Surgical outcomes of patients with inferior vena cava leiomyosarcoma

IntroductionPrimary vascular leiomyosarcomas are incredibly rare and have a poor prognosis. The purpose of this study was to analyze the surgical outcomes of patients with primary inferior vena cava (IVC) leiomyosarcoma. MethodsWe performed a retrospective review of IVC leiomyosarcoma resections performed at a single tertiary care hospital from 2014 to 2023. A total of 13 cases were analyzed, including 10 women and 3 men. The presenting symptoms, tumor characteristics, operative management, postoperative complications, and survival rates were assessed for each patient. ResultsThe median patient age was 59 years (quartile [Q]1, 52 years; Q3, 68 years). The median tumor size was 7.0 cm (Q1, 6 cm; Q3, 12 cm). The median mitotic rate was 6 per 10 high-power fields (Q1, 2.5; Q3, 15.5). All 13 patients underwent grossly negative tumor resection, with 9 (69%) having microscopically negative margins (R0). No patient had lymph node involvement. The IVCs were managed with ligation in four patients for tumors already occluding the IVC and bovine pericardial patch angioplasty in seven patients or primary repair in two patients for patent IVCs. Concomitant right nephrectomy was performed in seven patients. Left renal vein ligation was performed in three patients, but no left nephrectomies were performed. Significant postoperative complications included one patient with lower extremity compartment syndrome, two patients with severe leg swelling, and one patient with arm swelling. The 30-day mortality rate was zero. Using the Kaplan-Meier product limit method, disease-specific survival was estimated to be 93%. ConclusionsSurgical resection is a feasible and effective oncologic treatment option for patients with IVC leiomyosarcoma. The IVC can be safely managed by ligation, primary repair, or patch angioplasty, depending on the prior patency of the IVC.

ADAM17: A novel treatment target for aneurysms

PurposeThe mechanisms underlying abdominal aortic aneurysms (AAAs) are still not fully understood, previous researches showed ADAM17 is increased in aneurysm. We hypothesized that inhibiting ADAM17 can decrease AAA formation and progression. Materials and methodsAneurysm models were established in mouses and rats by aortic adventitial CaCl2 incubation and aortic pericardial patch angioplasty respectively. In mouse, control (no treatment) or SA/HA hydrogel loaded with TAPI-1 (ADAM17 inhibitor) were adventitial applied; in rat, control and TAPI-1 coated pericardial patch were used in rat aortic pericardial patch angioplasty. Samples were harvested on day 14 or 30 and analyzed by immunofluorescence. Bioinformatics analysis and immunostaining analysis were carried out to confirm the therapeutic potential of ADAM17 in the human AAA. ResultsADAM17 was highly expressed in mouses, rats and human aneurysms. Adventitial application of SA/HA hydrogel loaded TAPI-1 or TAPI-1 conjugated pericardial patch can decrease AAA formation and progression in mouses and rats, respectively. Bioinformatic analysis showed ADAM17 promotes transformation of M1 macrophages and synthetic vascular smooth muscle cells, together with immunostaining analysis and results from animal models, the therapeutic potential of ADAM17 in the human AAA were confirmed. ConclusionWe showed that local delivery of ADAM17 inhibitor can inhibit aneurysm formation and progression in mouse and rat, these results showed ADAM17 plays an important role in the aneurysm formation and may be a potential treatment target.

Internal Jugular to Atrial Bypass with Spiral Vein Graft for Superior Vena Cava Syndrome

The patient was a 41-year-old woman with a history of postpartum class New York Heart Association class II cardiomyopathy. She had undergone biventricular implanTable cardiac defibrillator lead extraction. The procedure was complicated by laceration of the superior vena cava, innominate artery, and confluence of the left subclavian artery. Emergent repair with full cardiopulmonary bypass with bovine pericardial patch angioplasty was performed. During the course of 2 years, the patient had developed superior vena cava syndrome. She described worsening symptoms of severe headaches, blurred vision, near syncope with bending over, and a choking sensation despite sleeping at a 45° head-up angle. Additionally, the patient had developed moderate tricuspid valve regurgitation, which required repair. We elected to repair this patient with a spiral vein graft, in addition to tricuspid valve annuloplasty. Bilateral great saphenous veins were harvested and wrapped around a 36F chest tube such that a 9-mm × 12-cm spiral vein graft was created (Fig 1). After repair of the tricuspid valve, the right internal jugular vein was exposed, and the graft was tunneled through the thoracic inlet and inside a 12-mm ringed polytetrafluoroethylene graft to provide external support. The graft was then anastomosed from the right internal jugular vein to the right atrium. The patient did well and was discharged 6 days postoperatively. During follow-up, the patient has done very well (Fig 2). An Observational Study of Bovine Pericardial Patch Angioplasty as a surgical option for Infected Aortic Aneurysms

Background: An infected aortic aneurysm (IAA) is a rare and life-threatening disease. The aim of this study is to elucidate mid-term outcomes of IAA, incorporating patch angioplasty with bovine pericardium. Materials and Methods: Between June 2011 and August 2020, eight patients (one woman, seven men) with infected aneurysms of the thoracic and abdominal aorta were treated surgically. Results: There was no surgical/hospital death. There were two late deaths at follow-up. One patient (patient 6) was re-admitted to the hospital with vomiting blood four months after surgery and died despite treatment. Another patient (patient 5) died at his home 12 months after surgery, and the cause of death was unknown. Six patients remained alive with no evidence of recurrent aneurysm formation due to infection at a mean follow-up of 70.3 (range, 13-110) months. Conclusions: BPPA could be one of the surgical options for IAA, especially in patients with severe adhesion around the aneurysm.

A novel technique using long segment patch angioplasty maturation to increase the maturation rate of arteriovenous fistulas

ObjectiveGiven the superior patency of arteriovenous fistulas (AVFs) and the decreased risk of infection compared with arteriovenous grafts, the Kidney Disease Outcomes Quality Initiative guidelines have recommended the fistula-first approach. However, ∼20% to 60% of all fistulas will fail to mature. We have described our experience with a novel technique using bovine pericardial patch angioplasty to increase the rate of AVF maturation. MethodsWe used 2-cm × 9-cm-long or 2.5-cm × 15-cm-long segment pericardial patch angioplasty to assist in the maturation of AVFs. A single-center, retrospective cohort study was conducted of all patients who had undergone patch angioplasty maturation (PAM) for AVFs that had failed to mature. The outcomes of interest were maturation status and patency, censored by the death and last known follow-up dates. ResultsFrom March 2007 to October 2019, 139 patients had undergone PAM. Follow-up data were available for 137 of the 139 patients (98.6%), with 126 AVFs (92.0%) progressing to maturation. Of the 126 patients with AVFs that had progressed to maturity, the previous hemodialysis (HD) method was known for 88 patients (69.8%). Of these 88 patients, 70 (79.5%) had previously been receiving HD via an HD catheter. Using a Kaplan-Meier estimator censored for death and loss to follow-up, the assisted primary patency rates at 1, 2, and 3 years were 87.3%, 78.1%, and 68.0%, respectively. Of the 137 patients, 69 (54.8%) had required no additional interventions after patch angioplasty. The complications requiring intervention were stenosis (n = 45; 32.8%), thrombosis (n = 10; 7.3%), infection (n = 3; 2.2%), steal syndrome (n = 3; 2.2%), noninfected wound complications (n = 1; 0.8%), and pseudoaneurysm (n = 1; 0.8%). The average interval to intervention after patch angioplasty was 4.56 months. ConclusionsLong-segment bovine pericardial PAM can be performed safely to treat nonmaturing AVFs, with a 92.0% successful maturation rate and patency rates comparable to those for AVFs. PAM should be a consideration for patients with nonmaturing AVFs.

A Novel Technique to Increase the Maturation Rate of Arteriovenous Fistulae

Given the superior patency of arteriovenous (AV) fistulae and the decreased risk of infection compared with AV grafts, the Kidney Disease Outcomes Quality Initiative guidelines recommend the Fistula First approach. Unfortunately, approximately 18% to 53% of all AV fistulae fail to mature. Our objective is to describe a single-center experience with a novel technique using bovine pericardial patch angioplasty to increase the rate of AV fistula maturation.

Long-term outcomes of bovine pericardial patch angioplasty for recurrent stenosis in vascular access: A UK single-centre experience.

Stenosis of an arteriovenous fistula or arteriovenous graft for dialysis is a common problem. Stenosis may lead to a number of problems including failure of the fistula. Treatment of stenosis is commonly with percutaneous angioplasty or surgical patch angioplasty with autologous vein or synthetic graft or patch. Here we report the use of bovine pericardial patch angioplasty as an alternative mode of patch angioplasty when percutaneous intervention has failed. Patients with fistulae treated with bovine patch angioplasty between 2013 and 2016 were identified from a prospectively maintained renal access database (Cyberren®). Patient records were reviewed, noting type and location of access, patency, re-interventions and complications post bovine pericardial patch. A total of 40 patients were identified as having undergone bovine pericardial patch angioplasty between January 2013 and April 2016. The median time from fistula formation to bovine pericardial patch was 15 months (interquartile range (IQR): 5-43). Median duration of follow-up was 14 months (IQR: 5-18). Primary patency at 6, 12, 18 and 24 months was 91%, 66%, 61% and 54%, respectively. Secondary patency was 94%, 91%, 80% and 77% at the same time points, respectively. In total, 15 patients required 23 re-interventions post bovine pericardial patch. There has been no report of infection or aneurysm formation. Bovine pericardial patch angioplasty for recurrent stenosis in arteriovenous fistulae for dialysis access provides a robust alternative to other surgical patches with the added advantage of allowing early needling and relative resistance to infection. Rates of restenosis and failure post bovine pericardial patch are low.

Transforming Growth Factor-β1 Inhibits Pseudoaneurysm Formation After Aortic Patch Angioplasty.

Pseudoaneurysms remain a significant complication after vascular procedures. We hypothesized that TGF-β (transforming growth factor-β) signaling plays a mechanistic role in the development of pseudoaneurysms. Rat aortic pericardial patch angioplasty was associated with a high incidence (88%) of pseudoaneurysms at 30 days, with increased smad2 phosphorylation in small pseudoaneurysms but not in large pseudoaneurysms; TGF-β1 receptors were increased in small pseudoaneurysms and preserved in large pseudoaneurysms. Delivery of TGF-β1 via nanoparticles covalently bonded to the patch stimulated smad2 phosphorylation both in vitro and in vivo and significantly decreased pseudoaneurysm formation (6.7%). Inhibition of TGF-β1 signaling with SB431542 decreased smad2 phosphorylation both in vitro and in vivo and significantly induced pseudoaneurysm formation by day 7 (66.7%). Normal healing after aortic patch angioplasty is associated with increased TGF-β1 signaling, and recruitment of smad2 signaling may limit pseudoaneurysm formation; loss of TGF-β1 signaling is associated with the formation of large pseudoaneurysms. Enhancement of TGF-β1 signaling may be a potential mechanism to limit pseudoaneurysm formation after vascular intervention.

Patch Angioplasty in the Rat Aorta or Inferior Vena Cava.

Pericardial patches are commonly used in vascular surgery to close vessels. To facilitate studies of the neointimal hyperplasia that forms on the patch, we developed a rat model of patch angioplasty that can be used in either a vein or an artery, creating a patch venoplasty or a patch arterioplasty, respectively. Technical aspects of this model are discussed. The infra-renal IVC or aorta are dissected and then clamped proximally and distally. A 3 mm venotomy or arteriotomy is performed in the infrarenal inferior vena cava or aorta of 6 to 8 week-old Wistar rats. A bovine pericardial patch (3 mm x 1.5 mm x 0.6 mm) is then used to close the site using a 10-0 nylon suture. Compared to arterial patches, venous patches show increased neointimal thickness on postoperative day 7. This novel model of pericardial patch angioplasty can be used to examine neointimal hyperplasia on vascular biomaterials, as well as to compare the differences between the arterial and venous environments.

Vascular Access Site Injury After Transfemoral Transcatheter Aortic Valve Insertion

Vascular complications associated with transfemoral transcatheter aortic valve insertion (TAVI) are a frequent untoward consequence of arterial sheath insertion. We report the prevalence of anatomic arterial injury with open artery access TAVI. This review consisted of 53 consecutive patients who received transfemoral Sapien 23 or 26 mm TAVI via the 22- or 24-French sheath through open artery access from November 2008. The median age of the patients was 83 years (range, 59 to 94), and there were 31 males (58%). All arteries were examined to determine the optimal point of access and then explored after removal of the sheath to document injury. The accessed artery had a minimal lumen diameter of 8.9 mm (7.2 to 14.7) and a sheath-to-artery ratio of 1.0 (0.6 to 1.2). The point of access was the common femoral artery in 33 patients (62%) and the external iliac artery in 20 (38%). Exploration of the artery demonstrated localized arterial injury in 22 patients (42%). Female gender was predictive of injury (p = 0.03), but arterial access site location, presence of arterial access calcification, sheath-to-artery ratio, and sheath size were not. Techniques of artery repair included primary closure in 34 patients (64%), pericardial patch angioplasty in 15 (28%), and graft interposition in 4 (8%). There was one minor groin wound complication. Open artery access allows for direct examination and optimal access site choice of the vessel during transfemoral TAVI. Localized arterial injury was commonly encountered after 22- and 24-French sheath insertion, but the associated artery repair complication rate was low.

Congenital extensive central venous thrombosis with chylous ascites and chylothoraces

We describe a case of congenital extensive central venous thrombosis presenting as polyhydramnios and massive ascites, requiring amnioreduction prenatally and refractory chylous ascites and chylothoraces postnatally. Echocardiography, computed tomography angiogram (CTA), and magnetic resonance venogram (MRV) were helpful in defining the nature and extent of the lesion. The patient underwent staged procedures of repeated abdominal paracentesis, chest drain insertion, and right internal jugular vein exploration initially. Subsequently, open thromboembolectomy from the upper venous system veins and pericardial patch angioplasty of the right internal jugular and right innominate veins were required and managed by catheter-directed thrombolysis for the residual thrombosis with successful recovery.

Pericardial Patch Angioplasty Heals via an Ephrin-B2 and CD34 Positive Cell Mediated Mechanism

ObjectivePericardial patches are commonly used in vascular surgery to close arteriotomies. The mechanism of early healing after patch implantation is still not well defined. We used a rat aortic patch model to assess pericardial patch healing and examined Ephrin-B2, a marker of arterial identity, expression within the post-implantation patch. We also determined whether endothelial progenitor cells (EPC) are associated with early patch healing in the arterial environment.MethodsWistar rats (200–250 grams) underwent infrarenal aortic arteriotomy and then closure via bovine or porcine pericardial patch angioplasty. Control groups included subcutaneously implanted patches. Patches were harvested at 0–30 days and analyzed by histology, immunohistochemistry, immunofluorescence and Western blot as well as quantitative PCR.ResultsPrior to implantation, pericardial patches are largely composed of collagen and are acellular. Following arterial implantation, increasing numbers of CD68-positive cells as well as Ephrin-B2 and CD34 dual-positive cells are found within both bovine and porcine pericardial patches, whereas the infiltrating cells are negative for vWF and α-actin. Porcine patches have a luminal monolayer of cells at day 7, compared to bovine patches that have fewer luminal cells. Subcutaneously implanted patches do not attract Ephrin-B2/CD34-positive cells. By day 30, both bovine and porcine pericardial patches develop a neointima that contains Ephrin-B2, CD34, and VEGFR2-positive cells.ConclusionBoth CD68-positive and Ephrin-B2 and CD34 dual-positive cells infiltrate the pericardial patch early after implantation. Arteriotomy closure via pericardial patch angioplasty shows patch adaptation to the arterial environment that may involve a foreign body response as well as localization of EPC. Arterial remodeling of pericardial patches support endothelialization and may represent a paradigm of healing of scaffolds used for tissue engineering.

Intermediate-term outcome of carotid endarterectomy with bovine pericardial patch closure compared with Dacron patch and primary closure

Multiple studies have established that patch angioplasty following carotid endarterectomy (CEA) reduces the risk of subsequent stroke and restenosis compared with primary closure. Previous reports have also demonstrated bovine pericardium to be associated with similar rates of postoperative complications and restenosis compared with other patch materials. Due to favorable handling and sonographic properties, bovine pericardium has become increasingly popular as a patch option in recent years. However, the intermediate- and long-term performance of this material remains incompletely defined. Through a retrospective analysis of our carotid endarterectomy experience, we sought to compare the bleeding, infection, and pseudoaneurysm rates with bovine pericardium patch closure to those with Dacron patch and primary closure. In this study, 1331 primary carotid endarterectomies performed in our institution between 1996 and 2008 were grouped according to the method of arteriotomy closure: primary closure (PC) (216, 16.3%), Dacron patch angioplasty (DPA) (642, 48.2%), and bovine pericardial patch angioplasty (BPA) (457, 34.3%). Demographic variable and postoperative outcome measures collected real-time via a designated database manager were assessed by univariate and multivariate analysis. Mean follow-up for the entire cohort was 46.1 months. There were no statistically significant differences in rates of postoperative wound infection, hematoma, pseudoaneurysm formation, or 30-day stroke or 30-day mortality among the three groups. Combined 30-day stroke and death was significantly lower in the PC cohort (0.5% vs 2.3% DPA vs 2.4% BPA; P = .94, BPA vs DPA; P = .001, BPA vs PC; P = .001, DPA vs PC), while 5-year restenosis after both DPA (2.0% ± 0.6%) and BPA (1.1% ± 0.6%) was significantly lower compared with PC (5.2% ± 1.6%) (P = .03, DPA vs PC; P = .008, BPA vs PC; P = .14, BPA vs DPA). Five-year survival following BPA (77.9% ± 3.6%) was significantly improved compared with PC (66.9% ± 3.5%) and DPA (60.8% ± 2.1%) in univariate analysis (P = .24, DPA vs PC; P = .01, BPA vs PC; P = .03, BPA vs DPA), with statin use (P = .004) and male gender (P = .05) being positive predictors of enhanced survival on multivariate analysis. This single-institution, retrospective review represents the largest reported experience with BPA after CEA to date and is the only report comparing outcomes after BPA to PC or to DPA. Our experience further demonstrates that patch angioplasty is protective against restenosis after CEA compared with PC. Equivalent rates of perioperative bleeding, infection, and pseudoaneurysm formation were seen with each closure strategy in this study.

Results of Carotid Endarterectomy with Pericardial Patch Angioplasty: Rate and Predictors of Restenosis

Routine patch angioplasty after carotid endarterectomy (CEA) is believed to decrease the incidence of recurrent stenosis. The results of autogenous vein, Dacron, and PTFE used as a patch material have been described. Bovine pericardium has more recently been introduced as a patch material. We studied 61 of 73 consecutive patients who underwent isolated CEA with pericardial patch angioplasty to determine the incidence of restenosis and variables associated with restenosis. All patients had intraoperative completion duplex examination performed, and no patient had residual stenosis or anatomic defects at the end of the procedure. All procedures were performed under general anesthesia with the use of a Javid shunt. Mean age was 72.8 +/- 7.8 years, 41% were female, and 62% were asymptomatic. Hypertension was present in 72%, elevated cholesterol in 80%, and history of coronary artery disease in 44%. Recurrent stenosis of >50% was considered to be significant. Our study focuses on 61 of 73 patients who had follow-up duplex ultrasound data available. There were no perioperative neurologic events, reoperations for bleeding, or deaths. Mean duplex follow-up available in 61 patients was 13.1 +/- 5.1 months. Thirty-six patients had 1-15% stenosis, 15 patients had 16-49%, and 10 patients had 50-79%. In the 50-79% group, the mean systolic velocity was 154 +/- 25 cm/sec and the mean end diastolic velocity (EDV) was 36 +/- 16 cm/sec. The highest EDV in the 50-79% group was 56 cm/sec. No patients had stenosis in the 80-99% range. There were no late neurologic events and no late reinterventions. Kaplan-Meier restenosis-free survival at 1 year was 95.6%. Significant univariate predictors of recurrent stenosis of >50% were younger age (68 vs. 74 years, p = 0.04) and presence of preoperative symptomatic disease (35% vs. 5%, p = 0.004). Stepwise multiple logistic regression indicated the most significant predictor of restenosis was the presence of preoperative symptoms (p = 0.008). Stepwise Cox regression analysis also showed preoperative symptomatic status was the only significant factor for restenosis (p = 0.019), with a relative risk of 6.65 and a 95% confidence interval of 1.36-32.4. In conclusion, pericardial patch angioplasty is associated with minimal early adverse events. Restenosis with pericardial patch angioplasty is not uncommon, but high-grade restenosis did not occur in this study. The presence of preoperative symptoms and younger age were the most significant predictors of restenosis.

Treatment of recurrent pulmonary vein stenoses with endovascular stenting and adjuvant oral sirolimus

The increasing use of radiofrequency catheter ablation for the cure of atrial fibrillation has led to iatrogenic pulmonary vein stenosis as a new clinical entity. The optimal diagnostic modality and treatment for pulmonary vein stenosis and restenosis remain unclear. We report the successful treatment of pulmonary vein restenosis following percutaneous balloon angioplasty, and for the first time, following surgical bovine pericardial patch angioplasty, with endovascular stenting and adjuvant oral sirolimus. Both patients remain asymptomatic at 1 year follow-up without evidence of restenosis.

Traumatic coronary artery dissection

A 14-year-old boy sustained blunt chest trauma resulting in dissection of the left main coronary artery, postinfarction left ventricular aneurysm, mitral regurgitation, and tricuspid regurgitation. He underwent pericardial patch angioplasty of the left main coronary artery, left ventricular aneurysmectomy, mitral valvuloplasty, and tricuspid annuloplasty. The patient continues to do well 4 years after operation.

Bovine Pericardial Patch Angioplasty in Carotid Endarterectomy

Patch angioplasty during carotid endarterectomy (CEA) has been shown to reduce the incidence of both early and late complications. Controversy continues, however, over the ideal patch material. Bovine pericardium (Vascu-Guard Biovascular Inc., Saint Paul, MN) offers an attractive alternative to other patch materials because of its handling and suturing characteristics that are similar to that of autogenous material. This study examines the perioperative and midterm results of bovine pericardial patch angioplasty during CEA. We studied 112 patients who underwent 129 CEAs with bovine pericardial patch angioplasty during an 18-month period. Data were collected regarding demographics, operative indications, perioperative complications, and the occurrence of late adverse outcomes based primarily on follow-up arterial duplex studies. Among this group there were 63 male (56%) and 49 female (44%) patients whose mean age was 71.8 +/- 9.1 years. In these patients there was the typical distribution of atherosclerotic risk factors. Seventy-four patients (66%) had symptomatic disease preoperatively and the remaining 38 patients (34%) were asymptomatic. Temporary cranial nerve palsy occurred in three patients (2%). There were no perioperative strokes, acute occlusions, bleeding episodes requiring reoperation, or deaths. The patients were followed up to 54 months postoperatively with a mean follow-up time of 41.7 +/- 4.4 months. During this period two patients (2%) developed three significant restenoses (70-99%). All required reoperation. There were no asymptomatic occlusions, infections, aneurysms, or rupture. These data demonstrate that bovine pericardial patch angioplasty during CEA is associated with a low incidence of both perioperative and midterm adverse outcomes.

Left Main Coronary Reconstruction for Ostial Stenosis with Patent Ductus Arteriosus

A case of congenital ostial stenosis of the left main coronary artery with a large short patent ductus arteriosus is described. Revascularization was performed by autologous pericardial patch angioplasty and the ductus was closed simultaneously with a fabric patch.

Ductus Arteriosus Aneurysm in Infancy

An infant with symptoms of congestive heart failure at 6 months of age was found by cardiac catheterization at the age of 1 year to have a fusiform aneurysm of the ductus arteriosus as well as discrete stenosis of the left pulmonary artery and a secundum atrial septal defect. The aneurysm was resected, and a pericardial patch angioplasty corrected the peripheral pulmonary stenosis. The atrial septal defect was closed in a separate procedure. To our knowledge, this case is one of four ductal aneurysms reported in infants that have been diagnosed before operation and successfully treated. The pathophysiology and literature are reviewed.
