The role of denture as periodontal splint

Periodontal disease causes damage to the structure of periodontal tissue characterized by the presence of tooth mobility. Splint is one of the therapies to support the healing of periodontal tissue that aims to provide stability so that occlusal pressure can be evenly distributed to other teeth. The design of dentures that can act as splint is important to maintain the stability of the teeth that mobile while restoring the chewing function. Some designs of denture that can be used as periodontal splint, namely external removable permanent splint and internal fixed permanent splint.

Delayed Repositioning in Teeth with Horizontal Root Fracture: Two Case Reports

Introduction: Horizontal root fracture (HRF) generally has a good prognosis of healing at fracture line after repositioning and flexible splinting. However, various factors such as delayed referral may unfavorably influence close reduction of firmly displaced coronal fragments and the long-term prognosis of healing at fracture line. Case 1: A 25-year-old woman with HRF in her maxillary central incisors was referred 1 week after trauma. Repositioning of the displaced coronal fragment was not successful for the left central incisor. Despite questionable prognosis for this case, reduction and flexible splinting was performed after removing it’s coronal fragment, minor curettage in alveolar socket and immediate replanting. Calcium hydroxide dressing and MTA plug placement for the coronal fragment were carried out after 1 and 3 weeks, respectively. The crown was restored and a minor permanent splint was applied after splint removal. Case 2: The above protocol was applied for a 17-year-old boy with HRF in his left maxillary central incisor. He referred 3 weeks after trauma with a firmed displaced coronal fragment. At four-year follow-up in both patients, the teeth were clinically in function and the patients were asymptomatic. The periapical radiographs revealed complete healing at fracture lines.

Is there any place for spontaneous healing in deep palmar burn of the child?

Child palm burns arise by contact and are often deep. The singular difficulty of such a disease comes from the necessity of the child growth and from the potential occurrence of constricted scars. In order to avoid sequelae, the actual gold standard is to practice an early excision of the burn, followed by a skin graft. The aim of this study is to evaluate the results of spontaneous healing combined with rehabilitation versus early skin grafting and rehabilitation concerning the apparition of sequelae. We performed a retrospective study in two burn centers and one rehabilitation hospital between 1995 and 2010. Eighty-seven hands have been included in two groups: one group for spontaneous healing and the other group for excision and skin grafting. Every child benefited from a specific rehabilitation protocol. The two main evaluation criteria were the duration of permanent splint wearing and the number of reconstructive surgery for each child. The median follow-up duration is about four years. The two groups were comparable. For the early skin grafting group, the splint wearing duration was 1/3 longer than for the spontaneous healing group. Concerning the reconstructive surgery, half of the grafted hands needed at least one procedure versus 1/5 of spontaneous healing hands. Our results show the interest of spontaneous healing in palmar burn in child, this observation requires a specific and intense rehabilitation protocol.

Loosening of the teeth is one of the symptoms of periodontal disease characterized by loss of bonding as well as vertical bone damage. Loosening can be caused by damage to the bone that supports the teeth, trauma from occlusion, and the presence of inflammation of the gingiva to the expansion of the network of supporters who are more patologik and processes in the jaw. Purpose: one of the ways to control and stabilize the loosening of the teeth is splinting. Splinting is indicated on the State loosening teeth degree 3 with severe bone damage. Literature review: Periodontal splint is a tool that can be used for stabilization or loosening of teeth that immobilisasi experience. Splint consists of temporary and permanent splint splint. Indication temporary splint is for stabilization of teeth rocking before and during therapy with the goal to reduce the peridontal trauma at the time of care and speed up the healing process, such as the ligature wire splint. Discharging permanent splint is part of this phase of the restoration or reconstruction phases of periodontal care. Permanent Splint is extremely limited usage. Used when actually used to increase the stability of pressure oklusal. Summary: the selection of the alternatives of periodontal splint adapted on certain conditions, such as economic status and health status of the patient.

Splint is one of the therapies to support the periodontal healing; aims to stabilize the teeth so the occlusal load canbe distributed equally to all other teeth. Periodontal splint can be used temporarily or permanently. The removableframe partial denture can be functioned not only for replacing the missing teeth but also as permanent splint. Initialtherapy has been implemented to all cases on this report, consists of scaling, root planning, occlusal adjustment andsurgical therapy including curettage, flap operation, but apparently teeth mobility still existed. In order to supportthe periodontal tissue health, the frame of removable partial denture is needed as splint. It can be concluded thatimplementation of the removable partial denture with framework as splint to support the periodontal treatmentshould be provided after the initial and surgical therapy.

Removable dental prosthesis as periodontal treatment method

The objectives of prostheses are to restore mastication force, improve esthetics and maintain gingival health. The construction and function of prosthesis restoration are mutually interdependent with condition of periodontal tissues. A properly constructed prosthesis is an integral phase of complete treatment of periodontal disease in order to maintain periodontal tissues health. This paper reports case of a man aged 47 years who came to Dental Specialist Clinic in Oral and Dental Hospital Faculty of Dentistry Universitas Padjadjaran with chief complaint of mobility in almost all his teeth and they seems to look longer. The patient has no systemic disease and did not want his teeth to be extracted. Clinical and panoramic radiographic and laboratoris examinations has been done. During treatment, oclusal adjustment and splinting had been done on tooth 33,34,35 and tooth 44,43 also tooth 43,42 splinting with composite. The following treatment was the acrylic removable partial denture for upper jaw while mandible was fitted a frame denture which functioned as a semi permanent splint. One month post treatment, patien felt comfort and the denture was well functioning.

Penanggulangan Kerusakan Periodontal Akibat Pemakaian Rubber Ligature (Laporan Kasus)

Orthodontic treatment become more popular and interest by the people. Therefore a sufficient knowledge of dentist is very necessary. Improper planning and treatment or using unsuitable equipment wil damage periodontal tissue. If it's happened and untreated carefully as soon as possible, the teeth will fall out. A boy with eight years old got an inflammation and hardly mobility teeth (°4). The condition is happen after using orthodontic rubber ligature to overcome central diastema. Inflammation become harden by leaving of rubber ligature on undetection apical part. A patient got a plaque control, medication, flap operation, temporary and semi permanent splinting. Agter seven month tratment, patient had a minimal inflammation and decreased of mobility teeth until degree two.

Successful conduit repair using aortic homograft in a jehovah's witness child

A 10-year-old female child of the Jehovah's Witness faith presented with congenitally corrected transposition of the great arteries (S,L,L), pulmonary atresia, and a ventricular septal defect. A successful surgical correction was performed using an aortic homograft as a valved extracardiac conduit without the use of homologous blood or blood products. We used permanent splinting of the sternum with a methyl methacrylate resin plate to prevent compression of the conduit.

Resinous plate for permanent sternal splinting in patients with extracardiac conduits

During operations on patients with extracardiac conduits, sternal reapproximation occasionally results in critical compression of the conduit and serious deterioration in hemodynamic function. Permanent splinting of the sternum with a methyl methacrylate resinous plate provides an adequate substernal space for the extracardiac conduit.

Significance of occlusion in the etiology and treatment of early, moderate, and advanced periodontitis.

Elimination of lesions diagnostic of trauma from occlusion is an essential part of complete periodontal therapy and restoration of health in the masticatory system for such patients. This can be achieved by orthodontic treatment, temporary splinting, bite-planes, occlusal adjustment and permanent splinting of teeth. Occlusal therapy may be required during periodontal treatment for trauma from occlusion and to enhance occlusal stability at any stage of periodontitis, but is most often needed in advanced periodontitis. Splinting of hypermobility of self-limiting trauma from occlusion is not indicated. Splinting may be required in addition to occlusal adjustment in moderate to severe periodontitis when trauma from occlusion is progressive.

Pin-retained composites for splinting lower anterior teeth

Splinting of teeth follows a pattern of either a temporary type of splint or a permanent type of splint. The classic temporary splint is composed of wires which circumferentially engage the six lower incisors and impart a degree of rigidity and support to them, especially when the wires are stabilized with one of the unfilled resins.' These techniques often rely upon surface anchorage to enamel via the acid-etch method. The major disadvantage of this method is danger of fracture at the composite-enamel junction. Effective permanent splinting of lower anterior teeth may be accomplished by crowning the teeth or by anchoring them to a lingual plate of gold with pins. These pins may be an integral part of a gold casting,2. 3 or they may be screwed to the lingual gold plate.4. 5 To ensure permanence they are cemented to the teeth to convert them into a single rigid unit. The major disadvantages of the pin-retained casting are its expense and difficulty in fabrication. The method described in this article might b e classified as semipermanent. It involves the use of the acid-etch concept, composite resin, and interlocking TMS Minim pinst in the lingual embrasure (Fig. 1).

A simplified technique for treating mallet fingers: Tenodermodesis

Tenodermodesis is a useful surgical procedure in cases of mallet fingers where conservative treatment has failed. The results have been satisfactory in 22 of 26 patients. These results all have been achieved within 60 days and with no more than 35 days of permanent splinting of the distal part of the digit. The final range of motion depends on the patient and on the patient's age; this procedure should not be proposed for elderly patients. The operation is not performed until 3 months after the initial trauma and until formal request from a patient who cannot tolerate this small disability. We recommend local anesthesia for this procedure. Thus with a small incision, without hospitalization, and with minimal splinting and immobilization, this procedure provides a simple treatment for a minor lesion, a principle which never should be forgotten.

One-piece cast permanent splint using a die-investing technic (author's transl)

A system of using a new refractory die material was introduced to fabricate a permanent splint for mobile teeth with the indirect method and one-piece casting.This technic was clinically applied for three female patient mouth stabilizations and has produced favorable results.Far less time, labor and skill than other technics are required by this die-investing technic.

Pin-retained amalgam splints

Stabilization of mobile teeth is accomplished, in this simple new technic, through cemented stainless steel pins that lock teeth in position. The procedure requires only one appointment, costs far less than precision gold casting, and offers hope, in the pilot study, of being an adequate permanent splint.

With the use of the findings of many fundamental investigations, a system was established to make not only individual inlays or crowns but also extensive fixed partial dentures or splints by the indirect method and one-piece castings. A procedure for making a permanent splint for mobile teeth by this system has been described. This technique provides favorable and accurate results, and requires far less labor, time, and skill than other techniques.

Occlusal traumatism—its pathological effects and diagnosis

The pathological effects of occlusal disturbances may be seen in the temporomandibular articulation and related musculature and in certain periodontal tissues. Periodontal traumatism may be primary or secondary. The treatment of primary traumatism usually requires only changes in the force applied on a tooth; treatment of secondary traumatism requires many procedures, sometimes including permanent splinting. Occlusal overloading is not an etiological factor in gingivitis, gingival hyperplasia, or pocket formation, and ordinarily will not cause gingival recession, gingival clefts or resorption of the alveolar crest.
