On the Relationship between MMA Levels in Blood Products and Donor Sex, Age, and Donation Frequency.

As the aging process accelerates, the age structure of blood donors turns to older and even aged groups. Methylmalonic acid (MMA), a byproduct of propionate metabolism, may be upregulated in the serum of older adults. As a mediator of chronic disease and tumor progression, the MMA content in blood products has become the focus of research. Absolute concentrations of MMA in blood products were determined based on the liquid chromatography-tandem mass spectrometry, so as to analyze how they were affected by donors' age, sex, and frequency of blood donation. The MMA content in leukocyte-depleted suspended red blood cell (lds-RBC) was significantly higher than that in fresh plasma (p<0.0001). The MMA content among five age groups showed no difference in either fresh plasma or lds-RBCs. The MMA content in fresh plasma was similar in the parameters of the sex, whereas that in lds-RBCs was higher in males than that in females (p=0.035). There were no significant differences in MMA content when it comes to different frequencies of blood donors in either fresh plasma or lds-RBCs. Additionally, there was no significant difference or clear trend in the rate of elevated plasma MMA levels among different sexes, age groups, and blood donation frequency groups. MMA in the blood products from donors in China does not compromise the safety of blood transfusions for cancer patients. Nevertheless, there is a need to focus on MMA levels in Chinese and to develop race-specific and age-specific normal reference ranges.

Plasma vitamin B12, methylmalonic acid and heart rate variability in healthy young Indian adults.

Prevalence of vitamin B12 deficiency in the Indian population is not known, however; it is considered to be higher than in the Western population. Vitamin B12 deficiency is generally diagnosed by the plasma vitamin B12 level. Metabolites of vitamin B12 such as homocysteine (Hcy) and methylmalonic acid (MMA) are considered to be better markers to diagnose vitamin B12 deficiency at the tissue level. Autonomic neuropathy in vitamin B12 deficiency appears to precede other neurological signs. One of the recent techniques to evaluate autonomic neuropathy is heart rate variability (HRV). We evaluated 14 healthy young adults to explore the association of plasma vitamin B12, MMA, and Hcy levels with HRV. Resting lead II ECG was recorded and power spectral analyses were performed. Plasma MMA level was significantly and negatively correlated with the log-transformed low frequency (r = - 0.74, p = 0.002) and total power spectra (r = - 0.55, p = 0.03) of HRV in absolute units. Low frequency (LF) (r = - 0.56, p = 0.03) and high frequency (HF) (r = 0.57, p = 0.03), when represented in normalized units, were also correlated significantly with plasma MMA. In summary, plasma MMA but not vitamin B12 was significantly associated with HRV indices in a young adult population, suggesting that a tissue-level marker of vitamin B12 deficiency is more closely correlated with functional changes.

The methionine synthase reductase 66A&gt;G polymorphism is a maternal risk factor for spina bifida

The methionine synthase reductase (MTRR) enzyme restores methionine synthase (MTR) enzyme activity and therefore plays an essential role in homocysteine remethylation. In some studies, the 66A>G polymorphism in the MTRR gene was associated with increased neural tube defect (NTD) risk. Using a case-control design, we studied the association between the MTRR 66A>G polymorphism and spina bifida risk in 121 mothers, 109 spina bifida patients, 292 control women, and 234 pediatric controls. Possible interactions between the MTRR 66A>G variant and the MTR 2756A>G polymorphism, the MTHFR 677C>T variant, plasma vitamin B12, and plasma methylmalonic acid (MMA) levels were examined in the 121 mothers and 292 control women. Meta-analyses were conducted to set the results of the case-control study in the context of eligible literature on the relation between the MTRR 66A>G variant and NTD risk. Finally, a transmission disequilibrium test was performed for 82 complete mother-father-child triads to test for preferential transmission of the MTRR risk allele. In our case-control study, the MTRR 66A>G polymorphism had no influence on spina bifida risk in children [odds ratio (OR) 0.6, 95% confidence interval (CI) 0.4-1.1]. The MTRR 66GG genotype increased maternal spina bifida risk by 2.1-fold (OR 2.1, 95% CI 1.3-3.3). This risk became more pronounced in combination with the MTHFR 677TT genotype (OR 4.0, 95% CI 1.3-12.5). Moreover, we demonstrate a possible interaction between the MTRR 66GG genotype and high plasma MMA levels (OR 5.5, 95% CI 2.2-13.5). The meta-analyses demonstrated that the maternal MTRR 66GG genotype was associated with an overall 55% (95% CI 1.04-2.30) increase in NTD risk and that the MTRR 66GG genotype did not increase NTD risk in children (OR 0.96, 95% CI 0.46-2.01). These data show that the MTRR 66GG genotype is a maternal risk factor for spina bifida especially when intracellular vitamin B12 status is low.
