Common Tumor in an uncommon location: Pleomorphic Adenoma of nasal cavity - a case Report.

Pleomorphic adenoma (PA) is the most common benign salivary gland tumor of which parotid gland involvement is the most common. PA may arise from minor salivary glands as well, however, PA is very rare in the sinonasal and nasopharyngeal areas. It usually affects middle aged females. They are frequently misdiagnosed due to high cellularity and myxoid stroma, leading to delay in diagnosis & further appropriate management. Here we present a case report of a female who presented with gradually progressive nasal obstruction, on examination found to have a nasal mass in right nasal cavity. A large and invasive pleomorphic adenoma of nasal cavity: a rare case report and review of literatures

BackgroundPleomorphic adenomas are common tumors of major salivary glands. However, the localization in nasal cavity originating from the lateral wall is exceptional.This work is a presentation of a very rare case that presents the diagnostic challenge, considering the multitude of malignant and benign differential diagnosis. A discussion of surgical approach, and the possible complications associated with it.Case presentationThis is the case of an invasive nasal pleomorphic adenoma in a 48-year-old patient; the patient underwent complete excision through combined left lateral rhinotomy and functional endoscopic sinus surgery.ConclusionPleomorphic adenoma is a very rare benign tumor of the nasal cavity. Clinical diagnosis is generally difficult regarding the multitude of differential diagnosis. Confirmation is established upon histological aspect.This is a case of a very rare large pleomorphic adenoma of nasal cavity originating from the lateral wall, associated with differential diagnosis, and surgical approaches, as well as perceivable complications.The strength of this work is to point out the importance of complete surgical extirpation and thorough follow-up to prevent recurrences and malignant transformation.

Endoscopic excision of a giant pleomorphic adenoma of the nasal septum

Pleomorphic adenoma of nasal cavity is very rare. Approximately 80% is found in the nasal septum while the remaining 20% is found in the lateral nasal wall. Histologically, pleomorphic adenoma of the nasal cavity differs from the tumor arising from the major salivary glands. It contains more cellularity and epithelial components than the stromal elements. The treatment is total excision with long term follow up as there is potential risk of recurrence and malignancy. Endonasal endoscopic excision has lesser morbidity with better prognosis. In this case report, we highlighted the successful endoscopic excision of an extensive posteriorly located pleomorphic adenoma.

Intranasal pleomorphic adenomas can arise either from septum or lateral nasal wall. They are very rare entity and occasionally misdiagnosed due to their atypical histopathology. Giant Intranasal Pleomorphic Adenoma: A Rare Case Report

ABSTRACT Pleomorphic adenoma, a heterogeneous mixture of epithelial and myoepithelial cells, commonly found in major salivary glands, accounts for about 3% of benign head and neck tumors. Intranasal pleomorphic adenoma (INPA) is an extremely rare variant forming a mere 8% of all pleomorphic adenomas. Majority of these arise from the nasal septum (82-90%) with only about 8% showing attachment to the lateral nasal wall. Various theories explaining etiopathological basis of INPA have been proposed but none explains its predominance in females in 3rd to 6th decade of life. Hallmark of histopathological picture of pleomorphic adenoma is epithelial tissue mixed with tissues of myxoid, chondroid or mucoid origin. But INPA shows predominantly epithelial component with scanty stroma. Considering its high cellularity and anatomical site, INPA is often misdiagnosed as unilateral nasal malignancy. Early diagnosis ensures complete excision and is paramount to make patient disease free, considering its high potential of recurrence. Hereby, we are presenting a case of giant pleomorphic adenoma of nasal cavity, arising from lateral nasal wall, causing erosion of the base of skull, with the intent of drawing attention to this rare diagnosis. Huge pleomorphic adenoma of nasal cavity.

Pleomorphic adenoma is a common benign tumour of the salivary gland that has elements of both epithelial and mesenchymal tissue. It is commonly seen in major salivary glands, parotid being the most common site followed by submandibular gland. However it can sometimes arise from minor salivary glands that are distributed throughout the upper aerodigestive tract and paranasal sinuses. We report a case of a 45 yrs. Old male patient, who presented with progressive nasal blockage and intermittent minor episodes of epistaxis from left nostril for 1 ½ years. Lateral Rhinotomy revealed a well encapsulated lesion attached to the nasal septum, which was excised completely. Histopathology revealed it to be Pleomorphic Adenoma without any features of malignancy. Lesion is a rare one hence reported.

患儿男,1岁.因左侧鼻塞流黄鼻涕3个月,左眼球突出1周于2003年5月9日收入院.患儿家长于3个月前发现患儿左侧鼻塞伴流黄鼻涕,睡眠时出现打鼾张口呼吸,并逐渐加重,1周前出现眼球突出.专科检查:鼻部外形无明显畸形,鼻中隔向右偏曲,左侧鼻腔内可见肿物生长,表面有脓性分泌物.左眼睑肿胀,左眼球向前外侧突出,眼球运动正常.CT示左侧鼻腔、上颌窦和筛窦有密度增强影。
