Unleashing the potential: transarterial chemoembolization combined with intra-arterial infusion of bevacizumab for unresectable hepatocellular carcinoma.

The purpose of this study is to compare the efficacy and safety of transarterial chemoembolization (TACE) alone with transarterial chemoembolization combined with the arterial infusion of bevacizumab (TACE + Bev) in patients with unresectable hepatocellular carcinoma (uHCC). A retrospective analysis was conducted on 446 uHCC patients treated with TACE or TACE + Bev between January 2021 and March 2023. The study evaluated objective response rate (ORR), disease control rate (DCR), progression-free survival (PFS), overall survival (OS), and adverse events in both treatment groups. Finally, the TACE group comprised 295 patients, and the TACE + Bev group comprised 151 patients. Patients in the TACE + Bev group exhibited significantly prolonged median PFS (7.9months vs. 10.3months, P = 0.013) and median OS (16.1months vs. 21.4months, P = 0.041), improved ORR (26.8% vs. 37.7%, P = 0.017) and DCR (71.5% vs. 80.8%, P = 0.033) compared to the TACE group. Multifactorial Cox analysis identified alpha-fetoprotein (AFP) > 400ng/ml as an independent prognostic factor for PFS and OS. Meanwhile, portal vein cancer thrombosis and distant metastasis are poor prognostic factors for OS. The overall incidence of adverse events was similar between the two groups. In comparison with the TACE group, the TACE + Bev group demonstrated efficacy in improving outcomes for patients with uHCC with a manageable safety profile.

Clinical outcomes of transarterial chemoembolization in Child-Turcotte Pugh class A patients with a single small (≤3cm) hepatocellular carcinoma.

Transarterial chemoembolization (TACE) is one of the standard modalities used to treat unresectable hepatocellular carcinoma (HCC), but the effectiveness of TACE for treating patients with a solitary small (≤3cm) HCC and well-preserved liver function has not been definitively established. This study aimed to determine the therapeutic impact of TACE in patients with these characteristics. This multicenter (four university hospitals) retrospective cohort study analyzed the medical records of 250 patients with a solitary small (≤3cm) HCC and Child-Turcotte-Pugh (CTP) class A liver function diagnosed over 10years. Posttreatment outcomes, including overall survival (OS), recurrence-free survival (RFS), and adverse events, were assessed following TACE therapy. One hundred and thirty-eight of the 250 patients (55.2%) treated with TACE achieved complete remission (CR). Overall median OS was 77.7months, and median OS was significantly longer in the CR group than in the non-CR group (89.1 vs. 58.8months, P=0.001). Median RFS was 19.1months in the CR group. Subgroup analysis identified hypertension, an elevated serum albumin level, and achieving CR as significant positive predictors of OS, whereas diabetes, hepatitis c virus infection, and tumor size (>2cm) were poor prognostic factors of OS. The study demonstrates the effectiveness of TACE as a viable alternative for treating solitary small (≤3cm) HCC in CTP class A patients.

Overexpression of cyclin F/CCNF as an independent prognostic factor for poor survival in clear cell renal cell carcinoma

Cyclin F (encoded by CCNF gene) has been reported to be implicated in the pathobiology of several human cancers. However, its potential clinical significance in clear cell renal cell carcinoma (ccRCC) remains unknown. The present study aimed to evaluate the potential significance of cyclin F, assessed by immunohistochemical (IHC) staining and molecular (bioinformatics) techniques, as a prognostic marker in ccRCC in relation to clinicopathological features and outcomes. IHC staining was performed using two independent ccRCC tissue array cohorts, herein called tissue macroarray (TMA)_1 and tissue microarray (TMA)_2, composed of 108 ccRCCs and 37 histologically normal tissues adjacent to the tumor (NAT) and 192 ccRCCs and 16 normal kidney samples, respectively. The mRNA expression data were obtained from The Cancer Genome Atlas (TCGA) and the Gene Expression Omnibus (GEO) public datasets, followed by bioinformatics analysis of biological mechanisms underlying prognosis. The relationship between immune cell infiltration level and CCNF expression in ccRCC was investigated using the Tumor Immune Estimation Resource 2.0 (TIMER2) and Gene Expression Profiling Interactive Analysis 2 (GEPIA2). Cyclin F expression was significantly elevated in ccRCC lesions compared to both NAT and normal renal tissues. Likewise, CCNF mRNA was markedly increased in ccRCCs relative to non-cancerous tissues. In all analyzed cohorts, tumors with features of more aggressive behavior were more likely to display cyclin F/CCNF-high expression than low. Furthermore, patients with high cyclin F/CCNF expression had shorter overall survival (OS) times than those with low expression. In addition, multivariable analysis revealed that cyclin F/CCNF-high expression was an independent prognostic factor for poor OS in ccRCC. Enrichment analysis for mechanistically relevant processes showed that CCNF and its highly correlated genes initiate the signaling pathways that eventually result in uncontrolled cell proliferation. CCNF expression was also correlated with immune cell infiltration and caused poor outcomes depending on the abundance of tumor-infiltrating immune cells in ccRCC. Our findings suggest that cyclin F/CCNF expression is likely to have an essential role in ccRCC pathobiology through regulating multiple oncogenic signaling pathways and affecting the tumor immune microenvironment and may serve as prognostic biomarker and promising therapeutic target in ccRCC.

Controlling Nutritional Status (CONUT) Score is Associated with Overall Survival in Patients with Hepatocellular Carcinoma Treated with Conventional Transcatheter Arterial Chemoembolization: A Propensity Score Matched Analysis.

This study aims to evaluate the prognostic value of controlling nutritional status (CONUT) score in determining the prognosis of patients with hepatocellular carcinoma (HCC) treated with conventional transcatheter arterial chemoembolization (cTACE). This study retrospectively analyzed 936 patients who underwent cTACE for HCC between January 2012 and December 2018, and divided them into two groups based on their CONUT score. To balance the bias in baseline characteristics, propensity score matched (PSM) analysis was conducted. The Kaplan-Meier method was used to establish a cumulative survival curve, and the log-rank test was employed to determine differences in overall survival (OS) and progression-free survival (PFS) among the CONUT score groups. Furthermore, the Cox proportional hazard model was employed to assess the correlation between CONUT score and OS and PFS, whereby hazard ratios (HRs) and 95% confidence intervals (95% CIs) were computed. Before PSM, the median OS for the low (≤ 3) and high (≥ 4) CONUT group (558 vs. 378 patients) was 21.7 and 15.6months, respectively, and the median PFS was 5.7 and 5months. Following PSM, both the low and high CONUT score groups comprised 142 patients. The low CONUT score group exhibited a significantly longer OS compared to the high CONUT score group, as determined by the log-rank test (median OS 22.2 vs. 17.0months, P = 0.014). No significant association was observed between CONUT group and PFS (median PFS 6.4 vs. 4.7months, log-rank test, P = 0.121). Cox proportional hazard regression analysis revealed that a CONUT score of ≥ 4 was an independent risk factor for OS in patients with HCC who underwent cTACE (HR = 1.361; 95% CI: 1.047-1.771; P = 0.022). These findings were consistent across most subgroup analyses. A high CONUT score has been found to be a prognostic factor for poorer OS in patients with HCC who underwent cTACE. Abstract 1108: The impact of adjuvant radiotherapy (ART) and survival prediction by machine learning in intracranial hemangiopericytoma (HPC)

Abstract Background: Intracranial HPC is a rare brain tumor that accounts for less than 1% of all primary central nervous system malignancies. Surgery is the primary treatment, and while radiation may reduce the risk of local recurrence, its overall effectiveness remains uncertain. Therefore, it is crucial to develop reliable predictions for personalized care and improved management. In this study, we used machine learning (ML) techniques to predict the survival of patients with intracranial HPC. Methods: Data were obtained from the Surveillance, Epidemiology, and End Results (SEER) database (2000-2019). Patients with other malignancies or lack of pathological diagnosis were excluded. T-tests and chi-square tests compared variables, while Kaplan-Meier, log-rank tests, and Cox regression identified prognostic factors for overall survival (OS) and cancer-specific survival (CSS). Patient records were randomly divided into training (80 %) and validation (20 %) sets. Univariate Cox regression and least absolute shrinkage and selection operator were used to identify the significant factors linked to survival, which were used to establish a nomogram for predicting the 3- and 5-year OS and CSS. Results: Of the 367 included patients, 121 underwent surgery and 246 underwent surgery + ART. The median survival time was 54 months. Patients who underwent surgery had 5-year OS rates of 61% and 86% for CSS, while those treated with surgery + ART had 81% and 96% survival rates, respectively. The two treatment groups showed significant differences in the OS and CSS (p<0.022 and p<0.017, respectively). Age ≥ 50 years was a poor prognostic factor for OS, whereas ART, localized stage and regional stage were good prognostic factors. No significant factors were associated with CSS. To construct the OS Nomogram, three predictors, age, stage, and treatment method, were chosen. In the training and validation sets, the C-indices are 0.799 and 0.736, respectively. The receiver operating characteristic curves (AUCs) for the 3- and 5-year OS in the training set were 0.594 and 0.617, respectively, whereas those in the validation set were 0.56 and 0.59. For the CSS nomogram, two factors, age and stage, were utilized. The C-indices for the training and validation sets are 0.814 and 0.774, respectively. In the training set, the AUCs for 3- and 5-year CSS were 0.585 and 0.605, respectively, and 0.563 in the validation set. The most important risk factor was the distant stage, while the most important protective factor was age less than 50 years. Conclusion: This study shows that ART has a significant impact on the treatment of Intracranial HPC and provides better survival outcomes. ML has been used to create prediction models to support personalized patient care. These findings provide valuable insights into the management of this rare brain tumor and the potential of data-driven approaches to improve treatment and outcomes. Evaluation of the prognostic significance of clinical features of tumoral lesions in an extensive series of mycosis fungoides.

Tumors indicating the advanced stage of mycosis fungoides (MF) have a rich clinical spectrum. Although it is known that the prognosis of MF generally worsens following the development of tumors, some cases may have a relatively indolent course, and the role of clinical characteristics regarding prognosis has still not been well understood. MF patients were retrospectively evaluated regarding the development of tumors. Besides demographic characteristics, data of the subtype and stage of the disease were recorded. The clinical features of tumors, including number (<5, 5-10, 11-20, or >20), location, dimension (diameter of ≥5 cm), presence of ulceration, and surrounding inflammation, were noted. Univariate and multivariate analyses evaluated the relationship between overall survival (OS) with demographic and clinical features. Among 730 consecutive MF patients, tumors developed in 8.2% (n = 60), of whom 46.7% were diagnosed with advanced-stage MF from the beginning. The most common subtype was folliculotropic MF (53.3%). Most patients (55%) had multiple tumors, and the most frequent localization was the trunk (71.7%). Most tumors presented as smooth-surfaced, indurated papules and/or nodules (70%), while others were reddish-purple, occasionally accompanied by ulceration (50%), perilesional inflammation (23.3%), and attaining large dimensions (25%). Mortality was recorded in 51.7% of patients, and the 5-year OS rate from the diagnosis of tumors was 49%. Independent poor prognostic factors for OS in multivariate analysis included older age at the time of diagnosis, presence of tumors at the initial MF diagnosis, presence of over 20 tumors, and the existence of large tumors. Tumoral MF seen in older patients, the first diagnosis of MF in this stage, presenting with generalized and large tumors, seems to be a predictive factor for OS.

Utility and optimal management of planned drug holidays during lenvatinib treatment in patients with unresectable differentiated thyroid cancer: a real-world multi-center study.

Lenvatinib achieves favorable therapeutic outcomes for patients with radioactive iodine therapy refractory differentiated thyroid cancer (DTC); however, its use is associated with a high incidence of adverse events. To avoid severe adverse events, planned drug holidays (PDH) have been proposed. This study aimed to evaluate treatment effects, identify prognostic factors, and investigate the usefulness of PDH in patients with unresectable DTC who received lenvatinib across the multi-institutions. Fifty-one patients with unresectable DTC treated with lenvatinib were evaluated retrospectively. Overall survival (OS) and progression-free survival (PFS) were calculated, and prognostic factors were assessed. OS, PFS, and time to treatment failure (TTF) were compared between patients with and without PDH. Lenvatinib administration schedule was evaluated in PDH. The 3-year OS and PFS rate were 53.5% and 42.1%, respectively. Multivariate analysis revealed that presence of maximum size of lung metastasis ≥10 mm was independent prognostic factor for poorer OS and PFS, and histology other than papillary thyroid carcinoma was the independent prognostic factor for poorer PFS. Twenty-five patients (49%) treated with PDH. There were significant differences in OS, PFS, and TTF between patients with and without PDH. Various schedules were used in PDH. Eight (32%) patients required switch to the different administration schedule. Our results suggest that PDHs may extend OS, PFS, and TTF. In patients with PDH, various schedules used for lenvatinib administration highlight the difficulty in determining a uniform administration schedule. Therefore, it is crucial to consider the optimal lenvatinib administration schedule on a case-by-case basis.

Impact of preoperative prealbumin levels on long-term prognosis in patients with gastric cancer after gastrectomy: a retrospective cohort study.

The relationship between preoperative prealbumin levels and long-term prognoses in patients with gastric cancer after gastrectomy has not been fully investigated. This study clarified the effect of preoperative prealbumin levels on the long-term prognosis of patients with gastric cancer after gastrectomy. This retrospective cohort study included consecutive patients who underwent radical gastrectomy for primary pStage I-III gastric cancer and whose preoperative prealbumin levels were measured between May 2006 and March 2017. Participants were categorized according to their preoperative prealbumin levels into high (≥22mg/dL), moderate (15-22mg/dL), and low (<15mg/dL) groups. The overall survival (OS) in the three groups was compared using the log-rank test, and prognostic factors were identified using Cox proportional hazards regression analysis. The median follow-up duration was 66months. Of 4732 patients, 3649 (77.2%) were classified as high, 925 (19.6%) as moderate, and 158 (3.3%) as low. Lower prealbumin levels were associated with poorer prognoses (P<0.001). Multivariate analysis showed that prealbumin levels of 15-22mg/dL [hazard ratio (HR): 1.576, 95% confidence interval (CI): 1.353-1.835, P<0.001] and <15mg/dL (HR: 1.769, 95% CI: 1.376-2.276, P<0.001) were independent poor prognostic factors for OS. When analyzed according to the cause of death, prealbumin levels were associated with other-cause survival, but not cancer-specific survival. Preoperative prealbumin levels correlated with OS in patients with gastric cancer after gastrectomy; the lower the prealbumin level, the worse is the prognosis. Prealbumin levels may be associated with other-cause survival.

Surgical Outcomes in Non-Small Cell Lung Cancer with Distant Metastasis: The Prognostic Significance of Delayed Metastasis Diagnosis.

The diagnosis of distant metastasis on preoperative examinations for non-small cell lung cancer (NSCLC) can be challenging, leading to surgery for some patients with uncertain metastasis. This study evaluated the prognostic impact of delayed diagnosis of metastasis on patients who underwent upfront surgery. The study enrolled patients who underwent lobectomy or pneumonectomy for NSCLC between June 2010 and December 2017 and evaluated the presence of distant metastasis before surgery. Overall survival (OS) for patients with stage IV cancer was compared with that for patients without metastasis, and the prognostic factors were analyzed. Of 3046 patients (mean age, 63 years; 1770 men), 100 (3.3 %) had distant metastasis, diagnosed preoperatively in 1.4 % (42/3046) and postoperatively in 1.9 % (58/3046) of the patients. The two most common metastasis sites diagnosed after surgery were contralateral lung (22/58, 37.9 %) and ipsilateral pleura (16/58, 27.6 %). The OS (median, 42.7 months) for the patients with stage IV cancer diagnosed postoperatively was comparable with that for the patients with stage IIIB cancer (P = 0.865), whereas the OS (median OS, 91.7 months) for the patients with stage IV cancer diagnosed preoperatively was better than for the patients with stage IIIB cancer (P = 0.001). Among the patients with distant metastasis, squamous cell type (hazard ratio [HR], 3.15; P = 0.002) and systemic treatment for metastasis (HR, 2.42; P = 0.002) were independent predictors of worse OS. Among NSCLC patients undergoing upfront surgery, the OS for the patients with stage IV cancer diagnosed postoperatively was comparable with that for the patients with stage IIIB cancer. For patients with stage IV disease, squamous cell type and systemic treatment for metastasis were prognostic factors for poorer OS.

Improved Clinical Staging System for Localized Pancreatic Cancer Using the ABC Factors: A TAPS Consortium Study.

Previous studies suggest that besides anatomy (A: resectable, borderline resectable [BR], or locally advanced [LA]) also biologic (B: carbohydrate antigen 19-9 [CA 19-9]) and conditional (C: performance status) factors should be considered when staging patients with localized pancreatic ductal adenocarcinoma (PDAC). The prognostic value of the combined ABC factors has not been quantitatively validated. In this retrospective cohort study, we evaluated patients with localized PDAC treated with initial (modified) fluorouracil with leucovorin, irinotecan, and oxaliplatin ([m]FOLFIRINOX) at five high-volume pancreatic cancer centers in the United States and the Netherlands (2012-2019). Multivariable Cox proportional hazards analysis was used to investigate the impact of the ABC factors for overall survival (OS). Overall, 1,835 patients with localized PDAC were included. Tumor stage at diagnosis was potentially resectable in 346 (18.9%), BR in 531 (28.9%), and LA in 958 (52.2%) patients. The baseline CA 19-9 was >500 U/mL in 559 patients (32.5%). Performance status was ≥1 in 1,110 patients (60.7%). Independent poor prognostic factors for OS were BR disease (hazard ratio [HR], 1.26 [95% CI, 1.06 to 1.50]), LA disease (HR, 1.71 [95% CI, 1.45 to 2.02]), CA 19-9 >500 U/mL (HR, 1.36 [95% CI, 1.21 to 1.52]), and WHO performance status ≥1 (HR, 1.31 [95% CI, 1.16 to 1.47]). Patients were assigned 1 point for each poor ABC factor and 2 points for LA disease. The median OS for patients with score 0-4 was 49.7, 29.9, 22.0, 19.1, and 14.9 months with corresponding 5-year OS rates of 47.0%, 28.9%, 19.2%, 9.3%, and 4.8%, respectively. The ABC factors of tumor anatomy, CA 19-9, and performance status at diagnosis were independent prognostic factors for OS in patients with localized PDAC treated with initial (m)FOLFIRINOX. Staging of patients with localized PDAC at diagnosis should be based on anatomy, CA 19-9, and performance status.

Prognostic value of preoperative geriatric nutritional risk index in intrahepatic cholangiocarcinoma after hepatectomy: a single‑center retrospective cohort study.

Patients with malignant tumors are prone to develop nutritional disorders. The Geriatric Nutritional Risk Index (GNRI) is a new prognostic indicator for assessing the nutritional status. This study was performed to evaluate whether the preoperative GNRI can serve as a prognostic factor in patients with intrahepatic cholangiocarcinoma (ICC) undergoing curative surgery. This study included 123 consecutive patients with ICC who were treated with curative surgery. Kaplan-Meier analysis was performed to calculate the recurrence-free survival (RFS) and overall survival (OS), and Cox regression analysis was used to evaluate prognostic factors. Of the 123 patients, 82 were male and 41 were female. The median age of the patients was 70years, and the median follow-up period was 37.0months (interquartile range, 16.2-71.7months). The patients were classified by the median GNRI into a low GNRI group (GNRI < 105) and high GNRI group (GNRI ≥ 105). The patients in the low GNRI group had a significantly poorer prognosis in terms of RFS and OS than the patients in the high GNRI group (RFS, p = 0.0201; OS, p < 0.0001). Lymph node metastasis [hazard ratio (HR), 4.66; 95% confidence interval (CI), 2.46-8.85], postoperative complications (HR, 2.38; 95% CI, 1.32-4.31), and a low GNRI (HR, 2.53; 95% CI, 1.42-4.50) were independent poor prognostic factors for OS. The GNRI may be a useful prognostic indicator in patients with ICC undergoing curative hepatectomy.

Prognostic scores in pulmonary large cell neuroendocrine carcinoma: A retrospective cohort study

IntroductionPulmonary large cell neuroendocrine carcinoma (PLCNEC) is a rare but aggressive subtype of lung cancer with an incidence of approximately 3 %. Identifying effective prognostic indicators is crucial for guiding treatments. This study examined the relationship between inflammatory markers and PLCNEC patient overall survival (OS) and sought to determine their prognostic significance in PLCNEC. MethodsPatients diagnosed with PLCNEC between 2007 and 2022 at the oncology center, were retrospectively included. Patients who underwent surgery were pathologically re-staged post-surgery. Potential prognostic parameters (neutrophil/lymphocyte ratio, platelet/lymphocyte ratio [PLR], panimmune inflammatory value, prognostic nutritional index and modified Glasgow prognostic score [mGPS]) were calculated at that time of diagnosis. ResultsSixty patients were included. The median follow-up was 23 months. Thirty-eight patients initially diagnosed with early or locally advanced. The mGPS was identified as a poor prognostic factor that influenced disease free survival (DFS) fourfold (p = 0.03). All patients' median OS was 45 months. Evaluating factors affecting OS in all patients, statistically significant relationships were observed between OS and the prognostic nutritional index (p = 0.001), neutrophil/lymphocyte ratio (p = 0.03), platelet/lymphocyte ratio (p = 0.002), and pan-immunoinflammatory value (p = 0.005). Upon multivariate analysis, the platelet/lymphocyte ratio was identified as an independent poor prognostic factor for OS, increasing the mortality risk by 5.4 times (p = 0.002). ConclusionmGPS was significantly linked with prognosis in non-metastatic PLCNEC, with patients with higher mGPS exhibiting poorer long-term DFS. This finding contributes to the evolving understanding of PLCNEC. The multivariable predictive model we employed suggests that PLR is an independent predictor of OS at all stages. A lower PLR was correlated with worse overall survival. Thus, PLR can be a readily accessible and cost-effective prognostic factor in PLCNEC patients.

Diagnostic challenges in low-grade central osteosarcoma.

Low-grade central osteosarcoma (LGCOS), a rare type of osteosarcoma, often has misleading radiological and pathological features that overlap with those of other bone tumours, thereby complicating diagnosis and treatment. We aimed to analyze the clinical, radiological, and pathological features of patients with LGCOS, with a focus on diagnosis, treatment, and outcomes. We retrospectively analyzed the medical records of 49 patients with LGCOS (Broder's grade 1 to 2) treated between January 1985 and December 2017 in a single institute. We examined the presence of malignant features on imaging (periosteal reaction, cortical destruction, soft-tissue invasion), the diagnostic accuracy of biopsy, surgical treatment, and oncological outcome. Based on imaging, 35 of 49 patients (71.4%) exhibited malignant features. Overall, 40 of 49 patients (81.6%) had undergone a biopsy before en-bloc resection: 27 of 40 patients (67.5%) were diagnosed on the first biopsy, which was more accurate when carried out by open rather than needle biopsy (91.3% vs 35.3% diagnostic accuracy, respectively; p < 0.001). Of the 40 patients treated by en-bloc resection, surgical margins were wide in 38 (95.0%) and marginal in two (5.0%). Furthermore, nine of 49 patients (18.4%) underwent curettage (intralesional margin) without previous biopsy. All patients with a positive margin developed local recurrence. Distant metastases occurred in five of 49 patients (10.2%). The mean five-year overall survival (OS) and distant relapse-free survival (D-RFS) were 89.3% (SD 5.1%) and 85.7% (SD 5.5%), respectively. Univariate analysis showed that the occurrence of distant metastasis was a poor prognostic factor for OS (hazard ratio 11.54, 95% confidence interval (CI) 1.92 to 69.17; p < 0.001). Local recurrence was a poor prognostic factor for D-RFS (HR 8.72, 95% CI 1.69 to 45.0; p = 0.002). The diagnosis of LGCOS can be challenging because it may present with non-malignant features and has a low diagnostic accuracy on biopsy. If precisely diagnosed, LGCOS can be successfully treated by surgical excision with wide margins.

Expression and Clinical Significance of Exosome Component 4 in Newly Diagnosed Patients with Diffuse Large B-Cell Lymphoma

To explore the expression of Exosome Component 4（EXOSC4） in the tissues of newly diagnosed patients with diffuse large B-cell lymphoma (DLBCL) and its clinical significance. The expression of EXOSC4 protein in the tissues of 181 newly diagnosed DLBCL patients was analyzed by immunohistochemical staining. Clinical data were collected. The correlation between EXOSC4 protein expression in the tissues of newly diagnosed DLBCL patients and clinical features were analyzed and its prognostic significance. The positive rate of EXOSC4 protein expression was 68.51% in the tissues of 181 newly diagnosed DLBCL patients. These patients were divided into two groups, with 44 cases in high expression group and 137 cases in low expression group. There were no significant differences in age, gender, B symptoms, serum lactate dehydrogenase (LDH) level, Eastern Cooperative Oncology Group (ECOG) score, Ann Arbor stage, extranodal disease, International Prognostic Index (IPI) score, National Comprehensive Cancer Network IPI (NCCN-IPI) score, and cell origin between the two groups (P>0.05). Cox multivariate regression analysis showed that high EXOSC4 protein expression in tissues was an independent poor prognostic factor for OS and PFS in newly diagnosed DLBCL patients (all P<0.05). K-M survival analysis showed that newly diagnosed DLBCL patients with high EXOSC4 protein expression had significantly shorter overall survival (OS) and progression free survival (PFS) than those patients with low EXOSC4 protein expression (all P<0.05). High EXOSC4 protein expression in tissues of newly diagnosed DLBCL patients is an independent poor prognostic factor for survival.

Non-T-Depleted Haploidentical Transplantation with Post-Transplant Cyclophosphamide in Patients with Relapsed/Refractory Secondary Versus De Novo AML: A Study from the ALWP/EBMT

Background: Secondary acute myeloid leukemia (sAML) is a distinct type of AML. Transplantation, a potentially curative therapy, outcomes are inferior compared to de novo AML. We recently demonstrated (J Hematol Oncol 2023) that results of non-T-cell depleted haploidentical stem cell transplantation (HaploSCT) with post-transplant cyclophosphamide (PTCy) in sAML in complete remission are not significantly different from those in denovo AML. However, the challenge in relapse (Rel)/primary refractory (PR) AML is much higher. Methods:The study aim was to compare the outcomes of HaploSCT with PTCy, performed between 2010 and 2022, in first Rel(Rel1)/PR sAML versus those in Rel1/PR de novo AML. Statistical tests included a multivariate analysis (MVA) adjusting for potential confounding factors using a Cox proportional-hazards regression model for main outcomes. Results: A total of 719 patients (pts) met the inclusion criteria, 129 with sAML and 590 with de novo AML. Median follow-up was 45.6 (IQR] 39.1-57.9) and 43.5 (IQR, 37.5-48.0) months for pts with sAML and de novo AML, respectively (p=0.20). Pts with de novo AML were younger, with a median age of 55.4 (range 18-77.8) versus 61.3 (21-78.8) years, (p&amp;lt;0.0001). The median year of the transplant was 2018 (2010-2011) in pts with sAML and 2017 (2010-2022) in those with de novo AML (p=0.62) and 65.1% and 57.6%, were male (p=0.11). In 81.4% of sAML pts, the antecedent hematological disorder was myelodysplastic syndrome. The cytogenetic risk was categorized as intermediate (58.6% vs 59.4%), adverse (37.4% vs 34.7%), and favorable (4% vs 5.9%) for pts with sAML and de novo AML, respectively (p=0.75) (data missing for 145 pts). A higher percentage of pts with sAML vs de novo AML had PR disease (73.6% vs 58.6%) (p=0.002). HCT-CI was higher in the sAML vs the de novo AML group, with HCT-CI &amp;gt;3 in 40.3% vs 21.9%, respectively (p&amp;lt;0.0001), while the Karnofsky performance status (KPS) did not differ. There was no difference in the frequency of cytomegalovirus (CMV) seropositivity and female donor to male pt combination between the two pt groups. Reduced intensity conditioning was received by 60.2% and 52.2% of the sAML and de novo AML pts, respectively (p=0.1), with thiotepa/busulfan/fludarabine being the most frequent regimen for both groups (38.8% vs 42%, respectively). Graft source was mainly peripheral blood stem cells (PB) in both sAML (69.8.5%) and de novo (66.8%) groups. All pts received PTCy as graft- versus-host disease (GVHD) prophylaxis in combination with immunosuppression, which was cyclosporine A (CSA)/ mycophenolate mofetil in 41.1% vs 52.9% and CSA/tacrolimus in 41.1% vs 33.6% respectively. Cumulative incidence of absolute neutrophil count (ANC) &amp;gt;0.5 x 10 9/L at day 60 was 83.5% vs 88.4% (p=0.13), respectively. Day 180 incidence of acute (a) GVHD II-IV and III-IV was 20% vs 26.9% (p=0.12) and 8.9% vs 10.4%, (p=0.21), respectively. The 2-year (y) total and extensive chronic (c) GVHD were 25.3% vs 20.7% (p=0.27) and 12.5 % vs 10.3% (p=0.46), respectively. In MVA HaploSCT outcomes did not differ significantly between the Rel1 /PR sAML and de novo groups; 2-y non-relapse mortality (NRM) hazard ratio (HR)=1.38 (95% CI 0.96-1.98, p=0.083). Infections and GVHD were the cause of death in 20.8% and 9.3% of pts who died. The HR for 2-y relapse incidence (RI) was 0.68 (95% CI 0.4.7.-1.00, p=0.051) and RI was the main cause of death in 40.9% and 59.3% of pts who died, respectively. The 2-y leukemia-free survival (LFS), overall survival (OS), and GVHD-free, relapse-free survival (GRFS) did not differ between the two groups with HR=0.99 (95% CI 0.76-1.28, p&amp;lt;0.94), HR=0.99 (95% CI 0.77-1.29, p=0.973) and HR=0.99 (95% CI 0.77-1.27, p=0.94), respectively. Adverse-risk cytogenetics was a poor prognostic factor for RI and both higher age (per 10y) and lower KPS were poor prognostic factors for NRM. Adverse-risk cytogenetics and lower KPS were poor prognostic factors for LFS, OS, and GRFS. In addition, pt CMV seropositivity was a poor prognostic factor for OS. Finally, PB grafts were associated with increased risk of all grades and severe aGVHD. Conclusions: In this registry-based retrospective analysis, we observed similar outcomes, of HaploSCT with PTCy for Rel1/PR sAML compared to Rel1/PR de novo AML, with HaploSCT being able to rescue about one-third of the pts. However, relapse is still 43%. The recently approved novel agents for sAML will hopefully further improve sAML outcomes.

Venetoclax-Based Therapy before Allogeneic Hematopoietic Cell Transplantation for Acute Myeloid Leukemia

Introduction The BCL-2 inhibitor venetoclax (VEN) is effective in older patients with untreated acute myeloid leukemia (AML). Few reports exist on the use of VEN as a bridging therapy for allogeneic hematopoietic cell transplantation (HCT) for AML. Previous studies have shown that NPM1 and IDH2 gene mutations are associated with high response rates to VEN therapy. We performed a retrospective cohort study to analyze the efficacy of VEN-based bridging therapy before HCT. Methods We included 46 patients who were eligible for HCT and received bridging therapy combined with VEN between April 2021 and June 2023 at our center. We evaluated response rates to VEN and HCT outcomes. None of the patients received VEN prior to the study. The response rate to VEN was evaluated using the 2022 European LeukemiaNet (ELN) criteria. We estimated overall survival (OS) using the Kaplan-Meier method, log-rank test, and Gray's test for univariate analysis of cumulative incidence of relapse (CIR) and non-relapse mortality (NRM). We used the Cox proportional hazards model for a multivariate analysis of OS. We extracted DNA from the bone marrow or peripheral blood, and analyzed gene mutations using next-generation sequencing (NGS), targeting 70 genes known to be recurrently mutated in myeloid malignancies. Results Of the 46 patients, 32 had de novo AML, 7 had de novo MDS/AML, 5 had secondary AML, 1 had secondary MDS/AML, and 1 had myeloid sarcoma. The median age was 59 (21-70) years. Nineteen were women and 7 had a history of HCT. The regimens combined with VEN were azacitidine in 39 cases and low-dose cytarabine in 7. According to the 2022 ELN risk classification by genetics at diagnosis, 7 were favorable, 11 were intermediate, and 27 were at adverse risk. Nine patients were in hematological complete remission (CR) / CR with incomplete count recovery (CRi) while 37 were not, when VEN was initiated. The maximum response rates among the patients who started VEN treatment in the non-CR group were: 18 with CR/CRi (49%) and no response in 19 (51%). The median time from the first dose of VEN to HCT was 70 (27-315) days. Forty-one patients underwent HCT and 19 (43%) were in CR/CRi at HCT. The reasons for discontinuation of VEN were HCT in 34 cases, failure in 9, and adverse events in 4 (hematological toxicity in 3 cases and infection in 1). Sixteen patients died due to primary disease (n=9), infection (n=5), thrombotic microangiopathy (n=1), and multi-organ failure (n=1). The median post-HCT observation period of survivors was 156 (25-775) days, and at 1-year after HCT, OS was 49.6% (95% CI: 28.0-67.9), CIR was 40.6% (95% CI: 24.1-56.6) and NRM was 15.2% (95% CI: 5.5-29.5). At 1-year after HCT, outcomes of the patients who were in CR/CRi at HCT was better than that of those who were not (OS, 80.8% vs. 26.4%, p=0.002; CIR, 13.4% vs. 61.8%, p&amp;lt;0.01; NRM, 0% vs. 26.7%, p&amp;lt;0.05). In 35 of the patients who underwent the first HCT, the 1-year OS was 88.9% in those who were in CR and 36.5% in those who were not (p=0.015). Thirty cases were non-CR at the start of the VEN regimen prior to their first HCT. This group had a 1-year OS of 57.8% (95% CI, 27.4-79.3). In 11 patients who achieved CR before HCT and 18 others who did not, the 1-year OS rates were 83.3% and 39.9%, respectively (p=0.10). Genetic analysis was performed by NGS in 37 patients before starting VEN, revealing mutations in TP53 (n=9), RUNX1 (n=7), IDH1 (n=6), ASXL1 (n=6), and DNMT3A (n=6) (Fig. 1A). Alterations in MDS-related genes (n=20) and kinase-related genes ( KIT, PTPN11, FLT3, KRAS, n=11) were frequent. Among the patients receiving their first HCTs, the 1-year OS was 0% in the group with TP53 mutations, and 63.9% in the wild-type TP53 group (p=0.04). The OS was worse in patients with mutated TP53 or kinase-related genes (n=19) than those without either mutation (n=15) (Fig. 1B, 1-year OS 28.7% vs. 72.9%, p=0.029). A multivariate analysis of OS revealed that patients who were not in CR at HCT (HR, 25.01 [95% CI, 2.406-259.9], p=0.007), those who previously underwent HCT (HR, 36.81 [95% CI, 4.599-294.7], p=0.035), and those with TP53 or kinase-related gene mutations (HR, 6.311 [95% CI, 1.137-350.0], p=0.035) had significantly lower OS rates. There was no significant difference in OS between the different ELN risk categories. Conclusions Non-CR at the time of HCT, a history of HCT, and mutations in TP53 or kinase-related genes were found to be independent poor prognostic factors for OS after VEN-based bridging treatment followed by HCT.

Elevation of Urinary Liver-Type Fatty Acid-Binding Protein Is a Harbinger of Poor Patient Prognosis after Allogeneic Stem Cell Transplantation

Several recent studies have demonstrated that urinary levels of liver-type fatty acid-binding protein (L-FABP) can be used to stratify the prognosis of cardiac disease, cardiac intensive care unit admission, cirrhosis, and coronavirus disease 2019. Our initial prospective study revealed that urinary L-FABP (uL-FABP) was associated with a high probability of acute kidney injury after stem cell transplantation (SCT); however, the relevance of elevated uL-FABP to the prognosis of patients undergoing SCT remains to be determined. We aimed to investigate whether uL-FABP levels can be used to stratify patient prognosis after SCT. To achieve this aim, we conducted a new long-term follow-up study using data from patients enrolled in our preceding prospective cohort study. Patients were classified into high and low uL-FABP groups based on levels measured at baseline (ie, before initiating the conditioning regimen), using an uL-FABP cutoff of 8.4 μg/gCr, which was determined based on data from healthy adults. uL-FABP levels were also measured on days 0, 7, and 14 after SCT. Cox proportional hazard regression was used to examine the effects of each factor on survival outcomes, and Fine-Gray regression was used in the presence of competing risks. Multivariate analysis incorporating confounders was then performed for factors with P < .1 in univariate analysis. In total, 20 of 84 patients (23.8%), 57 of 84 patients (67.9%), 34 of 49 patients (69.4%), and 34 of 46 patients (73.9%) were classified into the high uL-FABP group at baseline and on days 0, 7, and 14, respectively. The 5-year overall survival (OS) rate was 23.9% in the high uL-FABP group and 68.9% in the low uL-FABP group. The multivariate analysis identified a high uL-FABP level at baseline as a significant prognostic factor for poor OS (hazard ratio [HR], 3.54; P=.002). The 5-year cumulative incidence rate for nonrelapse mortality (NRM) was 50.0% in the high uL-FABP group and 19.9% in the low uL-FABP group. In the multivariate analysis, high uL-FABP at baseline was a significant prognostic factor for NRM (HR, 3.37; P=.01). uL-FABP levels did not significantly stratify the cumulative incidence of relapse (HR, 2.13; P=.11). uL-FABP levels on days 0, 7, and 14 were not significant predictors of survival. High uL-FABP level before initiation of conditioning significantly influences OS and NRM following SCT, whereas a high uL-FABP level at any point after the conditioning regimen does not. Our results show that measuring uL-FABP level at baseline may be a simple way to predict survival in patients undergoing SCT.

FLT3 Mutations in Acute Myeloid Leukemia: Unraveling the Molecular Mechanisms and Implications for Targeted Therapies.

Acute myeloid leukemia (AML) is a heterogeneous and aggressive form of blood cancer characterized by theuncontrolled proliferation of myeloid precursor cells in the bone marrow. It affects individuals of all ages,with incidence increasing notably in those over 65 years old. Despite advancements in treatment, overallsurvival rates remain unsatisfactory, underscoring the need for a deeper understanding of the disease. Among the various genetic alterations implicated in AML pathogenesis, mutations in the FLT3 (Fms-like tyrosine kinase 3) gene haveemerged as significant contributors to leukemogenesis. The FLT3 ​​​​​gene encodes atype III receptor tyrosine kinase crucial in regulating normal hematopoiesis. Approximately one-third ofAML patients carry FLT3 mutations, making it one of the most frequently mutated genes in the disease. FLT3mutations can be classified into internal tandem duplications (ITDs) and point mutations in the tyrosinekinase domain (TKD).FLT3 mutations are associated with adverse clinical features and are independent prognostic factors for pooroverall survival and decreased remission rates in AML patients. Understanding the molecular mechanisms underlying FLT3 mutations in AML is critical for improving risk stratification, prognosis assessment, and the development of targeted therapies.By reviewing the current literature, this study aims to elucidate the functional consequences of FLT3 mutations in AML pathogenesis, explore the interaction of FLT3 signaling with other oncogenic pathways, and assess the prognostic significance of FLT3 mutations in clinical practice, providing information that can guide future research directions and facilitate the development of more effective therapeutic strategies.

Proton beam therapy for extrahepatic biliary tract cancer: Analysis with prospective multi-institutional patients' registration database, Proton-Net.

To examine the role of proton beam therapy (PBT) in the treatment of extrahepatic biliary tract cancer (EBC). We analyzed the data accumulated in the Proton-Net database, which prospectively registered all individual patient data treated with PBT in all Japanese proton institutions from May 2016 to June 2019. The primary endpoint was overall survival (OS), and the secondary endpoints were local control (LC), progression-free survival (PFS), and toxicity. Ninety-three patients with unresectable and/or recurrent EBC were treated with PBT using a median prescribed dose of 67.5Gy (RBE) (range, 50-72.6Gy) in 25 (22-30 fractions). With a median follow-up of 16.3months, the median survival time was 20.1months and the 2-year OS was 37.8%. Two-year PFS and LC rates were 20.6% and 66.5%, respectively. Poor liver function (Child-Pugh B, C), a narrower distance between the tumor and digestive tract (2cm >), and a larger tumor diameter (2cm <) were identified as poor prognostic factors for OS. PBT-related grade 3 ≤ acute and late adverse events occurred in 5.4% and 4.3% of patients, respectively, including one gastrointestinal late toxicity (duodenal ulcer). This is the largest prospectively accumulated series of PBT for EBC, and PBT showed favorable outcomes with acceptable toxicity profiles.

Background: Differentiating high-grade neuroendocrine carcinomas (HGNEC) is difficult. We aimed to assess the clinical features and survival outcomes of unclassified HGNEC (uHGNEC) and to compare it with small-cell lung cancer (SCLC). &#x0D; Material and Methods: This was a retrospective and observational study of HGNEC patients. Kaplan-Meier method was used to estimate progression-free survival (PFS) and overall survival (OS). Cox-regression analyses were used to determine the risk factors independently associated with PFS and OS.&#x0D; Results: One hundred twenty-one patients [uHGNEC (n = 35), SCLC (n = 86)] were analysed. The primary tumour was mostly right-sided, located in the centre of the lungs. The IASLC stage at diagnosis was locally advanced in 43 (35.5%) patients and advanced in 78 (64.5%) patients. uHGNEC and SCLC groups shared similar clinical features. The study population's median PFS and OS were 8.8 (95%Cl 7.29 – 10.30) and 10.9 (95%Cl 9.9 – 11.8) months, respectively. uHGNEC- and SCLC groups had a similar PFS (9.4 vs 8.6 months, p = 0.99) and OS (12 vs 10.7 months, p = 0.51). The six-month, one- and two-year PFS and OS of two groups were also similar. Among all patients, a right-sided tumour (HR: 1.558, 95%Cl 1.044 – 2.325, p = 0.03) and advanced-stage disease (HR: 1.928, 95%Cl 1.292 – 2.877, p = 0.001) were prognostic factors for poor OS. Cox-regression analysis indicated that histopathology did not have an impact on PFS and OS.&#x0D; Conclusion: HGNEC patients who cannot be classified pathologically behave like SCLC.
