Relationship between Muscle Cross-Sectional Area by MRI and Muscle Thickness by Ultrasonography of the Triceps Surae in the Sitting Position.

The purpose of this study was to determine the differences in the muscle cross-sectional area (MCSA) of the triceps surae in the supine and sitting positions using magnetic resonance imaging (MRI), and the relationship between the MCSA of the triceps surae in the sitting position and muscle thickness (MT), assessed using MRI and ultrasonography, respectively. This study included 16 healthy young male participants. The measurement positions were 90° flexion of the knee joint and neutral position of the ankle joint in the sitting or supine positions. Using an open-configuration MRI system with a vertical gap and ultrasonography, we measured the MCSA and MT of the soleus muscle and the medial and lateral heads of the gastrocnemius muscle at three selected locations in the ventral part of the muscle. As a result, the 50% portion of the soleus muscle and the 25% and 50% portions of the gastrocnemius medial and lateral heads were higher in the sitting position than in the supine position. Furthermore, only 50% of the gastrocnemius medial head showed a correlation between the MCSA and MT. When using the MT of the triceps surae as an indicator of muscle volume in the sitting position, the muscle site should be considered.

Advances in basic science methodologies for clinical diagnosis in female stress urinary incontinence.

We provide an overview of advanced imaging techniques currently being explored to gain greater understanding of the complexity of stress urinary incontinence (SUI) through better definition of structural anatomic data. Two methods of imaging and analysis are detailed for SUI with or without prolapse: 1) open magnetic resonance imaging (MRI) with or without the use of reference lines; and 2) 3D reconstruction of the pelvis using MRI. An additional innovative method of assessment includes the use of near infrared spectroscopy (NIRS), which uses non-invasive photonics in a vaginal speculum to objectively evaluate pelvic floor muscle (PFM) function as it relates to SUI pathology. Advantages and disadvantages of these techniques are described. The recent innovation of open-configuration magnetic resonance imaging (MRO) allows images to be captured in sitting and standing positions, which better simulates states that correlate with urinary leakage and can be further enhanced with 3D reconstruction. By detecting direct changes in oxygenated muscle tissue, the NIRS vaginal speculum is able to provide insight into how the oxidative capacity of the PFM influences SUI. The small number of units able to provide patient evaluation using these techniques and their cost and relative complexity are major considerations, but if such imaging can optimize diagnosis, treatment allocation, and selection for surgery enhanced imaging techniques may prove to be a worthwhile and cost-effective strategy for assessing and treating SUI.

Effects of upright weight bearing and the knee flexion angle on patellofemoral indices using magnetic resonance imaging in patients with patellofemoral instability.

This study analysed the effects of upright weight bearing and the knee flexion angle on patellofemoral indices, determined using magnetic resonance imaging (MRI), in patients with patellofemoral instability (PI). Healthy volunteers (control group, n=9) and PI patients (PI group, n=16) were scanned in an open-configuration MRI scanner during upright weight bearing and supine non-weight bearing positions at full extension (0° flexion) and at 15°, 30°, and 45° flexion. Patellofemoral indices included the Insall-Salvati Index, Caton-Deschamp Index, and Patellotrochlear Index (PTI) to determine patellar height and the patellar tilt angle (PTA), bisect offset (BO), and the tibial tubercle-trochlear groove (TT-TG) distance to assess patellar rotation and translation with respect to the femur and alignment of the extensor mechanism. A significant interaction effect of weight bearing by flexion angle was observed for the PTI, PTA, and BO for subjects with PI. At full extension, post hoc pairwise comparisons revealed a significant effect of weight bearing on the indices, with increased patellar height and increased PTA and BO in the PI group. Except for the BO, no such changes were seen in the control group. Independent of weight bearing, flexing the knee caused the PTA, BO, and TT-TG distance to be significantly reduced. Upright weight bearing and the knee flexion angle affected patellofemoral MRI indices in PI patients, with significantly increased values at full extension. The observations of this study provide a caution to be considered by professionals when treating PI patients. These patients should be evaluated clinically and radiographically at full extension and various flexion angles in context with quadriceps engagement. Explorative case-control study, Level III.

The effects of a dynamic patellar realignment brace on disease determinants for patellofemoral instability in the upright weight-bearing condition.

BackgroundPatellar stabilizing braces are used to alleviate pain and prevent subluxation/dislocation by having biomechanical effects in terms of improved patellar tracking. The purpose of this study is to analyze the effects of the dynamic patellar realignment brace, Patella Pro (Otto Bock GmbH, Duderstadt, Germany), on disease determinants in subjects with patellofemoral instability using upright weight-bearing magnetic resonance imaging (MRI).MethodsTwenty subjects (8 males and 12 females) with lateral patellofemoral instability were studied in an open-configuration magnetic resonance imaging scanner in an upright weight-bearing position at full extension (0° flexion) and 15° and 30° flexion with and without the realignment brace. Disease determinants were defined by common patellofemoral indices that included the Insall–Salvati Index, Caton–Deschamps Index, and the Patellotrochlear Index to determine patella height and patella tilt angle, bisect offset, and tuberositas tibiae–trochlear groove (TT–TG) distance to determine patellar rotation and translation with respect to the femur and the alignment of the extensor mechanism.ResultsAnalyses of variance revealed a significant effect of the brace with reduction of the three patellar height ratios, patella tilt angle, and bisect offset as well as TT–TG distance. Post hoc pairwise comparisons of the corresponding conditions with and without the realignment brace revealed significantly reduced patella height ratios, patella tilt angles, and bisect offsets at full extension and 15° and 30° flexion. No significant differences between the TT–TG distances at full extension but significant reductions at 15° and 30° flexion were observed when using the realignment brace compared to no brace.ConclusionsThis study suggests that the dynamic patellar realignment brace is capable of improving disease determinants in the upright weight-bearing condition in the range of 0° to 30° flexion in patients with patellofemoral instability.

Characteristics of geometric distortion correction with increasing field-of-view in open-configuration MRI

Open-configuration magnetic resonance imaging (MRI) systems are becoming increasingly desirable for musculoskeletal imaging and image-guided radiotherapy because of their non-claustrophobic configuration. However, geometric image distortion in large fields-of-view (FOV) due to field inhomogeneity and gradient nonlinearity hinders the practical applications of open-type MRI. We demonstrated the use of geometric distortion correction for increasing FOV in open MRI. Geometric distortion was modeled and corrected as a global polynomial function. The appropriate polynomial order was identified as the minimum difference between the coordinates of control points in the distorted MR image space and those predicted by polynomial modeling. The sixth order polynomial function was found to give the optimal value for geometric distortion correction. The area of maximum distortion was<1 pixel with an FOV of 285mm. The correction performance error was increased at most 1.2% and 2.9% for FOVs of 340mm and~400mm compared with the FOV of 285mm. In particular, unresolved distortion was generated by local deformation near the gradient coil center.

MRI-guided stereotactic aspiration of brain abscesses by use of an optical tracking navigation system

Owing to the high risk of abscess drainage by craniotomy, imaging-guided stereotactic aspiration is considered an ideal choice in the management of brain abscesses. Interventional magnetic resonance imaging (MRI) represents a valuable technique for the treatment of brain abscess as a guiding modality. To evaluate the safety and efficacy of an interventional MRI system in performing the procedure. Thirteen brain abscesses in 11 patients were treated with percutaneous aspiration. All procedures were performed solely under the guidance of a 0.23-T open-configuration MRI scanner with optical tracking. Clinical and imaging follow-up was at 1 week, 1 month, 3 months, and 6 months. The changes of abscess, MRI features, and clinical symptoms were recorded. Procedure efficacy and safety were evaluated by success rate, procedure time, decrease of abscess, recovery rate, and complication. Descriptive statistical analysis was performed. MRI-guided stereotactic aspirations were performed successfully in 13/13 (100%) abscesses. The mean operating time was 70 min (range, 45-100 min). Follow-up MRI at 1 week after the procedure showed average reduction of abscesses by 60% (2.1/3.5). And the abscesses continued to get smaller by up to 89.7% (3.14/3.5) at 1-month follow-up. All cavities resolved at the end of the 6-month follow-up period. The recovery rate was 100% for fever, headache, vomiting, papilledema, meningismus, altered sensorium, 75% (3/4) for hemiparesis, and 83.3% (5/6) for epilepsy. There were no complications. Punctures of brain abscesses with subsequent aspiration can be performed safely and efficiently by monitoring the procedure using an open interventional MRI system.

Patellar maltracking is prevalent among patellofemoral pain subjects with patella alta: An upright, weightbearing MRI study

The purpose of this study is to determine if patellar maltracking is more prevalent among patellofemoral (PF) pain subjects with patella alta compared to subjects with normal patella height. We imaged 37 PF pain and 15 pain free subjects in an open-configuration magnetic resonance imaging scanner while they stood in a weightbearing posture. We measured patella height using the Caton-Deschamps, Blackburne-Peel, Insall-Salvati, Modified Insall-Salvati, and Patellotrochlear indices, and classified the subjects into patella alta and normal patella height groups. We measured patella tilt and bisect offset from oblique-axial plane images, and classified the subjects into maltracking and normal tracking groups. Patellar maltracking was more prevalent among PF pain subjects with patella alta compared to PF pain subjects with normal patella height (two-tailed Fisher's exact test, p<0.050). Using the Caton-Deschamps index, 67% (8/12) of PF pain subjects with patella alta were maltrackers, whereas only 16% (4/25) of PF pain subjects with normal patella height were maltrackers. Patellofemoral pain subjects classified as maltrackers displayed a greater patella height compared to the pain free and PF pain subjects classified as normal trackers (two-tailed unpaired t-tests with Bonferroni correction, p<0.017). This study adds to our understanding of PF pain in two ways-(1) we demonstrate that patellar maltracking is more prevalent in PF pain subjects with patella alta compared to subjects with normal patella height; and (2) we show greater patella height in PF pain subjects compared to pain free subjects using four indices commonly used in clinics.

Pharyngeal airway in children with sleep-disordered breathing in relation to head posture

The aim of this study was to compare the pharyngeal airway size in different cranio-cervical postures in children with sleep-disordered breathing (SDB) and asymptomatic control children. We tested the hypothesis that the site of the most constricted pharyngeal airway in SDB children is not affected by head posture. The study group comprised 29 children (14 boys, 15 girls, mean age 7.3 ± 1.37 years, range 4.8-9.8 years) with SDB. A control group of 29 age- and gender-matched healthy children was recruited. The pharyngeal airway of the children was imaged with a low-field 0.23-T open-configuration magnetic resonance imaging scanner in three different head postures: neutral, extension, and flexion. Inter-group differences in pharyngeal variables were analyzed using two-sided paired t test and intra-group differences using Bonferroni and Šidak correlations for multiple comparisons. Nasopharyngeal airway was not affected by head posture. Head extension had no significant effect on the most constricted retropalatal airway in the SDB group, but it increased the hypopharyngeal airway (P = 0.000) significantly when compared to neutral head posture. The hypopharyngeal airway increased more in the SDB group than in the asymptomatic children (P = 0.031). The retropalatal oropharynx was on the whole significantly narrower in the SDB children compared with the controls in all head postures (neutral P = 0.000, extended P = 0.001, flexed P = 0.000). Head posture is an important factor in maintaining airway patency. We suggest that the effectiveness of neuromuscular compensation may be compromised in constricted retropalatal airways. Instead, the hypopharyngeal airway is enlarged suggesting possible neuromuscular compensation for oropharyngeal narrowing in children with SDB.

Magnetic Resonance Imaging–Guided Biopsy of Musculoskeletal Lesions Using Open Low-Field Systems

With the development of open-configuration magnetic resonance imaging (MRI) systems, magnetic resonance-compatible navigational tools, and fast pulse sequences, MRI-guided biopsy of musculoskeletal lesions has evolved into an effective and safe, minimally invasive technique. Magnetic resonance-guided percutaneous biopsy of musculoskeletal lesions is especially suited for lesions that are detectable only with MRI, lesions that require double-angulated needle paths, and for patients in which radiation exposure needs to be avoided. In this article, we review pertinent principles, techniques, and clinical applications of low-field MRI for biopsy procedures in the musculoskeletal system.

Validity of upper airway assessment in children:A clinical, cephalometric, and MRI study

To test the hypothesis that the capability of two-dimensional lateral cephalogram in recognizing pharyngeal obstruction is poor compared with the capability of three-dimensional magnetic resonance imaging (MRI) and clinical observation of tonsillar size. The study participants were 36 prepubertal children (19 male, 17 female; mean age 7.3 ± 1.43 years, range 4.8-9.8 years) with sleep-disordered breathing diagnosed by nocturnal polygraphy. Pharyngeal airway was imaged with a low-field open-configuration MRI scanner. Tonsillar size was clinically determined and lateral skull radiographs were taken and measured. Pearson correlation coefficients were calculated between the clinical, cephalometric, and MRI variables. Nasopharyngeal and retropalatal cephalometric variables had a significant positive correlation with the MRI findings. Both techniques showed the narrowest measurement to be located in the retropalatal region. Clinical assessment of tonsillar size correlated inversely with MRI findings such as minimal retropalatal cross-sectional airway area (P = .000), minimal retroglossal cross-sectional airway area (P = .015), and intertonsillar airway width (P = .000). Cephalometric soft palate and tonsillar area correlated with clinical tonsillar size (P = .001). The hypothesis is rejected. The findings confirm that the lateral cephalogram is a valid method for measuring dimensions of the nasopharyngeal and retropalatal region. When evaluating oropharyngeal size, clinical assessment of tonsillar size is a relatively reliable method.

MRI-guided percutaneous nephrostomy: a feasibility study

The purpose of this study was to assess the feasibility and safety of magnetic resonance imaging (MRI)-guided PCN in an open-configuration low-field MRI system. Eight patients were prospectively enrolled in the study. The degree of the dilatation of the renal collecting system varied from minimal to severe. All procedures were performed solely under MRI guidance with a 0.23-T open configuration C-arm-shaped MRI system with interventional optical tracking. In each case, PCN was performed with a MRI-compatible drainage kit using the Seldinger technique. Seven out of eight nephrostomies were successfully performed under MRI guidance. All PCN procedures in dilated renal collection systems were successful; however, nephrostomy catheter could not be placed in a nondilated system. The mean time needed for the MRI-guided PCN was 26 min. No major complications occurred during the procedure or follow-up. MRI-guided PCN in dilated renal collection system is feasible and safe. The presented technique has limitations that necessitate further technical developments before the procedure can be applied to nondilated kidneys and recommended for routine clinical use.

Interventional MRI for Oncologic Applications

Interventional radiology plays an important role in the diagnosis and treatment of many oncologic disorders. Magnetic resonance imaging (MRI) has excellent soft-tissue contrast capabilities and allows visualization of anatomical details of many organs that is not possible with other imaging modalities. MRI also is multiplanar, has the ability to display tissue temperature changes, and uses no ionizing radiation. With an open configuration MRI system, the radiologist may stand alongside the patient and view images, all while performing an interventional procedure. In this article, we explain the rationale for using MRI to guide interventions, focus on technical aspects of biopsy, cryoablation, focused ultrasound, and brachytherapy, and provide a primer for the interventional radiologist wishing to use MRI to guide oncologic interventions.

The effect of macronutrients on gastric volume responses and gastric emptying in humans: a magnetic resonance imaging study

The effects of macronutrients on gastric volume changes, emptying, and gastrointestinal symptoms are incompletely understood. Three liquid meals of 500 ml (fat emulsion, 375 kcal; protein solution, 375 kcal; glucose solution, 400 kcal) were infused into the stomach of 12 healthy volunteers on three occasions. Studies were performed in seated body position using an open-configuration magnetic resonance imaging (MRI) system. MRI imaging sequences, assessing stomach and meal volumes, were performed prior to and at times t = 0, 3, 6, 9, 12, 15, 25, 35, 45, 60, 75, and 90 min after meal administration. Areas under the curve for the early emptying phase (0-15 and 0-45 min) were calculated, and characteristics of the volume curves were analyzed by a gastric emptying model. Gastrointestinal symptoms were assessed by a self-report scale. Initial (t = 0 min) and early postprandial gastric volumes were highest for glucose because of lower initial emptying. However, in the early emptying phase the characteristics of the volume curves for stomach and meal were uniform for all macronutrients. Perceptions of fullness and satiety were linearly associated with postprandial gastric volumes, but not with macronutrient composition. Isovolumic macronutrient meals modulate gastric volume response by initial meal emptying patterns. Macronutrient specific accommodation responses, as shown in barostat studies, are not reflected as gastric volume responses under noninvasive conditions.

MRI-guided abdominal biopsy in a 0.23-T open-configuration MRI system

The purpose of this study was to test the hypothesis that when ultrasound (US) guidance is not feasible, abdominal biopsies can be performed safely and accurately under magnetic resonance imaging (MRI) guidance in a low-field environment. MRI-guided abdominal biopsy was performed on 31 consecutive patients, in whom US-guided abdominal biopsy was not possible because the lesion was not visualized in US (n=27) or an US-guided procedure was not considered safe (n=4). The locations of the lesions were liver (n=14), pancreas (n=6), lymph node (n=4), retroperitoneal mass (n=3), adrenal gland (n=3) and spleen (n=1). The average size of the lesion was 2.2 cm (range 1-4 cm) in maximum diameter. All procedures were done by using a 0.23-T open-configuration C-arm-shaped MRI scanner with interventional optical tracking equipment and software. Fine-needle aspiration (FNA) biopsy was performed on all 31 patients; 18 patients underwent both FNA biopsy and cutting needle core biopsy. Procedures were evaluated for diagnostic sensitivity, specificity and accuracy as well as procedure time and complications. The FNA biopsy specimens were adequate for interpretation in 27 (87%) of 31 cases. Two of these proved to be false-negative findings during follow-up or subsequent biopsy. The final diagnosis was malignant in 15 and benign in 16 patients. The sensitivity, specificity and accuracy of FNA biopsy were 71, 100 and 81%, respectively. Of the 18 core-needle biopsies, one was determined false-negative owing to nonrepresentativeness. The sensitivity, specificity and accuracy of histological samples were 90, 100 and 94%, respectively. The needle time was 19 min on average and the mean room time was 1 h 48 min. No immediate or late complications occurred. MRI-guided abdominal biopsy can be performed safely and accurately in a low-field environment in patients for whom an US-guided procedure is not feasible.

Acromiohumeral distance in a seated position in persons with impingement syndrome.

To compare the acromiohumeral distance (AHD) of both shoulders in subjects with a unilateral shoulder impingement syndrome (SIS) and healthy subjects in a seated position during free shoulder movements of large amplitude. Using a 0.5-Tesla SIGNA-SP/i trade mark open-configuration magnetic resonance imaging (MRI) system, the AHD was measured at several arm positions, in a standardized seated position. In flexion (rest to 130 degrees ), the mean AHD of SIS varied from 8.3 to 2.8 mm, as compared to from 8.7 to 4.1 mm for asymptomatic contralateral shoulders. In abduction (rest to 110 degrees ), the AHD varied from 8.1 to 3.4 mm and from 8.8 to 4.6 mm for SIS and asymptomatic shoulders, respectively. The smallest AHD (P <.01) was observed at 110 degrees. From 80 degrees of arm elevation, the AHD was significantly smaller in impingement than in asymptomatic shoulders (P <.05). The AHD of asymptomatic shoulders did not differ from that of healthy subjects. In SIS, the AHD at rest was correlated with its reduction at all shoulder positions (r(p) = 0.62-0.88), but was a poor predictor of the smallest AHD observed at 110 degrees (flexion, R(2) = 0.09; abduction, R(2) = 0.08). In the critical arm elevation positions for impingement, AHD is discriminative for SIS, compared to asymptomatic shoulders and healthy ones. AHD at rest is a good indicator of the magnitude of its reduction in elevation, but not of the smallest AHD in arm elevation.

Feasibility of MR Imaging–Guided Breast Lumpectomy for Malignant Tumors in a 0.5-T Open-Configuration MR Imaging System

Rationale and Objectives The authors performed this study to develop the technology for and evaluate the utility of a 0.5-T vertical open-configuration magnetic resonance (MR) imaging system for imaging-guided breast lumpectomy of malignant tumors. Materials and Methods Twenty women with breast cancer underwent MR imaging–guided lumpectomy in a 0.5-T vertical open-configuration MR system. During lumpectomy, pre- and postresection images were acquired with and without contrast material. Images were used both for tumor localization and to monitor resection. If residual enhancement was observed, additional resection was performed in an effort to secure negative pathologic margins. Results The procedure evolved over time with technology innovations and improvements. Specifically, instruments were acquired that were compatible with MR imaging breast procedures, the echo time with the Dixon technique was modified to optimize image quality, contrast material injection was timed for maximum lesion enhancement, breath-hold image acquisition was instituted, and the biopsy cavity was filled with saline and the incision closed before image acquisition. Conclusion All breast lesions were identified despite limited spatial resolution and fat-suppression techniques. The evolved techniques facilitated intraoperative margin evaluation and prompted additional surgical margin resection in five patients, sparing four from an additional surgical procedure. Intraoperative MR imaging has the potential to improve the complete pathologic excision of invasive breast cancer.

Dynamic lateral patellar tilt in the anterior cruciate ligament-deficient knee. A magnetic resonance imaging analysis.

An open-configuration magnetic resonance imaging scanner was used to document patellar tracking abnormalities in 11 anterior cruciate ligament-injured knees. The contralateral normal knees were used as controls. Images were obtained with the quadriceps muscles at rest (knee flexion at 40 degrees, 25 degrees, and 10 degrees) and with the quadriceps muscles contracted (knee flexion at 40 degrees and 25 degrees). When the quadriceps muscles were at rest there were no differences in patellar alignment between the anterior cruciate ligament-injured knees and the contralateral normal knees. When the quadriceps muscles were maximally contracted at 40 degrees of flexion, the patellae of the anterior cruciate ligament-injured knees tilted laterally 3.6 degrees relative to the resting state. When the quadriceps muscles were contracted at 25 degrees of flexion, the patellae of the anterior cruciate ligament-injured knees tilted laterally approximately 4 degrees relative to the resting state. Quadriceps-active lateral patellar tilt at 25 degrees of flexion was greater in the anterior cruciate ligament-injured knees than in the contralateral normal knees, and it correlated with instrumented measurements of anterior tibial translation. Dynamic lateral patellar tilt during open kinetic chain exercises and during other activities that produce anterior tibial translation may contribute to extensor mechanism dysfunction in the anterior cruciate ligament-injured knee.

MR-guided Transjugular Portosystemic Shunt Placement in a Swine Model

To evaluate the performance of portal venous puncture with use of magnetic resonance (MR) guidance, and to place a transjugular intrahepatic portosystemic shunt (TIPS) in a swine model. A study of 12 swine was performed to evaluate the ability of interventional MR imaging to guide portal vein puncture and TIPS placement. Six swine had catheters placed in the right hepatic vein under C-arm fluoroscopy. A nitinol guide wire was left in the vein and the animals were then moved into an open configuration MR imaging unit. A TIPS needle set was used to puncture the portal vein using MR fluoroscopy. The animals were transferred to the C-arm, and venography confirmed portal vein puncture. A follow-up study was performed in six additional swine to place a TIPS using only MR imaging guidance. MR tracking was used to advance a catheter from the right atrium into the inferior vena cava. Puncture of the portal vein was performed and a nitinol stent was placed, bridging the hepatic parenchyma. MR venogram confirmed placement. Successful portal vein puncture was achieved in all animals. The number of punctures required decreased from 12 in the first animal to a single puncture in the last eight swine. A stent was successfully placed across the hepatic tract in all six swine. Real-time MR imaging proved to be a feasible method to guide portal vein puncture and TIPS placement in pigs.

Intraoperative Magnetic Resonance Imaging Guidance for Intracranial Neurosurgical Procedures

A 0.5 Tesla open configuration magnetic resonance imaging (MRI) system (SignaSP [GE Medical Systems; Milwaukee, WI]) is sited in an operating room environment at Brigham and Women's Hospital for MRI-guided surgical and interventional procedures. The vertical gap provides patient access to two surgeons or interventionalists, whereas open flexible transmit-receive coils allow exposure of the surgical field. A major application of this system at Brigham and Women's Hospital is intraoperative MRI guidance for intracranial biopsies and open craniotomies. During the past 2œ years, more than 200 intracranial procedures have been performed using intraoperative MRI guidance. For intracranial biopsies, MRI guidance provides several advantages, including high sensitivity for lesion localization; interactive navigation, eliminating the need for a stereotactic frame; multiplanar image confirmation of biopsy site location; and early detection of complications. Based on our experience with open craniotomies, we think that image guidance significantly improves the ability of the neurosurgeon to resect abnormal tissue selectively while preserving adjacent eloquent areas. Furthermore, our experience indicates that intraoperative imaging offers advantages over conventional stereotactic image guidance, including the ability to compensate for shifts in brain position during surgery, and allows for early identification of complications. Intraoperative MRI-guided neurosurgery is a multidisciplinary effort involving the neurosurgeon, neuroradiologist, technologist, nursing staff, and engineers. In our experience, there is little doubt that MRI guidance improves the technical aspects of the neurosurgical procedures; however, the impact of this technology on clinical outcomes and cost effectiveness await further study.
