Effect of Mirabilite and Rhubarb in Gynecological Laparoscopy

Recently, traditional Chinese medicine (TCM) nursing intervention has been proven to have advantages in the nursing of various diseases. Among these interventions, TCM external treatments are commonly employed in TCM nursing intervention. In this study, we aim to investigate the clinical efficacy of the external application of mirabilite and rhubarb in the perioperative nursing of gynecological laparoscopic surgery. This study included a total of 92 patients who underwent gynecological laparoscopic surgery from January 2022 to January 2023 as the study objects. They were randomly assigned into two groups, namely the conventional (Con) group (n = 46) and the TCM group (n = 46). The Con group received routine perioperative nursing care for laparoscopy, while the TCM group received topical application of rhubarb and mirabilite in conjunction with routine nursing care. We found that the combination of routine nursing and external treatment with mirabilite and rhubarb significantly reduces the ambulation time, gastrointestinal function recovery time, first defecation time, and length of hospital stay for patients undergoing gynecological laparoscopic surgery (P < 0.05). The external application of mirabilite and rhubarb effectively alleviates postoperative pain in patients (P <0.05). Furthermore, there was a significant increase in IL-6 and IL-17 levels after surgery in both groups (P <0.05); However, after receiving external treatment, patients experienced relief from their inflammatory state to some extent (P <0.05). These findings demonstrate that the perioperative nursing method involving the external application of mirabilite and rhubarb can enhance recovery in patients with gynecological diseases after laparoscopic surgery.

Strengthening an OR Nurse Residency Program: Enhancing Education and Achieving Accreditation.

This article discusses the implementation and enhancement of an OR nurse residency program at a health care system in North Carolina. Recognizing the unique challenges and skills required in perioperative nursing, program coordinators aimed to support new nurses transitioning into the OR setting. Key enhancements included incorporating AORN's Periop 101: A Core Curriculum to provide consistent, evidence-based perioperative education and pursuing accreditation through the Commission on Collegiate Nursing Education to ensure the highest standards of excellence. The program also emphasizes simulation training, preceptor guidance, and continuous feedback to increase residents' proficiency and confidence. The integration of Periop 101 has led to successful outcomes, including high pass rates on comprehensive exams and residents achieving the Certified Foundational Perioperative Nurse credential offered by the Competency & Credentialing Institute. This article highlights the importance of structured education, leadership involvement, and continuous improvement to develop competent and skilled OR nurses.

Caring for Patients With Pheochromocytomas in the Perioperative Setting: Earn 1.4 Contact Hours.

Pheochromocytomas are rare, complex catecholamine-secreting tumors of the adrenal medulla. Patients with pheochromocytomas may experience episodes of life-threatening hypertension, and untreated tumors can be fatal. The nonspecific signs and symptoms (eg, headaches, palpitations, sweating) make diagnosing these tumors difficult. A comprehensive patient history, clinical evaluation during episodic hypertension, and biochemical test results can facilitate a definitive diagnosis. The only curative treatment is minimally invasive tumor removal. Because the tumor may recur, patients require follow-up visits for approximately 10years after surgery. Preoperative evaluations can include cardiac and imaging studies and genetic testing. Preoperative preparation involves controlling hypertension and planning for hypotension after tumor removal. Perioperative nurses should be prepared to assist anesthesia professionals with placing invasive hemodynamic monitors and responding to blood pressure changes. An interdisciplinary team can create a guideline or protocol for nurses to follow to enhance outcomes when caring for patients with pheochromocytomas.

Multifaceted implementation strategy to improve the evaluation of penicillin allergies in perioperative patients: a pre-post feasibility implementation study.

The U.S. Centers for Disease Control and Prevention encourages nurses to evaluate penicillin allergies as part of hospital-based antibiotic stewardship programs. We evaluated the feasibility of an implementation strategy to improve nurses' comprehensive documentation of penicillin allergies. We defined feasibility as the uptake and acceptability of documentation procedures. Six-month pre-post feasibility implementation study. Outpatient surgical areas of an academic medical center located in the U.S. The implementation strategy was guided by the Capability, Opportunity, Motivation Model for Behavior Change and included, building an interdisciplinary coalition to iteratively evaluate the implementation effort, educational meetings with surgical prescribers and perioperative nurses, the development and distribution of educational pocket cards, and structured communication messages in the electronic medical record. A total of 426 patients with 487 penicillin allergy records (216 records pre-implementation period, 271 records post-implementation period) were analyzed. Penicillin allergy documentation contained the following information in the pre- versus post-implementation period: symptoms of the reaction (87% vs 87%), timing/years since reaction (8% vs 26%), onset of reaction in relation to taking penicillin (0% vs 21%), how symptoms resolved (0% vs 21%), and penicillin re-exposure (3% vs 21%). Focus groups revealed nurses perceived documentation procedures as highly acceptable. Major drivers of acceptability included the perceived effectiveness of a detailed allergy history and self-efficacy in conducting a detailed allergy history. Nurses perceived the comprehensive documentation of penicillin allergy history intervention as acceptable, and uptake improved following a theory-informed implementation strategy. We offer implementation strategy components to facilitate nurses' engagement in penicillin allergy evaluation.

An extensive review of pre-operative nursing care for high-risk obstetric surgeries

Healthcare professionals have particular difficulties while performing high-risk obstetric procedures, such as hysterectomies for excessive bleeding, cesarean sections in complex pregnancies, and surgery for ectopic pregnancies. In order to guarantee the safety and wellbeing of the mother and the fetus, perioperative nursing is essential. The purpose of this review is to highlight important nurse interventions, possible side effects, and evidence-based procedures that enhance surgical outcomes in high-risk obstetric situations. The results highlight the significance of important nurse interventions, thorough patient evaluations, and multidisciplinary cooperation throughout the perioperative phase.

Relationship of the Perioperative Nurse Work Environment With a Culture of Safety and High Reliability

PurposeThe purpose of this study was to examine dimensions of the perioperative work environment and their associations with the operating room (OR) culture of safety and high-reliability characteristics. DesignA predictive correlational design using a cross-sectional survey was used. MethodsA convenience sample of perioperative nurses (N = 96) was used to evaluate strength of associations of their work environment with OR safety culture and high-reliability organizational culture. In addition, the best predictive models for OR culture of safety and high-reliability characteristics were used. FindingsFindings indicated that the five dimensions of the work environment are significant predictors of a safety culture; however, only Collegial Relationships and Nursing Foundations for Quality of Care significantly predict high reliability. ConclusionsDue to the extraordinary responsibility perioperative nurses hold to maintain safety standards for surgical patients, it is critical to fortify elements of the nurse work environment.

Perioperative Observation and Nursing of a Child with Facial Venous Malformation Treated with Interventional Embolization

<i>Background</i>: Venous malformations (VMs) are a rare disease with an incidence of only about 1/10,000 in children, but VMs can expand as the child grows, appearing more in shallow skin areas such as the neck of the head, can cause discomfort or degradation, and therefore, can seriously affect the physical and mental health of the child. There are numerous treatment options for VMs, and interventional embolosclerotherapy has become the clinically preferred treatment for VMs due to its many advantages. <i>Case Presentation</i>: We summarize the perioperative care experience of a 10-year-old child with severe right facial malformation diagnosed as facial venous malformation (VMs) who underwent interventional embolization during hospitalization. After 14 days of careful treatment and nursing by medical staff, the child was discharged successfully. <i>Discussion</i>: The key points of our perioperative care for this patient include paying attention to the mental health of the child and providing psychological support; strengthening health education and paying attention to the prevention of falls; preoperative preparation; intraoperative care; observation of the condition; pain assessment and care; strengthening discharge guidance; and doing a good job in continuous nursing. <i>Conclusion</i>: For the nursing of the child with facial venous malformations, it is necessary to pay attention not only to their physiological care but also to their psychological care to help them recover as soon as possible.

ABSTRACTMany people experience low back pain caused by degenerative disease of the lumbar spine; this includes spinal stenosis, spondylolisthesis, disc degeneration, and disc herniation. Conservative management of degenerative disease, which includes physical therapy, lifestyle modifications, and medications, is the initial approach. If this approach fails, a surgical procedure may be the next step. Previously, open lumbar surgery would have been the planned procedure; today, minimally invasive spine surgery gives patients another choice. Perioperative nurses should understand how minimally invasive spine surgery differs from the open procedure, the steps involved during a minimally invasive procedure, the benefits of this type of procedure, and nursing considerations for patients undergoing minimally invasive spinal procedures.

From Knowledge Transfer to Capability Development: The Future of PBL + CBL Teaching Method in Operating Room Nursing Education

Concomitant with the advancement of contemporary medical technology, the significance of perioperative nursing has been increasingly accentuated, necessitating elevated standards for the pedagogy of perioperative nursing. Presently, the PBL (problem-based learning) pedagogical approach, when integrated with CBL (case-based learning), has garnered considerable interest. An extensive literature review has been conducted to analyze the application of the PBL-CBL fusion in the education of perioperative nursing. Findings indicate that this integrative teaching methodology not only enhances students’ theoretical knowledge, practical competencies, and collaborative skills but also contributes to the elevation of teaching quality. In conclusion, the PBL-CBL teaching approach holds immense potential for broader application in perioperative nursing education. Nevertheless, it is imperative to continually refine this combined pedagogical strategy to further enhance the caliber of perioperative nursing instruction and to cultivate a greater number of exceptional nursing professionals in the operating room setting.

Newly Graduated Perioperative Nurses' Experiences of Transitioning to Clinical Practice: A Qualitative Explorative Secondary Analysis.

The aim of this secondary analysis was to explore newly graduated perioperative nurses' experiences of transitioning to clinical practice to gain more in-depth knowledge of this phenomenon. A qualitative explorative study of secondary data was used, and this study was reported following the Consolidated Criteria for Reporting Qualitative Research checklist. A secondary analysis of data collected between March 2019 and November 2020 via 16 semi-structured interviews with newly graduated perioperative nurses. The interviews were conducted 3 to 5 months after they graduated from five different educational institutions in Norway and entered clinical practice as perioperative nurses. A three-step phenomenological hermeneutical analysis inspired by Ricoeur's theory was applied: naïve reading, structural analysis and comprehensive understanding. We identified three themes: expectations of competence, facilitation for newly graduated perioperative nurses and the transition from being a student to a newly qualified perioperative nurse. Newly graduated perioperative nurses emphasised the critical role of supportive facilitation, manageable expectations and support from leaders and colleagues, which seemed essential for a successful transition to clinical practice. This study addresses knowledge essential for universities, universities colleges and clinical practice, specifically healthcare organisations, to understand the transition process for newly graduated perioperative nurses. This can support the establishment of improved practices to ensure patient safety via more specific education and mentoring and more accurate expectations of competence. Bibliometric Analysis of Studies on Surgical Smoke: Bibliometric Analysis

PurposeThe aim of this study was to examine the studies on surgical smoke by bibliometric analysis and science mapping methods. DesignBibliometric analysis and science mapping methods study. MethodsData were obtained by searching the SCOPUS database. The keywords “surgical smoke,” “operating room,” “operating room staff,” and “nurse” were used. Due to the ongoing scientific flow to the database, the search was performed on December 25, 2023 as a single day. FindingsAs a result of the research, a total of 51 studies were reached. We found 49 research articles and 2 review publications in the field of nursing and medicine. Most publications related to surgical smoke were published in 2021 (21.5% n: 11), and the journals with the highest number of publications were the Journal of Perioperative Nursing (5.9% n: 3) and AORN Journal (5.9% n: 3). The most common keyword in the studies was “surgical smoke.” "Surgical smoke" was used in common with all other keywords, the most publications on the subject were made in the United States, the most publications were made in English (92% n: 47), and the most citations were made by Barrett and Garber in 2003 with 250 citations (25.4%). ConclusionsThis study provides a comprehensive overview of the current state of research on surgical smoke, highlighting its prevalence in recent literature. The findings underscore the need for continued investigation and awareness among health care professionals. The results obtained are important in terms of understanding the current situation in the relevant literature and will provide resources for new studies to be carried out by nursing and other surgical team members in the future.

Development and Usability Assessment of Virtual Reality- and Haptic Technology-Based Educational Content for Perioperative Nursing Education

Background/Objectives: In perioperative nursing practice, nursing students can engage in direct, in-person clinical experiences in perioperative environments; however, they face limitations due to infection and contamination risks. This study aimed to develop and evaluate educational content for perioperative clinical practice for nursing students using virtual reality (VR) and haptic technology. Methods: The program, based on the Unity Engine, was created through programming and followed the system development lifecycle (SDLC) phases of analysis, design, implementation, and evaluation. This program allows nursing students to engage in perioperative practice using VR and haptic technology, overcoming previous environmental limitations and enhancing practical and immersive experiences through multi-sensory stimuli. Results: Expert evaluations indicated that the developed content was deemed suitable for educational use. Additionally, a usability assessment with 29 nursing students revealed high levels of presence, usability, and satisfaction among the participants. Conclusions: This program can serve as a foundation for future research on VR-based perioperative nursing education.

Guidelines in Practice: Sterile Technique.

Sterile technique is the collective actions taken to reduce potential contaminants in the OR and maintain a sterile field during operative or other invasive procedures. Practicing and applying the principles of sterile technique is a cornerstone of perioperative nursing and providing safe patient care. The recently updated AORN "Guideline for sterile technique" provides perioperative nurses with information on the principles of and processes associated with sterile technique. This article provides an overview of the guideline and discusses recommendations for actions to take before preparing the sterile field, while preparing and maintaining the sterile field, for limiting movement, for surgical wound classification, and for quality. It also includes a scenario describing the implementation of isolation technique (ie, clean closure) for colon procedures. Perioperative nurses should review the guideline in its entirety and apply the recommendations when practicing sterile technique.

Embracing Our Differences: Advancing Diversity, Equity, and Inclusion Through Perioperative Nursing Education.

A focus on diversity, equity, and inclusion (DEI) in a health care system is critical to ensure patient safety and improve the health outcomes of all surgical patients and communities. Some patient populations continue to experience disparities that negatively affect their burden of illness, ability to access quality care, and health-related outcomes. Unconscious (or implicit) bias among clinicians can lead to unintentional discrimination against marginalized groups. Organization and perioperative leaders should recognize the importance of developing a workplace culture committed to DEI and implement strategies to educate and empower clinicians to embrace these initiatives. When developing educational offerings on DEI for personnel, educators should use a framework and a variety of educational strategies, such as listening tours, computer-based learning, simulations, and journal and book clubs, to engage teams focused on optimizing the health and well-being of the diverse surgical populations in their communities.

An observational study on the modulation of anxiety, depression, and adverse event incidence in painless colonoscopy patients through venous access nursing.

To investigate the clinical significance of venous access nursing in modulating anxiety, depression, and adverse event incidence among patients undergoing painless colonoscopy. Sixty patients who underwent painless colonoscopy in our hospital from September 2021 to September 2022 were selected as the control group (CG, receiving routine perioperative nursing). Sixty patients who underwent painless colonoscopy in our hospital from October 2022 to August 2023 were taken as the study group (SG, receiving venous access nursing modulation). After nursing, patients in the SG exhibited lower scores of the Self-Rating Anxiety Scale and the Self-Rating Depression Scale than those in the CG (P < .05). The SG showed significantly lower mean Ottawa Bowel Preparation Score, but exhibited higher adequacy of bowel preparation compared with the CG (P < .05). The colonoscopy insertion time was (7.18 ± 1.02) minutes in the SG and (8.69 ± 1.00) minutes in the CG, and the colonoscopy withdrawal time was (4.66 ± 1.66) minutes in the SG and (5.64 ± 1.06) minutes in the CG, which showed statistically significant differences between the 2 groups (P < .05). The total incidence of adverse events did not have statistical significance between the 2 groups (P > .05). Application of venous access nursing in patients undergoing painless colonoscopy may help alleviate adverse emotions and shorten colonoscopy insertion time and colonoscopy withdrawal time, thus warranting recognition for its safety and efficacy.
