Background: In this study, we evaluated the use of percutaneous internal jugular vein cannulation for open cardiac surgeries and reported our experience on the cannulation and the procedure. Methods: Between February 2010 and October 2012, 42 patients (23 males, 19 females; mean age 45±21.9 years; range 2-82 years) who were operated through percutaneous internal jugular vein cannulation were retrospectively analyzed. An Edwards® Fem- Flex II percutaneous femoral artery cannula which was designed for arterial procedures was used. Cannulation was performed percutaneously with the Seldinger technique. Sixteen Fr cannulas were used for adults with a body surface area less than 1.7 m2, while 18 Fr cannulas were used for patients adults with a body surface area above 1.7 m2. Minimally invasive cardiac surgery was performed on 25 patients, whereas redo operations were required in 17 patients. Central venous pressure was recorded by cannulae which were advanced through the left internal jugular vein to monitor the adequacy of superior vena cava drainage. Results: No hospital mortality was observed in the study group. Internal jugular venous cannulation was accomplished without a major percutaneous complication in all patients. An additional atrial cannula was inserted following median sternotomy due to displacement of the femoral venous cannula in three patients. There was no significant difference in central vein pressure and vacuum use among the adult patients who were inserted 16 and 18 Fr cannulas. Conclusion: The use of femoral artery cannulas for percutaneous internal jugular vein cannulation is technically easy and can be performed with no serious complications. It also offers adequate superior vena cava drainage and facilitates procedures which require opening of right-sided cardiac structures with peripheral vascular cannulas.

Iatrogenic pseudoaneurysms, particularly of the femoral artery (a common site of pseudoaneurysms because the femoral artery is often accessed for diagnostic procedures), can be treated with external compression and ultrasound-guided compression (UGC) or UGC alone. Successful external compression of a subclavian artery pseudoaneurysm is difficult to accomplish because of the depth of the artery and the overlying position of the clavicle. These factors often limit access to the neck of the pseudoaneurysm and, therefore, make UGC difficult. The lack of noninvasive access to the subclavian artery for compressive techniques often means that the patient will require surgery for the pseudoaneurysm repair (a thoracotomy is typically required to obtain proximal control of the artery). Endovascular approaches have been developed and are increasingly used to treat inaccessible pseudoaneurysms.

Endovascular stented graft repair of a pseudoaneurysm of the subclavian artery caused by percutaneous internal jugular vein cannulation: case report

In high-risk patients endovascular repair of a pseudoaneurysm with a stented graft is a safe and reasonable treatment option that can preclude significant morbidity and shorten hospital stay. We report a case of pseudoaneurysm of the subclavian artery after internal jugular vein cannulation that was treated successfully with an endovascularly inserted, stented graft. The case report highlights the importance of recognizing this unusual but serious complication of percutaneous internal jugular vein catheterization through careful clinical examination, prompt duplex scanning, and arteriography.

To find out whether internal jugular vein cannulation with a soft silastic hemodialysis access catheter causes jugular vein thrombosis, the authors carried out Doppler ultrasound examinations on 96 patients receiving hemodialysis who had undergone 144 separate catheter insertion episodes in 116 veins. Two internal jugular vein thromboses were found in 101 veins that had been the site of percutaneous insertions only. In addition, 5 internal jugular vein thromboses were identified in 15 veins that had been cannulated surgically with the Quinton PermCath. The authors conclude that percutaneous internal jugular vein cannulation for hemodialysis access causes an acceptably low incidence of jugular vein damage. This strengthens the case for preferential use of the internal jugular vein for vascular access in patients with end-stage renal failure, and suggests that percutaneous cannulation is less damaging than surgical insertion.
