A novel “proximal first” Inoue balloon catheter for retrograde aortic valvuloplasty: Initial case report

The Inoue balloon, invented for percutaneous transseptal mitral commissurotomy for mitral stenosis, is initially dilated in the distal portion and then the proximal portion, forming an hourglass shape that stabilizes the balloon at the mitral valve orifice with a pulling action. The device has been successfully applied to antegrade aortic valvuloplasty; the hourglass shape stabilizes the balloon across the aortic valve without rapid ventricular pacing. Subsequently, an Inoue balloon was developed for retrograde aortic valvuloplasty using the same design as the antegrade balloon. The hourglass-shaped balloon, however, has difficulty maintaining stability across the aortic valve because the distal portion of the balloon, which inflates first, directly receives systolic pressure from the left ventricle. A novel Inoue balloon was invented to overcome this issue by altering the manner of inflation: the proximal portion inflates first, followed by the distal portion. Theoretically, the proximal portion, which initially inflates just above the aortic valve, remains secure across the valve by applying a pressing force that counteracts the systolic forward pressure. Furthermore, a radiopaque marker is placed at the center of the balloon to enable rapid and precise balloon adjustment. We present a case involving a 65-year-old female hemodialysis patient with severe symptomatic aortic stenosis who we successfully treated with balloon aortic valvuloplasty using the novel Inoue balloon followed by transcatheter aortic valve implantation. The present case demonstrates the feasibility and effectiveness of the novel "proximal first" Inoue balloon for retrograde balloon aortic valvuloplasty for severe aortic stenosis.

Averted Catastrophic Transseptal Puncture: Percutaneous Transseptal Mitral Commissurotomy in a Patient With Undiagnosed Azygous Continuation of the Inferior Vena Cava

A case is presented of a patient requiring percutaneous mitral balloon commissurotomy. During positioning of the transseptal needle, an unusual needle position on the lateral projection prompted the physician to abandon the puncture and do an angiogram of the inferior vena cava. This revealed an azygous vein continuation. The valve was treated at a later date under general anesthesia with transesophageal echo guidance. This case illustrates how reliance on the posterior-anterior projection to find the optimal point for transseptal puncture can be very misleading and how crucial the lateral projection is.

Frequency of atrial septal defect in patients of rheumatic mitral stenosis undergoing percutaneous transseptal mitral commisurotomy.

Objectives: To determine the frequency of atrial septal defect in patients of rheumatic mitral stenosis undergoing percutaneous trans septal mitral commissurotomy. Study Design: Cross sectional study. Setting: Faisalabad Institute of Cardiology, Faisalabad. Period: 15-07-2018 to 14-01-2019. Material & Methods: Study was approved by hospital ethical committee. A total of 45 patients were included in the study that fulfilled the study inclusion criteria and gave written informed consent. PTMC was performed using Inoue balloon technique through right femoral venous route under local anesthesia. Transthoracic echocardiography was done after 48 hours of PTMC for ASD assessment. Results: Out of 45 patients there were 17 (37.8%) males and 28 (62.2%) females. The mean age was (29.06±10.85) years. There were 9 (20%) patients with NYHA functional class II, 28(62.2%) with NYHA functional class III and 8(17.8%) with NYHA functional class IV symptoms. ASD was detected in 15 (33.3%) patients after 48 hours of PTMC. Conclusion: In this study frequency of ASD in patients of mitral stenosis undergoing PTMC was 33.3%. Results of this study can help to identify the patients who will be potential candidates for closure of residual ASD.

Cardio-embolic stroke due to valve tissue embolization during Percutaneous Transseptal Mitral Commissurotomy (PTMC)

Percutaneous Transseptal Mitral Commissurotomy (PTMC) has replaced surgical commissurotomy as a treatment of choice in selected patients of rheumatic mitral stenosis. Various randomized trials have shown PTMC to be equal or superior to surgical commissurotomy in terms of hemodynamic improvement as well as long term survival. Systemic embolism is one of the dreaded complications of PTMC, which is reported in 0.5–5% of cases and involves cerebral circulation in 1% of cases. Most of the time, periprocedural embolism during PTMC is caused by the mobilization of preexisting thrombus in the left atrial appendage. We report an unusual case of acute stroke due embolization of mitral valve tissue during PTMC.

Percutaneous transseptal mitral commissurotomy in a patient with absent right superior vena cava and aneurysmally dilated coronary sinus

Percutaneous transseptal mitral commissurotomy (PTMC) is the first line of treatment for selected patients of rheumatic mitral stenosis. Transseptal puncture is the most crucial step for performing a safe and successful PTMC. The distorted cardiac anatomy can lead to technical difficulties and increase the risk of serious complications. Isolated persistent left superior vena cava in a patient with visceroatrial situs solitus is rare. We report a case of successful PTMC in a patient with distorted anatomy due to aneurysmal dilatation of coronary sinus secondary to the persistent left superior vena cava, absent right superior vena cava and bulging interatrial septum.

Bedside Percutaneous Transseptal Mitral Commissurotomy under Sole Transthoracic Echocardiographic Guidance in a Critically Ill Patient

Percutaneous transvenous mitral commissurotomy is an effective and safe alternative to surgical treatment, in selected patients of rheumatic mitral stenosis. It is usually performed under fluoroscopic guidance in the catheterization laboratory. We report the successful performance of emergency mitral commissurotomy by the Inoue balloon at bedside under sole transthoracic echocardiographic guidance in a critically ill patient.
