Effect of Latanoprostene Bunod on Optic Nerve Head Blood Flow.

Topical latanoprostene bunod increases capillary oxygen saturation and blood volume at the optic nerve head in healthy individuals. This study aimed to evaluate the effect of topical latanoprostene bunod on optic nerve blood volume and oxygen saturation in a population of healthy participants. In this prospective double-blind crossover study, 23 healthy participants aged from 21 to 62 years were recruited. Optic nerve head capillary blood volume (ONHvol) and oxygen saturation (ONHSaO2) baselines were measured over a period of 2 hours using multichannel spectroscopic reflectometry and were remeasured after a 7-day once-daily instillation regimen of either latanoprost 0.005% or latanoprostene bunod 0.024%. After a 30-day washout period, participants were crossed over to the alternate product, and measurements were repeated. Participants were used as their own baselines to calculate variation in ONHvol and ONHSaO2 across time and pharmacological agents. The Friedman test was used to establish significant differences in optic nerve head parameters from baseline values, and Conover post hoc analysis was carried for multiple between-group comparisons. Latanoprostene bunod 0.024% induced a significant increase of 4% in ONHSaO2 compared with latanoprost 0.005% (P < .001). Furthermore, latanoprostene bunod increased ONHvol levels by more than twofold at all time points (P < .001 at T60, T90, and T120). The increase in ONHvol was 66.2% higher than levels achieved with latanoprost at T60 (P = .001), 47% higher at T90 (P < .001), and 45% higher at T120 (P < .01). Latanoprostene bunod 0.024% induces a significant increase in optic nerve head blood volume and oxygen saturation in healthy subjects, when compared with latanoprost 0.005%. Future studies are needed to evaluate whether similar responses are elicited in patients suffering from glaucomatous optic neuropathy.

Increased Optic Nerve Head Capillary Blood Flow in Early Primary Open-Angle Glaucoma.

PurposeBlood flow in the optic nerve head (ONH) is known to be reduced in eyes with advanced glaucoma. However, experimental results from non-human primates suggest an initial increase in ONH blood flow at the earliest stages of damage. This study assesses flow and pulsatile hemodynamics across a range of severities to test the hypothesis that this also occurs in human glaucoma.MethodsLaser speckle flowgraphy was used to measure average mean blur rate (MBRave) within ONH tissue (a correlate of capillary blood flow) and the pulsatile waveform in 93 eyes with functional loss and 74 glaucoma suspect/fellow eyes without functional loss. These were compared against results from 92 healthy control eyes. Parameters produced by the instrument's software were age-corrected, then compared between groups using generalized estimating equation models.ResultsThe mean MBRave in the control eyes was 12.5 units. In glaucoma suspect/fellow eyes, the mean was 16.4 units, higher with P < 0.0001. In eyes with functional loss, the mean was 13.8 units, lower than eyes without functional loss with P < 0.0001, although still higher than control eyes with P = 0.0096. Analysis of the pulsatile waveform suggested that the deceleration in flow as it approaches its maximum across the cardiac cycle was delayed in glaucoma.ConclusionsBlood flow within ONH capillaries was higher in glaucoma suspect eyes than in healthy controls. It was less elevated in eyes that had developed functional loss. The mechanisms causing these changes and their relation to concurrent changes in pulsatile hemodynamics remain under investigation.

Effect of isovolumic hemodilution on oxygen delivery to the optic nerve head

To investigate the effect of isovolumic hemodilution on the tissue oxygenation of the optic nerve head (ONH). In 9 miniature pigs (6 - 12 kg), hemodilution was performed by replacing 100 - 140 ml of blood by an equivalent volume of 6 % hydroxyethyl starch in saline. The blood flow parameters in the ONH microcirculation, namely velocity (BVel), volume (BVol) and flow (BF), were measured by laser Doppler flowmetry. Microelectrodes placed at approximately 50 micrometer from the disk surface and the phosphorescence quenching technique were used to measure pO 2 in the vitreous (pO 2 (vitr)) and in the ONH capillary blood (pO 2 (blood)), respectively. Arterial blood pressure, hematocrit (Hct) and gas content were documented. In all animals, hemodilution reduced Hct by 30 +/- 6 %. All the flow parameters increased markedly. Simultaneously, pO 2 (blood) (baseline: 31 +/- 3 mm Hg) dropped to approximately 75 % of its initial value, while pO 2 (vitr) (baseline: 26 +/- 8 mm Hg) increased by 15 +/- 8 %. Hemodilution leads to an increase of pO 2 (vitr) reflecting a similar rise of the ONH tissue pO 2. This effect results from an enhanced blood perfusion of the ONH, which over-compensates the decrease of the blood O 2 content.

Diversity of Response of Optic Nerve Head Circulation to Timolol Maleate in Gel-Forming Solution

This randomized, double-masked, placebo-controlled, two-period crossover study was conducted to investigate the effects of 0.5% timolol maleate in gel-forming solution on intraocular pressure (IOP) and blood circulation in the optic nerve head in patients with untreated ocular hypertension. The effects of 0.5% timolol in gel-forming solution on IOP and optic nerve head capillary blood speed were studied in 12 patients with untreated ocular hypertension. Optic nerve capillary blood speed was measured using the laser Doppler technique before and at the end of each treatment period. In each patient, IOP decreased after treatment with timolol (mean decrease 16.8% versus placebo). Systemic blood pressure and pulse rate did not differ significantly after treatment with topical timolol from values after placebo. The mean change from baseline in Doppler broadening was 10.6% greater after treatment with timolol than after placebo. There was no significant change in mean Doppler broadening from baseline after treatment with either timolol or placebo. However, optic nerve head capillary blood speed increased in six patients, and was within the range of placebo response in six patients after treatment with timolol. Spearman correlation analysis of the baseline with Doppler broadening measurements after treatment showed a correlation for placebo but not for timolol. The percent change in Doppler broadening after timolol treatment was correlated with iris color. These results indicate that administration of timolol for 4 weeks reduces IOP in patients with ocular hypertension and generally does not change the blood circulation in the optic nerve head. Individual patients, however, showed variable changes in optic nerve head circulation after topical administration of timolol. Although the sample size was small, these changes in optic nerve head circulation were correlated with iris color.

Reproducibility of Retinal and Optic Nerve Head Blood Flow Measurements with Scanning Laser Doppler Flowmetry

To evaluate the interobserver variability and the reproducibility of retinal and optic nerve head capillary blood flow measurements performed with a new noninvasive equipment, the scanning laser Doppler flowmeter (Heidelberg Engineering, Heidelberg, Germany). Blood flow measurements were performed during three independent sessions in six patients with glaucoma and five normal subjects using the scanning laser Doppler flowmeter (SLDF), which allows the visualization of perfused capillaries and vessels of the retina and optic nerve head and enables the quantification of capillary blood volume, flow, and velocity in any selected area of the perfusion map. To evaluate the interobserver variability in selecting the areas in the perfusion map to be measured, three observers tried to locate the same areas in the perfusion map of images obtained during the first session. To evaluate the reproducibility of the measurements, the observers measured correspondent areas in the peripapillary retina and in the optic nerve head of images from the three sessions. Areas of different sizes (10 x 10 pixels and 4 x 4 pixels) were measured. The agreement between readings performed by the three observers was very good, with the reliability coefficient for the various parameters varying from 0.90 to 0.98. The reproducibility of retinal and lamina cribrosa measurements with the 10 x 10 pixel square target was good (reliability coefficient for the different parameters ranging from 0.70 to 0.85) and much better than the reproducibility of the 4 x 4 pixel target. The measurements performed in the neuroretinal rim area also had poor reproducibility. The measurements from the patients with glaucoma tended to be more reproducible than those from normal subjects. The SLDF allows reproducible blood perfusion measurements of retinal and lamina cribrosa areas when a target square of 10 x 10 pixels is used.

Optic Nerve Head Circulation After Topical Calcium Channel Blocker

Our purpose was to investigate the effects of the calcium channel blocker verapamil on intraocular pressure and blood circulation in the human optic nerve head. The effects of three different concentrations of topical verapamil (0.063%, 0.125%, and 0.25%) on intraocular pressure and optic nerve head capillary blood speed were measured in 12 healthy normal subjects. In a randomized, double-masked design, each subject received one drop of either verapamil or placebo in one eye and the opposite treatment in the fellow eye. Anterior optic nerve circulation was assessed at baseline and 90 min after instillation of the drops using the laser Doppler technique. The intraocular pressure was significantly reduced compared with baseline in both verapamil- and placebo-treated eyes at each concentration. The reductions of intraocular pressure were greater in verapamil-treated eyes (12-17%) than in placebo-treated eyes (9-12%). No systemic effect on heart rate or blood pressure was detected after administration of topical verapamil. The capillary blood speed in the optic nerve head was increased in both verapamil- and placebo-treated eyes at each concentration, although the only statistically significant increases were with the 0.25% concentration. The mean +/- SEM increase compared with baseline at the 0.25% concentration was 10.4 +/- 3.6% in verapamil-treated eyes (p = 0.017), and 11.6 +/- 4.4% in placebo-treated eyes (p = 0.026). These results indicate that topical administration of verapamil lowers the intraocular pressure and increases the capillary blood speed in the optic nerve head in normal subjects. Changes measured in verapamil-treated eyes were also observed in placebo-treated eyes, indicating a crossover effect.

Measurement of Superficial Optic Nerve Head Capillary Blood Velocities by Scanning Laser Fluorescein Angiography

Scanning laser ophthalmoscopy is capable of producing video fluorescein angiograms with high spatial and temporal resolution. Digital image analysis of these angiograms has been used to produce measures of capillary flow velocities in perimacular capillaries. We have recently utilized the same technique to measure flow velocities in the superficial capillary layers of the optic nerve head. We performed video fluorescein angiograms using the Ro-denstock (Germany) scanning laser ophthalmoscope. The passage of hyperflu-orescent particles through optic nerve head capillaries was imaged in five healthy subjects (two men and three women). The flow velocity for each of eight to 10 capillaries in each subject was measured three to five times. The mean capillary blood flow velocity for all subjects was 2.71 mm/s. These results are similar to those obtained in perifoveal capillaries. We have demonstrated that scanning laser fluorescein angiography can be adapted for measuring superficial optic disk capillary flow velocities. The possibility remains for the imaging of capillaries in deeper portions of the optic nerve head by using the confocol abilities of the scanning laser ophthalmoscope. Future use of this technique may aid our understanding of the pathophysiology of optic neuropathy.

Effect of Timolol on Human Retinal, Choroidal and Optic Nerve Head Circulation

In a double-masked, randomized, placebo-controlled study, we evaluated the effect of topical timolol maleate 0.5% on the retinal, choroidal, and optic nerve head circulation in 5 healthy volunteer subjects. Changes in the pulsatile component of choroidal blood flow (PCBF) were determined from measurements of the ocular pulse wave. Changes in the retinal arterial blood flow rate (RBF) and optic nerve head capillary blood speed (CBS) were determined by laser Doppler velocimetry and monochromatic photography. In timolol-treated eyes, PCBF decreased by 32 +/- 12% (p = 0.0007). Changes in RBF and CBS were not statistically significant. In the contralateral placebo-treated eyes, PCBF decreased by 15 +/- 8% (p = 0.006) and RBF increased by 18 +/- 10% (p = 0.002). The change in CBS was not statistically significant.
