Poster 234: Establishing the Minimal Clinically Important Difference and Patient Acceptable Symptomatic State After Isolated Arthroscopic Posterior Medial Meniscal Root Repair

Objectives: Root tears encompass avulsion injuries affecting the bony or soft tissue root attachments of the meniscus, as well as radial tears occurring within 1 cm of these attachment points. Meniscal root tears have been increasingly recognized as contributors to early onset osteoarthritis and disrupted joint kinematics and insights from animal, cadaveric, and clinical studies have resulted in a paradigm shift from meniscectomy towards repair. The purpose of this study is to define the minimal clinically important difference (MCID) and patient-acceptable symptomatic state (PASS) for patient-reported outcome measures (PROMs) following isolated posterior medial meniscal root repair (PMMR) and to investigate the role of preoperative, demographic, and intraoperative variables for predicting achievement of these thresholds. We hypothesized that patients have a greater likelihood of achieving MCID with lower preoperative outcome scores and a greater likelihood to achieve PASS with higher preoperative outcome scores. Furthermore, certain preoperative and intraoperative factors are likely to influence achievement of MCID and PASS following PMMR. Methods: This is an IRB approved retrospective study of prospectively collected data from a single, high-volume institution. Inclusion criteria were patients who underwent arthroscopic PMMR performed by one of three fellowship-trained attending surgeons with a minimum of two year postoperative follow up. Exclusion criteria consisted of patient with KL grade >3 osteoarthritis and concomitant ligamentous procedures. Patients who had ipsilateral knee surgery within 2 years of undergoing meniscal root repair were also excluded. PROMs were collected from January 2017 to September 2023 and an electronic medical record was reviewed for demographic variables, intraoperative details, and postoperative complications. PROMs analyzed in this study include the International Knee Documentation Committee Score (IKDC), the Knee Injury and Osteoarthritis Outcome Score Jr. (KOOS Jr.) and the Veterans Rand 12 Physical Function (VR12 P). MCID was determined to be one-half the standard deviation of the difference from baseline in PROM scores at minimum two-year follow-up. PASS thresholds were calculated using an anchor-based approach. Receiver operating characteristic (ROC) analysis for the calculation of PASS was conducted and optimal thresholds were calculated using the Youden Index to maximize the sensitivity and specificity of threshold values. Multivariate logistic regression analyses were conducted to identify patient-related factors at baseline and intraoperative variables associated with the attainment of MCID and PASS. Results: Seventy patients were included in the final analysis. The demographics of included patients is listed in Table 1. The mean preoperative score, mean postoperative score, MCID threshold, MCID achievement rate, PASS threshold, and PASS achievement rate for IKDC, KOOS Jr., and VR12 physical are listed within Table 2. The predictive value for all PASS thresholds was good to excellent, with AUC values for IKDC, KOOS Jr., and VR12 Physical being 0.858, 0.864, and 0.755, respectively (Figure 1). For IKDC, lower BMI (OR 0.73, p = 0.03) and lower preoperative score (OR 0.94, p = 0.02) were predictive of MCID achievement, whereas lower BMI (OR 0.87, p < 0.01) and greater preoperative score (OR 1.04, p = 0.04) were predictive of PASS achievement. For KOOS Jr., use of two sutures (OR 6.98, p < 0.01) was predictive of MCID achievement, whereas use of two sutures (OR 6.91, p < 0.01) and lower BMI (OR 0.81, p < 0.01) were predictive of PASS achievement. For VR12 Physical scores, younger age (OR 0.92, p = 0.04), lower BMI (OR 0.84, p = 0.01), and lower preoperative score (OR 0.89, p < 0.01) were predictive of MCID achievement, whereas use of two sutures (OR 7.24, p = 0.02), lower BMI (OR 0.92, p < 0.01), and greater preoperative score (OR 1.19, p = 0.02) were predictive of PASS achievement. Conclusions: This study defines the thresholds for MCID and PASS achievement at minimum two-year follow-up for IKDC, KOOS Jr., and VR12 Physical scores in a large cohort of patients undergoing isolated posterior-medial meniscal root repair at a single, high-volume institution. More patients achieved MCID than PASS. Lower BMI, use of fewer sutures, and preoperative scores were the most common predictors of MCID and PASS achievement. Additionally, younger age was an independent predictor of MCID achievement for VR-12 Physical Function.

Achievement of the patient acceptable symptom state following total shoulder arthroplasty with nonspherical humeral head and inlay glenoid

Background Total shoulder arthroplasty (TSA) with a nonspherical humeral head component and inlay glenoid is a bone preserving treatment for glenohumeral arthritis. This study aims to describe minimum two year patient reported outcomes, patient acceptable symptomatic state (PASS) achievement, and complications following TSA with this prosthesis. Methods A retrospective review of patients undergoing TSA with nonspherical humeral head and inlay glenoid was performed. Outcomes included Single Assessment Numeric Evaluation (SANE) scores, American Shoulder and Elbow Surgeons (ASES) scores, and complications. SANE and ASES scores were compared to established PASS threshold values to determine PASS achievement. Results 56 TSA in 53 patients were identified. The mean age was 64.5 years, 64% were male, and mean follow-up was 29.2 ± 4.9 months (24.0–42.8). Two complications (3.6%) were observed: one subscapularis tear requiring revision to reverse TSA and one traumatic minimally displaced greater tuberosity fracture successfully treated nonoperatively. The mean SANE score was 84.3 ± 16.9 (40–100) and 77% of patients surpassed the PASS threshold of 75.5. The mean ASES score was 85.3 ± 15.7 (40–100) and 77% of patients surpassed the PASS threshold of 76. Discussion Patients undergoing TSA with a nonspherical humeral head and inlay glenoid demonstrated high PASS achievement rates and few complications at short-term follow-up.

Gender-based differences in mid-term clinical outcomes and patient acceptable symptomatic state attainment after arthroscopic rotator cuff repair: Minimum 2-year follow up

ObjectiveThere is paucity of literature on the impact of patients’ gender on recovery and treatment success after arthroscopic rotator cuff repair. This study investigates the effect of gender on patient-reported outcomes preoperatively and postoperatively (minimum 2 years), and to determine if gender affects the attainment of patient-acceptable symptomatic state (PASS) thresholds. Methods266 patients (117 males, 149 females), who underwent primary arthroscopic rotator cuff repair for atraumatic, full-thickness tears, were included. Functional outcomes and pain scores were collected preoperatively and postoperatively. Percentage of attainment of PASS for the various outcome scores was calculated and compared between males and females. ResultsWomen had statistically significantly poorer functional outcome and pain scores preoperatively and at 1 and 2 years postoperatively (P ​< ​0.01). They also experienced less improvement in outcome scores throughout the postoperative period. Women had statistically significantly lower rates of PASS attainment at 2 years postoperatively. ConclusionWomen experience greater pain and poorer shoulder function compared with men preoperatively, and up to 2 years postoperatively. Shoulder Instability–Return to Sports After Injury Scale Shows That Lack of Psychological Readiness Predicts Outcomes and Recurrence Following Surgical Stabilization

PurposeThis study aimed to evaluate the predictive ability of psychological readiness to return to sports on clinical outcomes and recurrences in athletes who return to sports following shoulder instability surgery. MethodsA retrospective analysis was performed of patients who underwent shoulder instability surgery between September 2020 and October 2021 (arthroscopic Bankart repair or Latarjet procedure) with a minimum follow-up of 2 years. Patients were grouped according to the achievement of psychological readiness to return to play using the SIRSI scale (≥ 55 points) measured at 6 months following surgery. Recurrences were measured and functional outcomes were evaluated by the Visual Analogue Scale (VAS), Rowe, and Athletic Shoulder Outcome Scoring System (ASOSS). The minimal clinically important difference (MCID) for the VAS and Rowe scores was calculated using the distribution-based method of ½ standard deviation of the delta (difference between postoperative and preoperative scores). The patient acceptable symptomatic state (PASS) for the VAS scale was set at 2.5 based on previous literature. To evaluate the predictive ability of SIRSI a regression model analysis and a receiver operating characteristic (ROC) curve were used. ResultsA total of 108 who achieved psychological readiness (PSR) and 41 who did not (NPSR) met the study criteria. PSR achieved significantly higher percentages of MCID and PASS thresholds for VAS than NPSR (MCID: 68.5% vs 48.7%, p=0.026; PASS: 92.5% vs 58.5%, p<0.001). However, there were no differences in the percentage of patients achieving MCID for the Rowe score between groups (98.1% vs 100%, p=0.999). The only strongest independent predictor of postoperative outcomes was being psychologically ready to return to sports. The SIRSI scale had an excellent predictive ability for recurrences (area under curve 0.745, 95% CI 0.5-0.8). Of those who sustained a recurrence, 20% were not psychologically ready compared to 4.3% who were (p= 0.002). A power analysis was not conducted for this study. ConclusionThe SIRSI scale is associated with postoperative clinical outcomes and recurrences in patients who returned to sports following shoulder instability surgery. Patients who were not psychologically ready following shoulder instability surgery had worse clinical outcomes with fewer patients achieving clinically significant outcomes (PASS and MCID) for pain, and a higher risk of recurrence. Stratification of the minimal clinically important difference, substantial clinical benefit, and patient acceptable symptomatic state after total shoulder arthroplasty by implant type, preoperative diagnosis, and sex

Clinical significance, as opposed to statistical significance, has increasingly been utilized to evaluate outcomes after total shoulder arthroplasty (TSA). The purpose of this study was to identify thresholds of the minimal clinically important difference (MCID), substantial clinical benefit (SCB), and patient acceptable symptomatic state (PASS) for TSA outcome metrics and determine if these thresholds are influenced by prosthesis type (anatomic or reverse TSA), sex, or preoperative diagnosis. A prospectively collected international multicenter database inclusive of 38 surgeons was queried for patients receiving a primary aTSA or rTSA between 2003 and 2021. Prospectively, outcome metrics including ASES, shoulder function score (SFS), SST, UCLA, Constant, VAS Pain, shoulder arthroplasty smart (SAS) score, forward flexion, abduction, external rotation, and internal rotation was recorded preoperatively and at each follow-up. A patient satisfaction question was administered at each follow-up. Anchor-based MCID, SCB, and PASS were calculated as defined previously overall and according to implant type, preoperative diagnosis, and sex. The percentage of patients achieving thresholds was also quantified. A total of 5851 total shoulder arthroplasties (TSAs) including aTSA (n=2236) and rTSA (n=3615) were included in the study cohort. The following were identified as MCID thresholds for the overall (aTSA+rTSA irrespective of diagnosis or sex) cohort: VAS Pain (-1.5), SFS (1.2), SST (2.1), Constant (7.2), ASES (13.9), UCLA (8.2), SPADI (-21.5), and SAS (7.3), Abduction (13°), Forward elevation (16°), External rotation (4°), Internal rotation score (0.2). SCB thresholds for the overall cohort were: VAS Pain (-3.3), SFS (2.9), SST 3.8), Constant (18.9), ASES (33.1), UCLA (12.3), SPADI (-44.7), and SAS (18.2), Abduction (30°), Forward elevation (31°), External rotation (12°), Internal rotation score (0.9). PASS thresholds for the overall cohort were: VAS Pain (0.8), SFS (7.3), SST (9.2), Constant (64.2), ASES (79.5), UCLA (29.5), SPADI (24.7), and SAS (72.5), Abduction (104°), Forward elevation (130°), External rotation (30°), Internal rotation score (3.2). MCID, SCB, and PASS thresholds varied depending on preoperative diagnosis and sex. MCID, SCB, and PASS thresholds vary depending on implant type, preoperative diagnosis, and sex. A comprehensive understanding of these differences as well as identification of clinically relevant thresholds for legacy and novel metrics is essential to assist surgeons in evaluating their patient's outcomes, interpreting the literature, and counseling their patients preoperatively regarding expectations for improvement. Given that PASS thresholds are fragile and vary greatly depending on cohort variability, caution should be exercised in conflating them across different studies.

Determining minimal clinically important difference and patient-acceptable symptom state after arthroscopic isolated subscapularis repair

BackgroundMinimal Clinically Important Difference (MCID) and Patient Acceptable Symptomatic State (PASS) have emerged as patient-based treatment assessments. However, these have not been investigated in patients undergoing arthroscopic isolated subscapularis repair (AISR). The primary purpose of this study was to determine the MCID and PASS for commonly used patient-reported outcomes in individuals who underwent AISR. The secondary purpose was to assess potential associations between preoperative and intraoperative patient characteristics and the MCID and PASS. MethodsA retrospective analysis was conducted on prospectively collected data for patients who underwent primary AISR between 2011 and 2021 at a single institution, with minimum 2-year postoperative follow-up. Functional outcomes were assessed using the American Shoulder and Elbow Surgeons (ASES) score, Subjective Shoulder Value (SSV), and Visual Analog Scale (VAS) pain scale. The MCID was determined using the distribution-based method, while PASS was evaluated using AUC (Area Under the Curve) analysis. To investigate the relationship between preoperative variables and the achievement of MCID and PASS thresholds, Pearson and Spearman coefficient analyses were employed for continuous and non-continuous variables, respectively. ResultsA total of 77 patients with a mean follow-up of 58.1 months were included in the study. The calculated MCID values for VAS pain, ASES, and SSV were 1.2, 10.2, and 13.2, respectively. The PASS values for VAS pain, ASES, and SSV were 2.1, 68.8, and 68, respectively. There was no significant correlation between tear characteristics and the likelihood of achieving a MCID or PASS. Female sex, worker’s compensation status, baseline VAS pain score, and baseline ASES score, exhibited weak negative correlations for achieving PASS for VAS pain and ASES. ConclusionThis study defined the MCID and PASS values for commonly used outcome measures at short-term follow-up in patients undergoing AISR. Tear characteristics do not appear to impact the ability to achieve a MCID or PASS after AISR. Female sex, worker’s compensation claim, and low baseline functional scores have weak negative correlations with the achievement of PASS for VAS pain and ASES scores.

Safe and Effective Is Not Always Acceptable: The Case for PASS Scores in Foot and Ankle Orthopaedic Surgery

Category: Other Introduction/Purpose: Determining the success of a clinical intervention can be challenging even with modern patient-reported outcomes. Traditionally studies have used the Minimum Clinically Important Difference (MCID), the smallest difference in an outcome that is felt to have clinical significance. More recently some authors have emphasized the alternative model of the Patient Acceptable Symptomatic State (PASS), the threshold of a PRO defining where patients consider their overall status to be acceptable. PASS scores have not yet seen wide use in the foot and ankle literature, but may add insight to surgical outcomes beyond simply measuring the minimal changes implied by the MCID. Methods: The most universal patient-reported outcome in the foot and ankle literature is the simple Visual Analog Pain Scale (VAS), in which a patient rates their pain along an analog scale between 0 to 10 cm. Articles in Foot and Ankle International from December 2020 to August 2022 that reported VAS pain scores and variance following an intervention were included. Although specific PASS thresholds for VAS scores in many conditions in the foot and ankle have not been determined analytically, values ranging from 2.0 to 3.0 are typical for orthopaedic conditions, corresponding to the intuitive statement “I would like my pain to be less than 2 or 3 out of 10.” A PASS threshold of 2.0 has been used previously in the foot and ankle literature. The studies were therefore analyzed against PASS thresholds for the VAS of 2.0, 2.5, and 3.0. Results: Twenty-one studies were included. Five had multiple internal groups, yielding a total of 27 total sets of VAS scores.All 27 (100%) showed improvements well beyond the MCID and were considered successful. 17 (81%) of the 21 studies subjectively described their intervention as “reliable”, “good”, “effective” or “satisfactory”. Despite this, 8 (38%) of the 21 studies reported final mean VAS scores for all internal groups above a PASS threshold of 2.0. An analytically calculated weighted average final VAS pain score for all studies was 1.78 +/-1.89. Based upon this, the expected percentages of patients included in FAI studies whose results failed to meet VAS PASS thresholds were 45% for a threshold of 2.0, 35% for a threshold of 2.5, and 26% for a threshold of 3.0. Conclusion: Studies in the foot and ankle literature almost universally report positive results, both subjectively in their discussions and by statistically demonstrating improvement of patient-reported outcomes beyond the MCID. PASS threshold analysis yields a much less positive interpretation and emphasizes the number of negative outliers in the population who do not find their outcome acceptable. A fuller picture of the outcome of foot and ankle procedures and overall patient satisfaction can be gained from reporting both changes in the mean and comparing final outcomes to PASS thresholds.

Quantifying success after first revision reverse total shoulder arthroplasty: the minimal clinically important difference, substantial clinical benefit, and patient acceptable symptomatic state.

When patients require revision of primary shoulder arthroplasty, revision reverse total shoulder arthroplasty (rTSA) is most commonly performed. However, defining clinically important improvement in these patients is challenging because benchmarks have not been previously defined. Our purpose was to define the minimal clinically important difference (MCID), substantial clinical benefit (SCB), and patient acceptable symptomatic state (PASS) for outcome scores and range of motion (ROM) after revision rTSA and to quantify the proportion of patients achieving clinically relevant success. This retrospective cohort study used a prospectively collected single-institution database of patients undergoing first revision rTSA between August 2015 and December 2019. Patients with a diagnosis of periprosthetic fracture or infection were excluded. Outcomes scores included the ASES, raw and normalized Constant, SPADI, SST, and University of California, Los Angeles (UCLA) scores. ROM measures included abduction, forward elevation (FE), external rotation (ER), and internal rotation (IR) score. Anchor-based and distribution-based methods were used to calculate the MCID, SCB, and PASS. The proportions of patients achieving each threshold were assessed. Ninety-three revision rTSAs with minimum 2-year follow-up were evaluated. Mean age was 67 years, 56% were female, and average follow-up was 54 months. Revision rTSA was performed most commonly for failed anatomic TSA (n=47), followed by hemiarthroplasty (n=21), rTSA (n=15), and resurfacing (n=10). The indication for revision rTSA was most commonly glenoid loosening (n=24), followed by rotator cuff failure (n=23), subluxation and unexplained pain (n=11 for both). The anchor-based MCID thresholds (% of patients achieving) were as follows: ASES,20.1 (42%); normalized Constant,12.6 (80%); UCLA,10.2 (54%); SST,0.9 (78%); SPADI,-18.4 (58%); abduction,13° (83%); FE,18° (82%); ER,4° (49%); and IR,0.8 (34%). The SCB thresholds (% of patients achieving) were as follows: ASES,34.1 (25%); normalized Constant,26.6 (43%); UCLA,14.1 (28%); SST,3.9 (48%); SPADI,-36.4 (33%); abduction,20° (77%); FE,28° (71%); ER,15° (15%); and IR,1.0 (29%). The PASS thresholds (% of patients achieving) were as follows: ASES,63.5 (53%); normalized Constant,59.1 (61%); UCLA,25.4 (48%); SST,7.0 (55%); SPADI,42.4 (59%); abduction,98° (61%); FE,110° (56%); ER,19° (73%); and IR,3.3 (59%). This study establishes thresholds for the MCID, SCB, and PASS at minimum 2-years after revision rTSA, providing physicians an evidence-based method to counsel patients and assess patient outcomes postoperatively.

To define Minimally Clinically Important Difference (MCID) and Patient AcceptableSymptomatic State (PASS) threshold scores after open gluteus medius and/or minimus repair. Primary open gluteus medius and/or minimus repair patients from November 2013 to March 2020 were identified. Patient reported outcomes (PROs) were assessed preoperatively, 1- and 2-year follow-up, including the Hip Outcome Score Activities of Daily Living (HOS-ADL), modified Harris Hip Score (mHHS), International Hip Outcome Tool-12 (iHOT-12) and Visual Analog Scale (VAS) Pain. Thresholds for achieving a MCID and PASS postoperatively were calculated using the distribution method and receiver operator curve analysis; 25 patients (24 females, 1 male, age: 69 ± 6.8 years, body mass index: 26.9 ± 5.0 kg/m2) were included in final analyses. MCID threshold scores for HOS-ADL, mHHS, iHOT-12 and VAS Pain were calculated as 11.1, 6.2, 15.3 and 14.0, respectively. PASS threshold scores for each of the PROs were as follows: HOS-ADL (71.9), mHHS (60.0), iHOT-12 (49.2) and VAS Pain (36.8). MCID thresholds for HOS-ADL, mHHS, iHOT-12 and VAS Pain were achieved by 58.3%, 83.3%, 66.7% and 57.1% of patients, respectively. PASS thresholds for HOS-ADL, mHHS, iHOT-12 and VAS Pain were achieved by 52.4%, 44.8%, 65% and 59.1% of patients, respectively. Open gluteus medius and/or minimus repair results in a high rate of achievement of clinically significant outcomes at a minimum of 2 years postoperatively. MCID threshold values for HOS-ADL, mHHS, iHOT-12 and VAS Pain were 11.1, 6.2, 15.3 and 14.0, respectively. PASS threshold values for HOS-ADL, mHHS, iHOT-12 and VAS Pain were 71.9, 60.0, 49.2 and 36.8, respectively. The majority of patients achieved clinically significant outcomes with 81.3% and 77.3% achieving MCID and PASS for at least one PRO, respectively.

Quantifying Threshold Scores for Patient Satisfaction After Massive Rotator Cuff Repair for the Interpretation of Mid-Term Patient-Reported Outcomes

To establish minimal clinically important difference (MCID) and patient acceptable symptomatic state (PASS) values for 4 patient-reported outcomes (PROs) in patients undergoing arthroscopic massive rotator cuff repair (aMRCR): American Shoulder and Elbow Surgeons (ASES) score, Subjective Shoulder Value (SSV), Veterans Rand-12 (VR-12) score, and the visual analog scale (VAS) pain. In addition, our study seeks to determine preoperative factors associated with achieving clinically significant improvement as defined by the MCID and PASS. A retrospective review at 2 institutions was performed to identify patients undergoing aMRCR with minimum 4-year follow-up. Data collected at the 1-year, 2-year, and 4-year time points included patient characteristics (age, sex, length of follow-up, tobacco use, and workers' compensation status), radiologic parameters (Goutallier fatty infiltration and modified Collin tear pattern), and 4 PRO measures (collected preoperatively and postoperatively): ASES score, SSV, VR-12 score, and VAS pain. The MCID and PASS for each outcome measure were calculated using the distribution-based method and receiver operating characteristic curve analysis, respectively. Pearson and Spearman coefficient analyses were used to determine correlations between preoperative variables and MCID or PASS thresholds. A total of 101 patients with a mean follow-up of 64 months were included in the study. The MCID and PASS values at the 4-year follow-up for ASES were 14.5 and 69.4, respectively; for SSV, 13.7 and 81.5; for VR-12, 6.6 and 40.3; and for VAS pain, 1.3 and 1.2. Greater infraspinatus fatty infiltration was associated with failing to reach clinically significant values. This study defined MCID and PASS values for commonly used outcome measures in patients undergoing aMRCR at the 1-year, 2-year, and 4-year follow-up. At mid-term follow-up, greater preoperative rotator cuff disease severity was associated with failure to achieve clinically significant outcomes. Level IV, case series.

Quantifying Surgeon Intuition Using a Judgment Analysis Model: Surgeon Accuracy of Predicting Patient-Reported Outcomes in Patients Undergoing Hip Arthroscopy for Femoroacetabular Impingement Is Moderate at Best

To quantify surgeon intuition, determine whether a surgeon's prediction of outcomes after hip arthroscopy correlates with actual patient-reported outcomes (PRO), and identify differences in clinical judgment between expert and novice examiners. This prospective, longitudinal study was conducted at an academic medical center on adults undergoing primary hip arthroscopy for treatment of femoroacetabular impingement. A Surgeon Intuition and Prediction (SIP) score was completed preoperatively by an attending surgeon (expert) and physician assistant (novice). Baseline and postoperative outcome measures included legacy hip scores (e.g., Modified Harris Hip score) and Patient-Reported Outcomes Information System tools. Mean differences were assessed using t-tests. Generalized estimating equations assessed longitudinal changes. Pearson correlation coefficients (r) evaluated associations between SIP score and PRO scores. Data from 98 patients (mean age 36 years, 67% female) with complete data sets at 12-month follow-up were analyzed. Weak-to-moderate strength correlations were seen between SIP score and PRO scores (r= 0.36 to r= 0.53) for pain, activity and physical function. Significant improvements were seen in all primary outcome measures at 6 and 12 months postoperatively when compared to baseline scores (P < .05), with about 50% to 80% of patients achieving the minimum clinically important difference and patient acceptable symptomatic state thresholds postoperatively. An experienced, high-volume hip arthroscopist had only weak-to-moderate ability to intuitively predict PRO. Surgical intuition and judgment were not superior in an expert examiner compared to a novice. Level III, retrospective comparative prognostic trial.

Determining Clinically Meaningful Outcomes after Isolated Acetabuloplasty in the Context of Age and Arthritic Severity: A Retrospective Review of Prospectively Collected Data

AbstractOutcomes after isolated acetabuloplasty in mixed-type femoroacetabular impingement (FAI) patients can be favorable but are relatively unknown in the context of clinically important outcome variables (CIOVs) such as minimum clinically important difference and patient acceptable symptomatic state (PASS). Prognostic roles of age and osteoarthritis (OA) in hip arthroscopy are still unclear. We aimed to evaluate isolated acetabuloplasty outcomes including CIOVs while controlling for age and arthritis.This was a retrospective review of prospectively collected data. We analyzed patients with available 1-year follow-up for Patient-Reported Outcome Measurement Information System (PROMIS) scales for Physical Function (PF) and Pain Interference (PI). Patients were divided into two age groups (&lt; 40 years old vs. ≥ 40 years old) and two Outerbridge arthritis grade groups (grade 0–I vs. grade II–IV). Demographic, radiographic, and surgical characteristics as well as revision surgeries and complications were extracted. Our outcome was the proportion of patients that achieved values over literature-reported CIOV thresholds for PROMIS-PF and PROMIS-PI.A total of 63 patients of 96 eligible (65.6%) had minimum 1-year data. Characteristics were comparable between patients aged younger than 40 and 40 and over as well as between grade 0 to I and grade II to IV patients. Overall, average PROMIS-PF and PROMIS-PI scores improved. Approximately 81.0% of patients reached values over CIOV threshold (71.4% for PROMIS-PF, 54.0% for PROMIS-PI), with 44.4% reporting these outcomes for both PROMIS-PF and PROMIS-PI. Approximately 30.2% of patients reported values over PASS threshold for both PROMIS-PF and PROMIS-PI, with 25.4% of patients achieving values above all CIOV thresholds. Only one patient required revision surgery. When evaluating the effect of age while controlling for OA severity and vice versa, there were no significant outcome differences (p &gt; 0.615). When comparing patients reporting values above CIOV threshold versus those that did not, there were no differences in age (41.2 ± 13.0 [37.7, 44.8] vs. 45.9 ± 13.2 [38.4, 53.4], p = 0.265) or Outerbridge grade distribution (p = 0.177).This study uses CIOVs to describe clinically meaningful isolated acetabuloplasty outcomes for mixed-type FAI. Most patients over a wide spectrum of age and OA achieved favorable clinically meaningful outcomes following surgery. Isolated acetabuloplasty can alleviate symptomatology in mixed-type FAI patients on a case-by-case basis.

Time to Achieving Clinically Significant Outcomes After Meniscal Allograft Transplantation.

PurposeTo determine the time to achieving minimal clinically important difference (MCID) and patient acceptable symptomatic state (PASS) for commonly administered patient-reported outcome (PRO) measures and risk factors affecting achievement of clinically significant outcomes in patients undergoing meniscal allograft transplantation (MAT).MethodsA prospectively maintained MAT registry was retrospectively reviewed from April 2014 to May 2019. Patients who underwent revision MAT or did not complete preoperative PROs or one post operative time point were excluded. Patients who underwent concomitant procedures were included in the analysis. PROs were administered preoperatively and at 6 months, 1 year, and 2 years postoperatively. Previously defined MCID and PASS thresholds were utilized and Kaplan-Meier survival curve analysis with interval censoring was used to calculate the cumulative percentages of MCID and PASS achievement at each follow-up time interval (5-7, 11-13, and 23-25 months).ResultsEighty patients (age: 28.35 ± 9.76, 50% male) who completed preoperative, 6-month (n = 69, 86% compliance), and 1-year (n = 76, 95% compliance) PROs were included. The majority of patients (>50%) achieved MCID and PASS on most included PROs. Workers’ compensation status was found to significantly delay achievement of MCID and PASS on all PROs except for PASS on Knee Injury and Osteoarthritis Outcome Score (KOOS) quality of life (QoL). Higher body mass index (BMI) significantly delayed time to achieving MCID on KOOS Pain and activities of daily living (ADL), as well as PASS on KOOS Symptoms and KOOS QoL.ConclusionThis study suggests that the majority of patients have clinically significant improvements in pain and function after MAT, with more than 50% of patients experiencing clinically significant improvement within the first postoperative year. Workers’ compensation status and high BMI may prolong time to achievement of MCID and PASS after MAT.

Poster 209: Clinically Significant Outcomes and Survival of Meniscal Allograft Transplantation at Minimum 5-Year Follow-up

Objectives:In the meniscal deficient patient, meniscal allograft transplantation (MAT) has demonstrated promising early- and mid-term outcomes. Despite exhibiting improvement when compared to baseline values, outcomes at long-term follow-up have been shown to gradually decline secondary to graft failure and osteoarthritic development. However, it remains largely unknown if patient reported outcome measures (PROM) at long-term follow-up continue to meet thresholds for clinically significant outcome (CSO) measures such as minimum clinically important difference (MCID) and patient acceptable symptomatic state (PASS). The purpose of this investigation was to analyze CSOs after MAT at a minimum 5-year follow-up, while evaluating graft survival and overall patient satisfaction.Methods:A retrospective review of a prospectively collected, single-institution database spanning from 1999-2016 was performed to identify patients who underwent MAT using the bridge-in-slot technique and completed baseline PROMs with a minimum of 5-year follow-up. Baseline patient and surgical characteristics were collected. Preoperative and final follow-up PROMs were analyzed to determine whether patients met established MCID and PASS thresholds for International Knee Documentation Committee (IKDC; 9.9 and 36.0, respectively) and Lysholm (12.3 and 66.5, respectively) scores. Knee Injury and Osteoarthritis Outcome Score (KOOS) subscales were also collected, though not analyzed for CSO attainment. Failure was defined as revision MAT, conversion to unilateral or total knee arthroplasty, and macroscopic graft failure during second-look arthroscopy.Results:A total of 275 underwent MAT, of which 34% (n=95/275) completed PROMs with a minimum of 5-year follow-up (mean follow up, 8.5 ± 3.2 years [range, 5.0 – 16.8]). Mean patient age was 29.5 ± 9.9 years, with a mean symptom duration of 5.3 ± 5.5 years (Table 1). The majority of patients received concomitant procedures at the time of index MAT, including osteochondral allograft transplantation (43.1%) and anterior cruciate ligament reconstruction (20.0%) (Table 2). Mean PROMs at final follow-up significantly improved when compared to preoperative values for IKDC, Lysholm, and KOOS subscales (all P < .01; Figure 1). MCID and PASS for the IKDC survey were met by 77.8% and 38.8% of patients at final follow up, respectively. MCID for Lysholm score was achieved by 69.2% of patients, while only 2.6% met PASS (Table 3). Failure was reported in 20% (n=19/95) of patients, occurring at a mean 3.8 ± 3.0 years following surgery (Figure 2). Overall, 91% of patients with an intact meniscal allograft reported being satisfied at final follow-up.Conclusions:At a minimum of 5-year follow-up, significant improvements in PROMs were appreciated when compared to preoperative values. MCID was achieved in a total of 77.8% of patients for IKDC and 69.2% for Lysholm. PASS thresholds were met by 38.8% of patients for IKDC and 2.6% of patients for Lysholm. Failure was reported in 20% of patients. Outcomes of reverse shoulder arthroplasty following failed superior capsular reconstruction.

Superior capsular reconstruction (SCR) can be used for massive irreparable rotator cuff tears in the absence of significant degenerative changes; however, those who fail an SCR may require reverse shoulder arthroplasty (RSA). The effect of a previously performed SCR on outcomes following RSA remains unknown. Subjects who underwent RSA from May 2015 to January 2021 at 2 separate institutions were retrospectively identified through prospectively collected databases. Patients who underwent RSA after failed SCR were matched to those who underwent RSA after failed rotator cuff repair (RCR) based on the number of previous ipsilateral shoulder procedures (n=1, 2, ≥3) and secondarily by age within 5 years. American Shoulder and Elbow Surgeons Standardized Shoulder Assessment Form (ASES) score, Single Assessment Numeric Evaluation (SANE), visual analog scale (VAS) for pain, and Western Ontario Osteoarthritis of the Shoulder index (WOOS) scores were compared between groups. The minimal clinically important difference (MCID), substantial clinical benefit (SCB), and patient acceptable symptomatic state (PASS) thresholds were calculated to determine clinically relevant differences between groups. Forty-five patients were included (32 RSA following RCR, 13 following SCR). There were more smokers (P=.001) and worker's compensation cases (P=.034) in the SCR group. The RCR cohort was older (P=.007) and had a greater incidence of mental health (P>.999) and somatic disorders (P=.698), although these did not reach statistical significance. The mean follow-up for the RCR and SCR groups were 24.2±23.3 and 20.4±14.9 months following RSA, respectively (P=.913). The time from index RCR or SCR to RSA were 94.4±22.2 and 89.2±5.3 months, respectively (P=.003). Pre- and postoperative range of motion were similar between groups, as was the overall change in forward flexion (P=.879), abduction (P=.971), and external rotation (P=.968) following RSA. The RCR group had lower postoperative VAS pain (P=.009), higher SANE (P=.015), higher ASES (P=.008), and higher WOOS (P=.018) scores. The percentage achieving the MCID (P=.676) and SCB (P>.999) were similar; however, 56.7% of the RCR group met the SANE PASS threshold compared with 0.0% in the SCR group (P=.005). There were no differences in postoperative complications (P=.698) or revision rates (P=.308) following RSA between cohorts. When matched for number of previous procedures to the ipsilateral extremity and age, patients who underwent RSA following failed SCR had worse clinical outcome scores than their RSA following failed RCR counterparts. No patient in the SCR group met the SANE PASS threshold, whereas more than half of the RCR group did.

Paper 26: Defining Clinically Significant Improvement on Patient-Reported Outcomes Measurement Information System Test for Patients Undergoing Hip Arthroscopy for the Treatment of Femoroacetabular Impingement Syndrome at 1-Year Follow-Up

Objectives:While minimal clinically important difference (MCID), patient acceptable symptomatic state (PASS), and substantial clinical benefit (SCB) have been defined for hip-specific legacy patient reported outcome measures, they have not been defined for the Patient-Reported Outcomes Measurement Information System (PROMIS) instruments for patients undergoing hip arthroscopy. The purpose of the study was to define the MCID, PASS, and SCB thresholds for the PROMIS Physical Function (PF) computerized adaptive test (CAT) and PROMIS Pain Interference (PI) instruments in patients undergoing hip arthroscopy for femoroacetabular impingement syndrome (FAIS).Methods:Patients undergoing primary hip arthroscopy between August 2018 and January 2019 for the treatment of FAIS were retrospectively analyzed. Patients received PROMIS-PF, PROMIS-PI, Hip Outcome Score Activities of Daily Living, Hip Outcome Score Sport Subscale, modified Harris Hip Score, and international Hip Outcome Tool-12 preoperatively and at 1 year postoperatively. MCID was calculated using the distribution method, while PASS and SCB were calculated using an anchor-based method. Patients achieving clinically significant outcomes (CSOs) were compared to those who failed to achieve via chi-square and independent sample t-tests, and a multivariate logistic regression was conducted to determine predictors of CSO achievement.Results:124 patients with an average age of 32.7±12.3 were included in the analysis. The threshold scores required to achieve MCID, PASS, and SCB, respectively, were as follows: PROMIS-PI (-3.1, 53.7, 51.9) and PROMIS-PF (3.3, 47.0, 49.9). Patients achieved any MCID, PASS, and SCB for PROMIS scores at a rate of 89.0%, 71.8%, and 62.1%, respectively compared to 87.1%, 76.6%, and 71.8% for legacy PROMs. For PROMIS-PF, higher preoperative PROMIS-PF score was a positive predictor of CSO achievement, and patients achieving SCB were significantly younger (30.3±12 vs 35.6±12, p=0.017) with significantly lower BMIs (24.7±6.4 vs. 27.9±7, p=0.009). Preoperative chronic pain and past history of orthopaedic surgery were negative predictors of PROMIS-PI CSO achievement while higher (worse) preoperative PROMIS-PI scores were a positive predictor.Conclusions:Our study defined the MCID, PASS, and SCB for the PROMIS PF and PI CAT at 1-year postoperatively. CSO is achieved via PROMIS at a rate comparable, but slightly less than legacy hip PROMs. In addition, patients with higher preoperative PROMIS scores, younger age, and lower BMIs were more likely to achieve a CSO while preoperative chronic pain and past history of orthopaedic surgery were negative predictors of CSO achievement.

Defining the Patient Acceptable Symptom State Using the Forgotten Joint Score 12 After Hip Arthroscopy

PurposeTo contextualize the Forgotten Joint Score (FJS-12) by identifying a patient acceptable symptomatic state (PASS) threshold for patients undergoing hip arthroscopy and to investigate factors which correlated with postoperative FJS-12 score.MethodsAll patients who underwent hip arthroscopy for femoroacetabular impingement (FAI) under the care of a single surgeon between January 2018 and November 2019 were prospectively identified and included. Exclusion criteria were Tönnis classification grade 2 or greater. Data (including FJS-12, EuroQol-5 Dimension-5L [EQ-5D-5L], visual analog scale (VAS), and 12-item International Hip Outcome Tool (iHOT-12) scores) were available before surgery and at a minimum of 1 year after surgery. PASS was calculated using an anchor-based approach and receiver operator characteristic curve analysis. Pearson correlation analysis was used to correlate preoperative and postoperative factors with postoperative FJS-12 score.ResultsSeventy-seven patients (54 female, 23 male; mean age 30.3 years [standard deviation {SD} 8.2]) were included. Linked longitudinal follow-up data were available for 65 patients (84%) at a mean of 23.8 months (SD 6.4). Six patients required reoperation. Mean postoperative FJS-12 score was 46.5 (SD 33.1) and mean change in score was 27.2 (SD 30.6, P < .001). The PASS threshold for the FJS-12 was 38.5 (sensitivity 80%, specificity 88%), and the area under the curve was 0.852 (95% confidence interval 0.752-0.951). Overall, 53.8% of patients achieved this score. Postoperative FJS-12 score has moderate correlations with preoperative EQ-5D-5L, iHOT-12, and FJS-12 scores, and strong correlations with EQ-5D-5L, iHOT-12 and VAS scores after surgery.ConclusionsWe report a postoperative PASS threshold of 38.5 points for the FJS-12 after hip arthroscopy for FAI in a United Kingdom population. This value can act as a quantifiable target for clinicians using the FJS-12 to monitor patient outcomes in practice. FJS-12 has strong correlations with EQ-5D-5L, iHOT-12, and VAS at a minimum 12 months after surgery.Clinical RelevanceWe have calculated the patient acceptable symptomatic state of the Forgotten Joint Score to be 38.5 points at short-term follow-up. This can assist clinicians in determining whether surgical interventions related to hip arthroscopy for FAI are meaningful to the patient.

Time to Achieving Clinically Significant Outcomes after Meniscal Allograft Transplantation

Time to MCID and PASS is presented in Table 1. Previously reported PASS values for KOOS Pain, Symptoms, and Sport did not reach an AUC>0.70 and thus were not included in analysis. Worker’s compensation status was found to delay time to achieving MCID for all PROs (HR=0.238-0.305, P=0.008-0.020) and PASS for KOOS Symptoms (HR=0.171, P = 0.026) and IKDC (HR=1.88, P<0.001) (Table 2 and 3). Higher preoperative PRO score was associated with mildly delaying the time to achieving MCID for all PROs (HR=0.947-0.970, P<0.001), while higher preoperative PRO scores were associated with shorter time to achieving PASS on KOOS Symptoms and KOOS QOL (HR=1.030-1.043, P=0.001). Greater BMI (HR=0.946, P=0.020) and the number of focal chondral defects (HR=0.083-0.255, P=0.007-0.029) was associated with delayed MCID for KOOS ADL and QOL, respectively, while concomitant realignment surgery decreased time to MCID on KOOS Sports (HR=2.542, P=0.008).Conclusions:This study defined the time to achieving MCID and PASS on IKDC and KOOS subscores for patients undergoing MAT. Worker’s compensation status, BMI, and the presence of focal chondral defects may prolong time to achievement of MCID and PASS after MAT.

Determining Clinically Meaningful Thresholds for the Nonarthritic Hip Score in Patients Undergoing Arthroscopy for Femoroacetabular Impingement Syndrome

The purpose of this study was to improve the interpretability of the Nonarthritic Hip Score (NAHS) by determining the minimal clinically important difference (MCID), patient acceptable symptomatic state (PASS), and substantial clinical benefit (SCB) after hip arthroscopy for femoroacetabular impingement. The secondary aim was to identify variables associated with achievement of the thresholds. Patients who underwent hip arthroscopy for femoroacetabular impingement and completed postoperative questionnaires between August 2019 and March 2020 were included. Patients were excluded if they underwent previous ipsilateral hip surgery, underwent gluteus medius repair, or had a previous hip condition. The MCID, PASS, and SCB thresholds were calculated for the NAHS at minimum 1-, 2-, and 5-year follow-up. Distribution- and anchor-based methods with receiver operating characteristic analysis were used to determine the thresholds. Multivariate logistic regression was performed to determine predictors of achieving the MCID and PASS. The study included 343 hips with an average follow-up period of 48 months. On the basis of the distribution-based approach, the MCID was 8.7 using a method in which the standard deviation of the baseline score was divided by 2. By use of a method in which the standard deviation of the change score was divided by 2, MCID scores of 9.1, 8.3, and 12.6 resulted at a minimum of 1, 2, and 5 years, respectively. The PASS thresholds for these time points were 81.9, 85.6, and 81.9. The absolute SCB thresholds were 91.9, 94.4, and 93.1 and the change score thresholds were 30.6, 24.4, and 29.3 for a minimum of 1, 2, and 5 years, respectively. The preoperative NAHS was positively associated with achievement of the PASS and inversely related to achievement of the MCID. This study provides important clinical thresholds for the NAHS. These thresholds were determined for minimum 1-, 2-, and 5-year time points. The MCID was determined as 8.7, the PASS ranged between 81.9 and 85.6, and the absolute SCB value ranged from 91.9 to 94.4. The preoperative NAHS was found to be positively associated with achievement of the PASS and inversely related to achievement of the MCID. Level IV, retrospective case series.

Establishing thresholds for achievement of clinically significant satisfaction at two years following shoulder arthroplasty: the patient acceptable symptomatic state

BackgroundThe patient acceptable symptomatic state (PASS) represents clinical improvement following surgery that patients deem as satisfactory; however, thresholds for achieving this clinical benchmark have not yet been established beyond 1 year following primary and reverse shoulder arthroplasty. MethodsA prospectively maintained registry was retrospectively queried for patients undergoing either a primary TSA or RTSA at a single institution between November, 2015 and October, 2017. All patients completed the ASES, SANE, and Constant patient-reported outcome (PRO) scores at the 2-year time point. PASS threshold scores were calculated using anchor-based methodology with subsequent calculation of achievement rates. Multivariate logistic regression was used to identify demographic and clinical variables associated with the likelihood of PASS achievement. ResultsA total of 93 TSA (mean age 61.4 ± 8.5 years, 32.3% female) and 111 RTSA (mean age 70.0 ± 8.1 years, 51.4% female) patients met final inclusion criteria. One-hundred sixty-five patients (80.8%) responded to the binary anchor question as “satisfied” regarding their surgical outcome. The PASS cutoffs for ASES, SANE, and Constant were 81.7, 85.7, and 24.0, respectively (AUC > 80%), with PASS achievement rates ranging from 50.7% to 59.9% for the entire study population and 66.7% to 83.3% for a risk-stratified subgroup. Patients undergoing TSA had a significantly higher PASS achievement rate relative to RTSA (P = .010) for ASES. Independent risk factors for decreased PASS achievement included lower preoperative PRO score, prior shoulder surgery, sedentary lifestyle, smoking, workers’ compensation status, and diabetes mellitus (DM). ConclusionThe majority of patients reach the PASS benchmark by 2 years after shoulder arthroplasty, with an optimal subgroup of risk-stratified patients achieving PASS at a rate greater than 80% for ASES. Patients undergoing TSA have a higher likelihood of reaching the PASS than patients undergoing RTSA, independent of other variables. Lower baseline outcome scores, prior shoulder surgery, smoking, and sedentary lifestyle are associated with lower PASS rates after TSA. Worker's compensation status and comorbid DM are predictive of reduced PASS rates after RTSA. Primary diagnosis and glenoid morphology did not have a significant impact on the rate of achieving the PASS. This information is valuable for patient selection and counseling prior to shoulder arthroplasty. Level of EvidenceLevel III; Prognostic Study; Retrospective Case Series.
