Improvement of double photon emission Compton imaging using angular and polarization correlations in cascade photons

A Compton camera is a gamma ray imaging system that produces images by superposing Compton cones calculated from Compton kinematics. However, the reconstructed images exhibit a high background. In this study, the angular correlation and polarization correlation were used to decrease the background. 111In, which emits 171 keV and 245 keV cascade gamma rays, was imaged using Compton camera modules in the ring structure. The 3D list-mode ordered subset expectation maximization method was used to reconstruct the images. Probability modeling of events reflecting angular and polarization correlations was applied to the image-reconstruction algorithm. The results were compared with those of a typical Compton camera method using only 245 keV Compton cones. The sensitivity of the proposed method was 1.03% of that of the typical method. However, compared to the typical method, the application of angular and polarization correlations improved the signal-to-background ratio. The proposed method is expected to be useful in capturing low-background images wherein a ring-type Compton camera and cascade photon-emitting nuclides can be used.

Comparison of the detectability of hot lesions on bone SPECT using six state-of-the-art SPECT/CT systems: a multicenter phantom study to optimize reconstruction parameters.

Single-photon emission computed tomography with X-ray computed tomography (SPECT/CT) systems have diversified due to the remarkable developments made by each manufacturer. This study aimed to optimize the reconstruction parameters of six state-of-the-art SPECT/CT systems and compare their image quality of bone SPECT.SPECT images were acquired on SPECT/CT systems, including Symbia Intevo, Discovery NM/CT 670, Discovery NM/CT 870 CZT, Brightview XCT, and VERITON-CT. SIM2 bone phantom with tough lung phantoms on both sides of the spinal inserts that simulate the thorax was used for image quality assessment. SPECT images were obtained at individual workstations using an ordered subset expectation maximization method with three-dimensional resolution recovery, as well as CT attenuation and scatter correction, subset 2, iteration 12-84, and a full width at half maximum 10-mm Gaussian smooth filter. An automatic image analysis software dedicated to SIM2 bone phantom was used to assess the contrast-to-noise ratio (CNR), relative recovery coefficient, percentage of coefficient of variance, contrast, and detectability. The optimal parameters for each system were defined with superior detectability of spherical lesions and noise characteristics, as well as the highest CNR.All systems exhibited better image quality indexes using the optimal parameters than using the manufacturer's recommended parameters. The detectability of all systems was in agreement while using the optimal parameters.Detectability agreement can be achieved by optimizing the reconstruction parameters for different reconstruction algorithms, which can further improve the image quality. Therefore, future research should focus on optimal reconstruction parameters for SPECT alone.

Bayesian penalized likelihood PET reconstruction impact on quantitative metrics in diffuse large B-cell lymphoma.

Evaluate the quantitative, subjective (Deauville score [DS]) and reader agreement differences between standard ordered subset expectation maximization (OSEM) and Bayesian penalized likelihood (BPL) positron emission tomography (PET) reconstruction methods. A retrospective review of 104 F-18 fluorodeoxyglucose PET/computed tomography (CT) exams among 52 patients with diffuse large B-cell lymphoma. An unblinded radiologist moderator reviewed both BPL and OSEM PET/CT exams. Four blinded radiologists then reviewed the annotated cases to provide a visual DS for each annotated lesion. Significant (P < .001) differences in BPL and OSEM PET methods were identified with greater standard uptake value (SUV) maximum and SUV mean for BPL. The DS was altered in 25% of cases when BPL and OSEM were reviewed by the same radiologist. Interobserver DS agreement was higher for OSEM (>1 cm lesion = 0.89 and ≤1 cm lesion = 0.84) compared to BPL (>1 cm lesion = 0.85 and ≤1 cm lesion = 0.81). Among the 4 readers, average intraobserver visual DS agreement between OSEM and BPL was 0.67 for lesions >1cm and 0.4 for lesions ≤1 cm. F-18 Fluorodeoxyglucose PET/CT of diffuse large B-cell lymphoma reconstructed with BPL has higher SUV values, altered DSs and reader agreement when compared to OSEM. This report finds volumetric PET measurements such as metabolic tumor volume to be similar between BPL and OSEM PET reconstructions. Efforts such as adoption of European Association Research Ltd accreditation should be made to harmonize PET data with an aim at balancing the need for harmonization and sensitivity for lesion detection.

Retrospective fractional dose reduction in Tc-99m cardiac perfusion SPECT/CT patients: A human and model observer study

BackgroundIn the ongoing efforts to reduce cardiac perfusion dose (injected radioactivity) for conventional SPECT/CT systems, we performed a human observer study to confirm our clinical model observer findings that iterative reconstruction employing OSEM (ordered-subset expectation-maximization) at 25% of the full dose (quarter-dose) has a similar performance for detection of hybrid cardiac perfusion defects as FBP at full dose. MethodsOne hundred and sixty-six patients, who underwent routine rest-stress Tc-99m sestamibi cardiac perfusion SPECT/CT imaging and clinically read as normally perfused, were included in the study. Ground truth was established by the normal read and the insertion of hybrid defects. In addition to the reconstruction of the 25% of full-dose data using OSEM with attenuation (AC), scatter (SC), and spatial resolution correction (RC), FBP and OSEM (with AC, SC, and RC) both at full dose (100%) were done. Both human observer and clinical model observer confidence scores were obtained to generate receiver operating characteristics (ROC) curves in a task-based image quality assessment. ResultsAverage human observer AUC (area under the ROC curve) values of 0.725, 0.876, and 0.890 were obtained for FBP at full dose, OSEM at 25% of full dose, and OSEM at full dose, respectively. Both OSEM strategies were significantly better than FBP with P values of 0.003 and 0.01 respectively, while no significant difference was recorded between OSEM methods (P = 0.48). The clinical model observer results were 0.791, 0.822, and 0.879, respectively, for the same patient cases and processing strategies used in the human observer study. ConclusionsCardiac perfusion SPECT/CT using OSEM reconstruction at 25% of full dose has AUCs larger than FBP and closer to those of full-dose OSEM when read by human observers, potentially replacing the higher dose studies during clinical reading.

Performance of SwiftScan planar and SPECT technology using low-energy high-resolution and sensitivity collimator compared with Siemens SPECT system.

A new low-energy high-resolution-sensitivity (LEHRS) collimator was developed by General Electric (GE) Healthcare. SwiftScan planar and single photon emission computed tomography (SPECT) systems using LEHRS collimator were developed to achieve the low-dose and/or short-time acquisition. We demonstrated the performance of SwiftScan planar and SPECT system with LEHRS collimator using phantoms. Line source, cylindrical and flat plastic dish phantoms were used to evaluate the performance of planar and SPECT images for four patterns of Siemens LEHR, GE LEHR, GE LEHRS and SwiftScan using two SPECT-CT scanners. Each phantom was filled with 99mTc solution, and the spatial resolution, sensitivity and image uniformity were calculated from the planar and SPECT data. The full-width at half maximum (FWHM) values as a system spatial resolution of Siemens LEHR, GE LEHR and GE LEHRS were approximately 7.4 mm. GE LEHRS showed a lower FWHM value by increasing the blend ratio in Clarity2D processing. The system sensitivity of GE LEHRS increased by approximately 30% compared with that of GE LEHR and was similar to that of Siemens LEHR. The FWHM values of SPECT with an filtered back projection (FBP) method were approximately 10.3 mm. The FWHM values of the ordered subset expectation maximization (OSEM) method were better with an increase in iteration values. The differential uniformities of Siemens LEHR, GE LEHR, GE LEHRS and GE SwiftScan using the FBP method were approximately 15.1%. The differential uniformity of OSEM method was higher with an increase in the iteration value. The SwiftScan planar and SPECT have a high sensitivity while maintaining the spatial resolution compared with the conventional system.

Passive gamma emission tomography with ordered subset expectation maximization method

Gamma-ray emission tomography (GET) was developed as a potential verification tool to visualize the passive gamma-ray emitter sources of fuel rods. In GET, maximum likelihood–expectation maximization (MLEM) was employed as an iterative reconstruction method. However, as convergence iteration in the algorithm is proportional to the pixel size, convergence is slow and the calculation cost for practical application is high. Therefore, an ordered subset expectation maximization method (OSEM) was used, and the rod-wise relative gamma-ray emitter distribution of a BWR 10 × 10 mock-up fuel assembly was reconstructed to evaluate the performance of the OSEM. The OSEM enabled reconstruction comparable to that of MLEM with an effective decrease in the number of iterations.

Assessment of SPECT images using UHRFB and other low-energy collimators in brain study by Hoffman phantom and manufactured defects

The performance of collimators was investigated using low-energy high-resolution (LEHR) and low-energy ultra-high-resolution (LEUHR) collimators in SPECT system to study the brain physiology by injecting 99mTc-HMPAO radionuclide. Ultra-high-resolution fan beam (UHRFB) images were compared to other low-energy collimator images by constructed defects embedded into Hoffman brain phantom as lateral, deepness and metastasis designs with slices of Perspex. The defects were constructed by CNC machine as circular slices and ring flaws ranging in size from 2 to 16 mm to detect low-concentration regions. The data acquisition was done in tomographic mode with minimum radii and different matrix sizes, data acquisition time and number of projections. The collected data were reconstructed using two methods, filtered backprojection and ordered subset expectation maximization (OSEM). By two radiologists and via thallium and V-shaped markers, the minimum diameters of the constructed flaws inside the abnormal area of the brain phantom were diagnosed to 4.0 ± 0.4, 6.0 ± 0.7 and 8.0 ± 0.9 mm for UHRFB, LEUHR and LEHR, respectively. In OSEM method, the desirable ranges of the filter parameters were 4–8 iterations and 2–4 subsets for FB collimator. The outcomes showed that FB collimator provides the better resolution in axial, coronal and sagittal views using brain perfusion phantom and data from eight patients on tinnitus disease. The UHRFB with converging geometry confirmed the better detection of defect regions due to the lowest ratio of cold/background counts. This study may be useful for early detection of disease transference, monitoring of treatment response and disease progression. A CNC machine was used to make disks for Hoffman brain phantom as metastases and ring defects in evaluation of SPECT images.

Imaging technology for myocardial perfusion single-photon emission computed tomography 2018 in Japan.

Recently, nuclear cardiology has dramatically advanced by a new technology development such as the device, short-term acquisition system, image reconstruction algorithm andimage analysis. Although these innovations have been gradually employed in routine examinations, we did not investigate the current use of image acquisition, image reconstruction, and image analysis with myocardial perfusion single-photon emission computed tomography (MPS). We investigated the current status of MPS imaging technology in Japan. We carried out a survey using a Web-based questionnaire system, the opening of which was announced via e-mail, and it was available on a website for 3 months. We collected data on the current use of MPS with 201Tl and/or 99mTc agents with respect to routine protocols, image acquisition, image reconstruction, and image analysis. We received responses to the Web-based questionnaire from 178 and 174 people for 99mTc and 201Tl MPS, respectively. The routine protocols of MPS of stress-rest and rest-stress MPS on 1-day protocols with 99mTc were 41.2% and 14.5%, respectively, and the rest-only scan response rate was 23.7%, whereas that of 201Tl MPS was 65.9% with stress-rest MPS, 19.0% with rest-only MPS, and 10.9% with stress-rest MPS adding a rest scan 24h after injection. The filtered back projection (FBP) method is most commonly used image reconstruction method, yielding 70.5% for 99mTc MPS and 76.8% for 201Tl MPS, including combined FBP and ordered subset expectation maximization method. The results for no-correction (NC) images were 49.2% with 99mTc MPS and 55.2% with 201Tl MPS including the response of NC and combined attenuation correction (AC) and scatter correction (SC) (i.e., ACSC) images. The AC or ACSC images of 99mTc and 201Tl were provided by 30-40% of the institutions surveyed. We investigated the current status of MPS imaging technology in Japan, and found that although the use of various technical developments has been reported, some of these technologies have not been utilized effectively. Hence, we expect that nuclear medicine technology will be used more effectively to improve diagnosis.

OA165] Optimization of quantitative 99MTC-MAA SPECT/CT imaging for 90Y RADIOEMBOLIZATION: A 3D-printed phantom study

Purpose Radioembolization (RE) with 90Y-microspheres has gained widespread acceptance as a safe and effective technique for treating liver malignancies. Accurate quantification in RE is a key step in treatment planning optimization and it is becoming a pressing issue in the light of the Directive 2013/59/EURATOM, stating that individual dose planning must be enforced in legislation by EU member states by February 2018. The aim of this study was to develop a SPECT/CT imaging protocol for quantitative imaging optimization in RE based on a novel anthropomorphic 3D-printed phantom. Methods and materials A SPECT/CT system provided with a novel software for imaging quantification was used to assess the quantitative accuracy of 99mTc-MAA SPECT imaging for treatment planning in RE. Gamma camera calibration factors and recovery coefficients were determined performing preliminary SPECT/CT acquisitions of a homogeneous and a NEMA/IEC phantom, respectively. The quantification procedure was then validated using a 3D-printed anthropomorphic phantom provided with a fillable liver section and multiple inserts. Raw data reconstruction was performed using two independent software: i) the built-in xSPECT package, using both the Ordered Subset Expectation–Maximization (OSEM) and the Ordered Subset Conjugated Gradient (OSCG) algorithm; ii) the commercially available QSPECT software based on OSEM method. All reconstructed images were manually fused and appropriate regions of interest (ROIs) were drawn on the MIM 6.1.7 system. Results The measured activity concentration is consistent within 3% with the expected activity concentration values. This is in keeping with the overall accuracy ( 10%) stated by the manufacturer. No substantial differences were reported between xSPECT with OSCG or OSEM in terms of relative activity. However, a 20% difference was found between ROIs extracted from images reconstructed with xSPECT and QSPECT using OSEM. Several artifacts were visible in the QSPECT reconstruction, likely due to the limited area used for attenuation and scatter corrections within the software. Conclusions The investigated protocol based on the innovative xSPECT tool allows an accurate absolute quantification within 3%. QSPECT software provides the poorest results with several artifacts as well as intrinsic dishomogeneities.

Positron Range Visualization Using the Analytical and Iterative Image Reconstruction Methods From a Preclinical PET Scanner With LYSO Scintillators Coupled to SiPM Detectors: Comparison With Simulated Results

An evaluation of the image quality of the preclinical positron emission tomography scanner consisting of the SensL's SMT C-series silicon photomultiplier (SiPM) arrays coupled with double jaw layer lutetium-yttrium oxyorthosilicate (LYSO) crystals was achieved. The simulated data of a Jaszczak phantom filled with Na-22 and O-15 were compared with reconstructed data of the same phantom when it was filled with Na-22. Two different reconstruction methods were applied in order to visualize any positron range effects that occur within the phantom. The filtered back-projection (FBP) and ordered-subset expectation maximization (OSEM) algorithms provided useful information regarding the spatial resolution of the scanner, showing that these sensors can visually correct the structure of the phantom. The OSEM method appeared to be more reliable than FBP, even when a Compton correction was applied on the data.

Comparison of TOF-PET and Bremsstrahlung SPECT Images of Yttrium-90: A Monte Carlo Simulation Study.

Objective(s):Yttrium-90 (90Y) is a beta particle nuclide used in targeted radionuclide therapy which is available to both single-photon emission computed tomography (SPECT) and time-of-flight (TOF) positron emission tomography (PET) imaging. The purpose of this study was to assess the image quality of PET and Bremsstrahlung SPECT by simulating PET and SPECT images of 90Y using Monte Carlo simulation codes under the same conditions and to compare them.Methods:In-house Monte Carlo codes, MCEP-PET and MCEP-SPECT, were employed to simulate images. The phantom was a torso-shaped phantom containing six hot spheres of various sizes. The background concentrations of 90Y were set to 50, 100, 150, and 200 kBq/mL, and the concentrations of the hot spheres were 10, 20, and 40 times of those of the background concentrations. The acquisition time was set to 30 min, and the simulated sinogram data were reconstructed using the ordered subset expectation maximization method. The contrast recovery coefficient (CRC) and contrast-to-noise ratio (CNR) were employed to evaluate the image qualities.Results:The CRC values of SPECT images were less than 40%, while those of PET images were more than 40% when the hot sphere was larger than 20 mm in diameter. The CNR values of PET images of hot spheres of diameter smaller than 20 mm were larger than those of SPECT images. The CNR values mostly exceeded 4, which is a criterion to evaluate the discernibility of hot areas. In the case of SPECT, hot spheres of diameter smaller than 20 mm were not discernable. On the contrary, the CNR values of PET images decreased to the level of SPECT, in the case of low concentration.Conclusion:In almost all the cases examined in this investigation, the quantitative indexes of TOF-PET 90Y images were better than those of Bremsstrahlung SPECT images. However, the superiority of PET image became critical in the case of low activity concentrations.

Does applying resolution recovery to normal databases confer an advantage over conventional 3D-stereotactic surface projection techniques?

We evaluated a novel normal database (NDB) generated using single photon emission computed tomography (SPECT) data obtained from healthy brains by using a SPECT/CT system, analyzed using a resolution recovery (RR) technique applied to the three-dimensional stereotactic surface projection (3D-SSP) technique. We used a three-dimensional ordered subset expectation maximization method (3D-OSEM) with applied scatter correction (SC), attenuation correction, and RR to reconstruct the data. We verified the accuracy of the novel NDB's values (Z, extent, and error scores), and compared the novel NDB to the 3D-SSP technique by using simulated misery perfusion-related patient data from a conventional NDB. In addition, Z, extent, and error scores at the precuneus, cuneus, and posterior cingulate were compared under different reconstruction conditions by using the patient data. In the simulation, Z scores decreased when using the novel NDB corrected using computed tomography-based attenuation correction (CTAC), SC, and RR. The extent scores of the posterior cingulate increased using the novel NDB, relative to the other NDBs. The error score with the novel NDB without RR decreased by 15% compared to that of the conventional NDB. Z scores generated from patient data decreased in the novel NDB with RR. The extent scores tended to decrease in the novel NDB with RR. The extent scores in the novel NDB with RR improved at the posterior cingulate, compared to the scores with the other NDBs. However, applying RR to the novel NDB conferred no advantage because the cut-off of the current Z score must be reconsidered when using the additive RR technique.

Optimization of the filter parameters in 99mTc myocardial perfusion SPECT studies: the formulation of flowchart

Myocardial perfusion single photon emission computed tomography (SPECT) is typically subject to a variation in image quality due to the use of different acquisition protocols, image reconstruction parameters and image display settings by each institution. One of the principal image reconstruction parameters is the Butterworth filter cut-off frequency, a parameter strongly affecting the quality of myocardial images. The objective of this study was to formulate a flowchart for the determination of the optimal parameters of the Butterworth filter for filtered back projection (FBP), ordered subset expectation maximization (OSEM) and collimator-detector response compensation OSEM (CDR-OSEM) methods using the evaluation system of the myocardial image based on technical grounds phantom. SPECT studies were acquired for seven simulated defects where the average counts of the normal myocardial components of 45° left anterior oblique projections were approximately 10-120counts/pixel. These SPECT images were then reconstructed by FBP, OSEM and CDR-OSEM methods. Visual and quantitative assessment of short axis images were performed for the defect and normal parts. Finally, we formulated a flowchart indicating the optimal image processing procedure for SPECT images. Correlation between normal myocardial counts and the optimal cut-off frequency could be represented as a regression expression, which had high or medium coefficient of determination. We formulated the flowchart in order to optimize the image reconstruction parameters based on a comprehensive assessment, which enabled us to perform objectively processing. Furthermore, the usefulness of image reconstruction using the flowchart was demonstrated by a clinical case.

Tomographic image of prompt gamma ray from boron neutron capture therapy: A Monte Carlo simulation study

Purpose of paper is to confirm the feasibility of acquisition of three dimensional single photon emission computed tomography image from boron neutron capture therapy using Monte Carlo simulation. Prompt gamma ray (478 keV) was used to reconstruct image with ordered subsets expectation maximization method. From analysis of receiver operating characteristic curve, area under curve values of three boron regions were 0.738, 0.623, and 0.817. The differences between length of centers of two boron regions and distance of maximum count points were 0.3 cm, 1.6 cm, and 1.4 cm.

Conceptual Design and Simulation Study of an ROI-Focused Panel-PET Scanner

Positron emission tomography (PET) is an important imaging modality for clincial use. Conventionally, the PET scanner is generally built to provide a roomy enough transverse field-of-view (FOV) for imaging most adults’ torsos. However, in many cases, the region-of-interest (ROI) for imaging is usually a small area inside the human body. Therefore, to fulfill a PET system which provides an FOV comparable in size to the target ROI seems appealing and more cost effective. Meanwhile, such a PET system has the potential for portable or bedside application with the reduced system size. In this work, we have investigated the feasibility of using dual-headed panel-detectors to build an ROI-focused PET scanner. A novel windowed list-mode ordered subset expectation maximization method was developed to perform the ROI image reconstruction. With this method, the ROI of the object can be reconstructed from the coincidences whose position determined by time-of-flight (TOF) measurements was inside the ROI. Monte Carlo simulation demonstrates the feasibility of detecting lesions not less than 1 cm in diameter, with a 300 ps full width at half maximum timing resolution. As a critical system performance, the impact of TOF information on image quality has been studied and the required TOF capability was assessed. With enhanced timing resolution, the distortions and artifacts were reduced effectively. The further improved TOF capability also shows a noticeable improvement of detection performance for low uptake lesions, as well as the recovery speed of lesion contrast, which is of practical significance in the lesion detection task.

A study on the evaluation of usefulness depending on the image reconstruction method in bone SPECT scan

The recent advances in image-processing techniques have led to the development of many methods to reduce the scan time without degrading the image quality. In particular, tomography has improved image reconstruction methods with the concomitant improvement of high-quality images. In this study, PRECEDENCE 16 was used to reconstruct images using the filtered back projection method, which is generally used, and the Astonish method and three-dimensional ordered-subsets expectation maximization method, which are based on repetition techniques. In qualitative and quantitative analysis of the reconstructed images, a comparison was made between images with different acquisition times and between images with the same acquisition time, which aimed at determining the optimal method for reconstructing high-quality images. A blind test for qualitative analysis confirmed almost no difference in image quality depending on the image acquisition time. Furthermore, in quantitative analysis, there was no significant difference in image quality depending on the image acquisition time. On the other hand, the results of the analysis in the image reconstruction method with the same acquisition time demonstrated a significant difference. The images reconstructed by the Astonish method, which uses a repetition technique, are believed to be excellent because they have high resolution and provide clinical diagnostic information. This study confirmed that the reconstruction method with a repetition technique could be used to improve image quality and reduce the scan time, despite not being in general use until recently due to the lengthy time needed for image reconstruction and lack of storage space.

90Y PET-based dosimetry after selective internal radiotherapy treatments

The decay of 90Y has a minor branch to the O+ first excited state of 89Zr, the de-excitation of which to the fundamental state is followed by a β+–β- emission that has been used recently for biodistribution assessment after selective internal radiotherapy (SIRT) treatments. The purpose of the present study is to demonstrate the feasibility of 90Y PET imaging for dose assessment after radioembolization with 90Y microspheres. Activity quantification was validated through preliminary phantom studies using a cylindrical body phantom composed of six inserts of different volumes filled with a calibrated amount of 90Y microspheres. A GE Discovery ST PET/CT scanner provided with bismuth germinate (BGO) crystals was used for image acquisition. Images were reconstructed with an ordered subset expectation–maximization method. The effect of object size and the effect of the number of iterations on dose evaluation and volume recovery were investigated. Microsphere dose distribution was then evaluated on one patient (one lesion) who underwent liver SIRT treatment. Dose calculations were made with a MATLAB-based code developed in our department. Dedicated Monte Carlo calculations were executed to evaluate dose S-values for the 90Y source. The activity distribution derived from 90Y PET acquisitions was convolved with the voxel S-values to obtain a three-dimensional absorbed dose distribution and dose–volume histograms. Dosimetry studies carried out on the body phantom with ordered subset expectation–maximization algorithm, three iterations, provided an accuracy of 7.62% in determining the absorbed dose in the largest insert. The dose difference increases as the insert size reduces. Preliminary results on a patient provided a high-resolution absorbed dose distribution map. An average dose of 139.3 Gy was evaluated for the tumor area, with a maximum dose as high as 237.9 Gy. The absorbed dose to the healthy liver was below the tolerance dose of 35 Gy (33.8 Gy). A clear correlation between absorbed dose and tumor response was observed at 18F-fluorodeoxyglucose PET acquired 6 months after treatment. According to our experience, 90Y PET is a promising and reliable technique for microsphere dose assessment and might pave the way for a patient-specific PET-based dosimetry after liver SIRT treatments.

A Novel Phantom for Evaluating Contrast Resolution in Single Photon Emission Computed Tomography (SPECT)

We have designed a phantom to evaluate acquisition and reconstruction parameters using contrast transfer function (CTF). The goal of this study was to evaluate the accuracy of the phantom for contrast resolution. The phantom consisted of spaced (0-14 mm, 1 mm intervals) pairs of cubic containers (5 mm wide, 20 mm long, and 50 mm high). The phantom's accuracy was examined by comparing a real value of a measured count profile using the phantom with a theoretical value obtained by the line spread function (LSF) using a line source. A SPECT acquisition was 256×256 matrix (pixel size: 0.9×0.9 mm(2)), 360 degrees at 30 s/view. The radius of rotation was set to 15 cm, and the types of collimators were low energy high resolution (LEHR) and low middle energy general purpose (LMEGP). Reconstructions were performed with filtered backprojection and ordered subsets expectation maximization method (10 iterations, 10 subsets) with collimator-detector response correction. The actual measured count profile and CTF accorded closely with the theoretical one. The line pair (LP) phantom, obtained with smaller pixels, was really accurate. The size of cubic containers poses as a minimal problem for accurately evaluating the contrast resolution and plotted counts profile. This phantom could be a useful method for evaluating acquisition and reconstruction parameters in SPECT.

Incorporating Quantitative Single Photon Emission Computed Tomography into Radiation Therapy Treatment Planning for Lung Cancer: Impact of Attenuation and Scatter Correction on the Single Photon Emission Computed Tomography–Weighted Mean Dose and Functional Lung Segmentation

To assess the impact of attenuation and scatter corrections on the calculation of single photon emission computed tomography (SPECT)-weighted mean dose (SWMD) and functional volume segmentation as applied to radiation therapy treatment planning for lung cancer. Nine patients with lung cancer underwent a SPECT lung perfusion scan. For each scan, four image sets were reconstructed using the ordered subsets expectation maximization method with attenuation and scatter corrections ranging from none to a most comprehensive combination of attenuation corrections and direct scatter modeling. Functional volumes were segmented in each reconstructed image using 10%, 20%, …, 90% of maximum SPECT intensity as a threshold. Systematic effects of SPECT reconstruction methods on treatment planning using functional volume were studied by calculating size and spatial agreements of functional volumes, and V(20) for functional volume from actual treatment plans. The SWMD was calculated for radiation beams with a variety of possible gantry angles and field sizes. Functional volume segmentation is sensitive to the particular method of SPECT reconstruction used. Large variations in functional volumes, as high as >50%, were observed in SPECT images reconstructed with different attenuation/scatter corrections. However, SWMD was less sensitive to the type of scatter corrections. SWMD was consistent within 2% for all reconstructions as long as computed tomography-based attenuation correction was used. When using perfusion SPECT images during treatment planning optimization/evaluation, the SWMD may be the preferred figure of merit, as it is less affected by reconstruction technique, compared with threshold-based functional volume segmentation.

90Y bremsstrahlung emission computed tomography using gamma cameras

This study demonstrates images obtained by (90)Y bremsstrahlung emission computed tomography (BECT), and characterizes the system performance of gamma cameras. (90)Y BECT images of phantoms were acquired using a gamma camera equipped with a medium energy general purpose parallel-hole collimator. Three energy window widths of 50% (57-94 keV) centered at 75 keV, 30% (102-138 keV) at 120 keV, and 50% (139-232 keV) at 185 keV were set on a (90)Y bremsstrahlung spectrum. The images obtained with three energy windows were reconstructed using filtered back projection (FBP) and ordered subsets expectation maximization (OSEM) methods. The images of the sum window were obtained by fusing the images of the 75, 120, and 185 keV windows. The OSEM method improved the full width at half maximum by 20% and the standard deviation by 9% compared with the FBP method. BECT displayed (90)Y biodistribution and quantified (90)Y activity. BECT images obtained with OSEM method using the 120 keV window showed the highest resolution and lowest uncertainty. The sum window showed the highest sensitivity, while its resolution was 10% inferior to that of the 120 keV window. One whole-body image can be taken over 100 min using the sum window. An absorber to cover the body surface reduced background by 30%. (90)Y BECT imaging can be used for patient assessment without modifying current treatment procedures.
