Implementation of Nasal CPAP Weaning Guidelines in Preterm Infants.

Continuous positive airway pressure (CPAP) benefits preterm infants with respiratory distress, including reduced bronchopulmonary dysplasia (BPD) incidence, surfactant use, and extubation failure. Successful CPAP weaning also promotes oral feeding. However, there is no consensus on the optimal weaning of CPAP in neonates. This study aims to determine the effects of CPAP (CPAP) weaning guideline implementation on neonatal outcomes. CPAP gradual pressure weaning guidelines were implemented in the Penn State Children's Hospital NICU in 2020. We included baseline data from infants (Epoch1) before bubble CPAP implementation in 2018-19. We included infants (Epoch2) after implementing the guidelines during 2020-21. The inclusion criteria were infants <32 weeks gestation with CPAP support. Compliance with the CPAP weaning guidelines was the primary process measure. Primary outcome measures included successful CPAP wean on the first attempt. Balancing measures used were total days on respiratory support and length of hospital stay. 195 infants were included in this study, 95 infants in Epoch 1 before bubble CPAP implementation and 100 infants in Epoch 2 after implementing guidelines. Infants in the two Epochs were similar in median gestational age at 29 vs 30 weeks (p=0.47) and were similar in median birth weight at 1190 vs 1130 grams (p=0.73). After implementing weaning guidelines, the successful weaning off CPAP improved from 9.5% to 54% (p<0.001). The total days needed to achieve full oral feeds decreased by 7 days (29 vs 22 median days, p<0.001). The BPD incidence was not significantly different between the two Epochs at 17% vs 16%, p= 0.87. There was no difference in total days of respiratory support, total length of stay, the number of infants discharged on home nasogastric feeding, and demographic variables. The implementation of the bubble CPAP weaning guideline improves the successful weaning of CPAP and promotes oral feeding in preterm infants.

Development of the assessment scale for preterm oral feeding readiness (ASPOFR)

ObjectiveThe study was planned to develop measurement tool for assessing the readiness of preterm infants to transition to oral feeding. MethodsA literature review and a panel of experts were used to develop the questionnaire to analyze the Assessment Scale for Preterm Oral Feeding Readiness (ASPOFR). The recent version of the questionnaire was composed of 16 items and applied to a sample of 153 infants. ResultThe content validity ratio of the scale items ranged from 0.85 to 1.00 and the content validity index was 0.98. The Cronbach's alpha for the ASPOFR scores was 0.938. ROC was 95.5%, and an optimal cut-off value of ASPOFR was 20. ConclusionASPOFR has been verified to be an effective and accurate instrument to determine the initiation of oral feeding in preterm infants.

Introduction of oral feeding in premature infants on high flow nasal cannula in a level IV neonatal intensive care unit: a quality improvement initiative.

Failure to reach full oral feeding remains a significant barrier for premature infants to discharge home. Postmenstrual age (PMA) at first oral feeding is significantly associated with the length of hospital stay (LOS). Single-center QI to introduce oral feeding to infants on high-flow nasal cannula (HFNC) by reducing the flow to 2 L during feeds. To reduce PMA at first oral feeding and reduce the LOS. To introduce oral feeds in 40% of infants who are on ≤4 L HFNC by the end of 12 months. Over 12 months, SMART aim reached with 100% enrollment. PMA at first oral feeding decreased from a median of 42.4w ((IQR) (40,46.6) to 37.8w (35.8,43.2), PMA at discharge decreased from 47w (44.6,50.7) to 42.6w (41.3,48.8). Allowing oral feeding in infants while on HFNC is feasible. This approach can significantly reduce PMA at first and full oral feeding.

Effects of human milk odor stimulation on feeding in premature infants: a systematic review and meta-analysis

Previous studies suggested odor stimulation may influence feeding of premature neonates. Therefore, this systematic review and meta-analysis of randomized controlled trials was conducted to assess the effect of human milk odor stimulation on feeding of premature infants. All randomized controlled trials related to human milk odor stimulation on feeding in premature infants published in PubMed, Cochrane, Library, Medline, Embase, Web of science databases and Chinese biomedical literature databases, China National Knowledge Infrastructure, China Science and Technology Journal Database (VIP) and Wanfang Chinese databases were searched, and The Cochrane Handbook 5.1.0 was used to evaluate the quality and authenticity of the literature. Relevant information of the included studies was extracted and summarized, and the evaluation indexes were analyzed using ReviewManager5.3. The retrieval time was from the establishment of the database to July 28, 2022.12 articles were assessed for eligibility, and six randomized controlled studies were eventually included in the meta-analysis (PRISMA). A total of 6 randomized controlled studies with 763 patients were finally included in the study, and the quality evaluation of literatures were all grade B. Human milk odor stimulation reduced the transition time to oral feeding in premature infants [SMD = − 0.48, 95% CI (− 0.69, − 0.27), Z = 4.54, P < 0.00001] and shortened the duration of parenteral nutrition [MD = − 1.01, 95% CI (− 1.70, − 0.32), Z = 2.88, P = 0.004]. However, it did not change the length of hospitalization for premature infants [MD = − 0.03, 95% CI (− 0.41, 0.35), Z = 0.17, P = 0.86]. The implementation of human milk odor stimulation can reduce the transition time to oral feeding and the duration of parenteral nutrition in premature infants, but further studies are needed to determine whether it can reduce the length of hospital stay in premature infants. More high-quality, large-sample studies are needed to investigate the effect of human milk odor stimulation on the feeding process and other outcomes in premature infants.

Effects of implementing non-nutritive sucking on oral feeding progression and outcomes in preterm infants: A systematic review and meta-analysis.

Preterm infants have imperfect neurological development, uncoordinated sucking-swallowing-breathing, which makes it difficult to realize effective oral feeding after birth. How to help preterm infants achieve complete oral feeding as soon as possible has become an important issue in the management of preterm infants. Non-nutritive sucking (NNS), as a useful oral stimulation, can improve the effect of oral feeding in preterm infants. This review aimed to explore the effect of NNS on oral feeding progression through a meta-analysis. We systematically searched PubMed, CINHAL, Web of Science, Embase, Cochrane databases, China's National Knowledge Infrastructure (CNKI), Wanfang and VIP database from inception to January 20, 2024. Search terms included 'non-nutritive sucking' 'oral feeding' and 'premature.' Eligibility criteria involved randomized controlled studies in English or Chinese. Studies were excluded if they were reviews, case reports, or observational studies from which valid data could not be extracted or outcome indicators were poorly defined. The meta-analysis will utilize Review Manager 5.3 software, employing either random-effects or fixed-effects models based on observed heterogeneity. We calculated the mean difference (MD) and 95% confidence interval (CI) for continuous data, and estimated pooled odds ratios (ORs) for dichotomous data. Sensitivity and publication bias analyses were conducted to ensure robust and reliable findings. We evaluated the methodological quality of randomized controlled trials (RCTs) utilizing the assessment tool provided by the Cochrane Collaboration. A total of 23 randomized controlled trials with 1461 preterm infants were included. The results of the meta-analysis showed that NNS significantly shortened time taken to achieve exclusive oral feeding (MD = -5.37,95%CI = -7.48 to-3.26, p<0.001), length of hospital stay(MD = -4.92, 95% CI = -6.76 to -3.09, p<0.001), time to start oral feeding(MD = -1.41, 95% CI = -2.36 to -0.45, p = 0.004), time to return to birth weight(MD = -1.72, 95% CI = -2.54 to -0.91, p<0.001). Compared to the NNS group, the control group had significant weight gain in preterm infants, including weight of discharge (MD = -61.10, 95% CI = -94.97 to -27.23, p = 0.0004), weight at full oral feeding (MD = -86.21, 95% CI = -134.37 to -38.05, p = 0.0005). In addition, NNS reduced the incidence of feeding intolerance (OR = 0.22, 95% CI = 0.14 to 0.35, p<0.001) in preterm infants. NNS improves oral feeding outcomes in preterm infants and reduces the time to reach full oral feeding and hospitalization length. However, this study was limited by the relatively small sample size of included studies and did not account for potential confounding factors. There was some heterogeneity and bias between studies. More studies are needed in the future to validate the effects on weight gain and growth in preterm infants. Nevertheless, our meta-analysis provides valuable insights, updating existing evidence on NNS for improving oral feeding in preterm infants and promoting evidence-based feeding practices in this population.

Nurses working in neonatal nursery units play a crucial role in assessing the preterm infants feeding readiness cues for initiation of oral feeding and implementing the evidence-based intervention to support the development of feeding skills. Objectives: To assess the effect of educational intervention on neonatal nurses’ practices regarding oral Motor stimulation on early transition from to oral oral feeding in preterm infants. Methods: A quasi-experimental single group study conducted in Children Hospital Lahore from May to July 2023. Pre and post intervention data was collected through the observational checklist from 36 nurses working in Neonatology. Nurses were educated through PowerPoint presentation and direct demonstration on infants. Results: Descriptive statistics showed that Majority of nurses were having diploma in nursing, 4-10 years’ experience and belonged to 31-45 years age category. In inferential statistics Paired sample t-test revealed that the majority of nurses (100%, n=36) had incompetent practices before training which were improved (91.6%, n=33) after educational intervention with significant p-value&lt;0.05. Conclusions: The nurses who received training revealed competent practices so we conclude that educational intervention was effective to improve the nurses’ practices.

The Effects of Pleasant Odors on Transition From Gavage to Oral Feeding in Preterm Infants: A Systematic Review and Meta-analysis

Background: Gavage feeding is associated with adverse effects, such as irritations in the mouth, hyperactivity, problems in gag reflex, bradycardia, and the parents’ unacceptance. Accordingly, it is essential to undertake a comprehensive review of the effects of breast milk and other pleasant odors that may reduce the transition time from gavage to oral feeding in preterm infants. Objectives: Aim of review is to compare of effects of breast milk and other pleasant odors with the control group on the transition from gavage to oral feeding in preterm infants. Methods: In this systematic review, online English databases, such as PubMed, Scopus, ISI Web of Science, and Cochrane Library were systematically searched without any time limitation up to February 10, 2022. The modified Jadad scale was also used to assess the quality of the articles. The inclusion criteria comprised all clinical trials evaluating the effect of aromatherapy on odors on the transition from gavage to oral feeding in preterm infants. Results: 8 studies were assessed with the effects of aroma inhalation on the transition from gavage to oral feeding. Meta-analysis of these studies showed that preterm infants transitioned from tube feeding (gavage) to oral feeding earlier in the groups stimulated by pleasant odors (standardized mean difference [SMD]=₋0.47, 95% CI, ₋0.64%, ₋0.3%, heterogeneity, P=0.32; I2=13%, 8 trials) and odor of breast milk (SMD=₋0.61, 95% CI, ₋0.64%, ₋0.31%, heterogeneity, P=0.85, I2=0%, 6 trials) in comparison to the control group. Conclusions: Our meta-analysis supports using breast milk and other pleasant odors as a safe and noninvasive way of reducing the transition time from gavage to oral feeding in preterm infants. Health providers should use effective and family-friendly interventions to care for their premature infants.

Effectiveness of Oral-Motor Stimulation on Oral Feeding in Premature Infants: A Protocol for Systematic Review and Meta-Analysis of Controlled Randomized Trials.

Premature infants (born before 37 weeks of gestational age) frequently experience feeding difficulties due to underdeveloped oral motor skills and poor chewing, swallowing, and breathing coordination. In order to improve oral feeding efficiency in these infants, Oral-Motor Stimulation (OMS) has been used in various studies. This systematic review study will aim to assess the effectiveness of OMS for oral feeding in preterm infants. The authors will follow the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. They will conduct a search in electronic databases, including PubMed, Scopus, Web of Science, Cochrane Central Register of Controlled Trials in The Cochrane Library (CENTRAL), Medline via PubMed, and Cumulative Index to Nursing and Allied Health Literature (CINAHL) for nursing and related healthcare texts without language restrictions from the first month of 1991 to the fifth month of 2024 to achieve the study objectives. All Randomized Controlled Clinical Trials (RCT) examining the effect of OMS on oral feeding in preterm infants will be included in this study. The primary outcome of this systematic review will be oral feeding, and the secondary outcomes will include duration of hospitalization, weight gain, and feeding efficiency. Two independent reviewers will select and extract data for the study. The Cochrane Risk of Bias Tool (RoB2) will be used to evaluate potential biases in the study. Publication bias will be evaluated using funnel plots, Begg's, and Egger's tests. The degree of heterogeneity among the studies will be assessed using the I2 statistic and the χ2 test. Analyses of subgroups will also be carried out. All meta-analyses will be conducted using Stata V.14. This systematic review protocol for preterm infants will aim to promote evidence-based decision-making and support the development of clinical practice guidelines in preterm feeding.

Effects of Complete Oral Motor Intervention and Nonnutritive Sucking Alone on the Feeding Performance of Premature Infants: A Systematic Review and Meta-Analysis.

We explore the effect of complete oral motor intervention (OMI) and nonnutritive sucking (NNS) alone on oral feeding in preterm infants through a meta-analysis. We searched the Embased, Medline, Cochrane Library, and Web of Science databases for randomized controlled trials up to 8 August 2023, based on established selection criteria. Quality evaluations of the studies were carried out by applying both the Cochrane risk of bias assessment tool and the Jadad scale. The outcome measures of three clinical indicators included transition time to oral feeding, weight gain, and hospitalization duration. We conducted a meta-analysis using a random-effects model to determine the pooled effect sizes, expressed as standardized mean differences (SMDs) and their corresponding confidence intervals (CIs). Additionally, we undertook a subgroup analysis and meta-regression to investigate any potential moderating factors. Eight randomized controlled trials with 419 participants were selected. Meta-analysis revealed that receiving a complete OMI had significantly reduced transition time compared with those receiving NNS alone in preterm newborns (SMD, -1.186; 95% CI, -2.160 to -0.212, p = 0.017). However, complete OMI had no significant effect on shortened hospitalization duration (SMD, -0.394; 95% CI, -0.979 to 0.192, p = 0.188) and increased weight gain (SMD, 0.346; 95% CI, -0.147 to 0.838, p = 0.169) compared with NNS alone. In brief, a complete OMI should not be replaced by NNS alone. However, we were unable to draw decisive conclusions because of the limitations of our meta-analysis. Future well-designed randomized controlled trials are necessary to confirm our conclusion.

Vanilla odor promotes oral feeding in premature infants–A randomized controlled trial

ProposeIntroducing early oral feeding in premature infants is important because it supports intestinal maturation and helps prevent infections. In addition, early oral feeding is likely to contribute to improved neurocognitive outcomes in preterm infants. Several holistic therapeutic strategies have been developed to improve feeding skills, food tolerance, and the ability to drink independently, including practices such as early breastfeeding, oral stimulation, and subsequent olfactory stimulation. Based on several studies using olfactory stimulation with food odors (vanilla, breast milk) to promote oral feeding in preterm infants this study was conducted to test the following hypothesis: Does olfactory stimulation with vanilla or milk odor (breast milk or formula) lead to a reduction in the time required for nasogastric tube weaning in premature infants older than 26 + 6 weeks of gestational age? In addition, does it influence secondary outcomes such as length of hospital stay, weight development, and attainment of greater amounts of independently consumed food? MethodsPremature with complete or partial feeding by gastric tube and without ventilation were included. For this study, 207 infants over 26 + 6 gestational weeks were randomized into three different study groups. Before each feeding, an olfactory presentation was made with milk odor, a vanilla Sniffin’ Stick, or a control stick. In the final analysis, 165 infants were included (87 males, 78 females). At the time of randomization, infants were on average 12 ± 9.5 days old. ResultsWhile the influence of vanilla and milk odor did not provide a significant difference from the control for the primary outcome, a secondary analysis showed a significant group difference in the cumulative amount of independently drunk food consumed in the first ten days was the highest amount in the vanilla group. This time period was chosen due to the high dropout rate after the first ten days. In addition, there was a promising significance for earlier hospital discharge for prematurely below 32 weeks of gestation receiving vanilla odor stimulation in comparison to milk odor stimulation. ConclusionAlthough the primary outcome of this study (gastric tube removal) did not provide significant results, a significant benefit of vanilla olfactory stimulation for preterm infants was demonstrated in subgroup analysis above milk odor stimulation. Younger preterm infants seem to benefit from the stimulation.

The Evaluation of Oral Feeding in Preterm Infants: Turkish Validation of the Early Feeding Skills Assessment Tool

Objective:The Early Feeding Skills Assessment Tool (EFS) is a valid and reliable tool for evaluating preterm infants’ oral feeding skills and readiness during the transition to oral feeding. There is currently no instrument with tested validity and reliability available to evaluate the oral feeding skills and readiness of preterm infants in Turkey. The aim of this study was to fill the need for such an instrument for use in the Turkish population by adapting and validating the EFS for the assessment of Turkish preterm infants during the transition to oral feeding.Material and Methods:This methodological, cross-sectional study included 107 preterm infants. Validity of the EFS-Turkish was tested with linguistic, content, and construct validity analyses, and its reliability was tested using internal consistency and item analyses.Results:A panel of experts confirmed the content validity of the items in the EFS-Turkish (content validity index = 0.97). Cronbach’s alpha for the total instrument was 0.95, supporting its internal consistency reliability. Item–total correlations ranged from 0.58 to 0.83 (P < .001). Confirmatory factor analysis confirmed the established EFS structure of 19 items and 5 factors. The tool demonstrated good model fit statistics (χ2/df = 2.24; P < .001).Conclusion:The EFS-Turkish is a valid and reliable instrument for use in neonatal intensive care units to evaluate the feeding skills of preterm infants during the transition to oral feeding. The use of the EFS-Turkish is recommended to facilitate the safe and successful development of preterm infants’ oral feeding skills and to plan evidence-based initiatives.

The clinical experience of early skin-to-skin contact combined with non-nutritive comfort sucking in mothers of preterm infants: a qualitative study

BackgroundIn most areas of China, mothers typically do not participate in early care of preterm infants in NICU. This study aims to examine the early experience of mothers of preterm infants participating in skin-to-skin contact combined with non-nutritive comfort sucking in China.MethodsThis qualitative research study used one-on-one, face-to-face, semi-structured in-depth interviews. Eighteen mothers who participated in early skin-to-skin contact combined with non-nutritive comfort sucking were interviewed in the NICU of a tertiary children’s hospital in Shanghai between July and December 2020. Their experiences were analyzed using the inductive topic analysis method.ResultsFive themes about skin-to-skin contact combined with non-nutritive comfort sucking were identified, including alleviation of maternal anxiety and fear during mother infant separation, reshaping the maternal role, promotion of active breast pumping, enhances the mother’s willingness to actively breast feed and building the maternal confidence in baby care.ConclusionSkin-to-skin contact combined with non-nutritive comfort sucking in the NICU can not only enhance the identity and responsibility of the mother’s role, but also provide non-nutritive sucking experience for promoting the establishment of oral feeding in preterm infants.

The Effects of Cup Feeding and Finger Feeding Techniques on Weight Gain in Preterm Infants Admitted to the Neonatal Intensive Care Unit

Background: Weight is one of the main indicators of physical growth and brain development in infants, especially preterm infants. Feeding preterm infants is an ongoing challenge for those in charge of feeding infants, especially those with very low birth weights. Feeding difficulties in premature infants lead to long hospital stays and increased costs. Objectives: This study aimed to compare the effects of cup feeding and finger feeding techniques on weight gain in premature infants. Methods: This randomized clinical trial study was conducted on 90 premature infants with an average weight of 1784.3 ± 497 g admitted to the neonatal intensive care unit (NICU) of Ali Bin Abi Talib (AS) Hospital located in Zahedan, the southeast of Iran, in 2021. The infants were randomly divided into 3 groups: Cup feeding, finger feeding, and control groups (each with 30 infants). The intervention was performed for 3 consecutive days without interruption, even on holidays for each infant. The amount and frequency of feeding each infant depended on the birth weight and the diagnosis of a specialist. The infant’s weight was recorded before and after the intervention. Data analysis was performed using SPSS version 22 using analysis of variance (ANOVA), paired-samples t-test, and chi-square test. Results: The average weight of infants significantly increased in the cup feeding (P &lt; 0.01) and finger feeding (P &lt; 0.01) groups compared with the control group. The highest weight gain was recorded in the finger feeding group (109.55 g), followed by the cup feeding group (65.8 g) and the control group (31.1 g; P = 0.001). The ANOVA results suggested that the finger feeding technique was most effective in gaining weight and achieving independent oral feeding in premature infants.

Evaluation of the Effect of Oral Motor Stimulation Exercises on Feeding Skills in Premature Infants.

Oral feeding in preterm infants is a complex and dynamic process involving oral motor development and interaction between the neurological, cardiorespiratory, and gastrointestinal systems. Oral motor stimulation (OMS) is defined as stimulating the oropharyngeal components such as the lips, jaw, tongue, and soft palate with fingers in preterm infants to increase their feeding skills. In this study, we aimed to evaluate the effect of OMS exercises on the sucking and swallowing skills of preterm infants and demonstrate the utility of objective scales to evaluate infants' readiness for oral feeding. This single-center, prospective cohort study was conducted between June 1st and December 31st, 2020, which included preterm infants born at ≤34 weeks of gestation and admitted to the neonatal intensive care unit of our hospital. All procedures of the OMS program were performed once a day, 5 times a week by a language and speech therapist who is an expert in oral feeding skills (OFS) staging and non-nutritive sucking (NNS) scoring. All infants were followed up until discharge with a weekly evaluation of OFS staging and NNS scoring. A total of 50 infants were included in this prospective cohort study. The mean birth weight was 1376.9±372 g, and the median gestational age was 30 weeks (interquartile range: 25-34). The comparison of OFS stages on day 5 and day 10 of OMS revealed a significant increase (p<0.001). Similarly, there was a significant improvement in the NNS scores on days 5 and 10 compared to the baseline. In preterm infants, OMS during the transition from gavage feeding to oral feeding improves feeding skills.

Does Kangaroo Care Have an Effect on Transition Time from Gavage Feeding to Full Oral Feeding in Premature Babies?

Kangaroo care is a safe and effective alternative method to conventional neonatal care for newborn babies. The aim of this study was to evaluate the effect of kangaroo care on the transition time to full oral feeding in preterm infants fed by gavage. This is a randomized controlled trial. This study was conducted in a level III neonatal intensive care unit of a university hospital in eastern Turkey 50 premature babies with a birth weight of≥1000 g and a gestational age of 27-36 weeks, and their mothers were included in the study. The cases were randomly divided into two groups: kangaroo care, which would be applied up to five days a week, and standard care. Records of cases were kept regularly from their hospitalization until they reached full oral feeding. Premature babies in the kangaroo care group reached full oral feeding at 29.20±8.06 days after birth, while babies in the standard care group reached full oral feeding at 44.60±21.90 days. The transition period from gavage feeding to reaching full oral feeding was 13.60±6.83 days in the kangaroo care group, and 22.10±7.38 days in the standard care group. The difference was statistically significant (p=0.007). Kangaroo care is an effective method to reduce the transition time from gavage feeding to full oral feeding for premature babies.

Prefeeding interventions improve oral feeding in preterm infants.

To assess whether oral stimulation (OS), nonnutritive sucking (NNS) and combined tactile/kinesthetic (T/K) interventions can improve the effects of oral feeding in preterm infants. A retrospective review was performed from 2014 to 2016, in which one hundred thirty preterm infants were separated into two intervention groups (the OS+NNS group and the OS+NNS+T/K group) and one control group. Infants in the two intervention groups received 30min of interventions a day. All interventions started 48h after stopping nasal continuous positive airway pressure until participants reached complete oral feeding. The transition times of the OS+NNS, OS+NNS+T/K, and control groups from the introduction of oral feeding to independent oral feeding were 9.03±0.58, 7.20±0.28, and 12.17±0.64 days, respectively (P<0.05). The infants' weights at full oral feeding in the OS+NNS, OS+NNS+T/K, and control groups were 1834.58±47.96, 1999.17±92.62, and 1725.87±40.34g, respectively (P=0.007). Further post hoc analyses indicated that the weight gain at full oral feeding in the OS+NNS and OS+NNS+T/K groups were more significant than the control group (P= 0.012 and P= 0.036, respectively)Conclusion: OS+NNS and OS+NNS+T/K interventions could shorten the transition time from tube feeding to independent oral feeding; OS+NNS and OS+NNS+T/K interventions improved weight gain compared to the control group. Furthermore, the OS+NNS+T/K group was superior to the OS+NNS group regarding transition time and weight gain.

Impact of a nurse education programme on oral feeding in a neonatal unit.

Premature neonates often experience feeding difficulties during their hospital stay, and evidence-based interventions have been shown to improve feeding outcomes. This study investigated whether an infant-cue based nurse educational feeding bundle accelerates the achievement of independent oral feeding in neonates in a neonatal intensive care unit. A quality improvement study with a pre, during and post intervention test design. All premature neonates admitted to the unit were eligible. The feeding programme included a four-month nurse training module and nurse coaching. A hundred and twenty-five nurses or nurse assistants attended the programme and 706 neonates were included. The median time to independent oral feeding (IOF) was 40, 36 and 37 days, respectively, for pre, during and post intervention. The reduction in time to IOF observed during the post-intervention period compared with the baseline period was significant (HR=1.32, CI 95%: 1.01-1.74). No difference was noted in the length of hospital stay between the three study periods. An infant-cue based nurse educational feeding bundle can promote earlier achievement of IOF in preterm neonates. This quality improvement study demonstrates the impact that a nurse-driven intervention in neonatal care can have on improving practice. Feeding interventions involve the early introduction of oral feeding, non-nutritive sucking (NNS), and oral motor stimulation, and should be individualized for each neonate. These individualized feeding interventions applied by all nurses and assistant nurses, can facilitate the achievement of earlier independent oral feeding in preterm infants and should be included in neonatal critical care nurse education programs.
