Factors Related to The Utilization of Integration Health Program (Posbindu) Among Older Adults in Indonesia: A Scoping Review.

Indonesia is experiencing an increase in the elderly population, which is a challenge for health services. One of the health programs specifically designed for older people is the integration health program, known as "Posbindu". Posbindu is a community-based health service that provides health promotion, disease prevention, and early detection services for the elderly. However, its utilization has not been maximized, so a study was conducted to analyze the factors related to the utilization of Posbindu in Indonesia. The research design used a scoping review and data analysis narrative approach. The researcher searched articles using the PCC framework, with the elderly as the population (including pre-elderly), the factors as concept, and the context as utilization of Posbindu. It used Indonesian and English keywords in the Google Scholar, Pubmed, ProQuest, and Researchgate databases. Then, the article selection process used the PRISMA-ScR flow chart, and 201 articles were obtained that matched the inclusion criteria, 192 articles, and the final result was nine articles were analyzed. The results of the article analysis showed that the average value of older people's participation in Posbindu was 47.51%, which means it did not reach the target. Factors related to this are predisposition factors found in eight articles (intentions, knowledge, attitudes, education, traditions, beliefs, and control over actions), enabling factors in five articles (distance to Posbindu, access to information, and quality of Posbindu for older people), and reinforcing factors in seven articles (family support and health voluntary support). From all p-values for these factors, the most strongly related to Posbindu for the elderly was reinforcing elements, with a p-value of less than 0.03 in all analyzed articles. So, it can be concluded that the reinforcing factor is the most related to the utilization of the elderly Posbindu. So, it is crucial to involve all sector to increase the participation of older people in Posbindu program.

The participation of hard-to-reach older people in the research and development process of health technologies from the perspective of multipliers-A qualitative analysis.

The participation of older people in research and development processes has long been called for but has not been sufficiently put into practice. In addition, participation is often late and not particularly intensive, so that certain older groups of people are underrepresented in the development of health technologies (HT). Heterogeneity, e.g., between urban and rural populations, in access to and motivation for participation is also rarely taken into account. The aim of this study was to investigate form and phases of participation for hard-to-reach older people in the research and development process of HT. The qualitative study among multipliers was conducted using focus groups and telephone interviews and took place in a city and an adjacent rural area in northwestern Lower Saxony, Germany. A content analysis of the data was undertaken using deductive-inductive category formation. Seventeen participants (13 female) took part in the study (median age 61, 33-73). Participants from both areas identified particular forms and phases of participation in the research and development process. Longer forms of participation for hard-to-reach groups and the development process of technologies for older people from the rural area were viewed as challenges. Passive and active access strategies are needed to achieve sufficient heterogeneity in the research and development process. Trusted multipliers can play an important role in gaining access to hard-to-reach older people, but also during the research process. Apart from facilitating factors (e.g., age-specific study materials), inhibiting factors such as contact anxieties are also indicated. Only urban participants mention financial/material incentives and community as possible motivations. The results provide important insights from the perspective of multipliers. They show specificities in access and participation for rural areas and for hard-to-reach older people. Many older people may have uncertainties about research projects and HT. Multipliers can assume a key role to help reduce these uncertainties in the future.

The effect of outdoor activities on the medical expenditure of older people: multiple chain mediating effects of health benefits

BackgroundWith the global aging population, attention to the health and medical issues of older adults is increasing. By analyzing the relationship between older people's participation in outdoor activities and medical expenditure, this study aims to provide a scientific basis for improving their quality of life and reducing the medical burden.MethodsData on outdoor activity participation, medical expenditures, and relevant variables were collected through questionnaires and databases. A multi-chain mediation effect model was established to analyze the impact of outdoor activities on the medical expenditure of older people, considering mediation effects and heterogeneity.ResultsResults revealed that increased participation in outdoor activities among older adults correlated with lower medical expenditures. Outdoor activities positively influenced their health by improving mental health, cognition, eating habits, and activities of daily living, resulting in reduced medical expenditures. Robustness tests confirmed the consistent effect of outdoor activities on older people's medical expenditure.ConclusionThese findings contribute to understanding the relationship between outdoor activities, health, and medical expenditure in older people, guiding policy formulation and interventions. Encouraging and supporting older adults in outdoor activities can enhance their quality of life and alleviate medical resource strain. The study's conclusions can also inform health promotion measures for other populations and serve as a basis for future research in this area.

Long-Term Care Residents' Perceptions of Integrated Care From Physio-Psycho-Social Perspectives: A Qualitative Inquiry.

In response to the aging population, an integrated care policy has been put forward and implemented in China. The key aspect of this policy is the reform of services within long-term care facilities, representing a significant shift and innovation. This study aims to explore the perceptions and experiences of integrated care among older residents living in long-term care facilities. A descriptive qualitative design was applied in our study. Utilizing a purposive sampling method, 18 older adults from 5 long-term care institutions in Shanghai, China were selected. Data were collected through semi-structured in-depth interviews from October 2022 to April 2023, continuing until data saturation was reached. Directed content analysis was applied to analyze the interview data, guided by the Fundamentals of Care (FoC) Framework. Five themes and 11 subthemes were identified, including (1) Policy level: positive attitude and negative feelings. (2) Physiological level: satisfied basic daily life, primary medical services accessibility and chronic care management enhancement needed. (3) Psychological level: need for psychological support and need for self-perception recognition. (4) Relationship level: enhancement of humanistic care and need for a family atmosphere. (5) Social level: interpersonal communication constrained by the times and inadequate social engagement. Strengthening the awareness and participation of older people in the integrated care policy, and fully meeting the diverse needs including advanced medical care, chronic disease management, personalized life care, psychological support, humanistic care and social engagement will contribute to the improvement of the policy, so as to better adapt the demographic shift.

Educational activities of older people in Poland, Czechia and Slovakia

Population ageing is just as important a modern phenomenon as it is global. However, the process is not uniform on a scale of continents and within Europe, with the latter exhibiting the most advanced and widespread population ageing. Nowhere is it as dynamic as in the former Eastern Bloc. It is true for Poland, Slovakia, and Czechia as well. The researches involves pilot surveys in the three countries of interest. The part of the survey results discussed here covers the educational activity of the population aged 60 and over from rural areas. Rural areas were chosen for the study because, despite the abundance of learning opportunities for older people in Poland, Czechia, and Slovakia, rural areas have much less to offer in terms of education, also regarding seniors. Hence, it is necessary to identify the opinions and needs of the older rural population. Participation of older people in any form of education is a critical component of active ageing. Importantly, learning is a way for older people to be active in society. The educational activity is not just a fun way to spend time for seniors, but it can help improve their social participation, fend off ageism, and enhance the social and cultural capital of local communities. Still, for educational activity to play all these roles, a broad educational offer is needed fit for specific needs and capabilities of all groups of society, including rural seniors.

USING TRANSDISCIPLINARY CO-PRODUCTION TO GAIN INSIGHTS INTO SPATIAL JUSTICE AND AGE-FRIENDLY CITIES

Abstract Globally, a popular policy approach to address the two interrelated challenges of population ageing and urbanisation has been to adopt the World Health Organization’s Age-Friendly City framework, a model that can help to identify and address barriers to the well-being and participation of older people. This paper reports early findings from the ‘Ageing in Place in Cities’ project. The research was conducted from a critical gerontology perspective and embedded a spatial justice perspective by centring the concepts of equity, diversity and co-production in our underpinning theoretical framework. Discussing findings from seven case study cities (all who were early adopters of the Age-Friendly Cities framework), this paper discusses how a transdisciplinary co-production approach has been used to gain greater understanding of age-friendly initiatives in each city. Using mixed methods, we first developed descriptive profiles for each city using both census and other local demographic data to explore how the populations have changed over time, and narratives and policy documents detailing the initiatives used to support ageing in place. Second, we conducted semi-structured interviews with a variety of actors in each city to examine how the cities delivered age-friendly change. We integrated the data using a comparative case study approach to draw out insights across the cities. Our findings reveal the role of key leaders and stakeholders in age-friendly cities and will be of interest to policy makers and academics interested in population ageing and urban change. Further, our transdisciplinary co-production methods present a novel approach to conducting age-friendly research.

Health and work-related factors as predictors of still being active in working life at age 66 and 72 in a Swedish population: A longitudinal study.

Health and work environment are known factors in being active in working life beyond legal retirement. To investigate sociodemographic, health and work environment factors as possible predictors of being active in working life at ages 66 and 72. Secondly, investigate eventual changes over time, shortly after a major reform in the Swedish pension system, and predictors of still being active in working life at age 66. We used a longitudinal design with two separate cohorts of people at age 60. One baseline assessment was made in 2001-2003 with two 6 years follow-ups, and one in 2007-2009 with one 6 years follow-up. Data were accessed through a Swedish national population-based study and analysed using logistic regression. To examine possible differences between the two cohorts, interaction terms with each independent variable were analysed. Being a man and working in a profession that requires at least three years of university education predicted that the person would still be active in working life at age 66 and 72. Additionally, having a light level of physical activity at work and being diagnosed with fewer than two diseases, also predicted still being active in working life at age 66. Only physical activity at work showed significant changes over time. Shortly after a major reform of the public pension system, there was an increase in participation in working life after age 66 and 72. However, gender, profession, and health factors are still important considerations regarding older people's participation in working life.

Social participation in older people with hearing impairment in Chinese community: A latent profile analysis

ObjectivesHearing impairment may lead to increased communication difficulties for older people, making their social participation less optimistic. However, there is little research on the social participation of older people with hearing impairment, especially based on the characteristics of their social participation. This study aimed to identify different social participation profiles in older people with hearing impairment and to explore sociodemographic characteristics, disease-related characteristics and psychosocial factors with different social participation profiles. MethodsA cross-sectional study of 300 older people with hearing impairment using the sociodemographic questionnaire, the Impact on Participation and Autonomy Questionnaire, the Lubben Social Network Scale-6, Medical Outcomes Study Social Support Survey and Geriatric Depression Scale-15 from May to August 2023 in a community of Beijing, China. Latent profile analysis was used to analyse the latent profiles of social participation in elderly with hearing impairment. Multiple logistic regression was used to explore the predictors of different profiles. ResultsThe social participation of older people with hearing impairment in the community can be classified into three potential profiles: Profile 1 - high social participation group (76.05 %), Profile 2 - moderate social participation group (17.34 %), Profile 3 - low social participation group (6.61 %). Age, types of chronic diseases, self-reported health, severity of hearing impairment, social network, social support and depression were predictors of different profiles. ConclusionsNurses should pay attention to the characteristics, depression, social network and support of older people with different hearing impairment to improve social participation in different profiles. Implications and recommendationsThis was the first study exploring latent profiles of social participation in older people with hearing impairment. Insights from this study are useful for gerontological nursing to distinguish different profiles and further identify the characteristics of different profiles in older people with hearing impairment by characterizing the level of social participation in the community and better implement interventions according to profiles.

Pragmatic Evaluation of a Low-Threshold Sports Program for Older Adults in Group Homes.

Physical activity (PA) reduces with older age, ill health, and disability. For these groups, guidance recognizes the benefit of small increases in PA and low-intensity PA. This study evaluated a low-threshold intervention that addressed known barriers to older people's participation in PA in residential care and sheltered housing. Ten, competitive sport sessions were delivered by coaches at 49 sites with the aim that they be sustained in-house. Using quasi-experimental methods, participants reported reduced sitting time, increased moderate/vigorous PA, increased participation in sports and improved scores for both health quality of life and fear of falling at 6months. The program engaged 29% of residents and was sustained at 50% of sites at 8months. The findings suggest that low-threshold sports programs that overcome known barriers to older people's participation in PA have the potential to provide a gateway to increased PA in group homes and to be sustained in-house.

Ethical conditions for research with the elderly: a narrative review [Version 1; Peer Review - In Progress

Introduction: the population aging raises the need for the inclusion of older people in research, and therefore, the review of ethical guidelines that grant protection to this age group, especially in conditions of vulnerability. Objective: to find out about the ethical conditions involved in research with older people and the protection they should be afforded as research subjects. Methods: narrative review of 15 original articles published between 2001 and 2021, indexed in the PUBMED, Web of Science, CINAHL, and SCOPUS databases. The descriptors used were Ethics in Research and Aged/ Elderly. Results: the main findings point to the poverty of research on aging issues and unjustified discrimination in the participation of older people in clinical studies. An update is provided on the ethical conditions for research on older people, according to the requirements set out by Emanuel. Conclusions: the need to include older people in clinical research is imperative, for which it is necessary to integrate advances in Chilean legislation that allow the participation of this age group in different situations of vulnerability while maintaining the conditions of protection contemplated in international ethical guidelines and agreements. Participatory research and methods that include the community can be an opportunity to approach research within a framework of respect and citizen education

The article analyzes the social activity and participation of older people in the context of active ageing. The research task was to reconstruct the repertoire of social activity/ participation of today’s older Russians and the meanings they put into this activity, and to answer the question of how this activity fits into the concept of active aging (longevity). In the analysis, we focused on practices and the meanings attributed to them among the two age groups of informants. The results obtained were compared with the Active Aging Index (AAI) as the most popular tool for determining the activity of the older people. The empirical base consisted of 28 in-depth interviews with people over 60 years (14 with working and 14 with non-working). The data of our study demonstrate a much richer repertoire of social activity and participation of the older than the that is supposed to be calculated within the framework of the AAI. The repertoire of activity is influenced by age after 75+ forms of activity becomes less and it drifts towards private life (family, caring for grandchildren and relatives) «dacha» deals, leaving of lost opportunities. The older age 75+ do not try to return to their previous activities, but, on the contrary, strive for a daily routine and involvement in family affairs, as well as various daily activities and hobbies that allow them to feel involved in social life. Postponement of aging in the age 60+ group is associated with maintaining activity at the level of middle age and active replacement of lost activities of the employment. The prescriptive and formalized nature of the indexes (in particular, AAI) does not take into account the really existing wide repertoire of social activity and participation of the older, and also does not allow reconstructing individual meanings and values of activity and linking it all with overall life satisfaction. &nbsp

Participation of older people in learning studies

The participation and meaningful engagement of older people are strongly supported because of their individual and communal benefits. Currently there is a lack of general understanding of how older people participate in research activities. The purpose of this review was to examine the ways older people participate in learning studies. A search of abstracts of empirical studies published in English was conducted in three databases between 2015 and 2019 using scoping review methodology. The results showed that most often older people did participate as study subjects in clinical studies. Other participant roles included informants, partners, and multiple roles. The review addressed a paucity in qualitative and participatory roles in older people’s learning studies. All participant roles are still needed to provide various standpoints for learning studies. Further studies are suggested to provide various meaningful and participatory ways for older people to get involved in research activities.

The Configuration of Older Users as Drivers of Innovation in the Design of Digital Technologies

This paper develops hypotheses on the discovery of older people as "users" in the publicly funded development of digital technologies and underlying innovation policy motives. We then describe the effects of this innovation policy on the development of products and older people as their users in the context of Ambient Assisted Living (AAL). To this end, we reconstruct the involvement of users in two AAL funding programs, one at the European level and one at the national level (Germany). Based on this, we discuss the resulting consequences by describing how older people are configured as users in technology development with a focus on the concept of user-centered design (UCD) and what this configuration means for the developed technologies as well as for the older users. We describe how the participation of older people in technology development projects is a complex task that is not without controversy in social science research on user participation. We conclude by arguing for alternative technology development strategies and funding practices.

P08-16 Neighborhood walkability and older people's participation in leisure activities

BackgroundThe neighborhood environment may enhance or restrict older people's opportunities to participate in leisure activities, and thus impact quality of life. Walkability depicts the environment's suitability for walking to different destinations. Little is known concerning about the relation between environment walkability and participation in leisure activities. Our purpose was to study whether neighborhood's objective and perceived walkability were related to participation in various leisure activities outside the home.MethodsCross-sectional data of LISPE consisted of 848 community-dwelling people aged 75-90 living in the municipalities of Jyväskylä and Muurame, Finland. Participants' home addresses were geocoded, and walkability index (mixed land-use, street connectivity, and population density) was calculated using geographic information system and categorized into tertiles. From a checklist, participants chose all infrastructure-based facilitators present in their neighborhood, which draw them to outdoor mobility (e.g., good lighting, services close); the sum of these was used as an indicator of perceived walkability. Participation in leisure activities outside the home was self-reported based on frequency and classified as participation (yes/no) in group activities (≥1x/week), physical activity (≥1x/week), and non-group cultural and other activities (≥1x/month). For each leisure activity type, logistic regression models were conducted for walkability index and perceived walkability separately. Analyses were adjusted age, sex, years of education, weekly car use, walking difficulties, and number of chronic conditions.ResultsLogistic regression showed that the older people who lived in areas with high walkability index were more likely to participate in cultural and other activities activity at least once a month (OR = 1.64, 95% Cl = 1.14-2.36) and less likely to participate in physical activity at least once a week (OR = 0.61, 95% Cl = 0.39-0.94, fully adjusted model) than older people living in areas with low walkability index. Older people reporting higher perceived walkability participated more often in physical activity than those who reported lower walkability (OR = 1.14, 95% Cl = 1.05-1.23).ConclusionsLiving in the area of high walkability enhanced participation in cultural activities and decreased participation in physical activity. However, higher perceived walkability may motivate older people to be physically active. Environment which offers good infrastructure for outdoor mobility provides opportunities to participate in specific leisure activities.

Impact of Built Environment and Neighborhood on Promoting Mental Health, Well-being, and Social Participation in Older People: an Umbrella Review.

The built environment, especially in the context of the neighborhood, affects older people's health. This umbrella review aims to summarize the associations between factors and interventions in the built environment as regards modifying or improving mental health, well-being, social inclusion and participation in the elderly. We searched articles in the following databases: PubMed, Embase, Cochrane Library, Scopus, Avery Index, Sage, Web of Science, Health Evidence, and Google Scholar, without any time limits. The factors and interventions examined have been classified into three categories (urban infrastructure, green infrastructure, built environment), and we have assessed their relationships with each of the health outcomes. Eight reviews have been included. The results show a positive association between factors and interventions and health outcomes, even though this tends to differ with respect to some of the elements (study design, sample size, built environment and health outcome measurements, and the quality of the primary studies included in the reviews) in the selected studies. In conclusion, the present study suggests and confirms that acting on the built environment has a positive impact on mental health and social inclusion.

The Participation of Older People in the Development of Group Housing in The Netherlands: A Study on the Involvement of Residents from Organisational and End-User Perspectives

The establishment of new housing initiatives for older people begins with the participation of (future) residents. This study explored how participation is experienced by both facilitators and (future) residents and what lessons are learned regarding the facilitation of meaningful participation. Participation was studied through semi-structured interviews and focus group sessions from the perspective of 34 (future) residents and facilitators involved in participation processes in a diverse set of four housing projects from the Netherlands. The results focused on three phases: the initiation phase, the concepting and development phase, and the transition towards an established form of group housing. From the outset of such processes, it was important to involve all relevant stakeholders and to create a shared vision about the participation process. Discussions in small groups, the use of references, creative elements, and the creation of the right atmosphere were experienced as valuable during the concepting and design phase. In the third phase, the role of the organisation and residents needed to be discussed again. Participation should be a continuous process, during which trust, communication and having an open attitude are key. This study showed how innovative approaches can contribute to the creation of an environment in which older people can impact the actual design of housing, and make it more inclusive.

Volunteer Service and Well-Being of Older People in China.

PurposeThe social support theory suggested that involving older people in social activities could increase their level of social participation and interaction, which in turn improved their well-being. However, there has been a heated controversy about whether participating in volunteer services could enhance the well-being of older people, especially for the Chinese sample.MethodBased on the data from the China Health and Retirement Longitudinal Study (CHARLS) in 2013, this paper used an ordered probit model to examine the impact of older people's participation in volunteer services on their well-being, as well as the differences in the impact across groups and the specific transmission mechanism.ResultThe empirical study found that Chinese older people's participation in volunteerism significantly enhanced their well-being, which remained robust after eliminating the possible effects of self-selection. Further heterogeneity analysis revealed that for female, non-party members and older adults with good economic status, participation in volunteerism has a higher increase in well-being. The mediating effect test indicated that older people's participation in volunteerism affected well-being mainly through enhancing positive emotions.ConclusionIt is necessary to promote the participation of older people in volunteer services and to clarify the role of government support and advocacy. Proper guidance is given to change the role of older people as care recipients to that of service providers and caregivers, and to continuously enrich the programmes and content of volunteer service to safeguard the well-being of older people.

Relationship between volunteer group participation and depressive symptoms in older Japanese: A 3-year JAGES longitudinal study using propensity score matching

Objectives In this study, we aim to clarify the optimal threshold for the frequency of volunteer group participation among older people aged 65 years and above, which is expected to prevent the risk of developing depression.Methods We utilized longitudinal data from 2013 to 2016, collated by the Japan Gerontological Evaluation Study for people aged ≥65 years living in 24 municipalities and not certified as requiring long-term care. In addition, those who were not depressed in 2013 (≥5 points on the Geriatric Depression Scale-15) were followed up for 3 years and classified according to the frequency of participation in the volunteer group in 2013 (≥once per year, ≥once per month, ≥once per week). The odds ratio (OR) of being newly depressed in 2016 was determined using the propensity score matching method and t-test.Results The frequency of volunteer group participation for 9,722 (25.0%), 6,026 (15.5%), and 2,735 (7.0%) older individuals was≥once per year, once per month, and once per week, respectively. A comparison of the balanced attributes of the volunteer group participation with those of the non-participation group using propensity scores revealed that the risk of developing depressive symptoms was significantly lower in the former compared with the latter for the frequency of ≥once per month and OR of 0.82 (95% confidence interval: 0.72, 0.93). The ORs were 0.92 (0.83, 1.02) and 0.82 (0.68, 1.00) for the ≥once per year and once per week groups, respectively.Conclusion Older people's participation in a volunteer group ≥once per month was effective in reducing the risk of developing depressive symptoms after 3 years. This suggests that increasing the opportunities and community places where older people can be involved as volunteers even once a month may be employed as an effective measure to prevent depression.

Older Men's Participation in Cognitive Training: A Review.

Background: Cognitive training for healthy older adults living in a community is an essential resource that allows them to live at home for as long as possible. Objective: The purpose of the review is to examine the degree of participation of males and females in longitudinal studies of cognitive training. Moreover, we want to identify if these studies include the gender variable in their analyzes or reflect on its importance. Method: This review considered longitudinal cognitive training studies were published in English and Spanish and conducted with healthy older adults living in a community. Results: The Advanced Cognitive Training for Independent and Vital Elderly (ACTIVE) study (in English) and the Memoria Mejor (MM) Longitudinal Study (in Spanish), both illustrate the trend of sex/gender treatment of the studies reviewed: a) high participation of older people seventy and more years b) recruiting stratified by age and sex; c) males are disproportionately underrepresented in cognitive training studies [24% - 14%]; d) the evaluation measures (baseline, follow-up, and final) and dropout data are provided but not stratified by age and sex/gender. Conclusions: Researchers demonstrate awareness about the impact of sex/gender differences but do not focus on it. Understanding sex/gender differences are necessary for understanding not only that these differences occur, but also why they occur; this will allow policies or intervention programs with approaches that are more equitable for both sexes/genders to be formulated.

Risk of all-cause mortality and its association with health status in a cohort of community-dwelling older people: FIBRA study.

This article aims to estimate the risk of death according to sociodemographic characteristics, chronic diseases, frailty, functional capacity, and social participation in older people as well as determine the median time of death in relation to health status and social participation. A retrospective longitudinal study was conducted with older people (≥65 years) in 2008-09 and 2016-17 in the city of Campinas and the subdistrict of Ermelino Matarazzo in the city of São Paulo. Face-to-face interviews were conducted at community centers and the participants' homes. The cumulative incidence of death was estimated and associations with the predictor variables were analyzed using Poisson multiple regression. The Kaplan-Meier method and the log-rank test were also used. Among the 741 individuals located at follow-up, 192 had deceased. The incidence of death was higher among those who reported having heart disease and those who were dependent on others regarding the performance of instrumental activities of daily living. The incidence of death was lower among women, individuals in the highest income stratum, and those who performed three or more activities related to social inclusion. No differences in median survival times were found. Predictors of mortality can contribute to broadening knowledge on the singularities of the aging process.
