Geospatial epidemiology of coronary artery disease treated with percutaneous coronary intervention in Crete, Greece

Coronary artery disease (CAD) constitutes a leading cause of morbidity and mortality worldwide. Percutaneous coronary intervention (PCI) is indicated in a significant proportion of CAD patients, either to improve prognosis or to relieve symptoms not responding to optimal medical therapy. Thus the annual number of patients undergoing PCI in a given geographical area could serve as a surrogate marker of the total CAD burden there. The aim of this study was to analyze the potential, spatial patterns of PCItreated CAD patients in Crete. We evaluated data from all patients subjected to PCI at the island's sole reference centre for cardiac catheterization within a 4-year study period (2013-2016). The analysis focused on regional variations of yearly PCI rates, as well as on the effect of several clinical parameters on the severity of the coronary artery stenosis treated with PCI across Crete. A spatial database within the ArcGIS environment was created and an analysis carried out based on global and local regression using ordinary least squares (OLS) and geographically weighted regression (GWR), respectively. The results revealed significant inter-municipality variation in PCI rates and thus potentially CAD burden, while the degree and direction of correlation between key clinical factors to coronary stenosis severity demonstrated specific geographical patterns. These preliminary results could set the basis for future research, with the ultimate aim to facilitate efficient healthcare strategies planning.

Percutaneous coronary intervention before transcatheter aortic valve implantation: A propensity score matched analysis

BackgroundThe role of percutaneous coronary intervention (PCI) in patients with stable coronary artery disease (CAD) who subsequently undergo transcatheter aortic valve replacement (TAVR) remains uncertain. Therefore, we conducted this study to assess the association of PCI before TAVR with mortality and cardiovascular outcomes. MethodsWe used the TriNetX database (Jan 2012 - Aug 2022) and grouped patients into PCI (3 months or less) before TAVR and no PCI. We performed propensity score matched (PSM) analyses for outcomes at 30 days and 1 year. ResultsOf 17,120 patients undergoing TAVR, 2322 (14 %) had PCI, and 14,798 (86 %) did not have PCI before TAVR. In the PSM cohort (2026 patients in each group), PCI was not associated with lower all-cause mortality at 30 days (HR: 1.25, 95 % CI: 0.82–1.90) or 1 year (HR: 1.02, 95 % CI: 0.83–1.24). Frequency of repeat PCI after TAVR was low in both no PCI vs. PCI (2.4 % vs. 1.2 %) at 1 year; PCI was associated with a lower rate of repeat PCI (HR: 0.49, 95 % CI: 0.30–0.80). Sensitivity analysis revealed an E-value of 3.5 for repeat PCI (E-value for lower CI for HR: 1.81). PCI was not linked to reductions in MI, heart failure exacerbation, all-cause hospitalization, major bleeding, or permanent pacemaker/implantable cardioverter defibrillator. ConclusionThis analysis showed that PCI prior to TAVR was not associated with improvement in all-cause mortality. However, PCI was associated with a reduced rate of repeat PCI at 1 year.

Distal Radial Access for Coronary Procedures in a Large Prospective Multicenter Registry: The KODRA Trial

BackgroundDistal radial access (DRA) as an alternative access route lacks evidence, despite its recent reputation. ObjectivesThe aim of this study was to evaluate the safety and feasibility of DRA on the basis of daily practice. MethodsThe KODRA (Korean Prospective Registry for Evaluating the Safety and Efficacy of Distal Radial Approach) trial was a prospective multicenter registry conducted at 14 hospitals between September 2019 and September 2021. The primary endpoints were the success rates of coronary angiography (CAG) and percutaneous coronary intervention (PCI). The secondary endpoints included successful distal radial artery puncture, access-site crossover, access site–related complications, bleeding events, and predictors of puncture failure. ResultsA total of 4,977 among 5,712 screened patients were recruited after the exclusion of 735 patients. The primary endpoints, the success rates of CAG and PCI via DRA, were 100% and 98.8%, respectively, among successful punctures of the distal radial artery (94.4%). Access-site crossover occurred in 333 patients (6.7%). The rates of distal radial artery occlusion and radial artery occlusion by palpation were 0.8% (36 of 4,340) and 0.8% (33 of 4,340) at 1-month follow-up. DRA-related bleeding events were observed in 3.3% of patients, without serious hematoma. Multilevel logistic regression analysis identified weak pulse (OR: 9.994; 95% CI: 7.252-13.774) and DRA experience <100 cases (OR: 2.187; 95% CI: 1.383-3.456) as predictors of puncture failure. ConclusionsIn this large-scale prospective multicenter registry, DRA demonstrated high success rates of CAG and PCI, with a high rate of puncture success but low rates of distal radial artery occlusion, radial artery occlusion, bleeding events, and procedure-related complications. Comparative Analysis of Angiographic Parameters and Percutaneous Coronary Intervention Outcomes in Diverse Populations in Türkiye.

Due to the conflict in Syria since 2011, more than 3.5 million Syrian citizens reside in Türkiye. Because Syrian patients were underrepresented in previous studies on percutaneous coronary intervention (PCI) outcomes, we aimed to analyze the severity of coronary artery disease and in-hospital outcomes of PCI in this population. We retrospectively analyzed 142 Syrian patients who underwent PCI at our center between June 2020 and October 2022 and compared the data with that of age- and sex-matched Turkish patients (n = 271) who also underwent PCI. We assessed comorbidities, coronary anatomy features, procedural complications, and in-hospital cardiovascular outcomes (Major Adverse Cardiac and Cerebrovascular Events, MACCE). The mean age of the study population was 57 ± 12 years, with 15% being female. Clinical indication and coronary anatomy features did not differ between the groups. However, the Synergy between Percutaneous Coronary Intervention with Taxus and Cardiac Surgery (SYNTAX) score was higher in Syrian patients compared to Turkish patients (16 [11-22] vs. 12 [8-20]; P < 0.001). Complex PCI rates, in-hospital bleeding and contrast nephropathy were similar in both groups. MACCE was comparable between Syrian and Turkish patients (13 [4.8%] vs. 13 [9.2%], P = 0.083). Comparable MACCE rates were observed in both ethnic groups undergoing PCI, despite a higher SYNTAX score in Syrian patients. Future research should investigate whether similar in-hospital MACCE rates are observed in other regions of Türkiye and whether long-term cardiovascular outcomes differ between Turkish and Syrian patients.

Rural cardiac arrest care and outcomes in Texas

IntroductionOut-of-hospital cardiac arrest (OHCA) victims in rural communities have worse outcomes despite higher rates of bystander cardiopulmonary resuscitation (CPR) than urban communities. In this retrospective cohort study we attempt to evaluate selected aspects of the continuum of care, including post-arrest care, for rural OHCA victims, and we investigated factors that could contribute to rural areas having higher rates of bystander CPR. MethodsWe analyzed 2014–2020 Texas Cardiac Arrest Registry to Enhance Survival (TX-CARES) data for adult OHCAs. We linked TX-CARES data to census tract data and stratified OHCAs into urban and rural events. We created a mixed-model logistic regression to compare cardiac arrest characteristics, pre-hospital care, and post-arrest care between rural and urban settings. We adjusted for confounders and modeled census tract as a random intercept. We then compared different regression models evaluating the association between response time and bystander CPR. ResultsWe included 1202 rural and 28,288 urban cardiac arrests. Comparing rural to urban OHCAs, rates of bystander CPR were significantly higher in rural communities (49.6% v 40.6%, aOR 1.3 95% CI 1.1–1.5). The median response time for rural (11.5 min) was longer than urban (7.3 min). The occurrence of an ambulance response time of <10 min was notably less common in rural communities when compared to urban areas (aOR 0.2, 95% CI 0.2–0.2). For post-arrest care the rates of percutaneous coronary intervention (PCI) were higher in rural than urban communities (aOR 1.7, 95% CI 1.01–2.8). The rates of AED and TTM were similar between urban and rural communities. Survival to hospital discharge was significantly lower in rural communities than urban communities (aOR 0.6, 95% CI 0.4–0.7). Although not significant, rural communities had lower rate of survival with a cognitive performance score (CPC) of 1 or 2 (aOR 0.7, 05% CI 0.6–1.003). We identified no association between response time and bystander CPR. ConclusionPatients in rural areas of Texas have lower survival after OHCA compared to patients in urban areas, despite having significantly greater rates of bystander CPR and PCI. We did not find a link between response time and bystander CPR rates.

Percutaneous treatment of left main coronary artery disease - temporal trends from large registry and ten-year follow-up. a single centre experience

Abstract Background Due to a large amount of jeopardized myocardium, patients with significant left main coronary artery (LMCA) stenosis have an increased risk of death. For years coronary artery bypass grafting has been considered as the treatment of choice. Recent years brought evidence that led to the reconsideration of percutaneous coronary intervention (PCI) as a valuable alternative. Purpose The purpose of this study was to assess temporal trends in clinical practice and analyse the very long-term outcomes of LMCA PCI in a large tertiary hospital in Poland. Methods Group of 998 individuals undergoing LMCA PCI was selected from medical records of 44,989 patients hospitalized in years 2008-2021. Data on mortality were obtained from the Centre for Information Technology. Temporal trends were calculated for period 2008-2014 as compared to 2015-2021. The 10-year event rate was calculated with the Kaplan-Meier estimator, univariate (ULR), and multivariate logistic regression (MLR). Results The rate of LMCA PCI increased from 2.21% in 2008 to 6.88% in 2021 (p&amp;lt;0.001). 736 (73.8%) patients were male, the median age was 71 years (interquartile range 16). 409 (41.0%) of patients presented with acute coronary syndrome (ACS), 545 (54.6%) with chronic coronary syndrome (CCS). In the analysed periods concomitant diabetes mellitus (DM) rate increased from 27.44% to 42.5% (p&amp;lt;0.001), use of intravascular imaging (IVI) increased from 22.28% to 42.72% (p&amp;lt;0.001), use of rotational atherectomy increased from 8.36% to 18.94% (p&amp;lt;0.001). The 30-day mortality decreased from 10.31% to 4.23% (p&amp;lt;0.001), and 1-year mortality decreased from 18.66% to 12.05% (p=0.007). In the 10-year follow-up 399 (40.0%) patients died. In ULR, death was associated with femoral access (OR 1.766, 95% CI 1.308-2.386) and absorbed radiation dose (OR for increase in 1 Gy 1.192, 95% CI 1.080-1.304). The provisional stenting yielded better performance than two-stent techniques (OR 0.635, 95% CI 0.491-0.821). In MLR, death at 10 years was associated with age (OR 1.028, 95% CI 1.007-1.050), ACS (OR 1.725, 95% CI 1.134-2.624), atrial fibrillation (OR 1.540, 95% CI 1.002-2.367), lower glomerular filtration rate (OR for decrease in 1 ml/min/1.73 m2 1.011, 95% CI 1.001-1.022), and lower left ventricular ejection fraction (OR for decrease in 1% 1.045, 95% CI 1.029-1.060). Better outcomes were associated with previous revascularization (OR 0.668, 95% CI 0.453-0.986), use of IVI (OR 0.518, 95% CI 0.353-0.759), drug-eluting stents (OR 0.238, 95% CI 0.106-0.536) and bigger balloon diameter used for stent optimization (OR for increase in 1 mm 0.889, 95% CI 0.801-0.986). Conclusions In the analysed period the rate of LMCA PCI, number of complex procedures, and co-morbidities increased, yet the prognosis has improved. The 10-year death rate analysis revealed that advancements in percutaneous device technology and procedural techniques are associated with more favourable outcomes.

Mortality after cardiovascular surgery and percutaneous coronary intervention prior to versus during the COVID-19 pandemic

Abstract Background The COVID-19 pandemic disrupted the usual delivery of health care services globally. However, little is known about its specific impact on cardiovascular procedure rates and outcomes. Purpose To compare rates of isolated coronary artery bypass graft (CABG) surgery, isolated open valve surgery (iValve) and percutaneous coronary intervention (PCI), and 30-day all-cause mortality among patients undergoing these procedures before versus during the COVID-19 pandemic. Methods We undertook a retrospective cohort study of patients aged 18 years and older in Ontario, Canada, comparing procedure and outcome event rates between April 1, 2018 and March 31, 2020 (pre-pandemic) with those between April 1, 2020 and March 31, 2022 (pandemic). Procedures were identified from hospital records, available for all patients hospitalized in the province, and were calculated per 100,000 population. Deaths were identified from the Ontario Health Insurance Plan Registered Persons Database. Multivariable logistic regression models were used to compute mortality rates adjusted for relevant sociodemographic and clinical characteristics. Additionally, we estimated the number of procedures needed to result in one excess death as the difference between the two periods divided by the pre-pandemic rate. Results During the study period, 24,823, 7,768 and 86,549 patients underwent CABG, iValve and PCI respectively. Between the pre-pandemic and pandemic periods, CABG, iValve and PCI rates declined 18.8%, 19.3% and 14.7%, respectively (Figure). Overall, the 30-day mortality rates were 1.9%, 2.4% and 2.6% following CABG, iValve and PCI, respectively. Adjusted 30-day mortality after CABG increased from 1.92 per 100 procedures (95% CI 1.69-2.15) pre-pandemic to 2.25 (2.01-2.49) during the pandemic (p=0.043). Adjusted 30-day mortality also increased among patients receiving iValve [from 2.25 (1.78, 2.71) to 3.29 (2.76, 3.82); p=0.016] and PCI [from 2.41 (2.28, 2.54) to 2.74 (2.60, 2.88); p&amp;lt;0.001]. These differences translate to an estimated excess of one death per 303 CABG surgeries, 96 iValve surgeries and 303 PCIs during the pandemic period. Conclusions In Ontario, Canada, CABG, iValve and PCI rates decreased during the COVID-19 pandemic and had not fully returned to pre-pandemic levels by two years following its initiation. While 30-day mortality rates following cardiovascular procedures significantly increased during the pandemic, the incremental number of additional deaths was small.

Outcomes of drug eluting stents vs. drug eluting balloons. A single center experience

Abstract Introduction Drug eluting stents (DES) are considered the default treatment option in the current era of percutaneous coronary interventions (PCI). Nevertheless, the use of drug eluting balloons (DEB) remains highly relevant, especially in the setting of in stent restenosis (ISR), and de-novo small vessel disease or bifurcation lesions. Material and method Among 17,630 consecutive PCI procedures performed in our tertiary medical center between 2010 and 2021, we identified 16,435 procedures performed using DES and 1,195 using DEB. Endpoints included mortality and major adverse cardiac events [MACE: death, myocardial infarction (MI), target vessel revascularization (TVR), and coronary artery bypass surgery (CABG)] at 1 year. Propensity score matching was used to create a well-balanced cohort. Results and discussion: Mean age was similar (65.7±12.1 vs. 65.8±11.1, p=0.85), but less female patients (16.7% vs. 21.2%, p&amp;lt;0.01) were observed in the DEB group. DEB patients presented with higher rates of diabetes, hypertension, and prior CABG (62.1% vs. 48.1%, 84.9% vs. 73.2% and 18.3% vs. 10.1% respectively, p&amp;lt;0.01 for all), as compared to DES patients. There were also higher rates of PCI for bifurcation lesions and in-stent restenosis (18.3% vs. 15.1% and 56.0% vs. 7.8% respectively, p&amp;lt;0.01 for both) in the DEB group. After propensity and adjustment for confounders, 370 matched pairs were compared. 1-year MACE rates were significantly higher (HR-1.51, 95%CI 1.04-2.18, p=0.03) in the DEB group, as compared to the DES group. This was driven by higher MI (6.8% vs. 1.9%, p&amp;lt;0.01), TVR (20.5% vs. 11.4%, p&amp;lt;0.01) and CABG (12.2% vs. 7.3%, p=0.02) rates in the DEB group. Adjusted rates of 1-year mortality (HR-1.20, 95%CI 0.75-1.67, p=0.45) did not differ between the two groups. Conclusions Treatment with DEB was associated with increased MACE at 1-year, driven by higher rates of MI, TVR and CABG, as compared to DES.Freedom from MACE

Abstract 15025: Impact of Type of Transcatheter Heart Valve and Anatomic Features on Complexity of Percutaneous Coronary Intervention in Patients After Transcatheter Aortic Valve Replacement

Introduction: Approximately half of the patients with severe aortic valve stenosis (AS) undergoing transcatheter aortic valve replacement (TAVR) present with concomitant coronary artery disease (CAD). Percutaneous coronary intervention (PCI) after TAVR is gaining increasing importance. Consequently, we investigated the factors that influence PCI procedures in patients following TAVR. Methods: We included consecutive 633 patients with severe aortic stenosis who underwent TAVR at our institution from 2016-2022. The primary endpoint was the time from puncture to initial coronary angiography. Results: The median age was 84 (81 - 87) years old, 39% were male, and 173 (27.3%) had concomitant coronary artery disease. Of these, 39 (22.2%) were treated conservatively, 22 (12.5%) underwent coronary artery bypass and TAVR, 64 (36.5%) underwent PCI before TAVR, and 46 (26.2%) underwent PCI after TAVR. The success rate of PCI after TAVR was 100% for both types of valves. The median initial angiography time from puncture was 16.2 minutes (10.6 - 29.1), significantly longer in the Self-expandable valve (SEV) group [22.0 minutes (14.1 - 59.2)] compared to the Balloon-expandable (BEV) group [14.9 minutes (8.1 - 21.1)] (p = 0.026). PCI to the right coronary artery [29.0 min (17.2 - 49.5)] took significantly longer than PCI to the left coronary artery [14.0 min (8.0 - 18.1)] (p = 0.001). Conversely, there was no significant association with CT measurements such as aortic angle, annulus area, coronary height, or valve size, and transcatheter heart valve size (p &gt; 0.05). Factors that predicted difficulty in engaging coronary arteries (&gt;30 min from puncture to first contrast) were SEV [odds ratio 6.4, 95% confidence interval (1.2 - 38.2), p = 0.024], RCA [odds ratio 6.0, 95% confidence interval (1.3 - 33.4), p = 0.020]. Conclusions: Despite the high completion rate of PCI after TAVR, the use of SEV and the approach to the right coronary artery can pose challenges.

Abstract 18559: Sex Differences In Acute Myocardial Infarction Related Cardiogenic Shock Across Different Levels Of Care

Background Previous registry data have suggested important sex-based differences in management and clinical outcomes for patients with acute myocardial infarction related cardiogenic shock (AMI-CS). Methods: We classified 11 hospitals in a metropolitan health network as different levels of care (LOC) for CS based on their highest mechanical circulatory support (MCS) capability; L1: Durable LVAD, L1A: ECMO, L2: Impella. We identified all patients ≥ 18 years of age treated for AMI-CS (ICD-10 diagnosis codes) from 2016 to 2022. Sex differences in management and hospital outcomes are reported. Descriptive statistics and logistic regression were performed using PRISM version 9. Results: In our cohort (n=1,956), women were a minority in L1 (30%) and L1A (31%) and L2 (41%) and were older with decreasing LOC, L1: 72y. (61-81), L1A: 74y. (64-82) and L2: 81y. (72-88), p&lt;0.01 for trend.Across LOC, left heart catheterization (LHC) and percutaneous coronary interventions (PCI) were used similarly in women and men, while coronary artery bypass grafting (CABG) in surgical centers (L1 and L1A) and MCS in PCI centers (except L1A) were used more frequently in men (Figure). After adjustment for LOC and other relevant covariates, female sex (aOR: 1.35, 95%CI: 1.09-1.67; p=0.007), age (aOR: 1.04, 95%CI: 1.03-1.05, p&lt;0.001) and admission cardiac arrest (aOR: 5.36, 95%CI: 3.63-8.54, p&lt;0.001) were associated with higher hospital mortality. Conclusion: In a metropolitan health system, despite similar rates of LHC and PCI across different LOC, women with AMI-CS received less CABG and MCS than men and had higher adjusted hospital mortality.

Abstract 406: Texas Out of Hospital Cardiac Arrest Disparities for Rural Patient Care From Statewide Out-of-Hospital Cardiac Arrest Data Registry - The Texas Cares Project

Introduction: Out-of-hospital cardiac (OHCA) arrest victims from rural settings suffer worse outcomes. We evaluated the association between rural setting and post-arrest care and outcomes. Hypothesis: Receiving treatment for OHCA at a rural hospital will be associated with a lower rate of post OHCA care and lower rate of favorable neurologic outcomes (cerebral performance score 1 or 2). Methods: We performed a retrospective analysis of the Texas Cardiac Arrest Registry to Enhance Survival (CARES) from 2014-2021. We included adult OHCAs not listed as DNR. We defined hospital urbanicity as the proportion of OHCAs originating from rural census tracts. We defined hospitals as rural if more than 25% of included OHCAs were rural. We evaluated the association between rural hospital status and patient and prehospital care characteristics - age, race, initial rhythm, response time, bystander CPR - and hospital care characteristics - targeted temperature management (TTM) and percutaneous coronary intervention (PCI) rates. We evaluated the association between rural hospital status and favorable neurological outcomes with logistic regression, adjusted for patient and prehospital factors, hospital care, and census tract median household income. Results: We included 31,569 encounters with OHCA at 161 unique hospitals. The rural hospital group included 1,213 OHCA encounters (3.8%) at 25 hospitals (15.5%). Patient factors between groups were similar other than a higher rate of white patients in the rural group. Rates of angiography (16.1% non rural, 17.1% rural), PCI (8.2%, 8.7%), and TTM (30.6%,34.7%) were not statistically different. Treatment at rural hospitals was not associated with a statistically significant decreased odds of good neurological outcome (aOR 1.02, 95%ci 0.83-1.24). Conclusion: Treatment for OHCA at rural hospitals was not associated with a lower rate of post OHCA care or lower rate of favorable neurologic outcome

Abstract 18822: Disparities in Access and Long-Term Survival in St Elevation Myocardial Infarction Patients the Brazilian Public and Private Health Services- Victim Registry

Introduction: This study aims to evaluate the impact of disparities in access to specialized services and reperfusion therapies on the long-term survival of patients with ST-elevation myocardial infarction (STEMI) treated under the Unified Health System (SUS) and those covered by Private Healthcare Insurance (PHI). Scarce data exist for regions characterized by significant socioeconomic disparities. Methods: The VICTIM registry was utilized to compare disparities among STEMI patients treated under SUS and PHI. The registry involved all Percutaneous Coronary Intervention (PCI) hospitals in Sergipe, the smallest state in Brazil, consisting of one facility for SUS patients and three facilities for private patients. We examined the patient's opportunity to reach a PCI hospital, reperfusion data from 2014 to 2018, and long-term survival through telephone interviews between 2020 and 2021. Long-term outcomes evaluated using the Gehan-Breslow test with Dunn correction, and significance level p&lt;0.05. Results: Out of the 1.082 patients evaluated, 893 (82.53%) received treatment under SUS, while 189 (17.47%) covered by PHI. SUS patients had to travel a distance six times greater from the site of pain to the PCI hospital (58.2 km vs. 8.7 km; p&lt;0.001); had lower rates of reperfusion (49.8% vs. 67.2%; p&lt;0.001) and primary PCI (47.2% vs. 66.5%; p&lt;0.001); and often had to pass through at least one institution before reaching the PCI hospital (76.1% vs. 41%; p&lt;0.001). The time from symptom onset to seeking help was similar for both groups, but the overall time from symptom onset to arrival at the PCI hospital was longer for public service patients (10.3h vs. 5.3h; p&lt;0.001). Fibrinolytic therapy was underused (2.5% vs. 1.1%; p=0.411). Long-term survival results displayed in Figure 1. Conclusions: SUS patients exhibited higher long-term mortality rates, longer travel distances, and reduced access to reperfusion therapies compared to PHI patients, indicating disparities in care.

48-Month Clinical Outcomes and Prognostic Factors in an All-Comers Population with Acute Coronary Syndrome and Chronic Coronary Syndrome Undergoing Percutaneous Coronary Intervention with a Sirolimus-Eluting Stent.

We characterized the performance as well as safety of a second-generation thin-strut sirolimus-eluting stent with a biodegradable polymer, Alex Plus (Balton, Poland), deployed in the acute coronary syndrome (ACS) setting. We enrolled patients who were subjected to percutaneous coronary intervention (PCI) between July 2015 and March 2016 and took into consideration demographics, clinical and laboratory data, and clinical outcomes. We defined the primary endpoint as the 48-month rate of major cardiovascular adverse events (MACE), including cardiac death, myocardial infarction (MI), or target lesion revascularization (TLR). The secondary endpoints were all-cause death, cardiac death, MI, and TLR rates at 12-, 24-, 36-, and 48 months. We enrolled 232 patients in whom 282 stents were implanted, including 88 ACS and 144 chronic coronary syndrome (CCS) patients. The mean age of the ACS population was 67 ± 13 years old, and 32% of it consisted of females. Patients with ACS were characterized by lower rates of arterial hypertension (85.2% vs. 95.8%, p = 0.004), dyslipidemia (67% vs. 81.9%, p = 0.01), prior MI (34.1% vs. 57.6%, p < 0.001), and prior PCI (35.2% vs. 68.8%, p < 0.001). At 48 months, among the ACS patients, the rates of MACE, death, cardiac death, MI, and TLR were 23.9%, 11.4%, 7.9%, 9.1%, and 10.2%, respectively. No stent thrombosis cases were reported. Multivariable Cox regression revealed that the statistically significant MACE predictors were massive calcifications in coronary arteries (HR 9.0, 95% CI 1.75-46.3, p = 0.009), post-dilatation (HR 3.78, 95% CI 1.28-11.2, p = 0.016), prior CABG (HR 6.64, 95% CI 1.62-27.1, p = 0.008), vitamin K antagonist use (HR 5.99, 95% CI 1.29-27.8, p = 0.022), and rivaroxaban use (HR 51.7, 95% CI 4.48-596, p = 0.002). The study findings show that Alex Plus was effective and safe in a contemporary cohort of real-world ACS patients undergoing primary PCI. The outcomes were comparable between the ACS and chronic coronary syndrome patients, with a trend of lower TLR in ACS patients at 4 years.

Temporal trends in percutaneous coronary intervention in Australia: A retrospective analysis from 2000–2021

ObjectiveThe aim of this study was to describe the trend in percutaneous coronary intervention (PCI) with insertion of a stent in Australia from 2000/01 to 2020/21 and investigate trends in same-day versus non-same-day discharge following PCI. A secondary aim was to compare the rate of coronary artery bypass grafting (CABG) with PCI procedures, while a third aim was to compare marked PCI trend changes with the PCI guidelines during the study period. BackgroundPCI with stent deployment is the most common form of interventional treatment for coronary artery disease, and its use has been expanding since 2000. However, there is a lack of descriptive studies of the national trend in Australia. MethodsAll procedures for PCI and CABG were extracted across 21 years (2000/01 to 2020/21) from the Australian Institute of Health and Welfare data. Age-standardized rates were calculated using the Australian standard population as of June 2001. The ratio of PCI to CABG procedures was also calculated. Trends for PCI were stratified by age, gender, and same-day or overnight discharge episodes. Linear regression analysis was done to compare the age-standardized rates across different age categories. Segmented regression analysis was performed to ascertain the change in the age-standardized rates of PCI during the study period. Whether the changepoints in the trend were matched with guideline updates was also assessed. ResultsThere were 751 728 PCI procedures in persons aged 30 years and above between 2000/01 and 2020/21. The age-standardized rate for the study period showed that persons aged 60-74 years had a higher rate of procedures (102.7) compared to persons aged 30-59 years (81.3) and 75 years and older (61.8) (P < 0.001). There were two statistically significant changepoints in the overall trend; 2005/06 and 2013/14, matched with the change in PCI guidelines. Despite the lower number of procedures for same-day discharge episodes, there has been an increasing trend since 2014/15. More than two-thirds of all stenting procedures were the insertion of a single stent. PCI to CABG procedure ratio increased from 0.6 in 2000/01 to 1.8 in 2020/21. ConclusionsThere was a varying trend in the age-standardized rate of PCI with a peak in 2005/06. The trend appears to be stabilizing in the later part of the study period, but the rate for same-day discharge episodes showed an increasing trend after 2014/15. There is consistency with changepoints in the trend and updated PCI guideline recommendations. The ratio of PCI with insertion of a stent to CABG procedure increased substantially across the study period.

Sex Differences in Fractional Flow Reserve– or Intravascular Ultrasound–Guided Percutaneous Coronary Intervention

BackgroundA recent randomized trial reported fractional flow reserve (FFR)-guided percutaneous coronary intervention (PCI) strategy was noninferior to the intracoronary ultrasound (IVUS)-guided PCI strategy with respect to clinical outcomes with fewer revascularizations. ObjectivesThis study sought to investigate the sex differences in treatment and clinical outcomes according to physiology- or imaging-guided PCI strategies. MethodsIn this secondary analysis of the FLAVOUR (Fractional Flow Reserve or Intravascular Ultrasonography to Guide PCI) trial, the impact of sex on procedural characteristics, PCI rate, and outcomes according to different strategies and treatment types (PCI vs deferral of PCI) was analyzed. The primary outcome was target vessel failure (TVF) at 24 months, defined as a composite of cardiac death, target vessel myocardial infarction, and target vessel revascularization. ResultsOf 1,619 patients, 30% were women. Compared with men, women had a smaller minimal lumen area, smaller plaque burden, and higher FFR. They had a lower PCI rate (40.8% vs 47.9%; P = 0.008), which was mainly contributed by FFR guidance. Overall, women showed a lower TVF rate (2.4% vs 4.5%). According to the treatment type, the cumulative incidence of TVF was lower in women than in men among those with the deferral of PCI (1.7% vs 5.2%). However, this trend was not observed in patients who underwent PCI. In both women and men, there were no differences in clinical outcomes between the FFR- and IVUS-guided strategies. ConclusionsIn cases of intermediate stenosis, despite receiving fewer interventions, women had more favorable outcomes than men. The use of FFR led to a lower PCI rate but had a similar prognostic value compared with IVUS in both women and men.

Outcomes of Intravascular Ultrasound-Guided Percutaneous Coronary Intervention Among Patients With Acute Coronary Syndrome

Intravascular ultrasound (IVUS) use in percutaneous coronary intervention (PCI) improves outcomes. However, data on outcomes of IVUS-guided PCI in patients presenting with acute coronary syndrome (ACS) is scarce. Therefore, we sought to study the utilization rate and outcomes of IVUS-guided PCI in patients with ACS. Using the National Readmission database, we identified all patients with ACS who underwent PCI from 2016 to 2019. We used a 1:1 propensity-matched analysis to compare the outcome of patients with ACS who underwent PCI with and without IVUS. In 1,263,997 patients with ACS, 563,521 (44.6%) underwent PCI without IVUS and 40,095 (3.17%) underwent IVUS-guided PCI. A Propensity scored matched comparison of PCI with and without IVUS showed IVUS-guided PCI was associated with a lower risk of in-hospital mortality (odds ratio 0.74, 95% confidence interval 0.64 to 0.85, p <0.01) compared with PCI without IVUS. The utilization of IVUS increased from 2.64% in 2016 to 4.10% in 2019, p <0.001. In conclusion, IVUS-guided PCI is associated with lower in-hospital mortality in patients with ACS, yet the current utilization of IVUS-guided PCI remains low across the United States.

The effect of ethnicity and social deprivation on outcomes after resuscitated out-of-hospital cardiac arrest

Background Out-of-hospital cardiac arrest (OOHCA) is a common cause of morbidity and mortality. There is substantial ethnic variation in OOHCA outcomes, though it is unclear if these discrepancies are due to arrest circumstances or socioeconomic and biological factors. Purpose To investigate the association of ethnicity, social deprivation and co-morbidities with rates of coronary artery disease, subsequent treatment and outcome in resuscitated OOHCA patients with primary cardiac aetiology. Methods 526 patients with resuscitated OOHCA of suspected cardiac aetiology were included in the study registry between 1-May-2012 and 31-December-2020. Ethnicity, social deprivation, arrest circumstances, comorbidities and extent of coronary artery disease (CAD) were measured. The primary endpoint was 12-month mortality. Results Of 526 patients (median age 62.0±21.1, 74.1% male), 78.1% had shockable rhythms, 83.6% were witnessed and 77.6% had bystander cardiopulmonary resuscitation (CPR). 414 patients (78.7%) were White, 35 (6.7%) were Asian, and 77 (14.6%) were Black. Black patients had more co-existent hypertension and cardiomyopathy, but less prior coronary revascularisation compared with White/Asian patients. There were no ethnic differences in location, witnesses, or bystander CPR, but Black patients had more non-shockable rhythms. Admission serum lactate and creatinine levels were higher in Black patients, but troponin levels were lower than in White/Asian patients. Black patients received less immediate coronary angiography (CAG) and percutaneous coronary intervention (PCI), but had lower rates of CAD, with less CAD complexity (lower SYNTAX and Jeopardy scores) than White/Asian patients. Mortality at 12 months was highest amongst Black followed by Asian and then White patients. In Black patients, this excess mortality was driven by higher rates of multi-organ dysfunction but lower cardiac death than White, and in particular Asian patients, amongst whom cardiac death was highest. Stratifying patients by socioeconomic status showed that higher social deprivation was associated with non-shockable and unwitnessed OOHCA but other demographics, arrest circumstances, rates of CAG and PCI were similar and no difference in mortality or cause of death at 12 months was observed. In a multi-variable logistic regression analysis, age, location of OOHCA, witnessed arrest and Black compared to White ethnicity were independent predictors of 12-month mortality, whilst social deprivation was not. Conclusion Black patients have higher mortality after resuscitated OOHCA than White patients due to differing underlying aetiology rather than differences in arrest circumstances or social deprivation. This should be reflected in cardiac arrest pathways, and patients conveyed to cardiac arrest centres should have access to coronary angiography but also advanced 24/7 imaging, renal replacement therapy and other advanced critical care therapies.
