This case highlights an open retroperitoneal approach to explant an embolized left atrial appendage occlusion device in the visceral aorta. Endovascular retrieval of the device was deemed to be an inferior alternative given the significant mural thrombus that extended toward the superior mesenteric artery and celiac artery from the device, as demonstrated on computed tomography angiogram, and the associated risk of embolizing the visceral vessels or the lower extremities.
