Impact of vascular biomarkers and supine hypertension on cardiovascular outcomes in hypertensive patients: first results from the Cardiovascular Prognostic COUPLING Study in Japan.

The prognostic impact of vascular biomarkers and supine blood pressure (BP) is not well understood. The multicenter, prospective Coupling study determined the prognostic impact of vascular biomarkers and supine BP in outpatients aged ≥30 years with ≥1 cardiovascular risk factor. Occurrence of major cardiovascular events during follow-up was recorded. The primary outcome was time to onset of a major cardiovascular event. Office and supine BP, the cardio-ankle vascular index (CAVI), and the ankle-brachial index (ABI) were determined annually. Of the 5109 participants in the Coupling study, 4716 were analyzed (51.9% male, mean age 68.5 ± 11.4 years); participants mostly had hypertension treated based on seated office/home BP according to relevant guidelines. During a median follow-up of 5.0 years (interquartile range 3.6-5.2), 231 major cardiovascular events occurred. After adjustment for age, sitting office systolic BP, and other covariates, a 1-unit increase in CAVI (hazard ratio [HR] 1.12, 95% confidence interval [CI] 1.01-1.24) and a 0.1-unit decrease in ABI (HR 1.41, 95% CI 1.18-1.68) were significantly associated with cardiovascular event risk; risk was greatest when CAVI was ≥8.0 and ABI was ≤1.10. Uncontrolled supine hypertension (≥140/90 mmHg) was also significantly associated with adjusted cardiovascular event risk (HR 1.36, 95% CI 1.02-1.81); seated office BP control was not significantly associated with cardiovascular event risk. Increased arterial stiffness, mildly lower ABI, and supine hypertension are risk factors for cardiovascular events during standard clinical practice. Supine evaluation of BP and vascular biomarkers has highlighted a blind spot in current hypertension management (Clinical trial registration: University Hospital Medical Information Network Clinical Trials Registry, UMIN000018474).

Improving Mental Health Outcome in Hypertensive Patients: The Role of Relaxation Techniques

Background: Relaxation techniques (progressive muscle relaxation and deep breathing) have an impact on the patients with hypertension as they produce a relaxation response within the body and thus, are efficient in lowering the stress and blood pressure in the patients. Objective: This study aims to determine the role of relaxation techniques (progressive muscle relaxation and deep breathing) on improving mental health outcome in hypertensive patients (depression, anxiety and stress). Methodology: A Quasi-experimental design used to guide this study, it was conducted by use of a pre-posttest approach during the period 26th of November, 2023 to 7th of April, 2024. A non-probability (purposive) sampling has been performed during a selection of (100) patients who attending Outpatient Clinic in Al-Diwaniyah Teaching Hospital, (50) patients as control group and (50) patients as study group. The researcher used a socio-demographic and psychological parameter scale (DASS-21). Results: The findings of the current study show highly statistically significant differences in post-test between study and control group (at P ≤ 0.001). On other hand, there is no statistically significant difference in the pre-test between the two group of all psychological parameter (at P ≤ 0.05). Conclusion: Based on the study's findings, progressive muscle relaxation and deep breathing exercises can both proved to be significantly effective in improve mental health in hypertensive patients (depression, anxiety and stress). Recommendations: In order to reduce hypertension and other psychological parameters in patients with essential hypertension, the researcher suggested combining pharmacological therapies with non-drug therapy (relaxation techniques intervention) in the management of hypertension.

The Relationship Between Right Ventricular Longitudinal Strain and Adverse Outcome in Hypertensive Patients: 10-year Follow-up.

Previous studies showed the importance of right ventricular (RV) remodeling in patients with arterial hypertension and RV longitudinal strain was recognized as very sensitive parameter for detection of subtle cardiac impairment. However, its clinical importance in arterial hypertension has not been established so far. The present study aimed to evaluate the association between RV longitudinal strain (global and free-wall) on adverse outcomes measured by MACE in the large group of hypertensive patients who were followed for mean period of 10 years. This retrospective study finally included 544 hypertensive patients who underwent full echocardiographic examination including 2D speckle tracking imaging. between January 2010 and December 2014. MACE was considered as the primary outcome and it was defined by all-cause mortality, cardiovascular mortality, myocardial infarction, coronary artery by-pass, coronary stent implantation, stroke, development of heart failure, and occurrence of atrial fibrillation during follow-up. Patients who experienced MACE were older than those who did not. There was no difference in demographic and clinical parameters between MACE and non-MACE patients. There was no difference in RV diameter, but MACE patients had higher RV wall thickness. RV systolic function parameters were similar between the two groups. RV global and free-wall longitudinal strain were significantly lower in MACE patients (-22.3 ± 3.6 vs. -24.7 ± 3.9%, p < 0.001 and - 25.8 ± 4.2 vs. -28.1 ± 4.5%, p < 0.001; respectively). Reduced RV GLS [OR 1.10; 95%: 1.02-1.20] and reduced RV free-wall longitudinal strain [OR 1,21; 95%CI: 1.05-1.39] were independently of clinical and echocardiographic parameters related with adverse outcome measured by MACE. RV GLS and RV free-wall longitudinal strain were independently related with adverse outcomes during 10-year follow-up in initially uncomplicated hypertensive patients.

Pharmacogenomic Study of Selected Genes Affecting Amlodipine Blood Pressure Response in Patients with Hypertension.

Despite the availability of various antihypertensive medications, the response to these medications varies among individuals. Understanding how individual genetic variations affect drugs treatment outcomes is a key area of focus in precision medicine. This study investigated the correlation between single nucleotide polymorphisms (SNPs) in selected genes (CACNA1C, CACNA1D, ABCB1, ACE, ADBR2, and NOS1AP) and the blood pressure (BP) control by amlodipine. Four hundred individuals of Pashtun ethnicity undergoing amlodipine treatment for hypertension were included in the present study and divided into the controlled (BP less than 140/90mmHg) and uncontrolled (BP greater than 140/90mmHg) hypertension groups. Blood samples (3 mL) were collected from each participant, and DNA was extracted using the Kit method. Ten SNPs in amlodipine pharmacogenes were selected and genotyped using real-time PCR with the TaqMan® system. Logistic regression model was used to determine the association between SNPs and the amlodipine BP response. Notable association were observed between SNP rs2239050/CACNA1C and amlodipine blood pressure response, with GG genotype carriers demonstrating a better response (P=0.004) than individuals carrying CC or CG genotypes. SNP rs312481/CACNA1D also exhibited a positive pharmacogenetic association, Individuals with the GG genotype showing a considerable reduction in BP (P=0.021) compared to participants with AA or GA genotypes. In case of SNP rs429/ACE individuals carrying TA genotype were less likely to achieve BP control (P=0.002) than AA genotype carriers. Our finding suggests that the SNPs rs2239050/CACNA1C, rs312481/CACNA1D and rs429/ACE influence amlodipine blood pressure response in patients with hypertension. It is recommended that prior knowledge of amlodipine associated pharmacogenetic variants is important that could improve its treatment outcomes in hypertensive patients.

Excessive Daytime Sleepiness in Hypertensive Patients: The Role of Major Depressive Disorder.

There is a special relationship between major depressive disorder and excessive daytime sleepiness. However, given the negative impact of excessive daytime sleepiness on life quality and cardiovascular outcome in hypertensive patients, the objective of this study was to investigate the potential role played by major depressive disorder in the occurrence of this complaint for this particular subpopulation. Data from 1404 hypertensive patients recruited from the Sleep Unit's polysomnographic recordings database were analyzed. A score >10 on the Epworth Sleepiness Scale was used to define excessive daytime sleepiness in this study. Logistic regression analyses were performed to investigate the risk of excessive daytime sleepiness associated with major depressive disorder in hypertensive patients. Excessive daytime sleepiness was frequent (40.0%) in our sample of hypertensive patients. After adjustments for major confounding factors, multivariate logistic regression analyses demonstrated that unlike remitted major depressive disorder, only current major depressive disorder was associated with a higher risk of excessive daytime sleepiness in hypertensive patients. Given this potential implication of current major depressive disorder in the occurrence of excessive daytime sleepiness for hypertensive patients, it is therefore essential to achieve the complete remission of this psychiatric disorder to avoid negative consequences associated with this complaint in this particular subpopulation.

Systolic blood pressure time in target range within 24 hours and incident heart failure: insights from the real-world setting.

Systolic blood pressure (SBP) time in target (TTR) over months were associated with lower risk of adverse clinical outcomes in hypertensive patients, whether short-term of 24-h SBP TTR was effective in predicting heart failure (HF) risk in the general population remained unclear. This prospective study aimed to investigate the association of 24-h SBP TTR with HF in the real-world settings. Based on Kailuan study, 24-h SBP target range defined as 110-140 mmHg was calculated with linear interpolation. Among 5152 participants included in the analysis, 186 (3.61%) cases of incident HF occurred during a median follow-up of 6.96 years. Compared with participants with SBP TTR of 0 to <25%, those with TTR of 75% to 100% had 47% lower risk of HF (hazard ratio [HR], 0.53; 95% confidence interval [CI], 0.32-0.89). The restricted spline curve depicted an inverse relationship between SBP TTR and incident HF. Additionally, the addition of SBP TTR, rather than mean SBP and SBP variation, to a conventional risk model had an incremental effect on the predictive value for HF, with integrated discrimination improvement value of 0.31% (P = 0.0003) and category-free net reclassification improvement value of 19.79% (P = 0.0081). Higher SBP TTR was associated with a lower risk of incident HF. Efforts to attain SBP within 110 to 140 mmHg may be an effective strategy to prevent HF.

Influence of multiple risk factor control level on cardiovascular outcomes in hypertensive patients.

The relationship between the level of baseline risk factor control and cardiovascular outcomes in hypertensive patients with blood pressure intervention is not well understood. It is also unclear whether the level of baseline risk factor control is persuasively associated with cardiovascular outcomes in hypertensive patients with blood pressure lowering strategy. We performed an analysis of the Strategy of Blood Pressure Intervention in the Elderly Hypertensive Patients (STEP) trial. Participants without complete baseline risk factor data were excluded. The primary outcome was a composite of cardiovascular events and all-cause mortality. Cox proportional hazard models were used to calculate the hazard ratio (HR) and estimate association between risk factor control levels (≥6, 5, 4, and≤3) and cardiovascular outcomes. A total of 8337 participants were involved in the analysis and the median follow-up period was 3.19 years. Each additional risk factor uncontrolled was associated with a 24% higher cardiovascular risk (HR 1.24, 95% CI 1.11-1.37). Compared with participants with optimal risk factor control, those with≤3 factors control exhibited 95% higher cardiovascular risk (HR 1.95, 95% CI 1.37-2.77). The corresponding protective effects of multiple risk factor modification were not influenced by intensive or standard antihypertensive treatment (P for interaction=0.71). A stepwise association was observed between cardiovascular risk and the number of risk factor control in hypertensive patients. The more risk factor was modified, the less cardiovascular risk was observed, irrespective of different blood pressure lowering strategies. Comprehensive risk factor control strategies are warranted to reduce cardiovascular disease risk in hypertensive patients. Trial registration STEP ClinicalTrials.gov number, NCT03015311. Clinical outcome in hypertensive patients treated with amlodipine plus bisoprolol or plus valsartan

Objective Several guidelines do not recommend beta-blocker as the first-line treatment for hypertension because of its inferior efficacy in stroke prevention. Combination therapy with beta-blocker is commonly used for blood pressure control. We compared the clinical outcomes in patients treated with amlodipine plus bisoprolol (A + B), a ß1-selective beta-blocker and amlodipine plus valsartan (A + V). Methods A population-based cohort study was performed using data from the Taiwan National Health Insurance Research Database. From 2012 to 2019, newly diagnosed adult hypertensive patients who received initial amlodipine monotherapy and then switched to A + V or A + B were included. The efficacy outcomes included all-cause death, atherosclerotic cardiovascular disease (ASCVD) event (cardiovascular death, myocardial infarction, ischemic stroke, and coronary revascularization), hemorrhagic stroke, and heart failure. Multivariable Cox proportional hazards model was used to evaluate the relationship between outcomes and different treatments. Results Overall, 4311 patients in A + B group and 10980 patients in A + V group were included. After a mean follow-up of 4.34 ± 1.79 years, the efficacy outcomes were similar between the A + V and A + B groups regarding all-cause death (adjusted hazard ratio [aHR] 0.99, 95% confidence interval [CI] 0.83–1.18), ASCVD event (aHR 0.97, 95% CI 0.84–1.12), and heart failure (aHR 1.06, 95% CI 0.87–1.30). The risk of hemorrhagic stroke was lower in A + B group (aHR 0.70, 95% CI 0.52–0.94). The result was similar when taking death into consideration in competing risk analysis. The safety outcomes were similar between the 2 groups. Conclusions There was no difference of all-cause death, ASCVD event, and heart failure in A + B vs. A + V users. But A + B users had a lower risk of hemorrhagic stroke.

Medication adherence and therapeutical outcome in hypertension patients at the Mlati II health center

Background: Hypertension is a chronic disease that needs to be treated properly and continuously. One of the critical successes of therapy is medication adherence by patients.Objective: Determine the correlation between medication adherence and therapeutic outcome in hypertension patients at the Mlati II health center.Methods: This is a non-experimental study with a cross sectional design analysis, by purposive sampling technique. Adherence was obtained in the form of adherence levels measured using the MMAS-8 questionnaire, therapeutic outcomes assessed by measuring a patient's blood pressure. Then the data were analyzed using the Chi-square test to determine the relationship between the level of adherence and therapeutic outcomes.Results: From 76 hypertension patients, the majority aged 46-55 years (44.7%), female gender (69.7%), senior high school (39.5%), employment status did not work (55.3%), long suffering from hypertension &lt;5 years (76.3%), received single therapy (61.8%) with the type of antihypertension drug amlodipine (59.2%), low adherence category rate (39.5%), outcome of therapy with uncontrolled blood pressure (60,5%), and significancy level p = 0,997&gt;0,05Conclusion: There is no relationship between the level of adherence to the use of antihypertension drugs and the outcome therapy of controlled blood pressure at the Mlati Il health center

Comparison of the Effects of Nitroglycerin, Labetalol and Lidocaine in Hypertensive Patients in Attenuation of the Endotracheal Intubation Reflex

Background: Laryngoscopy and tracheal intubation integral parts of general anesthesia which may cause acute haemodynamic instability. Hypertensive patients are more prone of developing cardiovascular complications such as pulmonary oedema, cardiac failure and cerebrovascular haemorrhage. To attenuate such intubation reflex, some drugs are used, e.g., opioids. lidocaine sodium nitroprusside, nitroglycerin, beta blockers, calcium channel blockers etc. Objective: The purpose of the study is to see the effectiveness of nitroglycerin, labetalol and lidocaine in attenuation of intubation reflex and their anesthetic outcome in hypertensive patients. Methods: This randomized controlled study was carried out in the Department of Anesthesia, Pain, Palliative and Intensive care Medicine, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh, between March and September of 2021. A total of 90 patients were included: 30 in each group as per inclusion and exclusion criteria. Group A patients received inj. nitroglycerin 2 minutes before intubation, while group B patients received inj. labetalol 0.25mg/kg 5 minutes before intubation, and group C was given 2% lidocaine 1.5mg/kg 2 minutes before intubation. Haemodynamic status of all patients was checked before and after intubation. Results: Mean age of the patients was 44.4±10.9 years in group A, 47.6±9.4 years in group B and 46.4±10.6 years in group C. Heart rate after intubation was low and statistically significant in groups-B after 1.2 and 5 minutes. Regarding mean arterial pressure (MAP), group A patients had significantly low blood pressure. Rate pressure product (RRP) of the patients was significantly low in group B at 1 minute, 2 minutes and 5 minutes. Regarding ECG changes sinus tachycardia was observed in 26.6%, 86.6% and 20% in group A, group B and group C immediately after intubation. Groups B patients showed lower incidence of sinus tachycardia than that of two other groups and showed no premature ventricular contractions, whereas group A (6.7%) and group C (3.3%) showed premature ventricular contractions. Conclusion: Labetalol showed better rhythm control, mean arterial pressure and less incidence of tachycardia. To summarize, labetalol is safer and more effective than nitroglycerin and lidocaine to attenuate the endotracheal intubation reflex in hypertensive patients. Mugda Med Coll J. 2023; 6(2): 64-70

THE IMPACT OF HYPERTENSION ON COVID-19 SEVERITY: A GLOBAL REVIEW

Hypertension, also known as high blood pressure, is a prevalent medical condition that can exacerbate the severity of coronavirus disease 2019 (COVID-19). Studies from around the globe have shown that hypertensive patients infected with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) tend to experience more severe symptoms and have poorer outcomes. A meta-analysis of eight studies from China found that hypertensive patients were over twice as likely to develop severe COVID-19 illness that requires intensive care unit admission, mechanical ventilation or results in death. Several other analyzes of data from China, Italy, and the United States have come to similar conclusions, showing roughly double the risk of adverse outcomes in hypertensive patients. In summary, hypertension appears strongly linked to worse COVID-19 outcomes across the world. Patients should maintain good blood pressure control and take recommended precautions to avoid SARS-CoV-2 infection. Clinicians should closely monitor hypertensive patients diagnosed with COVID-19 due to their increased risk of critical illness. Continued research on how hypertension influences COVID-19 severity can help guide therapeutic approaches for this high-risk population.

Different systolic blood pressure targets and outcomes: a systematic review and meta-analysis of randomized trials

Abstract Background Blood pressure (BP)-lowering is accompanied by a reduction of fatal or nonfatal outcomes in hypertensive patients. However, the achieved systolic BP target associated with higher outcome benefits remains unsettled. Purpose The aim was to determine the effects of BP-lowering treatment on outcome incidence at different achieved systolic BP strata. Methods A systematic review was conducted to identify randomized trials in electronic databases (Pubmed and CENTRAL, search between 1966 and 11/2022) for the effect of BP-lowering treatment against placebo or less aggressive treatment on the composite outcome of major cardiovascular (CV) events (i.e., myocardial infarction, stroke, and heart failure). Selected studies reported outcomes in patients with or without a history of previous CV disease. Studies were categorized according to the achieved level of systolic BP in the active or more actively treated group. Risk ratios (RR) were calculated with their 95% confidence interval (CI) under the random-effects model. Results We included 51 primary prevention studies (i.e., 16 trials in which systolic BP was lowered to values no less than 140 mmHg, n=36,189 patients; 22 trials with achieved systolic BP between 130 and 140 mmHg, n=84,952; and 13 trials with achieved systolic BP less than 130 mmHg, n=41,660) and 44 secondary prevention studies (i.e., 11 trials in which systolic BP was lowered to values no less than 140 mmHg, n=12,518 patients; 19 trials with achieved systolic BP between 130 and 140 mmHg, n=71,449; and 14 trials with achieved systolic BP less than 130 mmHg, n=39,085). For a standard systolic/diastolic BP reduction of 10/5 mmHg, the relative risk of the composite outcome was reduced in all achieved systolic BP strata, without a significant trend from higher to lower systolic BP targets, in both prevention groups. In addition, there was no difference in the absolute risk reduction of the combined outcome, as calculated by the number of events prevented in every 1000 patients treated for 5 years, between the systolic BP targets in each prevention group. However, only in primary prevention did a significant negative incremental trend from higher to lower systolic BP targets for the residual risk of the combined outcome become apparent. Conclusions Although antihypertensive treatment provides CV protection to all hypertensive patients, the lower systolic BP target is associated not only with outcome benefits but also with a lower rate of residual risk in primary prevention only.

Abstract 16573: Orthostatic Blood Pressure Changes and Cognitive Function: Insights From the SPRINT MIND Trial

Introduction: The association between orthostatic blood pressure (BP) changes and cognitive function has not been thoroughly investigated. Moreover, whether orthostatic hypotension or hypertension modifies the effects of intensive BP control on cognitive outcomes is unknown. Hypothesis: We hypothesized that orthostatic BP changes were associated with cognitive function and the effects of intensive BP control on cognitive outcomes might be different among individuals with/without orthostatic hypotension or hypertension. Methods: We performed a post hoc analysis of SPRINT MIND, a randomized controlled trial evaluating the effects of intensive vs. standard SBP control on cognitive outcomes in hypertensive patients. The relationships between baseline orthostatic BP changes (defined as baseline standing systolic BP [SBP]–baseline seated SBP) and probable dementia (PD) and mild cognitive impairment (MCI) were analyzed. The competing risk proportional hazards regression and cubic spline regression were used in analysis. Results: A total of 8547 participants (mean [SD] age, 67.9 [9.3] years; 5543 [64.9] men) were included in this analysis. In the crude model, the restricted cubic spline analysis showed L-shaped associations between orthostatic SBP change and new-onset PD and MCI (all P for nonlinearity&lt;0.05). However, these associations all disappeared after adjustment for covariates. Moreover, orthostatic diastolic BP change was not associated with the PD or MCI incidence. Intensive BP control did not alter the risk of PD or MCI in patients with/without orthostatic hypotension or hypertension. Conclusions: Orthostatic BP changes were not associated with the risk of PD or MCI; baseline orthostatic hypotension or hypertension did not influence the effects of intensive BP control on cognitive outcomes. These results are hypothesis-generating and merit further study.

Abstract 11900: Potassium Magnesium Citrate is Superior to Potassium Chloride in Reversing Metabolic Side Effects of Chlorthalidone

Thiazide-type diuretics (TD) are the first-line treatment of hypertension in many guidelines because of its consistent benefit in lowering BP and cardiovascular risk. TD is also known to cause excess risk of diabetes mellitus (DM), which may limit its long-term use. Although hypokalemia was thought to be the main mechanism of TD-induced dysglycemia, potassium (K) supplementation alone does not consistently prevent TD-induced elevation in fasting plasma glucose (FPG). In addition to reducing serum K, TD triggers magnesium (Mg) depletion. However, it is unknown if Mg supplementation attenuates metabolic side effects of TD. Accordingly, we conducted a randomized double-blinded clinical trial in 60 hypertensive patients without DM to compare effects of KCl (40 meq daily) vs. KMgCitrate (Cit) powder containing identical amount of K, 20 meq of Mg, and 74 meq of Cit daily during chlorthalidone (CTD) treatment. Each patient received CTD alone (25 mg/day) for 3 weeks prior to randomization. The primary endpoint was the change in FPG after 4-month of treatment from baseline (CTD alone) after adjusting for baseline FPG as a covariate. The mean age of subjects was 59±11 years (45% Female, 30% Black participants). We found that CTD induced a significant rise in FPG, insulin, and a significant fall in serum K and 24-h urinary citrate excretion (all p &lt; 0.05). We found that KMgCit significantly increased serum K, 24-hour urinary Mg, and 24-hour urinary Cit excretion; and attenuated the rise in FPG which was not observed with KCl (Figure 1 A-C). There were no significant differences in liver fat by MRI or fasting plasma insulin between the 2 groups. In conclusion, our study showed beneficial effects of magnesium supplementation in maintaining serum K and reversing effects CTD on plasma glucose, which may improve safety profile and optimize long-term cardiovascular outcomes in hypertensive patients.

Ratio of waist circumference to body mass index: A novel predictor of clinical outcome in hypertension patients.

We aim to investigate the influence of waist circumference and body mass index (BMI) on all-cause death and cardiovascular-specific death in patients with hypertension. This prospective cohort study, based on waist circumference and body mass index measurements in patients with hypertension, provided risk estimates of all-cause mortality and cardiovascular events. The waist circumference-to-BMI ratio (WtBR) is an anthropometric measure integrating waist circumference and BMI. We utilized multivariable Cox regression analysis, restricted cubic spline model, Kaplan-Meier plot, random forest analysis, and sensitivity analysis to assess the relationship of WtBR with all-cause mortality. Subsequently, Fine-Gray competing risk regression models were applied to precisely evaluate the probability of cardiovascular-specific death attributed to high WtBR. The results indicate that thea deceased group showed significantly higher WtBR and lower BMI compared with the alive groups (P<.05), while no significant difference was observed in waist circumference (P=.373). When analyzed as continuous, the risk of all-cause death elevated with increasing WtBR in the adjusted model with an HR of 2.42 (95% CI, 2.06-2.85). The restricted cubic spline illustrated an elevated risk of all-cause mortality as WtBR increased (J-shaped curve). Nevertheless, WtBR showed no significant association with cardiovascular-specific death and the prediction model exhibited a reliable performance in the testing set. This study supported that WtBR, an anthropometric measure, is independently associated with all-cause death in hypertensive patients. It's advisable to routinely assess waist circumference in hypertensive patients regardless of BMI, in order to more effectively manage the risk of obesity-related health.

Prevalence of anemia and associated factors among adult hypertensive patients in Referral Hospitals, Amhara Regional State

Anemia is a risk factor for adverse cardiovascular disease outcomes in hypertensive patients. Chronic anemia increases preload, reduces afterload, and leads to increased cardiac output in hypertension patients. In the long term, this may result in maladaptive left ventricular hypertrophy, which in turn is a well-recognized risk factor for cardiovascular disease outcomes and all-cause mortality in hypertension. Low hemoglobin and hematocrit levels might be strongly indicate hypertensive end-organ damage, specifically kidney failure. Therefore, this study determined the prevalence of anemia and associated factors among hypertensive patients in Referral Hospitals, Amhara Regional State, Ethiopia, in 2020. An institution-based cross-sectional study was conducted in Amhara Regional Referral Hospitals from February 20 to April 30, 2020. Random and systemic sampling techniques were used to select 428 study participants. Data were entered and coded in to Epi data version 3.0 and then exported into STATA 14 for analysis. In bivariable logistic regression, variables with a p-value of < 0.25 were included in multivariable logistic regression. Using a 95% confidence interval, variables having a p-value ≤ 0.05 in multivariable logistic regression were declared as statistically significant variables. In this study, a total of 428 study participants were involved with 99.5% response rate. The prevalence of anemia among hypertensive patients was 17.6%, with a 95% CI (14.3–21.5%). Estimated glomerular filtration rate (eGFR) < 90 ml/min (AOR = 2.77, 95% CI 1.56- 4.92)], duration of hypertension (HTN) ≥ 5 years (AOR = 2.37, 95% CI 1.36–4.15), uncontrolled blood pressure (AOR = 1.91, 95% CI 1.08–3.35), and higher pulse pressure (AOR = 1.05 (95% CI 1.02–1.08) were significantly associated with anemia. Nearly one out of five hypertensive patients had anemia. Impaired estimated glomerular filtration rate, duration of HTN, blood pressure status, and pulse pressure were the independent predictors of anemia among hypertensive patients. Screening hemoglobin level at a regular interval is recommended for the hypertensive patients to take an appropriate intervention.

The Impact of Pharmaceutical Care Implementation on The Incidence of Drug-Related Problem and Clinical Outcome of Hypertension Patients at Puskesmas Lubuk Pakam in 2021

&#x0D; &#x0D; &#x0D; &#x0D; The role of pharmacists in the implementation of pharmaceutical care has been shown to improve the outcome of therapy in hypertension patients in various countries. This study aimed to implement pharmaceutical care for hypertension patients and determine its impact on the incidence of drug-related problems (DRPs) and clinical outcomes of hypertension patients in Puskesmas Lubuk Pakam. This study used a comparative experimental method before and after the intervention of 73 hypertension patients in March-August 2021. Identification of the of DRPs was using the PCNE V9.00 standard and blood pressure values were obtained from direct examination of patients. Data were analyzed using the Wilcoxon Signed-Rank test. The results showed that the average DRPs incidence was significantly reduced after the intervention (observation, interview, and education) on the hypertension patients. The average blood pressure before intervention was 154.38 ± 16.20 mmHg and after intervention became 144.04 ± 15.94 mmHg (p value = 0.000). Based on the results of the study, it can be concluded that the application of pharmaceutical care can reduce the incidence of DRPs and improve clinical outcomes in hypertension patients in Puskesmas Lubuk Pakam.&#x0D; &#x0D; &#x0D; &#x0D;

Health literacy among adult hypertensive patients in a tertiary hospital in South-South, Nigeria

Background: Health literacy is a concept that seeks to help patients to understand, appraise, use health information and decide on issues related to their health. Hypertension is a chronic disease and global health concern with consequences of increased health care cost and reduced quality of life. Good health literacy level with adherence to medication and good patient attitude are expected to improve outcomes in hypertensive patients. Objective: The study was undertaken to determine the relationship between health literacy, adherence to medication and patient health attitude so as to expose patients to the concept of health literacy and how they can improve their health. Method: Following ethical approval, a descriptive cross sectional study of 120 adult hypertensive patients attending DELSUTH was carried out over a six months period. Only those who met the inclusion criteria participated. Data was collected through interview based administered structured questionnaire with closed ended questions. Questionnaire had 4 sessions which included; sociodemography, health literacy level, health attitude and adherence to medications. Data was analysed with SPSS. Health literacy score and patient attitude were graded good, fair and poor while adherence to medications was assessed by asking the patient's if they took their medications regularly. Results: With p value set at 0.05, there was statistical significance between health literacy and level of education of adult hypertensive patients (p=0.007) There was also statistical significance between health literacy and patient's attitude (p=0.007) Conclusion: level of education is strongly linked to the health literacy level of a patient and this inturn impacts patient's attitude to health care.

Korelasi Tingkat Pengetahuan Terhadap Keberhasilan Terapi Pasien Hipertensi di Puskemas

Knowledge can affect the success of therapeutic outcomes in hypertensive patients. Blood pressure must be controlled to prevent risk factors for other cardiovascular diseases. The purpose of this study was to analyze the relationship between knowledge of patients with hypertension and the success of therapy outcomes that showed controlled blood pressure. This observational cross-sectional study was conducted from March to June 2021 at Tangerang District Health Centers. A sample of 85 outpatients with hypertension was administered the HK-LS questionnaire to measure their level of knowledge, while the results of therapeutic outcomes were measured using blood pressure. The results showed that 60% of patients had intermediate knowledge and only 28.24% of the subjects had well-controlled blood pressure. There was a correlation between knowledge and therapy outcomes (p&lt;0.05) with coef. correlation was -0.336, means a weak and inverse correlation. Comorbidity and sex were significantly correlated with knowledge (p&lt;0,05). The level of knowledge needs to be improved to decrease the number of comorbidities or risk of complications to raise the goal of therapy.

Neuron-specific enolase in hypertension patients with acute ischemic stroke and its value forecasting long-term functional outcomes

BackgroundNeuron Specific Enolase (NSE), a neuro-biochemical protein marker, may correlate with the prognosis of stroke patients. Moreover, hypertension is the most common comorbidities in patients with acute ischemic stroke (AIS), and the relationship between NSE levels and long-term functional outcomes in such an increasingly large population is unclear. The aim of the study was to investigate the relationships mentioned above and optimize the prediction models.MethodsFrom 2018 to 2020, 1086 admissions for AIS were grouped as hypertension and non-hypertension, while hypertension group was randomly divided into development and validation cohorts for internal validation. The severity of the stroke was staged by National Institutes of Health Stroke Scale (NIHSS) score. Stroke prognosis after 1 year of follow up was documented by modified Rankin Scale (mRS) score.ResultsAnalysis revealed the following findings:(i) Serum NSE levels increased greatly in hypertension subjects with poor functional outcomes(p = 0.046). However, there was no association in non-hypertension individuals(p = 0.386). (ii) In addition to the conventional factors (age and NIHSS score), NSE (OR:1.241, 95% CI: 1.025–1.502) and prothrombin time were significantly related to the incidence of unfavorable outcomes. (iii)Based on the above four indicators, a novel nomogram was established to predict the prognosis of stoke in hypertension patients with the c-index values of 0.8851.ConclusionsOverall, high baseline NSE is associated with poor 1-year AIS outcomes in hypertension patients, suggesting NSE may be a potential prognostic and therapeutic target for stroke in hypertension patients.
