Interior Design Based on Internet Thinking

With the continuous development of society and economy, Internet thinking is gradually integrated into interior design, bringing new possibilities for interior design. This paper takes the interior design of general hospital outpatient department based on Internet thinking as an example to explore the interior design based on Internet thinking. Through the questionnaire survey and interview method and other scientific research methods, the author collected relevant data and carried out statistics and analyses, and concluded that the field of interior design should be combined with the current way of thinking about the Internet to innovate, and integrate the interior space into the combination of modern science and technology and interior design, so as to enable the interior design industry to develop rapidly. Under the influence of Internet thinking, interior design pays more attention to the interactive relationship between users and space. This mode of thinking enables designers to better understand user needs and provide users with more personalised and comfortable space design.

Prevalence of Dyslipidemia, Drug Therapy Problems, and Medication Adherence in Type 2 Diabetes Mellitus Patients in North Central Nigeria

Background Patients with diabetes mellitus have an increased risk of developing dyslipidemia, predisposing them to macro- and microvascular consequences such as coronary heart disease. Aim The aim of this study was to assess the prevalence of dyslipidemia, drug therapy problems (DTPs), and medication adherence in type 2 diabetes mellitus (T2DM) patients in a tertiary hospital in North Central Nigeria. Method This study was a cross-sectional convenient sampling of eligible patients conducted in the General Outpatient Department of General Hospital, Ilorin, from March to May 2022. A validated questionnaire was administered to obtain all relevant information on sociodemographic information, and blood samples were collected in a medium plain heparinized tube and sent to the laboratory where fasting blood sugar, high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) values were calculated as appropriate. Morisky's Medication Adherence Scale 8 (MMAS-8) was used to capture patient's adherence level, and DTP was assessed using the Pharmaceutical Care Network Europe (PCNE) Criteria version 7. Ethical approval was obtained from the Ministry of Health, Ilorin. Statistical Package for Social Sciences was used to analyze the data collected for descriptive and inferential statistics. Results The mean age of the respondents was 60 ± 0.7 years. Of the 60 respondents, 25 (41.67%) were males and 35 (58.33%) were females. All the respondents had dyslipidemia, 30% respondents were identified with a DTP of nonadherence, 16.7% needed additional drug/monitoring, dosage was too low in 11.7%, and 11.6% could not afford their medication due to high cost. In all, 45 (75%) respondents had poor sugar control and 33 (55%) had poor blood pressure control. Using MMAS-8, the majority (41, [68.30%]) of the respondents were found to have poor adherence to their diabetic medications. Conclusion High prevalence of dyslipidemia, poor medication adherence, and DTPs were found in the T2DM patients in this study.

A Cross-Sectional Study about Demographic and Medical Characteristics of Hypertensive Pregnant Women in Two Secondary Healthcare Facilities, in Ilorin, Nigeria

Background Hypertension is one of the major causes of related maternal deaths worldwide and it is one of the most common medical disorders encountered during pregnancy. Aim This study aimed at assessing the demographic and medical characteristics of hypertensive pregnant women in two secondary healthcare facilities, in Ilorin, Nigeria Method This was a cross-sectional study among 104 pregnant women with hypertension attending the outpatient department of General Hospital and Civil Service Clinic, Ilorin between March and May 2021. A validated self-administered questionnaire was used to obtain information on sociodemographics. Data on the medical characteristics of the respondents and their medications were extracted from the respondent's medical files. Ethical approval was obtained from the Ministry of Health and General Hospital, Ilorin. For statistical analysis for categorical measurements, the frequencies and percentages were computed. Results Seventy-five percent of the correspondents were below 36 years old, 97% were married, 79% had tertiary education, and 74% were above 24 weeks of gestation. About 68% of the correspondents had blood pressure more than or equal to 140/90 and 10% had blood pressure more than 160 mm Hg systolic or more than 100 mm Hg diastolic. About 78.8% of the correspondents had a family history of hypertension, 13.5% had a family history of diabetes mellitus, and 22.1% had a family history of obesity. About 27.9% had pre-existing hypertension, 61.5% developed hypertension before the end of 20 weeks of gestation, and 10.6% developed hypertension after 20 weeks of gestation. About 24% had their antihypertensive medication changed during pregnancy, 38.5% were treated with methyldopa, 31.7% with nifedipine, and 27% with nifedipine and methyldopa. Majority (83.6%) of the respondents were not adherent to their medications. Conclusion Two-third of the patients had poor blood pressure control irrespective of the fact that all were under drug therapy. Family history and large body mass index are the main risk factors for the development of hypertension during pregnancy. Noncompliance may play a significant role in no drug response.

Assess the Knowledge and Attitude Regarding Child Abuse and its Prevention Among Family Members of the Child Who Are Attending Mamata General Hospital Outpatient Department, Khammam, Telangana

A study to assess the knowledge and attitude regarding Child Abuse and its prevention among family members of the child who are attending Mamata General Hospital Outpatient Department, Khammam, Telangana, with a view to develop an information booklet on Child abuse and its prevention. Objectives were to describe the socio-demographic variables of family members, to assess the levels of knowledge and attitude scores regarding Child Abuse and its prevention among family members, to find out the association between knowledge and attitude scores of family members with their selected socio-demographic variables, to develop and distribute an information booklet regarding Child Abuse and its prevention to family members Methodology: A quantitative survey approach and Descriptive research design was used for this study. The study was conducted at Mamata General Hospital Outpatient Department, Khammam, Telangana. The sample comprised of 50 family members of the child age group between 21-50 years and sample was selected by Non-probability - Convenience Sampling Technique. The data was collected by Interview technique and tool used for the study was structured interview schedule. Results: The study findings shows that majority of family members levels of knowledge regarding child abuse among 50, Majority of them 37(74%) had adequate knowledge, 13(26%) had moderately adequate knowledge and none of them had an inadequate knowledge.The study findings shows that attitude scores regarding child abuse among 50 family members, all of them had a favorable attitude 50(100%) and none of them had an unfavorable and moderately favorable attitude. Chi square is not applicable for association between knowledge, attitude scores and selected socio demographic variables as the expected values are less than 5 at p value < 0.05 level. Hence only proportions are calculated Conclusion: The study concluded that majority of the family members had adequate knowledge and all of them had a favorable attitude but some of them had moderately adequate knowledge levels, which indicates that still there is a need to increase the knowledge levels of family members regarding child abuse and its prevention.

PREVALENCE AND CORELLATES OF PREHYPERTENSION AND HYPERTENSION AMONG ADULTS ATTENDING OUTPATIENT DEPARTMENT IN TERTIARY CARE HOSPITAL

Introduction: India and many other developing countries are facing a problem of epidemiological transition from communicable diseases to noncommunicable diseases. Globalization brought the life style and behavioural modications and in turn increased the prevalence of life style disorders such as hypertension and diabetes mellitus etc. Aim and objectives: to know the prevalence of pre hypertension in age group of 19 to 45 years .to know the prevalence of hypertension in age group of 19 to 45 years to know association of various risk factors with pre hypertension and hypertension in adult age group. Methodology-a hospital based observational pilot study conducted among 770 adults between age groups of 19 to 45 years of age in government general hospital outpatient department, which is a tertiary care hospital that caters needs of entire NTR District, A.P. Results: it was observed that the prevalence of hypertension and pre hypertension was 24.7 % and 23.6% among study subjects. Uncontrolled hypertension was observed in 7.7% of study subjects. Various study factors like age, gender, presence of chronic illness, presence of alcohol consumption, presence of sedentary life style and absence of physical activity, junk food consumption, presence of high BMI contributed signicantly as possible risk factors for hypertension and prehypertension and showed statistically signicant association in this study

Analysis of changes in the expression levels of peripheral blood immunoregulatory T Lymphocytes in children with bronchial asthma accompanied by recurrent infection.

Objectives:To investigate the changes in the expression levels of peripheral blood immunoregulatory T lymphocytes in children with bronchial asthma accompanied by recurrent infection.Methods:Eighty children with bronchial asthma admitted to the Third Outpatient Department of General Hospital of Northern Theater Command from January 2017 to January 2019 were retrospectively selected and divided into two groups according to whether they had recurrent infection: the asthma group (n=49) and the recurrent infection group (n=31). Another 40 children in the same age group who had a health checkup or a surgical hospital preoperative examination were selected as the control group. Peripheral venous blood of each group was taken to measure the expression levels of T lymphocyte subsets (CD3+, CD4+, CD8+, CD4+/CD8+), and regulatory T cells (Treg) and helper T lymphocytes (Th1, Th2, Th17) in CD4+ subsets were detected at the same time. Moreover, Th1/Th2 values were calculated, and the levels of interleukin-4 (IL-4), interleukin-17 (IL-17), and interleukin-21 (IL-21) were measured.Results:The expression levels of CD3+ lymphocyte subsets were similar among the recurrent infection group, the asthma group, and the control group (P>0.05), and there were statistically significant differences in CD4+, CD8+ and CD4+/CD8+ lymphocyte subsets among the three groups (P<0.05). Statistically significant differences were observed in Treg, Th1, Th2, Th17 and Th1/Th2 values among the recurrent infection group, asthma group and control group (CD3+, CD4+, CD8+, CD4+/CD8+<0.05), and the expression levels of IL-4, IL-17 and IL-21 in the three groups also showed statistically significant differences (P<0.05).Conclusion:Bronchial asthma with recurrent infection may have a close bearing on the expression level of peripheral blood immunoregulatory T lymphocytes, and indirect examination of peripheral blood T lymphocyte level in children can be used clinically to determine the incidence of the disease, which is of high clinical application value.

Effectiveness of Pune shoulder rehab protocol on patients with frozen shoulder

Context: Frozen shoulder is a common condition which is seen in 2%–5% of the general population. It is a self-limiting condition which affects the range of motion of the patients in a capsular pattern. Aims: Although Pune Shoulder Rehabilitation Programme (PSRP) is being used for treating this condition still no study has been documented to find its efficacy as compared to conventional physiotherapy methods. Settings and Design: This was a quasi-experimental study and the study was done at Smt. Kashibai Navale General Hospital and Physiotherapy Outpatient Department, Pune. Institutional ethical clearance was taken on 7/8/2018 ref no. SKNCOPT/IEC/16/2018 before the commencement of this study. Subjects and Methods: Based on the study's inclusion criteria, 32 subjects were selected. The PSRP exercises were taught to each subject and given 10 repetitions with a hold of 10 s. For at least 45 min, each patient was treated. Patient's shoulder range of motion and visual analog scale were taken at the pretreatment, 2nd week, 4th week, and 6th week, respectively. Statistical Analysis Used: The data were analyzed using Primer software with a level of significance P < 0.05. Repeated measure ANOVA was used to compare the outcome measures within the group. Results: The findings showed that over the 6th-week protocol, statistically significant improvements were found in pain and shoulder range of motion. Conclusion: PSRP exercise protocol is effective in increasing the range of motion and decreasing the pain in the shoulder caused by frozen shoulder.

Service evaluation of an outpatient exercise class after critical illness

Objective To evaluate an outpatient exercise rehabilitation programme following critical illness and make recommendations to improve the service for patients. Design Mixed-method design using qualitative interviews with people who have recently completed the programme and audit data from the preceding two years. The programme is hosted within a UK district general hospital outpatient therapy department. Participants Adults who completed the physical rehabilitation programme between June 2016 and June 2018 were eligible to participate. Participants were interviewed in their own homes using a semi-structured format (n = 8). Audit data included 25 sets of participant data within this time frame which equates to around 3% of patients discharged from the critical care unit in the same period. Outcome measures Interviews were transcribed verbatim and analysed using grounded theory methodology. The primary outcome measure from audit data was exercise capacity (6-minute walk test). Secondary outcome measures were Health-Related Quality of Life (HRQOL), Anxiety and Depression (HADS). Results Participants struggled for independence and described the physical and psychological challenges associated with this. Audit data showed significant improvements in physical function, anxiety, depression, and HRQOL following the programme. Conclusions Physical and emotional challenges, recruitment, accessibility to resources and delivery of information to patients were barriers to implementation of an outpatient exercise programme. This service may promote physical function and vitality for patients, but findings are not generalisable due to small sample size and limited demographic information.

To do, or not to do, that is the question: Appropriateness of the Requested CT and MR Imaging Studies in the Philippine General Hospital Outpatient Department Based on the American College of Radiology Appropriateness Criteria

Objective. To determine the rate of the appropriateness of the requested CT and MRI procedures in the Outpatient Department of a tertiary hospital in the Philippines from January to June 2018 using the American College of Radiology Appropriateness Criteria (ACR-AC). Methods. This retrospective research reviewed outpatient CT and MR imaging requests and cross-referenced with the corresponding ACR-AC guideline available for the written clinical diagnosis. Results. Four hundred thirty-six (436) (56%) of the 774 retrieved requests were CT scan studies, while the remaining 338 (44%) are MR imaging procedures. Cross-referencing withACR-AC, the rate of appropriateness across all patients is high at 96.6%, with a 95% confidence interval of 95.0% and 97.6%. The rates were not significantly different between MRI and CT (p-value = 0.4502). Likewise, there was no significant difference in rates of appropriateness for the body parts abdomen, cranial, chest, and spine (p-value = 0.6502). Conclusion. Although the results were high, relative to the few available international studies, the importance and potential of the ACR-AC cannot be disregarded. The ACR-AC serves as a guide in selecting the appropriate imaging test given a clinical situation. This may equate to better patient management, considering all patient-related factors.

Knowledge, Attitude, and Practices Towards Tuberculosis Among Clients Visiting Tepi General Hospital Outpatient Departments, 2019.

BackgroundTuberculosis, which is an infectious disease, is one of the leading causes of morbidity and mortality in developing countries. Ethiopia is facing high tuberculosis burdens. Even if it is preventable and curable, individuals’ KAP towards the disease is one of the bottlenecks in decreasing the disease burdens. TGH, located in the Sheka zone, is one of the remote areas and the KAP towards TB is unknown. Therefore, the current study was undertaken in TGH to assess the KAP towards TB.Materials and MethodsA cross-sectional study was conducted among 415 randomly selected participants. A structured questionnaire was used to collect the data by a face-to-face interview from May 23 to June 23/2019. Data were entered into Epidata 3.1 and exported to SPSS version 21 for descriptive analysis.ResultsA total of 345 (83%) respondents have heard about TB, while 76 (18%) respondents said persistence productive cough as symptoms of tuberculosis. Only 9.9% of participants mentioned bacteria as the cause of the diseases and 170 (41%) considered that the transmission is via air droplets. The majority (70%) of participants responded that its transmission is not preventable and overall 236 (56.9%) had high overall knowledge about TB. Thirty percent of the respondents considered that TB is serious to the area while 29% considered TB is not very serious for them. Fifty-three percent of the participants were having a favorable attitude towards tuberculosis. The majority (85%) did not cover their mouth while coughing, while 79.5% did not screen for tuberculosis and 82% of participants have not received any health education about TB. Overall, 44.6% practiced TB prevention.ConclusionThe majority of the study participants had high overall knowledge and positive attitude towards tuberculosis prevention, which are not seen in the practice of tuberculosis. Effective educational programs should be implemented to overcome the problem.

The outpatient department of General Hospital A was selected as the study building, and the particle count of infectious microbes was analyzed according to the three outdoor air induction ratios. In the recirculation mode (OAir: 0%), the numbers of infectious microbe particles in the waiting room and examining room were not reduced by the direction of indoor airflow and recirculation. In the general air-conditioning method (OAir: 30%), the reduction in the numbers of infectious microbe particles in the waiting and examining rooms were slow due to lack of ventilation and congestion of indoor airflow, and slightly affected the doctor’s office through the corridor. In the case of 100% outdoor air system (OAir: 100%), the number of infectious microbe particles was reduced rapidly compared with the cases of recirculation and general air-conditioning, and it was shown to be suitable for preventing airborne infectious microbe diffusion. When the waiting room pressure was adjusted to -1.5pa based on the general air-conditioning method, airflow merged from the examining room toward the waiting room, increasing the number of infectious microbe particles in the waiting room but not leading to external diffusion and intra-diffusion. When the examination room pressure was –1.5pa, airflow was stagnated in the direction of the corridor or the nursing desk instead of air being induced from the waiting room to the examination room, which was not suitable for indoor airflow induction.

The purpose of this study is to measure and analyze indoor air flow in order to determine the possibility of HVAC operations causing airborne infections in outpatient departments of general hospitals in South Korea. Among 10 general hospitals, the emergency department in the children’s ward in the K University General Hospital using a typical central HVAC system was selected as the measurement space for this study. The indoor air flow direction was confirmed by measuring the interzonal air flow rate, differential pressure and tracer gas concentration for the central HVAC system of the study space, and three different changes in the outdoor air intake rate were applied to determine the interzonal air flow diffusion state. The study space had a high possibility of receiving patients that could cause airborne infections and there were risks of secondary infections from the airflow diffusing into adjacent spaces such as the otorhinolaryngology department or the waiting room corridor. The measurement results showed that using an HVAC system with 100% outdoor air reduced the concentration of gas rapidly, thereby lowering the probability of infection, and that the airflow did not diffuse into adjacent spaces when the study space had negative pressure.

Operationalization of diagnostic criteria of DSM-5 somatic symptom disorders

BackgroundThe aim of this study was to test the operationalization of DSM-5 somatic symptom disorder (SSD) psychological criteria among Chinese general hospital outpatients.MethodsThis multicenter, cross-sectional study enrolled 491 patients from 10 general hospital outpatient departments. The structured clinical “interview about cognitive, affective, and behavioral features associated with somatic complaints” was used to operationalize the SSD criteria B. For comparison, DSM-IV somatoform disorders were assessed with the Mini International Neuropsychiatric Interview plus. Cohen’s к scores were given to illustrate the agreement of the diagnoses.ResultsA three-structure model of the interview, within which items were classified as respectively assessing the cognitive (B1), affective (B2), and behavioral (B3) features, was examined. According to percentages of screening-positive persons and the receiver operator characteristic (ROC) analysis, a cut-off point of 2 was recommended for each subscale of the interview. With the operationalization, the frequency of DSM-5 SSD was estimated as 36.5% in our sample, and that of DSM-IV somatoform disorders was 8.2%. The agreement between them was small (Cohen’s к = 0.152). Comparisons of sociodemographic features of SSD patients with different severity levels (mild, moderate, severe) showed that mild SSD patients were better-off in terms of financial and employment status, and that the severity subtypes were congruent with the level of depression, anxiety, quality of life impairment, and the frequency of doctor visits.ConclusionsThe operationalization of the diagnosis and severity specifications of SSD was valid, but the diagnostic agreement between DSM-5 SSD and DSM-IV somatoform disorders was small. The interpretation the SSD criteria should be made cautiously, so that the diagnosis would not became over-inclusive.

The Role of Triple Combination Topical Agents in the Treatment of Facial Melasma

Melasma is an acquired, symmetrical hypermelanosis of the face and is particularly seen in women with skin types IV to VI living in areas with intense UVR. So it is a great problem in our country. Treatment of melasma remains a challenge. Topical therapy with a triple combination topical agents appears to be the most clinically effective initial therapy for patients with melasma. A clinical trial was conducted to find out the efficacy of the triple combination of 4% hydroquinone, 0.05% tretinoin and 0.01% fluocinolone acetonide cream in the treatment of facial melasma. Fifty clinically diagnosed cases of melasma attending the Skin and VD outpatient department of General Hospital, Faridpur were selected randomly. Majority (54%) were between 21 to 25 years of age and most of them (86%) were female. After 20 weeks of treatment with triple combination agents, the average MASI score decreased significantly (P&lt;.001). Reduction of the severity of melasma by 20 weeks is 82% and by 8 weeks is 71%. Regarding the distribution of melasma, 50% were found centrofacial, 45% malar and 5% mandibular. As side effects of the treatment, 36% developed erythema, 16% developed burning sensation, 6% developed pruritus and 4% developed desquamation. This study demonstrates that every night application of triple combination topical agent has significant effect on melasma.Faridpur Med. Coll. J. Jul 2017;12(2): 68-70

Prevalence and Recognition of Depressive Disorder in Three Medical Outpatient Departments of General Hospitals in Beijing, China.

This purpose of this study was to explore the prevalence and recognition of depressive disorders in cardiology, gastroenterology, and neurology outpatient departments of general hospitals. Patients screened with a Hospital Anxiety and Depression Scale score of 8 or higher were interviewed by psychiatrists using Mini-International Neuropsychiatric Interview (MINI). Prevalence of depressive disorders within the cohort was determined, sociodemographic data were analyzed for correlations to a depression diagnosis, and comparisons between the surveys and the clinical diagnosis were done to assess recognition of depressive disorders by physicians. Of the patients screened for this study (1552 cases), 12.8% were diagnosed with depressive disorders by MINI, with major depressive disorder, depression due to general medical conditions, and dysthymia having prevalence values of 10.8%, 1.4%, and 0.6%, respectively. As compared with MINI, physicians only recognized 27.6% of any of the depressive disorders. Among the complaints examined, both mood problems and sleeping problems predicted the probability of recognition.

Diagnosis and treatment of dementia in neurology outpatient departments of general hospitals in China

IntroductionThe status of dementia diagnosis and treatment of neurology outpatients in general hospitals in China remains unclear. MethodsFrom neurology outpatients at 36 randomly selected hospitals, we first collected baseline data concerning the number of dementia doctors, memory clinics, and patients diagnosed with dementia. In stage 2, we intervened based on drawbacks discovered in stage 1, implementing a dementia initiative program. In stage 3, we reinvestigated the outpatients to determine the effects of intervention. ResultsAfter intervention, all 36 hospitals had established memory clinics (205 dementia doctors) compared with only 6 (47 dementia doctors) before intervention. The percentage of patients diagnosed with dementia significantly increased from 0.10% (536 dementia patients of 553,986 outpatients) in stage 1 to 0.41% (2482 dementia patients of 599,214 outpatients) in stage 3. DiscussionProper diagnosis and treatment are unavailable to many dementia patients because of a lack of dementia doctors and memory clinics in China.

Female sexual dysfunction: A comparative study in drug naive 1(st) episode of depression in a general hospital of South Asia.

Background:Women's sexual dysfunction is found to be highly prevalent in western and Indian literature. Limited studies are available on drug naive depression in western literature and in Indian population.Aim:To determine the prevalence rate and symptom profile of female sexual dysfunctions in patients with untreated depression.Design:A cross-sectional study in the psychiatry out-patient department of general hospital in South India.Materials and Methods:Following written informed consent female sexual functioning index (FSFI) and Arizona Sexual Experience Scale (ASEX) – female version and Hamilton Depression Rating Scale (HAMD - 17 item) on 30 cases and 30 controls was administered. Sociodemographic data, pattern and type of sexual dysfunctions were enquired. Data were analyzed using descriptive statistics, contingency co-efficient analysis and stepwise multiple regression.Results:The mean score of HAMD 17 item in study group was 19.13. The study showed that female sexual dysfunction was 70.3% in study group compared to 43.3% in control FSFI scores above 16 in HAMD had dysfunction of 76% with FSFI in study group. With ASEX-F sexual dysfunction was 73.3% in study compared to 20% in control. Scores above 16 in HAMD had 80% of sexual dysfunction with ASEX-F in study group.Conclusion:The study found that ASEX-F co-related better with HAMD 17 item. Following the onset of depression, the incidence of sexual dysfunction started at an early age in women.

This study attempts to evaluate the interior space of outpatient department of general hospital in aspects of nurses. The subjects are 97 nurses who work in outpatient department of H general hospital, Busan. Evaluating items are ones related to space layout, furniture, interior climate, color and finishes of outpatient department. Also, evaluating spaces are waiting area, doctor's room, treatment room, nurse station, and resting and dressing area for nurses in outpatient department. The results are ; 1) The space layout type of outpatient area in general hospital which each department consists of waiting area, doctors offices, treatment room and etc. was evaluated positively by nurses. 2) The waiting area and doctor's room was evaluated relatively positive, the treatment room was more negatively than these two rooms. But nurse related spaces such as nurse station and resting and dressing room were evaluated relatively negative. 3) In aspects of interior elements, furniture, and color and finishes were rated to be relatively positive. However, air conditions were rated negatively. 4) The finishes and dressing area in treatment room were rated relatively low. This would be a basic data for planning the outpatient area of general hospital.
