Introduction: Patients with end-stage renal disease who start active treatment with peritoneal dialysis have an increased risk of developing infections. Methods: This paper will present information from relevant professional/scientific sources regarding modern diagnostic and therapeutic modalities for the treatment of infections in patients on peritoneal dialysis. Topic: Infections associated with peritoneal dialysis are: peritoneal catheter exit site infections, tunnel infections and peritonitis. The symptoms of the mentioned infections vary from mild ones such as erythema and secretion at the exit point of the peritoneal catheter to pronounced symptomatology in the development of peritonitis accompanied by abdominal pain and elevated body temperature. The most common cause of peritonitis is infection of the exit site of the catheter, and the two main causes of infection are Staphylococcus aureus and Pseudomonas aeruginosa. If the existence of an infection is suspected, it is necessary to sample the dialysate for cytological examination and culture and to take a swab of the exit site of the catheter. Treatment begins with empiric antibiotic therapy, then it is corrected according to the antibiogram, and the exit site is treated locally with an antibiotic. If there is no therapeutic response after five days of intraperitoneal therapy in peritonitis, it is recommended to remove the catheter. Conclusion: According to the current guidelines of the International Society for Peritoneal Dialysis (ISPD), timely prevention of infections, diagnosis and treatment of peritoneal dialysis-related infections are necessary to prolong patient survival.

Knowledge of Nutrition Care for Children on Peritoneal Dialysis at National Hospital of Pediatrics, Vietnam

Background: Nutrition is critically important for chronic kidney diseases, especially for children on CAPD (continuous ambulatory peritoneal dialysis). Nutrition not only plays the role of medication but also can control most abnormal metabolism disorders and preserve the residual renal functions. However, most patients’ families just focus on peritoneal dialysis while ignoring nutrition for the patients. Objective: Survey the knowledge of nutrition care for children with CAPD prescription. Method: Cross-sectional study on the nutritional status of 31 children undergoing CAPD. Interview and assess knowledge on kidney diseases with peritoneal dialysis, knowledge on nutrition care and nutrition practice of 31 mothers with children undergoing CAPD via a designed questionnaire. Result: The ratio of malnutrition of children on CAPD was 37.8%. Knowledge on caregiving and hygiene for CAPD was good with 74.4% mothers knowing about complications of peritonitis, 64.4% was aware of peritoneal catheter exit-site infection. However knowledge on nutrition was limited, only 25.8% mothers having knowledge on nutrition for the children. 9.7% mothers could meet the requirement in nutrition practice. Recommendation: It is necessary to enhance nutrition communication and counselling to achieve the expected treatment outcomes.

Peritonitis and catheter exit-site infection in patients on peritoneal dialysis at home.

Objective: to analyze the complications related to peritonitis and catheter exit-site infections, in patients on peritoneal dialysis at home. Method: quantitative and cross-sectional study, carried out with 90 patients on peritoneal dialysis at home, in a municipality in the Northeast region of Brazil. For data collection, it was used two structured scripts and consultation on medical records. Descriptive analysis and comparison tests among independent groups were used, considering p<0.05 as level of statistical significance. Results: by comparing the frequency of peritonitis and the length of treatment, it was found that patients over two years of peritoneal dialysis were more likely to develop peritonitis (X²=6.39; p=0.01). The number of episodes of peritoneal catheter exit-site infection showed association with the length of treatment (U=224,000; p=0.015). Conclusion: peritonitis and catheter exit-site infection are associated with the length of treatment.

Diverticular Disease and Treatment with Gastric Acid Inhibitors do not Predispose to Peritonitis of Enteric Origin in Peritoneal Dialysis Patients

Enteric peritonitis (EP) is an infrequent complication of peritoneal dialysis (PD), with severe consequences for peritoneal membrane viability and patient outcome. Factors such as diverticular disease and gastric acid inhibitors have been implicated in its appearance. We investigated several risk factors, including those mentioned below, that can influence the development of EP. Retrospective cross-sectional study. Tertiary-care public university hospital. Fifty-seven PD patients treated in our PD unit during August 1998. A barium enema was performed on 50 of the 57 patients (the remaining 7 patients refused it) in order to exclude the presence of diverticulosis. All episodes of peritonitis occurring in those patients, including EP, were registered. Enteric peritonitis was defined as that caused by gram-positive, gram-negative, or fungus micro-organisms that colonized the intestinal tract, excluding episodes secondary to genitourinary tract or peritoneal catheter exit-site infections. Twenty-four patients showed diverticular disease in the barium enema, but only 5 of them (21%) had any EP episode. Five of the 26 patients with no diverticula (19%) had EP. Fifty-five episodes of peritonitis were reported in 21 patients; 15 episodes of EP (27.3% of all) developed in 11 patients. Seven of the 11 patients (64%) required peritoneal catheter removal and 3 of them (27%) finally were transferred to hemodialysis due to consequences of the EP episode. Logistic regression analysis did not find any of the independent variables analyzed (age, sex, time on PD, type of PD, peritoneal transport parameters, presence of polycystic kidney disease, constipation or diverticulosis, or treatment with gastric acid inhibitors, or phosphate-binding agents) to be risk factors for developing EP. Neither diverticulosis nor treatment with gastric acid inhibitors seem to be risk factors for developing peritonitis of enteric origin in PD patients. This type of peritonitis has to be promptly identified and treated in order to diminish the high frequency of peritoneal catheter removal and PD dropout due to such episodes.

Adherence of Staphylococcus aureus isolated in peritoneal dialysis-related exit-site infections to HEp-2 cells and silicone peritoneal catheter materials.

Peritoneal catheter exit-site infections cause a relevant morbidity in peritoneal dialysis patients and are frequently caused by Staphylococcus aureus. We tested the hypothesis that adherence of exit-site-derived S. aureus to epithelial cells and peritoneal catheter silicone tubes discriminates virulent and less virulent strains. The binding of isolated S. aureus to an epithelial cell line (HEp-2) and to silicone tubes was analyzed using light-microscopy or radioactive labeling of bacteria. Of 378 exit-site swabs, 99 (26%) were positive for microbial growth. S. aureus was cultured in 25 of 99 positive swabs; three of 13 swabs taken in exit-site infections grade 3 and 4 that had tested positive for S. aureus. Adherence of S. aureus from exit-site infections grade 2, 3 and 4 to Hep-2 cells did not differ from adherence of bacteria isolated from asymptomatic or moderately inflamed catheter exit sites (grade 0-2). However, binding of S. aureus to silicone tubes was enhanced in grade 0/1 compared with grade 2-4 exit-site isolates. Staphylococcus aureus is an important pathogen in CAPD-related exit-site infection being isolated in about 6.6% of all exit-site swabs (and in 25% of all positive swabs). Silicone-adhesive strains may be of more clinical significance in peritoneal dialysis patients since adhesion to silicone was increased in S. aureus strains isolated in more severe exit-site infections.

Exit. Site Care -is it Time for a Change?

Peritoneal catheter exit-site infections (ESI's) continue to impact significantly on morbidity and catheter longevity. The controversy concerning protocols for daily exit-site care continues for frequency, methodology, cleansing agent, and dressing. Routine daily exit-site care prior to January 1991 consisted of daily showers using liquid soap, povidone scrub, rinsing the shower, and drying with a 4 x 4-in. gauze pad. Catheters have always been immobilized, either with tape or an immobilizing device. Hydrogen peroxide was used only when needed to soften crust formation prior to showering. A light dressing, usually 2 x 2 in., was optional. A recent survey revealed that povidone iodine was the antiseptic of choice for catheter care in 75% of the respondents. However, povidone iodine irritates and dries the skin predisposing it to infection. ESI's are prospectively monitored as part of our quality improvement (QI) program. An incidence of 0.76 episodes/patient-year was noted between January 1989 and May 1991. Given the relative high frequency of ESI's, the protocol was modified and introduced during the January-May 1991 time frame. Routine care now consists of daily showers using only CC-500, a gentle antibacterial cleaner (Care-Tech Laboratories, Inc.), rinsing in the shower, and drying with a 4 x 4-in. gauze. Use of hydrogen peroxide and dressings has remained the same. Additionally, a protocol addressing the prophylaxis for traumatized exist sites was initiated. The incidence of ESI's has dropped significantly to 0.12 episodes/patient-year. Although our population size is small (n = 18), this study does point out the utility of prospectively monitoring trends for appropriate indicators within a QI program.(ABSTRACT TRUNCATED AT 250 WORDS)

The Influence of Peritoneal Catheter Exit-Site Infections on Peritonitis, Tunnel Infections, and Catheter Loss in Patients on Continuous Ambulatory Peritoneal Dialysis

The importance of exit-site infections (ESIs) as a source of peritonitis and catheter loss in continuous ambulatory peritoneal dialysis (CAPD) patients is unknown. We collected data on 137 CAPD patients over a 5-year period (2,052 cumulative patient months). Patients with a history of ESIs were more likely to have peritonitis and tunnel infections than patients without a history of ESIs. A larger percentage of patients with a history of ESIs lost catheters and transferred to hemodialysis than those without such a history, independent of the effect of peritonitis. These data confirm the importance of reducing the incidence of ESIs. More information is needed to determine the nature of the relationship between ESIs and peritonitis.
