Fibronectin as a Marker of Myocardial Remodeling in Patients with Depression and Chronic Heart Failure

Introduction Depression is a significant issue in chronic heart failure (HF), with a prevalence of about 20–40%, which is 4–5% higher than in the general population (Mbakwem A., et al., 2016).Objectives The purpose of this study was to evaluate plasma fibronectin levels in patients with depression and chronic heart failure.Methods A total of 80 patients with HF II-III NYHA classes due to chronic coronary artery diseases (CAD) were observed. All patients were divided into two groups: Group 1 - 20 individuals without signs of depression, and Group 2 - 60 individuals with depression. The diagnosis of HF was confirmed based on ESC guidelines (2021). Depression was diagnosed using several questionnaires (Zung Self-Rating Depression Scale, Beck Depression Inventory, Hamilton’s Depression Scale). Standard laboratory and instrumental tests were conducted. The plasma levels of fibronectin and interleukin-1β (IL-1β) were identified using ELISA methods. Statistical analyses were performed using Statistica system software, version 12.0.ResultsThe average plasma fibronectin concentration in patients with depression and HF was 1.24 times higher than a similar indicator in HF patients without depression: (259.63±5.71) μg/ml versus (203.41±9.51) μg/ml (p<0.05). The conducted correlation analysis indicated a moderate positive correlation between the level of fibronectin and the number of neutrophils in peripheral blood (r=0.35; p<0.05), the level of fibronectin and the magnitude of endogenous intoxication according to the erythrocyte absorption ability test (r=0.44; p<0.01), the level of fibronectin and IL-1β concentration (r=0.39; p<0.05), and an inverse correlation with left ventricle ejection fraction (r=0.32; p<0.05).ConclusionsThus, the plasma fibronectin content in patients with depression and ischemic HF serves as a marker of the progression of myocardial remodeling processes and the intensity of the inflammatory process.Disclosure of InterestNone Declared

Elevated Plasma Fibronectin Concentrations in Patients with Ossification of the Posterior Longitudinal Ligament and Ossification of the Ligamentum Flavum

Fibronectin is a glycoprotein involved in a wide variety of cellular activities, including the development of bone tissues. To study the relevance of fibronectin to the development of ossification of the posterior longitudinal ligament (OPLL) and ossification of the ligamentum flavum (OLF), plasma fibronectin concentrations in 30 consecutive patients with OPLL or OLF and 20 age-and-sex-matched control subjects were measured with an immunonephelometric method. Plasma fibronectin concentrations were significantly elevated in the patients with OPLL or OLF, independently of associated endocrinologic abnormalities, when compared with the control subjects. Because fibronectin is one of the essential factors in endochondral ossification, it is probable that the higher fibronectin concentration in plasma acts as a promoter of the development of OPLL and OLF.

Plasma fibronectin concentrations in patients with HIV infection and visceral leishmaniasis.

Concentrations of circulating fibronectin were studied in plasma of 22 patients with acquired immunodeficiency syndrome and 24 healthy blood donors. There were significant differences between the plasma fibronectin values of the patient population with AIDS (mean 2 SD = 206 +/- 86 mg/l) and of the control group (mean +/- 2 SD = 297 +/- 92 mg/l). AIDS patients with visceral leishmaniasis showed a significant diminution of plasma fibronectin concentration (mean +/- 2 SD = 132 +/- 31 mg/l). A significant increase in plasma fibronectin concentrations was noted after 14 days of treatment with antimony (mean 2 SD = 239 +/- 67 mg/l). These results suggest that the measurement of fibronectin concentrations in patients with AIDS especially with concurrent infection, contributes to diagnosis. A significant decrease in plasma fibronectin concentrations was observed in patients with AIDS and visceral leishmaniasis.

Plasma fibronectin concentrations in patients with human immunodeficiency virus infection.

Plasma fibronectin (PFN) concentrations were assessed in 21 patients with AIDS, in seven with AIDS-related complex (ARC), in 17 asymptomatic seropositive patients, and in 36 age and sex matched healthy control subjects. A single radial immunodiffusion technique was used to determine PFN concentration. A significant decrease in PFN concentrations was observed in patients with ARC and AIDS (especially in those patients with Pneumocystis carinii pneumonitis). On the other hand, normal PFN concentrations were observed in asymptomatic seropositive patients. The determination of PFN concentration in patients with AIDS and ARC may contribute to the diagnosis of such patients.

Effects of different plasma substitutes on plasma fibronectin concentrations in patients undergoing abdominal surgery

Plasma fibronectin is important for host defences as an opsonin of the mononuclear phagocyte system (MPS). The effects of three types of plasma substitutes, crystalloid, gelatin and dextran solutions, on plasma fibronectin levels were studied in patients undergoing abdominal surgery in relation to serum total protein and serum C-reactive protein (CRP) levels. Plasma fibronectin levels decreased during induction of anaesthesia and infusion of an electrolyte solution before infusion of the plasma substitute, and they were further decreased during operation after infusion of plasma substitutes and remained depressed on the first postoperative day in all study groups (P less than 0.05-0.01). In the crystalloid and dextran groups, plasma fibronectin levels exceeded preoperative levels by the 4th-5th postoperative day (P less than 0.05-0.01), but in the gelatin solution group they remained depressed (P less than 0.01). Serum total protein levels remained lower than preoperative values throughout the study period (P less than 0.05-0.01), whereas CRP levels were many times higher on days 1 and 4-5 postoperatively in all groups (P less than 0.01).

Plasma Fibronectin Concentrations in Patients With Severe Burn Injury

Plasma fibronectin concentrations were measured in 33 patients with major burn injury. Changes in plasma fibronectin concentration were related to the day of burn injury, surgery and to the development of sepsis. Within 24 hours of the burn injury and surgical treatment, a decrease of the plasma fibronectin concentration was observed, which returned to normal values within 72 hours. A decrease in the plasma fibronectin concentration was observed as sepsis developed, while recovery from septicaemia was associated with plasma fibronectin concentration returning to normal values.

Plasma fibronectin concentration in patients with acquired consumptive coagulopathies

Plasma fibronectin was assayed in 179 hospitalized patients referred for workup of possible acquired coagulopathy. Based on laboratory results and chart review, these patients were classified as having no coagulopathy (N=36), defibrination syndrome (N=31), compensated defibrination syndrome (N=100), microangiopathic thrombocytopenia (N=7), and primary fibrinolysis (N=5). Compared to patients with no coagulopathy, fibronectin concentration was reduced in patients with defibrination syndrome (p < 0.005) and compensated defibrination syndrome (p < 0.10). Fibronectin concentration was not reduced in patients with microangiopathic thrombocytopenia and primary fibrinolysis. In patients with defibrination syndrome, the reduction of fibronectin was correlated to the degree of liver impairment. This finding is consistent with the liver being the primary site of synthesis of plasma fibronectin. Fibronectin was significantly correlated to plasminogen and antithrombin III. The sensitivity of fibronectin for the diagnosis of coagulopathy is low except for patients with defibrination syndrome.

Plasma, obtained just prior to diagnostic liver biopsy in 71 patients with various liver diseases, was examined by electroimmunoassay using immunoglobulin against human fibronectin and purified plasma fibronectin as standard. The plasma fibronectin concentration was not significantly different from age- and sex-matched healthy controls in patients with chronic persistent or chronic active hepatitis (n = 7), primary biliary cirrhosis (n = 8), alcoholic fatty liver (n = 9), alcoholic hepatitis (n = 10), and alcoholic cirrhosis (n = 16). Patients with acute viral hepatitis (type A (n = 2); type B (n = 7); type non A, non B (n = 1] had significantly (P less than 0.01) raised plasma fibronectin concentrations (median 506 mg/l (range 339-804] compared to controls (median 399 mg/l (range 304-462]. Morbidly obese patients with fatty liver (n = 11) had significantly (P less than 0.001) raised plasma fibronectin concentrations (median 610 mg/l (range 429-862] compared to controls (median 361 mg/l (range 303-419].

By quantitative electroimmunoassay the plasma fibronectin concentration was determined in 42 patients with chronic renal failure and treated with haemodialysis. The plasma fibronectin concentration was significantly reduced, independent of the haemodialysis, compared to a control group. Delayed wound healing seen in patients with chronic renal failure might be a result of the reduced plasma fibronectin.
