Efficacy of parent-child interaction therapy for children born premature.

Premature children are known to be at a high risk of developing behavioral problems. This study examined the effectiveness of parent-child interaction therapy (PCIT) in reducing behavioral problems in young children born premature. The study included 18 child-parent pairs with children born at less than 35 weeks of gestation (range: 23-34 weeks, median: 31.0 weeks) and aged 27-52 months (median: 38.0 months). They were assigned to either the PCIT group (n = 7) or the non-PCIT group (n = 11) based on maternal desire for treatment. The study was designed to examine the effects of PCIT. Specifically, the Eyberg Child Behavior Inventory (ECBI) intensity score, ECBI problem score, and Parenting Stress Index Short Form (PSI-SF) scores were compared before treatment and after 6 months. In the PCIT group, the mean ECBI intensity score was 135.7 (SD = 13.5; T-score = 64) at baseline and 90.1 (SD = 15.5; T-score = 46) at post-assessment, the mean ECBI problem score was 9.8 (SD = 1.9; T-score = 54) at baseline and 4.4 (SD = 3.1; T-score = 44) at post-assessment, the mean PSI-SF total score was 60.1 (SD = 4.8; 95%tile) at baseline and 49.6 (SD = 5.6; 85%tile) at post-assessment, showing a significant improvement (ECBI intensity scores: p < 0.001, d = 2.03; ECBI problem scores: p < 0.001, d = 1.94; PSI-SF total scores: p = 0.004, d = 0.86). On the other hand, none of the scores showed significant change in the non-PCIT group. The PCIT can be considered as a potential treatment option for behavioral problems in young children born premature.

Parent-child interaction therapy for children in foster care and children with posttraumatic stress: Exploring behavioral outcomes and graduation rates in a large state-wide sample

ObjectiveParent-Child Interaction Therapy (PCIT) is an evidence-based treatment to reduce child disruptive behaviors and has been used for children with co-occurring posttraumatic stress symptoms (PTS). However, many families are faced with barriers that interfere with treatment completion. Engagement and attrition issues remain a continuous concern for families accessing needed services for youth. Furthermore, these issues are exacerbated for children in foster care. MethodsThe present study explored differences in behavioral outcomes and graduation rates for children in foster care, (n = 206), non-foster care children with a history of trauma (n = 249), and non-foster care children without a history of trauma (n = 1,522) who participated in PCIT. Data was collected in community mental health agencies across the state of Oregon. ResultsThere was a significant decrease in caregiver reported disruptive behavior scores for all groups following PCIT. There were no significant differences found in graduation rates and average number of sessions attended between the groups; however, the graduation rate for this sample was low (17.8%). ConclusionsResults demonstrate that PCIT can be similarly effective in reducing problematic behaviors for children placed in foster care when compared to children not in foster care. Practical Implications: Given the low graduation rate, is imperative that we continue to investigate strategies to enhance treatment engagement and reduce barriers to services.

20.4 EFFECTIVENESS OF PARENT-CHILD INTERACTION THERAPY FOR PATIENTS WITH AND WITHOUT PSYCHOTROPIC MEDICATIONS: A RETROSPECTIVE CHART REVIEW

Parent-Child Interaction Therapy (PCIT) is an evidence-based approach for young children (aged 2-7 years) with disruptive behavior problems. Currently, there are no guidelines and limited data regarding combined treatment with medication and PCIT for children with severe symptomatology. In this study, we sought to examine a sample of youths undergoing PCIT with and without concurrent pharmacotherapy to examine potential correlates of medication use, and factors influencing the effectiveness of therapy as assessed with the Eyberg Child Behavior Inventory (ECBI). We hypothesized that children receiving PCIT with concurrent psychotropic medications would have a greater improvement in ECBI scores than children receiving PCIT monotherapy.

A community evaluation of Parent-Child Interaction Therapy for children with prenatal substance exposure

Children with prenatal substance exposure (PSE) often have behavior problems, but few studies have demonstrated that behavior therapy can be effective for these children. The current study evaluated the efficacy of Parent-Child Interaction Therapy (PCIT) for improving behavior problems in a sample of 116 children with PSE using archival data from a Midwestern PCIT clinic. Analyses included mean comparisons of pre- and post-treatment measures of child and parent behavior, prediction of drop-out from treatment, and evaluation of the potential complicating effects of PSE on treatment response. Results indicate that treatment effects of PCIT for children with PSE appear similar to the outcomes observed elsewhere in the PCIT literature. There was no indication that maternal polysubstance use alters child response to treatment. Attrition was high, but similar to other community trials of PCIT. The current study lends support to the use of PCIT for children with PSE who have behavior problems.

Parent-Child Interaction Therapy for Children with Autism Spectrum Disorder: Research, Training, and Clinical Considerations

This research briefly promotes the inclusion of Parent-Child Interaction Therapy (PCIT) for children with Autism Spectrum Disorder (ASD) in a continuum of empirically-supported ASD treatments. PCIT is a manualized, short-term intervention that improves child compliance and the caregiver-child bond, and is an empirically-supported treatment backed by over 40 years of research. Caregivers are often unprepared to handle the needs of children with ASD presenting with comorbid behavioral problems. As a result, families frequently require mental health services for their children on the autism spectrum; however, access to empirically supported treatments for these families is limited. Furthermore, many mental health providers feel unequipped to treat this special population. Families with children on the autism spectrum are in desperate need of quality, time-limited, evidence-based treatments targeting disruptive behaviors. PCIT is a well-established treatment for disruptive behaviors that represents a promising treatment for complementing other evidenced-based ASD services. Research shows that after PCIT, children with ASD demonstrate improvements in disruptive behavior, social awareness, adaptability, and positive affect. Currently, the PCIT-ASD literature provides a case for conducting PCIT with preschool children who are in the higher functioning range of the autism spectrum (Levels 1 and 2) and display comorbid behavioral problems. Providing PCIT clinicians with training about the special needs of children with ASD could lead to improved access to services for this population. This paper accomplishes the following objectives: 1) Provides an overview of PCIT, 2) Summarizes the PCIT-ASD research, 3) Reviews PCIT-ASD clinical considerations and training requirements, and 3) Suggests future directions for PCIT-ASD research.

Parent-Child Interaction Therapy for Children with Developmental Delay: The Role of Sleep Problems.

Sleep problems are common and associated with externalizing behavior problems in young children, particularly among young children with developmental delay (DD). The aims of the current study, which was a secondary data analysis of 2 previously conducted randomized controlled trials, were to assess whether parent-child interaction therapy (PCIT) led to decreases in child sleep problems and whether initial sleep problems moderated the effect of PCIT on child behavior. The study participants were 44 children (M = 49.19 months, SD = 13.1) with DD or borderline DD and with co-occurring clinically significant levels of externalizing behavior problems and their mothers (M = 35.9 years, SD = 7.3). These participants were randomly assigned to either an immediate treatment group or a waitlist control group. Findings revealed a significant direct effect of PCIT on decreases in sleep problems. Additionally, moderation analyses revealed that lower levels of child sleep problems at pretreatment were associated with greater improvements in observed child compliance compared with higher levels of child sleep problems at pretreatment. This study extends previous findings by providing support for the preliminary efficacy of PCIT in reducing sleep problems in children with DD and borderline DD and highlighting the role of sleep problems as a factor associated with differential treatment effects in behavioral parenting intervention research.

The effectiveness of parent-child interaction therapy for children with autism spectrum disorder and their parents

This study examined the efficacy of Parent Child Interaction Therapy on alleviating behavioral issues among children with Autism Spectrum Disorder (ASD) as well as its efficacy in improving the mother s parenting stress, depression and parent-child interactions. Twenty mother-child dyads were randomly assigned to either the experimental group or the waitlist control group. The PCIT program was conducted in 12 sessions, once a week for an hour in the experimental group. Primary findings were followings: first, following manualized PCIT treatment, mothers reported a decrease in the level of children s disruptive behavior, parenting stress, and depression. Second, the frequency of positive interactions increased while the number of negative interactions decreased. Third, after the completion of the PCIT program, mothers in the experimental group reported high levels of satisfaction with the intervention. Treatment gains were maintained at 3-6-12month follow-up assessments. This study gives evidence to support PCIT as an efficacious intervention program for Korean families who have young children with ASD.

P-310 - Parent-child interaction therapy for children with high functioning autism: a single-subject report

As parent-child interaction therapy (PCIT; Eyberg et al, 1995) relies on social contingencies, it was assumed that it would not be appropriate for children with autism spectrum disorders (ASD). Yet, many behaviors of children with high functioning autism (HFA) are reinforced by social attention. The purpose of this study was to investigate the efficacy of PCIT for young children (ages 2 to7) with HFA and behavioral problems. In this study an A-B single-subject experimental design across five subjects was used. Their behavioral problems were assessed by the Eyberg Child Behavior Inventory (ECBI) in each session. Findings demonstrate improvements in parental reports of child behavior for all participants following the implementation of PCIT especially through the second phase of the treatment. Clinical implications regarding the possible benefits of PCIT for improving the effectiveness of the program as well as limitations and future directions for research and treatment are discussed.
