Osteomyelitis and Pathologic Fracture of Mandible in Undiagnosed Osteopetrosis.

Osteopetrosis (OP) is a heterogeneous group of rare, heredity bone disorders with variable clinical features involving the bones of the body. OP is characterized by increased bone density, which is caused by aberrant osteoclast-mediated bone resorption. This syndromic disorder comes with a series of problems and, unless recognized and treated early, can lead to a multitude of further grave complications. We report a rare case of a female patient who reported chronic unhealed extraoral draining sinus present over the left submandibular region with pathologic fracture of the left mandibular angle, which, if, was diagnosed early with the identification of the osteopetrosis syndrome, could have been managed more conservatively.

Bilateral pathological fracture of mandible associated with osteoradionecrosis

AbstractPathological fractures comprise <2% of mandibular fractures. Common aetiological factors include surgical procedures, osteonecrosis of the jaw, infection and tumours (Int J Oral Maxillofac Surg 27: 186–190, 1998). In preparation for radiotherapy, prophylactic extractions are carried out in advance to achieve complete mucosal coverage prior to commencing radiotherapy. We report a case of bilateral pathologic fractures of the mandibular angle in a patient 2 months after completion of radiotherapy who had undergone surgical removal of bilateral mandibular wisdom teeth a fortnight prior to radiotherapy.

Osteomyelitis of the mandible leading to pathological fracture in a tuberculosis patient: A case report and review of literature

Osteomyelitis is an inflammatory condition of the bone, beginning in the medullary cavity and haversian systems and extending to involve the periosteum of the affected area. Although other etiological factors, such as traumatic injuries, radiation, and certain chemical substances, among others, may also produce inflammation of the medullary space, the term osteomyelitis is mostly used in the medical literature to describe a true infection of the bone induced by pyogenic microorganisms. Tuberculous osteomyelitis is an uncommon disease entity. Here, we present a case of osteomyelitis of mandible leading to pathological fracture of mandible, in which pulmonary tuberculosis involvement was detected only subsequently. The oral physicians thus play a crucial role in recognizing high-risk patients and initiating prompt isolation and evaluation.

Unusual oral complications of herpes zoster infection: Report of a case and review of literature

A case of herpes zoster infection with unusual oral complications involving the mandibular division of the trigeminal nerve is presented. The post-herpetic complications of osteonecrosis, spontaneous exfoliation of teeth, and subsequent pathologic fracture of mandible in the absence of concurrent predisposing factors in a 65-year-old man are demonstrated. Forty-one cases with osteonecrosis and spontaneous exfoliation of teeth previously presented in the literature are reviewed. This is the first report of pathologic fracture after herpes zoster infection.
