Omega-3 polyunsaturated fatty acids in the prevention of relapse in patients with stable bipolar disorder: A 6-month pilot randomized controlled trial

This study investigated the efficacy and safety of omega-3 polyunsaturated fatty acids (n-3 PUFAs) in relapse prevention of bipolar disorder (BD), addressing the shortcomings of current medications. Thirty-one stable BD patients were randomized to receive n-3 PUFAs or placebo for 6 months and intergroup differences in the incidence of the recurrence of bipolar depression were assessed. Differences in depression severity, manic symptoms, and routine biochemical parameters were also assessed. Interestingly, n-3 PUFAs demonstrated a favorable preventive effect on bipolar depression recurrence (p=0.005; Log-Rank) and reduced depression severity compared to placebo, and were well-tolerated, suggesting their potential as a safe prophylactic therapy for BD.

Use of polyunsaturated fatty acids in prevention and treatment of gastrointestinal diseases, obesity and cancer

Summary Fatty acids are important structural and functional elements of human body. We can distinguish several types: among others polyunsaturated fatty acids, which include omega-3 fatty acids (ω-3PUFA) and omega-6 fatty acids(ω-6PUFA). The first group has pleiotropic health-promoting effects, while the second group, ω-6PUFA, negatively affects the homeostasis of the human body and contributes to the development of numerous diseases. Both the amount and the relative ratio of these acids in the diet is an important factor affecting health and quality of life. Laboratory and clinical studies indicate that ω-3PUFA have a positive effect on the therapy of illnesses such as obesity and inflammatory bowel disease (IBD). ω-3 PUFA supplementation also appears to have a helpful effect in the adjuvant treatment of colorectal cancer and recovery.

The effect of omega-3 polyunsaturated fatty acids in prevention of postoperative atrial fibrillation development in patients undergoing coronary artery bypass graft surgery

To estimate the effect of omega-3 polyunsaturated fatty acids (PUFAs) in prevention of postoperative atrial fibrillation (POAF) in patients with coronary artery disease undergoing coronary artery bypass graft (CABG) surgery. Methods. Studied were 306 patients who underwent CABG. All the patients were divided into two groups depending on the prescription of omega-3 PUFAs in the pre- and postoperative period by randomization method: 1 group comprised 158 patients without PUFAs (82.7% men, median age 63.0 (57.0;67.0) years, 2 group - 148 patients with PUFAs (89.3% men, median age 60.0 (57.0;64.0) years). PUFAs were prescribed 2000 mg daily starting with 5 days before CABG and 1000 mg daily after CABG for 21 days. In all the patients, we studied interleukin (IL)-6, IL-8, IL-10, NT-proBNP, troponin, superoxide dismutase (SOD), malondialdehyde (MDA), glutathione and omega-3 index. Results. During the observation period POAF occurred in 29.7% patients of group 1, and in 16.9% patients of group 2 (p = 0.009). In postoperative period patients of group 1 had median IL-6 level 39.3% higher (p = 0.001) and median IL-10 level - 20.2% higher (p = 0.01) comparing with group 2. In patients of group 2 we found SOD median level 78.9% lower (р<0.0001) and MDA median level 33.8% lower compared to the patients of group 1 (р=0.03). In postoperative period in group 2 the median level of docosahexaenoic acid was 55% higher (p = 0.03), and omega-3 index - 43.4% higher (h = 0.04) compared with group 1. Conclusion. We found that patients with PUFAs had less activation of inflammation and oxidative stress after CABG than patients without PUFAs. The patients with PUFAs also had increased levels of docosahexaenoic acids and omega-3 index compared to the patients without PUFAs. We also found a significant decrease of the prevalence of POAF after CABG in patients with PUFAs.

A Novel Anti-Inflammatory Role of Omega-3 PUFAs in Prevention and Treatment of Atherosclerosis and Vascular Cognitive Impairment and Dementia.

Atherosclerosis is an inflammatory chronic disease affecting arterial vessels and leading to vascular diseases, such as stroke and myocardial infarction. The relationship between atherosclerosis and risk of neurodegeneration has been established, in particular with vascular cognitive impairment and dementia (VCID). Systemic atherosclerosis increases the risk of VCID by inducing cerebral infarction, or through systemic or local inflammatory factors that underlie both atherosclerosis and cognition. Omega-3 and omega-6 polyunsaturated fatty acids (PUFAs) are involved in inflammatory processes, but with opposite roles. Specifically, omega-3 PUFAs exert anti-inflammatory properties by competing with omega-6 PUFAs and displacing arachidonic acid in membrane phospholipids, decreasing the production of pro-inflammatory eicosanoids. Experimental studies and some clinical trials have demonstrated that omega-3 PUFA supplementation may reduce the risk of different phenotypes of atherosclerosis and cardiovascular disease. This review describes the link between atherosclerosis, VCID and inflammation, as well as how omega-3 PUFA supplementation may be useful to prevent and treat inflammatory-related diseases.

Adverse plasma fatty acid composition in patients with femoral neck fracture1

Our study aimed to examine the status of plasma fatty acids (FAs), inflammatory markers, and lipid peroxidation in patients with femoral neck fractures. The study included 20 patients (64-86 years) with femoral neck fractures indicated for surgery and a control group of 17 elderly subjects without fractures or serious chronic diseases. Plasma was obtained during the first 12 h postfracture and presurgery and 7 days postop. Compared to the control, patients had significantly higher saturated FA (SFA) and monounsaturated FA as well as increased TNF-α and IL-6. Opposite to that, levels of individual and total n-6 polyunsaturated FA (PUFA), individual and total n-3 PUFA, n-6/n-3 ratio, and levels of thiobarbituric acid reactive substances (TBARS) were markedly lower in the patient than in the controls. On the seventh day after the surgery, we showed a further rise in the SFA, oleic acid, and TNF-α and reductions of n-6 PUFA and IL-6. Taken together, our results suggest that altered FA status, especially reduced PUFA, may influence hip fracture repair and even contribute to femoral fracture susceptibility in the elderly. A potential benefit from nutritional intervention with PUFA in prevention and (or) fracture healing should be considered.

Infertility is a growing problem worldwide. Currently, in vitro fertilization (IVF) is widely performed to treat infertility. However, a high percentage of IVF cycles fails, due to the poor developmental potential of the retrieved oocyte to generate viable embryos. Fatty acid content of the follicular microenvironment can affect oocyte maturation and the subsequent developmental competence. Saturated and monounsaturated fatty acids are mainly used by follicle components as primary energy sources whereas polyunsaturated fatty acids (PUFAs) play a wide range of roles. A large body of evidence supports the beneficial effects of n-3 PUFAs in prevention, treatment, and amelioration of some pathophysiological conditions including heart diseases, cancer, diabetes, and psychological disorders. Nevertheless, current findings regarding the effects of n-3 PUFAs on reproductive outcomes in general and on oocyte quality more specifically are inconsistent. This review attempts to provide a comprehensive overview of potential molecular mechanisms by which n-3 PUFAs affect oocyte maturation and developmental competence, particularly in the setting of IVF and thereby aims to elucidate the reasons behind current discrepancies around this topic.

N-3 polyunsaturated fatty acids for prevention of postoperative atrial fibrillation: updated meta-analysis and systematic review.

N-3 polyunsaturated fatty acids (PUFA) have been postulated to have an anti-arrhythmic effect on postoperative atrial fibrillation (POAF), with conflicting results among studies. This study on pooled data evaluated the effect of PUFA on POAF among patients undergoing cardiac surgery. The Pubmed, EMBASE, and CENTRAL databases were searched without restriction on language for randomized controlled trials on the effect of PUFA on POAF that were published before August 31, 2017. The incidence of POAF was extracted as primary endpoint. Pooled data were assessed by using a random-effects model. Out of 269 articles identified, 14 studies with 3570 patients were eligible and included in the meta-analysis. PUFA reduced incidence of POAF (RR 0.84 [95% CI 0.73-0.98], P = 0.03). The funnel plot and fail-safe number suggested insignificant publication bias. In sensitivity and subgroup analyses, (1) PUFA was effective in preventing POAF for eicosapentaenoic acid (EPA)/DHA < 1 (0.51 [0.36-0.73], P = 0.0003) but not EPA/DHA > 1 or unknown; (2) the efficacy in reducing POAF was apparent when placebo was usual care (0.59 [0.44-0.80], P = 0.0005), but not when placebo was non-fish oils; and (3) PUFA reduced POAF after CABG (0.68 [0.47-0.97], P = 0.03), but not other cardiac surgery. PUFA appears to reduce the incidence of POAF. However, the said protective effect may be influenced by EPA/DHA ratio, with < 1 appearing preferable. PUFA efficacy on POAF prevention appeared insignificant when compared with non-fish oils and only apparent in the setting of CABG alone. Further studies are needed to confirm the effect of PUFA on POAF and to assess the proper use of PUFA against POAF.

Curcumin and long-chain Omega-3 polyunsaturated fatty acids for Prevention of type 2 Diabetes (COP-D): study protocol for a randomised controlled trial.

BackgroundLifestyle interventions, including increase in physical activity and dietary counselling, have shown the ability to prevent type 2 diabetes (T2D) in high-risk state individuals, but the prevalence is still skyrocketing in Australia, in line with global prevalence. Currently, no medicines are approved by the Therapeutic Goods Administration in Australia for the management of prediabetes. Therefore, there is a need of developing a safer, biologically efficacious and cost-effective alternative for delaying the transition of individual health state from prediabetes into T2D. In the current trial we propose to evaluate the effects of curcumin and/or long-chain omega-3 polyunsaturated fatty acids on improving glycosylated haemoglobin as a primary outcome, along with secondary outcomes of glycaemic indices, lipid profile and inflammatory parameters.Methods/designEighty individuals diagnosed with prediabetes, aged between 30 and 70 years, will be randomly assigned to double placebo, curcumin alone, fish oil alone or double active groups according to a computer-generated randomisation sequence for 12 weeks. At baseline and post-intervention visits participants will be asked to provide blood samples and undergo body composition measurements. A blood sample is used for estimating glycaemic profiles, lipid profiles and inflammatory parameters (C-reactive protein, whole blood cell count, adiponectin, leptin, interleukin-6). The interim visit includes review on compliance with supplements based on capsule log and capsule count, adverse events and anthropometric measurements. In addition to these procedures, participants provide self-reported questionnaires on dietary intake (using a 3-day food record), a physical activity questionnaire and medical history.DiscussionThis trial aims to determine whether curcumin and/or long-chain omega-3 polyunsaturated fatty acids affect surrogate markers of glycaemic control which is relevant to delaying T2D. To date 38 participants completed the trial. No changes have been made to the clinical protocol post recruitment. The Effect of Walnut Oil Consumption on Blood Sugar in Patients With Diabetes Mellitus Type 2.

BackgroundPrevalence of diabetes mellitus type 2 (DM) is increasing globally. Considering the potential role of poly-unsaturated fatty acids in prevention of DM type 2 and lipid profiles improvement, some studies have been carried out on walnut. However, there are no studies on control of blood sugar in DM type 2 patients using walnut.ObjectivesThe current study aimed to evaluate the effect of walnut oil on blood sugar in DM type 2 patients.MethodsThis randomized control clinical trial was performed on 100 patients with DM type 2. For the experiment group (n = 50), walnut oil (15 g/day for three months) was added to their diet, while the control group (n = 50) did not undergo any interventions. Before initiation of the experiment and after the experiment, the systolic and diastolic blood pressure (SBP and DBP) levels, fasting blood sugar (FBS) and HbA1c were measured.ResultsThe two groups were not significantly different for SBP, DBP, body weight, and Body Mass Index. HbA1c level decreased significantly in the experiment group by 7.86% ± 21.97 (P = 0.005) from 7.00 ± 1.08 before the intervention to 6.37 ± 1.29 after the intervention. Also, FBS level decreased significantly by 8.24% ± 16.77 (P = 0.001); from 158.37 ± 48.16 before the intervention to 137.91 ± 23.24 after the intervention in the experimental group. These changes in the control group were not significant.ConclusionsConsumption of walnut oil (15 g/day for three months) was shown to improve blood glucose level but, no changes were noted for bodyweight and blood pressure in type two diabetic patients.

Omega-3 Polyunsaturated Fatty Acids in Prevention of Mood and Anxiety Disorders.

Psychiatric disorders in general, and major depression and anxiety disorders in particular, account for a large burden of disability, morbidity and premature mortality worldwide. Omega-3 polyunsaturated fatty acids (PUFAs) have a range of neurobiological activities in modulation of neurotransmitters, anti-inflammation, anti-oxidation and neuroplasticity, which could contribute to psychotropic effects. Here we reviewed recent research on the benefits of omega-3 PUFA supplements in prevention against major depression, bipolar disorders, interferon-α-induced depression patients with chronic hepatitis C viral infection, and posttraumatic stress disorder. The biological mechanisms underlying omega-3 PUFAs’ psychotropic effects are proposed and reviewed. Nutrition is a modifiable environmental factor that might be important in prevention medicine, which have been applied for many years in the secondary prevention of heart disease with omega-3 PUFAs. This review extends the notion that nutrition in psychiatry is a modifiable environmental factor and calls for more researches on prospective clinical studies to justify the preventive application of omega-3 PUFAs in daily practice.

Dietary n-3/long-chain n-3 polyunsaturated fatty acids for prevention of sporadic colorectal tumors: A randomized controlled trial in polypectomized participants

To address preventive effects of n-3 PUFAs/LC n-3 PUFAs on CRTs, a randomized controlled trial was conducted. One-hundred four experimental group participants were advised to increase intake of n-3 PUFAs, including fish/shell fish, fish oil supplements and perilla oils, and to decrease consumption of n-6 PUFAs and fats/oils as a whole for 24 months. One-hundred one control group participants were only cautioned to reduce consumption of fats/oils as a whole. Random allocation was satisfactorily attained, and participants sufficiently complied with our regimen. Intakes, plasma concentrations, and compositions of the RBC and sigmoid colon membranes of n-3 PUFAs, LC n-3 PUFAs, EPA and DHA increased, and the ratios of n-6 PUFAs/n-3 PUFAs and AA/LC n-3 PUFAs decreased without any adverse response. Twenty-four months after the intervention, the multivariate-adjusted hazard ratio (95% confidence intervals) was estimated to be 0.805 (0.536–1.209) with a signal towards the reduced CRT incidence.

Therapeutic potential of marine n-3 fatty acids in CABG patients

Dietary intake of marine n-3 polyunsaturated fatty acids (PUFA) has beneficial effects in various cardiac disorders. Few studies have, however, investigated the therapeutic potential of n-3 PUFA in patients undergoing coronary artery bypass grafting (CABG). Five heterogeneous randomized studies on n-3 PUFA for prevention of postoperative atrial fibrillation have yielded conflicting results. Increased venous graft patency rates following CABG were seen in another study in patients treated with n-3 PUFA. Finally, supplements with n-3 PUFA postoperatively have been associated with a lower risk of repeat revascularization and lower mortality in patients with poor ventricular function. Data are still few, and more studies are needed to clarify the therapeutic potential of n-3 PUFA in patients undergoing CABG.

Fish, Marine n−3 Fatty Acids, and Atrial Fibrillation – Experimental Data and Clinical Effects

Marine n−3 polyunsaturated fatty acids (PUFA) may have beneficial effects in relation to atrial fibrillation (AF) with promising data from experimental animal studies, however, results from studies in humans have been inconsistent. This review evaluates the mechanisms of action of marine n−3 PUFA in relation to AF based on experimental data and provides a status on the evidence obtained from observational studies and interventional trials. In conclusion, there is growing evidence for an effect of marine n−3 PUFA in prevention and treatment of AF. However, further studies are needed to establish which patients are more likely to benefit from n−3 PUFA, the timing of treatment, and dosages.

Polyunsaturated fatty acids for prevention of atrial fibrillation: a 'fishy' story

Bianconi et al ., on page 174 Several reports from population-based surveys and large randomized clinical trials have shown a remarkable 45–68% reduction in the rates of sudden cardiac death (presumably arrhythmic) with high dietary fish consumption or omega-3 polyunsaturated fatty acid (PUFA) supplements and have prompted interest in employing PUFAs as antiarrhythmic drugs.1,2 Three randomized clinical trials in patients at particularly high risk of sudden arrhythmic death, i.e. patients with internal cardioverter-defibrillators (ICDs) for secondary prevention, have followed, but surprisingly only one of these studies has shown any protective effect of PUFAs and one other even hinted at their proarrhythmic potential in some subgroups.3 Nevertheless, the quest into the antiarrhythmic effects of PUFAs has continued in patient populations with atrial fibrillation (AF). There appears to be a good rationale behind this. Universal constituents of biological membranes, PUFAs—mainly eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA)—regulate membrane fluidity, modulate the activity of multiple membrane proteins, and counteract the arrhythmogenic effects of atrial stretch.4 Other potential antifibrillatory mechanisms include anti-inflammatory and antioxidant actions, mitigation of endothelial dysfunction, and regulation of profibrotic activity of mitogen-activated protein kinases and matrix metalloproteinases.5 In addition, PUFAs may reduce the adverse impact of underlying heart disease, e.g. by vasodilatation, blood pressure reduction, and improved contractile function of the myocardium. However, most relevant antiarrhythmic action of PUFAs in AF probably results from their direct electrophysiological effects on several ion channels, such as I Na, I Kur, I KAch, I to, and I Ca,L currents, and the Na+/Ca2+ exchanger.6,7 The effects of PUFAs on structural …

PUFA for Prevention and Treatment of Dementia?

Polyunsaturated fatty acids (PUFA) play a crucial role in cerebral structure and function. Omega-3 PUFA is an exciting area of research, with docosahexaenoic acid (DHA) emerging as a new potential agent for prevention of cognitive decline and treatment of Alzheimer's disease. Preclinical studies suggest that DHA maintains membrane fluidity, improves synaptic and neurotransmitter functioning, enhances learning and memory performances and displays neuroprotective properties. Several epidemiological studies supported the association between Omega-3 PUFA consumption and a lower prevalence of dementia. Although data are divergent, a growing body of evidence supports the view that regular consumption of dietary fish and seafood (which are rich in omega-3 PUFA) prevents cognitive decline. Finally, at present, few data are available from randomized clinical trials (RCTs). on the association between cognition and Omega-3. Ongoing RCTs that assess the effect of Omega-3 might provide new evidence on prevention and treatment of dementia. In this review, we summarize preclinical and clinical research suggesting that DHA exerts beneficial effects on cognitive function with ageing.

Multi-targeted therapy of cancer by omega-3 fatty acids

Omega-3 (n-3) and omega-6 (n-6) polyunsaturated fatty acids (PUFAs) are essential fatty acids necessary for human health. Currently, the Western diet contains a disproportionally high amount of n-6 PUFAs and low amount of n-3 PUFAs, and the resulting high n-6/n-3 ratio is thought to contribute to cardiovascular disease, inflammation, and cancer. Studies in human populations have linked high consumption of fish or fish oil to reduced risk of colon, prostate, and breast cancer, although other studies failed to find a significant association. Nonetheless, the available epidemiological evidence, combined with the demonstrated effects of n-3 PUFAs on cancer in animal and cell culture models, has motivated the development of clinical interventions using n-3 PUFAs in the prevention and treatment of cancer, as well as for nutritional support of cancer patients to reduce weight loss and modulate the immune system. In this review, we discuss the rationale for using long-chain n-3 PUFAs in cancer prevention and treatment and the challenges that such approaches pose in the design of clinical trials.

Modulation of Bcl‐2 family and activation of caspase by linoleic acid in human gastric cancer cells

Recent epidemiologic studies and animal model studies support a possible role for polyunsaturated fatty acids in prevention of carcinogenesis or inhibition cancer cell growth. However, the molecular mechanisms of their anti-proliferative action on cancer cell growth are not known completely. In this study, we investigated the effects of linoleic acid on the cell proliferation of AGS human gastric cancer cells in order to understand its anti-proliferative mechanism. MTT assay indicated that treatment of AGS cells to linoleic acidresulted in the inhibition of cell proliferation in a concentration-dependent (50 uM ~ 200 uM) manner. The anti-proliferative effect of AGS cells by linoleic acid was associated with apoptotic cell death as measured by flow cytometric analysis and DAPI staining. Linoleic acid treatment resulted in the up-regulation of a pro-apoptotic Bax expression and a concomitant inhibition of anti-apoptotic Bcl-2 expression. Additionally, linoleic acid treatment induced the proteolytic activation of caspase-3, which is believed to play a central role in the apoptotic signaling pathway and degradation of poly(ADP-ribose) polymerase and phospholipase C-gamma 1 protein. Taken together, these findings provide important new insights into the possible molecular mechanisms of the anti-cancer activity of linoleic acid.
