expression. In Study 2, volunteers with high neuroticism were randomized in a double-blind design to receive the selective serotonin reuptake inhibitor citalopram (20 mg) or placebo for 7 days. On the last day of treatment, eye gaze movement during face presentation and the recognition of different emotional expressions was assessed. In Study 1, highly neurotic volunteers showed reduced eye gaze towards the eyes vs mouth region of the face compared with low neurotic volunteers. In Study 2, citalopram increased gaze maintenance over the face stimuli compared with placebo and enhanced recognition of positive vs negative facial expressions. Longer ocular exploration of happy faces correlated positively with recognition of positive emotions. Individuals at risk for psychopathology presented an avoidant pattern of ocular exploration of faces. Short-term SSRI administration reversed this bias before any mood or anxiety changes. This treatment effect may improve the capacity to scan social stimuli and contribute to the remediation of clinical symptoms related to interpersonal difficulties.

Emotion recognition processing as early predictor of response to 8‐week citalopram treatment in late‐life depression

In subjects with depression, exposure to antidepressants improves recognition of positive emotions. This phenomenon, which occurs early in the course of treatment, has been proposed as the initial step in the mechanism of action to subsequent therapeutic effects of antidepressants. To this date, it has not been well examined among older depressed patients. Older subjects with non-psychotic major depressive disorder were treated with citalopram in an 8-week open-label study. The main predictor of response and remission was the change in emotion recognition between baseline and day 7. Covariates included executive functions, baseline anxiety level, medical comorbidity, level of subjective stress, serum citalopram level, and level of social support. Twenty-seven patients were considered for final analysis. Overall, accuracy of emotion recognition significantly improved between baseline (75%) and day 7 (83%) (X(2) = 34.50, df = 1, p < 0.001). Improvement to identify happy expressions occurred at 25% and 50% intensity with ceiling effect at 0%, 75%, and 100%. Change in emotion processing was marginally significant in predicting antidepressant response at day 56. Multivariate analysis showed that emotion processing is a significant predictor of response and remission when considered along with perceived level of social support. Recognition of mildly intense happy expression, which improved early in the course of citalopram treatment, predicts subsequent antidepressant response and remission when considered along with perception of social support. Further studies would be necessary to examine specific neural substrates in the affective network involved in the acute therapeutic action of antidepressant in late-life depression.
