708-P: Hypertension Control among People with Diabetes Using a Self-Management Multicondition Digital Platform

Introduction: Digital multicondition platforms seek to assist persons with diabetes in management of their diabetes and comorbidities. Hypertension is reported in over two-thirds of patients with type 2 diabetes. Both are major risk factors for cardiac, stroke, and kidney disorders. Dario’s digital therapeutics platform for chronic conditions supports self-management of diabetes and high blood pressure. The platform enables digital evaluation of blood glucose and blood pressure levels in real time and supports interventions based on clinical indicators. Method: A retrospective data evaluation was performed on the Dario data base since 2019. A population of active users who started with hypertension stage 1 as baseline was identified. Blood glucose and blood pressure readings were assessed at first month and 6th month of use. A subgroup of users who started at hypertension stage 2 was evaluated as well. Systolic, diastolic pressures and average blood glucose were evaluated along of the study. Results: For the 2554 users identified with diabetes and hypertension, more than two thirds improved their systolic blood pressure by 13 mmHg (P<0.001; 144±14 to 131±13) . A group of 38.7% (N=990) moved to a lower hypertensive stage (P<0.001) . The subset of 1367 users with stage 2 hypertension improved their systolic blood pressure from 150±12.4 to 141±15.2 mmHg on average and 43.9% (N=600) improved their blood pressure by more than mmHg over six months (P<0.001) . A subgroup of 3users who started at high-risk blood glucose levels significantly reduced their blood glucose average by 15% over 6 months (232.4±46 to 198±65 mg/dL) (P<0.001) . Conclusion: A multicondition digital therapeutic platform may promote behavioral modifications and result in sustainable improvements in both glycemic control and blood pressure levels. 860-P: Users with Type 2 Diabetes Using a Digital Platform Experienced Sustained Improvement in Blood Glucose Levels

Introduction: Reduction of high readings and maintaining blood glucose target levels (fasting and post-meal) are great challenges in managing diabetes. Digital engagement can play a key role in enhancing patient understanding of their condition and helping them make better decisions regarding their actual situation. The Dario platform allows for real time evaluation of blood glucose levels change over time and for a precise intervention based on actual behavioral and clinical indicators. Method: A retrospective data evaluation (Q1:2018-2019) was performed on the Dario® data base. A population of active users (18 measurements per month with the Dario® System on average) with type 2 diabetes (T2D), non-Insulin treated was evaluated over a full year. High blood glucose readings (180-400 mg/dL, >250 mg/dL), fasting readings (<126 mg/dL) and post-meal readings (<180mg/dL) ratios were assessed in their first month of use until the 12th month. Results: For 9,200 users with T2D, non-Insulin users, the average ratio of high glycemia events (180-400 mg/dL) from entire set of measurements was reduced by 26% (18.62% vs. 23.43%) while readings of >250mg/dL were reduced by 33% (4.65% vs. 6.93%) over a year. Fasting measurements analysis revealed an increase of 16% in ratios of readings <126 mg/dL per entire set of fasting measurements (40.59% vs. 34.92%) on average. Post-meal readings ratio of <180 mg/dL per entire post-meal measurements increased by 5% (73.75% vs. 70.42%) on average over a year. Conclusion: Digital diabetes platform results in a large population are consistent with previous studies and show the potential to promote behavior modification in users with T2D. The study demonstrates that digital management platform may assist user to better control their blood glucose levels and sustain behavioral change. This observational data presents an improvement in high glycemia readings ratios, and an increase in prediabetes fasting blood glucose levels sustained over one year. Disclosure Y. Social Contract 2.0 : Terms of Service Agreements and Political Theory

This essay advances the thesis that the most influential and important political documents of the twenty-first century are not neces-sarily the constitutions and charters written for new or reconfigured nation states, but the often-overlooked terms of service agreements that users must sign (or, more precisely, click agree) in order to participate in social networks like Facebook, Second Life, Google+, Bebo, etc. These agreements, which constitute the principle governing documents of online social worlds (Grimes et al., 2008), constitute a kind of postmodern, post-nation state social contract. As such, they articulate, structure, and regulate not only the kind of social interactions and political opportunities that are available within these global networks but determine what forms of social activity and affiliation are considered to be appropriate, acceptable, and even possible. The examination, which takes a critical approach to this subject, will be divided into three parts. The first situates these agreements within the history and lineage of modern political thought in general and social contract theory in particular. The second pursues a critical reading of the Terms governing Facebook, a social network with a population of active users that now exceeds Brazil making it the fifth most populous polity on the plant. The final section extrapolates the broader social and political consequences of these agreements, arguing that informed users not only need to read these documents carefully but also need to recognize the way these seemingly unimportant texts define and delimit the very conditions of (im)possibility for social involvement and interaction in the 21st century. Although the subject addressed by the essay is contemporary and a product of the late 20th and early 21st centuries, the method of its presentation is rather anachronistic and deliberately dissimulates the rhetoric and tone of an 18th century treatise. And this has been done, I can tell you dear reader, not out of affectation, but because the subject matter will have required nothing less.

Sizing up online social networks

While the size of popular online social networks such as MySpace and Twitter has been reported to be in the tens or hundreds of millions of users (and growing), little is known about the fraction of users who have either deleted or abandoned their accounts. Therefore, the growth of an OSN's overall user population and, more important, its population of active users cannot easily be determined. In this article we describe a measurement technique to infer the fine-grained growth in the total number of allocated accounts for a class of OSNs that include MySpace and Twitter and are characterized by two features. First, they assign numerical user IDs using a format and allocation strategy that can be determined. Second, a fraction of their users have abandoned these OSNs shortly after creating their accounts, and these short-lived users (called 'tourists') are scattered across the ID space. By exploiting these two properties, we are able to determine the growth in total and valid user accounts for MySpace and Twitter since their inception. For valid user accounts, we also derive the fraction of active users in the system at the time of our experiment, where we define the activity of a user in terms of the recency of her last visit to the OSN. In the case of MySpace and Twitter, our results show that the active population of these OSNs is typically an order of magnitude smaller than the reported (total) population.
