Prognostic Value of Computed Tomography Texture Features in Patients with Hepatocellular Carcinoma Following Stereotactic Ablative Radiotherapy

Background: Hepatocellular carcinoma (HCC) is the sixth most common cancer worldwide. Cases of HCC in Africa and East Asia account for 80% of all HCC cases around the world. China is one of the countries with a high incidence rate of HCC. Objectives: This case-control study aimed to explore the prognostic value of computed tomography (CT) texture features in patients with HCC following stereotactic ablative radiotherapy (SABR). Patients and Methods: A total of 100 HCC patients, treated with SABR from January 2019 to January 2021, were divided into good prognosis (n = 57) and poor prognosis (n = 43) groups. The patients’ general data and CT texture features were then compared. Factors associated with a poor prognosis were investigated in a multivariate logistic regression analysis. A clinical feature model, a CT texture feature model, and a joint model of clinical features and CT texture features were established, and their prognostic values were evaluated by plotting the receiver operating characteristic (ROC) curves. Moreover, a nomogram prediction model was developed according to the multivariate analysis results, and its prediction efficiency was assessed. Results: Age ≤ 40 years, serum alpha-fetoprotein level > 9 ng/mL, gamma-glutamyl transpeptidase > 60 U/L, aspartate aminotransferase > 40 U/L, lesion size > 5 cm, unsmooth tumor margins, no tumor capsule or incomplete capsule, multiple tumors, portal phase CT value of cancer > 135%, and a relative washout ratio > -24% in the portal phase of cancer were risk factors for a poor prognosis in HCC patients after SABR. The area under the ROC curve and sensitivity and specificity of the joint model were 0.817 (95% confidence interval [CI]: 0.773 - 0.861, P < 0.001), 80.47%, and 91.05%, respectively, which significantly exceeded those of the other two models. The nomogram prediction model showed high accuracy and validity. Conclusion: The texture features of CT images before SABR are of a high prognostic value for HCC patients and contribute to the selection of appropriate treatment protocols.

Non-Rigid Liver Registration in Liver Computed Tomography Images Using Elastic Method with Global and Local Deformations

The study of follow-up liver computed tomography (CT) images is required for the early diagnosis and treatment evaluation of liver cancer. Although this requirement has been manually performed by doctors, the demands on computer-aided diagnosis are dramatically growing according to the increased amount of medical image data by the recent development of CT. However, conventional image segmentation, registration, and skeletonization methods cannot be directly applied to clinical data due to the characteristics of liver CT images varying largely by patients and contrast agents. In this paper, we propose non-rigid liver segmentation using elastic method with global and local deformation for follow-up liver CT images. To manage intensity differences between two scans, we extract the liver vessel and parenchyma in each scan. And our method binarizes the segmented liver parenchyma and vessel, and performs the registration to minimize the intensity difference between these binarized images of follow-up CT images. The global movements between follow-up CT images are corrected by rigid registration based on liver surface. The local deformations between follow-up CT images are modeled by non-rigid registration, which aligns images using non-rigid transformation, based on locally deformable model. Our method can model the global and local deformation between follow-up liver CT scans by considering the deformation of both the liver surface and vessel. In experimental results using twenty clinical datasets, our method matches the liver effectively between follow-up portal phase CT images, enabling the accurate assessment of the volume change of the liver cancer. The proposed registration method can be applied to the follow-up study of various organ diseases, including cardiovascular diseases and lung cancer.

Radiomic Feature-Based Predictive Model for Microvascular Invasion in Patients With Hepatocellular Carcinoma.

ObjectiveThis study aimed to build and evaluate a radiomics feature-based model for the preoperative prediction of microvascular invasion (MVI) in patients with hepatocellular carcinoma.MethodsA total of 145 patients were retrospectively included in the study pool, and the patients were divided randomly into two independent cohorts with a ratio of 7:3 (training cohort: n = 101, validation cohort: n = 44). For a pilot study of this predictive model another 18 patients were recruited into this study. A total of 1,231 computed tomography (CT) image features of the liver parenchyma without tumors were extracted from portal-phase CT images. A least absolute shrinkage and selection operator (LASSO) logistic regression was applied to build a radiomics score (Rad-score) model. Afterwards, a nomogram, including Rad-score as well as other clinicopathological risk factors, was established with a multivariate logistic regression model. The discrimination efficacy, calibration efficacy, and clinical utility value of the nomogram were evaluated.ResultsThe Rad-score scoring model could predict MVI with the area under the curve (AUC) of 0.637 (95% CI, 0.516–0.758) in the training cohort as well as of 0.583 (95% CI, 0.395–0.770) in the validation cohort; however, the aforementioned discriminative approach could not completely outperform those existing predictors (alpha fetoprotein, neutrophilic granulocyte, and preoperative hemoglobin). The individual predictive nomogram which included the Rad-score, alpha fetoprotein, neutrophilic granulocyte, and preoperative hemoglobin showed a better discrimination efficacy with AUC of 0.865 (95% CI, 0.786–0.944), which was higher than the conventional methods’ AUCs (nomogram vs Rad-score, alpha fetoprotein, neutrophilic granulocyte, and preoperative hemoglobin at P < 0.001, P = 0.025, P < 0.001, and P = 0.001, respectively). When applied to the validation cohort, the nomogram discrimination efficacy was still outbalanced those above mentioned three remaining methods (AUC: 0.705; 95% CI, 0.537–0.874). The calibration curves of this proposed method showed a satisfying consistency in both cohorts. A prospective pilot analysis showed that the nomogram could predict MVI with an AUC of 0.844 (95% CI, 0.628–1.000).ConclusionsThe radiomics feature-based predictive model improved the preoperative prediction of MVI in HCC patients significantly. It could be a potentially valuable clinical utility.

CT Image-Based Texture Analysis to Predict Microvascular Invasion in Primary Hepatocellular Carcinoma.

The objective of this study was to determine the clinical value of computed tomography (CT) image-based texture analysis in predicting microvascular invasion of primary hepatocellular carcinoma (HCC). CT images of patients with HCC from May 2017 to May 2019 confirmed by surgery and histopathology were retrospectively analyzed. Image features including tumor margin, tumor capsule, peritumoral enhancement, hypoattenuating halo, intratumoral arteries, and tumor-liver differences were assessed. All patients were divided into microvascular invasion (MVI)-negative group (n = 34) and MVI-positive group (n = 68). Preoperative CT images were further imported into MaZda software, where the regions of interest of the lesions were manually delineated. Texture features of lesions based on pre-contrast, arterial, portal, and equilibrium phase CT images were extracted. Thirty optimal texture parameters were selected from each phase by Fisher's coefficient (Fisher), classification error probability combined with average correlation coefficient (POE+ACC), and mutual information (MI). Finally, receiver operating characteristic curve analysis was performed. The results showed that the Edmonson-Steiner grades, tumor size, tumor margin, and intratumoral artery characteristics were significantly different between the two groups (P = 0.012, < 0.001, < 0.001, = 0.003, respectively). There were 58 parameters with significant differences between the MVI-negative and MVI-positive groups (P < 0.001 for all). Among them, 12, 14, 17, and 15 parameters were derived from the pre-contrast phase, arterial phase, portal phase, and equilibrium phase respectively. According to the ROC analysis, optimal texture parameters based on the pre-contrast, arterial, portal, and equilibrium phases were 135dr_GLevNonU (AUC, 0.766; the cutoff value, 1055.00), Vertl_RLNonUni (AUC, 0.764; the cutoff value, 5974.38), 45dgr_GLevNonU (AUC, 0.762; the cutoff value, 924.34), and Vertl_RLNonUni (AUC, 0.754; the cutoff value, 4868.80), respectively. Texture analysis of preoperative CT images may be used as a non-invasive method to predict microvascular invasion in patients with primary hepatocellular carcinomas, and further to guide the treatment and evaluate prognosis. The most valuable parameters were derived from the gray-level run-length matrix.

THE VALUATIONS OF COMPUTED TOMOGRAPHY IN NON TRAUMATIC ACUTE ABDOMINAL PAIN AT HUE&amp;NBSP;UNIVERSITY OF MEDICINE AND PHARMACY HOSPITAL

Background: Acute abdominal pain (AAB) accounts for 4-10% of hospitalizations at the emergency department. Diagnostic imagings, especially computed tomography (CT) plays an important role for etiological diagnosis and management, improve the accuracy from 71% to 93%. The increasing of indications and wide availability of the method increases ionizing radiation exposure. Purposes: Emphasing the importance of CT for AAB and evaluating the diagnosed value of portal single phase CT which aims at reducing radiation exposure. Materials and methods: A study on 90 patients with non traumatic AAB admitted to Hue University of Pharmacy and Medicine Hospital from July 2019 to March 2020. The CT results were correlated with surgical or final diagnosis. Results: Frequent diagnoses were acute appendicitis (20%), diverticulitis (17.8%), urinary stones (17.8%) and small or large bowel obstructions (14.4%). The sensitivity and specificity of multiphasic CT are 96.4% and 100%; the sensitivity and specificity of ultrasound and portal single phase CT combination are 97.6% and 100. The diagnostic match of the two protocols achieved a high value of 98.9%. DLP redution is 67%. Conclusions: CT has a high sensitivity and specificity in AAP etiological diagnosic. Combination of ultrasound and portal single phase CT is useful in diagnosis some AAP cases. Evaluation of Texture Analysis for the Differential Diagnosis of Mass-Forming Pancreatitis From Pancreatic Ductal Adenocarcinoma on Contrast-Enhanced CT Images.

Purpose: To investigate the potential of computed tomography (CT) imaging features and texture analysis to differentiate between mass-forming pancreatitis (MFP) and pancreatic ductal adenocarcinoma (PDAC).Materials and Methods: Thirty patients with pathologically proved MFP and 79 patients with PDAC were included in this study. Clinical data and CT imaging features of the two lesions were evaluated. Texture features were extracted from arterial and portal phase CT images using commercially available software (AnalysisKit). Multivariate logistic regression analyses were used to identify relevant CT imaging and texture parameters to discriminate MFP from PDAC. Receiver operating characteristic curves were performed to determine the diagnostic performance of predictions.Results: MFP showed a larger size compared to PDAC (p = 0.009). Cystic degeneration, pancreatic ductal dilatation, vascular invasion, and pancreatic sinistral portal hypertension were more frequent and duct penetrating sign was less frequent in PDAC compared to MFP. Arterial CT attenuation, arterial, and portal enhancement ratios of MFP were higher than PDAC (p < 0.05). In multivariate analysis, arterial CT attenuation and pancreatic duct penetrating sign were independent predictors. Texture features in arterial phase including SurfaceArea, Percentile40, InverseDifferenceMoment_angle90_offset4, LongRunEmphasis_angle45_offset4, and uniformity were independent predictors. Texture features in portal phase including LongRunEmphasis_angle135_offset7, VoxelValueSum, LongRunEmphasis_angle135_offset4, and GLCMEntropy_angle45_offset1 were independent predictors. Areas under the curve of imaging feature-based, texture feature-based in arterial and portal phases, and the combined models were 0.84, 0.96, 0.93, and 0.98, respectively.Conclusions: CT texture analysis demonstrates great potential to differentiate MFP from PDAC.

Optimal CT protocol for the diagnosis of active bleeding in abdominal trauma patients

ObjectivesThe aim of this study is to compare the radiologic diagnostic performance of arterial phase, portal phase and combined phase computed tomography (CT) for traumatic abdominal injury. In addition, this study is attempted to decrease lifetime attributable risks (LARs) of cancer due to radiation exposure by using optimal CT protocol. Materials and methodsA total of 114 consecutive patients with a traumatic abdominal injury and an abdominal hematoma on CT were enrolled at a single tertiary regional trauma center between January 2016 and March 2017. Each CT protocol set was independently reviewed by three radiologists, and the diagnostic performance of all three CT phases were compared with regard to the capability to detect active bleeding, contained vascular injuries, and organ injuries. Additionally, LARs for cancer incidence and mortality were calculated using dose-length product values, for each phase of CT. ResultsThe pooled area under the receiver operating characteristic curves for the diagnosis of active bleeding, contained vascular injuries, and organ injuries ranged from 0.910 to 0.922, 0.643 to 0.723, and 0.948 to 0.915 for arterial, portal, and combined phase CT, respectively. There was no statistically significant difference in the diagnosis of active bleeding and organ injuries for any combination of two phase sets. The mean LARs for cancer incidence was 0.059%, 0.062% and 0.121% during arterial, portal and combined phase CT, respectively. ConclusionSingle phase CT could be a potential protocol for abdominal trauma patients. Use of single phase CT could significantly decrease the incidence of radiation-associated cancer in the future.

Task-Driven Dictionary Learning Based on Mutual Information for Medical Image Classification.

We present a novel variant of the bag-of-visual-words (BoVW) method for automated medical image classification. Our approach improves the BoVW model by learning a task-driven dictionary of the most relevant visual words per task using a mutual information-based criterion. Additionally, we generate relevance maps to visualize and localize the decision of the automatic classification algorithm. These maps demonstrate how the algorithm works and show the spatial layout of the most relevant words. We applied our algorithm to three different tasks: chest x-ray pathology identification (of four pathologies: cardiomegaly, enlarged mediastinum, right consolidation, and left consolidation), liver lesion classification into four categories in computed tomography (CT) images and benign/malignant clusters of microcalcifications (MCs) classification in breast mammograms. Validation was conducted on three datasets: 443 chest x-rays, 118 portal phase CT images of liver lesions, and 260 mammography MCs. The proposed method improves the classical BoVW method for all tested applications. For chest x-ray, area under curve of 0.876 was obtained for enlarged mediastinum identification compared to 0.855 using classical BoVW (with p-value 0.01). For MC classification, a significant improvement of 4% was achieved using our new approach (with p-value = 0.03). For liver lesion classification, an improvement of 6% in sensitivity and 2% in specificity were obtained (with p-value 0.001). We demonstrated that classification based on informative selected set of words results in significant improvement. Our new BoVW approach shows promising results in clinically important domains. Additionally, it can discover relevant parts of images for the task at hand without explicit annotations for training data. This can provide computer-aided support for medical experts in challenging image analysis tasks.

Computed tomography texture analysis to facilitate therapeutic decision making in hepatocellular carcinoma.

This study explored the potential of computed tomography (CT) textural feature analysis for the stratification of single large hepatocellular carcinomas (HCCs) > 5 cm, and the subsequent determination of patient suitability for liver resection (LR) or transcatheter arterial chemoembolization (TACE). Wavelet decomposition was performed on portal-phase CT images with three bandwidth responses (filter 0, 1.0, and 1.5). Nine textural features of each filter were extracted from regions of interest. Wavelet-2-H (filter 1.0) in LR and wavelet-2-V (filter 0 and 1.0) in TACE were related to survival. Subsequently, LR and TACE patients were divided based on the wavelet-2-H and wavelet-2-V median at filter 1.0 into two subgroups (+ or −). LR+ patients showed the best survival, followed by LR-, TACE+, and TACE-. We estimated that LR+ patients treated using TACE would exhibit a survival similar to TACE- patients and worse than TACE+ patients, with a severe compromise in overall survival. LR was recommended for TACE- patients, whereas TACE was preferred for LR- and TACE+ patients. Independent of tumor size, CT textural features showed positive and negative correlations with survival after LR and TACE, respectively. Although further validation is needed, texture analysis demonstrated the feasibility of using HCC patient stratification for determining the suitability of LR vs. TACE.

Assessment of a Model-Based, Iterative Reconstruction Algorithm (MBIR) Regarding Image Quality and Dose Reduction in Liver Computed Tomography

The purpose of this study was to assess the image quality of half-dose (HD) liver computed tomography (CT) using a model-based iterative reconstruction algorithm (MBIR) compared with reference dose (RD) using filtered back projection (FBP) and the HD CT images using FBP and adaptive statistical iterative reconstruction (ASIR). A total of 103 patients suspected of having liver metastases underwent liver CT including HD portal venous phase imaging. Among these patients, 73 had undergone RD liver CT reconstructed using FBP, and the HD portal phase CT scans were separately reconstructed using FBP and MBIR. For the other 30 patients, the HD CT images were reconstructed using FBP, ASIR, and MBIR. The CT attenuation coefficients and the mean image noise of various sites, including the liver, the aorta, the main portal vein (MPV), and the subcutaneous fat, were measured, and the contrast-to-noise ratios (CNRs) of the metastatic lesion to the liver and the MPV to the liver were calculated. Two radiologists reviewed each image set with regard to image noise, image quality, lesion conspicuity, and diagnostic acceptability. Compared with RD CT, there was a 46.1% decrease in CT dose index volume with HD CT. Image noise was significantly lower in the HD images reconstructed with MBIR than in both the HD FBP images and the RD FBP images (P < 0.001). Compared with the RD FBP and HD FBP images, the CNRs of the metastatic lesion to the liver and the MPV to the liver were higher in the HD MBIR images (P < 0.001). Despite the presence of the unique whirling artifacts of the MBIR images, the HD MBIR images were of good to excellent quality and were not inferior to RD FBP images regarding the lesion conspicuity, the image quality, and the diagnostic acceptability (P > 0.05). Half-dose MBIR also showed less image noise, higher CNRs, and superior image quality compared with HD ASIR (P < 0.001). The HD MBIR images showed less noise, higher CNR, and better image quality than the HD ASIR and HD FBP images did; they also provided less image noise, higher CNR, and similar image quality compared with those of RD FBP images.

Is dual-phase abdominal CT necessary for the optimal detection of metastases from renal cell carcinoma?

To determine whether dual-phase abdominal computed tomography (CT) detected more metastases than portal-phase CT alone in patients with renal cell carcinoma (RCC). Audit committee approval was obtained. A retrospective audit was undertaken in 100 patients who underwent both arterial and portal phase CT. The CT images were independently reviewed by two consultant radiologists. The presence of metastases in the liver, pancreas, and contralateral kidney were recorded for each phase of contrast enhancement. Metastases were identified in the liver in 27 patients, pancreas in 12, and contralateral kidney in 23 patients. Nine of the 27 (33%) liver metastases, three of the 12 (25%) pancreatic metastases, and two of the 23 (9%) renal metastases were only detected in the arterial phase, whilst four of the 27 (15%) liver metastases, three of the 12 (25%) pancreatic metastases, and two of the 23 (9%) renal metastases were only detected in the portal phase. Nine patients (9%) had metastases only visualized in the arterial phase, and six (6%) only in the portal phase. Detection of metastases only visible in the arterial phase led to a change of management in two patients (2%). The audit results support our current standard of dual-phase abdominal CT for optimal detection of RCC metastases.

Simple weight-based contrast dosing for standardization of portal phase CT liver enhancement

To investigate the use of a weight-based volume of contrast media to optimize portal enhancement in patients undergoing abdominal computed tomography (CT). Thirty-one patients were assessed to establish whether a relationship existed between their weight and the portal liver enhancement achieved. Three methods of estimating weight were evaluated to establish which was the most appropriate to use in clinical practice. One hundred patients were then examined using 100 ml contrast media and 100 further patients using a weight-based contrast volume as dictated by a look-up table. The enhancement achieved by each technique was assessed. A good correlation was shown between patient weight and contrast enhancement when a fixed volume of contrast media was used (r=-0.825, p<0.0001). Asking the patient was shown to be the most appropriate method for estimating their weight. The mean portal liver enhancement using the fixed dose and weight-adjusted dose were 110 HU (SD=25.1) and 108 HU (SD=11.9), respectively. Weight-adjusted dose brought 37% more patients into the "ideal" enhancement range of 100-125 HU. The use of a simple, practical, weight-based look-up table to decide contrast media volumes during portal phase liver CT can greatly reduce inter-patient variability compared to a fixed-volume technique.

Diagnostic performance of contrast-enhanced computed tomography in the immediate assessment of radiofrequency ablation success in colorectal liver metastases

To determine the accuracy of contrast-enhanced multislice computed tomography (CT) in the assessment of treatment success immediately after CT-guided radiofrequency (RF) ablation. 26 patients with 38 Colorectal liver metastasis (CRM) were treated by CT-guided RF ablation. Pre-contrast and portal phase CT features before and immediately after ablation were retrospectively evaluated quantitatively and qualitatively: Influence of attenuation characteristics, safety margin, congruency between tumor and coagulation, and morphological criteria (shape, margin distinction, margin configuration, and margin continuity) were investigated. Findings were statistically analyzed with regard to local tumor progression. Mean observation period for follow-up scans was 6.4 months (range: 3-40 months). Attenuation characteristics, safety margin, and congruency had no significant effect on the probability of local tumor progression. Coagulations whose margin was categorized as "discontinuous" were significantly more often associated with local recurrence (p = 0.038). No significant influence on local recurrence could be detected regarding coagulation shape, margin distinction, and configuration. Computed tomography imaging immediately after RF ablation allows for morphological characterization of the coagulation and provides a valid baseline status for follow-up imaging. However, in CRM, morphological image criteria and attenuation characteristics have limited predictive value for immediate detection of persistent tumor.

Hepatic Enhancement in Colorectal Cancer: Texture Analysis Correlates with Hepatic Hemodynamics and Patient Survival

Perfusion imaging of the liver has attracted interest as a potential means for earlier detection of hepatic metastases, but the techniques are complex and do not form part of routine imaging protocols. This study assesses whether the hemodynamic status of the liver of patients with colorectal cancer but apparently normal hepatic morphology is reflected by texture features within a single portal-phase contrast enhanced computed tomography (CT) image and correlates texture with overall survival. Portal-phase CT images from 27 patients with colorectal cancer but no apparent hepatic metastases were processed using a band-pass filter that highlighted image features at different spatial frequencies. A range of parameters reflecting liver texture on filtered images were correlated against CT hepatic perfusion index (HPI) and patient survival. After image filtration, entropy values from hepatic regions were inversely correlated with HPI (r=-0.503978, P=.007355), and directly correlated with survival (r=0.489642, P=.009533). An entropy value below 2.0 identified four patients who died within 36 months of their CT scan with sensitivity 100% and specificity 65% (P<.03). The hemodynamic status of the liver is reflected by subtle changes in coarse texture on portal phase images that can be revealed by texture analysis. Texture analysis has the potential to identify colorectal cancer patients with an apparently normal portal phase hepatic CT but reduced survival.

In Search of Biologic Correlates for Liver Texture on Portal-Phase CT

The acceptance of computer-assisted diagnosis (CAD) in clinical practice has been constrained by the scarcity of identifiable biologic correlates for CAD-based image parameters. This study aims to identify biologic correlates for computed tomography (CT) liver texture in a series of patients with colorectal cancer. In 28 patients with colorectal cancer, total hepatic perfusion (THP), hepatic arterial perfusion, and hepatic portal perfusion (HPP) were measured using perfusion CT. Hepatic glucose use was also determined from positron emission tomography (PET) and expressed as standardized uptake value (SUV). A hepatic phosphorylation fraction index (HPFI) was determined from both SUV and THP. These physiologic parameters were correlated with CAD parameters namely hepatic densitometry, selective-scale, and relative-scale texture features in apparently normal areas of portal-phase hepatic CT. For patients without liver metastases, a relative-scale texture parameter correlated inversely with SUV (r = -0.587, P = .007) and, positively with THP (r = 0.512, P = .021) and HPP (r = 0.451, P = .046). However, this relative texture parameter correlated most significantly with HPFI (r = -0.590, P = .006). For patients with liver metastases, although not significant an opposite trend was observed between these physiologic parameters and relative texture features (THP: r < -0.4, HPFI: r > 0.35). Total hepatic blood flow and glucose metabolism are two distinct but related biologic correlates for liver texture on portal phase CT, providing a rationale for the use of hepatic texture analysis as a indicator for patients with colorectal cancer.

Diagnostic accuracy of portal-phase CT and MRI with mangafodipir trisodium in detecting liver metastases from colorectal carcinoma

To compare the diagnostic accuracy of single section spiral computed tomography (CT) and magnetic resonance imaging (MRI) with tissue-specific contrast agent mangafodipir trisodium (MnDPDP) in the detection of colorectal liver metastases. One hundred and twenty-five consecutive patients undergoing surgery for primary and/or metastatic disease were evaluated using CT (5 mm collimation and reconstruction interval, pitch 2), two-dimensional fast spoiled gradient echo (2D FSPGR) T1 and single shot fast-spin echo (SSFSE) T2 weighted breath-hold MRI sequences, performed before and after intravenous administration of MnDPDP. The reference standards were intraoperative ultrasound and histology. The per-patient accuracy of CT was 72.8 versus 78.4% for unenhanced MRI (p = 0.071) and 82.4% for MnDPDP-enhanced MRI (p = 0.005). MnDPDP-enhanced MRI appeared to be more accurate than unenhanced MRI but this was not significant (p = 0.059). The sensitivity of CT was 48.4% versus 58.1% for unenhanced MRI (p = 0.083) and 66.1% for MnDPDP-enhanced MRI (p = 0.004). The difference in specificity between procedures was not significant. The per-lesion sensitivity was 71.7, 74.9 and 82.7% for CT, unenhanced MRI, and MnDPDP-enhanced MRI, respectively; the positive predictive value of the procedures was respectively 84.0, 96.0 and 95.8%. MnDPDP-enhanced MRI provided a high level diagnostic confidence in 92.5% of the cases versus 82.5% for both unenhanced MRI and CT. The kappa value for inter-observer variability was >0.75 for all procedures. The diagnostic accuracy and sensitivity of MnDPDP-enhanced MRI is significantly higher than single section spiral CT in the detection of colorectal cancer liver metastases; no significant difference in diagnostic accuracy was observed between unenhanced MRI and MnDPDP-enhanced MRI.

Eosinophilic Hepatic Necrosis

To compare the findings of magnetic resonance (MR) imaging with those of computed tomography (CT) of focal liver lesions related to peripheral eosinophilia. For 12 patients with peripheral eosinophilia (>7%) examined with hepatic MR imaging and CT, 52 focal hepatic lesions larger than 0.5 cm, including 31 lesions simultaneously found on the 2 imaging modalities, were subjected to a comparative analysis of their imaging features. The total number of lesions distinguished from background liver was 39 (75%) on MR imaging and 44 (85%) on CT scans. On arterial phase images of 10 patients with comparable data, homogeneously hyperintense lesions were demonstrated more frequently (P = 0.006) on MR imaging (16 [50%] of 32 lesions) than on CT scans (4 [13%] of 32 lesions). Only 7 (22%) of the 32 hypoattenuating lesions on portal phase CT were depicted as hypointense lesions on portal phase MR images in 12 patients. On delayed phase images in 8 patients, the number of hyperintense lesions on MR images (9 [56%] of 16) was greater (P = 0.077) than that seen on the CT scans (4 [25%] of 16). For many focal hepatic lesions related to peripheral eosinophilia, dynamic MR imaging more easily demonstrates lesional enhancement on arterial and delayed phases than CT scans. Because of the higher degree of lesional enhancement of MR imaging compared with CT, the lesion-to-liver contrast may not be sufficient to distinguish the lesion from the background liver, resulting in decreased sensitivity of portal phase dynamic MR imaging.

Arterioportal shunt: prevalence in small hemangiomas versus that in hepatocellular carcinomas 3 cm or smaller at two-phase helical CT.

To compare the prevalence of arterioportal (AP) shunting associated with (a) small (< or =3 cm) hemangiomas and (b) hepatocellular carcinomas (HCCs) (< or =3 cm) at two-phase helical computed tomography (CT). Two-phase helical liver CT was performed in 107 patients (61 men, 46 women; age range, 25-73 years; mean, 48.6 years) with 169 small hemangiomas and in 384 patients (292 men, 92 women; age range, 18-82 years; mean, 58.3 years) with 598 HCCs 3 cm or smaller. Diagnosis of HCC was verified with histologic findings (n = 30) or typical imaging and clinical findings (n = 568); that of all hemangiomas was verified with typical imaging and clinical findings. Three radiologists retrospectively reviewed all CT images in consensus. Contrast material-enhanced CT scans were obtained during the hepatic arterial and portal venous phases. AP shunt was considered to be present when wedge-shaped or irregularly shaped homogeneous enhancement peripheral to tumor appeared at hepatic arterial phase CT and isoattenuation or slight hyperattenuation in that area appeared at portal phase CT. The prevalence of AP shunting associated with hemangiomas and that associated with HCCs were compared with multivariate model testing. Speed of lesion enhancement (rapid enhancement, when extent of intratumoral enhancement at hepatic arterial phase CT was >50%; slow enhancement, when extent of intratumoral enhancement was < or =50%) and presence of AP shunt were correlated with chi2 or Fisher exact testing. AP shunts were more frequently found in hemangiomas (36 lesions [21.3%]) than in HCCs (25 lesions [4.2%]) (P <.001). Twenty-four (38%) of the 64 hemangiomas with rapid enhancement had AP shunts, whereas only 12 (11.4%) of the 105 hemangiomas with slow enhancement had AP shunts (P <.001). There was no significant difference between prevalence of AP shunt in the 573 HCCs with rapid enhancement (24 lesions, 4.2%) and that in the 25 HCCs with slow enhancement (one lesion, 4.0%). AP shunts were more frequently seen at two-phase helical CT in small hepatic hemangiomas than in HCCs and thus represent a suggestive but not specific finding of hemangioma. Small hemangiomas with AP shunts tend to show rapid rather than slow enhancement.
