A call for increased surveillance: monitoring the use of hydroxychloroquine to prevent retinopathy

Dear Sir / Madam. Hydroxychloroquine (HCQ) and chloroquine (CQ) are the main drugs used in the control and treatment of malaria. Although the toxicity of these drugs is rare, it has been reported in high-dose ingestions or chronic intravenous administration. Of these retinal manifestations are one of the most common following the poisoning of CQ. A recent study by Melles et al. found that the overall risk of hydroxychloroquine retinopathy is 8.6% after chronic use with higher dose of HCQ dose being associated with a progressively greater risk for incident retinopathy (1). Retinopathy can lead to serious complications in patients; in fact in most patient’s vision loss. In Melles et al’s study, it mostly led to complaints like reading difficulties, diminished vision, missing center vision, glare, hazy vision, light flashes, and metamorphopsia, while some patients were asymptomatic. The majority of patients exhibited a fundoscopic bull's-eye sign. Other signs include paracentral, central, and peripheral visual field loss, which were present in almost all patients. In the early stages of retinopathy, color vision is typically unaffected, but it too begins to be compromised in the later stages. (2) Malaria is endemic in Pakistan. In 2023 pooled malaria prevalence in Pakistan was found to be 23.3%, with Plasmodium vivax, Plasmodium falciparum, and mixed infection rates of 79.13%, 16.29%, and 3.98%, respectively. According to the World Health Organization (WHO), Pakistan is one of the seven countries in the Eastern Mediterranean Region that account for 98% of the total malaria burden in the region. Around 217 million people in Pakistan are at moderate risk of malaria and 63 million are at high risk. Approximately 0.47 million malaria cases and 800 deaths have been reported in 2020(3). The recent rise in climate change-induced flooding and other weather changes, waterborne diseases such as malaria, are also rising leading to higher HCQ use. Furthermore, CQ and HCQ are also being used in treating covid 19(4). This, combined with factors such as lack of surveillance, self-medication practises, and rampant over-the-counter use in Pakistan greatly increase the risk of patients taking improperly high doses of HCQ or CQ and, consequently, suffering from retinopathy. To prevent the HCQ-associated retinal toxicity proper surveillance methods should be employed, and the prescription and distribution of these drugs should be monitored. Molecular and entomological surveillance of malaria vectors in urban and rural communities of Benguela Province, Angola

BackgroundMalaria is a major public health problem in Angola, with Anopheles gambiae sensu lato (s.l.) and An. funestus s.l. being the primary vectors. This study aimed to clarify the information gaps concerning local Anopheles mosquito populations. Our objectives were to assess their abundance, geographical dispersion, and blood-feeding patterns. We also investigated their insecticide resistance. Molecular methods were used to identify sibling species, determine the origin of blood meals, measure Plasmodium falciparum infection rates, and detect the presence of knockdown resistance (kdr) mutations.MethodsAdult mosquitoes were collected indoors using CDC light traps from nine randomly selected households at two sentinel sites with distinct ecological characteristics. The samples were collected from 1 February to 30 June 2022. Anopheles mosquitoes were morphologically identified and subjected to molecular identification. Unfed Anopheles females were tested for the presence of P. falciparum DNA in head and thorax, and engorged females were screened for the source of the blood meals. Additionally, members of An. gambiae complex were genotyped for the presence of the L1014F and L1014S kdr mutations.ResultsIn total, 2226 adult mosquitoes were collected, including 733 Anopheles females. Molecular identification revealed the presence of Anopheles coluzzii, An. gambiae senso stricto (s.s.), An. arabiensis, and An. funestus s.s. Notably, there was the first record of An. coluzzii/An. gambiae s.s. hybrid and An. vaneedeni in Benguela Province. Plasmodium falciparum infection rates for An. coluzzii at the urban sentinel site and An. funestus s.s. at the rural site were 23.1% and 5.7%, respectively. The L1014F kdr mutation was discovered in both resistant and susceptible An. coluzzii mosquitoes, while the L1014S mutation was detected in An. gambiae s.s. for the first time in Benguela Province. No kdr mutations were found in An. arabiensis.ConclusionsThis study provides valuable insights into the molecular characteristics of malaria vectors from the province of Benguela, emphasising the need for continuous surveillance of local Anopheles populations regarding the establishment of both kdr mutations for tailoring vector control interventions.Graphical abstract

A High Proportion of Malaria Vector Biting and Resting Indoors despite Extensive LLIN Coverage in Côte d'Ivoire.

Malaria is still a leading cause mortality in Côte d'Ivoire despite extensive LLINs coverage. We present the results of an entomological survey conducted in a coastal and in an inland village with the aim to estimate Anopheles gambiae sensu lato (s.l.) female's abundance indoor/outdoor and Plasmodium falciparum infection rate and analyze the occurrence of blood-feeding in relation to LLINs use. Pyrethrum spray (PSC) and window exit traps (WT) collections were carried out to target endophagic/endophilic and endophagic/exophilic females, respectively. Data on LLINs use in sampled houses were collected. (1) high levels of malaria transmission despite LLINs coverage >70% (~1 An. gambiae s.l. predicted mean/person/night and ~5% Plasmodium falciparum infection rate); (2) 46% of females in the PSC sample were blood-fed, suggesting that they fed on an unprotected host inside the house; (3) 81% of females in WT were unfed, suggesting that they were leaving the house to find an available host. Model estimates that if everyone sleeps under LLINs the probability for a mosquito to bite decreases of 48% and 95% in the coastal and inland village, respectively. The results show a high proportion of mosquito biting and resting indoors despite extensive LLINs. The biological/epidemiological determinants of accounting for these results merit deeper investigations.

Malaria prevalence in Pakistan: A systematic review and meta-analysis (2006–2021)

Malaria is one of the major public health issues globally. Malaria infection spreads through mosquito bites from infected female Anopheles mosquitoes. This study aims to conduct a systematic review and meta-analysis on malaria prevalence in Pakistan from 2006 to 2021. We searched PubMed, Science Direct, EMBASE, EMCare, and Google Scholar to acquire data on the prevalence of malaria infections. We performed a meta-analysis with a random-effects model to obtain the pooled prevalence of malaria, Plasmodium vivax, and Plasmodium falciparum. Meta-analysis was computed using R 4.1.2 Version statistical software. I2 and time series analysis were performed to identify a possible source of heterogeneity across studies. A funnel plot and the Freeman-Tukey Double Arcsine Transformed Proportion were used to evaluate the presence of publication bias. Out of the 315 studies collected, only 45 full-text articles were screened and included in the final measurable meta-analysis. Pooled malaria prevalence in Pakistan was 23.3%, with Plasmodium vivax, Plasmodium falciparum, and mixed infection rates of 79.13%, 16.29%, and 3.98%, respectively. Similarly, the analysis revealed that the maximum malaria prevalence was 99.79% in Karachi and the minimum was 1.68% in the Larkana district. Amazingly, this systematic review and meta-analysis detected a wide variation in malaria prevalence in Pakistan. Pakistan's public health department and other competent authorities should pay close attention to the large decrease in mosquito populations to curb the infection rate.

Malaria Transmission and Vector Resistance to Insecticides in a Changing Environment: Case of Simbock in Yaoundé-City, Cameroon

Ecological upheavals resulting from uncontrolled urbanization can lead to significant changes in vector borne diseases’ profiles, thus requiring a thorough revision of their prevention and control strategies. The current study aimed at characterizing malaria vector populations in the Simbock neighborhood of Yaoundé-city (Cameroon), in relation to its urbanization scheme. Adult mosquitoes were captured by human landing catches (HLC) in- and outdoors prior to (2000–2006) and during infrastructural development (2014–2016). Anophelines were morphologically identified and analyzed for Plasmodium (P.) falciparum circumsporozoite protein detection using the ELISA technique. Species of the Anopheles (An.) gambiae complex were identified using SINE-PCR. Adult An. gambiae s.l. from larvae collected between 2014 and 2017 were tested for susceptibility to insecticides (0.1% bendiocarb, 4% DDT, 0.75% permethrin and 0.05% deltamethrin) with or without piperonyl butoxide (PBO) synergist, using WHO standard bioassays. The Hot Oligonucleotide Ligation Assay was used to detect the knockdown resistance (kdr) L995F/S mutations. Overall, nine malaria vector species were identified in 2000-2006, mostly An. moucheti (49%), An. nili (13.5%) and An. gambiae s.l. (12%); the six remaining species were represented at less than 3% each. However, only three species were found in 2014-2016, with increasing proportions of An. gambiae s.l. (67%) and An. funestus (32%) (P<0.0001). An. gambiae s.l. consisted An. coluzzii (> 85%) and An. gambiae (<15%) species during the two study periods. Plasmodium falciparum infection rates were 2.1% and 1.0% in 2000-2006 and 2014-2016 respectively (P=0.4), with decreasing entomological inoculation rates (EIR) from 0.34 infective bites per man per night (ib/m/n) to 0.02 ib/m/n (P<0.0001). Anopheles gambiae s.l. was resistant to DDT and permethrin [<40% mortality rates (MR)], and deltamethrin (65-89% MR), but fully susceptible to bendiocarb (100% MR). Pre-exposure of mosquitoes to PBO resulted in 90-100% MR to deltamethrin but not to permethrin. Furthermore, the two kdr L995F/S resistance alleles were recorded at 0.64 and 0.006 frequencies respectively. This study highlights a shift from rural to urban malaria transmission in Simbock, coupled with DDT and pyrethroid resistance in An. gambiae s.l. Combination vector control interventions, e.g., PBO nets and bendiocarb indoor residual spraying are needed in such areas.

Species Composition and Plasmodium falciparum Infection Rates of Anopheles gambiae s.l. Mosquitoes in Six Localities of Kwara State, North Central, Nigeria

Entomological data gathering is essential for monitoring malaria vector disease risks and selection of appropriate interventions for the protection of exposed human populations. This study assessed the relative abundance, species composition, and sporozoite infection rates of indoor resting An. gambiae s.l. malaria vectors in six communities across 3 Local Government Areas in Kwara State, Nigeria. Total number of mosquitoes collected by Pyrethrum Spray Catch method over a period of eighth months were correlated with rainfall values in the area. Plasmodium falciparum sporozoite infection rates and sibling species identification of collected An. gambiae s.l. mosquito samples were determined by ELISA and PCR respectively. Results showed a positive correlation (r = 0.639, p = 0.08) between rainfall and numbers of Anopheles mosquitoes in the study areas. The overall composition of the An. gambiae s.l sibling species in the collected samples from all the six communities showed the predominance of An. gambiae s.s 298 (75.3%) compared to An. coluzzii 94(23.7%) and An. arabiensis 4(1.0%). However, the sporozoite infection rate of An. coluzzii (22.3%) was higher compared to An. gambiae s.s (12.8%) and An. arabiensis (0%). Mean numbers of An. gambiae s.l mosquitoes were significantly higher in Ilorin west LGA compared to Asa (F = 17.81, P < 0.001) and Ilorin East LGAs (F = 22.81, P < 0.001). Sporozoite rates of both An. gambiae s.s and An. coluzzii sibling species were higher in Ilorin West communities (Aiyede 21%, Ogundele 32%) compared to Asa (Idi Emi 11.1%, Lasoju 5.1%) and Ilorin East (Oke Oyi 2.4%, Ote-efan 0%) communities. Prevalence of sporozoite-infected An. gambiae s.s and An. coluzzii indoors highlight the need for effective insecticide treated bed-nets interventions to protect the residents from malaria risks. Higher numbers of An. coluzzii in the swampy rice marshed Ilorin West LGA communities require larval source management as an additional strategy for effective malaria vector control.

<i>Anopheles leesoni</i> Evans 1931, a Member of the <i>Anopheles funestus</i> Group, Is a Potential Malaria Vector in Cameroon

Background: Understanding the biology of Anopheles malaria vector species is essential to planning effective and sustainable malaria control strategies in endemic countries. This study reported the implication of Anopheles leesoni in malaria transmission in Cameroon, Central Africa. Methods: Mosquitoes were collected in three localities from May 2015 to March 2018 using electric aspirators and Centers for Disease Control light traps (CDC-LT). Anopheles funestus sensu lato (s.l.) mosquitoes were identified as species using polymerase chain reaction assay (PCR). Furthermore, Plasmodium falciparum infection status was determined using the enzyme-linked immunosorbent assay (ELISA) method. Results: A total of 12,744 Anopheles mosquitoes were collected by electric aspirator (N = 4844) and CDC-LT (N = 7900). Anopheles funestus s.l. (86.95%) was the major species and the main malaria vector in rural savannah and rural forest sites followed by A. gambiae s.l. (13.05%) whereas in urban areas, A. gambiae s.l. was by far the most abundant representing 91.45% of Anopheles mosquitoes collected. Two members of the A. funestus group were identified among 1389 analysed by PCR: 1307 A. funestus sensu stricto (s.s.) (94.10%) and 82 A. leesoni (5.9%). Plasmodium falciparum infection rate was 21.04% in A. funestus s.s. For the first time, A. leesoni was found positive for P. falciparum (infection rate: 10.98%) in Cameroon. Conclusion: A very high P. falciparum infection rate was observed in this study in A. funestus s.s., highlighting its high implication in malaria transmission in Cameroon. Furthermore, the detection of P. falciparum infection in A. leesoni calls for more attention towards this neglected vector species.

Entomological and Molecular Surveillance of Anopheles Mosquitoes in Freetown, Sierra Leone, 2019.

Background: Malaria is endemic in Sierra Leone, with stable and perennial transmission in all parts of the country. At present, the main prevention and control measures for mosquito vectors here involve insecticide treated nets (ITN) and indoor residual spraying (IRS). The most recent entomological surveillance was conducted prior to the civil war, between 1990 and 1994. Therefore, a new entomological surveillance required to support targeted malaria control strategies.Methods: Anopheles mosquitoes were collected between June and December 2019 using the light trap method. On these, we conducted species identification, analyzed seasonal fluctuation and Plasmodium infection rate, and monitored insecticide resistance.Results: Surveillance of seasonal fluctuation showed that there were two peak of Anopheles density in July (mean 13.67 mosquitoes/trap/night) and October (mean 13.00 mosquitoes/trap/night). Meanwhile, the lowest Anopheles density was seen in early September. Ninety-one representatives of Anopheles gambiae s.l. were selected and identified as An. coluzzii (n = 35) and An. gambiae s.s. (n = 56) using PCR. An. coluzzii and An. gambiae s.s. were found to be heterozygous resistant to the knockdown resistance (kdr) L1014F mutation (100%). Meanwhile, the East African mutation (kdr L1014S) was absent in the tested mosquitoes. Three mosquitoes that tested positive for the parasite, had an individual Plasmodium falciparum infection rate of 12.50, 16.67, and 14.29%. The sampling dates of positive mosquitoes were distributed in the two periods of peak Anopheles mosquito density.Conclusion: This study identified the dominant Anopheles species in Freetown as An. gambiae while the predominant species within the An. gambiae complex was An. gambiae sensu stricto. Surveillance of seasonal fluctuations and high P. falciparum infection rates in Anopheles indicate that the alternation of drought and rainy seasons from June to July, and from October to November, are the key periods for malaria control and prevention in Freetown, Sierra Leone. The high frequency of kdr allele mutations in An. gambiae calls for close monitoring of vector susceptibility to insecticides and tracing of resistance mechanisms in order to develop more effective vector control measures and strategies.

Identification of main malaria vectors and their insecticide resistance profile in internally displaced and indigenous communities in Eastern Democratic Republic of the Congo (DRC)

BackgroundMalaria remains a major public health concern in the Democratic Republic of the Congo (DRC) and its control is affected by recurrent conflicts. Médecins Sans Frontières (MSF) initiated several studies to better understand the unprecedented incidence of malaria to effectively target and implement interventions in emergency settings. The current study evaluated the main vector species involved in malaria transmission and their resistance to insecticides, with the aim to propose the most effective tools and strategies for control of local malaria vectors.MethodsThis study was performed in 52 households in Shamwana (Katanga, 2014), 168 households in Baraka (South Kivu, 2015) and 269 households in Kashuga (North Kivu, 2017). Anopheles vectors were collected and subjected to standardized Word Health Organization (WHO) and Center for Disease Control (CDC) insecticide susceptibility bioassays. Mosquito species determination was done using PCR and Plasmodium falciparum infection in mosquitoes was assessed by ELISA targeting circumsporozoite protein.ResultsOf 3517 Anopheles spp. mosquitoes collected, Anopheles gambiae sensu lato (s.l.) (29.6%) and Anopheles funestus (69.1%) were the main malaria vectors. Plasmodium falciparum infection rates for An. gambiae s.l. were 1.0, 2.1 and 13.9% for Shamwana, Baraka and Kashuga, respectively. Anopheles funestus showed positivity rates of 1.6% in Shamwana and 4.4% in Baraka. No An. funestus were collected in Kashuga. Insecticide susceptibility tests showed resistance development towards pyrethroids in all locations. Exposure to bendiocarb, malathion and pirimiphos-methyl still resulted in high mosquito mortality.ConclusionsThis is one of only few studies from these conflict areas in DRC to report insecticide resistance in local malaria vectors. The data suggest that current malaria prevention methods in these populations are only partially effective, and require additional tools and strategies. Importantly, the results triggered MSF to consider the selection of a new insecticide for indoor residual spraying (IRS) and a new long-lasting insecticide-treated net (LLIN). The reinforcement of correct usage of LLINs and the introduction of targeted larviciding were also included as additional vector control tools as a result of the studies.

Studies on Plasmodium falciparum Infection Rates among Patients Attending General Hospitals in Benue State, Nigeria

Plasmodium falciparum is the most virulent and prevalent malaria parasite in Nigeria .This study aimed to determine the prevalence of malaria infection among patients at General Hospitals in Benue State. A total of 1200 patients were examined in this study. Blood samples were collected by finger prick onto clean slides and into the round sample well of PfRDTs. Thick and thin blood films were prepared for microscopic examination. The overall prevalence of malaria infection was 34.8%. A questionnaire was used to determine some demographic factors. Prevalence of malaria in relation to residence, rural area recorded higher prevalence of 42.2% than urban area with prevalence of 23.8%. Chi square analysis showed a significant difference (p < 0.05) in prevalence in relation to residence. The Prevalence of malaria in relation to age groups, age between 6-10 and 7-15 recorded higher infection rate of 54.5% and 51.5% respectively. While, age group >46 recorded 17.5%. The female patients 36.2% were more infected than the males 33.1%.Patients that had informal education recorded higher prevalence rate of 89.2% and those that are farmers had 57.9%. Chi square analysis however showed that the difference was significant (p < 0.05). A significant difference (P<0.05) was observed between patients that used insecticide spray alone as malaria preventive methods (70.1%) compared to patients that used combined methods of prevention (17.2%). Malaria still remains prevalent among patients in Benue State, Nigeria.

Uneven malaria transmission in geographically distinct districts of Bobo-Dioulasso, Burkina Faso

BackgroundUrbanization is a main trend in developing countries and leads to health transition. Although non-communicable diseases are increasing in cities of low-income countries, vector-borne diseases such as malaria, are still present. In the case of malaria, transmission is lower than in rural areas, but is uneven and not well documented. In this study, we wanted to evaluate intra-urban malaria transmission in a West African country (Burkina Faso).MethodsA cross-sectional study on 847 adults (35 to 59 year-old) and 881 children (6 months to 5 year-old) living in 1045 households of four districts (Dogona, Yeguere, Tounouma and Secteur 25) of Bobo-Dioulasso was performed between October and November 2013. The districts were selected according to a geographical approach that took into account the city heterogeneity. Malaria prevalence was evaluated using thick and thin blood smears. Human exposure to Anopheles bites was measured by assessing the level of IgG against the Anopheles gSG6-P1 salivary peptide. Adult mosquitoes were collected using CDC traps and indoor insecticide spraying in some houses of the four neighbourhoods. The Anopheles species and Plasmodium falciparum infection rate were determined using PCR assays.ResultsIn this study, 98.5% of the malaria infections were due to Plasmodium falciparum. Malaria transmission occurred in the four districts. Malaria prevalence was higher in children than in adults (19.2 vs 4.4%), and higher in the central districts than in the peripheral ones (P = 0.001). The median IgG level was more elevated in P. falciparum-infected than in non-infected individuals (P < 0.001). Anopheles arabiensis was the main vector identified (83.2%; 227 of the 273 tested mosquito specimens). Five P. falciparum-infected mosquitoes were caught, and they were all caught in the central district of Tounouma where 28.6% (14/49) of the tested blood-fed mosquito specimens had a human blood meal.ConclusionsThis study showed that urban malaria transmission occurred in Bobo-Dioulasso, in all the four studied areas, but mainly in central districts. Environmental determinants primarily explain this situation, which calls for better urban management.

Elevated Plasmodium infection rates and high pyrethroid resistance in major malaria vectors in a forested area of Cameroon highlight challenges of malaria control

BackgroundHigh coverage of long-lasting insecticidal nets (LLINs) is the cornerstone of the malaria control strategy of the national malaria control program (NMCP) in Cameroon, with a target of reducing malaria transmission to less than 10% by 2035. To this end, more than 20 million LLINs have been distributed to populations countrywide since 2011. The present study evaluated entomological indices and Anopheles susceptibility to pyrethroids in a rural forested area of south Cameroon with high coverage of LLINs.MethodsThe study was conducted between July 2014 and May 2016 in Obout, a village located in a rural forested area in south Cameroon. Resting mosquitoes were collected using electric aspirators and were identified to species using morphological criteria and PCR tools. Mosquito feeding preferences and infection status to Plasmodium falciparum were determined by ELISA and using TaqMan assays. The susceptibility of wild F1 adults to pyrethroids was monitored using WHO insecticide susceptibility bioassays.ResultsDuring the study period, 5,993 Anopheles mosquitoes were collected indoors both in rooms with and without nets. Two main vector species, namely An. funestus and An. gambiae, were identified in the locality, with An. funestus being by far the most abundant (89.68%). ELISA analysis revealed high percentage of blood meal taken exclusively on human (97.65–98.95%) supporting the high antropohilic behaviour of both species. Plasmodium falciparum infection rate detected by ELISA was high throughout the study period and varied between 3.28–14.04% (mean: 10.40%) in An. funestus, and between 5.55–22.22% (mean: 13.87%) in An. gambiae. This trend was confirmed by TaqMan assays, with P. falciparum infection prevalence of 23.33% in An. funestus. Significant decrease of mortality associated with high frequency of kdr mutation was observed in An. gambiae (deltamethrin: 36.6–56.45%; permethrin: 6–18.65%) indicating high level of resistance to pyrethroids. For An. funestus, resistance was marked for deltamethrin (mortality: 70.54–76.24%) than for permethrin (94.12–94.74%).ConclusionsOur study showed that despite LLINs, the population of Obout remains exposed to bites of highly infected An. funestus and An. gambiae mosquitoes, highlighting the challenges to controlling malaria in forested areas, especially in the presence of insecticide resistance.

Status of insecticide resistance in malaria vectors in Kwale County, Coastal Kenya

BackgroundThe strategy for malaria vector control in the context of reducing malaria morbidity and mortality has been the scale-up of long-lasting insecticidal nets to universal coverage and indoor residual spraying. This has led to significant decline in malaria transmission. However, these vector control strategies rely on insecticides which are threatened by insecticide resistance. In this study the status of pyrethroid resistance in malaria vectors and it’s implication in malaria transmission at the Kenyan Coast was investigated.ResultsUsing World Health Organization diagnostic bioassay, levels of phenotypic resistance to permethrin and deltamethrin was determined. Anopheles arabiensis showed high resistance to pyrethroids while Anopheles gambiae sensu stricto (s.s.) and Anopheles funestus showed low resistance and susceptibility, respectively. Anopheles gambiae sensu lato (s.l.) mosquitoes were further genotyped for L1014S and L1014F kdr mutation by real time PCR. An allele frequency of 1.33% for L1014S with no L1014F was detected. To evaluate the implication of pyrethroid resistance on malaria transmission, Plasmodium falciparum infection rates in field collected adult mosquitoes was determined using enzyme linked immunosorbent assay and further, the behaviour of the vectors was assessed by comparing indoor and outdoor proportions of mosquitoes collected. Sporozoite infection rate was observed at 4.94 and 2.60% in An. funestus s.l. and An. gambiae s.l., respectively. A higher density of malaria vectors was collected outdoor and this also corresponded with high Plasmodium infection rates outdoor.ConclusionsThis study showed phenotypic resistance to pyrethroids and low frequency of L1014S kdr mutation in An. gambiae s.l. The occurrence of phenotypic resistance with low levels of kdr frequencies highlights the need to investigate other mechanisms of resistance. Despite being susceptible to pyrethroids An. funestus s.l. could be driving malaria infections in the area.

Composition of Anopheles mosquitoes, their blood-meal hosts, and Plasmodium falciparum infection rates in three islands with disparate bed net coverage in Lake Victoria, Kenya

BackgroundSmall islands serve as potential malaria reservoirs through which new infections might come to the mainland and may be important targets in malaria elimination efforts. This study investigated malaria vector species diversity, blood-meal hosts, Plasmodium infection rates, and long-lasting insecticidal net (LLIN) coverage on Mageta, Magare and Ngodhe Islands of Lake Victoria in western Kenya, a region where extensive vector control is implemented on the mainland.ResultsFrom trapping for six consecutive nights per month (November 2012 to March 2015) using CDC light traps, pyrethrum spray catches and backpack aspiration, 1868 Anopheles mosquitoes were collected. Based on their cytochrome oxidase I (COI) and intergenic spacer region PCR and sequencing, Anopheles gambiae s.l. (68.52%), Anopheles coustani (19.81%) and Anopheles funestus s.l. (11.67%) mosquitoes were differentiated. The mean abundance of Anopheles mosquitoes per building per trap was significantly higher (p < 0.001) in Mageta than in Magare and Ngodhe. Mageta was also the most populated island (n = 6487) with low LLIN coverage of 62.35% compared to Ngodhe (n = 484; 88.31%) and Magare (n = 250; 98.59%). Overall, 416 (22.27%) engorged Anopheles mosquitoes were analysed, of which 41 tested positive for Plasmodium falciparum infection by high-resolution melting (HRM) analysis of 18S rRNA and cytochrome b PCR products. Plasmodium falciparum infection rates were 10.00, 11.76, 0, and 18.75% among blood-fed An. gambiae s.s. (n = 320), Anopheles arabiensis (n = 51), An. funestus s.s. (n = 29), and An. coustani (n = 16), respectively. Based on HRM analysis of vertebrate cytochrome b, 16S rRNA and COI PCR products, humans (72.36%) were the prominent blood-meal hosts of malaria vectors, but 20.91% of blood-meals were from non-human vertebrate hosts.ConclusionsThese findings demonstrate high Plasmodium infection rates among the primary malaria vectors An. gambiae s.s. and An. arabiensis, as well as in An. coustani for the first time in the region, and that non-human blood-meal sources play an important role in their ecology. Further, the higher Anopheles mosquito abundances on the only low LLIN coverage island of Mageta suggests that high LLIN coverage has been effective in reducing malaria vector populations on Magare and Ngodhe Islands.

Preliminary survey on Anopheles species distribution in Botswana shows the presence of Anopheles gambiae and Anopheles funestus complexes

BackgroundBotswana is one of the four front line malaria elimination countries in Southern Africa, with malaria control activities that include routine vector control. Past and recent studies have shown that Anopheles arabiensis is the only known vector of Plasmodium parasites in the country. This report presents a preliminary evaluation on Anopheles species composition in seven districts of Botswana with some inferences on their vectorial role.ResultsOverall, 404 Anopheles mosquito females were collected, of which 196 were larvae collected from several breeding sites, and 208 were adults obtained from indoor pyrethrum spray catches (PSC). Anopheles arabiensis (58.9%) accounted for the highest relative frequency in 5 out of 7 districts sampled. The other species collected, among those identified, were barely represented: Anopheles longipalpis type C (16.3%), Anopheles parensis (8.9%), Anopheles quadriannulatus (5.4%), and Anopheles leesoni (0.2%). PCR test for human β-globin on mosquitoes collected by PSC showed that An. arabiensis and An. parensis had bitten human hosts. Moreover, An. arabiensis showed a non-negligible Plasmodium falciparum infection rate in two sites (3.0% and 2.5% in Chobe and Kweneng West districts, respectively).ConclusionsThis work provides first time evidence of Anopheles diversity in several areas of Botswana. Anopheles arabiensis is confirmed to be widespread in all the sampled districts and to be vector of P. falciparum. Moreover, the presence of Anopheles funestus group in Botswana has been assessed. Further research, entomological surveillance activities and possibly vector control programmes need to be better developed and implemented as well as targeting outdoors resting vectors.

Malaria impact of large dams in sub-Saharan Africa: maps, estimates and predictions.

BackgroundWhile there is growing recognition of the malaria impacts of large dams in sub-Saharan Africa, the cumulative malaria impact of reservoirs associated with current and future dam developments has not been quantified. The objective of this study was to estimate the current and predict the future impact of large dams on malaria in different eco-epidemiological settings across sub-Saharan Africa.MethodsThe locations of 1268 existing and 78 planned large dams in sub-Saharan Africa were mapped against the malaria stability index (stable, unstable and no malaria). The Plasmodium falciparum infection rate (PfIR) was determined for populations at different distances (<1, 1–2, 2–5, 5–9 km) from the associated reservoirs using the Malaria Atlas Project (MAP) and WorldPop databases. Results derived from MAP were verified by comparison with the results of detailed epidemiological studies conducted at 11 dams.ResultsOf the 1268 existing dams, 723 are located in malarious areas. Currently, about 15 million people live in close proximity (<5 km) to the reservoirs associated with these dams. A total of 1.1 million malaria cases annually are associated with them: 919,000 cases due to the presence of 416 dams in areas of unstable transmission and 204,000 cases due to the presence of 307 dams in areas of stable transmission. Of the 78 planned dams, 60 will be located in malarious areas and these will create an additional 56,000 cases annually. The variation in annual PfIR in communities as a function of distance from reservoirs was statistically significant in areas of unstable transmission but not in areas of stable transmission.ConclusionIn sub-Saharan Africa, dams contribute significantly to malaria risk particularly in areas of unstable transmission. Additional malaria control measures are thus required to reduce the impact of dams on malaria.

Anopheles arabiensis seasonal densities and infection rates in relation to landscape classes and climatic parameters in a Sahelian area of Senegal.

BackgroundThe influence of environmental and climatic factors on malaria vector bionomics and transmission is an important topic in the context of climatic change particularly at macro-geographical level. Sahelian areas could be particularly affected due to heterogeneous features including high inter-annual variability in rainfall and others associated parameters. Therefore, baseline information on the impact of environmental and climatic factors on malaria transmission at micro-geographical level is required for vector risk management and implementation of control strategies.MethodsMalaria vectors were collected indoors by pyrethrum spray catches in 14 villages belonging to 4 different landscape classes (wooded savanna, shrubby savanna, bare soils and steppe) in the sylvo-pastoral area of Senegal. Plasmodium falciparum infection rates were determined using an indirect enzyme-linked immunosorbent assay (ELISA).ResultsAn. arabiensis was the predominant species in all landscape classes and was the only species collected at the end of the rainy season excepted in villages located in bare soils where it cohabited with An. coluzzii. Mean temperature and relative humidity showed similar variations in all the landscape classes covered whereas rainfall was more heterogeneous in terms of pattern, frequency and amount. The mean densities of An. arabiensis displayed high seasonal differences with peaks observed in August or September. A positive non-significant correlation was observed between An. arabiensis densities for rainfall and humidity whereas a negative non-significant correlation was reported for temperature. Plasmodium falciparum-infected mosquitoes were detected only in wooded savanna and bare soils villages.ConclusionsThese observations suggest key roles played by landscape classes and rainfall in malaria vector densities, infection rates and malaria transmission that could be more pronounced in villages situated in wooded savanna and bare soils. Due to the close relationship between environmental and meteorological parameters in this Sahelian region, additional studies on the impact of these parameters are required to further ascertain their association with entomological parameters involved in malaria transmission. Plasmodium falciparum infection rates for some Anopheles spp. from Guinea-Bissau, West Africa.

Presence of Plasmodium falciparum circumsporozoite protein (CSP) was detected by enzyme linked immunosorbent assay (ELISA) in a sample of Anopheles gambiae s.s., A. melas and A. pharoensis collected in Guinea-Bissau during October and November 2009. The percentage of P. falciparum infected samples (10.2% overall; confidence interval (CI): 7.45-13.6%) was comparable to earlier studies from other sites in Guinea-Bissau (9.6-12.4%). The majority of the specimens collected were identified as A. gambiae which had an individual infection rate of 12.6 % (CI: 8.88-17.6) across collection sites. A small number of specimens of A. coluzzii, A. coluzzii x A. gambiae hybrids, A. melas and A. pharoensis were collected and had infection rates of 4.3% (CI:0.98-12.4), 4.1% (CI:0.35-14.5), 11.1% (CI:1.86-34.1) and 33.3% (CI:9.25-70.4) respectively. Despite being present in low numbers in indoor collections, the exophilic feeding behaviors of A. melas (N=18) and A. pharoensis (N=6) and high infection rates observed in this survey suggest falciparum-malaria transmission potential outside of the protection of bed nets.

Insecticide resistance in malaria vector mosquitoes at four localities in Ghana, West Africa

BackgroundMalaria vector control programmes that rely on insecticide-based interventions such as indoor house spraying with residual insecticides or insecticide treated bed nets, need to base their decision-making process on sound baseline data. More and more commercial entities in Africa, such as mining companies, are realising the value to staff productivity of controlling malaria transmission in their areas of operation.This paper presents baseline entomological data obtained during surveys conducted for four mining operations in Ghana, West Africa.ResultsThe vast majority of the samples were identified as Anopheles gambiae S form with only a few M form specimens being identified from Tarkwa. Plasmodium falciparum infection rates ranged from 4.5 to 8.6% in An. gambiae and 1.81 to 8.06% in An. funestus. High survival rates on standard WHO bioassay tests were recorded for all insecticide classes except the organophosphates that showed reasonable mortality at all locations (i.e. > 90%). The West African kdr mutation was detected and showed high frequencies in all populations.ConclusionsThe data highlight the complexity of the situation prevailing in southern Ghana and the challenges facing the malaria vector control programmes in this region. Vector control programmes in Ghana need to carefully consider the resistance profiles of the local mosquito populations in order to base their resistance management strategies on sound scientific data.
