Sex-specific reference values for total, central, and peripheral latency of motor evoked potentials from a large cohort.

Differentiating between physiologic and altered motor evoked potentials (MEPs) to transcranial magnetic stimulation (TMS) is crucial in clinical practice. Some physical characteristics, such as height and age, introduce sources of variability unrelated to neural dysfunction. We provided new age- and height-adjusted normal values for cortical latency, central motor conduction time (CMCT), and peripheral motor conduction time (PMCT) from a large cohort of healthy subjects. Previously reported data from 587 participants were re-analyzed. Nervous system disorders were ruled out by clinical examination and magnetic resonance imaging. MEP latency was determined as stimulus-to-response latency through stimulation with a circular coil over the "hot spot" of the First Dorsal Interosseous and Tibialis Anterior muscles, during mild tonic contraction. CMCT was estimated as the difference between MEP cortical latency and PMCT by radicular magnetic stimulation. Additionally, right-to-left differences were calculated. For each parameter, multiple linear regression models of increasing complexity were fitted using height, age, and sex as regressors. Motor evoked potential cortical latency, PMCT, and CMCT were shown to be age- and height-dependent, although age had only a small effect on CMCT. Relying on Bayesian information criterion for model selection, MEP cortical latency and PMCT were explained best by linear models indicating a positive correlation with both height and age. Also, CMCT to lower limbs positively correlated with height and age. CMCT to upper limbs positively correlated to height, but slightly inversely correlated to age, as supported by non-parametric bootstrap analysis. Males had longer cortical latencies and CMCT to lower limbs, as well as longer PMCT and cortical latencies to upper limbs, even when accounting for differences in body height. Right-to-left-differences were independent of height, age, and sex. Based on the selected regression models, sex-specific reference values were obtained for all TMS-related latencies and inter-side differences, with adjustments for height and age, where warranted. A significant relationship was observed between height and age and all MEP latency values, in both upper and lower limbs. These set of reference values facilitate the evaluation of MEPs in clinical studies and research settings. Unlike previous reports, we also highlighted the contribution of sex.

Detection of C8/T1 radiculopathy by measuring the root motor conduction time

BackgroundRoot motor conduction time (RMCT) can noninvasively evaluate the status of the proximal root segment. However, its clinical application remains limited, and wider studies regarding its use are scarce. We aimed to investigate the association between C8/T1 level radiculopathy and RMCT.MethodsThis was a retrospective cross-sectional study. Subjects were extracted from a general hospital’s spine clinic database. A total of 48 C8/T1 root lesions from 37 patients were included, and 48 C8/T1 root levels from control subjects were matched for age, sex, and height. RMCT was measured in the abductor pollicis brevis muscle and the assessment of any delays owing to C8/T1 radiculopathy.ResultsThe RMCT of the C8/T1 radiculopathy group was 1.7 ± 0.6 ms, which was significantly longer than that in the control group (1.2 ± 0.8 ms; p = 0.001). The delayed RMCT was independently associated with radiculopathy (adjusted odds ratio, 1.15; 95% confidence interval, 1.06–1.27; p = 0.011) after adjusting for the peripheral motor conduction time, amplitude of median compound motor nerve action potential, and shortest F-wave latency. The area under the Receiver Operating Characteristic curve for diagnosing C8/T1 radiculopathy using RMCT was 0.72 (0.61–0.82). The RMCT was significantly correlated with symptom duration (coefficient = 0.58; p < 0.001) but was not associated with the degree of arm pain.ConclusionOur findings illustrate the clinical applicability of the RMCT by demonstrating its utility in diagnosing radiculopathy at certain spinal levels.

Normal Values of Central, Peripheral, and Root Motor Conduction Times in a Healthy Korean Population.

Central, peripheral, and root motor conduction times (CMCTs, PMCTs, and RMCTs, respectively) are valuable diagnostic tools for spinal cord and motor nerve root lesions. We investigated the normal values and the effects of age and height on each motor conduction time. This study included 190 healthy Korean subjects who underwent magnetic stimulation of the cortex and spinous processes at the C7 and L1 levels. Recording muscles were abductor pollicis brevis and abductor digiti minimi in the unilateral upper limb and extensor digitorum brevis and abductor hallucis in the contralateral lower limb. F-wave and compound motor nerve action potentials were also recorded. Central motor conduction time was evaluated as the difference between cortical motor evoked potential onset latency and PMCT using calculation and spinal stimulation methods. Root motor conduction time was computed as the difference between spinal stimulated and calculated CMCTs. The average age and height of the participants were 41.21 ± 14.39 years and 164.64 ± 8.27 cm, respectively; 39.5% (75/190) patients were men. In the linear regression analyses, upper limb CMCTs showed a significant and weak positive relationship with height. Lower limb CMCTs demonstrated a significant and weak positive relationship with age and height. Peripheral motor conduction times were significantly and positively correlated with age and height. Root motor conduction times showed no significant relationship with age and height, except for abductor pollicis brevis-RMCT, which had a weak negative correlation with height. This study provides normal values of CMCTs, PMCTs, and RCMTs, which have potential clinical applications. When interpreting CMCTs, age and height should be considered.

Clinical, Neurophysiological, and MRI Markers of Fampridine Responsiveness in Multiple Sclerosis-An Explorative Study.

Objective: Persons with multiple sclerosis (PwMS), already established as responders or non-responders to Fampridine treatment, were compared in terms of disability measures, physical and cognitive performance tests, neurophysiology, and magnetic resonance imaging (MRI) outcomes in a 1-year explorative longitudinal study.Materials and Methods: Data from a 1-year longitudinal study were analyzed. Examinations consisted of the timed 25-foot walk test (T25FW), six spot step test (SSST), nine-hole peg test (9-HPT), five times sit-to-stand test (5-STS), symbol digit modalities test (SDMT), transcranial magnetic stimulation (TMS) elicited motor evoked potentials (MEP) examining central motor conduction times (CMCT), peripheral motor conduction times (PMCT) and their amplitudes, electroneuronography (ENG) of the lower extremities, and brain structural MRI measures.Results: Forty-one responders and eight non-responders to Fampridine treatment were examined. There were no intergroup differences except for the PMCT, where non-responders had prolonged conduction times compared to responders to Fampridine. Six spot step test was associated with CMCT throughout the study. After 1 year, CMCT was further prolonged and cortical MEP amplitudes decreased in both groups, while PMCT and ENG did not change. Throughout the study, CMCT was associated with the expanded disability status scale (EDSS) and 12-item multiple sclerosis walking scale (MSWS-12), while SDMT was associated with number of T2-weighted lesions, lesion load, and lesion load normalized to brain volume.Conclusions: Peripheral motor conduction time is prolonged in non-responders to Fampridine when compared to responders. Transcranial magnetic stimulation-elicited MEPs and SDMT can be used as markers of disability progression and lesion activity visualized by MRI, respectively.Clinical Trial Registration: www.ClinicalTrials.gov, identifier: NCT03401307.

Cervical Intraspinal Conduction Time by Magnetic Root Stimulation versus Conventional Electromyography in the Diagnosis of Cervical Radiculopathy

Background: Cervical radiculopathy (CR) is a pathological procedure that encompasses root of the cervical nerve. Cervical disc herniation, accompanied by cervical spondylosis, is among the most common causes of radiculopathy. Clinical neurophysiology is a medical field focused mainly on assessing activity in the nervous system and muscles being an expansion of neurological assessment, using particular same anatomical localization criteria as clinical testing. Objective: To evaluate the sensitivity and specificity of conduction time of the intraspinal segment of peripheral motor neurons in the diagnosis of CR using cervical root magnetic stimulation and conventional electromyography (EMG) study. Methods: Fifty patients (15 males and 35 females) aged 46.02±9.76 years with reported CR were subjected to an electrophysiological examination and control group consisted of 50 (36 females and 14 males) healthy volunteers aged 36.24±12.09 years. Analysis of sensory and motor nerve conduction, conventional needle EMG test, and motor evoked potentials (MEPs) for all (recording abductor pollicis brevis (APB), abductor digiti minimi (ADM), biceps brachii (BB), deltoid muscles) was conducted to determine the peripheral nerves, muscles, sensory and motor pathways. The motor evoked potential (MEP) parameters studied for the median, ulnar musculocutaneous and axillary nerves include latency of the spinal root {peripheral motor conduction time} (PMCT). Results: During direct cervical root stimulation, the PMCT of the median nerve shows the highest specificity (59%) than that of the ulnar nerve (57%), while the intraspinal latency shows the same specificity for both nerves (57%). In comparison, PMCT by kimura formula recording ADM during direct stimulation of the cervical root have the highest sensitivity (56%) than that of PMCT by kimura formula recording APB. Additionally, the recording of direct cervical root stimulation amplitude after deltoid muscle showed the highest specificity and sensitivity (57.0%, 58.0% respectively). On the other hand, PMCT shows the highest specificity and sensitivity while the BB muscle is recoded (62.0%, 49.0%). Conclusion: Overall, the current motor evoked potential study shows abnormalities in less than 60% of patients with cervical radiculopathy relative to conventional EMG needle study abnormalities that reached 90%. A cross-sectional comparison of performance, neurophysiological and MRI outcomes of responders and non-responders to fampridine treatment in multiple sclerosis – An explorative study

ObjectiveTo compare baseline physical and cognitive performance, neurophysiological, and magnetic resonance imaging (MRI) outcomes and examinetheir interrelationship inparticipants with Multiple Sclerosis (MS), already established aseither responder or non-responder to Fampridine treatment, andto examine associationswiththe expanded disability status scale (EDSS) and 12-item MS walking scale (MSWS-12). ConclusionNo differences were identified between responders and non-responders to Fampridine treatment regarding physical and cognitive performance, neurophysiological or MRI outcomes. The results call for cautious interpretation and further large-scale studies are needed to expand ourunderstanding of underlying mechanisms discriminating Fampridine responders and non-responders.CMCT may be used as a marker of disability and walking impairment, while SDMT was associated with white matter lesions estimated by MRI.ClinicalTrials.gov identifier: NCT03401307.

Determining the Corticospinal and Neuromuscular Responses Following a Warm-Up Protocol

Background: The effect of warming-up prior to exercise on increased neuromuscular transmission speed remains largely untested. Objective: This study used transcranial magnetic stimulation (TMS) and peripheral nerve stimulation (PNS) to quantify neuromuscular transmission along the corticospinal tract (CST) before and after a warm-up protocol of the elbow flexors. Method: Using a single-group, pre-test-post-test design, 30 participants (20 male; 10 female; mean age 26.3 ± 7.4 years) completed four sets of bicep curls that aimed to increase heart rate (HR) and biceps brachii (BB) muscle temperature by a minimum of 40 beats per minute (bpm) and 1°C, respectively. Single-pulse TMS was applied to the primary motor cortex, and over the cervical and thoracic (C7-T1) areas of the spine to quantify motor evoked potentials (MEPs) and spinal evoked potentials (SEPs), respectively. Central motor conduction time (CMCT) was determined by calculating the difference in latency time of the onset of MEPs and SEPs. Peripheral motor conduction time (PMCT) was calculated following stimuli from Erb’s point to the onset of the maximal compound muscle action potential twitch (MMAX latency). MMAX time to peak twitch was also measured. MMAX amplitude was used to normalize the MEP to quantify corticospinal excitability. Results: Following the warm-up, significant increases in mean heart rate (44.8 ± 11.7 bpm; P &lt; 0.001) and muscle temperature (1.4 ± 0.6°C; P &lt; 0.001) were observed. No changes were seen in corticospinal excitability (P = 0.39), CMCT (P = 0.09), or MMAX latency (P = 0.24). However, MMAX time to peak twitch was significantly reduced (P = 0.003). Conclusion: This study has shown that exercise-based warm-ups improve neuromuscular conduction velocity via thermoregulatory processes that result in the onset of muscle contraction being more rapid, but not as a result of changes in the efficacy of neural transmission along the CST.

Age, Height, and Sex on Motor Evoked Potentials: Translational Data From a Large Italian Cohort in a Clinical Environment.

IntroductionMotor evoked potentials (MEPs) to transcranial magnetic stimulation (TMS) are known to be susceptible to several sources of variability. However, conflicting evidences on individual characteristics in relatively small sample sizes have been reported. We investigated the effect of age, height, and sex on MEPs of the motor cortex and spinal roots in a large cohort.MethodsA total of 587 subjects clinically and neuroradiologically intact were included. MEPs were recorded during mild tonic contraction through a circular coil applied over the “hot spot” of the first dorsal interosseous and tibialis anterior muscles (TAs), bilaterally. Central motor conduction time (CMCT) was estimated as the difference between MEP cortical latency and the peripheral motor conduction time (PMCT) by cervical or lumbar magnetic stimulation. Peak-to-peak MEP amplitude to cortical stimulation and right-to-left difference of each parameter were also measured.ResultsAfter Bonferroni correction, general linear (multiple) regression analysis showed that both MEP cortical latency and PMCT at four limbs positively correlated with age and height. At lower limbs, an independent effect of sex on the same measures was also observed (with females showing smaller values than males). CMCT correlated with both age (negatively) and height (positively) when analyzed by a single regression; however, with a multiple regression analysis this significance disappeared, due to the correction for the multicollinearity within the dataset.ConclusionPhysical individual features need to be considered for a more accurate and meaningful MEPs interpretation. Both in clinical practice and in research setting, patients and controls should be matched for age, height, and sex.

Factors associated with an excellent outcome after conservative treatment for patients with proximal cervical spondylotic amyotrophy using electrophysiological, neurological and radiological findings

Objective: To investigate factors associated with excellent outcomes after conservative treatment in patients with proximal cervical spondylotic amyotrophy (CSA) using electrophysiological, radiological, and neurological findings. Design: Retrospective study. Setting: Yamaguchi University Hospital. Participants: Seventy-seven patients. Interventions: Erb-point-stimulated compound muscle action potentials (CMAP) were recorded in deltoid and biceps. The percentages of CMAP amplitudes on the affected side compared to the normal side in deltoid (PD) and biceps brachii (PB) were calculated. Central motor conduction time was calculated by subtracting peripheral motor conduction time from the onset latency of motor evoked potentials. Cervical lordotic angles, the diameter of C5 canal, and the intervertebral range at C4–C5 and C5–C6 levels were assessed on plain radiographs. Magnetic resonance imaging was used to assess the width of the intervertebral foramen (WIF) at C4–C5 and C5–C6 levels in the axial views. First visit and final follow-up strengths of most atrophic muscles were evaluated using manual muscle testing (MMT). Improvements in strength were classified as excellent (five grades recovered), good (more than one grade recovered), unchanged (no improvement), or poor (worsened). Results: The outcomes were excellent in 27 patients (35.1%), good in 22 (28.6%), unchanged in 26 (33.8%), and poor in two (2.5%). Factors associated with excellent outcomes were WIF at C4–C5 and C5–C6 on the normal side, PB, and CMAP amplitudes of the deltoid on the normal side. Conclusions: Patients with excellent outcomes originally had wide WIF at C4–C5 and C5–C6 levels and the absence of symptomatic spinal cord compression at C3–C4 and C4–C5 levels.

Reduced corticospinal responses in older compared with younger adults during submaximal isometric, shortening, and lengthening contractions.

The aim of this study was to assess differences in motor performance, as well as corticospinal and spinal responses to transcranial magnetic and percutaneous nerve stimulation, respectively, during submaximal isometric, shortening, and lengthening contractions between younger and older adults. Fifteen younger [26 yr (SD 4); 7 women, 8 men] and 14 older [64 yr (SD 3); 5 women, 9 men] adults performed isometric and shortening and lengthening dorsiflexion on an isokinetic dynamometer (5°/s) at 25% and 50% of contraction type-specific maximums. Motor evoked potentials (MEPs) and H reflexes were recorded at anatomical zero. Maximal dorsiflexor torque was greater during lengthening compared with shortening and isometric contractions ( P < 0.001) but was not age dependent ( P = 0.158). However, torque variability was greater in older compared with young adults ( P < 0.001). Background electromyographic (EMG) activity was greater in older compared with younger adults ( P < 0.005) and was contraction type dependent ( P < 0.001). As evoked responses are influenced by both the maximal level of excitation and background EMG activity, the responses were additionally normalized {[MEP/maximum M wave (Mmax)]/root-mean-square EMG activity (RMS) and [H reflex (H)/Mmax]/RMS}. (MEP/Mmax)/RMS and (H/Mmax)/RMS were similar across contraction types but were greater in young compared with older adults ( P < 0.001). Peripheral motor conduction times were prolonged in older adults ( P = 0.003), whereas peripheral sensory conduction times and central motor conduction times were not age dependent ( P ≥ 0.356). These data suggest that age-related changes throughout the central nervous system serve to accommodate contraction type-specific motor control. Moreover, a reduction in corticospinal responses and increased torque variability seem to occur without a significant reduction in maximal torque-producing capacity during older age. NEW & NOTEWORTHY This is the first study to have explored corticospinal and spinal responses with aging during submaximal contractions of different types (isometric, shortening, and lengthening) in lower limb musculature. It is demonstrated that despite preserved maximal torque production capacity corticospinal responses are reduced in older compared with younger adults across contraction types along with increased torque variability during dynamic contractions. This suggests that the age-related corticospinal changes serve to accommodate contraction type-specific motor control.

Diffusion tensor imaging reveals changes in microstructural integrity along compressed nerve roots that correlate with chronic pain symptoms and motor deficiencies in elderly stenosis patients.

Age-related degenerative changes in the lumbar spine frequently result in nerve root compression causing severe pain and disability. Given the increasing incidence of lumbar spinal disorders in the aging population and the discrepancies between the use of current diagnostic imaging tools and clinical symptoms, novel methods of nerve root assessment are needed. We investigated elderly patients with stenosis at L4-L5 or L5-S1 levels. Diffusion tensor imaging (DTI) was used to quantify microstructure in compressed L5 nerve roots and investigate relationships to clinical symptoms and motor neurophysiology. DTI metrics (i.e. FA, MD, AD and RD) were measured at proximal, mid and distal segments along compressed (i.e. L5) and intact (i.e. L4 or S1) nerve roots. FA was significantly reduced in compressed nerve roots and MD, AD and RD were significantly elevated in the most proximal segment of the nerve root studied. FA was significantly correlated with electrophysiological measures of root function: minimum F-wave latency and peripheral motor conduction time (PMCT). In addition, FA along the compressed root also correlated with leg pain and depression score. There was also a relationship between RD and anxiety, leg pain and disability score and AD correlated with depression score. Taken together, these data show that DTI metrics are sensitive to nerve root compression in patients with stenosis as a result of age-related lumbar degeneration. Critically, they show that the changes in microstructural integrity along compressed L5 nerve roots are closely related to a number of clinical symptoms associated with the development of chronic pain as well as neurophysiological assessments of motor function. These inherent relationships between nerve root damage and phenotype suggest that the use DTI is a promising method as a way to stratify treatment selection and predict outcomes.

Motor evoked potentials in patients with chronic whiplash-associated disorder grade II

Background and purposeIt is common belief that psychological problems influence the persistence of complains in patients with so-called mild whiplash-associated disorders (WADs). The usefulness of motor evoked potentials (MEPs) is investigated in patients with grade II WAD and remaining complains for more than 6 months. Patients and methodsTwenty consecutive patients, aged between 24 and 58 years, with persistent neck pain for months after a car accident were included. All patients had a magnetic resonance imaging (MRI) of the cervical spine and cord. Central (CMCT) and peripheral motor conduction times (PMCT) were evaluated by registration in the biceps brachii muscle (C5–C6) and in the abductor digiti minimi muscle (C7–C8–Th1). ResultsThirteen patients had prolonged CMCT or/and PMCT compared to 7 with normal values. On MRI discus bulging C5–C6, without abnormal signal changes in the cervical spinal cord was observed in 6 of the patients with disturbed MEPs compared to 3 without. Out of 7 patients, who had repeated MEPs after 6 months, 3 of them had an improvement of their conduction time. The patients with prolonged MEP conduction times were older than those with normal values (p=0.007). ConclusionsMEP examination has to be performed in all patients with persistent complains even in the absence of objective neurological signs and non-significant changes on imaging.

Cauda Equina Conduction Time Determined by F-Waves in Normal Subjects and Patients With Neurogenic Intermittent Claudication Caused by Lumbar Spinal Stenosis.

Lumbar spinal stenosis typically presents with neurogenic intermittent claudication. The aim of this study was to investigate cauda equina conduction time (CECT) in patients with neurogenic intermittent claudication caused by lumbar spinal stenosis and its relationship with age and body height in normal subjects. The study included 172 normal subjects (group C) (mean age 44.1 ± 16.6 years; mean height 163.7 ± 8.9 cm). Forty-seven patients (group L) (mean age 71.3 ± 8.7 years; mean height 158.8 ± 11.2 cm) underwent surgery because of neurogenic intermittent claudication in cauda equina type of lumbar spinal stenosis. Motor-evoked potentials from the abductor hallucis were recorded. Magnetic stimulation was delivered at the S1 spinous process. Compound muscle action potentials (CMAPs) and F-waves were also recorded after supramaximal electric stimulation of tibial nerves. The peripheral motor conduction time (PMCT) was calculated from the latencies of CMAPs and F-waves as follows: (CMAPs + F-waves - 1)/2. The CECT was calculated by subtracting the onset latency of the motor-evoked potentials from PMCT. The mean values for F-wave latencies, motor-evoked potential latencies, and CECT were 44.5 ± 3.3, 20.6 ± 1.8, and 3.4 ± 0.8 milliseconds, respectively. F-wave and motor-evoked potential latencies showed significant positive linear correlations with age and body height. However, no significant correlation was found between CECT and age (P = 0.43) or body height (P = 0.26). Mean CECT was 5.7 ± 1.5 in group L. There was a significant difference between groups C and L (P < 0.05). The CECT value of normal subjects was 3.4 ± 0.8 milliseconds regardless of age and body height. We suggest that CECT may be a useful factor to consider when evaluating patients with neurogenic intermittent claudication.

Effects of differences in age and body height on normal values of central motor conduction time determined by F-waves

Objectives: To investigate the effect on central motor conduction time (CMCT) based on the relationship between age and height in normal subjects.Design: Retrospective study.Methods: One hundred and ninety nine normal subjects (107 men and 92 women; mean age 39.0 ± 16.4 years; mean height 164.5 ± 8.8 cm) participated in the study. The approximate ages of subjects were as follows: 82 (20–29 years old), 32 (30–39 years old), 32 (40–49 years old), 28 (50–59 years old), and 25 (≧60 years old). The heights of 9, 49, 79, 53, and 9 subjects were <150 cm, 150–160 cm, 160–170 cm, 170–180 cm, and >180 cm, respectively. CMCT- abductor digiti minimi (ADM) and abductor hallucis (AH) were calculated by subtracting the peripheral motor conduction time (PMCT) from the onset latency of motor evoked potentials (MEPs) evoked by transcranial magnetic stimulation. PMCT was calculated from the latencies of the compound muscle action potentials (CMAPs) and F-waves as follows: (latency of CMAPs + latency of F-waves -1)/2.Outcome measures: CMCT-ADM and CMCT-AH.Results: The normative values were 5.2 ± 0.8 ms and 11.8 ± 1.3 ms for CMCT-ADM and CMCT-AH, respectively. CMCT-ADM was not significantly correlated with age (P = 0.196) and body height (P = 0.158). CMCT-AH had significantly positive, linear correlations with age and body height (CMCT-AH = 0.014 × age + 10.971, P = 0.011, R = 0.179 and CMCT-AH = 0.026 × body height + 7.158, P = 0.010, R = 0.182).Conclusions: We suggest normative values of 3.2–7.2 ms in CMCT-ADM for subjects exerting slight effort on ADM regardless age and body height. CMCT-AH had significantly positive, linear correlations with age and body height.

Stimulation of Cauda Equina With a Figure-of-Eight Magnetic Coil.

To record responses in the abductor hallucis muscle after lumbosacral stimulation with a figure-of-eight magnetic coil. To compare magnetic stimulation (MS), electrical stimulation (ES), and the peripheral motor conduction time. M and F waves and compound muscle action potentials were recorded after L1 MS, S1 MS, and L1 ES. The compound muscle action potential latency on L1 MS was 26.9 ± 2.6 milliseconds and on L1 ES was 25.6 ± 2.2 milliseconds, and the peripheral motor conduction time was 27.0 ± 2.5 milliseconds (analysis of variance P = 0.0008). The compound muscle action potential amplitude was 12 ± 11% of M-wave on L1 MS, 32 ± 18% on S1 MS, and 31 ± 17% on L1 ES. Dorsal MS reliably elicits responses in the abductor hallucis muscle. The latency of compound muscle action potentials on L1 MS is comparable to peripheral motor conduction time. A potential drawback of MS may be too weak stimulation at the proximal end of the cauda equina.

Vitamin B12 status does not influence central motor conduction time in asymptomatic elderly people: A transcranial magnetic stimulation study

Introduction: Vitamin B12 deficiency causes neurologic and psychiatric disease, especially in older adults. Subacute combined degeneration is characterized by damage to the posterior and lateral spinal cord affecting the corticospinal tract.Objective: To test corticospinal tract projections using motor evoked potentials (MEPs) by transcranial magnetic stimulation (TMS) in asymptomatic older adults with low vitamin B12 (B12) levels.Methods: Cross-sectional study of 53 healthy older adults (>70 years). MEPs were recorded in the abductor pollicis brevis and tibialis anterior muscles, at rest and during slight tonic contraction. Central motor conduction time (CMCT) was derived from the latency of MEPs and peripheral motor conduction time (PMCT). Neurophysiological variables were analyzed statistically according to B12 status.Results: Median age was 74.3 ± 3.6 years (58.5% women). Twenty-six out of the 53 subjects had low vitamin B12 levels (B12 < 221 pmol/l). MEPs were recorded for all subjects in upper and lower extremities. There were no significant differences in either latency or amplitude of MEPs and CMCT between low and normal B12 groups. There was a significant PMCT delay in the lower extremities in the low B12 group (p = 0.014).Conclusions: No subclinical abnormality of the corticospinal tract is detected in asymptomatic B12-deficient older adults. The peripheral nervous system appears to be more vulnerable to damage attributable to this vitamin deficit. The neurophysiological evaluation of asymptomatic older adults with lower B12 levels should be focused mainly in peripheral nervous system evaluation.

Motor evoked potentials by transcranial magnetic stimulation in healthy elderly people

Introduction: Transcranial magnetic stimulation (TMS) is a non-invasive, safe, and painless method for evaluating the corticospinal pathway. The population of older adults is growing, along with the prevalence of neurological diseases common to this group. Latency and amplitude of motor evoked potentials (MEPs) vary among healthy subjects and no reference normal values for MEPs in healthy older adults are available.Objective: To create a reference value for MEPs by TMS for healthy older adults.Methods: Descriptive study in 36 healthy 70-year-old and older subjects. A 90-mm circular coil Magstim® magnetic stimulator was applied over Cz and Fz. Recording was done in the abductor pollicis brevis and tibialis anterior muscles, at rest and during sustained tonic contraction. Central motor conduction time (CMCT) was derived from MEP latency and peripheral motor conduction time (PMCT). Values were related to age, gender, standing height, and knee height.Results: Mean age was 73.3 ± 2.4 years (58% female). In the upper extremity, average MEP latency was 23.3 ± 1.9 ms at rest and 19.9 ± 1.9 ms during tonic contraction. In the lower extremity, average MEP latency was 30.6 ± 2.5 ms at rest and 27.2 ± 2.3 ms during tonic contraction. There was a significant correlation between MEP latency and standing height, greater in the lower extremities. Female gender appeared as an independent factor determining lower MEP latency, but not CMCT, in upper and lower extremities.Conclusion: We have provided clinically useful reference values for MEPs by TMS in healthy adults older than 70 years of age. As in the younger population, standing height is important in defining normal MEPs. The difference between genders might be due to the lower height of women.

Abnormal Corticospinal Excitability in Patients with Disorders of Consciousness

BackgroundTranscranial magnetic stimulation (TMS) has been frequently used to explore changes in the human motor cortex in different conditions, while the extent of motor cortex reorganization in patients in vegetative state (VS) (now known as unresponsive wakefulness syndrome, UWS) and minimally conscious (MCS) states due to severe brain damage remains largely unknown. Objective/hypothesisIt was hypothesized that cortical motor excitability would be decreased and would correlate to the level of consciousness in patients with disorders of consciousness. MethodsCorticospinal excitability was assessed in 47 patients (24 VS/UWS and 23 MCS) and 14 healthy controls. The test parameters included maximal peak-to-peak M-wave (Mmax), F-wave persistence, peripheral and central motor conduction times, sensory (SEP) and motor evoked (MEP) potential latencies and amplitudes, resting motor threshold (RMT), stimulus/response curves, and short latency afferent inhibition (SAI). TMS measurements were correlated to the level of consciousness (assessed using the Coma Recovery Scale-Revised). ResultsOn average, the patient group had lower Mmax, lower MEP and SEP amplitudes, higher RMTs, narrower stimulus/response curves, and reduced SAI compared to the healthy controls (P < 0.05). The SAI alterations were correlated to the level of consciousness (P < 0.05). ConclusionsThe findings demonstrated the impairment of the cortical inhibitory circuits in patients with disorders of consciousness. Moreover, the significant relationship was found between cortical inhibition and clinical consciousness dysfunction.

Measurement of motor root conduction time at the early stage of Guillain–Barre syndrome

As they are mainly performed at distal nerve parts, routine electrophysiological examinations can fail to detect the abnormalities at the early stage of Guillain-Barre syndrome (GBS) because of predominant involvement of proximal nerve segments. Measurements focused on proximal conduction can provide additional findings. We investigated the diagnostic significance of motor root conduction time (MRCT) at the early stage of GBS. Study was performed in 30 healthy volunteers and within the first week of 12 patients with GBS. MRCT was calculated as the latency differences between motor nerve conduction time (MNCT) obtained by cervical magnetic stimulation and total peripheral motor conduction time obtained by electrical stimulation of ulnar nerve. Also MNCT/MRCT ratio was calculated in each subject. There were statistically significant differences between groups for MRCT (P < 0.0001) and MNCT/MRCT ratio (P < 0.0001). Although the F-wave latency at ulnar nerve was abnormal only in 33%, MRCT was significantly prolonged in 83% of patients. In three patients, prolongation of MRCT was the only abnormality at the first electrodiagnostic examination. Motor root conduction time as a non-invasive method can provide additional and confirmative information at the early stage of GBS in which routine nerve conduction studies may fail to detect the focal demyelination.
