Patient safety culture in private hospitals in China: a cross-sectional study using the revised Hospital Survey on Patient Safety Culture.

This study aimed to translate the revised Hospital Survey on Patient Safety Culture (HSOPSC 2.0) to Mandarin, evaluate its psychometric properties, and apply it to a group of private hospitals in China to identify the determinants associated with patient safety culture. A two-phase study was conducted to translate and evaluate the HSOPSC 2.0. A cross-cultural adaptation of the HSOPSC 2.0 was performed in Mandarin and applied in a cross-sectional study in China. This study was conducted among 3,062 respondents from nine private hospitals and 11 clinics across six cities in China. The HSOPSC 2.0 was used to assess patient safety culture. Primary outcomes were measured by the overall patient safety grade and patient safety events reported. Confirmatory factor analysis results and internal consistency reliability were acceptable for the translated HOSPSC 2.0. The dimension with the highest positive response was "Organizational learning - Continuous improvement" (89%), and the lowest was "Reporting patient safety event" (51%). Nurses and long working time in the hospital were associated with lower assessments of overall patient safety grades. Respondents who had direct contact with patients, had long working times in the hospital, and had long working hours per week reported more patient safety events. A higher level of patient safety culture implies an increased probability of a high overall patient safety grade and the number of patient safety events reported. The Chinese version of HSOPSC 2.0 is a reliable instrument for measuring patient safety culture in private hospitals in China. Organizational culture is the foundation of patient safety and can promote the development of a positive safety culture in private hospitals in China.

The impact of nurse empowerment program on patient safety culture in general public hospital in Indonesia

Background Patient safety is crucial to health-care quality and is one of the major parameters monitored by all health-care organizations around the world. Nurses play a critical role in improving the quality of care and patient safety because of their intensity in caring patients and families. Objectives This study aimed to determine the impact of nurse empowerment program on patient safety culture in general public hospital in Indonesia. Methods This study was designed as a quasi-experimental design in Indonesia. This program was conducted for three months with a five-day workshop (4 h for each day), followed by hanging up posters and handing out informative pamphlets to the experimental group's nurses and supervisors at their place of work in three common place that visited by nurses such as changing room, nurse station and near toilet. Results In the intervention group, patient safety culture practice score was significantly increased from 3.11(standard deviation [SD] = 1.43) before intervention to 4.89 (SD = 1.33) after three-month intervention (T2), with the t was 5.78, and p = .001. While in the control group, no significant increased score of patient safety culture practice before intervention and after three months intervention (3.05 ± 1.49 vs. 3.09 ± 1.25, t = 0.76, p = .516). Conclusion The implementation of this innovative empowerment program, which included nurses and supervisors, resulted in an increase in patient safety culture scores as well as growth in domain score. Therefore, this program can be used to advance these critical facets of patient safety culture, which are currently underdeveloped.

Impact of short-notice accreditation assessments on hospitals' patient safety and quality culture-A scoping review: A scoping review of the current literature to understand the impact of short-notice accreditation processes on health service organizations patient safety and quality culture.

To explore the published evidence describing the impact of short-notice accreditation assessments on hospitals' patient safety and quality culture. Arksey and O'Malley (2005)'s scoping study framework and Preferred Reporting Items for Systematic Reviews and Meta-Analysis Extension for Scoping reviews (PRISMA-ScR). A scoping review was conducted to identify papers that provided an evaluation of short-notice accreditation processes. All reviewers independently reviewed included papers and thematic analysis methods were used to understand the data. PubMed/MEDLINE, CINAHL, and ProQuest databases were searched to identify papers published after 2000. Totally, 3317 records were initially identified with 64 full-text studies screened by the reviewers. Five studies were deemed to meet this scoping review's inclusion criteria. All five studies reported variable evidence on the validity of health service or hospital accreditation processes and only three considered the concept of patient safety and quality culture in the context of accreditation. None of the five included studies report the impact of a short-notice accreditation process on a hospital's patient safety and quality culture. Limited evidence exists to report on the effectiveness of hospital short-notice accreditation models. No study has been undertaken to understand the impact of short-notice accreditation on patient safety and quality cultures within hospital settings. Understanding this topic will support improved hospital quality, safety, policy, and governance. To provide an understanding of the current knowledge base of short-notice accreditation models and its impact on hospital patient safety and quality culture. The Relationship of Organizational Culture and Patient Safety Culture on Service Development in The Hospital Dr. Ario Wirawan Salatiga

The primary objective of his research is to examine the relationship between the organizational culture and patient safety culture at the Dr. Ario Wirawan Pulmonary Hospital in Salatiga, specifically in the context of service development. Dr. Ario Wirawan Salatiga admitted 99 participants to the study at Lung Hospital. Data collection was conducted through the use of a questionnaire. Utilizing a non-probability sampling method for the sampling procedure. The data analysis technique included multiple linear regression analyses. The study's findings indicate that organizational culture has a pivotal role in stimulating service growth, exerting a distinct and substantial influence. The study's findings clearly indicate that the most prominent part of patient safety culture in hospitals is "teamwork in units." Conversely, the aspects that need enhancement include "staffing" and "handoff and transition." (2) A robust safety culture has a substantial and beneficial impact on the advancement of services.

Psychometric Analysis of the Indian Version of the Patient Safety Culture Tool (I-HSOPSC 2.0) Validation

Patient safety, which is both an overarching principle and a key factor in determining the quality of healthcare, continues to be a priority in healthcare systems on a global scale. A cross-sectional study in accredited Indian hospitals aimed to evaluate the HSOPSC V-2 (Hospital Survey on Patient Safety Culture). 1,603 healthcare professionals participated, assessing psychometric features using confirmatory factor analysis in SMART PLS 4. The average composite positive response rate was 64.8%. High positive responses (79%) were seen in ‘teamwork’, ‘communication about error’, and ‘hospital management support for patient safety’. However, ‘staffing and work pace’ (30%), ‘response to error’ (50%), ‘communication openness’, and ‘reporting patient safety events’ (both 59%) had lower positive responses, indicating room for improvement. Findings suggest I-HOSPSC 2.0’s content validity, reliability, and construct validity in measuring patient safety culture in Indian hospitals. It can aid administrators in assessing safety culture and improving patient safety and treatment efficacy. Notably, this is the inaugural HSOPSC V-2 validation research in the Indian healthcare context. Future research should explore the tool’s psychometric properties based on care setting levels due to substantial differences between public and private healthcare setups in India.

Enhancing Patient Safety Culture in Hospitals.

Patient safety has become a top priority for healthcare organizations. Abetter patient safety environment is associated with a lower probability of significant complications. Training programmers is critical to promoting patient safety and minimizing misunderstandings. The quality, performance, and productivity of the healthcare industry can be dramatically improved by changing the patient safety atmosphere operating within the hospital sector. Hospitals can significantly reduce medical errors and adverse events by implementing the program and training programmers to prioritize patient safety. This will improve patient outcomes and increase efficiency and effectiveness. Creating a patient safety culture within hospitals will contribute to a higher standard of care and improved overall performance in the healthcare industry. Hospitals can identify systemic problems and implement proactive measures to prevent future incidents by creating an environment in which healthcare professionals feel comfortable reporting errors. A patient safety culture encourages collaboration and open communication among healthcare teamsleading to more effective and coordinated care.

COMPARATIVE ANALYSIS OF PATIENT SAFETY CULTURE DYNAMICS IN UKRAINE AND THE USA HEALTHCARE FACILITIES REFERENCE TO ESTABLISHING SAFE HOSPITAL ENVIRONMENT

Introduction. WHO widely draws the attention of the world community to safety culture as an effective basis for ensuring safety in the hospital environment. However, WHO warns that "changing culture is more difficult than introducing new structural and organizational mechanisms". Therefore, systems for assessing patient safety culture in hospitals are widely used in many countries today.
 The aim of the study. To analyze and compare the dynamics of patient safety culture in Ukraine and the USA healthcare facilities.
 Materials and methods. The study utilized bibliographic, statistical, and sociological research methods. The research material consisted of a questionnaire survey of medical personnel on patient safety culture in 2016 (163 surveys from 3 healthcare facilities) and a questionnaire survey conducted in 2023 (174 surveys from 4 healthcare facilities). Results of patient safety culture monitoring in US hospitals for 2014 and 2021 were used for comparison. The survey of medical personnel was conducted using the AHRQ leadership questionnaire.
 Results. The percentage of positive responses for 5 out of the 12 characteristics of patient safety culture that we investigated in domestic healthcare facilities in 2023 was significantly lower than in 2016. The main reason for this negative trend in patient safety culture within domestic healthcare facilities can be attributed to the COVID-19 pandemic and Russia's military aggression, but the primary cause lies in the absence of a comprehensive strategy for improving this culture. Over the past 8 years, the proportion of respondents willing to disclose errors in their professional activities, as well as the mistakes of their colleagues and other incidents, without fearing that their mistakes and incident reports will harm them, has remained largely unchanged. The slight trend toward improvement in patient safety culture indicators in US hospitals, despite significant attention to it, indicates the challenges we face in the journey towards forming a positive safety culture.
 Conclusions. The prevailing reactive safety culture in domestic healthcare, along with the lack of positive changes, hinders the implementation of modern safety strategies in the medical field. The data from our own research and scientific literature highlight the urgent need for in-depth examination of the state of patient safety culture in domestic healthcare facilities and the development of a methodological framework adapted to international standards for evaluating safety culture in medical institutions. The formation of a positive national culture in general, and safety culture within healthcare facilities in particular, should be regarded as a crucial prerequisite for the post-war reconstruction of the country and its integration into the civilized, economically developed global community.

Factors influencing patient safety culture in hospitals: Literature review

Patient safety is a system that aims to enhance the safety of patient care and prevent injuries resulting from errors during the execution of action. According to the 2011 Minister of Health Regulation concerning Hospital Patient Safety, every hospital is required to undertake patient safety management because hospitals are the most likely locations for patient safety events to occur (Ministry of Health of the Republic of Indonesia, 2011). As a result, hospital culture and patient safety are intertwined. There are various factors that affect patient safety culture. Objectives: To determined the various factors that influence the culture of patient safety in a hospital. Methods: Article search is done through ScienceDirect database, Garuda Portal, and Google Scholar. The keywords entered use Indonesian and English, where for Indonesian keywords are "keselamatan pasien" DAN "rumah sakit", while for English are "patient safety" AND "factors" AND "hospital". From the articles found, the author took 5 articles whose articles were in accordance with inclusion criteria and considered relevant to the topic of discussion. Results: Factors that influence patient safety culture in hospitals include, 1) Economic Aspects; 2) Leadership; 3) Culture; 4) Risk Management System and Health Services; 5) Knowledge, Attitudes, and Actions; 6) Analysis of the Situation and Conditions of the Working Environment; 7) Stress Level and Workload of Nurses; 8) Mentoring Program; 9) Nursing Supervision and Evaluation of the Application of Patient Safety Goals; 10) Teamwork; and 11) Organizational Learning. Conclusions: There are various factors that influence the culture of patient safety in hospitals.

Patient Safety Culture Instrument: A Systematic Review

The instruments of Hospital patient safety culture are connected to organizational culture aspects. This study aimed to review and synthesize knowledge and explore various instruments to measure hospital patient safety culture. Articles from 2018 to 2022 were selected from Web of Science, Sage Journals, SpringerLink, and Scopus. The terms used were patient safety culture, safety culture, clinical supervision, web-based, electronic, and technology. The selection procedure was managed by PRISMA 2020, and the inclusion and exclusion criteria were determined using PICO. 2.120 articles were found after duplicates were eliminated and the titles, abstracts, and keywords began to be checked. After excluding an irrelevant study, only ten publications remained on assessing patient safety culture in hospitals. The prevailing definitions highlight patient safety culture as managing values, beliefs, attitudes, and competencies within a healthcare organization, influencing member behavior and the approach to errors, reporting, and learning. It aims to reduce and prevent unintended patient harm through various mechanisms and tools tailored to fit each institution's values. Paper-based or electronic-based instruments have advantages because they were created and utilized in numerous investigations. The survey response rate is also influenced by how healthcare personnel view the culture surrounding patient safety. This research is crucial for hospitals and the broader healthcare community as it equips them with the knowledge and tools needed to cultivate a culture of patient safety, reduce errors, and ultimately protect the well-being of patients. It addresses the evolving healthcare landscape, emphasizing customization, technology, and staff involvement in pursuing safer healthcare environments. Healthcare practitioners’ attitudes toward patient safety in hospital settings in Jeddah, Kingdom of Saudi Arabia

PurposePatient safety focuses on minimizing risks that might occur to patients during provision of healthcare. The purpose of this study was to explore healthcare practitioners’ attitudes towards patient safety inside different hospital settings in Jeddah, Kingdom of Saudi Arabia.Design/methodology/approachA descriptive, cross-sectional study was conducted on a sample of healthcare practitioners in main hospitals in Jeddah. Two main hospitals (one governmental and one private) were selected from each region of Jeddah (east, west, north and south), with a total number of eight out of thirty hospitals. Data were collected through the Attitudes to Patient Safety Questionnaire III that was distributed online. The questionnaire used a 5-point scale. Descriptive statistics were used. Comparisons were made by independent t-test and ANOVA. The statistical significance level was set at p < 0.05.FindingsThe study included 341 healthcare practitioners of different sexes and specialties in eight major governmental and private hospitals in Jeddah. “Working hours as error cause” subscale had the highest mean score (4.03 ± 0.89), while “Professional incompetence as error cause” had the lowest mean score (3.49 ± 0.97). The total questionnaire had a moderate average score (3.74 ± 0.63). Weak correlations between the average score of the questionnaire and sex, occupation and workplace were found (−0.119, −0.018 and −0.088, respectively).Practical implicationsHospitals need to develop targeted interventions, including continuing professional development programs, to enhance patient safety culture and practices. Moreover, patient safety training is required at the undergraduate education level, which necessitates health professions education institutions to give more attention to patient safety education in their curricula.Originality/valueThe study contributed to the existing literature on patient safety culture in hospital settings in Jeddah, Saudi Arabia. The insights generated by the study can inform targeted interventions to enhance patient safety culture in hospitals and improve patient outcomes.

Leadership to promote patient safety culture in public hospitals managed by social health organizations

PurposeThis paper aims to identify the kind of internal climate leaders should offer health-care professionals to promote a patient safety culture in public hospitals managed by social health organizations in Brazil.Design/methodology/approachTwo surveys were applied to health-care professionals working at three Brazilian public hospitals. The internal climate survey reached 1,013 respondents, and the patient safety culture survey reached 1,302 participants. Both factor and regression analyses were used to analyze the study model and determine how internal climate influences patient safety culture.FindingsResults indicate that to promote a patient safety culture among health-care professionals, leaders should generate an internal climate based on trust to foster pride in working in the hospital. Possibly, the trust dimension is the most important one and must be developed to achieve job satisfaction and provide better services to patients.Research limitations/implicationsAll the hospitals studied were managed by the same Organização Social de Saúde. Due to the limited responses concerning the respondents’ profiles, demographic variables were not analyzed.Practical implicationsThis research reveals that the trust and pride dimensions can most strongly influence a positive patient safety culture, helping hospital leaders face this huge managerial challenge of consistently delivering high standards of patient safety.Originality/valueThis research studies the promotion of a patient safety culture in public hospitals managed by social health organizations, characterized by greater flexibility and autonomy in health-care management and by a greater need for accountability.

Patient Safety Culture, Infection Prevention, and Patient Safety in the Operating Room: Health Workers' Perspective.

A hospital's patient safety culture affects surgical outcomes. Operating room safety culture has been overlooked despite the importance of patient safety. The AHRQ's Hospital Survey on Patient Safety Culture (HSOPSC) has been used worldwide to assess and enhance patient safety culture. This study examined how patient safety culture and infection prevention effect patient safety in the Operating Room (OR). This observational study used an online survey and included 143 OR workers. Descriptive statistics and multilinear regression were used to examine how patient safety culture and infection prevention affects level of patient safety. Most responders worked in excellent-accredited general hospitals. Most responders were male, aged between 26 to 40 years old, and had bachelor's degrees. Most were hospital-experienced nurses. Less than half had worked in units for over ten years. Organizational Learning - Continuous Improvement; Teamwork and Handoffs; and Information Exchange had the most positive responses in the OR. However, Staffing, Work Pace, and Patient Safety ranked lowest. Organizational Learning - Continuous Improvement and Hospital Management Support for Infection Prevention Efforts were found to affect OR patient safety level perceptions. According to the findings of our study, the overall patient safety culture in the operating room remains weak which highlights the importance of continuing efforts to improve patient safety in the OR. Further study could be directed to identify organizational learning in infection prevention to enhance the patient safety in the OR.

Patient Safety Culture Analysis in Dental Hospital Using Dental Office Survey on Patient Safety Culture Questionnaire: A Cross-cultural Adaptation and Validation Study.

The culture of safety and patient safety management in dental practice lags compared with medical practice. Hospitals strive to pursue quality and safety of healthcare services, with evidence of patient safety incidents in medical practice demonstrating the importance of promoting a safety culture. Measuring patient safety culture is a necessary first step to improving safety culture in clinical settings. As a hospital, dental hospital should improve the quality and culture of patient safety. Thus, our objective was to conduct a cross-cultural adaptation of a US measure of dental office patient safety culture for use in Indonesian dental hospitals. A cross-sectional study was conducted on 200 respondents at a dental hospital in Java, Indonesia. The first stage includes cultural adaptation and translation, followed by developing a questionnaire that was tested through expert agreement and analysis of validity and reliability using Spearman, Cronbach correlation coefficients, and correlation coefficients between classes. The Dental Office Survey on Patient Safety Culture consists of 58 items and 10 dimensions (overall perceptions of patient safety and quality, organizational learning, teamwork, staff training, work pressure and pace, management support for patient safety, office processes and standardization, communication about errors, communication openness, and patient care tracking/follow-up). A total of 200 respondents with a response rate of 61.5% and 77 invalid responses due to incomplete filling, so 123 respondents were analyzed. The validity test results on 38 question items from 10 dimensions, with a sign of 0.05%, 35 items are declared valid. The Indonesian version of the Dental Office Survey on Patient Safety Culture shows good validity and good reliability and has the potential to be used to evaluate patient safety culture in dental hospitals in Indonesia.

The relationship between patient safety culture and adverse events in Iranian hospitals: a survey among 360 nurses

BackgroundAdverse events have become a global problem and are an important indicator of patient safety. Patient safety culture is essential in efforts to reduce adverse events in the hospital. This study aimed to investigate the status of the patient safety culture, the frequency of adverse events, and the relationship between them in Qazvin's hospitals in Iran.MethodsThe present study is a descriptive-analytical study conducted in six hospitals in Qazvin, Iran, in 2020. The study population was nurses working in Qazvin hospitals. We collected data via a patient safety culture questionnaire and an adverse event checklist. Three hundred sixty nurses completed questionnaires. Multiple logistic regression was used to investigate the relationship between variables.ResultsThe highest mean of patient safety culture was related to the organizational learning dimension (3.5, SD = .074) and feedback and communication about errors (3.4, SD = 0.82). The participants gave the lowest score to dimensions of exchanges and transfer of information (2.45,=0.86) and management support for patient safety (2.62,Sd = 0.65). Management's support for patient safety, general understanding of patient safety culture, teamwork within organizational units, communication and feedback on errors, staff issues, and information exchange and transfer were significant predictors of adverse events.ConclusionThis study confirms patient safety culture as a predictor of adverse events. Healthcare managers should provide the basis for improving the patient safety culture and reducing adverse events through methods such as encouraging the reporting of adverse events and also holding training courses for nurses.

Comparing patient safety culture in primary, secondary and tertiary hospitals in Ghana.

This study compared patient safety culture among health professionals in tertiary, secondary and primary hospitals. We conducted a cross-sectional survey among thirteen primary, secondary and tertiary hospitals in Ghana. A structured questionnaire was administered to 1,656 health professionals. Data were analysed using descriptive statistics and One-Way Analysis of Variance (ANOVA). This study was conducted in the Greater Accra, Bono and Upper East regions, representing the southern, middle and northern ecological zones, respectively. Health professionals. The primary outcome was patient safety culture. Five patient safety culture dimensions were rated moderate positive response, while five were rated high positive response. We found a statistically significant difference in patient safety culture across primary, secondary and tertiary hospitals (p < 0.05). For instance, the mean difference between tertiary and secondary hospitals was statistically significant (p < 0.05). Additionally, the mean difference between tertiary and primary hospitals was statistically significant (p < 0.05). There was also a significant difference in the means between secondary and primary hospitals (p < 0.05). This study has demonstrated a variation in patient safety culture across Ghana's tertiary, secondary and primary hospitals. Background: The global prevalence of preventable adverse events in healthcare is a significant problem, with approximately 42.7 million annual accidents resulting in 23 million DALYs lost. Safety culture plays a crucial role in mitigating or exacerbating these incidents within hospitals. Objectives: To identify the results of the assessment of patient safety culture and the characteristics associated with patient safety culture in public general hospitals in various countries. Methods: This study was conducted between April-July 2021 utilizing a literature review approach using study searches through PubMed, Scopus, ScienceDirect, Google Scholar, and Garuda databases, and it discovered 17 papers from 2011-2021 that used the Hospital Survey on Patient Safety Culture (HSOPSC) as an instrument to assess patient safety culture. Results: This study found that patient safety culture at public general hospitals remains low, and hospital staff perception of safety has to be improved. Discussions: “Teamwork in hospital units” receives the most favorable replies, while “non-punitive response to error” receives the least positive responses. The majority of the employees assessed patient safety in their work unit as “excellent/very good/acceptable” and reported no incidents in the previous year. Staff and hospital characteristics were also found to be related to patient safety culture.

An Evaluation of Healthcare Safety Culture Among Healthcare Professionals in Secondary and Tertiary Public Hospitals in the Middle East Region.

Background and aim The provision of quality healthcare is initiated by a culture of patient safety. Understanding the patient safety culture (PSC) is a critical concept for all healthcare workers. We conducted this study to evaluate the PSC understanding among the Hamad Medical Corporation (HMC) staff members. Furthermore, to establish a local (HMC) reference point for providing quality health care based on a culture of patient safety. Method A Hospital Patient Safety Culture Survey (HSOPSC) was presented to our health system employees to assess their perceptions and understandings of PSC. The survey was self-administered. STATA Package version 12.0 culture software was used to analyze these data in terms of descriptive, correlational, and multivariate ordinal regression. Results This study targeted to survey 6,538 employees in HMC facilities, but only 5,583 responded, resulting in a percentage response rate of 85.4%. Ten facilities achieved 100% participation, and other HMC facilities had response rates ranging from 71.2% to 97.5%. Approximately 88.0% of the responders had direct patient contact. The HSOPSC survey resulted in an overall positive response rate of 62.4%. The dimensions with the highest positive response score were "teamwork within the Unit" followed by "organizational learning/continuous improvement" and "management support for patient safety" with a mean percent positive response (PPR) of 83.1%, 82.0%, and 79.2%, respectively. Conversely, there are three dimensions with the lowest positive response score, including "communication openness," "staffing," and "nonpunitive response to errors," with a mean PPR of 46.6%, 40.1%, and 27.7%, respectively. ANOVA and the student t-test revealed that men (64.3% ± 8.1%), employees with 11-15 years of experience in their specialty (65.8% ± 6.5%), and general hospital type (64.4% ±7.2%), were all significantly associated with differences in the overall perceptions of PSC. According to the study results, there was a moderate correlation between perceptions of PSC at the hospital and the following: Teamwork Across Units(RS= 0.43; p < 0.05), and Frequency of Events Reported (RS= 0.40; p < 0.05.). A regression analysis found that men, workers under 40 years of age, professionals with no direct contact with patients, employees with 11-15 years of experience in their specialty, intensive care staff, and general hospital staff were all significant predictors of overall favorable perceptions of the PSC. Conclusion PSC's understanding of HMC staff is moderate. Furthermore, this is the first study conducted for PSC understanding by the HMC staff in Qatar State. It is eligible to be considered a backbone and reference for new research projects about PSC in Qatari health facilities, if not worldwide.
