Analysis of risk factors for complications in echocardiography-guided percutaneous intramyocardial septal radiofrequency ablation

BackgroundThe feasibility of percutaneous intramyocardial septal radiofrequency ablation (PIMSRA) for the treatment of hypertrophic obstructive cardiomyopathy (HOCM) has been previously reported. However, limited investigation has been conducted regarding the complications associated with this procedure.ObjectiveThis study aims to analyze the risk factors affecting the occurrence of complications during PIMSRA, such as pericardial effusion, ventricular premature beats, and interventricular septal perforation. In this study, the optimal cut-off values for these risk factors are also explored, and corresponding strategies for prevention are proposed.MethodsA total of 101 patients diagnosed with HOCM who underwent the PIMSRA procedure from 2021 to 2022 were included in this retrospective analysis. Patients were classified into subgroups with or without complications based on procedural records. Univariate and multivariate regression analyses were conducted to identify independent risk factors for complications during the PIMSRA procedure.ResultsThere were 48 patients with complications and 53 patients without complications. The heart rate at the start of the procedure and the maximum left ventricular outflow tract gradient (LVOTG) were independent risk factors related to PIMSRA complications. The optimal cut-off values for predicting complication occurrence were a heart rate > 49 bpm at the start of the procedure (OR: 3.79, 95% CI: 1.64–8.78, p = 0.002) and a maximum LVOTG > 92 mmHg (OR: 2.57, 95% CI: 1.15–5.75, p = 0.022), respectively.ConclusionsThe occurrence of PIMSRA complications is primarily associated with the heart rate at the start of the procedure and the maximum LVOTG. It is recommended to establish a comprehensive control plan to minimize the risk of complications during PIMSRA procedures.

Comparison of clinical efficacy and prognosis between interventional occlusion and surgical treatment for acute myocardial infarction with ventricular septal perforation.

This retrospective study compared the clinical outcomes and prognostic factors in acute myocardial infarction-related patients with inter-ventricular septal perforation (AMI-VSP) who underwent interventional occlu-sion (IO group) or surgical intervention (SI group). Forty-six patients were ana-lyzed based on comprehensive clinical data, laboratory results, and follow-up evaluations. The SI group had higher EuroScore II scores and more patients in Killip class IV. Both treatments showed improved laboratory indices, but the IO group had higher left ventricular ejection fraction and lower levels of specific biomarkers. The postoperative hospital stay and total hospitalization time were shorter in the IO group. Survival rates did not significantly differ between the two groups during the follow-up period. Logistic regression analysis identified a history of coronary heart disease as a significant risk factor affecting progno-sis and survival rates. Both interventional occlusion and surgical intervention proved effective, with IO showing faster recovery and more favorable prognoses, while surgery was preferred for severe cases. Coronary heart disease was a key factor influencing postoperative survival in AMI-VSP patients.

Pros and Cons of Left Bundle Branch Pacing: A Single-Center Experience.

Left bundle branch pacing (LBBP) has recently emerged as a promising alternative modality for conduction system pacing. However, limited real-world data exists on the advantages and complications associated with LBBP. We analyzed the Rush conduction system pacing registry on LBBP to assess the success rates and complications associated with LBBP. All patients with an indication for permanent pacemaker or cardiac resynchronization therapy that underwent LBBP for various reasons from June 2018 to April 2020 were included in the analysis. A total of 57 of 59 patients underwent successful LBBP (success rate 97%). The average follow-up duration was 6.2±5 months. The implanted devices included 38 dual-chamber pacemakers, 17 cardiac resynchronization therapy defibrillators, and 2 cardiac resynchronization therapy pacing systems. The most common reason for performing LBBP was a high His-Bundle pacing threshold (n=23) at implant. The mean LBBP capture threshold at implant was 0.62±0.21 V at 0.4 ms which remained stable during follow-up at 0.65±0.68 V at 0.4ms. In 21 patients with cardiomyopathy, there was a significant improvement in left ventricle ejection fraction from 30±11% to 42±15%. A total of 7 lead-related complications (12.3%) were noted in the follow-up period. Three patients (5.3%) required lead revision during the follow-up period. Interventricular septal perforation occurred (as late sequela) after 2 weeks in one patient. LBBP can be achieved with a high success rate and low capture thresholds. Left ventricular dysfunction improved significantly during follow-up. Lead-related complications were relatively common occurring in 12.3% of initially successful implants. Lead revision was required in 3 (5%) of patients.

Radiological and hemodynamic parameters in patients with suspected ventricular aneurysm and interventricular septal perforation after acute myocardial infarction: A comparison of non-invasive and invasive diagnostic modalities.

Transthoracic echocardiography is a rapid, sensitive and non-invasive technique for diagnosing ventricular septal perforation. Furthermore, left ventricular angiography is generally used for left ventricular aneurysm but right heart catheterization is the gold standard for septal perforation following myocardial infarction. The objectives of the present study were to compare radiological and hemodynamic diagnostic parameters of non-invasive methods with those of right heart catheterization in patients with suspected ventricular aneurysm and interventricular septal perforation after acute myocardial infarction. Data regarding demographics and clinical characteristics, as well as right heart catheterization, echocardiography and angiographic parameters of 199 patients examined within 21 days after myocardial infarction due to suspected ventricular septal defect indicated by persistent colic pain in the pre-cardiac region were collected and analyzed. Coronary angiography identified 149 (75%) patients with single-vessel disease, 42 (21%) patients with two-vessel disease and 8 (4%) patients with triple-vessel disease. Transthoracic color Doppler echocardiography strengthened the diagnostic performance of right heart catheterization regarding segmental motor abnormalities but underestimated the right atrial pressure, systolic pulmonary artery pressure, mean pulmonary artery pressure and pulmonary capillary wedge pressure compared with right heart catheterization (P<0.0001 for all). Overall, there was no procedural complication requiring emergency intervention, no major complications and no conditions resulting in death due to diagnostic modalities. Transthoracic color Doppler echocardiography may strengthen the diagnostic performance of right heart catheterization regarding radiological measurements but underestimated hemodynamic measurements (level of evidence: 3).

Effects of preoperative cardiac rehabilitation nursing intervention for patients with advanced valvular heart disease on cardiac function and quality of life

Objective To explore the feasibility and safety of preoperative cardiac rehabilitation (CR) nursing intervention in patients with severe valvular disease, and to evaluate the effect of preoperative exercise-mediated cardiac rehabilitation nursing on the clinical outcomes of patients after operation. Methods From January to December of 2017, 108 patients with advanced valvular heart disease (AVHD) admitted in Department of Cardiothoracic Surgery, Nanjing Drum Tower Hospital, the Affiliated Hospital of Nanjing University Medical School were selected based on the diagnostic standard for AVHD. The AVHD patients should not be younger than 18 years old, left ventricular ejection fraction (LVEF) less than 40%, and need repair of interventricular septal perforation, resection of ventricular aneurysm and radiofrequency ablation of surgical atrial fibrillation simultaneously. Random number method was used to divide the patients into control group and cardiac rehabilitation nursing intervention group (CR group) . Mitral and aortic valve lesions were the main types of valvular lesions in the two groups, and the principles of preoperative treatment were the same. Patients in the control group received routine nursing after admission. The patients in the observation group were evaluated by 6 minute walking test (6MWT) after admission. According to the distance of 6MWT, MOTO Med nursing intervention was carried out in gradeⅠ andⅡ. Frequency was once a day, 30 minutes each time, the first movement resistance was 1 kg, and was adjusted appropriately according to the individual situation. Exercise training and aerobic training were carried out for gradeⅢ andⅣ, with emphasis on lower limb muscles, abdominal muscles and ectopectoralis once a day for 20 minutes each time. Results There was no significant difference in hospital stay, ICU stay time and mechanical ventilation time between the two groups (P>0.05) . The LVEF value of CR group was significantly higher than that of control group at the early stage after operation[ (42.5±6.2) vs . (31.0±4.5) , P 0.05) . Conclusions Preoperative cardiac rehabilitation nursing intervention can effectively improve the cardiac function of patients with severe valvular disease early after operation, increase the early exercise tolerance of patients after operation, improve the early self-care ability of patients after operation, and help patients recover better and faster. In the present study, we investigated the clinical usefulness of emergency 2DE in patients with AMI with or without complications, myocarditis, and SBE. The results were as foffows : 1) 2DE was useful in the early diagnosis of subendocardial infarctions associated with bundle brlanch block, where ECG was not useful in detecting AMI. 2) The complications of AMI, such as interventricular septal perforation, free wall rupture, and tend # on rupture, could be easily and promptly be diagnosed by 2DE. 3) 2DE was able to give information about the extent and the degree of the valvular lesions impaired by SBE, which was necessary for surgical treatment. 4) Myocarditis brought about myocardial inflammatory edema and asynergy, we could follow up serial changes throughout by 2DE. Thus, it was concluded that 2DE can play an important role for clinical diagnosis of acute cardiovascular diseases, because the method can be performed easily and noninvasively, and can provide valuable information which cannot be obtained by the other existing methods.

Effectiveness and limitation of the current coronary care unit in treatment of patients with complications following acute myocardial infarction.

For an appraisal of effectiveness and limitation of the current coronary care unit (CCU) in treatment of patients with complications following acute myocardial infarction (MI), consecutive 557 patients hospitalized to our CCU during five years were studied. In order to assess the severity of complicating pump failure, patients were divided into four classes according to Killip's classification. Number of patients, total mortality (%) and mortality due to pump failure (%) were as follows; 333 (6%, 2%) in class I, 118 (17%, 8%) in class II, 42 (19%, 17%) in class III and 55 (84%, 80%) in class IV. Warning arrhythmias, cardiac rupture and interventricular septal perforation complicated in 184 patients, 11 and 10, respectively, and mortality due to these complications was 2%, 91% and 84% in the order. Right ventricular infarction complicated in 54 patients, but only one patient died from right heart failure. Post-MI angina manifested in 156 patients and six of them died postoperatively. Our data indicate that improvement in mortality due to pulmonary edema and arrhythmias and reduction in incidence of cardiac rupture are evident effectiveness of the CCU, and that there is no improvement in survival rate of patients with cardiogenic shock even under the use of newly introduced vasodilator and catecholamine therapy or intraaortic balloon pumping.

Transvenous Pacing, Alternate Bilateral Bundle-Branch Block, and Syncope

The development of permanent transvenous pacing techniques has simplified the treatment of bradycardiac syndromes. Though relatively free of complications, permanent implanted transvenous pacing systems may present a variety of problems after implantation. Complications include perforation of the ventricular myocardium, interventricular septal perforation, dislodgment of the electrode catheter, changing myocardial thresholds, and generator failure. More recently it has been recognized that the pacing catheter may be accidentally positioned in the coronary sinus or its tributaries or both. We recently treated a patient who had recurrent syncope, and alternating right and left bundle-branch block following implantation of a permanent transvenous demand pacemaker. Further study indicated that the pacing catheter had been positioned via the coronary sinus into the posterior coronary vein. The exact incidence of both interventricular septal perforation and ventricular aneurysm subsequent to myocardial infarction is little known. In the reviewed literature there were only six attempts to repair both interventricular septal perforation and ventricular aneurysm. The longest survival was 24 months. Presented here is a case of interventricular septal perforation and ventricular aneurysm complicating myocardial infarction in which the patient has survived for more than two years following surgical correction.

A case of interventricular septal perforation caused by a transvenous catheter electrode is presented. This complication occurred during temporary pacing and was first detected by a change in the pattern of depolarization shown on the electrocardiogram and later confirmed by thoracic roentgenogram and cinefluorography. Predisposing factors are discussed, and the need for electrocardiographic and radiographic monitoring is emphasized.

Acute cor pulmonale in the absence of pulmonary embolism

The functional alterations which occur in acute cor pulmonale have been considered to fall within two groups: 1. 1. Sudden dilatation of the right ventricle with clockwise rotation of the heart along its longitudinal axis. 2. 2. Development of myocardial ischemia, which is the result of a reduction in coronary flow, diminished oxygenation of blood flowing through the lungs, and an increased oxygen requirement of the heart as a result of its increased work. It was shown that some of the electrocardiographic changes occurring in acute cor pulmonale may be explained by dilatation of the right ventricle and rotation of the heart; others are explained on the basis of the myocardial ischemia present. Since the terms acute cor pulmonale and acute pulmonary embolism have frequently been used interchangeably, the purpose of this article was to demonstrate that there may be causes or precipitating factors for acute cor pulmonale, other than pulmonary embolism. Two case reports, with necropsy, are presented in which pulmonary embolism was found absent, but in which massive destructive pulmonary disease with or without major thoracic surgery led to the development of the typical picture of acute cor pulmonale. The term acute cor pulmonale was then extended to include also those conditions in which a sudden left-to-right shunt is produced, as occurs in interventricular septal perforation, or rupture of an aortic aneurysm into the right ventricle. A list of the less frequent etiological conditions are presented which have been known to produce the picture of acute cor pulmonale. They all have in common a mechanism which leads to a sudden marked increase in the resistence against which the right ventricle must work.

The Captain had been in the regular Navy over 25 years. He had had no major illnesses. His blood pressure had been about 140 mm. Hg systolic and 80 mm. diastolic. Close questioning revealed that two years previously the patient had experienced a severe upper epigastric pain relieved by bicarbonate of soda and that two months prior to admission he had many mild similar experiences.
