Clinical and diagnostic features of a paraprostatic cyst of a true uterus masculinus in a male cat presented with tenesmus

AbstractA 5‐year 11‐month‐old, male, neutered, Bengal cross cat was examined for a 5‐week history of tenesmus. Positive contrast urethrocystography and computed tomography scan demonstrated a fluid‐filled structure occupying the pelvic canal. Midline coeliotomy and symphysiotomy revealed a large cystic structure dorsal to the urethra in the area of the prostate. This structure was communicating with the urethra via a duct and both vas deferens were inserting in it. The duct was ligated and partial ablation and omentalisation of the structure was performed. Histopathological examination identified features consistent with a true uterus masculinus. Bacteriology of the cystic fluid cultured Escherichia coli and Bacteroides fragilis. The cat recovered uneventfully and with no ongoing clinical issues.

Isolation of a Multidrug-Resistant vanA-Positive Enterococcus faecium Strain from a Canine Clinical Sample in Greece

An Enterococcus faecium strain was obtained from a paraprostatic cyst of a 17-year-old dog in Greece. Antibiotic susceptibility testing (AST) was accomplished by disc diffusion and MIC methods, and the isolate demonstrated a multidrug-resistant (MDR) phenotype against a great variety of antibiotics, such as β-Lactams, Quinolones, Macrolides, Tetracyclines, Rifampin, Nitrofurantoin, and surprisingly, Glycopeptides, Fosfomycin and Gentamicin (high-level). Molecular screening for Vancomycin resistance genes was carried out, and a vanA gene cluster was identified. To our knowledge, this is the first report of a vanA-positive E. faecium strain isolated from a companion animal in Greece. Importantly, this strain was related with the presence of paraprostatic cysts, a pathological condition requiring treatment. The presence of a highly resistant isolate in a canine clinical sample and the consequent need for treatment constitutes a new challenge for veterinarians due to the lack of available treatment options. Our findings indicate the occurrence of respective bacteria in companion animals, which could act as a reservoir of epidemic MDR strains or relevant mobile genetic elements (MGE) in the community, constituting a threat for public health.

CT imaging of dogs with perineal hernia reveals large prostates with morphological and spatial abnormalities.

The etiology of canine perineal hernia (PH) remains unclear, although as a disease of older male dogs, it is likely to be hormonal. The role of the prostate in the formation of PH has been questioned; however, prospective and systematic evaluation of prostates in these dogs is absent in the literature. In this prospective case-control study, CT imaging was used to assess prostatic changes in dogs with PH (n=46) and compare these findings with those of intact age-matched male dogs (n=23). Using the OsiriX® DICOM viewer, we measured prostatic volume and correlated it with the size of the dog by using the length of the sixth lumbar vertebra. In addition, we recorded spatial and morphological changes of the prostate, such as heterogenicity, intra- and paraprostatic cysts, and mineralizations, as well as prostatic location and rotation. We found that dogs with PH had larger prostates (P<.001) that more often contained cysts (P<.001) and had larger cyst diameters (P=.013) than age-matched controls. Prostates of PH dogs also contained paraprostatic cysts (17.4%) and focal mineralizations (32.6%), which were absent in the control group. Abnormal rotation and location of the prostate were common in dogs with PH. In conclusion, these findings support the use of CT as an adjunct diagnostic imaging modality for the evaluation of the prostate in dogs with PH. Further studies are needed to evaluate nonprostatic CT findings in the pelvic cavity of PH dogs.

Computer-assisted sperm analysis and comparative diagnostic imaging of benign prostatic hyperplasia in dogs by ultrasound, X-ray and computed tomography

Early diagnosis of benign prostatic hyperplasia (BPH) in dogs using imaging methods has become increasingly prevalent in recent years. That allows accurate differentiation of BPH from other diseases, as well as timely prophylaxis and treatment. The aim of the research was to make a comparative analysis of BPH diagnosis by ultrasound, X-ray and computed tomography (CT) and computer-assisted sperm analysis (CASA) of ejaculates from sexually mature male dogs. Two hundred and fifty patients of different breeds, 80 of whom with previous clinical history and BPH-related complaints at 3 to 7 years of age, were followed. The results of echographic studies of dogs with clinical signs of BPH showed structural prostate changes. Focal inflammation, small or larger intra- or paraprostatic cysts (7%), neoplasia (3%), acute or chronic prostatitis (15%) and prostate abscesses (3%) were detected. The remaining 72% of patients demonstrated evidence of hypo -to hyper heterogenic parenchyma with moderate heterogeneity, which are characteristic of BPH. X-ray findings about prostate volume showed that 85% of the patients studied had an increase in prostate volume, enlarged soft tissue shadow in the caudal abdomen or cranially to the pelvis but caudally to the bladder. These data were found out in 13% more patients than those with ultrasound data for BHP. In this study, computed tomography demonstrated structural changes and precise prostate size in all 10 patients with suspected BPH. The CASA results showed that the percentage of static sperm was significantly higher in dogs with BPH than in healthy dogs. At the same time, there was a decline in the percentage of cells with progressive movement at a rapid and medium rate of movement in patients with BPH compared to those data in healthy dogs. These changes could have a negative effect on the fertilising potential of the sperm. The obtained results suggested that the application of CT and CASA for early diagnosis of BPH in dog breeders from rare and valuable breeds is advisable and useful for timely prevention and treatment of the disease.

An unusual case of infected uterus masculinus in a dog

BackgroundParaprostatic cysts are large structures that develop between the prostate gland and urinary bladder, usually in older, intact dogs. Their incidence is reported to be 1.1–5.3% in dogs with prostatic disease. The aetiology of paraprostatic cysts is not fully understood, but they are believed to develop from the uterus masculinus. Whereas the uterus masculinus has been reported to communicate with the urethra in men and horses, no communication between the uterus masculinus and urethra has been identified in dogs.Case presentationAn entire male dog was presented with a bloody discharge from its penis and tenesmus of 5 days’ duration. A diagnosis of cystic uterus masculinus was made on the basis of the findings of abdominal ultrasonography and histopathology of tissues obtained during an exploratory laparotomy. In addition, a Sertoli cell tumour affecting both testes was diagnosed following scrotal castration. The cystic uterus masculinus was completely resected, after which the tenesmus and bloody discharge resolved. Thus, cystic uterus masculinus should be considered as a differential diagnosis for a paraprostatic cyst when such a lesion develops as part of the feminising effect of a Sertoli cell tumour.ConclusionsCystic uterus masculinus should be considered as a differential diagnosis for tenesmus and penile discharge, and for structures resembling paraprostatic cysts. This case report confirms that a uterus masculinus can communicate with the urethra in dogs, as in other species, and demonstrates endocrine responsiveness, manifesting as epithelial and glandular metaplasia and mucus production, with the potential for subsequent infection.

Unusual presentation of alveolar echinococcosis as prostatic and paraprostatic cysts in a dog

BackgroundAlveolar echinococcosis (AE) is caused by the larval stage (metacestode) of Echinococcus multilocularis. The domestic dog can act as a definitive host and harbor adult cestodes in its small intestine or become an aberrant intermediate host carrying larval stages that may cause severe lesions in the liver, lungs and other organs with clinical signs similar to AE in humans.Case presentationA case of canine AE, affecting the liver and prostate with development of multilocular hydatid paraprostatic cysts and possible lung involvement is described in an 8–year-old neutered male Labrador retriever dog.The dog presented with progressive weight loss, acute constipation, stranguria and a suspected soft tissue mass in the sublumbar region. Further evaluation included computed tomography of the thorax and abdomen, which revealed cystic changes in the prostate, a paraprostatic cyst, as well as lesions in the liver and lungs. Cytological examination of fine-needle aspirates of the liver, prostate and paraprostatic cyst revealed parasitic hyaline membranes typical of an Echinococcus infection and the presence of E. multilocularis-DNA was confirmed by PCR.The dog was treated with albendazole and debulking surgery was considered in case there was a good response to antiparasitic treatment. Constipation and stranguria resolved completely. Six months after the definitive diagnosis, the dog was euthanized due to treatment-resistant ascites and acute anorexia and lethargy.ConclusionsTo the authors’ knowledge, this is the first publication of an E. multilocularis infection in a dog causing prostatic and paraprostatic cysts. Although rare, E. multilocularis infection should be considered as an extended differential diagnosis in dogs presenting with prostatic and paraprostatic disease, especially in areas where E. multilocularis is endemic.

Een geïnfecteerde paraprostaatcyste met oorsprong in het prostaatparenchym werd gediagnosticeerd bij een zeven jaar oude, mannelijke, intacte Amerikaanse staffordshireterriër die aangeboden werd met dysurie. De diagnose werd gesteld aan de hand van de anamnese, het lichamelijk en echografisch onderzoek, het bacteriologisch onderzoek, het uitgebreid histopathologisch onderzoek en een DNA-test. De behandeling bestond uit het chirurgisch verwijderen van een groot deel van de cyste, omentalisatie van het restant gecombineerd met castratie en antibioticumtherapie. Immunohistochemische kleuringen toonden aan dat de cyste een urotheliale aflijning had. Een DNAtest sloot het persisterende-gang-van-Müller-syndroom (PMDS) uit. De cyste had een open verbinding met de urethra en ontstond vermoedelijk uit een embryonaal restant ter hoogte van de prostaat. De hond herstelde vlot van de ingreep maar de dysurie en incontinentie persisteerden. Bij de controle vier maanden postoperatief werd echografisch een nieuwe paraprostaatcyste aangetoond. Uit de echobegeleide punctie bleek de cyste geïnfecteerde urine te bevatten.

Two Cases of Paraprostatic Cysts in Castrated Male Dogs

Two castrated male dogs presented for evaluation of tenesmus. Presurgical evaluations included complete physical examinations, serum biochemistry, abdominal ultrasonography, and MRI (case 2 only). Paraprostatic cysts were diagnosed in both cases based on the results of abdominal ultrasonography, MRI, and histopathology of tissue samples obtained during exploratory laparotomy. To the authors' knowledge, the two cases presented herein are the first documented cases of paraprostatic cysts that developed after castration in male dogs. Paraprostatic cysts should be considered in the differential diagnoses for castrated male dogs with prostatic disease.

Paraprostatic Cyst within the Perineal Region of a Perineal Hernia in a Dog

ヘルニア内容が傍前立腺嚢胞であった会陰ヘルニアの症例（6歳、未去勢雄）に遭遇した。症例は直腸検査で会陰部に腫瘤を認め、超音波検査で腫瘤内の液体が確認された。手術時に会陰部の切開で被膜が露出し、多量の液体が排出された。摘出した被膜は病理組織学的検査で傍前立腺嚢胞と診断された。両疾患の併発は十分に考えられ、ヘルニア内容が傍前立腺嚢胞であることは稀であるが鑑別リストとして考慮する必要がある。

RELATIONSHIP BETWEEN PROSTATOMEGALY, PROSTATIC MINERALIZATION, AND CYTOLOGIC DIAGNOSIS

Canine prostatic disease is commonly evaluated with abdominal ultrasound and radiographs. Mineralization of the prostate is often reported, but the clinical relevance of this finding is currently not known. The-purpose of this study was to characterize the relationship between ultrasonographic and radiographic prostate mineralization and the final diagnosis. Medical records of 55 dogs with evidence of prostatomegaly or prostatic mineralization and a cytologic diagnosis were evaluated. Radiographs and ultrasound images were assessed for caudal retroperitoneal lymphadenopathy, vertebral lesions, or other signs of metastasis, and mineralization was assessed semiquantitatively. Twenty-two of 55 (40%) dogs had prostatic neoplasia. Regarding neoplasia, mineralization in neutered dogs had a positive predictive value (PPV) of 100%, a negative predictive value (NPV) of 50%, and a sensitivity and specificity of 84% and 100%, respectively. Mineralization in intact dogs had a PPV of 22%, an NPV of 96%, and a sensitivity and specificity of 67% and 77%, respectively. All neutered dogs with prostatomegaly but not prostatic neoplasia had bacterial prostatitis and were castrated within the previous 3 months. Intact dogs with prostatomegaly and mineralization but not neoplasia had paraprostatic cysts (n = 3), benign prostatic hyperplasia (n = 2) or prostatitis (n = 2). Mineralization score was not indicative of neoplasia. In conclusion, neutered dogs with prostatic mineralization were very likely to have prostatic neoplasia. Intact dogs were unlikely to have prostatic neoplasia if no mineralization was found on radiographs or ultrasound.

RADIOGRAPHIC AND ULTRASONOGRAPHIC FEATURES OF CANINE PARAPROSTATIC CYSTS

Mineralization of paraprostatic cysts is reported to be uncommon. This retrospective study was performed to review the imaging findings of eight histologically confirmed canine paraprostatic cysts. Radiographic patterns of mineralization seen are described. Four of the eight dogs had mineralized cysts. Mineralization seen on radiography was not consistently seen on ultrasonography, probably due to the lack of acoustic shadowing artefact from the small depth of mineralized tissue. It is thus concluded that mineralization of paraprostatic cysts is more common than implied in the literature, and that radiography is superior to ultrasonography in identifying its presence.

A 9-year-old, male mixed-breed dog was presented to the Veterinary Teaching Hospital, Gifu University, with a history of hematuria. Abdominal radiography revealed a big mass that was suspected a dilated bladder. By retrograde contrast cystogram, the bladder was located at the dorsal part of the mass. Abdominal ultrasonography revealed liquid and high echogenie material in the mass. The mass was suspected a paraprostatic cyst at operation. The cyst contained blood-like fluid and cauliflower-like metaplastic bone. According to histopathologic findings, the cyst was diagnosed a paraprostatic cyst. The origin of the cyst was not clear, because the epithelium of the cyst has been removed. Enlarged prostate gland also has been removed. After the operation, the dog has remained in good condition in spite of involuntary urination.

Cystic prostatic disease associated with adrenocortical lesions in the ferret (Mustela putorius furo).

A review of case materials at the Armed Forces Institute of Pathology identified six cases of prostatitis in domestic ferrets (Mustela putorius furo). Five of these ferrets (83%) had one or more cysts and the remaining ferret had a paraprostatic cyst. Three of the six ferrets (50%) exhibited various degrees of prostatic squamous metaplasia. Inflammation ranged from subacute to chronic-active or pyogranulomatous. In addition to the prostatic lesions, hyperplastic and/or neoplastic adrenocortical lesions were present in 4/6 (66%) ferrets; 1/6 (17%) ferrets had previously been clinically diagnosed with adrenal gland-associated endocrinopathy. The remaining ferret had previously had the right adrenal gland removed, but the reason for the removal is unknown. Based upon the histologic findings in these six ferrets, there appears to be an association between proliferative adrenal lesions and cystic prostatitis in domestic ferrets.

Paraprostatic cyst in a cat

ABSTRACTA paraprostatic cyst and lymphosarcoma were found in a neutered male cat that was being evaluated for monoparesis and pollakiuria. The cyst was first detected on physical examination as a palpable caudal abdominal mass. Survey radiography, intravenous urography and ultrasonography were used to further evaluate the cyst. The exact histological nature of the cyst was determined by post mortem examination to be a paraprostatic cyst of prostatic origin that communicated with the urethra. The lining of the cyst consisted of one to two layers of transitional epithelium which was continuous with prostatic ductules in several locations. This is the first reported paraprostatic cyst of prostatic origin in a cat.

ULTRASONOGRAPHIC FEATURES OF PARAPROSTATIC CYSTS IN NINE DOGS

The ultrasonographic features of paraprostatic cysts in nine dogs are described along with historical, clinical, surgical and pathologic findings. Cysts occurred predominantly in older, large breed dogs (mean age 8 years, range 3–11 years). The most common presenting complaints were depression, inappetance, stranguria, tenesmus and bloody penile discharge. A palpable abdominal mass was the most common physical finding. Ultrasonographically, paraprostatic cysts were usually large anechoic structures; many contained internal septa. Moderately large anechoic cavities or cysts were also detected in the prostatic parenchyma of five dogs, and in two of these communication with the paraprostatic cyst was visualized. There were no clearly distinct ultrasonographic criteria to discriminate septic from nonseptic paraprostatic cysts.

The diagnosis and management of paraprostatic and prostatic retention cysts in the dog

ABSTRACTEleven dogs were diagnosed as having solitary or multiple prostatic cysts in an abdominal, pelvic or perineal position. Common presenting signs included urinary dysfunction, faecal tenesmus and abdominal distension. Cystography was found to be the most consistently accurate technique for differentiating the cysts from the bladder and other abdominal soft tissue masses whilst urethrography often highlighted their prostatic origin.Complete surgical resection of the cysts was attempted in nine dogs, the complexity of dissection being dictated by cyst size and the extent of adhesions to adjacent organs. Nine dogs were castrated. Urinary incontinence continued post‐operatively in four dogs but later resolved in three of these. Seven dogs were alive and symptom‐free over post‐operative periods ranging from one to three years.Despite some of its technical difficulties, surgical excision appears to be a successful approach to even the larger prostatic cysts. Few of the post‐operative complications associated with other surgical procedures described for the management of cysts were encountered.
