The association between obesity, knee pain, and gait during stair descent in older adults with knee osteoarthritis

BackgroundObesity and knee osteoarthritis adversely affect activities of daily living in older adults. Together, the complexities of their interaction on mobility, including stair negotiation, are unresolved. The purpose of this study was to determine the relationship between obesity, pain, and stair negotiation in older adults with knee osteoarthritis. MethodsOlder adults with symptomatic knee osteoarthritis and overweight or obesity participated in the study (n = 28; age range = 57.0–78.0 yrs.; body mass index range = 26.6–42.8 kg•m−2). The Western Ontario and McMaster Universities Osteoarthritis Index pain subscale was used to measure knee pain. Measurements included a three-dimensional biomechanical analysis during descent on a set of force plate-instrumented stairs and a timed stair descent test. Pearson's r was used to determine associations between body mass index and pain, stair descent weight-acceptance phase vertical ground reaction force (vGRF) variables and lower extremity joint kinematics and kinetics, and timed stair descent performance. FindingsSignificant correlations existed between body mass index and pain (r = 0.41; p = 0.03), peak vGRF (r = 0.39; p = 0.04), vertical impulse (r = 0.49; p = 0.008), and peak ankle plantar flexor moments (r = 0.50; p = 0.007) in older adults with knee osteoarthritis. InterpretationGreater obesity in older adults with knee osteoarthritis was associated with greater knee pain and higher ankle joint loads during stair descent. These results support the recommendations of osteoarthritis treatment guidelines for weight-loss as a first-line of treatment for older adults with obesity and knee osteoarthritis.

Immediate improvements in post-stroke gait biomechanics are induced with both real-time limb position and propulsive force biofeedback

BackgroundParetic propulsion [measured as anteriorly-directed ground reaction forces (AGRF)] and trailing limb angle (TLA) show robust inter-relationships, and represent two key modifiable post-stroke gait variables that have biomechanical and clinical relevance. Our recent work demonstrated that real-time biofeedback is a feasible paradigm for modulating AGRF and TLA in able-bodied participants. However, the effects of TLA biofeedback on gait biomechanics of post-stroke individuals are poorly understood. Thus, our objective was to investigate the effects of unilateral, real-time, audiovisual TLA versus AGRF biofeedback on gait biomechanics in post-stroke individuals.MethodsNine post-stroke individuals (6 males, age 63 ± 9.8 years, 44.9 months post-stroke) participated in a single session of gait analysis comprised of three types of walking trials: no biofeedback, AGRF biofeedback, and TLA biofeedback. Biofeedback unilaterally targeted deficits on the paretic limb. Dependent variables included peak AGRF, TLA, and ankle plantarflexor moment. One-way repeated measures ANOVA with Bonferroni-corrected post-hoc comparisons were conducted to detect the effect of biofeedback on gait biomechanics variables.ResultsCompared to no-biofeedback, both AGRF and TLA biofeedback induced unilateral increases in paretic AGRF. TLA biofeedback induced significantly larger increases in paretic TLA than AGRF biofeedback. AGRF biofeedback increased ankle moment, and both feedback conditions increased non-paretic step length. Both types of biofeedback specifically targeted the paretic limb without inducing changes in the non-paretic limb.ConclusionsBy showing comparable increases in paretic limb gait biomechanics in response to both TLA and AGRF biofeedback, our novel findings provide the rationale and feasibility of paretic TLA as a gait biofeedback target for post-stroke individuals. Additionally, our results provide preliminary insights into divergent biomechanical mechanisms underlying improvements in post-stroke gait induced by these two biofeedback targets. We lay the groundwork for future investigations incorporating greater dosages and longer-term therapeutic effects of TLA biofeedback as a stroke gait rehabilitation strategy.Trial registrationNCT03466372

How Ankle Exoskeleton Assistance Affects the Mechanics of Incline Walking and Stair Ascent in Cerebral Palsy.

Graded terrains, like slopes and stairs, are particularly challenging for people with neurological disorders like cerebral palsy (CP) due to increased selective muscle control and muscle strength requirements. Lower-limb exoskeletons may be able to assist individuals with CP when navigating graded terrains. This study sought to determine the effects of untethered ankle exoskeleton assistance on lower-limb joint angles, moments, and muscle activity during up-incline walking and up-stair stepping in CP (n=7). We hypothesized that powered assistance would result in improved ankle mechanics (i.e., increased total ankle moments) across both terrains. During incline walking, we found that peak ankle dorsiflexion angle increased by $7^{\mathrm{o}}$(p=0.006) during walking with ankle assistance compared to walking without the device (Shod). Compared to without the device, the peak total ankle plantarflexor moment increased by 8% (p=0.022) while peak biological ankle plantarflexor moment decreased by 17% (p< 0.001). Incline walking with ankle assistance reduced stance phase muscle activity of the soleus (20%, p=0.010) and vastus lateralis (18%, p=0.004), and swing phase tibialis activity (19%, p=0.028) compared to Shod. During stair ascent with the device, the peak total ankle plantarflexor moment increased by 17% (p=0.011) and the peak knee extensor moment increased by 40% (p=0.018) compared to Shod. These findings provide insight into the biomechanical benefits of ankle exoskeleton assistance during incline and stair walking. This work aims to advance the use of robotic assistive technology to improve mobility for people with CP.

Association of Ankle Dorsiflexion and Landing Forces in Jumping Athletes.

Dorsiflexion range of motion restriction has been associated with patellar tendinopathy, but the mechanisms of how dorsiflexion restriction could contribute to knee overload remain unknown. Peak ankle dorsiflexion and ankle dorsiflexion excursion are negatively associated with peak vertical ground-reaction force (vGRF) and loading rate, and with peak patellar tendon force and loading rate, and positively associated with peak ankle plantar flexor moment. Cross-sectional study. Level 4. Kinematic and kinetic data of 26 healthy recreational jumping athletes were measured during a single-leg drop vertical jump. Pearson's correlation coefficients were calculated to establish the association between peak ankle dorsiflexion and ankle dorsiflexion excursion with peak vGRF and vGRF loading rate, with peak patellar tendon force and patellar tendon force loading rate, and with peak ankle plantar flexor moment. Ankle dorsiflexion excursion negatively correlated with peak vGRF loading rate (r = -0.49; P = 0.011) and positively correlated with peak ankle flexor plantar moment (r = 0.52; P = 0.006). In addition, there was a positive correlation between peak ankle dorsiflexion and peak vGRF (r = 0.39; P = 0.05). Ankle kinematics are associated with vGRF loading rate, ankle flexor plantar moment and peak vGRF influencing knee loads, but no association was observed between ankle kinematics and patellar tendon loads. These results suggest that increasing ankle dorsiflexion excursion may be an important strategy to reduce lower limb loads during landings but should not be viewed as the main factor for reducing patellar tendon force.

The effects of ankle-foot orthoses with plantar flexion stop and plantar flexion resistance using rocker-sole shoes on stroke gait: A randomized-controlled trial.

Objectives This study aims to evaluate the effect of two ankle-foot orthoses (AFOs), AFO with plantar flexion stop (AFO-PlfS), and AFO with plantar flexion resistance (AFO-PlfR), while wearing standard shoes and rocker-sole shoes. Patients and methods Between November 2017 and July 2018, in this randomized-controlled study, a total of 20 stroke patients (8 males, 12 females; mean age: 48.1 years; range, 33 to 65 years) in chronic phase were randomized to AFO groups (AFO-PlfS group, n=10 and AFO-PlfR group, n=10). Each group received the allocated AFO along with two kinds of shoes (standard shoe and rocker shoe) for a two-week adaptation. Two effects were separately evaluated: The orthotic effect and rocker shoe effect were defined as the evaluation of using an AFO wearing standard shoe compared to only standard shoe, and evaluation of using an AFO wearing rocker shoe compared to an AFO wearing standard shoe, respectively. The gait of each group was measured by three-dimensional motion analysis. Results A significant orthotic effect was found in both AFO groups in spatiotemporal parameters and maximum ankle dorsiflexion in the single-support phase. Additionally, the AFO-PlfR group showed a significant improvement in the parameters related to the first rocker of gait, but not for AFO-PlfS group concerning the orthotic effect. The rocker shoe effect was found in significant reduction of peak ankle plantar flexor moment and power ankle generation during preswing for both AFO groups. Conclusion According to the orthotic effect, an AFO-PlfR can create better function in the improvement of parameters related to the first rocker. Although a rocker shoe can facilitate rollover for weight progression in the third rocker of gait, it cannot make a strong push-off function in stroke survivors.

Normative Achilles and patellar tendon shear wave speeds and loading patterns during walking in typically developing children.

Motion analysis is commonly used to evaluate joint kinetics in children with cerebral palsy who exhibit gait disorders. However, one cannot readily infer muscle-tendon forces from joint kinetics. This study investigates the use of shear wave tensiometry to characterize Achilles and patellar tendon forces during gait. How do Achilles and patellar tendon wave speed and loading modulate with walking speed in typically developing children? Twelve typically developing children (9-16 years old) walked on an instrumented treadmill with shear wave tensiometers over their Achilles (n = 11) and patellar (n = 9) tendons. Wave speeds were recorded at five leg length-normalized walking speeds (very slow to very fast). Achilles and patellar tendon moment arms were measured with synchronized ultrasound and motion capture. The tendon wave speed-load relationship was calibrated at the typical walking speed and used to estimate tendon loading at other walking speeds. Characteristic Achilles and patellar tendon wave speed trajectories exhibited two peaks over a gait cycle. Peak Achilles tendon force closely aligned with peak ankle plantarflexor moment during pushoff, though force exhibited less modulation with walking speed. A second peak in late swing Achilles loading, which was not evident from the ankle moment, increased significantly with walking speed (p < 0.001). The two peaks in patellar tendon loading occurred at 12 ± 1% and 68 ± 6% of the gait cycle, matching the timing of peak knee extension moment in early stance and early swing. Both patellar tendon load peaks increased significantly with walking speed (p < 0.05). This is the first study to use shear wave tensiometry to characterize Achilles and patellar tendon loading during gait in children. These data could serve as a normative comparison when using tensiometry to identify abnormal tendon loading patterns in individuals who exhibit equinus and/or crouch gait.

What goes up must come down, part II: Consequences of jump strategy modification on dance leap landing biomechanics

ABSTRACT Knee injuries are common in jumping athletes; modifying jump strategy may impact loads placed on the body and reduce injury risk. The purpose of this study was to determine if modifying strategy in a saut de chat leap to focus on height would decrease sagittal plane knee loading. Biomechanical data were collected while 28 dancers performed saut de chat leaps with instructions to jump far (FAR) or jump high (UP). In the UP condition, there was greater vertical GRF and less braking GRF. Also in UP, lower extremity contact angle was greater (71.3 ± 2.9º FAR; 75.8 ± 3.3º UP; p = 0.0178), peak knee extensor moment was greater (2.8 ± 0.7 Nm FAR; 3.2 ± 0.8 Nm UP; p = 0.01), and peak ankle plantar flexor moment was lower (3.19 ± 0.4 Nm FAR; 2.94 ± 0.4 Nm UP; p < 0.01). A more acute LECA was related to greater braking force and braking force was related to greater knee extensor moments. Despite these relationships, we observed greater knee extensor moments in UP. While the relationship among these whole-body variables and knee joint loading exists, it may not be the primary factor driving load distribution during dance leap landings.

Altitude Training Mask Alters Ankle Joint Kinetics During Treadmill Walking

Elevation training masks are commonly used in strength and conditioning to simulate working in a hypoxic environment through resisted inspiration (RI). It is further suggested that RI improves the ability of the athlete’s muscle to utilize available oxygen carried by hemoglobin. Though some data exists regarding gross training performance in elevation training masks, little data is available regarding the effect of elevation training masks on lower extremity joint biomechanics during an exercise task. PURPOSE: to determine the effects of an altitude training mask on ankle joint kinetics during a treadmill walking task. METHODS: Seven healthy young adults performed two 10-minute treadmill walking tasks at 1.6 m/s in each of two conditions: normal walking (CON) and with RI. Three-dimensional kinematics and ground reaction forces (GRFs) were simultaneously recorded using a 6-camera motion capture system (250 Hz) and instrumented treadmill (1500 Hz). Peak ankle plantarflexor moments and powers were determined from the second (M2) and tenth (M10) minutes of the treadmill walking task. Two repeated measures ANOVAs were used to determine the effects of time and condition on peak plantarflexor moments and powers. RESULTS: No time by condition interactions were observed for plantarflexor moments (p = 0.26) or powers (p = 0.18). The RI condition was associated with greater plantarflexor moments (p = 0.04) and powers (p = 0.01) than the CON condition. No effects of time were observed for plantarflexor moments (p = 0.84) or powers (p = 0.63). CONCLUSIONS: These findings demonstrate that ankle joint kinetics are greater when performing a treadmill walking task when wearing an elevation training mask. Given the constant mechanical demand, these data suggest that a multi-joint mechanical adaptation occurred in response to the elevation training mask. Subsequent research may seek to address changes in joint contributions to the walking task when wearing an elevation training mask.

Short-term effects of a trunk modification program on patellofemoral joint stress in asymptomatic runners

ObjectivesTo evaluate short-term effects of a four-week gait retraining program using visual feedback on trunk flexion angle, patellofemoral joint (PFJ) stress, lower extremity biomechanics and motor skill automaticity. DesignLongitudinal interventional study. SettingsUniversity research laboratory. ParticipantsTwelve asymptomatic recreational runners (seven male and five female). Main outcome measuresTrunk kinematics as well as lower extremity kinematics and kinetics were assessed prior to training at week 1 (baseline) and week 2, 3, 4 and 8 (retention). PFJ stress was computed using a sagittal plane model. A dual-task procedure was performed to examine automaticity. ResultsAt week 8, runners demonstrated 10.1° increase in trunk flexion angle (p < .001) and 17.8% reduction in peak PFJ stress (p < .001) compared to baseline. This is associated with a 16.8% decrease in knee extensor moment and less than 2.5° change in knee flexion angle. Participants also showed 33.3% increase in peak hip extensor moment and small reduction in peak ankle plantar flexor moment. Lastly, runners demonstrated automaticity of the modified skill with a dual-task cost of less than 3%. ConclusionThe gait retraining program is effective to elicit short term changes in trunk position, PFJ stress, and automaticity of the new motor skill.

Ankle-foot orthosis alignment affects running mechanics in individuals with lower limb injuries.

Individuals with severe lower extremity injuries often require ankle-foot orthoses to return to normal activities. Ankle-foot orthoses alignment is a key consideration during the clinical fitting process and may be particularly important during dynamic activities such as running. To investigate how 3° changes in sagittal plane ankle-foot orthoses alignment affect running mechanics. Controlled laboratory study. Twelve participants with unilateral lower limb injury ran overground and lower extremity running mechanics were assessed. Participants wore their passive-dynamic ankle-foot orthoses in three alignments: clinically fit neutral, 3° plantarflexed from clinically fit neutral, and 3° dorsiflexed from clinically fit neutral. The 3° changes in sagittal alignment significantly influenced ankle mechanics during running. The plantarflexed alignment significantly decreased the peak ankle plantarflexor moment, peak knee extensor moment, and peak ankle and knee power absorption and generation compared to more dorsiflexed alignments. Alignment also altered footstrike angle, with dorsiflexed alignments associated with a more dorsiflexed footstrike pattern and plantarflexed alignments toward a more plantarflexed footstrike pattern. However, alignment did not influence loading rate. Small changes in ankle-foot orthoses alignment significantly altered running mechanics, including footstrike angle, and knee extensor moments. Understanding how ankle-foot orthoses design parameters affect running mechanics may aid the development of evidence-based prescription guidelines and improve function for ankle-foot orthoses users who perform high-impact activities. Understanding how ankle-foot orthoses alignment impacts biomechanics should be a consideration when fitting passive-dynamic devices for higher impact activities, such as running. Individual running styles, including footstrike patterns, may be affected by small changes in alignment.

Gait Deviations in Children With Osteogenesis Imperfecta Type I.

Osteogenesis imperfecta (OI) is a congenital connective tissue disorder often characterized by orthopaedic complications that impact normal gait. As such, mobility is of particular interest in the OI population as it is associated with multiple aspects of participation and quality of life. The purpose of the current study was to identify and describe common gait deviations in a large sample of individuals with type I OI and speculate the etiology with a goal of improving function. Gait analysis was performed on 44 subjects with type I (11.7±3.08 y old) and 30 typically developing controls (9.54±3.1 y old ). Spatial temporal, kinematic, and kinetic gait data were calculated from the Vicon Plug-in-Gait Model. Musculoskeletal modeling of the muscle tendon lengths (MTL) was done in OpenSim 3.3 to evaluate the MTL of the gastrocnemius and gluteus maximus. The gait deviation index, a dimensionless parameter that evaluates the deviation of 9 kinematic gait parameters from a control database, was also calculated. Walking speed, single support time, stride, and step length were lower and double support time was higher in the OI group. The gait deviation index score was lower and external hip rotation angle was higher in the OI group. Peak hip flexor, knee extensor and ankle plantarflexor moments, and power generation at the ankle were lower in the OI group. MTL analysis revealed no significant length discrepancies between the OI group and the typically developing group. Together, these findings provide a comprehensive description of gait characteristics among a group of individuals with type I OI. Such data inform clinicians about specific gait deviations in this population allowing clinicians to recommend more focused interventions. Level III-case-control study.

Hip flexor muscle dysfunction during walking at self-selected and fast speed in patients with aortoiliac peripheral arterial disease

ObjectiveIntermittent claudication aggravates physical function and is associated with an increased risk of death in patients with peripheral arterial disease (PAD). Previous studies on kinetic parameters (joint moment and power) of lower limbs in these patients have largely focused on the decline in the ankle plantar flexor moment and power at self-selected (SS) walking speed, which may not be an optimal condition to induce claudication pain. In the present study, we investigated the abnormalities in joint kinetic parameters in patients with PAD at both SS and at fast walking speeds. MethodsWe recruited 16 patients with aortoiliac PAD (4 unilateral and 12 bilateral) and 10 healthy controls. The participants were instructed to walk at SS and fast speeds along a 7-meter walkway embedded with a force plate. Spatiotemporal parameters and joint kinetic parameters of the lower limbs during the stance phase were recorded using a three-dimensional motion analysis device. ResultsCompared with the controls, patients with PAD showed a significant reduction in their walking speed, step length, stride length, and cadence. Further, a reduction in peak hip flexor moment at fast walking speed and in peak hip flexor generation power was observed in both modes of walking. However, no significant between-group differences were observed for the peak ankle plantar flexor moment or power at either walking speed. Multiple regression analysis showed peak hip flexor generation power was a strong contributor to reduction at both SS and fast walking speeds in patients with PAD. ConclusionsPatients with aortoiliac PAD walk slowly and show reduced kinetic parameters of the hip joint at both SS and fast walking speeds. Our results suggest that hip flexor muscles may be a useful target for exercise training in patients with aortoiliac PAD.

The influence of shoe drop on the kinematics and kinetics of children tennis players

This study investigated the immediate effects of reducing the shoe drop (i.e. the difference between the heel and the forefoot height) on the kinematics and kinetics of the lower extremities of children tennis players performing a tennis-specific movement. Thirteen children tennis players performed a series of simulated open stance forehands wearing 3 pairs of shoes differing only in the drop: 0 (D0), 6 (D6) and the control condition of 12 mm (D12). Two embedded forceplates and a motion capture system were used to analyse the ground reaction forces and ankle and knee joint angles and moments of the leading lower limb. In D6 compared with D12, the peak impact force was reduced by 24% (p = .004) and the ankle was less dorsiflexed at foot strike (p = .037). In D0 compared with D12, the peak impact force was reduced by 17% (p = .049), the ankle was less dorsiflexed at foot strike (p = .045) and the knee was more flexed at foot strike (p = .007). In addition, 4 out of 13 participants (31%) presented a forefoot strike pattern for some of the trials in D0. No difference was observed across shoe conditions for the peak knee extensor moment (p = .658) or the peak ankle plantarflexor moment (p = .071). The results provide preliminary data supporting the hypothesis that for children tennis players, using a 6-mm lower shoe drop might reduce heel impact forces and thus limit potentially impact-related injuries.

Ankle and knee kinetics between strike patterns at common training speeds in competitive male runners

The purpose of this study was to investigate the interaction of foot strike and common speeds on sagittal plane ankle and knee joint kinetics in competitive rear foot strike (RFS) runners when running with a RFS pattern and an imposed forefoot strike (FFS) pattern. Sixteen competitive habitual male RFS runners ran at two different speeds (i.e. 8 and 6 min mile−1) using their habitual RFS and an imposed FFS pattern. A repeated measures analysis of variance was used to assess a potential interaction between strike pattern and speed for selected ground reaction force (GRF) variables and, sagittal plane ankle and knee kinematic and kinetic variables. No foot strike and speed interaction was observed for any of the kinetic variables. Habitual RFS yielded a greater loading rate of the vertical GRF, peak ankle dorsiflexor moment, peak knee extensor moment, peak knee eccentric extensor power, peak dorsiflexion and sagittal plane knee range of motion compared to imposed FFS. Imposed FFS yielded greater maximum vertical GRF, peak ankle plantarflexor moment, peak ankle eccentric plantarflexor power and sagittal plane ankle ROM compared to habitual RFS. Consistent with previous literature, imposed FFS in habitual RFS reduces eccentric knee extensor and ankle dorsiflexor involvement but produce greater eccentric ankle plantarflexor action compared to RFS. These acute differences between strike patterns were independent of running speeds equivalent to typical easy and hard training runs in competitive male runners. Current findings along with previous literature suggest differences in lower extremity kinetics between habitual RFS and imposed FFS running are consistent among a variety of runner populations.

Long-term outcomes after multilevel surgery including rectus femoris, hamstring and gastrocnemius procedures in children with cerebral palsy

Background/AimsMultilevel surgical intervention is a common approach for the correction of gait abnormalities in children with cerebral palsy (CP). The short-term outcomes for the combination of rectus femoris transfer, hamstring lengthening and gastrocnemius lengthening have been well documented using three-dimensional motion analysis. However, the impact of time, growth, and puberty on these short-term outcomes of this combination of procedures is not well understood. The purpose of this study was to evaluate the long-term outcomes of these procedures on gait in patients with CP. MethodsTwenty-two patients underwent rectus femoris transfers, medial hamstring lengthenings and gastrocnemius lengthenings in combination with a selection of other soft tissue and/or bony procedures of the lower limb. All patients had a pre-operative motion analysis and post-operative analysis one and 11 years following surgery. ResultsSignificant changes in both clinical and gait variables from pre to 1 year post surgery confirmed the short-term gait benefits of this combination of surgical procedures. Long-term follow-up data indicated that the passive range of motion gains noted 1 year after surgery were lost at the knee and ankle. However, the improvements in ankle dorsiflexion and knee extension at initial contact were maintained over 11 years. As well, peak ankle dorsiflexion in stance was maintained and peak ankle plantar flexor moments and powers did not show declines long-term. Peak knee flexion showed a decline over the long-term, however, the timing of peak knee flexion in swing was maintained. ConclusionWhen compared to declines in gait kinematics in persons with CP without surgery, these results demonstrate the possible long-term benefits of surgical intervention.

Biomechanical Role of the Hip in Maintaining Balance during Standing Forward Reach

During reactions to external anterior-posterior (AP) postural perturbations, it has been demonstrated that the hip strategy (HS) is employed only when balance risk is high and the ankle strategy (AS) is inadequate. In quiet standing the ankle plays a major role in maintenance of AP balance while the role of the hip is limited to medial-lateral (ML) control. This has led to the hypothesis that use of HS during self-initiated AP movements that are relatively safe or involve small center of mass (COM) excursions indicates poor balance, possibly due to deficits in ankle musculature. Consequently, comprehensive balance tests like BESTest focus on hip strength in the frontal plane and ankle strength in the sagittal plane. PURPOSE: To elucidate the biomechanical role of the hip during self-initiated standing forward reach. This movement pattern is used in several clinical balance screens including BESTest, Berg Balance Scale and Functional Reach Test. METHODS: 5 healthy young participants stood with their right shoulder flexed to 90°. A yardstick was placed parallel to the floor at the level of the acromion. They were instructed to reach forward as far as possible without taking a step and without raising their heels off the floor. 3 trials each were collected with 2 different instructions - do not bend the hip (AS) and bend the hip (HS). 2 force plates and an 11 camera motion capture system were used to obtain joint angles and moments, and whole-body COM and center of pressure (COP) excursions. RESULTS: Reach distance normalized by height was significantly higher during HS (Δ= 4.5±1.3 cm; p=0.008). This was accompanied by a significantly higher peak hip extensor moment (Δ= 0.41±0.13 Nm/kg; p=0.014). However, the maximum COM and COP excursions and peak ankle plantarflexor moment were not significantly different between strategies. CONCLUSIONS: Using HS allowed participants to reach further without increasing the balance risk as assessed by COM and COP excursions. Based on these findings, it is likely that in a functional situation where an individual is required to reach a fixed distance, using HS will achieve the goal with smaller COP and COM excursions compared to AS. Therefore, use of HS may not necessarily be a last resort indicating absence of ankle strength and the hip should not be ignored during assessment and retraining of AP balance.

Understanding the effect of touchdown distance and ankle joint kinematics on sprint acceleration performance through computer simulation

This study determined the effects of simulated technique manipulations on early acceleration performance. A planar seven-segment angle-driven model was developed and quantitatively evaluated based on the agreement of its output to empirical data from an international-level male sprinter (100 m personal best = 10.28 s). The model was then applied to independently assess the effects of manipulating touchdown distance (horizontal distance between the foot and centre of mass) and range of ankle joint dorsiflexion during early stance on horizontal external power production during stance. The model matched the empirical data with a mean difference of 5.2%. When the foot was placed progressively further forward at touchdown, horizontal power production continually reduced. When the foot was placed further back, power production initially increased (a peak increase of 0.7% occurred at 0.02 m further back) but decreased as the foot continued to touchdown further back. When the range of dorsiflexion during early stance was reduced, exponential increases in performance were observed. Increasing negative touchdown distance directs the ground reaction force more horizontally; however, a limit to the associated performance benefit exists. Reducing dorsiflexion, which required achievable increases in the peak ankle plantar flexor moment, appears potentially beneficial for improving early acceleration performance.

Gait deviations in children with autism spectrum disorders: a review.

In recent years, it has become clear that children with autism spectrum disorders (ASDs) have difficulty with gross motor function and coordination, factors which influence gait. Knowledge of gait abnormalities may be useful for assessment and treatment planning. This paper reviews the literature assessing gait deviations in children with ASD. Five online databases were searched using keywords “gait” and “autism,” and 11 studies were found which examined gait in childhood ASD. Children with ASD tend to augment their walking stability with a reduced stride length, increased step width and therefore wider base of support, and increased time in the stance phase. Children with ASD have reduced range of motion at the ankle and knee during gait, with increased hip flexion. Decreased peak hip flexor and ankle plantar flexor moments in children with ASD may imply weakness around these joints, which is further exhibited by a reduction in ground reaction forces at toe-off in children with ASD. Children with ASD have altered gait patterns to healthy controls, widened base of support, and reduced range of motion. Several studies refer to cerebellar and basal ganglia involvement as the patterns described suggest alterations in those areas of the brain. Further research should compare children with ASD to other clinical groups to improve assessment and treatment planning.

Biomechanical Factors Associated With Time to Complete a Change of Direction Cutting Maneuver

Cutting ability is an important aspect of many team sports, however, the biomechanical determinants of cutting performance are not well understood. This study aimed to address this issue by identifying the kinetic and kinematic factors correlated with the time to complete a cutting maneuver. In addition, an analysis of the test-retest reliability of all biomechanical measures was performed. Fifteen (n = 15) elite multidirectional sports players (Gaelic hurling) were recruited, and a 3-dimensional motion capture analysis of a 75° cut was undertaken. The factors associated with cutting time were determined using bivariate Pearson's correlations. Intraclass correlation coefficients (ICCs) were used to examine the test-retest reliability of biomechanical measures. Five biomechanical factors were associated with cutting time (2.28 ± 0.11 seconds): peak ankle power (r = 0.77), peak ankle plantar flexor moment (r = 0.65), range of pelvis lateral tilt (r = -0.54), maximum thorax lateral rotation angle (r = 0.51), and total ground contact time (r = -0.48). Intraclass correlation coefficient scores for these 5 factors, and indeed for the majority of the other biomechanical measures, ranged from good to excellent (ICC >0.60). Explosive force production about the ankle, pelvic control during single-limb support, and torso rotation toward the desired direction of travel were all key factors associated with cutting time. These findings should assist in the development of more effective training programs aimed at improving similar cutting performances. In addition, test-retest reliability scores were generally strong, therefore, motion capture techniques seem well placed to further investigate the determinants of cutting ability.

The effect of trunk flexion on lower-limb kinetics of able-bodied gait

Able-bodied individuals spontaneously adopt crouch gait when walking with induced anterior trunk flexion, but the effect of this adaptation on lower-limb kinetics is unknown. Sustained forward trunk displacement during walking can greatly alter body center-of-mass location and necessitate a motor control response to maintain upright balance. Understanding this response may provide insight into the biomechanical demands on the lower-limb joints of spinal pathology that alter trunk alignment (e.g., flatback). The purpose of this study was to determine the effect of sustained trunk flexion on lower-limb kinetics in able-bodied gait, facilitating understanding of the effects of spinal pathologies. Subjects walked with three postures: 0° (normal upright), 25±7°, and 50±7° trunk flexion. With increased trunk flexion, decreased peak ankle plantar flexor moments were observed with increased energy absorption during stance. Sustained knee flexion during mid- and terminal stance decreased knee flexor moments, but energy absorption/generation remained unchanged across postures. Increased trunk flexion placed significant demand on the hip extensors, thus increasing peak hip extensor moments and energy generation. The direct relationship between trunk flexion and energy absorption/generation at the ankle and hip, respectively, suggest increased muscular demand during gait. These findings on able-bodied subjects might shed light on muscular demands associated with individuals having pathology-induced positive sagittal spine balance.
