Quality of Life (QoL) changes after orthognathic surgery: Do they correlate with the quantum of hard and soft tissue change?

Aim: To investigate the relationship between post orthognathic surgery changes in soft and hard tissue cephalometric variables in Class II and Class III patients and quality of life (QoL) measured using a condition-specific QoL questionnaire and to determine cephalometric predictors of the overall OQLQ after surgery. Methods: The sample included 50 orthodontic patients, 28 Class II and 22 Class III skeletal relationships whose treatment included orthognathic surgery. Correlations between cephalometric changes and Orthognathic Quality of Life Questionnaire (OQLQ) were tested by Pearson’s correlation. Multiple linear regression was used to determine cephalometric predictors of the overall OQLQ after surgery. Results: In Class II patients, OQLQ before surgery and changes in SNA were significant predictors for OQLQ after surgery. For Class III patients, OQLQ after surgery was significantly correlated with the change in mandibular plane angle and lower face height (r = 0.6 and r = 0.5 respectively). The decrease in facial angle was negatively correlated with OQLQ (r = -0.4). Mandibular plane angle, mandibular length, and OQLQ before surgery were significant predictors for OQLQ after surgery. Conclusion: QoL improved for Class II and III after orthognathic surgery. Changes in certain cephalometric measurements seem to predict quality of life after orthognathic surgery.

Health-related quality of life and satisfaction following orthognathic surgery: a prospective cohort study.

This study investigates the motivations for orthognathic surgery and assesses the quality of life (QoL) and satisfaction among patients treated at a hospital over 12months. We employed an Arabic version of the Orthognathic Quality of Life Questionnaire (OQLQ), used pre-surgery and at 2weeks, 3months, and 6months post-surgery. This included demographic data, the OQLQ, and visual analogue scales (VAS). The OQLQ, originally by Cunningham et al., was translated and adapted by Al-Asfour et al. Additional validated questions were added to both pre- and post-operative surveys. Of 136 participants (51 males, 85 females, average age 25.1), most underwent surgery for facial aesthetics (85.2%) and bite correction (57.3%). Treatments included various osteotomies. OQLQ scores significantly dropped from 63.3% pre-surgery to 23% at 6months, showing QoL improvement. 97.8% reported better psychological status post-surgery (p = 0.0001), with 94.1% satisfaction at 6months (p = 0.0001). The orthognathic surgery yielded positive outcomes in functional and psychological aspects, leading to high satisfaction and improved QoL in patients with dentofacial deformity.

Health-related quality of life, jaw function and sleep-disordered breathing among patients with dentofacial deformity.

Patients with dentofacial deformity (DFD) requiring orthognathic treatment have poor aesthetics, jaw function and psychological well-being, which potentially affect the quality of life. This study aimed to investigate the health-related general, oral and orthognathic quality of life, jaw function and sleep-disordered breathing at different stages of orthognathic surgical treatment. A total of 120 consecutive patients with DFD were recruited and grouped as pre-orthodontic treatment (group 1), pre-surgery (group 2), 4 months post-surgery (group 3), 24 months post-surgery (group 4) and in addition 30 controls without DFD (group 0). Outcomes were assessed using general health Short Form Survey (SF-36), Oral Health Impact (OHIP-14), Orthognathic Quality of Life Questionnaire (OQLQ), STOP-Bang and Jaw Function Limitation Scale (JFLS) questionnaires. In addition, presence or absence of pain was recorded. Data were tested with analysis of variance, Kruskal-Wallis test, Tukey post hoc test and structural equation modelling (SEM). Results revealed SF-36 (p = .814) and STOP-Bang (p = .143) total scores did not differ between control and treatment groups. In contrast, OHIP-14, OQLQ and JFLS total scores differed between groups (p = .001). Higher scores were observed in groups 1 (p = .001), 2 (p = .001) and 3 (p = .041) compared to group 0, indicating poor oral health in patients with DFD. Importantly, in group 4, oral health-related quality of life was better, and OHIP-14 (p = .936) and JFLS (p = .572) scores did not differ from controls. OQLQ scores of group 4 were significantly lower than group 1 (p = .001) but higher than group 0 (p = .013). SEM results revealed a significant negative associations of pain with JFLS and OQLQ; OHIP-14 with OQLQ; OHIP-14 with SF-36; and finally STOP-Bang with SF-36. Positive associations were observed between JFLS and OHIP-14; OHIP-14 and OQLQ. Oral health-related quality of life and jaw function appears to be improved 24 months after orthognathic surgery. Pain and limitation in jaw function had a negative association with health-related quality of life.

Assessment of Children and "Young" Adult Patients' Quality of Life after Prosthetic Treatment of Disorders of the Craniofacial Region-A Retrospective Study.

Prosthetic rehabilitation for children and juvenile patients with congenital or acquired craniofacial disorders is the area of activity of many clinicians and is a major diagnostic and therapeutic challenge for dentists. Studies were carried out on a group of 30 patients (10 female and 20 male) aged 2.5 to 30 years who were treated prosthetically due to congenital and acquired craniofacial disorders. The aim of this study was to assess the quality of life using the CPQ 11-14 (Child Perception Questionnaire 11-14), OQLQ (Orthognathic Quality of Life Questionnaire), and PIDAQ (Psychosocial Impact of Dental Aesthetics Questionnaire). Before the prosthetic treatments, the mean values in the individual questionnaires were, respectively, CPQ 11-14-54 points; OQLQ-44.4 points; and PIDAQ-47.6 points. The following values were obtained after the treatments: CPQ 11-14-12.43 points; OQLQ-22.27 points; and PIDAQ-34.03 points. All obtained differences were statistically significant. The total numerical results obtained in all questionnaires decreased, which means that prosthetic rehabilitation had a positive impact on the assessment of the quality of life based on individual categories.

The Dutch Version of the Orthognathic Quality of Life Questionnaire (OQLQ-NL): Validation for Cleft Patients.

The aim of this study is to validate the Dutch version of the Orthognathic Quality of Life Questionnaire (OQLQ-NL) for cleft patients who received orthognathic surgery. To compare the OQLQ-NL with the CLEFT-Q, we used a convenience sample of thirty-two cleft patients. Using the Cronbach's alpha coefficient for multiple item scales, internal reliabilities of the OQLQ-NL were evaluated. The OQLQ-NL was repeated at a two-week interval and the intraclass correlation coefficient was calculated, to establish of the test-retest reliability. The construct validity of the OQLQ-NL was evaluated by using Spearman's correlation to test its correlation with the CLEFT-Q. Thirty-two patients filled in the OQLQ-NL and CLEFT-Q. The OQLQ-NL had excellent results in internal reliability and test-retest reliability. The vast majority of the correlations between the domains and scales of the questionnaires were as expected. Data from this study and previous studies confirm the construct validity of the OQLQ-NL. Our results suggest the OQLQ-NL is a valid and reliable instrument for measuring quality of life in cleft patients who have received orthognathic surgery in the Netherlands.

Effect of surgery-first approach on quality of life and mental health of orthognathic patients: A systematic review and meta-analysis

ObjectivesThis study intends to explore the effects of the surgery-first approach (SFA) on quality of life and mental health of patients who undergo orthognathic surgery compared to the conventional three-stage approach (CTA). DataThe analysis included eight studies with a total of 307 patients, of which one was randomized controlled trial (RCT), one was clinical controlled trial (CCT), and six were non-randomized studies of interventions (NRSIs). SourcesElectronic databases such as Medline, Embase, Scopus, and Web of Science were searched for eligible trials up to April 2023. Study selectionRCTs, CCTs, and NRSIs, which compared the quality of life or mental health of orthognathic patients treated with SFA and CTA, were included in this study. The meta-analysis showed that the standardized mean differences (SMD) of Oral Health Impact Profiles-14 (OHIP-14) scores and the Orthognathic Quality of Life Questionnaire (OQLQ) between SFA and CTA were −1.58 (P = 0.05) and −2.99 (P < 0.00001) at the termination of the first-stage treatment, which altered to −0.94 (P = 0.54) and 0.09 (P = 0.65) after total treatment. Two studies applied the Psychosocial Impact of Dental Aesthetics Questionnaire (PIDAQ) and the Beck Depression Inventory (BDI-II) to examine mental health, resulting in a trend similar to the former scales. ConclusionIn contrast to the conventional procedure, orthognathic treatment with SFA can instantly enhance the quality of life at the end of the first-stage treatment but has similar effects after the overall treatment. Moreover, SFA has a positive impact on psychological conditions. Clinical significanceThis study first systematically reviewed the effect of SFA on patients' mental well-being. According to our findings, it is better to select SFA if possible. Otherwise, the patient's psychological condition should be monitored appropriately throughout decompensation for better well-being both physically and mentally.

Comparison between sex and polymorphisms on FKBP5: perception of quality of life in non-syndromic patients. A cross-sectional study

This study aimed to compare factors that influence perception of quality of life (QoL) in patients scheduled for orthognathic surgery. This was a cross-sectional study with 91 participants from two universities in Curitiba. The orthognathic quality of life questionnaire (OQLQ) was used to assess patients’ perceptions of their QoL. Sociodemographic data were collected and facial profiles classified into classes I, II, and III. DNA was extracted from oral mucosal cells and markers rs3800373 and rs1360780 for FKBP prolyl isomerase 5 were genotyped. Statistical analysis was performed using Kruskal-Wallis, Mann-Whitney, and chi-squared tests, with a significance level of 5%. There was a negative impact on general perception of QoL in females (p = 0.019) and in the domains of “oral function” (p=0.032) and “awareness of the deformity” (p=0.009). In the dominant model (CC/CT), the presence of at least one C allele for the rs1360780 marker had a negative impact on QoL in the “facial aesthetics” domain (p = 0.037). The negative impact on QoL was greater in females than in males. The perception of QoL was more negative in individuals with rs1360780 polymorphism on the FKBP5 gene and a CC/CT genotype than it was in those with a TT genotype.

Translation, Transcultural Adaptation, and Validation of the Hebrew Language Version of the Orthognathic Quality of Life Questionnaire for the Assessment of Quality of Life in Orthognathic Patients.

Dentofacial deformities impose a negative impact on quality of life (QOL). Orthognathic surgery is the main pillar of treatment for these conditions and has proven its impact on the improvement of the functional and psychosocial aspects of QOL. The Orthognathic Quality of Life Questionnaire (OQLQ), developed by Cunningham and colleagues, is a well-established instrument for assessing QOL in patients with dentofacial deformities. The aim of this study was to perform a translation, transcultural adaptation, and validation of a Hebrew version of the OQLQ. Transcultural adaptation was done following guidelines proposed by Beaton and colleagues resulting in a pilot study conducted on 20 patients undergoing orthognathic surgery. Internal consistency, reliability, and sensitivity were evaluated by means of Cronbach alpha, intraclass correlation coefficient (test-retest), and Wilcoxon test, respectively. Validity was assessed by comparing the OQLQ with the Hebrew version of the Oral Health Impact Scale-14 (Oral Health Impact Profile Scale-14) using the Spearman correlation test. Internal consistency showed a good correlation between domains and excellent test-retest reliability. Sensitivity to change was statistically significant in all but 3 questions. The Hebrew version of the OQLQ exhibited a strong correlation with Oral Health Impact Profile Scale-14 in total score and moderate to high correlations among domains. The Hebrew version of the OQLQ is a valid and reliable and specific instrument to measure QOL for Hebrew-speaking patients undergoing orthognathic surgery.

Meta-analysis of outcomes related to the quality of life after orthodontic-surgical treatment.

Orthognathic surgery causes functional and aesthetic changes, which could affect patients' quality of life (QOL). The current analysis assessed the impact of orthodontic-surgical treatment on the parameters affecting the QOL using different scoring systems. The criteria for inclusion were studies written in various languages that compared the effects of the intervention on patients' QOL before surgery and at various periods after surgery (3 weeks to several months), which resulted in including 19 studies into this meta-analysis. The outcomes of these studies underwent random-effect modeling to calculate the mean difference (MD) and 95% confidence intervals (95% CIs) of the impact of different surgical techniques on clinical parameters, and publication bias was analyzed with Begg's test. According to the total score of the Orthognathic Quality of Life Questionnaire (OQLQ), surgery significantly affected patients' QOL after 2 months or less (p = 0.049), up to 6 months (p < 0.001), and when comparing 2 months or less with up to 6 months (2-6 months) (p < 0.001). In addition, the total Oral Health Impact Profile-14 (OHIP-14) score showed a significant difference in the QOL after 6 months (p = 0.003) and up to 12 months (p = 0.002) after surgery. Therefore, orthodontic-surgical treatment significantly improves patients' QOL after surgery compared to before surgery.

The impact of orthognathic surgery on quality of life for class III dentofacial deformities

In this study, we aim to evaluate the quality of life in patients with Class III deformities after orthognathic surgery. A total of the 40 patients (26 female and 14 male) were included. The mean age of the patients was 24.85. Patients' ages ranged from 20 to 36 years. All patients received orthodontic treatment before surgery. Sagittal split ramus osteotomy was performed for single jaw patients. Le Fort I osteotomy and sagittal split ramus osteotomy was performed for double jaw patients. Patients completed the Oral Health Impact Profile 14 (OHIP-14) and Orthognathic Quality of Life Questionnaire (OQLQ) three times. [Preoperatively (T0), 1st week after orthognathic surgery (T1) and in the 6th - 12th months after orthognathic surgery (T2)]. There was a statistically significant difference in the dimensions of OHIP-14 when the preoperative (T0) score, postoperative 1st week (T1) score and postoperative 6th - 12th month (T3) score are compared among themselves except for psychological discomfort, physical disability, and handicap. OQLQ total score and preoperative (T0) score was greater than the postoperative 1st week (T1) score and the postoperative 1st week(T1) score was greater than the postoperative 6th - 12th month (T2) scores except oral function. When single jaw and double jaw surgeries were compared, no statistically significant difference was found between OHIP-14 and OQLQ total scores for preoperative, postoperative 1st week, and postoperative 6th - 12th months. When both OHIP-14 and OQLQ scores were examined after orthognathic surgery, the OHRQOL of patients with Class III dentofacial deformity improved significantly.

Evaluating the quality of life among patients undergoing orthognathic surgery using a customized questionnaire: A prospective study

Objectives:A questionnaire, specific to the clinical condition treated is important. The aim of this study was to evaluate the Quality of life (QOL) amongst patients undergoing orthognathic surgery using a customized questionnaire encompassing, patients’ perception of facial form and also patient perception of overall quality of care provided during the entire treatment.Material and Methods:This prospective, qualitative, observational study was carried out on 21 patients, who underwent Ortho-surgical management between Jan 2019 and Mar 2020. The Orthognathic QOL questionnaire was customized, Phase I, immediately prior; and Phase II, 6 months after surgery. Appropriate analysis was done to compare Phase I and Phase II, with the level of significance set to P &lt; 0.05.Results:“Aesthetics” was the most frequent reported reason for seeking treatment. The mean pre-/post-treatment difference for oral function was statistically significant. Majority (87%) of patients reported surgical outcomes met their expectations. Results, post-surgery; good hospital care (33.3%), supportive doctors (28.57%) and regular follow up (9.52%).Conclusion:“Patients’ perspective” of treatment, is often neglected, the immense importance of which has been duly reflected in our study. We recommend the use of our questionnaire in a similar, broad based multicentric study with a larger patient base, will aid in improving overall outcome of ortho-surgical interventions.

Quantifying health utilities of young adult orthodontic patients using the time trade-off method: a cross-sectional study.

To study the feasibility of time trade-off (TTO) method in quantifying health utility ratings in different types of malocclusion. In this cross-sectional study, 70 orthodontic patients aged 18 years or above, reporting for treatment/consultation, were included and interviewed. Malocclusion-related health utilities were assessed through the TTO method, and oral health-related quality of life was measured with the help of Orthognathic Quality of Life Questionnaire (OQLQ). Angle's classification of malocclusion was recorded. Bivariate analyses and multivariate Poisson's regression were done to find out an association between the oral health utility values, OQLQ and demographic and clinical characteristics. Patients with skeletal Class III malocclusion had lower health utility values than those with Class I and Class II malocclusions (p=0.013). Poisson's regression showed that Angle's Class II division 1 (0.90, CI 0.84 to 0.97), Class III (0.68, CI 0.59 to 0.95) and Skeletal malocclusion (0.79, CI 0.71 to 0.87) and OQLQ scores (1.0, CI 1 to 1.003) were found to be significant predictors of TTO utility scores. TTO utilities were found to be valid and well correlated with clinical findings. Health utilities could serve as useful and reliable markers of health-related quality of life (HRQL) among individuals or communities and help cost-effective preventive or intervention programs planning.

Assessment of the impact of orthodontic-surgical treatment on the quality of life of patients with mandibular prognathism

Background/Aim. Mandibular prognathism, as one of the more severe forms of dentofacial deformities, impairs the oral functions and appearance of the patient?s face and represents a psychosocial handicap with a negative impact on the quality of life (QoL). The aim of the study was to assess the impact of orthodontic-surgical (OS) treatment on the QoL of patients with mandibular prognathism. Methods. The study involved 40 patients (19 men and 21 women, with a mean age of 24.1 ? 4.1 years) who underwent the OS treatment for mandibular prognathism. All patients completed two questionnaires ? the Oral Health Impact Profile (OHIP-14) and the Orthognathic Quality of Life Questionnaire (OQLQ), be-fore the start of treatment and 12 months after the completion of treatment. Results. The results of both questionnaires showed an improvement in the QoL compared to the condition before the treatment. According to the OQLQ questionnaire, there was a postoperative improvement in the QoL (score 24.8 ? 12.9) compared to the preoperative period (score 51.3 ? 15.2). According to the results of the OHIP-14 questionnaire, there was a postoperative improvement in the QoL (score 11.3 ? 2.9) compared to the preoperative period (score 20.8 ? 6.9). The improvement of the QoL, 12 months after the treatment, occurred in all life segments, measured by both questionnaires. Conclusion. OS treatment of mandibular prognathism improves all oral functions, including the appearance of the patient?s face, thus improving the QoL.

Translation and validation of the Hungarian version of the Orthognathic Quality of Life Questionnaire

Introduction: Facial appearance undoubtedly has a strong influence on human life. While a harmonic and beautiful facial form may raise self-esteem, dentofacial deformities can lead not only to functional but also to aesthetic and psychosocial problems. Oral health-related quality of life is measured by several non-specific questionnaires but an adequate instrument for testing the quality of life in patients with dentofacial deformities in Hungarian is not yet available. Objective: Cross-cultural translation and validation of the most frequently used Orthognathic Quality of Life Questionnaire (OQLQ) into Hungarian. Method: The original 22-item OQLQ was translated from English to Hungarian (OQLQ-H) by a forward-backwards translation method. The questionnaire was filled out by patients who voluntarily agreed to take part in the study. Validity was evaluated according to the international guidelines. Reliability was evaluated by test-retest analysis. Internal consistency was measured by Cronbach’s alpha. Results: OQLQ-H proved to be a reliable questionnaire with good validity and internal consistency. Conclusion: OQLQ-H is a reliable and valid measurement tool to assess dentofacial deformities in the Hungarian speaking population and therefore its use is recommended.

Influence of personality traits on a patient's decision to accept orthognathic surgery for correction of dentofacial deformity.

To investigate the influence of personality traits in addition to quality of life (QoL) on the decision to accept orthognathic surgery. A total of 108 patients (68% female) aged 14-53 years (median, 18 years; interquartile range, 17-25.75 years), with skeletal malocclusions of Index of Orthognathic Functional Treatment Need grades 3-5 (moderate to very great need for surgery) were included in this cross-sectional study. Personality traits of extraversion, neuroticism, agreeableness, openness, consciousness, perfectionism, and self-esteem and dimensions of Orthognathic Quality of Life Questionnaire were compared between patients who accepted orthodontic preparation for orthognathic surgery and those who refused (n = 55 vs 53). Patients who accepted the suggested surgical procedure had higher age, perfectionism, facial esthetic (FE) concern, social aspect, and impairment of oral function (OF) as well as lower self-esteem with small to medium effect sizes (P ≤ .040; r = 0.198-0.399). Other personality traits and awareness of dentofacial deformity did not differ between the groups. In multiple logistic regression analysis, while controlling for sex, perfectionism, and self-esteem, the following predictors of acceptance of orthognathic surgery were higher: FE concern (odds ratio [OR], 3.4; 95% confidence interval [CI], 1.3-9.1), OF (OR, 3.0; 95% CI, 1.0-8.6), and age ≥18 years (OR, 2.7; 95% CI, 1.0-7.1; P < .001). Extraversion, neuroticism, agreeableness, conscientiousness, and openness do not significantly affect a patient's decision to accept orthognathic surgery. The influence of self-esteem and perfectionism is primarily on perception of alteration of QoL induced by dentofacial deformity.

Validity and reliability of the Turkish version of the orthognathic quality of life questionnaire in patients with dentofacial deformity.

Background Due to the lack of a specific quality of life (QoL) survey on dentofacial deformities (DFD) for Turkish speakers, the present research aimed to perform a translation of the English version of the Orthognathic Quality of Life Questionnaire (OQLQ) into Turkish (the OQLQ-TR) and provide cultural adaptation to the Turkish population.Material and Methods The process of this cross-cultural adaptation followed the six stages given in the guidelines that were proposed by Beaton et al. (2000), which comprised the following: 1) performing the initial translation, 2) synthesizing the translation, 3) performing the back translation, 4) presenting it to the expert committee, and 5) testing the prefinal version. Throughout the process of validating the Turkish version, the results of the OQLQ were compared with the Oral Health Impact Scale-14 (OHIP-14) and Short Form-36 (SF-36) questionnaires and the Visual Analogue Scale (VAS), which were previously validated in Turkish. All of these Turkish questionnaires (OHIP-14, SF-36, OQLQ) were applied to 69 patients at the Çukurova University Faculty of Dentistry.Results Analysis of the internal consistency of the OQLQ-TR exhibited good correlations for the domains. Moreover, the test-retest reliability also exhibited intra-class correlation coefficients that were excellent. The correlation between the OQLQ-TR and SF-36 was weak and negative. The OQLQ-TR exhibited good correlations with the OHIP-14 and VAS.Conclusions The OQLQ-TR was found to be valid, reliable, and reproducible. Thus, it has become a useful instrument for assessing the quality of life of Turkish-speaking patients with dentofacial deformity. Reliability and Validity of Japanese Version of Orthognathic Quality of Life Questionnaire

Quality of life （QOL） is now known to be one of the important prognostic factors in many clinical fields. Disease-specific quality of life is an important index as well as health related quality of life （HRQOL） to validate outcomes of medical treatments. The Orthognathic Quality of Life （OQLQ） index has been developed to measure QOL of dentofacial deformity. The aim of the present study was to develop a Japanese version of the OQLQ and examine its reliability and validity. Materials & methods: With the permission of the OQLQ developer, the original English version of the OQLQ was translated into Japanese with a forward-backward translation method. A pilot study was carried out on a sample of 22 patients, and a validity study was done on a sample of 25 patients. The reliability was verified with Cronbach’s coefficient alpha and item-total correlation. The validity study was performed after confirming the reliability of the questionnaire, using the test-retest method. HRQOL was also measured to verify the validity of the questionnaire with SF-8, and correlation was verified with Spearman’s correlation coefficient. All of the patients who participated in this study were new patients who were diagnosed with dentofacial deformity. Results: In the pilot study, the correlation coefficients of the item-total correlation score were −0.011 to 0.068. Some questions on oral function component showed a weak correlation, but Cronbach’s coefficient alpha of each component was 0.79-0.85, which revealed the reliability of the questionnaire. The paired t-test in the validity study showed no significant difference for most of the questions, confirming repeatability in the same patient and the validity of the questionnaire. Spearman’s correlation coefficient showed, for all four components of the OQLQ, moderate to strong correlation with some items of mental scales and MCS of SF-8. Conclusion: We concluded that the Japanese version of the OQLQ we developed is a reliable and valid questionnaire.

Quality of Life of Patients with Jaw Deformities

Recently, the importance of subjective measures such as patient satisfaction and quality of life has been recognized in clinical practice. Surgical orthodontic treatment for patients with jaw deformities not only improves maxillofacial morphology and occlusion, but also has a positive impact on oral and maxillofacial function, psychological and social aspects, and improves the quality of life of the patients. The Orthognathic Quality of Life Questionnaire, which was developed as a jaw deformity disease-specific quality-of-life scale, is also a useful method for assessing the quality of life in patients with jaw deformities. On the other hand, patient satisfaction is an important quality-of-life measure, but not all patients are satisfied with the results of surgical orthodontic treatment. Therefore, in order to increase patient satisfaction, it is essential to carefully analyze the patient’s complaints during the preoperative medical interview based on a correct diagnosis of jaw deformities, to fully discuss the surgical technique and the direction and amount of jaw movement with the patient, and to obtain sufficient informed consent for any possible complications.

Changes in health-related quality of life after orthognathic surgery: a multicenter study.

ObjectiveTo evaluate changes in general and oral health-related quality of life (HRQoL) in patients with dentofacial deformity undergoing orthognathic surgery, and whether these changes vary according to type of deformity.Material and methodsThis is a prospective longitudinal multicenter study of patients with dentofacial deformities (n = 90). The Orthognathic Quality of Life Questionnaire (OQLQ), Oral Health Impact Profile (OHIP-14), and Short-Form Health Survey version2 (SF-36v2) were self-completed by patients before surgery, 3 and 6 months after orthognathic surgery. Change was tested using paired t-test, and compared between Class II and Class III of dentofacial deformity by unpaired t-test. The magnitude of change was examined estimating the standardized response mean (SRM).ResultsThe OQLQ and OHIP-14 showed statistically significant improvements 6 months after surgery, compared with the pre-surgical evaluation, but the SF-36v2 only in the physical component summary. The SRM was large in OQLQ oral function (-1.11) and dentofacial facial aesthetics (-0.76) dimensions, and moderate in most of OHIP-14 dimensions. Differences in mean change between Class II and III were statistically significant for global scores of OQLQ (-10.08 vs -20.30, p = 0.0271) and OHIP-14 (-3.79 vs -10.56, p = 0.0144).ConclusionsA significant improvement was observed in oral HRQoL and in the physical component of general health in patients with dentofacial deformities Class II and III after orthognathic surgery. Improvement was greater among Class III than in Class II patients.Clinical relevanceThese results provide patients, oral health care professionals, and planners with valuable information to make evidence-based decisions and facilitate shared clinical decision-making, taking into account the patients’ perspective.
