Abdominal image quality and dose reduction with energy-integrating or photon-counting detectors dual-source CT: A phantom study

PurposeThe purpose of this study was to assess image-quality and dose reduction potential using a photon-counting computed tomography (PCCT) system by comparison with two different dual-source CT (DSCT) systems using two phantoms. Materials and methodsAcquisitions on phantoms were performed using two DSCT systems (DSCT1 [Somatom Force] and DSCT2 [Somatom Pro.Pulse]) and one PCCT system (Naeotom Alpha) at four dose levels (13/6/3.4/1.8 mGy). Noise power spectrum (NPS) and task-based transfer function (TTF) were computed to assess noise magnitude and noise texture and spatial resolution (f50), respectively. Detectability indexes (d’) were computed to model the detection of abdominal lesions: one unenhanced high-contrast task, one contrast-enhanced high-contrast task and one unenhanced low-contrast task. Image quality was subjectively assessed on an anthropomorphic phantom by two radiologists. ResultsFor all dose levels, noise magnitude values were lower with PCCT than with DSCTs. For all CT systems, similar noise texture values were found at 13 and 6 mGy, but the greatest noise texture values were found for DSCT2 and the lowest for PCCT at 3.4 and 1.8 mGy. For high-contrast inserts, similar or lower f50 values were found with PCCT than with DSCT1 and the opposite pattern was found for the low-contrast insert. For the three simulated lesions, d’ values were greater with PCCT than with DSCTs. Abdominal images were rated satisfactory for clinical use by the radiologists for all dose levels with PCCT and for 13 and 6 mGy with DSCTs. ConclusionBy comparison with DSCTs, PCCT reduces image-noise and improves detectability of simulated abdominal lesions without altering the spatial resolution and image texture. Image-quality obtained with PCCT seem to indicate greater potential for dose optimization than those obtained with DSCTs.

Technology Characterization Through Diverse Evaluation Methodologies: Application to Thoracic Imaging in Photon-Counting Computed Tomography

Objective Different methods can be used to condition imaging systems for clinical use. The purpose of this study was to assess how these methods complement one another in evaluating a system for clinical integration of an emerging technology, photon-counting computed tomography (PCCT), for thoracic imaging. Methods Four methods were used to assess a clinical PCCT system (NAEOTOM Alpha; Siemens Healthineers, Forchheim, Germany) across 3 reconstruction kernels (Br40f, Br48f, and Br56f). First, a phantom evaluation was performed using a computed tomography quality control phantom to characterize noise magnitude, spatial resolution, and detectability. Second, clinical images acquired using conventional and PCCT systems were used for a multi-institutional reader study where readers from 2 institutions were asked to rank their preference of images. Third, the clinical images were assessed in terms of in vivo image quality characterization of global noise index and detectability. Fourth, a virtual imaging trial was conducted using a validated simulation platform (DukeSim) that models PCCT and a virtual patient model (XCAT) with embedded lung lesions imaged under differing conditions of respiratory phase and positional displacement. Using known ground truth of the patient model, images were evaluated for quantitative biomarkers of lung intensity histograms and lesion morphology metrics. Results For the physical phantom study, the Br56f kernel was shown to have the highest resolution despite having the highest noise and lowest detectability. Readers across both institutions preferred the Br56f kernel (71% first rank) with a high interclass correlation (0.990). In vivo assessments found superior detectability for PCCT compared with conventional computed tomography but higher noise and reduced detectability with increased kernel sharpness. For the virtual imaging trial, Br40f was shown to have the best performance for histogram measures, whereas Br56f was shown to have the most precise and accurate morphology metrics. Conclusion The 4 evaluation methods each have their strengths and limitations and bring complementary insight to the evaluation of PCCT. Although no method offers a complete answer, concordant findings between methods offer affirmatory confidence in a decision, whereas discordant ones offer insight for added perspective. Aggregating our findings, we concluded the Br56f kernel best for high-resolution tasks and Br40f for contrast-dependent tasks.

Universal 120-kV Dual-Source Ultra-High Pitch Protocol on the Photon-Counting CT System for Pediatric Abdomen of All Sizes: A Phantom Investigation Comparing With Energy-Integrating CT.

The purpose of this study is to determine if a universal 120-kV ultra-high pitch and virtual monoenergetic images (VMIs) protocol on the photon-counting computed tomography (PCCT) system can provide sufficient image quality for pediatric abdominal imaging, regardless of size, compared with protocols using a size-dependent kV and dual-source flash mode on the energy-integrating CT (EICT) system. One solid water insert and 3 iodine (2, 5, 10 mg I/mL) inserts were attached or inserted into phantoms of variable sizes, simulating the abdomens of a newborn, 5-year-old, 10-year-old, and adult-sized pediatric patients. Each phantom setting was scanned on an EICT using clinical size-specific kV dual-source protocols with a pitch of 3.0. The scans were performed with fixed scanning parameters, and the CTDIvol values of full dose were 0.30, 0.71, 1.05, and 7.40 mGy for newborn to adult size, respectively. In addition, half dose scans were acquired on EICT. Each phantom was then scanned on a PCCT (Siemens Alpha) using a universal 120-kV protocol with the same full dose and half dose as determined above on the EICT scanner. All other parameters matched to EICT settings. Virtual monoenergetic images were generated from PCCT scans between 40 and 80 keV with a 5-keV interval. Image quality metrics were compared between PCCT VMIs and EICT, including image noise (measured as standard deviation of solid water), contrast-to-noise ratio (CNR) (measured at iodine inserts with solid water as background), and noise power spectrum (measured in uniform phantom regions). Noise at a PCCT VMI of 70 keV (7.0 ± 0.6 HU for newborn, 14.7 ± 1.6 HU for adult) is comparable (P > 0.05, t test) or significantly lower (P < 0.05, t test) compared with EICT (7.8 ± 0.8 HU for newborn, 15.3 ± 1.5 HU for adult). Iodine CNR from PCCT VMI at 50 keV (50.8 ± 8.4 for newborn, 27.3 ± 2.8 for adult) is comparable (P > 0.05, t test) or significantly higher (P < 0.05, t test) to the corresponding EICT measurements (57.5 ± 6.7 for newborn, 13.8 ± 1.7 for adult). The noise power spectrum curve shape of PCCT VMI is similar to EICT, despite PCCT VMI exhibiting higher noise at low keV levels. The universal PCCT 120 kV with ultra-high pitch and postprocessed VMIs demonstrated equivalent or improved performance in noise (70 keV) and iodine CNR (50 keV) for pediatric abdominal CT, compared with size-specific kV images on the EICT.

Quantitative accuracy of lung function measurement using parametric response mapping: A virtual imaging study.

Parametric response mapping (PRM) is a voxel-based quantitative CT imaging biomarker that measures the severity of chronic obstructive pulmonary disease (COPD) by analyzing both inspiratory and expiratory CT scans. Although PRM-derived measurements have been shown to predict disease severity and phenotyping, their quantitative accuracy is impacted by the variability of scanner settings and patient conditions. The aim of this study was to evaluate the variability of PRM-based measurements due to the changes in the scanner types and configurations. We developed 10 human chest models with emphysema and air-trapping at end-inspiration and end-expiration states. These models were virtually imaged using a scanner-specific CT simulator (DukeSim) to create CT images at different acquisition settings for energy-integrating and photon-counting CT systems. The CT images were used to estimate PRM maps. The quantified measurements were compared with ground truth values to evaluate the deviations in the measurements. Results showed that PRM measurements varied with scanner type and configurations. The emphysema volume was overestimated by 3 ± 9.5 % (mean ± standard deviation) of the lung volume, and the functional small airway disease (fSAD) volume was underestimated by 7.5±19 % of the lung volume. PRM measurements were more accurate and precise when the acquired settings were photon-counting CT, higher dose, smoother kernel, and larger pixel size. This study demonstrates the development and utility of virtual imaging tools for systematic assessment of a quantitative biomarker accuracy.

Qualitative and quantitative image quality of coronary CT angiography using photon-counting computed tomography: Standard and Ultra-high resolution protocols

PurposeWe aimed to identify the optimal reconstruction settings based on qualitative and quantitative image quality parameters on standard and ultra-high resolution (UHR) images using photon-counting CT (PCCT). MethodWe analysed 45 patients, 29 with standard and 16 with UHR acquisition, applying both smoother and sharper kernel settings. Coronary CT angiography images were performed on a dual-source PCCT system using standard (0.4/0.6 mm slice thickness, Bv40/Bv44 kernels, QIR levels 0–4) or UHR acquisition (0.2/0.4 mm slice thickness, Bv44/Bv56 kernels, QIR levels 0–4). Qualitative image quality was assessed using a 4-point Likert scale. Image noise (SD), signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were calculated in both the proximal and distal segments. ResultsOn standard resolution, larger slice thickness resulted in an average increase of 12.5 % in CNR, whereas sharper kernel led to an average 8.7 % decrease in CNR. Highest CNR was measured on 0.6 mm, Bv40, QIR4 images and lowest on 0.4 mm, Bv44, QIR0 images: 25.8 ± 4.1vs.8.3 ± 1.6 (p < 0.001). On UHR images, highest CNR was observed on 0.4 mm, Bv40, QIR4 and lowest on 0.2 mm, Bv56 and QIR0 images: 21.5 ± 3.9vs.3.6 ± 0.8 (p < 0.001). Highest qualitative image quality was found on images with Bv44 kernel and QIR level 3/4 with both slice thicknesses on standard reconstruction. Additionally, Bv56 with QIR4 on 0.2 mm slice thickness images showed highest subjective image quality. Preserved distal vessel visualization was detected using QIR 2–4, Bv56 and 0.2 mm slice thickness. ConclusionsPhoton-counting CT demonstrated high qualitative and quantitative image quality for the assessment of coronaries and stents.

Feasibility of improving image performance in photon-counting computed tomography using X-ray spectrum filtration

Photon-counting computed tomography (PCCT) is an emerging technology based on new energy-resolving X-ray detectors (photon-counting detectors (PCDs)) that provide promising image performance compared to the conventional energy-integrating CT (EICT). It has the potential to provide higher resolution and contrast, lower radiation dose, and fewer artifacts, which has led to significant research interest. This study proposes a method to further improve the image performance of the PCCT using an X-ray spectrum filtration technique. We conducted a feasibility study via simulations using aluminum (Al), beryllium (Be), sodium (Na), nickel (Ni), tin (Sn), neodymium (Nd), and tantalum (Ta) incorporating varying thicknesses using a PCD simulation toolkit (PcTK). The PCCT system used in this simulation was modeled to have a cadmium telluride-based PCD with four multi-energy thresholds of E = 40, 60, 80, 100 keV, assuming that the object received the same number of photons. Using the PCCT images obtained with the highest threshold (i.e., E4 = 100 keV), the image quality was evaluated quantitatively in terms of the signal difference-to-noise ratio (SDNR) and structural similarity (SSIM). Among the filtrations selected in this simulation, a filtration of 2.0 mmAl (inherent) and 1.5 mmSn (added) showed the best image quality. The SDNR and SSIM values measured in the PCCT image obtained with an added filtration of 1.5 mmSn were 3.73 and 0.81, approximately 1.9 and 1.8 times higher than those with no added filtration, respectively. Consequently, the X-ray spectrum filtration technique in PCCT is useful to further improve image performance.

Photon-counting CT in Thoracic Imaging: Early Clinical Evidence and Incorporation Into Clinical Practice.

Photon-counting CT (PCCT) is an emerging advanced CT technology that differs from conventional CT in its ability to directly convert incident x-ray photon energies into electrical signals. The detector design also permits substantial improvements in spatial resolution and radiation dose efficiency and allows for concurrent high-pitch and high-temporal-resolution multienergy imaging. This review summarizes (a) key differences in PCCT image acquisition and image reconstruction compared with conventional CT; (b) early evidence for the clinical benefit of PCCT for high-spatial-resolution diagnostic tasks in thoracic imaging, such as assessment of airway and parenchymal diseases, as well as benefits of high-pitch and multienergy scanning; (c) anticipated radiation dose reduction, depending on the diagnostic task, and increased utility for routine low-dose thoracic CT imaging; (d) adaptations for thoracic imaging in children; (e) potential for further quantitation of thoracic diseases; and (f) limitations and trade-offs. Moreover, important points for conducting and interpreting clinical studies examining the benefit of PCCT relative to conventional CT and integration of PCCT systems into multivendor, multispecialty radiology practices are discussed.

A systematic assessment and optimization of photon-counting CT for lung density quantifications.

Photon-counting computed tomography (PCCT) has recently emerged into clinical use; however, its optimum imaging protocols and added benefits remains unknown in terms of providing more accurate lung density quantification compared to energy-integrating computed tomography (EICT) scanners. To systematically assess the performance of a clinical PCCT scanner for lung density quantifications and compare it against EICT. This cross-sectional study involved a retrospective analysis of subjects scanned (August-December 2021) using a clinical PCCT system. The influence of altering reconstruction parameters was studied (reconstruction kernel, pixel size, slice thickness). A virtual CT dataset of anthropomorphic virtual subjects was acquired to demonstrate the correspondence of findings to clinical dataset, and to perform systematic imaging experiments, not possible using human subjects. The virtual subjects were imaged using a validated, scanner-specific CT simulator of a PCCT and two EICT (defined as EICT A and B) scanners. The images were evaluated using mean absolute error (MAE) of lung and emphysema density against their corresponding ground truth. Clinical and virtual PCCT datasets showed similar trends, with sharper kernels and smaller voxel sizes increasing percentage of low-attenuation areas below -950 HU (LAA-950) by up to 15.7±6.9% and 11.8±5.5%, respectively. Under the conditions studied, higher doses, thinner slices, smaller pixel sizes, iterative reconstructions, and quantitative kernels with medium sharpness resulted in lower lung MAE values. While using these settings for PCCT, changes in the dose level (13 to 1.3 mGy), slice thickness (0.4 to 1.5mm), pixel size (0.49 to 0.98mm), reconstruction technique (70keV-VMI to wFBP), and kernel (Qr48 to Qr60) increased lung MAE by 15.3±2.0, 1.4±0.6, 2.2±0.3, 4.2±0.8, and 9.1±1.6 HU, respectively. At the optimum settings identified per scanner, PCCT images exhibited lower lung and emphysema MAE than those of EICT scanners (by 2.6±1.0 and 9.6±3.4 HU, compared to EICT A, and by 4.8±0.8 and 7.4±2.3 HU, compared to EICT B). The accuracy of lung density measurements was correlated with subjects' mean lung density (p<0.05), measured by PCCT at optimum setting under the conditions studied. Photon-counting CT demonstrated superior performance in density quantifications, with its influences of imaging parameters in line with energy-integrating CT scanners. The technology offers improvement in lung quantifications, thus demonstrating potential toward more objective assessment of respiratory conditions.

Thin slice photon-counting CT coronary angiography compared to conventional CT: Objective image quality and clinical radiation dose assessment.

Photon-counting CT (PCCT) is the next-generation CT scanner that enables improved spatial resolution and spectral imaging. For full spectral processing, higher tube voltages compared to conventional CT are necessary to achieve the required spectral separation. This generated interest in the potential influence of thin slice high tube voltage PCCT on overall image quality and consequently on radiation dose. This study first evaluated tube voltages and radiation doses applied in patients who underwent coronary CT angiography with PCCT and energy-integrating detector CT (EID-CT). Next, image quality of PCCT and EID-CT was objectively evaluated in a phantom study simulating different patient sizes at these tube voltages and radiation doses. We conducted a retrospective analysis of clinical doses of patients scanned on a conventional and PCCT system. Average patient water equivalent diameters for different tube voltages were extracted from the dose reports for both EID-CT and PCCT. A conical phantom made of polyethylene with multiple diameters (26/31/36cm) representing different patient sizes and containing an iodine insert was scanned with a EID-CT scanner using tube voltages and phantom diameters that match the patient scans and characteristics. Next, phantom scans were made with PCCT at a fixed tube voltage of 120kV and with CTDIVOL values and phantom diameters identical to the EID-CT scans. Clinical image reconstructions at 0.6mm slice thickness for conventional CT were compared to PCCT images with 0.4mm slice thickness. Image quality was quantified using the detectability index (d'), which estimated the visibility of a 3mm diameter contrast-enhanced coronary artery by considering noise, contrast, resolution, and human visual perception. Alongside d', noise, contrast and resolution were also individually assessed. In addition, the influence of various kernels (Bv40/Bv44/Bv48/Bv56), quantum iterative reconstruction strengths (QIR, 3/4) and monoenergetic levels (40/45/50/55keV) for PCCT on d' was investigated. In this study, 143 patients were included: 47 were scanned on PCCT (120kV) and the remaining on EID-CT (74 small-sized at 70kV, 18 medium-sized at 80kV and four large-sized at 90kV). EID-CT showed 7%-17% higher d' than PCCT with Bv40 kernel and strength four for small/medium patients. Lower monoenergetic images (40keV) helped mitigate the difference to 1%-6%. For large patients, PCCT's detectability was up to 31% higher than EID-CT. PCCT has thinner slices but similar noise levels for similar reconstruction parameters. The noise increased with lower keV levels in PCCT (≈30% increase), but higher QIR strengths reduced noise. PCCT's iodine contrast was stable across patient sizes, while EID-CT had 33% less contrast in large patients than in small-sized patients. At 120kV, thin slice PCCT enables CCTA in phantom scans representing large patients without raising radiation dose or affecting vessel detectability. However, higher doses are needed for small and medium-sized patients to obtain a similar image quality as in EID-CT. The alternative of using lower mono-energetic levels requires further evaluation in clinical practice.

A New Iterative Metal Artifact Reduction Algorithm for Both Energy-Integrating and Photon-Counting CT Systems.

The aim of this study was to introduce and evaluate a new metal artifact reduction framework (iMARv2) that addresses the drawbacks (residual artifacts after correction and user preferences for image quality) associated with the current clinically applied iMAR. A new iMARv2 has been introduced, combining the current iMAR with new modular components to remove residual metal artifacts after image correction. The postcorrection image impression is adjustable with user-selectable strength settings. Phantom scans from an energy-integrating and a photon-counting detector CT were used to assess image quality, including a Gammex phantom and anthropomorphic phantoms. In addition, 36 clinical cases (with metallic implants such as dental fillings, hip replacements, and spinal screws) were reconstructed and evaluated in a blinded and randomized reader study. The Gammex phantom showed lower HU errors compared with the uncorrected image at almost all iMAR and iMARv2 settings evaluated, with only minor differences between iMAR and the different iMARv2 settings. In addition, the anthropomorphic phantoms showed a trend toward lower errors with higher iMARv2 strength settings. On average, the iMARv2 strength 3 performed best of all the clinical reconstructions evaluated, with a significant increase in diagnostic confidence and decrease in artifacts. All hip and dental cases showed a significant increase in diagnostic confidence and decrease in artifact strength, and the improvements from iMARv2 in the dental cases were significant compared with iMAR. There were no significant improvements in the spine. This work has introduced and evaluated a new method for metal artifact reduction and demonstrated its utility in routine clinical datasets. The greatest improvements were seen in dental fillings, where iMARv2 significantly improved image quality compared with conventional iMAR.

Intra-individual comparison of coronary CT angiography-based FFR between energy-integrating and photon-counting detector CT systems

BackgroundCoronary computed tomography angiography (CCTA)-based fractional flow reserve (CT-FFR) allows for noninvasive determination of the functional severity of anatomic lesions in patients with coronary artery disease. The aim of this study was to intra-individually compare CT-FFR between photon-counting detector (PCD) and conventional energy-integrating detector (EID) CT systems. MethodsIn this single-center prospective study, subjects who underwent clinically indicated CCTA on an EID-CT system were recruited for a research CCTA on PCD-CT within 30 days. Image reconstruction settings were matched as closely as possible between EID-CT (Bv36 kernel, iterative reconstruction strength level 3, slice thickness 0.5 mm) and PCD-CT (Bv36 kernel, quantum iterative reconstruction level 3, virtual monoenergetic level 55 keV, slice thickness 0.6 mm). CT-FFR was measured semi-automatically using a prototype on-site machine learning algorithm by two readers. CT-FFR analysis was performed per-patient and per-vessel, and a CT-FFR ≤ 0.75 was considered hemodynamically significant. ResultsA total of 22 patients (63.3 ± 9.2 years; 7 women) were included. Median time between EID-CT and PCD-CT was 5.5 days. Comparison of CT-FFR values showed no significant difference and strong agreement between EID-CT and PCD-CT in the per-vessel analysis (0.88 [0.74–0.94] vs. 0.87 [0.76–0.93], P = 0.096, mean bias 0.02, limits of agreement [LoA] −0.14/0.19, r = 0.83, ICC = 0.92), and in the per-patient analysis (0.81 [0.60–0.86] vs. 0.76 [0.64–0.86], P = 0.768, mean bias 0.02, LoA −0.15/0.19, r = 0.90, ICC = 0.93). All included patients were classified into the same category (CT-FFR > 0.75 vs ≤0.75) with both CT systems. ConclusionsCT-FFR evaluation is feasible with PCD-CT and it shows a strong agreement with EID-CT-based evaluation when images are similarly reconstructed.

Potential radiation dose reduction in clinical photon-counting CT by the small pixel effect: ultra-high resolution (UHR) acquisitions reconstructed to standard resolution.

To assess the potential dose reduction achievable with clinical photon-counting CT (PCCT) in ultra-high resolution (UHR) mode compared to acquisitions using the standard resolution detector mode (Std). With smaller detector pixels, PCCT achieves far higher spatial resolution than energy-integrating (EI) CT systems. The reconstruction of UHR acquisitions to the lower spatial resolution of conventional systems results in an image noise and radiation dose reduction. We quantify this small pixel effect in measurements of semi-anthropomorphic abdominal phantoms of different sizes as well as in a porcine knuckle in the first clinical PCCT system by using the UHR mode (0.2mm pixel size at isocenter) in comparison to the standard resolution mode (0.4mm). At different slice thicknesses (0.4 up to 4mm) and dose levels between 4 and 12mGy, reconstructions using filtered backprojection were performed to the same target spatial resolution, i.e., same modulation transfer function, using both detector modes. Image noise and the resulting potential dose reduction was quantified as a figure of merit. Images acquired using the UHR mode yield lower noise in comparison to acquisitions using standard pixels at the same resolution and noise level. This holds for sharper convolution kernels at the spatial resolution limit of the standard mode, e.g., up to a factor 3.2 in noise reduction and a resulting potential dose reduction of up to almost 90%. Using sharper convolution kernels, UHR acquisitions allow for a significant dose reduction compared to acquisitions using the standard detector mode. Acquisitions should always be performed using the ultra-high resolution detector mode, if possible, to benefit from the intrinsic noise and dose reduction. •Ionizing radiation used in computed tomography examinations is a concern to public health. •The ultra-high resolution of novel photon-counting systems can be invested towards a noise and dose reduction if only a spatial resolution below the resolution limit of the detector is desired. • Acquisitions should always be performed in ultra-high resolution mode, if possible, to benefit from an intrinsic dose reduction.

Impact of the confluence of cardiac motion and high spatial resolution on performance of ECG-gated imaging with an investigational photon-counting CT system: A phantom study

PurposePhoton-counting CT (PCCT) has higher spatial resolution that conventional EID CT which improves imaging of stationary coronary plaques and stents.. In this work, we evaluated the relationship between higher spatial resolution and motion acquisition on an investigational PCCT system. MethodsAn investigational photon-counting CT scanner (Siemens CounT) with ECG gating was used to image a coronary tree phantom with models of healthy, stenotic, and stented arteries using a motion simulator. Images were acquired with matched clinical parameters at rest and 60 beats per minute. An additional set of high dose stationary images were averaged to generate a motion-free, reduced noise reference. Scans were completed at standard (0.5 mm2) and high-resolution (0.25 mm2). Motion images were reconstructed at multiple phases. Regions of interest were drawn around vessels and segmented. Percentage difference from the reference standard was evaluated for vessel diameter and circularity. Mutual information between the reference and stationary and motion datasets was used as a measure of volumetric similarity. ResultsThe stenotic vessel showed the most variation from the reference when compared to healthy or stented vessels. Compared to standard resolution, high-resolution images had lower bias for diameter (-0.012 ± 0.19% vs −0.052 ± 0.14%) and lower variability for circularity (-0.13 ± 0.138% vs −0.12 ± 0.144%). Both differences were found to be statistically significant. High-resolution images had a slightly lower mutual information (1.28) than standard resolution (1.31). ConclusionThe higher spatial resolution enabled by photon-counting CT can be harnessed for cardiac imaging as the benefits of high spatial resolution acquisitions remain relevant in the presence of motion.

Exploration of the pulse pileup effects in a clinical CdTe-based photon-counting computed tomography.

High tube current generates a high flux of x-rays to photon counting detectors (PCDs) that can potentially result in the piling up of pulses formed by concurrent photons, which can cause count loss and energy resolution degradation. To evaluate the performance of clinical photon-counting CT (PCCT) systems in high flux, potentially influenced by pulse pileup effects, in terms of task-generic image quality metrics. A clinical phantom was scanned on a commercial PCCT scanner (NAEOTOM Alpha, Siemens) at 120kV under fourteen different tube current levels (40-1000mA) with a rotation time of 0.25 s and a pitch of 1. The dose levels corresponded to CTDIvol (32cm phantom) of 0.79-19.8mGy. CT sinograms were reconstructed using QIR-off mode (noniterative reconstruction algorithm), Br44 kernel, and a voxel size of . imQuest, an open-source MATLAB-based software package was used to calculate noise power spectrum (NPS), task transfer function (TTF), contrast-to-noise ratio (CNR), and CT number according to AAPM Task Group 233 metrology. The 50% cut-off frequency of TTF (f50 ) remained mostly constant across all higher tube currents for all inserts, namely polyethylene, bone, air, and acrylic. Using the lowest two data points (40 and 80mA), the expected relationship between noise magnitude and tube current was determined to be noise mA-0.47 . The measured noise magnitude were up to 11.1% higher than the expected value at the highest tube current. The average frequency of NPS (fav ) decreased from 0.32 to 0.29mm-1 as tube current increased from 40 to 1000mA. No considerable effects were observed in CT number measurement of any insert; however, CT numbers for air and bone changed almost monotonically as tube current increased. Absolute CNR increased monotonically for all inserts; however, the difference between measured and expected CNRs were approximately -6% to 12% across all tube currents. Increasing tube currents did not affect the spatial resolution, but slightly affected the CT number and noise measurements of the clinical PCCT system. However, the effects were only considerable at clinically irrelevant tube currents used on a small 20-cm phantom. In general clinical practices, automatic exposure control techniques are used to decrease the variation of flux on the detector, which alleviates the chances of detector saturation due to high count rates. The observed effects could be due to pulse pileup, signal-dependent filtration of the system, or nonlinearities in the reconstruction algorithm. In conclusion, either the deadtime of the detector used in the photon-counting CT system is shorter such that count losses due to pulse pileup are negligible, or pulse pileup has inconsiderable effects on the image quality of clinical photon-counting CT systems in routine clinical practice due to possible corrections applied on the system.

Comprehensive evaluations of a prototype full field-of-view photon counting CT system through phantom studies.

Objective. Photon counting CT (PCCT) has been a research focus in the last two decades. Recent studies and advancements have demonstrated that systems using semiconductor-based photon counting detectors (PCDs) have the potential to provide better contrast, noise and spatial resolution performance compared to conventional scintillator-based systems. With multi-energy threshold detection, PCD can simultaneously provide the photon energy measurement and enable material decomposition for spectral imaging. In this work, we report a performance evaluation of our first CdZnTe-based prototype full-size PCCT system through various phantom imaging studies.Approach.This prototype system supports a 500 mm scan field-of-view and 10 mmz-coverage at isocenter. Phantom scans were acquired using 120 kVp from 50 to 400 mAs to assess the imaging performance on: CT number accuracy, uniformity, noise, spatial resolution, material differentiation and quantification.Main results.Both qualitative and quantitative evaluations show that PCCT, under the tested conditions, has superior imaging performance with lower noise and improved spatial resolution compared to conventional energy integrating detector (EID)-CT. Using projection domain material decomposition approach with multiple energy bin measurements, PCCT virtual monoenergetic images have lower noise, and good accuracy in quantifying iodine and calcium concentrations. These results lead to increased contrast-to-noise ratio (CNR) for both high and low contrast study objects compared to EID-CT at matched dose and spatial resolution. PCCT can also generate super-high resolution images using much smaller detector pixel size than EID-CT and greatly improve image spatial resolution.Significance.Improved spatial resolution and quantification accuracy with reduced image noise of the PCCT images can potentially lead to better diagnosis at reduced radiation dose compared to conventional EID-CT. Increased CNR achieved by PCCT suggests potential reduction in iodine contrast media load, resulting in better patient safety and reduced cost.

Initial Clinical Images From a Second-Generation Prototype Silicon-Based Photon-Counting Computed Tomography System

To demonstrate the feasibility andpotential of using a second-generation prototype photon-counting computed tomography (CT) system to provide simultaneous high spatial resolution images and high spectral resolution material information across a range of routine imaging tasks using clinical patient exposure levels. The photon-counting system employs an innovative silicon-based photon-counting detector to provide a balanced approach to ultra-high-resolution spectral CT imaging. An initial cohort of volunteer subjects was imaged using the prototype photon-counting system. Acquisition technique parameters and radiation dose exposures were guided by routine clinical exposure levels used at the institution. Images were reconstructed in native slice thickness using an early version of a spectral CT reconstruction algorithm Samples of images across a range of clinical tasks were selected and presented for review. Clinical cases are presented across inner ear, carotid angiography, chest, and musculoskeletal imaging tasks. Initial reconstructed images illustrate ultra-high spatial resolution imaging. The fine detail of small structures and pathologies is clearly visualized, and structural boundaries are well delineated. The prototype system additionally provides concomitant spectral information with high spatial resolution. This initial study demonstrates that routine imaging at clinically appropriate patient exposure levels is feasible using a novel deep-silicon photon-counting detector CT system. Furthermore, a deep-silicon detector may provide a balanced approach to photon-counting CT, providing high spatial resolution imaging with simultaneous high-fidelity spectral information.

Constrained one-step material decomposition reconstruction of head CT data from a silicon photon-counting prototype.

Spectral CT material decomposition provides quantitative information but is challenged by the instability of the inversion into basis materials. We have previously proposed the constrained One-Step Spectral CT Image Reconstruction (cOSSCIR) algorithm to stabilize the material decomposition inversion by directly estimating basis material images from spectral CT data. cOSSCIR was previously investigated on phantomdata. This study investigates the performance of cOSSCIR using head CT datasets acquired on a clinical photon-counting CT (PCCT) prototype. This is the first investigation of cOSSCIR for large-scale, anatomically complex, clinical PCCT data. The cOSSCIR decomposition is preceded by a spectrum estimation and nonlinear counts correction calibration step to address nonideal detectoreffects. Head CT data were acquired on an early prototype clinical PCCT system using an edge-on silicon detector with eight energy bins. Calibration data of a step wedge phantom were also acquired and used to train a spectral model to account for the source spectrum and detector spectral response, and also to train a nonlinear counts correction model to account for pulse pileup effects. The cOSSCIR algorithm optimized the bone and adipose basis images directly from the photon counts data, while placing a grouped total variation (TV) constraint on the basis images. For comparison, basis images were also reconstructed by a two-step projection-domain approach of Maximum Likelihood Estimation (MLE) for decomposing basis sinograms, followed by filtered backprojection (MLE + FBP) or a TV minimization algorithm (MLE + TVmin ) to reconstruct basis images. We hypothesize that the cOSSCIR approach will provide a more stable inversion into basis images compared to two-step approaches. To investigate this hypothesis, the noise standard deviation in bone and soft-tissue regions of interest (ROIs) in the reconstructed images were compared between cOSSCIR and the two-step methods for a range of regularization constraintsettings. cOSSCIR reduced the noise standard deviation in the basis images by a factor of two to six compared to that of MLE + TVmin , when both algorithms were constrained to produce images with the same TV. The cOSSCIR images demonstrated qualitatively improved spatial resolution and depiction of fine anatomical detail. The MLE + TVmin algorithm resulted in lower noise standard deviation than cOSSCIR for the virtual monoenergetic images (VMIs) at higher energy levels and constraint settings, while the cOSSCIR VMIs resulted in lower noise standard deviation at lower energy levels and overall higher qualitative spatial resolution. There were no statistically significant differences in the mean values within the bone region of images reconstructed by the studied algorithms. There were statistically significant differences in the mean values within the soft-tissue region of the reconstructed images, with cOSSCIR producing mean values closer to the expectedvalues. The cOSSCIR algorithm, combined with our previously proposed spectral model estimation and nonlinear counts correction method, successfully estimated bone and adipose basis images from high resolution, large-scale patient data from a clinical PCCT prototype. The cOSSCIR basis images were able to depict fine anatomical details with a factor of two to six reduction in noise standard deviation compared to that of the MLE + TVmin two-stepapproach.

A systematic task-based image quality assessment of photon-counting and energy integrating CT as a function of reconstruction kernel and phantom size.

Image quality of photon-counting and energy integrating CT scanners changes with object size, dose to the object, and kernel selection. To comprehensively compare task-generic image quality of photon-counting CT (PCCT) and energy integrating CT (EICT) systems as a function of phantom size, dose, and reconstruction kernel. A size-variant phantom (Mercury Phantom 3.0) was used to characterize the image quality of PCCT and EICT systems as a function of object size. The phantom contained five cylinders attached by slanted tapered sections. Each cylinder contained two sections: one uniform for noise, and the other with inserts for spatial resolution and contrast measurements. The phantom was scanned on Siemens' SOMATOM Force and NAEOTOM Alpha at 1.18 and 7.51mGy without tube current modulation. CTDIvol was matched across two systems by setting the required tube currents, else, all other acquisition settings were fixed. CT sinograms were reconstructed using FBP and iterative (ADMIRE2 - Force; QIR2 - Alpha) algorithms with Body regular (Br) kernels. Noise Power Spectrum (NPS), Task Transfer Function (TTF), contrast-to-noise ratio (CNR), and detectability index (d') for a task of identifying 2-mm disk were evaluated based on AAPM TG-233 metrology using imQuest, an open-source software package. Averaged noise frequency (fav ) and 50% cut-off frequency for TTF (f50 ) were used as scalar metrics to quantify noise texture and spatial resolution, respectively. The difference between image quality metrics' measurements was calculated as IQPCCT - IQEICT . From Br40 (soft) to Br64 (sharp), f50 for air insert increased from 0.35mm-1 ±0.04 (standard deviation) to 0.76mm-1 ±0.17, 0.34mm-1 ±0.04 to 0.77mm-1 ±0.17, 0.37mm-1 ±0.02 to 0.95mm-1 ±0.17 for PCCT-T3D-QIR2, PCCT-70keV-QIR2, and EICT-ADMIRE2, respectively, when averaged over all sizes and dose levels. Similarly, from Br40 to Br64, noise magnitude increased from 10.86 HU±7.12 to 38.61 HU±18.84, 10.94 HU±7.08 to 38.82 HU±18.70, 13.74 HU±11.02 to 52.11 HU±26.22 for PCCT-T3D-QIR2, PCCT-70keV-QIR2, and EICT-ADMIRE2, respectively. The difference in fav was consistent across all sizes and dose levels. PCCT-70keV-VMI showed better consistency in contrast measurements for iodine and bone inserts than PCCT-T3D and EICT; however, PCCT-T3D had higher contrast for both inserts. From Br40 to Br64, considering all sizes and dose levels, CNR for iodine insert decreased from 52.30±46.44 to 12.18±10.07, 40.42±33.42 to 9.48±7.16, 39.94±37.60 to 7.84±6.67 for PCCT-T3D-QIR2, PCCT-70keV-QIR2, and EICT-ADMIRE2, respectively. Both PCCT image types, T3D and 70-keV-VMI exhibited similar or better noise, contrast, CNR than EICT when comparing kernels with similar names. For 512×512 matrix, PCCT's sharp kernels had lower resolution than EICT's sharp kernels. For all image quality metrics, except extreme low, every dose condition had a similar set of IQ-matching kernels. It suggests that considering patient size and dose level to determine IQ-matching kernel pairs across PCCT and EICT systems may not be imperative while translating protocols, except when the signal to the detector is extremely low.

A methodology for incorporating a photon-counting CT system into routine clinical use.

Photon-counting computed tomography (PCCT) systems are increasingly available in the U.S. following Food and Drug Administration (FDA) approval of the first clinical PCCT system in Fall 2021. Consequently, there will be a need to incorporate PCCTs into existing fleets of traditional CT systems. The commissioning process of a PCCT was devised by evaluating the degree of agreement between the performance of the PCCT and that of established clinical CT systems. A PCCT system (Siemens NAEOTOM Alpha) was evaluated using the American College of Radiology(ACR) CT phantom (Gammex 464). The phantom was scanned on the system and on a 3rd Generation EID CT system (Siemens Force) at three clinical dose levels. Images were reconstructed across the range of available reconstruction kernels and Iterative Reconstruction (IR) strengths. Two image quality metrics-spatial resolution and noise texture-were calculated using AAPM TG233 software (imQuest), as well as a dose metric to achieve target image noise magnitude of 10HU. For each pair of EID-PCCT kernel/IR strengths, the difference in metrics were calculated, weighted, and multiplied over all metrics to determine the concordance between systems. IR performance was characterized by comparing relative noise texture and reference dose as a function of IR strength for each system. In general, as kernel "sharpness" increased for each system, spatial resolution, noise spatial frequency, and reference dose increased. For a given kernel, EID reconstruction showed higher spatial resolution compared to PCCT in standard resolution mode. PCCT implementation of IR better preserved noise texture across all strengths compared to the EID, demonstrated by respective 20 and 7% shifts in noise texture from IR "Off" to IR "Max." Overall, the closest match for a given EID reconstruction kernel/IR strength was identified as a PCCT kernel with "sharpness" increased by 1 step and IR strength increased by 1-2 steps. Substantial dose reduction potential of up to 70% was found when targeting a constant noise magnitude.
