The Poole approach to a smoke-free pregnancy

Smoking tobacco is both a pharmacological addiction and a serious social and public health issue. Smoking cessation services for pregnant women save babies' lives and have the potential to improve the health of families and communities. Engaging with pregnant smokers and their households towards quitting smoking requires a whole-team approach. Protected time for the specialist smoking in pregnancy team and freedom to develop the service improvement approach has been key in the setting described in this paper. The approach to smoking cessation described in this paper is of the quality improvement arm within the ‘Saving babies' lives’ bundle of interventions in Poole Hospital NHS Foundation Trust. The ‘plan, study, do, act’ model is described with a description of its implementation using sequential learning, enquiry, testing and being open to new ideas and approaches. The main driver questions have been: ‘how do we reach pregnant smokers who are not motivated to quit?’ and ‘what approaches will potentiate success for this group?’

Integrating services to improve patient care

Over the past few years there has been an increasing need to reconfigure emergency care service delivery so that patients can be seen and treated as quickly as possible by the right person in the right place at the right time. This reconfiguration requires changes in culture and working practices, and the bringing together of previously disparate services. This article describes how the integration of three distinct but co-located emergency services at Poole Hospital NHS Foundation Trust was initiated through the process of practice development unit accreditation.

Low complication rate during intraperitoneal therapy through a totally implanted peritoneal access port in patients with ovarian cancer.

Fifty-three patients with histologically proven ovarian cancer were treated with intraperitoneally administered cisplatin or human recombinant interferon-alpha through a totally implanted peritoneal access port. A total of 281 treatment courses were given. No complications related to surgical implantation of the port were seen. Infectious complications, intra-abdominal problems or subcutaneous drug extravasation did not occur. In two patients the number of treatment courses was limited due to inflow obstruction. A totally implanted peritoneal access port proves to be a reliable route for the intraperitoneal treatment of patients with ovarian cancer. The strict aseptic technique we used contributes to its safety by preventing intra-abdominal infections.
