The effects of the predictability of acclimatory temperature on the growth and thermal tolerance of juvenile Spinibarbus sinensis

Heated effluent injection, cold hypolimnetic water inputs from dams, and extreme weather events can lead to unpredictable temperature fluctuations in natural waters, impacting fish performance and fitness. We hypothesized that fish exposed to such unpredictable fluctuations would exhibit weaker growth and enhanced thermal tolerance compared to predictable conditions. Qingbo (Spinibarbus sinensis) was selected as the experimental subject in this study. The qingbo were divided into a constant temperature group (C, 22 ± 0.5 °C), a predictable temperature fluctuation group (PF, 22 ± 4 °C, first warming, then cooling within a day) and an unpredictable temperature fluctuation group (UF, 22 ± 4 °C, the order of warming or cooling is random). After 40 days of temperature acclimation, the growth, metabolic rate, spontaneous activity, thermal tolerance, plasma cortisol concentration and liver hsp70 level of the fish were measured. Unexpectedly, neither the PF nor the UF group showed decreased growth compared to the C group. This could be attributed to the fact that temperature variation did not lead to a substantial increase in basic energy expenditure. Furthermore, feeding rates increased due to temperature fluctuations, although the difference was not significant. Both the PF and UF groups exhibited increased upper thermal tolerance, but only the UF group exhibited improved lower thermal tolerance and higher liver hsp70 levels compared to the C group. The qingbo that experienced unpredictable temperature fluctuations had the best thermal tolerance among the 3 groups, which might have occurred because they had the highest level of hsp70 expression. This may safeguard fish against the potential lethal consequences of extreme temperatures in the future. These findings suggested that qingbo exhibited excellent adaptability to both predictable and unpredictable temperature fluctuations, which may be associated with frequent temperature fluctuations in its natural habitat.

Effect of hospitalization on equine local intestinal immunoglobulin A (IgA) concentration measured in feces

During hospitalization horses may develop gastrointestinal conditions triggered by a stress-associated weak local immune system. The prospective, clinical trial was conducted to find out whether fecal immunoglobulin A (IgA) concentrations could be determined in hospitalized horses and how they changed during hospitalization and in response to various stressors. Samples were obtained from 110 horses and a control group (n = 14). At arrival in the hospital, horses were categorized into pain grades (1-5), and elective versus strenuous surgery (> 2 hours, traumatic and emergency procedures). Feces were collected on day 1, day 2, day 3, and day 7 in all horses. Blood samples were obtained at the same intervals, but additionally after general anaesthesia in horses undergoing surgery (day 2). IgA concentration in feces was determined by ELISA and measured in optical density at 450nm. The control group showed constant IgA concentrations on all days (mean value 0.30 OD450 ±SD 0.11, 1.26 mg/g; n = 11). After general anaesthesia fecal IgA concentrations decreased considerably independent of duration and type of surgery (P < 0.001 for elective and P = 0.043 for traumatic surgeries). High plasma cortisol concentrations were weakly correlated with low fecal IgA on the day after surgery (P = 0.012, day 3, correlation coefficient r = 0.113). Equine fecal IgA concentrations showed a decline associated with transport, surgery, and hospitalization in general, indicating that stress has an impact on the local intestinal immune function and may predispose horses for developing gastrointestinal diseases such as enterocolitis.

Effect of a combination of rhinoplasty and septoplasty on the development of acute postoperative syndrome and cortisol secretion into blood plasma

Purpose of the study: to evaluate the effect of a combination of rhinoplasty and septoplasty on the development of acute postoperative syndrome and the secretion of cortisol into the blood plasma.Material and methods. A study was conducted in a group of 98 patients who underwent septoplasty, rhinoplasty or rhinoseptoplasty. Using a Digital Rating Scale, the intensity of pain was assessed 3, 6, 24, 48 hours after the end of surgery. The day before, immediately before and 6 hours after surgical interventions, all patients underwent blood sampling to determine the concentration of cortisol in the blood plasma. The data obtained were compared within groups over time, as well as between groups at the corresponding assessment points.Results. The intensity of pain 3 hours after surgery was significantly lower in the group of patients after rhinoplasty and significantly decreased after 6 (p &lt; 0.001), 24 (p &lt; 0.01), 48 (p &lt; 0.001) hours after surgery, compared with patients who underwent septoplasty and rhinoseptoplasty. In the early postoperative period, patients from the rhinoplasty group had the lowest cortisol concentrations in the blood plasma, compared with patients after septoplasty (p &lt; 0.001) and rhinoseptoplasty (p &lt; 0.01). In patients who underwent rhinoseptoplasty, the level of cortisol in the blood was significantly higher compared with patients after septoplasty (p &lt; 0.01).Conclusion. The assessment of acute pain using a digital rating scale after rhinosurgical interventions showed that septoplasty provokes the development of a stronger acute pain syndrome in the early postoperative period, which proves its trauma compared to rhinoplasty. Simultaneous rhinoplasty and septoplasty in the first days after surgery contributes to an increase in the concentration of cortisol in blood plasma, as well as an increase in pain syndrome.

Behavioral and physiological responses of horses to ground-based adaptive horsemanship lessons for veterans with post-traumatic stress disorder (PTSD)

Little literature exists on horses in adaptive horsemanship (AH) despite concerns about their well-being. The study objective was to evaluate behavioral and physiological responses of horses to ground-based AH lessons for veterans with post-traumatic stress disorder (PTSD). Lessons were expected to alter horses’ hormone concentrations, behavior, and muscle activity. Geldings were assigned to AH (n=6; 20.3 ± 1.9 yrs., mean ± SE) or control (CON; stall in arena, n=6; 13.8 ± 1.7 yrs.) conditions for 8-week sessions based on current occupation (AH = equine-assisted services; CON = recreational riding). Plasma cortisol, epinephrine, norepinephrine, and oxytocin concentrations from samples at 0 (start of lesson), 3, 5, 25, and 30 (end) min were determined using assays validated in horses. Surface electromyography (sEMG) (masseter and brachiocephalic; Noraxon, Scottsdale, AZ, USA) and video were recorded continuously. Average rectified values (ARV) and median frequency (MF) were calculated (100 ms) after sEMG data were normalized, rectified, and filtered. The number, number of unique, and duration of stress related behaviors (ethogram) were recorded by three trained (ĸ ≥ 0.7) observers. Data were analyzed with repeated measures ANOVAs (significance P ≤ 0.05) with fixed effects of treatment, time point, week, and their interactions as appropriate and random effect of horse. CON horses had elevated cortisol concentrations (P = 0.0023) at 25 and 30 min. AH horses displayed fewer (P ≤ 0.0491) stress related and unique behaviors. CON horses were described as more (P < 0.0001) anxious, nervous, and stressed than AH horses (calm, comfortable, patient, and relaxed) in qualitative behavior analysis (22 observers). AH horses were less stressed than CON horses.

104 Maternal pre- and post-partum supplementation of Bacillus-based DFM enhanced cow and calf performance

Abstract This study evaluated the effects of maternal supplementation of Bacillus-based direct-fed microbials (DFM) on physiology and growth performance of Bos indicus-influenced cow-calf pairs. On d 0 (~139 d before expected calving date), 72 fall-calving, Brangus crossbred beef heifers (25 to 37.5% Bos indicus; 20 to 22 mo of age) pregnant with their first offspring were stratified by their initial body weight (BW; 431 ± 31 kg) and body condition score (BCS; 6.0 ± 0.36; scale 1 to 9), and randomly allocated into 1 of 12 bahiagrass pastures (1 ha and 6 heifers per pasture). Treatments were randomly assigned to pastures (6 pastures per treatment) and consisted of heifers supplemented with 1 kg/d of soybean hulls dry matter (DM) added (BAC) or not (CON) with a Bacillus-based DFM mixture (BovacillusTM; Chr. Hansen A/S, Hørsholm, Denmark) from d 0 to 242 (139 ± 4 d prepartum to 104 ± 4 d postpartum). Calves were weaned on d 242 (96 ± 30 d of age) and then allocated into 1 of 16 drylot pens and limit-fed the same concentrate at 3.25% of BW until d 319. Maternal treatment effects were not detected (P ≥ 0.29) for herbage allowance, forage nutritive value, and heifer activity on pasture. Heifer BCS on d 39 and 63 tended (P ≤ 0.09) to be greater, whereas heifer BCS on d 91 was greater (P = 0.01) for BAC vs. CON heifers and did not differ (P ≥ 0.20) between treatments on d 179 and 242. Plasma concentrations of glucose did not differ from d 0 to 63 (P ≥ 0.14) but were greater (P = 0.005) on d 179 and tended (P = 0.09) to be greater on d 242 for BAC vs. CON heifers. First offspring BW at birth, ADG from birth to early weaning, and BW weaning did not differ (P ≥ 0.19), but BW at drylot exit (d 319) was greater (P = 0.05) for BAC vs. CON calves. Maternal treatment effects were not detected (P ≥ 0.42) for calf serum concentrations of IgG at birth, and post-vaccination plasma concentrations of glucose, cortisol and haptoglobin. However, serum titers against bovine respiratory syncytial virus (BRSV) were greater (P = 0.04) for BAC vs. CON calves on d 287, whereas seroconversion against parainfluenza 3 virus (PI-3) was greater (P &amp;lt; 0.01) for BAC vs. CON calves on d 271. Thus, maternal supplementation of Bacillus-based DFM increased prepartum BCS gain and postpartum plasma concentrations of glucose of heifers and led to positive carryover effects on post-weaning BW gain and some indicators of humoral immune response to vaccination in their first offspring.

Preemptive Analgesia with Nonsteroidal Anti-Inflammatory Drugs in the Perioperative Period

Objective. A comparative assessment of the efficacy and safety of the preemptive use of ibuprofen and ketoprofen in patients undergoing elective surgery under general anesthesia.Material and methods. A multicenter randomized prospective study included 58 patients grouped into 2 arms. Ibuprofen 800 mg in Group 1 (N=32), and ketoprofen 100 mg in Group 2 (N=26) were administered intravenously 30 minutes prior to surgical procedure, and afterwards every 12 hours during patient’s stay in the intensive care unit. Efficacy and safety were assessed using a visual analog scale (VAS), patient’s need in opioid analgesics, laboratory parameters (serum levels of cortisol, cystatin C, CBC, coagulogram, TEG) and instrumental methods (algesimetry — qNOX).Results. VAS values were 32.4% lower in Group 1 vs Group 2 in the immediate postoperative period, P=0.003. By the end of Day 1 this difference was no longer visible following the use of promedol. There was a correlation between qNOX values at the end of surgery and VAS values at patient’s waking up from anesthesia (P=0.0007). Cortisol plasma concentrations in groups 1 and 2 did not differ significantly, P=0.105. The average daily promedol consumption in Groups 1 and 2 was 42±17.5 mg/day and 50±19.7 mg/day, respectively, P=0.022. Cystatin C concentrations in the first morning after surgery was 0.95±0.29 mg/l in the ibuprofen group, and 1.19±0.43 mg/l — in the ketoprofen group, P=0.027. Signs of renal dysfunction were documented in 4 out of 32 patients (12, 5%) from Group 1, and in 10 of 26 (38.5%) patients from Group 2 since the end of surgery and up to the first postop morning, the Chi-squared value was 0.031. Hemostasis was not affected by NSAIDs use in both groups.Conclusion. Ibuprofen provided more powerful analgesia, than ketoprofen in the postoperative period, while during surgical procedure both drugs showed similar anlgesic efficacy. Patients on ibuprofen required significantly fewer additional boluses of opioid analgesics. Both drugs showed no clinically significant effect on hemostasis and hematopoiesis. More rare occurrence of renal dysfunction in Group 1 patients is indicative of lower nephrotoxicity of ibuprofen.

Basal cortisol in relation to metyrapone confirmation in predicting adrenal insufficiency after pituitary surgery

PurposePituitary surgery can lead to post-surgical adrenal insufficiency with the need for glucocorticoid replacement and significant disease related burden. In patients who do not receive hydrocortisone replacement before surgery, at our center, an early morning plasma cortisol concentration using a cut-off value of 450 nmol/L 3 days after surgery (POD3) is used to guide the need for hydrocortisone replacement until dynamic confirmatory testing using metyrapone. The aim of this study was to critically assess the currently used diagnostic and treatment algorithm in patients undergoing pituitary surgery in our pituitary reference center.MethodsRetrospective analysis of all patients with a POD3 plasma cortisol concentration < 450 nmol/L who received hydrocortisone replacement and a metyrapone test after 3 months. Plasma cortisol concentration was measured using an electrochemiluminescence immunoassay (Roche). All patients who underwent postoperative testing using metyrapone at Amsterdam UMC between January 2018 and February 2022 were included. Patients with Cushing’s disease or those with hydrocortisone replacement prior to surgery were excluded.ResultsNinety-five patients were included in the analysis. The postoperative cortisol concentration above which no patient had adrenal insufficiency (i.e. 11-deoxycortisol > 200 nmol/L) was 357 nmol/L (Sensitivity 100%, Specificity 31%, PPV:32%, NPV:100%). This translates into a 28% reduction in the need for hydrocortisone replacement compared with the presently used cortisol cut-off value of 450 nmol/L.ConclusionEarly morning plasma cortisol cut-off values lower than 450 nmol/L can safely be used to guide the need for hydrocortisone replacement after pituitary surgery.

Plasma steroid concentrations reflect acute disease severity and normalise during recovery in people hospitalised with COVID-19.

Endocrine systems are disrupted in acute illness, and symptoms reported following coronavirus disease 2019 (COVID-19) are similar to those found with clinical hormone deficiencies. We hypothesised that people with severe acute COVID-19 and with post-COVID symptoms have glucocorticoid and sex hormone deficiencies. Samples were obtained for analysis from two UK multicentre cohorts during hospitalisation with COVID-19 (International Severe Acute Respiratory Infection Consortium/World Health Organisation [WHO] Clinical Characterization Protocol for Severe Emerging Infections in the UK study), and at follow-up 5 months after hospitalisation (Post-hospitalisation COVID-19 study). Plasma steroids were quantified by liquid chromatography-mass spectrometry. Steroid concentrations were compared against disease severity (WHO ordinal scale) and validated symptom scores. Data are presented as geometric mean (SD). In the acute cohort (n = 239, 66.5% male), plasma cortisol concentration increased with disease severity (cortisol 753.3 [1.6] vs. 429.2 [1.7] nmol/L in fatal vs. least severe, p < .001). In males, testosterone concentrations decreased with severity (testosterone 1.2 [2.2] vs. 6.9 [1.9] nmol/L in fatal vs. least severe, p < .001). In the follow-up cohort (n = 198, 62.1% male, 68.9% ongoing symptoms, 165 [121-192] days postdischarge), plasma cortisol concentrations (275.6 [1.5] nmol/L) did not differ with in-hospital severity, perception of recovery, or patient-reported symptoms. Male testosterone concentrations (12.6 [1.5] nmol/L) were not related to in-hospital severity, perception of recovery or symptom scores. Circulating glucocorticoids in patients hospitalised with COVID-19 reflect acute illness, with a marked rise in cortisol and fall in male testosterone. These findings are not observed 5 months from discharge. The lack of association between hormone concentrations and common post-COVID symptoms suggests steroid insufficiency does not play a causal role in this condition.

Variability in residual feed intake, nutrient utilisation and blood parametersin Murrah buffalo calves fed on total mixed ration

The present study was conducted on 18 female Murrah buffalo calves divided into two treatment groups to determine the residual feed intake, nutrient digestibility and blood metabolite parameters when fed with ration containing wheat (Triticum aestivum) straw, berseem (Trifolium alexandrinum) fodder as roughages and concentrate mixture to meet their requirements. Regular recording of body weight and feed intake was done during 90-day trial period. Residual feed intake (RFI) values were determined for individual animals based on which they were divided into low and high RFI groups. DM intake was lower in low RFI animals than high RFI animals. The blood glucose level and growth hormone level in low and high RFI group was comparable in both groups. Low RFI group had greater concentration of creatinine (1.62 vs. 1.21 mg/dL) as well as IGF-1 (136.70 vs. 120.38 ng/mL) whereas, concentration of total plasma protein level (6.80 vs 7.43 g/dL), albumin (3.46 vs 3.71 g/dl), aspartate aminotransferase (102.20 vs. 122.60 IU/L) and cortisol (3.48 vs. 5.32 ng/mL) were lesser in low RFI group with respect to high RFI group, indicating a positive correlation of RFI with total plasma protein, albumin, aspartate aminotransferase and cortisol and a negative correlation with plasma creatinine and IGF-1 levels in female Murrah buffalo calves. It was concluded that low RFI group of female Murrah buffalo calves showed better performance when fed with total mixed ration.

Plasma 6β-hydroxycortisol to cortisol ratio as a less invasive cytochrome P450 3A phenotyping method.

A less invasive evaluation method of cytochrome P450 3A (CYP3A) activity provides an important tool for personalized medicine. We aimed to clarify the usefulness of the plasma 6β-hydroxycortisol to cortisol concentration (6β-OHF/F) ratio as a minimally invasive CYP3A phenotyping method. Plasma 6β-OHF and cortisol concentrations were measured via liquid chromatography/tandem mass spectrometry. The plasma 6β-OHF/F ratio was compared with 6β-hydroxylation clearance of endogenous cortisol (CLm(6β); which we previously developed as an index of CYP3A activity) before, during and after oral contraceptive administration in 3 healthy women. The plasma 6β-OHF/F ratio was observed during oral clarithromycin administration. The plasma 6β-OHF/F ratio was also measured in 39 healthy participants. The plasma 6β-OHF/F ratio in 3 healthy women on Day 21 of starting oral contraceptive administration decreased by 39, 49 and 61% compared with Day 0. These values were similar to CLm(6β) values (43, 54 and 59%, respectively). Plasma 6β-OHF/F ratio and CLm(6β) exhibited a good correlation (r = .9053). The 6β-OHF/F ratio decreased from 0.00921 to 0.00577 only 3h following clarithromycin administration. The plasma 6β-OHF/F ratio ranged 0.00565-0.01556 in 39 healthy participants. Based on its close relationship with CLm(6β) and its decrease upon inhibition by clarithromycin, the plasma 6β-OHF/F ratio serves as an index of CYP3A activity. Using this minimally invasive index, we can identify patients with extremely low CYP3A activity before treatment initiation and optimize the initial drug dose, thereby mitigating the risk of severe adverse reactions.

Maternal contact and positive human interactions during lactation impacts piglet performance and behaviour during lactation

IntroductionEarly life interactions with the sow or humans can have lifelong consequences on welfare and performance in pigs. It was hypothesised that piglets that received increased maternal contact and positive human contact would display improved responses to stressful events and improved performance.MethodsNinety-eight litters were allocated to a 2 x 2 factorial design for maternal contact (MC+)/restricted maternal contact (MC-); and positive human contact (HC+)/no additional human contact (HC-). Modified farrowing crates were used to restrict maternal contact (MC-) and litters in the HC+ treatment received five minutes of daily positive human interaction (stroking). Behavioural and physiological responses were assessed during processing at three days of age and during a behaviour test in which piglets were introduced to an empty arena, novel object, human hand and human standing in the arena at 18 days of age. Observations of behaviour in the home pen and growth and survival of piglets to weaning were also assessed.ResultsAt processing, MC- piglets had more squeals (P = 0.015), tended to have more grunts (P = 0.063) and struggle attempts (P = 0.079), and had increased plasma cortisol concentrations (P = 0.009) after processing than MC+ piglets. During the behaviour test, HC+ piglets were more active (P = 0.014) and had more vocalisations (P &amp;lt; 0.05) in the empty arena than HC piglets. Furthermore, HC+ piglets were more likely to approach a human standing in the arena (P = 0.006) than HC- piglets. MC- piglets were less active (P = 0.008) and were less likely to approach the novel object (P = 0.049) than MC+ piglets. MC+ piglets tended to have higher weaning weights (P = 0.055) and more MC+ piglets were successfully weaned (P = 0.022) than MC- piglets. There was no significant effect of HC on piglet performance.DiscussionWhile positive handling did not affect behavioural and physiological stress response to processing, HC+ reduced fear of novelty and humans. These findings highlight the importance of early life experiences has on stress resilience early in life.

Calves were weaned on day 242 (96 ± 30 d of age) and then allocated into 1 of 16 drylot pens and fed the same concentrate at 3.25% of BW (DM) until day 319. Maternal treatment effects were not detected (P ≥ 0.29) for herbage allowance and forage chemical composition. Heifer BCS on days 39 and 63 tended (P ≤ 0.09) to be greater for BAC vs. CON heifers, whereas heifer BCS on day 91 was greater (P = 0.01) for BAC vs. CON heifers. Heifer BCS did not differ (P ≥ 0.20) between treatments on days 179 and 242. Administering the maternal appeasing substance before slaughter to improve carcass characteristics of finishing cattle.

Two experiments evaluated carcass characteristics of finishing steers administered the maternal bovine appeasing substance (mBAS) prior to slaughter. In Exp. 1, 954 Angus-influenced finishing steers housed in 6 original pens were used. Each original pen was split into a pair of experimental pens 14.3 d ± 3 d prior to slaughter, in a manner that number of steers and average pen body weight (BW; 636 ± 4kg) were similar. An oiler containing mBAS (Ferappease Finish Cattle 5%; FERA Diagnostics and Biologicals; College Station, TX) was added to one of the experimental pens 7 d prior to slaughter (n = 6), whereas the other pen did not contain an oiler (CON; n = 6). The oiler delivered 120mL of mBAS/steer during a 7-d period. Steer BW was recorded 7 d prior to and during loading (final BW) to the packing plant. No treatment effects were detected (P ≥ 0.51) for BW gain, final BW, and proportion of carcasses that graded Choice or Prime. Carcass dressing percentage was greater (P = 0.02) in mBAS compared with CON steers (65.9% vs. 64.2%; SEM = 0.5), which was not sufficient to impact hot carcass weight (HCW; P = 0.29). Incidence of dark-cutting carcasses did not differ between treatments (P = 0.23). In Exp. 2, 80 Angus-influenced finishing steers housed in 16 pens (5 steers/pen; 600 ± 4kg of BW) were used. Pens were arranged in 4 rows of 4 pens/row, and rows were alternately assigned to receive an oiler containing mBAS (n = 8) or mineral oil (CON+; n = 8) 7 d prior to slaughter. Oilers were designed to deliver 120mL/steer of mBAS or mineral oil during the 7-d period. Steer BW was recorded as in Exp. 1, and a blood sample was collected during exsanguination. No treatment effects were detected (P ≥ 0.20) for BW parameters, carcass marbling score, backfat thickness, Longissimus muscle area, yield grade, and proportion of carcasses that graded Choice or Prime. Carcass dressing was greater (P = 0.02) in mBAS steers compared with CON + (60.6 vs. 59.6%; SEM = 0.3) but HCW did not differ (P = 0.47) between treatments. Plasma cortisol concentration was less (P < 0.01) in mBAS steers compared with CON + (11.7 vs. 20.8ng/mL; SEM = 1.6). Incidence of dark-cutting carcasses did not differ (P = 0.53) between treatments. In summary, mBAS administration to finishing cattle using oilers during the last 7 d on feed alleviated the adrenocortical stress response elicited by the process of slaughter, which likely resulted in increased carcass dressing.

Transportation-induced nausea-like behavior in goats and the effects of anti-motion sickness medication.

This study investigated the nausea-like behavior induced by road transportation in goats, and the effects of an anti-motion sickness (MS) medication on this behavior. In the first experiment, 11 adult Shiba goats were road transported twice with either a saline (control) or a commercial anti-MS medication (Travelmin) injection at the first or second transportation. Almost all goats showed nausea-like behavior, which was defined as pointing their heads downward, closing their eyes, and staying relatively still. These goats did not respond when they were touched during blood collection. The anti-MS medication significantly reduced the total time spent in nausea-like behavior (P< 0.05) and tended to increase the frequency of escape attempts during blood collection (P< 0.1). In a second experiment, the effects of the anti-MS medication were examined in goats held under normal housing. The anti-MS medication increased the time spent feeding (P< 0.01) and reduced the time spent in rumination (P< 0.05) but did not change the frequency of lying down nor plasma cortisol concentrations. Our results indicate that the nausea-like behavior in transported goats might be induced, at least in part, by regulatory mechanisms similar to the MS.

Abstract. The basis of the treatment of bronchial asthma (BA) is anti-inflammatory therapy with the help of inhaled corticosteroids (ICS). Expectations of high efficacy (small particle size, high receptor affinity, high lipophilicity and lung residence time) and safety (low oral bioavailability, low systemic absorption, high protein binding and rapid systemic clearance) are placed on ICS. According to these characteristics, fluticasone propionate has the optimal therapeutic index. The aim: to study the possibilities of the use of fluticasone propionate in the cyclohaler device for the basic therapy of BA. Materials and methods. The study enrolled 376 patients who received fluticasone propionate 125 μg and 250 μg twice daily in a cyclohaler delivery device compared with the original fluticasone propionate 500 μg in patients with moderate asthma. All patients underwent spirometry, peak flowmetry, laboratory studies, registration of asthma symptoms, need in the rescue therapy, vital signs, adverse events. Results. The clinical and functional efficacy of fluticasone 125 μg twice daily is no inferior than the efficacy of the original fluticasone 500 μg twice daily. There is no clinically significant difference between the effectiveness of the drug in a dose of 125 and 250 μg when taken twice daily with the use of the new generation cyclohaler, which confirms the almost parallel linear dose-effect relationship in the range of medium and high doses of the drug. The incidence of the adverse effects was significantly lower in the group of patients treated with fluticasone 250 μg compared to patients in the other two groups. There were no differences between the groups in laboratory results. There were no differences in cortisol concentrations in blood plasma and 24-hour urine before and after 12 weeks of treatment in the groups using cyclohaler and the reference drug. Conclusion. Fluticasone propionate administered through a new generation cyclohaler at doses of 125 and 250 μg twice daily is an effective therapy for asthma of moderate severity, and the dose of 250 μg with the use of new generation cyclohaler is clinically equivalent to a twofold higher dose of the reference fluticasone propionate. The safety profile of fluticasone when using of the new generation cyclohaler can be compared with the safety profile of the reference substance.

Biomarkers in Endurance Exercise: Individualized Regulation and Predictive Value.

The high interindividual variability of exercise response complicates the efficient use of blood-based biomarkers in sports. To address this problem, a useful algorithm to characterize the individual regulation and predictive value of different candidate markers will be developed. Forty-nine participants completed two identical exercise trials. Blood samples were collected before, immediately after, 3 hours after, and 24 hours after completion of exercise. Plasma concentrations of interleukin (IL-) 1RA, IL-6, IL-8, IL-10, IL-15, creatine kinase (CK), cortisol, c-reactive protein (CRP), lactate dehydrogenase (LDH), and thiobarbituric acid reactive substances (TBARS) were measured. Individualized regulation was analyzed using k-means clustering and a Group Assignment Quality (GAQ) score. Regression trees with a bootstrapped-aggregated approach were used to assess the predictive qualities of the markers. For most of the markers studied, a distinction can be made between individuals who show a stronger or weaker response to a particular endurance training program. The regulation of IL-6, IL-8, IL-10, and CK exhibited a high degree of stability within the individuals. Regarding the predictive power of the markers, for all dependent variables, the most accurate predictions were obtained for cortisol and IL-8 based on the baseline value. For CK, a good prediction of recovery of maximal strength and subjective feeling of exhaustion can be made. For IL-1RA and TBARS, especially their reregulation can be predicted if the baseline level is known. Focusing individual variations in biomarker responses, our results suggest the combined use of IL-6, IL-8, IL-10, and CK for the personalized management of stress and recovery cycles following endurance exercise.

The value of plasma metanephrine measurements during adrenal vein sampling.

The assessment of primary aldosteronism incorporates adrenal vein sampling (AVS) to lateralize aldosterone excess. Current adrenal vein sampling protocols rely on concurrent cortisol measurements to assess successful cannulation and lateralization and may be inaccurate in the setting of autonomous cortisol secretion. We aimed to compare the measurement of plasma cortisol and metanephrine concentrations to assess cannulation and lateralization during AVS. This is a diagnostic accuracy study in a tertiary referral endocrinology department. Forty-one consecutive patients with confirmed primary aldosteronism undergoing AVS (49 procedures) were included. None had cortisol autonomy. The use of plasma metanephrine-based ratios were compared with standard cortisol-based ratios to assess cannulation and lateralization during ACTH-stimulated AVS. There was strong agreement between a cortisol selectivity index (SI) ≥5.0 and an adrenal vein (AV) to peripheral vein (PV) plasma metanephrine ratio (AVmet-PVmet) of ≥12.0 to indicate successful cannulation of the AV (n = 117, sensitivity 98%, specificity 89%, positive predictive value (PPV) 95%, negative predictive value (NPV) 94%). There was strong agreement between the standard cortisol-based SI and an AV plasma metanephrine-to-normetanephrine ratio (AVmet-AVnormet) of ≥2.0 to indicate successful cannulation (n = 117, sensitivity 93%, specificity 86%, PPV 94%, NPV 84%). There was strong agreement between the cortisol- or metanephrine-derived lateralization index (LI) > 4.0 for determining lateralization (n = 26, sensitivity 100%, specificity 94.1%, PPV 91.6%, NPV 100%). Ratios incorporating plasma metanephrines provide comparable outcomes to standard cortisol-based measurements for interpretation of AVS. Further studies are required to assess the use of metanephrine-derived ratios in the context of confirmed cortisol autonomy. Primary aldosteronism is a common cause of secondary hypertension, and adrenal vein sampling remains the gold standard test to assess lateralization. Cortisol-derived ratios to assess cannulation and lateralization may be affected by concurrent cortisol dysfunction, which is not uncommon in the context of primary aldosteronism. Our study showed comparable outcomes when using accepted cortisol-derived or metanephrine-derived ratios to determine cannulation and lateralization during adrenal vein sampling. Further research is required to validate these findings and to assess the use of metanephrine-derived ratios in the context of confirmed concurrent cortisol dysfunction.

Effects of rearing density and in-tank structure on the stress response in juvenile Chinook salmon (Oncorhynchus tshawytscha)

Current salmonid hatchery practices typically include high rearing densities without structure within the water column. In the present study, we assessed the effects of low (conservation hatchery) and high (production hatchery) rearing densities on the plasma cortisol response during simulated transport stress and whether density might interact with in-tank structure to further mitigate the stress response. After 10 months of rearing in their respective treatments, we subjected fish to a transportation simulation stressor that encompassed fish being caught, bucketed, and crowded at a lowered water depth in a smaller tank for 3 h, then caught and bucketed to a novel tank of similar size and flow. We sampled stressed and unstressed fish for plasma cortisol determination over a 23 h time period. Additionally, we determined resting cortisol secretion from head kidneys collected from unstressed fish at time 0 h from each rearing treatment to determine if resting cortisol secretion pattern was affected by the rearing conditions. There was no significant difference in size of the fish at the time of sampling; overall, they averaged 24.7 g in weight and 136.2 mm in fork length. Fish reared with in-tank structure at low density had the lowest plasma cortisol concentration during the stressor, while fish reared without in-tank structure at high density had the highest plasma cortisol concentration in response to the stressor. In-tank structure, high-density reared fish had less plasma cortisol than no-structure high-density fish, but was elevated over low-density in-tank structure fish. Rearing condition had no meaningful effect on resting cortisol secretion by the interrenals. We speculate that low density with in-tank structure rearing would ultimately lead to the highest survival rates when hatchery Chinook salmon are released into the wild due to their reduced plasma cortisol concentration in response to a stressor.

Evaluation of plasma cortisol during fasting test in patients with endogenous hyperinsulinemic hypoglycemia. Fifteen years experience

BackgroundEndogenous hyperinsulinemic hypoglycemia (EHH) is a rare clinical condition. The aim of this study was to evaluate baseline plasma cortisol concentration and its concentration during hypoglycemic crisis in fasting tests (FT) performed in our center. Secondarily, the aim was to establish the relationship between baseline cortisol and the time of evolution of EHH. Material and methodsA retrospective, observational, descriptive study was carried out which included patients with hypoglycemic disorder with positive FT. ResultsOf a total of 21 patients, 16 presented insulinoma, 1 nesidioblastosis, 2 malignant insulinoma and 2 EHH without pathological diagnosis. The time from the onset of symptoms to diagnosis was 2 years (Q1=1.5–Q2=5.5). The comparison between median baseline cortisol (BC)=11.8 mcg/dl (nmol/L 340.68) (Q1=9–Q3=14.1) and median cortisol during hypoglycemic episode (HC)=11.6 mcg/dl (nmol/L: 303.44) (Q1=7.8–Q3=16.1) showed no differences (Z=−0.08; P>.05). When correlating BC with HC, no significant relationship was observed (r=0.16; P>.05). When correlating the glycemic value in the crisis and the HC, a slight negative trend was found (r=−0.53; P=.01). In addition, we found that recurrent hypoglycemic disorder is associated with lower baseline cortisol values ​​the longer the time of its evolution. ConclusionWe confirmed that cortisol values ​​remain low during hypoglycemic episodes, reinforcing the hypothesis of lack of response of this counterregulatory hormone in cases of recurrent hypoglycemia.
