Efficacy assessment of shoulder rehabilitation with Contrex MJ robot in patients with periarthritis humeroscapularis

Objective: To assess the efficacy of shoulder rehabilitation program incorporating exercise with Contrex MJ robot in patients with periarthritis humeroscapularis. Subject and method: We evaluated a total of 62 patients (n = 62) suffered from the non-specific shoulder pain and frozen shoulder, who were admitted to our medical institution between June of 2016 and May of 2017. In these patients, the efficacy outcome measures were improved in Visual Analogue Scale, Range of motion. Result and conclusion: There were improvements of periarthritis humeroscapularis from five to fifteen day treatment period (p=0.04), there were not any cases with complications in our subjects.

THE EFFECT OF ELECTRONIC ACUPUNCTURE COMBINED WITH “QUYEN TY THANG” REMEDY; IN THE TREATMENT OF PERIARTHRITIS HUMEROSCAPULARIS’S TENDINITIS

Background: Periarthritis humeroscapularis is a common disease of soft tissue disoder, around 2% of population. Tendinitis is the most popular, 90% of periarthritis humeroscapularis casese, the main symptoms are pain and limited movement of shoulder joint that affects daily life of the patients. Objectives: 1. Investigating some clinical characteristics of patients periarthritis humeroscapularis. 2. To evaluate the treatment effects of electronic acupuncture combined with “Quyen ty thang” remedy in the treatment of periarthritis humeroscapularis’s tendinitis. Methods: 30 patients were diagnosed as periarthritis humeroscapularis’s tendinitisand treated by electronic acupuncture and “Quyen ty thang” remedy at Thua Thien Hue Traditional Medicine Hospital and Traditional Medicine Department of Hue Central Hospital. The study was designed by the method of prospective study, evaluate the results before and after treatment. Results: The mean of VAS before treatment is 5.67 ± 1.63, after treatment is 0.53 ± 1.14, the rate of no pain is 73.3%, mild pain is 20.0%, moderate pain is 6.7% and no severe pain. The mean of EFA before treatment is 8.43 ± 2.01, after treatment is 15.40 ± 2.01, the rate of patients with good type upward is 93.3%, including excellent is 36.6%, very good is 30.0%, good is 26.7% and no cases with poor results. Conclusions: The treatment of periarthritis humeroscapularis’s tendinitis combined with electronic acupuncture and “Quyen ty thang” remedy is a highly effective method. Platelet-Rich Plasma in the Treatment of “Frozen Shoulder”

Introduction: This paper presents the results of treating the condition known as frozen shoulder with the platelet-rich plasma method (PRP). Frozen shoulder (periarthritis humeroscapularis, adhesive capsulitis) is the third most common condition (after back pain and knee arthrosis) occurring in middle-aged people (40 to 60 people). Aim of the Study: To establish to which extent is platelet-rich plasma efficient in the treatment of painful shoulder syndrome. Patients and Methods: In the period between January 2013 and December 2015, in the HI Hospital for Surgical and Internal Medicine 'S.tetik', 54 female patients with clinical manifestations of a frozen shoulder were treated. The treatment consisted of three PRP administrations at 7-day intervals. The Quick Dash questionnaire was used for the results at the beginning of the treatment, as well as 30 days and 3 months after its completion. A checkup was conducted after a year. Results: In total, we treated 54 female patients whose average age was 52 years (37-72). Pain in the left shoulder was experienced by 37 patients, while 17 of them experienced pain in the right shoulder. The Quick Dash score prior to the PRP administration was 42 (35-52), while after the PRP treatment the score was 18 (13-26) after 30 days and 13 (11-23) after 3 months. Conclusion: By means of our protocol, implying the treatment of frozen shoulder with the platelet rich plasma method, it is possible to significantly reduce subjective difficulties of patients. Also, together with all other therapeutic procedures (analgesia, physical therapy, etc.), it may eventually lead to a complete recovery. Further work on the examination of pathophysiological effects of PRP and monitoring a large number of patients in multiple centers could result in scientifically proven standards for the application of PRP as a method of choice in the treatment of frozen shoulder.

Effect of different needling retaining times in clinical curative effect of acupuncture on the treatment of periarthritis humeroscapularis

Objective To observe the effect of different needling retaining times in clinical curative effect of acupuncture on the treatment of periarthritis humeroscapularis. Methods A total of 60 patients, who met the inclusion criteara, were randomly divided into 2 groups according to random number table method, 30 patients in each group. Both groups were treated with the same acupuncture therapy for 20 times, but the needling retaining times were different as 20 minutes and 40 minutes. The curative effect rates, Visual Analog Scale(VAS)and Constant-Murley Score(CMS)were evaluated. Results After the treatment, the VAS scoresin 40mins group was significantly lower than that in the 20 mins group(2.67 ± 1.03 vs. 3.60 ± 1.48, t=-3.251, P<0.01); the CMSin 40 mins group was significantly higher than the 20 mins group(73.20 ± 10.88 vs. 66.47 ± 12.62, t=-2.199, P<0.05). The curative effect rates of the 40 mins group was 26.7%(8/30), which was significantly higher than 3.3%(1/30)in the 20 mins group(χ2=4.706, P=0.030). The total effective rates in the 40 mins group was 90.0%(27/30), which was significantly higher than 96.7%(29/30)in the 20 mins group(χ2=0.268, P=0.605). Conclusions Acupuncture treatment for the patients with periarthritis humeroscapularis showed that the 40 minutes of needling retaining times had better symptom improvement and restore function effects than 20 minutes, however the total effective rate was no significant difference. Role and Efficacy of TENS versus SWD in the management of periarthritis shoulder

Introduction: Painful shoulder syndrome is also called periarthritis humeroscapularis or PHS syndrome. The shoulder is the most mobile joint, pain and reduced function may be seen because of different inflammatory and degenerative changes in soft tissues of the shoulder. It is most common musculoskeletal disorder after low back pain and cervical pain. Many approaches have been used in the treatment of periarthritis shoulder pain, including local heat, cryotherapy, transcutaneous eletrical nerve stimulation (TENS), short wave therapy (SWD), manipulation under anaethesia, acupuncture and exercise. The purpose of this study is to compare the effectiveness of TENS and SWD in decrease pain and improvement in function in Periarthritis shoulder. Methodology: A total of 40 patients coming in the year 2014 to the outpatient department with a confirmed diagnosis of periarthritis shoulder were included in this study. Two groups of 20 patients each were made. Group A: received SWD and Exercise therapy, whereas Group B received TENS and exercise therapy. Both the group of patients were assessed using the VAS scoring for pain and the Likert’s scale for the functional outcome. Result: Pain improved significantly in both the groups with more pain relief seen in group B as compared to group A. The functionality which was calculated using the Likert’s scale showed significant improvement in both the groups with slightly more increase in the functionality in group A as compared to group B. Discussion: The long term benefits are better achieved with the SWD as compared to the TENS whereas immediate pain relief is achieved with TENS much better than the SWD. Though this cannot be confirmed as firstly the sample size is very small and also a longer follow up may be required to assess long term functional outcome of both the treatment modalities. Conclusion: This study concludes by accepting the alternate hypothesis that is application of TENS treatment along with exercise therapy will improve function and pain relief as compared to SWD therapy. Whereas long term functional result is better with SWD.

Pathogenesis and surgical treatment of "periarthritis humeroscapularis" (author's transl)

The technique according to Dautry takes into special consideration the conflict between the supraspinal tendon and the coraco-acromial ligament. Postoperative follow-up treatment is of particular importance. Follow-up examinations were performed in 84 patients with a follow-up period between one and 14 years. The results were graded "good" to "very good" in 85% of the patients. The poor results are subjected to a special analysis. The relevant indication and contra-indication is shown for each case. The advantages offered by this method are discussed and compared with the other currently employed methods.

Development and prevention of frozen shoulder after acute aneurysm surgery

We conducted a study on periarthritis humeroscapularis or "frozen shoulder," a postoperative complication of aneurysm surgery. The purpose of this study was to seek the cause of this complication and the methods of preventing it in patients who undergo aneurysm surgery. The diagnosis of frozen shoulder was based on the clinical presence of shoulder pain and difficulty in raising arms that developed within 3 months of surgery. Sixty-four patients who underwent aneurysm surgery with no motor deficit were examined and classified into three groups: (1) early surgery (29 patients in the acute stage after subarachnoid hemorrhage); (2) delayed surgery (19 patients in the chronic stage); and (3) elective surgery (16 patients with unruptured aneurysms). The incidence of frozen shoulder was 41% in the early surgery group, 16% in the delayed surgery group, and 13% in the elective surgery group. The highest incidence of frozen shoulder was found to occur in the early surgery group and was attributed to the immobility of their upper extremities during postoperative treatment. Since patients who undergo surgery in the acute stage are often delirious and confused for several days after surgery, their arms are tied down by their sides in order to prevent them from inadvertently removing catheters such as the one for ventricular drainage. It seems that this manner of immobilizing the patient's arms is the cause of the development of frozen shoulder: Our study showed that if each arm was passively raised by turns above the patient's shoulder, the patient was able to maintain the range of motion of the upper arms and was less likely to develop frozen shoulder. Inactivity of the shoulder joints due to immobilizing the upper extremities of patients after acute aneurysm surgery seemed to cause the development of frozen shoulder. The incidence of this complication was greatly reduced by keeping the patient's upper arms raised alternately to maintain their range of motion after acute aneurysm surgery.

Shock waves in orthopedics

Extracorporeal shock waves have revolutionized urological stone treatment. Nowadays shock waves are widely used in orthopedics, too. This article reviews the applications of extracorporeal shock waves on bone and adjacent soft tissue. The osteoneogenetic effect of extracorporeal shock waves has been proven and can be used to treat pseudarthrosis with a success rate of around 75%. Shock waves have a positive effect in tennis and golfer's elbow, calcaneal spur, and the complex called "periarthritis humero-scapularis." The mechanism for this is not yet known, and results from large prospective and randomized studies are still lacking. However, the treatment has been performed many thousands of times. In patients in whom conservative treatment has failed surgery used to be the only choice, but its success rate barely exceeds that of shock wave therapy and surgery can still be done if shock wave therapy fails. Extracorporeal shock waves will have an impact on orthopedics comparable to its effect in urology. Scientific evaluations, professional certifications, quality assurance and reimbursement issues present great challenges.

Diagnostic aspects of beta 2-microglobulin amyloidosis.

Osteoarticular symptoms and signs, such as carpal tunnel syndrome, periarthritis humeroscapularis, synovial and joint inflammation, lead to the clinical diagnosis of beta 2-microglobulin (beta 2-M) amyloidosis. Radiologically, destructive arthropathy, spondylarthropathy and periarticular cystic bone radiolucencies can be demonstrated by plain X-ray and conventional and computed tomography. Magnetic resonance is used to visualize amyloid masses in special locations such as the cervical spine. Joint ultrasonography demonstrating thickening of synoviae and tendons has become a useful non-invasive diagnostic tool, although it is not specific for beta 2-M amyloidosis, and results depend on observer experience. Efforts to develop more specific methods for demonstration of amyloid deposits have led to two scintigraphic techniques based on the injection of radiolabelled proteins which are incorporated into the amyloid tissue. One method uses serum amyloid P component, a non-specific constituent of all types of amyloid. An alternative imaging technique based on radiolabelled beta 2-M as the specific precursor molecule of beta 2-M amyloidosis appears to be a more sensitive diagnostic method. Both tracer techniques have been used so far only in small single-centre clinical studies, and a more widespread application will require the introduction of recombinant material as the protein source for labelling. The 'gold standard' for the definitive diagnosis of beta 2-M amyloidosis, and also for reference in the evaluation of new diagnostic techniques, remains the histomorphological demonstration of beta 2-M amyloid by Congo red staining, green birefringence under polarized light, positive immunostaining with anti-beta 2-M antibodies, and electronoptic visualization of amyloid fibrils in tissue obtained invasively by biopsy, surgery or autopsy.

Inflammatory changes in shoulder arthrography

Arthrography of the shoulder joint in so-called periarthritis humeroscapularis often shows nodular contrast defects, irregular defects in the bursa and contrast-filling of irregular lymphatics; these are characteristic signs of a synovitis. Arthrography can provide an exact diagnosis, not only in suspected post-traumatic rotator cuff, tears, but also in other painful conditions that limit movement. It is superior to CT in demonstrating inflammatory changes.

The so-called "periarthritis humeroscapularis"--possibilities of an operative treatment.

The pathological alterations of the periarticular tissue in the differential diagnostics of so called periarthritis humeroscapularis are demonstrated. Their pathological mechanism is explained briefly. Clinical and radiological methods to differentiate the pathological symptoms are described. It has been tried to define the indication and the exact date for an operative intervention. Those operative treatments are demonstrated which allow the expectation of good results in clinical practice.

The effect of roentgen therapy in osteo-arthritis and periarthritis humeroscapularis.

Summary335 patients with osteoarthritis of the cervical spine, hip and knee or with periarthritis humeroscapularis (without osteoarthritis of the cervical spine) were treated with roentgen radiation. They were divided into three groups receiving a pretext dose (0 R), 450 R skin dose and 750 R skin dose, respectively. There was no difference in therapeutic results between the three groups except among the patients suffering from periarthritis humeroscapularis where the best results were obtained among patients receiving the highest dose (3×250 R).
