Increasing abdominal circumference in children – does it require diagnosis?

Introduction and purpose: Wilms tumor, also known as nephroblastoma, is the most prevalent malignant tumor of the kidney in the pediatric population. 90% of them are diagnosed in children before the age of 6. This tumor is more common in girls, and usually occurs unilaterally. The purpose of the study is to analyse and present the incidence of the patients with Wilms tumor in University Children’s Hospital in Lublin in comparison to other studies published on PubMed. Material and methods: The study included 34 patients with the Wilms tumor in the past medical history. They were selected among patients who had a computed tomography (CT) scan in the Department of Pediatric Radiology between November 2012 and October 2021. Results: The average diagnostic age of Wilms tumor patients was 3 years and 9 months. The unilateral occurrence of the tumor was more common, also, the presence of a tumor in the left kidney was more frequent than in the right one. Metastases were mainly located in the lungs and liver. The less common sites of metastasis were pleural recesses and paraaortic lymph nodes. Treatment was mostly based on nephrectomy and chemotherapy, and less frequently, on radiotherapy. In the majority of patients, treatment resulted in regression of cancer lesions. Only a few recurrences of the disease were reported. Conclusions: Successful treatment protocols for Wilms tumor (pre-operative chemotherapy and surgery) ensure a high survival rate. It is important to monitor the patient after treatment due to the risk of recurrence. During diagnostics, chest and abdominal CT should be considered because of the high incidence rate of lung and liver metastases.

Integrating human factors engineering into your pediatric radiology practice.

Human factors engineering involves the study and development of methods aimed at enhancing performance, improving safety, and optimizing user satisfaction. The focus of human factors engineering encompasses the design of work environments and an understanding of human mental processes to prevent errors. In this review, we summarize the history, applications, and impacts of human factors engineering on the healthcare field. To illustrate these applications and impacts, we provide several examples of how successful integration of a human factors engineer in our pediatric radiology department has positively impacted various projects. The successful integration of human factors engineering expertise has contributed to projects including improving response times for portable radiography requests, deploying COVID-19 response resources, informing the redesign of scheduling workflows, and implementation of a virtual ergonomics program for remote workers. In sum, the integration of human factors engineering insight into our department has resulted in tangible benefits and has also positioned us as proactive contributors to broader hospital-wide improvements.

Update on imaging recommendations in paediatric uroradiology: the European Society of Paediatric Radiology workgroup session on voiding cystourethrography.

Voiding cystourethrography (VCUG) is a fluoroscopic technique that allows the assessment of the urinary tract, including the urethra, bladder, and-if vesicoureteral reflux (VUR) is present-the ureters and the pelvicalyceal systems. The technique also allows for the assessment of bladder filling and emptying, providing information on anatomical and functional aspects. VCUG is, together with contrast-enhanced voiding urosonography (VUS), still the gold standard test to diagnose VUR and it is one of the most performed fluoroscopic examinations in pediatric radiology departments. VCUG is also considered a follow-up examination after urinary tract surgery, and one of the most sensitive techniques for studying anatomy of the lower genitourinary tract in suspected anatomical malformations. The international reflux study in 1985 published the first reflux-protocol and graded VUR into five classes; over the following years, other papers have been published on this topic. In 2008, the European Society of Paediatric Radiology (ESPR) Uroradiology Task Force published the first proposed VCUG Guidelines with internal scientific society agreement. The purpose of our work is to create a detailed overview of VCUG indications, procedural recommendations, and to provide a structured final report, with the aim of updating the 2008 VCUG paper proposed by the European Society of Paediatric Radiology (ESPR). We have also compared VCUG with contrast-enhanced VUS as an emergent alternative. As a result of this work, the ESPR Urogenital Task Force strongly recommends the use of contrast-enhanced VUS as a non-radiating imaging technique whenever indicated and possible.

Imaging in pediatric traumatology and orthopedics

Conventional or digital radiography is still the basis of imaging diagnostics of the skeletal system in pediatric patients. It is considered the gold standard for diagnosis, treatment selection, and follow-up. In addition, procedures such as ultrasound, computed tomography (CT), magnetic resonance imaging (MRI), and also nuclear medicine techniques can and should be used. It is advantageous to use trained radiology technicians who are familiar with the handling of children in X‑ray diagnostics. If there is no dedicated pediatric radiology department, it is recommended to follow the guidelines from radiology societies (as low as reasonably achievable [ALARA]) and radiation protection commissions. The present article describes how state-of-the-art tools such as dose monitoring systems and software-controlled image processing and also postprocessing can be used. The article provides information on how the various modalities can be optimally used in order to achieve the best result, i.e., diagnosis, with the least possible effort and burden for the child.

Use of CEUS for Imaging Evaluation of Pediatric Peritonsillar Abscess.

Peritonsillar abscess can be diagnosed by B-mode ultrasound and cross-sectional imaging. The latter (with MRI being the modality of first choice in children) is associated with higher effort and risk for pediatric patients due to the administration of X-rays and/or the need of sedation. The purpose of this study is to evaluate whether the introduction of CEUS into the diagnostic algorithm for suspected pediatric peritonsillar abscess is suitable and advantageous. Single-institution retrospective review of data of pediatric patients who were presented to the department of pediatric radiology for sonographic evaluation under the suspicion of peritonsillar abscess. Diagnostic performance of CEUS was evaluated by using surgical exploration or clinical follow-up as the reference standard. 284 children included in the study underwent B-mode ultrasound. Mean age of all patients was 6,23 years. Peritonsillar abscess was the diagnosis in 42 patients. Diagnosis of peritonsillar abscess was made by B-mode ultrasound alone in 13 of 42 patients (31 %). In 17 of 42 patients (40 %), diagnosis was made by a combination of B-mode ultrasound and CEUS. Sensitivity rose from 37 % to 86 % in cases where B-mode ultrasound remained unclear and CEUS was used. Contrast-enhanced ultrasound (CEUS) is suitable and efficient for the diagnosis of peritonsillar abscess in pediatric patients. It increases the sensitivity for the diagnosis of peritonsillar abscess and thereby reduces the need of additional cross-sectional imaging for the pediatric patients.

A "global village": promoting research and careers in the pediatric radiology community through diversity.

There is a severe shortage of pediatric radiologists in the USA and across the globe due to multiple factors. These severe shortages, along with increased clinical demand, growing research costs and limited funding sources place pediatric radiologists, particularly those in academic departments, under increasing time pressure, affecting their ability to maintain research productivity. In this paper, we model a new concept that should help boost the research efforts within the pediatric radiology community, while diversifying the academic workforce through the involvement of international medical graduates (IMGs). We describe the mutual advantages this concept could have on academic pediatric radiology departments and IMGs alike, as well as pose some of the unique challenges that could impact this concept and effective strategies to ensure success.

Human Bone Age Estimation of Carpal Bone X-Ray Using Residual Network with Batch Normalization Classification

Bone age is an index used by pediatric radiology and endocrinology departments worldwide to define skeletal maturity for medical and non-medical purposes. In general, the clinical method for bone age assessment (BAA) is based on examining the visual ossification of individual bones in the left hand and then comparing it with a standard radiographic atlas of the hand. However, this method is highly dependent on the experience and conditions of the forensic expert. This paper proposes a new approach to age estimation of human bone based on the carpal bones in the hand and using a residual network architecture. The classification layer was modified with batch normalization to optimize the training process. Before carrying out the training process, we performed an image augmentation technique to make the dataset more varied. The following augmentation techniques were used: resizing; random affine transformation; horizontal flipping; adjusting brightness, contrast, saturation, and hue; and image inversion. The output is the classification of bone age in the range of 1 to 19 years. The results obtained when using a VGG16 model were an MAE value of 5.19 and an R2 value of 0.56 while using the newly developed ResNeXt50(32x4d) model produced an MAE value of 4.75 and an R2 value of 0.63. The research results indicate that the proposed modification of the residual training model improved classification compared to using the VGG16 model, as indicated by an MAE value of 4.75 and an R2 value of 0.63.

Local reference and achievable dose levels for vascular and enterostomy access procedures in pediatric interventional radiology.

Knowledge of radiation quantities delivered in routine practice is an essential responsibility of a pediatric interventional radiology department. To review radiation indices in frequently performed vascular and enterostomy access procedures at a quaternary pediatric hospital to formulate dosimetric reference levels and achievable levels. A retrospective review of patient demographics, procedure information and quantitative dose metrics over a 2-year period was performed. Dosimetric details forcommon procedures (central line insertions, gastrostomy/gastrojejunostomy insertions and maintenance) were evaluated, correlated with demographic data and stratified acrossfive weight groups (0-5kg, 5-15kg, 15-30kg, 30-50kg, 50-80kg).Achievable (50th percentile) and reference (75thpercentile)levels with confidence intervals were established for each procedure. Within the evaluation period, 3,165 studies satisfied the inclusion criteria. Five were classified as device insertions (peripherally inserted central catheter, n=1,145; port-a-catheter, n=321; central venous line, n=285; gastrostomy-tube [G-tube], n=262, and gastrojejunostomy-tube [GJ-tube], n=66),and two were classified as maintenance procedures (G-tube, n=358, and GJ-tube, n=728, checks, exchanges and reinsertions). Representative reference and achievable levels were calculated for each procedure category and weight group. This work highlights the creation of local reference and achievable levels for common pediatric interventional procedures. These data establish a dosimetric reference to understand the quantity of radiation routinely applied, allowing for improved relative radiation risk assessment and enriched communication to interventionalists, health care providers, parents and patients.

A spectrum of findings on computed tomography in paediatric abdominal and pelvic tumours in a Ghanaian teaching hospital.

To review the Computed Tomography( CT )features of pediatric oncological patients with abdominal and pelvic tumours and correlate these findings with their histopathological diagnosis. This was a retrospective cross-sectional facility-based study. This study was conducted in the Pediatric Oncology Unit and Radiology Department of the Korle Bu Teaching Hospital. Fifty-six pediatric oncology patients with contrast-enhanced abdominal and pelvic CT scans. The abdominal and pelvic CT scans findings, patient biodata, and histopathology reports of oncology patients over four years were reviewed. Simple descriptive statistics using frequency distribution, percentages, means, and standard deviation were used to describe the various variables and presented tables. The four commonest tumours were nephroblastoma, neuroblastoma, lymphoma, and hepatoblastoma. The mean age at diagnosis was 4.8 years, with a slightly higher male predominance. The majority of the tumours were extremely large at presentation. Overall, the CT - histopathology concordance was 79.2%. Abdominal and pelvic CT scans play an important role in the diagnostic workup of pediatric malignancies by ensuring early and accurate diagnosis of these tumours. None declared.

Gender disparity in academic advancement: exploring differences among adult and pediatric radiologists.

Gender imbalance in research output and academic rank in academic radiology is well-documented and long-standing. Less is known regarding this imbalance among pediatric radiologists. To characterize gender differences for academic rank and scholarly productivity of pediatric radiologists relative to adult radiologists. During summer 2021, faculty data for the top 10 U.S. News & World Report ranked adult radiology programs and the top 12 largest pediatric hospital radiology departments were collected. Information regarding self-reported gender, age, years of practice and academic rank was accessed from institutional websites and public provider databases. The h-index and the number of publications were acquired via Scopus. Group comparisons were performed using Mann-Whitney and chi-square tests. Three hundred and sixty-four (160 women) pediatric and 1,170 (468 women) adult radiologists were included. Compared to adult radiologists, there were significantly fewer pediatric radiologists in advanced ranks (associate or full professor) (P = 0.024), driven by differences between male (P = 0.033) but not female radiologists (P = 0.67).Among pediatric radiologists, there was no significant difference in years in practice (P = 0.29) between males and females. There also was no significant difference in academic rank by gender (P = 0.37), different from adult radiology where men outnumber women in advanced ranks (P < 0.001). Male pediatric radiologists displayed higher academic productivity (h-index: 9.0 vs. 7.0; P = 0.01 and number of publications: 31 vs. 18; P = 0.003) than their female colleagues. Academic pediatric radiology seems to have more equitable academic advancement than academic adult radiology. Despite similar time in the workforce, academic output among female pediatric radiologists lags that of their male colleagues.

A Partial Esophageal Duplication in a Two-Year-Old Female-An Incidental Finding.

Non-cardiovascular findings on chest CT angiography in children with congenital heart disease: How important are they?

BackgroundThere are only a few publications about the non-cardiovascular findings in children with congenital heart diseases explored by chest CT angiography. The purpose of our study is to evaluate the prevalence of non-cardiovascular findings on chest CT angiographies in children with congenital heart disease and to raise awareness about their importance among the radiologists.MethodsWe retrospectively reviewed the 272 chest CT angiographies performed in our pediatric radiology department between January 2017 and march 2021 and extracted the data of the 180 patients positive for a congenital heart disease. Then from that pool, we sorted out the non-cardiovascular findings into significant and non-significant in regard of their relevance or not in the patient’s management.ResultsNon-cardiovascular lesions were found in 58% (105/180) of our patients, and 49% (88/180) of them presented at least one significant non-cardiac lesion. Lung and airways abnormalities were found in 41% (74/180) of the cases, representing the majority of the non-cardiovascular findings. Syndromic associations were found in 17% (28/180) of our patients, including 14 cases of heterotaxic syndrome.ConclusionNon-cardiovascular findings are common in children with congenital heart disease. Reporting these associated lesions is a requisite for an optimal therapeutic management of these children. Radiologists should never forget that a Chest CT angiography in children is first and foremost a chest CT.

Safety challenges related to the use of sedation and general anesthesia in pediatric patients undergoing magnetic resonance imaging examinations.

The use of sedation and general anesthesia has facilitated the significant growth of MRI use among children over the last years. While sedation and general anesthesia are considered to be relatively safe, their use poses potential risks in the short term and in the long term. This manuscript reviews the reasons why MRI examinations require sedation and general anesthesia more commonly in the pediatric population, summarizes the safety profile of sedation and general anesthesia, and discusses an amalgam of strategies that can be implemented and can ultimately lead to the optimization of sedation and general anesthesia care within pediatric radiology departments.

Implementation of a Software Distribution Intervention to Improve Workload Balance in an Academic Pediatric Radiology Department.

In our pediatric radiology department, radiographs (XR) are the shared responsibility of the body section and interpreted in addition to modality or site-specific assignments. Given an unequal contribution amongst radiologists to the XR workload, a software solution was developed to distribute radiographs and improve workload balance. Metrics to evaluate the intervention's effectiveness were compared before and after the intervention. Data was retrieved from the radiology analytics platform, scheduling software, and the peer learning database. Metrics were compared 12months pre (March 2018-February 2019) and 6months post (March 2019-August 2019) intervention on non-holiday weekdays, 7 am-5 pm. To evaluate the intervention's effectiveness, variance between radiologists' contributions to XR volume was assessed using Levene's and Fisher's tests. Changes in turnaround times (TATs) and error rates pre- and post-intervention were evaluated as secondary metrics. Following the intervention, the average number of XR interpreted on target rotations increased by 8.9% (p = 0.011) while the departmental volume of radiographs increased only 4.5%. The variance between radiologists' daily XR contribution was 21.3% (p < 0.0001) higher prior to the intervention. Days where target rotations read fewer than 5 XR decreased from 17.8 to 1.1% (p < 0.0001) after the intervention. Days in which more than 75% of all XR had a TAT less than 60min improved from 26.8 to 39.7% (p = 0.017) after the intervention. There was no statistically significant difference in error frequency (error rate 2.49% pre and 2.72% post, p = 0.636). In conclusion, the software intervention improved XR workload contribution with decreased variability. Despite increased volumes, there was an improvement in turnaround times with no effect on error rates.

Evaluation of Bone Age in Children: A Mini-Review.

Bone age represents a common index utilized in pediatric radiology and endocrinology departments worldwide for the definition of skeletal maturity for medical and non-medical purpose. It is defined by the age expressed in years that corresponds to the level of maturation of bones. Although several bones have been studied to better define bone age, the hand and wrist X-rays are the most used images. In fact, the images obtained by hand and wrist X-ray reflect the maturity of different types of bones of the skeletal segment evaluated. This information, associated to the characterization of the shape and changes of bone components configuration, represent an important factor of the biological maturation process of a subject. Bone age may be affected by several factors, including gender, nutrition, as well as metabolic, genetic, and social factors and either acute and chronic pathologies especially hormone alteration. As well several differences can be characterized according to the numerous standardized methods developed over the past decades. Therefore, the complete characterization of the main methods and procedure available and particularly of all their advantages and disadvantages need to be known in order to properly utilized this information for all its medical and non-medical main fields of application.

Imaging sedation and anesthesia practice patterns in pediatric radiology departments - a survey of the Society of Chiefs of Radiology at Children's Hospitals (SCORCH).

There are few data describing practice patterns related to the use of sedation/anesthesia for diagnostic imaging in pediatric radiology departments. To understand current practice patterns related to imaging with sedation/anesthesia in pediatric radiology departments based on a survey of the Society of Chiefs of Radiology at Children's Hospitals (SCORCH) in conjunction with the American College of Radiology's Pediatric Imaging Sedation and Anesthesia Committee. A multi-question survey related to imaging with sedation/anesthesia in pediatric radiology departments was distributed to SCORCH member institutions in January 2019. A single reminder email was sent. Descriptive statistical analyses were performed. Of the 84 pediatric radiology departments, 23 (27%) completed the survey. Fifty-seven percent of the respondents self-identified as academic/university-affiliated and 13% as a division/section in an adult radiology department. Imaging sedation (excluding general anesthesia) is commonly performed by pediatric anesthesiologists (76%) and intensive care unit physicians (intensivists, 48%); only 14% of departments expect their pediatric radiologists to supervise imaging sedation. Ninety-six percent of departments use child life specialists for patient preparation. Seventy percent of departments have preparatory resources available on a website, including simulation videos (26%) and audio clips (17%). Nearly half (48%) of the departments have a mock scanner to aid in patient preparation. Imaging sedation/anesthesia is most often scheduled at the request of ordering clinicians (65%), while 57% of departments allow schedulers to place patients into imaging sedation/anesthesia slots based on specified criteria. Imaging sedation/anesthesia practice patterns vary among pediatric radiology departments, and understanding current approaches can help with standardization and practice improvement.

MRI-Based 3-Dimensional Visualization Workflow for the Preoperative Planning of Nephron-Sparing Surgery in Wilms’ Tumor Surgery: A Pilot Study

Purpose. Due to the size and localization of Wilms’ tumor (WT), nephron-sparing surgery (NSS) is only possible in a limited number of cases. When NSS is considered, the surgeon preoperatively requires a thorough understanding of the patient-specific anatomy to prevent positive surgical margins and other complications. Through a collaboration between the radiology and pediatric surgery departments and 3D imaging specialists, a 3D visualization workflow was developed to improve preoperative planning of NSS for WT patients. Methods. The 3D visualization workflow combines a MRA sequence, a segmentation protocol, and augmented reality (AR) visualization, additional to in-house 3D printing. A noncontrast-enhanced MRA scan was added to the MRI protocol. MRI sequences were segmented with a segmentation protocol in an open-source software package. The resulting 3D models were visualized in AR with a HoloLens and 3D print. Results. In a pilot study, five WT patients eligible for NSS were preoperatively planned through the 3D visualization workflow. AR visualization software was fast and free to use and allowed adequate handling of the 3D holograms. The 3D printed models were considered convenient and practical for intraoperative orientation. The patient-friendly, fast, and low-cost 3D visualization workflow was easily implemented and appeared to be valuable for the preparation of NSS. Conclusion. This pilot study demonstrates how a strong collaboration between the pediatric surgery and radiology departments and 3D imaging specialists will help to shape the future of pediatric oncological surgery. This 3D visualization workflow aims to prepare pediatric oncological surgeons for nephron-sparing surgery in patients with Wilms’ tumors.

Reliability of chest radiograph interpretation for pulmonary tuberculosis in the screening of childhood TB contacts and migrant children in the UK

To determine interobserver agreement between paediatric radiologists interpreting tuberculosis (TB) screening chest radiographs (CXR) in children in the UK, and the TB detection rate. A retrospective review was undertaken of electronic request, notes, and CXRs (>7 years) in children exposed to an infectious case of TB and new entrants to the UK, at a tertiary children's hospital. Included were those with positive Mantoux/interferon gamma release assay (IGRA), positive culture, or high clinical suspicion of TB. CXR reports were categorised as normal, abnormal without features of TB, or abnormal with features of pulmonary TB. Three paediatric radiologists from a dedicated paediatric radiology department evaluated available CXRs, aware of the TB screening indication, using a published CXR reporting tool and blinded to the initial CXR report and to each other. A majority decision was collated, and considered lymphadenopathy and miliary nodules as diagnostic of primary TB. Interobserver agreement was calculated using Cohen's kappa. One hundred and forty-eight children underwent TB screening with a CXR. One hundred and twenty-five had available CXR reports and case notes, which indicated 20/125 (16%) had CXR features of TB. One hundred and twenty-one of the 125 had CXRs available to for panel review. Twenty of these 121 (17%) yielded a majority decision of pulmonary TB. Inter-reader agreement was moderate in all aspects (kappa 0.4-0.6). The high percentage of pulmonary TB on CXR (16% original reports; 17% by panel review) suggests that it is worthwhile investigating childhood TB contacts; however, the routine use and recommendation for CXR is questionable because of only moderate interpretation reliability (kappa 0.5), even by experts.

Perceived Impact of COVID-19 on Pediatric Radiology Departments Around the World: WFPI COVID-19 Task Force Survey Results from 6 Continents.

PurposeTo investigate how COVID-19 has impacted pediatric radiology practice around the world at the present time.Materials and MethodsThis study was based on a survey conducted by the World Federation of Pediatric Imaging (WFPI) COVID-19 task force. Pediatric radiology representatives from countries around the world were identified and asked to complete an electronic survey consisting of 17 questions related to the impact of, concerns surrounding, and education related to COVID-19 on pediatric radiology. For each outcome variable, a pair of 2-way frequency tables was constructed: one against COVID-19 prevalence, and another against region. Tests of independence of the tables’ row and column factors were performed based on Fisher’s exact test and odds ratios (OR) were estimated via logistic regression models corresponding to these 2-way tables.ResultsThe survey (response rate 87%, 72/83) comprised representatives from 71 countries and Hong Kong across 6 continents. 66/72 respondents (92%) indicated that COVID-19 has resulted in moderate (21/72, 29%), significant (36/72, 50%), or complete (9/72, 13%) change in radiology departments/practices in their countries. The two most frequent concerns over the next four months were personal/family health (54/72, 75%) and exposure (48/72, 67%). 79% (57/72) of respondents indicated some level of discomfort in identifying pediatric COVID-19 imaging manifestations. Changes in resident education were reported by 68/72 (94%) respondents, and 22/72 (31%) were concerned that the likelihood of current trainees pursuing a career in pediatric radiology will be impacted.ConclusionCOVID-19 has had a substantial negative impact on pediatric radiology practice around the world.

The role of child life in pediatric radiology.

Pediatric radiology departments rely heavily on a dedicated, efficient and collaborative multi-disciplinary health care team to provide efficient service and quality care to patients and families. Certified child life specialists are an essential part of this multi-disciplinary team. The main goal of the child life specialists is to improve the overall experience for patients and families. In addition, child life specialists, working in collaboration with the medical care team, help decrease the need for general anesthesia by providing patient pain management, distraction and coping techniques. These interventions result in improved patient safety, increased departmental efficiency and increased revenue. The role of child life specialists extends into the exam room, where their interventions help decrease procedure times and improve imaging quality. In this article, the authors discuss the key role of child life specialists in a pediatric radiology department and provide examples of how child life can impact patient safety, patient and family satisfaction, and operational efficiency.
