Effect of aspirin combination/clopidogrel combination on high-sensitivity C-reactive protein and P-selectin levels in patients with acute coronary syndrome

Purpose: To investigate the effect of combining aspirin with clopidogrel on high-sensitivity C-reactive protein (hs-CRP) and P-selectin in patients with acute coronary syndrome (ACS). Methods: The medical records of 198 ACS patients treated at Zhaotong Traditional Chinese Medicine Hospital, Zhaotong, China between January 2022 and January 2023 were analyzed retrospectively. Control group (88 patients) received aspirin (300 mg once, and subsequently a maintenance dose of 100 mg daily for 6 months), while the study group (118 patients) received the same dose of aspirin and clopidogrel (150 mg once, and a maintenance dose of 75 mg daily for 6 months). Levels of P-selectin, interleukin-6 (IL-6), hs-CRP, IL-8, tumor necrosis factor-α (TNF-α), efficacy, and incidence of adverse reactions in both groups were determined. Results: There was no significant difference in P-selectin and hs-CRP levels between the two groups before treatment (p > 0.05). However, P-selectin and hs-CRP levels in the study group decreased significantly compared to control group (p < 0.0001). Similarly, there was no significant difference in IL-8, IL-6 and TNF-α levels between the two groups before treatment (p > 0.05). However, IL-8, IL-6, and TNF-α reduced significantly after treatment (p < 0.0001), with more significantly lower levels observed in the study group (p < 0.001). There was no significant difference in the incidence of adverse reactions between the two groups. Conclusion: Aspirin combined with clopidogrel significantly lowers P- selectin, and hs-CRP, IL-8, IL-6, and TNF-α levels without worsening adverse reactions. A more comprehensive analysis of the combined use of aspirin with clopidogrel including all the associated mechanisms in the treatment of ACS is necessary.

Soluble P-selectin levels in patients with obstructive sleep apnea: a systematic review and meta-analysis.

Obstructive sleep apnea (OSA) patients are at increased risk for cardiovascular disease, stroke, atherosclerosis, hypertension, and venous thromboembolism. Elevated soluble P-selectin (sP-selectin) levels are also associated with increased risk of above diseases. But whether sP-selectin levels in OSA patients are higher than their counterparts remain unclear, since previous studies yielded inconsistent results. Therefore, a meta-analysis is warranted. PubMed, Embase, Cochrane Library, and Web of Science databases were searched for eligible studies. Studies were included if they reported sP-selectin levels of both OSA patients and non-OSA controls. Standardized mean differences (SMDs) with 95% confidence intervals (CIs) were calculated to determine the effect sizes. Nine eligible studies were finally evaluated. When all the studies were pooled, sP-selectin levels in OSA patients were significantly higher than that in controls (SMD = 0.54, 95% CI 0.29-0.78, I2 = 66%, p < 0.0001). In the subgroup analysis based on BMI matched groups, sP-selectin levels were significantly higher in OSA patients than that in controls (SMD = 0.52, 95% CI 0.27-0.76, I2 = 23%, p < 0.0001). In the subgroup analysis stratified by blood source, either serum sP-selectin levels or plasma sP-selectin levels in OSA patients were higher than that in controls. Moderate-to-severe OSA patients had significant higher sP-selectin levels (SMD = 0.80, 95% CI 0.45-1.15, I2 = 67%, p < 0.00001), while mild OSA patients showed no significant difference with controls. The pooled results reveal that OSA patients have higher sP-selectin levels than non-OSA controls. This conclusion remains unaltered in all subgroups other than the subgroup of mild OSA patients. Additional studies are warranted to better identify the role of sP-selectin as a potential biomarker in OSA patients.

Soluble P-Selectin Levels in Patients with Sickle Cell Disease Reflect Platelets' Activation Rather Than Endothelial Dysfunction

Background: Vascular occlusion is a major cause of the morbidity associated with sickle-cell disease (SCD). The tendency of sickle red blood cells to adhere to the vascular endothelium is believed to be a major contributor and possibly primary cause of the vaso-occlusive process. Therefore mediators of adhesion have become a potential new target for pharmacological therapy to combat the complications of SCD. One of the molecules involved in this process is P-Selectin or CD62P, a cell adherence molecule that is rapidly and chronically expressed on the surface of endothelial cells and platelets when activated. P-Selectin is found in the plasma of normal individuals at ng/mL concentrations. Circulating soluble P-Selectin (sP-Selectin) appears to be slightly smaller than native P-Selectin. An alternatively spliced mRNA encoding a form of human P-Selectin lacking the transmembrane anchoring domain has been reported for both megakaryocytes and endothelial cells, and evidence suggests that the majority of circulating sP-Selectin arises in this manner. We and others have reported elevated sP-Selectin levels in SCD even in steady phase. Recently, a humanized monoclonal antibody that binds to P-Selectin and blocks its interaction with P-selectin glycoprotein ligand 1 (PSGL-1) has been administered to patients with SCD, resulting in an amelioration of painful VOC. In this context we aimed to explore if sP-Selectin levels could be used to choose among the SCD patients those who might benefit from the new therapy. Patients and Methods: Eighty adult Caucasian patients with HbS/βthal at steady phase [40 patients under hydroxyurea (HU+) treatment and 40 patients without hydroxyurea (HU-) treatment] were included in this study, while 20 apparently healthy individuals of similar age and gender served as controls. Along with sP-Selectin levels, measured with the same method as reported previously (Human sP-Selectin/CD62P Immunoassay, R&amp;D Systems, Minneapolis, MN, USA), other parameters of hemolysis, inflammation, endothelial dysfunction, iron accumulation and clinical features of the disease were evaluated. Results are expressed as median values ± SEM. Results: We found that sP-Selectin levels were elevated in 45/80 (56%) patients with HbS/βthal compared to controls (108.2±6.3 vs. 69.3±4.1ng/mL, respectively, p&lt;0.001), independently of patients' βthal genotype and correlated strongly with PLT count (r=0.760, p&lt;0.001). Regarding HU treatment, sP-Selectin levels did not differ between (HU+) and (HU-) patients (112.5±9.8 vs. 100.3±7.4ng/mL, respectively, p&gt;0.07). No significance correlation was found between sP-Selectin levels and markers of: hemolysis (RPI: r=0.191, p&gt;0.100); LDH: r=0.103, p&gt;0.360 and bilirubin: r=0.171, p&gt;0.130); inflammation (hs-CRP: r= 0.002, p&gt;0.842); endothelial dysfunction (vWF:antigen: r=0.141, p&gt;0.210 and ADAMTS-13: r=0.089, p&gt;0.507). Regarding iron accumulation no correlation was found between sP-Selectin and ferritin levels (r=0.090, p&gt;0.438), while a weak negative correlation was found with hepcidin-25 levels (r=-0.283, p=0.018). Furthermore, no correlation was found between sP-Selectin levels and history of clinical complications such as VOC (p&gt;0.795), acute chest syndrome, venous and arterial thrombosis and mean pulmonary artery pressure values, (p&gt;0.402). Conclusion: In this study, we proceeded with an external validation procedure of sP-Selectin determination in patients with HbS/βthal (keeping the same methodology in a different cohort of patients), and we found elevated levels of sP-Selectin with the evidence of PLTs' secretion origin, as no correlation found with other markers of endothelial dysfunction and inflammation. Interestingly, we failed to find a significant link of sP-Selectin levels with other markers of disease severity and/or clinical features of SCD. Thus, we consider that the use sP-Selectin as a biomarker of assessment and treatment of endothelial dysfunction in patients with SCD is of almost negligible importance. Cytokines, Adhesion Molecules, and Matrix Metalloproteases as Predisposing, Diagnostic, and Prognostic Factors in Venous Thrombosis.

The inflammatory response is a well-established part of, and a prerequisite for, venous thrombosis. To better understand the pathophysiology of venous thrombosis and to identify improved diagnostic biomarkers, further studies of the relationship between inflammation and coagulation are needed. We review previous studies concerning inflammatory biomarkers in venous thromboembolism, in particular cytokines, soluble adhesion molecules and matrix metalloproteases as predisposing, diagnostic and prognostic factors in venous thrombosis. Elevated cytokines and genetic alterations coding for cytokines are found in several patient cohorts which indicate that cytokines are involved as predisposing factors in venous thrombosis development. Increased levels of pro-inflammatory cytokines are detected both in animal models and in patients with acute venous thrombosis and clinical trials, although currently without evident diagnostic value. Adhesion molecules are crucial in the development of venous thrombosis, especially P-selectin seems important in initiating leukocyte accumulation and adhesion to endothelium for subsequent platelet accumulation. Several studies have demonstrated increased soluble P-selectin levels in patients with venous thrombosis, emphasizing its potential role as diagnostic marker and also as a therapeutic target. Matrix metalloproteases are essential effectors during venous thrombosis resolution and may impact vessel wall fibrosis, and together with their natural occurring inhibitors are crucial in acute and chronic thrombosis pathophysiology. Furthermore, studies in animal models of venous thrombosis have demonstrated anti-inflammatory treatment to be effective in terms of thrombus resolution and reduction of vessel wall damage, without increase in bleeding risk during the course of treatment. Thus, soluble mediators should be further investigated both as possible biomarkers and therapeutic targets in venous thromboembolic disease.

Soluble P-Selectin Level in Patients with Deep Venous Thrombosis

Background: Venous thromboembolic disease (VTE) remains a significant source of morbidity and mortality. As non-specific subjective complaints and a lack of objective clinical examination findings complicate the diagnosis of both deep venous thrombosis (DVT) and pulmonary embolism. Objective diagnostic testing is required to confirm or exclude the presence of venous thromboembolism before subjecting patients to unnecessary long-term anticoagulation. Aim of the Work: This study was aimed to measure the level of soluble P-selectin (sPsel) in Egyptian patients with DVT and asses its diagnostic value in relation to other clinical data and radiological examination. Subjects and Methods: This study was carried out on 80 individuals, attending Ain Shams University Hospitals between October 2015 and March 2016 after Ethical committee approval. They were divided into two groups.: Group I, 50 patients who were positive for DVT by duplex ultrasound. They were 41 males and 9 females. Their ages ranged 20 - 77 years, with a mean of 46.20 ± 11.78 years. They were 41 (82%) males and 9 (18%) females, with a male to female ratio of 4.55:1. Group II: 30 healthy subjects with no clinical signs, symptoms, or history of DVT. They were 23 (76.7%) males and 7 (23.3%) females, with a male to female ratio of 3.28:1. Results: There was a highly significant difference between patients (group I) and controls (group II) as regards P-selectin, the best cut off was70.5 ng/ml with 98% sensitivity, 100% specificity, a NPV of 96.8% and a PPV of 100%. Conclusion: Diagnostic cut-off levels of P-selectin in cases with DVT is 70.5 mg/L that can safely differentiate patients who are free from DVT from others who are positive for DVT or those who would eventually develop thrombosis regardless their primary duplex ultrasound scanning results. Recommendations: As our study recommends the use of serum P-selectin as diagnostic biomarker in DVT alone; in addition to the newly estimated cut-off levels for these biomarker, further studies on larger number of cases are needed for more evaluation of these cut-off values and to establish whether they could be used to guide anticoagulation therapy when duplex ultrasound is unavailable.

Markers of endothelial dysfunction and leucocyte activation in Saudi and non-Saudi haplotypes of sickle cell disease.

Sickle cell disease (SCD) is an autosomal recessive inherited hemoglobinopathy, characterized by chronic hemolysis and recurrent vaso-occlusive crisis (VOC). This study investigates changes in leucocyte subsets and the relationship between cell adhesion molecule expression and disease manifestations in patients during steady state and acute VOC. We compared soluble E-selectin and P-selectin levels in 84 SCD patients, in steady state and during VOC to 84 healthy controls. Using immunophenotyping, we also compared lymphocyte subsets in these three groups. Further, we compared E-selectin and P-selectin levels in patients of Saudi ethnicity to non-Saudi patients, in all three groups. Lymphocyte subsets showed high percentages of total T lymphocytes, T helper and suppressor lymphocytes, B lymphocytes as well as NK cells in patients with SCD during steady state, while B lymphocytes and NK cells were significantly higher during acute VOC crisis. High levels of both soluble E-selectin (sE-selectin) and soluble P-selectin (sP-selectin) markers were demonstrated in the serum of patients with SCD during both steady state and acute VOC. Levels of selectins were significantly higher in acute VOC. The immunophenotypic expression of L-selectin, on leucocytes, was high in SCD both during steady state and during acute VOC in comparison to normal control subjects. There was no significant difference in all three study groups between Saudi and non-Saudi patients. These findings suggest that patients with SCD have increased expression of adhesion molecules: E-selectin and P-selectin, which play an important role in the pathogenesis of VOC. Despite the distinct phenotype of Saudi patients with SCD, there was no significant difference in levels of soluble E-selectin and soluble P-selectin between Saudi and non-Saudi patients in all three groups. While sickle cell disease is a well-recognized state of chronic inflammation, the role of specific adhesion molecules is steadily unraveling. Studies are underway to investigate the potential role of selectin antagonists, for prevention and reversal of acute vascular occlusions in SCD patients.

Elevated platelet activation in patients with chronic urticaria: a comparison between aspirin-intolerant and aspirin-tolerant groups

BackgroundPlatelets are actively involved in immune inflammatory processes that release inflammatory mediators. Platelet activation has been reported in various inflammatory diseases; however, few studies have described platelet involvement in chronic urticaria (CU). ObjectiveTo investigate platelet-activation markers, namely P2Y12 receptor and P-selectin expression, and soluble P-selectin level in patients with aspirin-intolerant CU (AICU) and aspirin-tolerant CU (ATCU). MethodsForty-eight patients with CU and 25 normal controls were enrolled in this study. Aspirin intolerance in patients with CU was confirmed by an oral provocation test. P2Y12 and P-selectin expressions on platelets were measured using flow cytometry; soluble P-selectin level in plasma was measured by enzyme-linked immunosorbent assay. To study the functional effects of aspirin, platelets were treated with aspirin (2 mmol/L) and the expressions of P2Y12 and P-selectin were compared between the AICU and ATCU groups. ResultsThe expression of P2Y12 was significantly higher in patients with CU compared with controls, whereas no significant difference was noted in the expression of P-selectin level. The levels were not significantly different according to urticaria symptom score, symptom control status, and aspirin intolerance. Soluble P-selectin level was significantly higher in the AICU group than in the ATCU group compared with controls. Aspirin did not significantly suppress P2Y12 and P-selectin expressions on platelets in the AICU group, whereas significant suppression was noted in the ATCU group. ConclusionThese findings suggest that increased platelet activation contributes to skin inflammation in patients with AICU and those with ATCU. The functional difference of platelets in response to aspirin may contribute to persistent skin inflammation in patients with AICU.

Pretreatment with different loading doses of clopidogrel influences P-selectin levels in patients undergoing percutaneous coronary intervention: results from the ARMYDA-2 (antiplatelet therapy for reduction of myocardial damage during angioplasty) SELECT substudy

ARMYDA-2 (Antiplatelet therapy for Reduction of MYocardial Damage during Angioplasty) trial demonstrated improved clinical outcome in patients undergoing percutaneous coronary intervention (PCI) pretreated with 600 vs. 300 mg clopidogrel loading dose. ARMYDA-2 SELECT is a prospectively planned subanalysis to investigate the effects of those different loading regimens on P-selectin levels. From the ARMYDA-2 population, we investigated a subgroup of 84 patients (41 randomized to a 600 mg and 43 to a 300 mg clopidogrel loading dose given at a mean time of 6 h before PCI), in whom soluble P-selectin levels were measured at baseline (at the time of clopidogrel administration), immediately after the procedure, and after 8 and 24 h. In the overall study population, a significant decrease of P-selectin levels was observed from baseline to intervention (from 91 ± 10 to 53 ± 15 ng/ml; P < 0.001). Baseline P-selectin levels were similar in the two groups, whereas at the time of intervention they were significantly lower in the high-dose arm (50 ± 13 vs. 58 ± 15 ng/ml; P = 0.048). P-selectin values between the two arms were not different at the subsequent determinations. The lowest procedural P-selectin levels were observed in patients of the 600 mg arm who had no postprocedural increase of troponin-I above normal limits (P ≤ 0.040). Pretreatment with clopidogrel before PCI decreases peri-procedural P-selectin levels; moreover, a 600 mg clopidogrel loading dose, compared with the 300 mg regimen, is associated with reduction of peri-procedural myocardial damage and significant attenuation of P-selectin levels at the time of intervention. These results may help in identifying mechanisms underlying clinical benefit of the high clopidogrel load in PCI.

Combined use of D-dimer and P-selectin for the diagnosis of splenic or portal vein thrombosis following splenectomy

To investigate the predictive value of combined use of D-dimer and P-selectin for splenic or portal vein thrombosis (SPVT) after splenectomy. A prospective study was carried out in 82 patients who had undergone splenectomy for portal hypertension secondary to hepatic cirrhosis. Plasma levels D-dimer and P-selectin were measured before and after the surgery.27 (30.1%) patients developed SPVT following the portal hypertension surgery. The post-operative D-dimer and P-selectin levels in patients with SPVT were significantly higher than in those without PVT (n = 55, P < 0.01). The receiver-operated characteristics curves (ROC) of D-dimer and P-selectin showed a significant predictive value in SPVT (D-dimer, Az = 0.880, P < 0.01; P-slectin, Az = 0.933, P < 0.01). The sensitivity and specificity of D-dimer (> 500 µg/mL) in diagnosing SPVT were 88.9% and 78.2% respectively. P-selectin > 90 ng/mL had an 85.2% sensitivity and 85.5% specificity for SPVT. The combination of the D-dimer and P-selectin criteria yielded an 82.0% sensitivity and 97.6% specificity for SPVT. In Conclusion, there was a significant elevation in plasma D-dimer and P-selectin in patients with post-operative SPVT. A combination of plasma D-dimer and P-selectin may be used as biomarkers to screen or diagnose SPVT following splenectomy.

Thrombopoietic Cytokine and P-Selectin Levels in Patients with Multiple Myeloma Undergoing Autologous Stem Cell Transplantation: Decrease in Posttransplantation P-Selectin Levels Might Predict the Degree of Maximum Response

PurposeThis study was designed to determine the pretransplantation levels of thrombopoietic cytokines, which have a fundamental role in both megakaryopoiesis and myeloma pathogenesis and P-selectin in patients with multiple myeloma undergoing autologous hematopoietic stem cell transplantation (AHSCT) and to correlate the cytokine levels with time to platelet recovery. The effect of AHSCT on the levels of the cytokines and its correlation with maximum disease response was also investigated. Patients and MethodsThe levels of thrombopoietin, interleukin (IL)-6, IL-11, IL-1β, and P-selectin was measured before and 30 days after AHSCT in 32 patients with a median age of 55 years. The median time to platelet recovery was day +11 (range, 0-14 days) without any significant correlation with pretransplantation cytokine levels. ResultsNo significant change was observed in thrombopoietic cytokines after AHSCT, whereas serum P-selectin levels showed a significant decrease after AHSCT (P = .001). The decrease in P-selectin was found to be significant in patients who achieved complete remission (P1 = .008) and partial remission (P2 = .018) early after AHSCT. Our data suggest that the level of thrombopoietic cytokines does not have a role in time to platelet recovery. ConclusionThe change in P-selectin levels early after transplantation could be a surrogate marker in determining the maximum posttransplantation response.

Objective To explore the dynamic changes of von willebrand factor(VWF)and Pseleetin in the finger-replanted patients,and the relationship between the prognosis of the surgery and hypercoagulability.Methods From December 2004 to December 2006,eishty finger-replanted patients were recruited to our study.with 40 healthy volunteers as controls.Plasma VWF and P-selectin were detected by enzyme-linked immunosorbent assay(EUSA)in both controls and patients before or after replantation.Results The VWF and P-selectin levels had significant differences between the replantations and controls(F=14.76,11.76,P＜0.01).The VWF levels in the patients of 1,4,8,16 hours after replantation were(1 715±493),(1 396±549),(1 266±504),(1 163±436)U/L respectively,all markedly higher than the controls(P＜0.01).The P-selectin levels in patients of 1,4,8,16,24 hours after operation were(14.7±2.6),(12.5±3.0),(11.8±3.2),(11.1±3.0)、(10.5±2.6)μg/L,significanfly higher than the controls(P＜0.01).The VWF levels in patients of pre-replantion and the 1,4,8,16,24,48,72 hours after replantation were(854±209),(1 535±389),(1 177±407),(1 040±283),(958±216),(829±193),(777±151),(713±137)U/L in successful group,and were(1 202±164),(2 333±243),(2 146±161),(2 039±244),(1 865±170),(1 645±283),(1 427±331),(1 188±262)U/L in unsuccessful groups.They were all significantly different at the same test-time points between two groups(t=4.44,5.12,6.10,8.43,10.17,8.85,5.10.4.61,P＜0.05).The P-selectin levels in patients of 1,4,8,16,24,48,72 hours after replantation were(13.9±2.5),(11.2±2.0),(10.2±1.6),(9.6±1.2),(9.2±0.9),(9.5±0.6),(9.3±0.4)μg/L in successful group,and(17.2±1.0),(16.9±1.0),(17.0±1.3),(16.1±1.1),(14.9±1.5),(13.8±1.4),(12.8±1.2)μg/L in unsuccessful group.Significant difference existed at the same testtime points between two groups again(t=5.22.9.91,10.35,12.79,9.46.9.45,9.33,P＜0.01).After replantation,both VWF and P-selectin were rapidly elevated and went to the summit 4 hours later,then declined to pre-replantation level about 24 to 48 hours later after replantation.Conclusions VWF and P-selectin were associated with the hypercoagulability.Dynamic monitoring VWF and p-selectin may be useful in determining the existence of hypercoagulability and the therapy of anti-coagulability. See related article, pages 149–156 In this issue of Circulation Research , Woollard et al identify a critical role of pathologically elevated levels of soluble P-selectin (sP-selectin) found in the plasma of patients with peripheral arterial occlusive disease (PAOD) that promotes the activation of neutrophils and induces their adhesion to fibrinogen and platelet monolayers.1 P-selectin is an inflammatory adhesion molecule expressed on activated platelets and endothelial cells.2 During the last two decades, the role of inflammatory cells and adhesion molecules in the development and progression of vascular diseases has become apparent, and it is now recognized that many of the cellular and molecular events that underlie atherosclerotic vascular disease are inflammatory in nature.3,4 One manifestation of atherosclerosis is PAOD, which is characterized by intermittent claudication and critical limb ischemia.5 The first case of intermittent claudication caused by arterial occlusive disease was described as early as 1831 by Jean Bouley, and a few years later Heinrich Erb proposed walking as a therapy for intermittent claudication6; however, the precise mechanism of development of PAOD as well as the optimal treatments of this disease still remain to be determined. Numerous studies have identified soluble inflammatory markers including acute phase proteins and adhesion molecules as risk factors for cardiovascular disease, but it is unclear whether they play causative roles as active participants in the initiation and maintenance of the inflammation (mediators) or whether they merely indicate the existence of the disease (markers). Soluble P-selectin and the success of thrombolysis in acute myocardial infarction

Background: P-Selectin mediates adhesive interactions between platelets, leukocytes and endothelium to form thrombi. Our purpose was to investigate plasma soluble(s) P-selectin levels in patients with acute myocardial infarction (aMI) and the effect of thrombolysis on P-selectin levels. Methods: Patients with aMI within the first 6 h of chest pain were enrolled prospectively. sP-selectin levels were determined by ELISA in the plasma of patients with aMI ( n=32), stable angina ( n=18), and healthy controls ( n=15). Samples were obtained before, 3 and 24 h after reperfusion therapy with tissue plasminogen activator. Seven patients showed recurrent angina or failure to reperfuse. Results: sP-selectin levels were significantly higher in aMI group than other groups (86.7±8.7 ng/ml, P<0.05). sP-selectin levels were similar in stable angina and control groups (28.8±4.4 vs. 25.4±7.3 ng/ml, P=NS). A significant increase in sP-selectin levels was observed 3 h after successful thrombolysis and this was followed by a decrease to near the baseline level late after reperfusion. But patients with failed reperfusion showed sustained high sP-selectin levels after 24 h of thrombolysis ( P<0.05). Conclusion: The plasma sP-selectin level is elevated in aMI and it increases further following thrombolytic therapy. This increase is probably induced by activation of endothelial cells or platelets after myocardial ischemia and reperfusion during aMI. As the elevated levels are sustained in patients with failed reperfusion, serial P-selectin levels may be used as a non-invasive indicator of successful thrombolysis in aMI.

High serum level of P-selectin in patients responded to interferon- α treatment for chronic hepatitis C

Circulating soluble P-selectin levels in sera of patients with chronic hepatitis C treated with natural interferon- α were investigated. Serum soluble P-selectin levels gradually increased after venesection when the sera were incubated at room temperature without serum separation, but the levels did not exceed more than 300 ng ml −1 during 3 h. Serum soluble P-selectin levels in patients who completely responded to interferon therapy were significantly higher than those in healthy subjects and other patients. Those levels in complete responders (CR) increased more than the base line levels 2 weeks after the initiation of the therapy and decreased to the base line levels 6 months after the end of the therapy, but the levels were still significantly higher than those in healthy subjects and non-responders (NR). The present preliminary study suggests that serum soluble P-selectin level is another useful marker for predicting response to natural interferon- α treatment in patients with chronic hepatitis C in addition to the established viral factors identified previously.
